Abstract:
Introduzione: l'indagine nasce dallo studio del 118 come servizio di salute pubblica. I riferimenti teorici si rifanno alla Psicologia dell'emergenza, ai fenomeni di stress degli operatori ed al concetto di empowerment come strategia preventiva di tali fenomeni. L'obiettivo č quello di osservare il benessere percepito dagli operatori di una Centrale Operativa del 118, attraverso la loro descrizione delle attivitŕ svolte e delle principali difficoltŕ incontrate. Metodo: interviste semi-strutturate a 18 infermieri, analizzate con metodo qualitativo basato sulla Grounded Theory. Risultati: le maggiori difficoltŕ percepite sono dovute alla specificitŕ della Centrale Operativa, basata su rapporti con l'utenza mediati dal telefono che non consente una valutazione diretta della gravitŕ del problema portato. Le interviste hanno inoltre rilevato aspetti di grande intensitŕ emozionale fonti potenzialmente di stress e di burnout. Discussione e conclusioni: i risultati rivelano la percezione di scarsa padronanza da parte degli operatori delle modalitŕ di conduzione degli interventi telefonici. Emerge il bisogno di strutturare strategie di gestione dello stress, la richiesta di maggior confronto con i colleghi e la necessitŕ di sviluppare empowerment per una migliore conduzione del servizio.

Abstract:
Lo studio intende analizzare l'evoluzione della depressione materna nella transizione alla genitorialitŕ, valutando il ruolo di alcuni fattori di rischio quali l'insicurezza dell'attaccamento, la presenza di problematiche psicopatologiche e di eventi di vita stressanti. La ricerca, condotta su un campione di 30 primipare (etŕ: M = 31.60; d.s. = 4.12) ha previsto un disegno longitudinale. Nella prima rilevazione, al 3° trimestre di gravidanza, sono state somministrate l'Edinburgh Depression Scale (EDDS, Cox, Holden e Sagovsky, 1987); la Mini-International Neuropsychiatric Interview (MINI, Sheehan et al., 1998); la Scala degli Eventi Stressanti (Paykel et al., 1980) e l'Attachment Style Interview (ASI: Bifulco et al., 2002). A 10 giorni e a 3 mesi dopo il parto, č stata prevista solo la somministrazione telefonica dell'EDDS. Le analisi dei dati hanno messo in luce un decremento lineare della depressione dalla gravidanza al postnatale e un'associazione significativa tra la depressione prenatale e postnatale e tutti e tre i fattori di rischio considerati; tali fattori sono, a loro volta, significativamente associati tra loro. I risultati ottenuti sottolineano la possibilitŕ di individuare i fattori di rischio per la genitorialitŕ giŕ in gravidanza e l'utilitŕ, di conseguenza, di implementare interventi di sostegno al fine di rendere meno problematica l'assunzione del ruolo genitoriale e la strutturazione della relazione madre-bambino.

Abstract:
Come si evince dall’analisi della letteratura, l’approccio transorale ai calcoli prossimali e ilo-parenchimali della ghiandola sottomandibolare rappresenta una valida alternativa alla scialoadenectomia tradizionale. Lo scopo di questo studio è quello di valutare i risultati chirurgici, ecografici e soggettivi di questa tecnica conservativa. Tra Gennaio 2003 e Settembre 2015 sono stati trattati con l’approccio transorale scialoendoscopico-assistito 479 pazienti affetti da calcoli ilo-parenchimali sottomandibolari palpabili, non mobili, di dimensioni superiori ai 7 mm. Tutti i pazienti sono stati sottoposti ad un follow-up clinico, ecografico e ad una valutazione soggettiva dell’outcome chirurgico per mezzo di un apposito questionario somministrato attraverso un’intervista telefonica. Il successo chirurgico definito come completa asportazione del calcolo ilo-parenchimale è stato ottenuto in 472 pazienti (98.5%); in sette casi (1.5%) si è verificato un insuccesso chirurgico, riguardante esclusivamente calcoli parenchimali puri. Un anno dopo l’intervento chirurgico, 408 pazienti (85.1%) non riferivano più sintomi ostruttivi, 59 pazienti (12.3%) riferivano sintomi ostruttivi ricorrenti e 12 (2.6%) infezioni ricorrenti. Dei 54 pazienti (11.2%) affetti da litiasi sottomandibolare ricorrente, 52 sono stati sottoposti ad una seconda procedura terapeutica, nello specifico in 29 casi a scialoendoscopia, in 2 casi a litotrissia pneumatica intracorporea, in 8 casi a chirurgia transorale, in 6 casi ad un ciclo di litotrissia extracorporea, in 7 casi a scialoadenectomia sottomandibolare. La maggior parte dei pazienti (75.2%) ha riferito un dolore post-chirurgico di grado lieve. Al termine del follow-up, i sintomi riferiti da 454 pazienti (94.8%) erano migliorati dopo il secondo trattamento e la ghiandola sottomandibolare affetta era stata preservata nel 98.5% dei casi. La chirurgia transorale scialoendoscopico-assistita dei calcoli ilo-parenchimali della ghiandola sottomandibolare rappresenta un’opzione terapeutica sicura, efficace e conservativa e la preservazione del dotto e del parenchima ghiandolare permette di esplorare il sistema duttale attraverso l’ostio naturale in caso di ricorrenza della patologia. L’utilizzo combinato dell’approccio transorale e delle altre tecniche mini-invasive permette di trattare con successo la maggior parte dei pazienti affetti da litiasi ilo-parenchimale sottomandibolare.

The whole work deals with problems of crisis intervention concentrated on a specific area of working place in the telephone line of confidence. This work includes the theory concerning our theme, from the general characteristics of crises to crisis intervention. Further it engages in the telephonic crisis intervention. It explains main rules of crisis intervention and deals with suicidal themes and questions of ethics. The whole work contains the description, classification, comparison and summary of information and facts. In the conclusion, it focuses on the possibilities and limitations during communication between both parts of telephone calls.

(in English): The topic of my thesis is the suicidal tendencies of elderly people. The topic is examined from the point of view of crisis hotlines. I describe terms such as old age, telephone crisis intervention and basic developmental needs (Pesso Boyden System Psychomotor) in the theoretical part. The following practical part focuses on crisis hotlines in the Czech Republic and their statistics concerning suicidal calls. The next part of the research is the analyses of interviews with social workers working on crisis hotlines and the analyses of notes from the talks with suicidal clients. I propose different instruments for the prevention of the suicidal tendencies of elderly people in the discussion.

The thesis explains the concept of crisis, crisis intervention and telephone crisis intervention, its procedures and techniques. It's focused on history of Helplines not only in Czech Republic but also in the world. It looks closely at the format and principles involved in Helpline call management. The thesis identifies requirements that are imposed on trained specialists and presents some of the institutions that organize training in telephone crisis intervention. Not only Czech but also international organizations focusing on telephone crisis interventions are introduced. A telephone counselors might be at risk for so-cold "burnout syndrome", therefore, the thesis includes chapters devoted to supervision practice and mental hygiene. Finally, the specific Helpline calls are listed.

Abstract:
Syndrom vyhoření je jev, kterým se vědecká obec začala zabývat poměrně nedávno. Teprve před 30 lety se vyhoření dostalo do zorného pole psychologů, kteří začali zkoumat jak vzniká, jaká jsou jeho stádia, co kjeho rozvoji přispívá a jaké jsou možnosti prevence. Postupně se znalosti o vyhoření prohlubovaly a vyvíjely. Naše republika se vyhořením a mechanismy s ním souvisejícími začala zabývat později. Pro studium syndromu vyhoření u nás existuje zázemí v podobě odborné literatury. Stále narůstá počet absolventských prací a dalších výzkumných studií. Vzhledem ke zvětšujícímu se tlaku na pracovní výkon v co nejkratším čase za co nejméně nákladů je třeba se syndromem vyhoření počítat. Důležité je pracovat dál na postupech a preventivních opatřeních, pomocí kterých bude možné rostoucí tlak vyrovnávat a chránit jedince před rizikem vzniku a rozvoje syndromu vyhoření. V této oblasti naše společnost trochu zaostává, prevencí se začala většina zaměstnanců zabývat teprve s pádem komunismu, ale systémy, které se zabývají prevencí syndromu vyhoření, se stále zdokonalují. Pohled odborné veřejnosti na syndrom vyhoření se postupně proměňuje. Nejdřív bylo vyhoření spojováno pouze s pomáhajícími profesemi. Okruh těch, které syndrom vyhoření postihuje, se stále rozšiřuje - začalo se hovořit o tom, že ohrožené jsou i další...

Graduation theses is composed as a theoretical paper and is divided into six chapters. First chapter is focused on concept of crisis. Beside the explication of the concept of crisis, it also describes types of crises according to Baldwin. Posibilities of help in crisis are mentioned at the end of the first chapter. Second chapter is concerned with crisis intervention and it begins with its brief history. It also describes its forms, principals and phases. Third chapter defines and analyses telephonical crisis intervention. As in the previous chapter, it also begins with both czech and world history of telephonical crisis intervention. Other paragraphs describe its advantages and disadvantages. Following paragraphs outline stages of telephonical crisis intervention and a dealing with emotions at helpline. Fourth chapter presents the personality of the telephonical crisis intervention worker and it also mentions problem of burnout. Fifth chapter contains the most common topics of helpline callings. Last, sixth chapter, is focused on analysis of real practical case.

Diploma thesis is focused on the sphere of specific standards and process applied on their creation. Concretely, it is aimed at the branch of telephonic crisis intervention. Specific standards of social services quality are part of the quality control tool that can be used in social services sphere. Contrarily to other tools, specific standards are created, requested and controlled by the leading organization of individual social services providers - in Czech environment it is called association. The thesis introduces standards in context of other quality control tools. Mainly, standards are distinguished from statutory ones. Regarding diploma thesis direction, telephonic crisis intervention is proposed to the reader. The purpose of this thesis is to introduce the problem of suicidal behavior of children, adolescents and young adults as well as the kind of work with such people in telephone crisis intervention. The theoretical part is based primarily on literature written by Czech as well as foreign authors. It describes the phenomenon of suicide, forms and kinds of suicidal behavior with the focus on the influence of developmental characteristics to a suicidal crisis within the above mentioned focus group. Further the specific ways of work and approach to this group of clients are mapped from the point of view of telephone intervention. The ethics of working with a client is treated as a subtheme of this thesis. The practical part studies the approach of workers in phone intervention to suicidal clients and the way workers deal with possible ethical dilemmas. A methodological approach of qualitative research with semi-structured interview was applied. Interviews were conducted with selected participants from Linka Bezpeci.

The diploma thesis tries to do a critical evaluation of potential benefits and risks and limits of Internet psychological counseling. The theoretical part of the thesis describes the the issue of online help from general definition of counselling, through various approaches to definitions of Internet counselling. This part also deals with the history of the online form of help, including the context of development abroad and any differences or specifics in the Czech Republic. Attention is also given to various forms of online psychological help with emphasis on application in our environment. Finally the text introduces potential opportunities and risks of this form of counselling. Empirical part of the diploma thesis focuses on issues of online psychological counseling by a combined research design. The research part of the thesis tries to map the possibilities and limits of web psychological counselling from the perspective of the client. The focus of qualitative research is the analysis of interviews with online counseling clients, supplemented with analysis of chat conversations and case studies. Qualitative results are enriched by quantitatively conceived research. (in English) The aim of this work is to perform an in-depth analysis of the workplace of the children's crisis line - Linka bezpečí, focusing on the work of consultants with repeat callers of this line. The chosen subject was processed by a quantitative method (finding the number of calls with long-term clients on the crisis line during the six-month period, the duration of these calls, the topics that repeat callers bring to the calls on the line, etc.) and the qualitative method - organization a focus group with Linka bezpečí line's staff to find out their potential to work with long-term clients. The main result of the thesis is a summary of the findings, which make the work with the long-term calling clients easy for the workers and which, on the contrary, make it difficult, along with practical tips for practice.

The thesis Safety line workers mental hygiene deals about psychohygiene of consultants, who provide crisis intervention to children and adolescents over the phone. Topics such as mental hygiene and phone crisis intervention are elaborated in the first part of the thesis and the Safety line organization is discussed in detail as well. An empirical part of the thesis deals about types of psychohygiene which are used by the Safety line consultants. It has been discovered, that the Safety line workers use forms of psychohygiene, which the organization has to offer. A chance to discuss specific problems with shift team lead is mostly appreciated by them as well as the fact, that there are teams at the Safety line therefore consultants are not alone for solving issues. Hypotheses used for discovering the connection between socio-demografic entries and psychohygiene haven't been rejected except one. The only hypothesis, which could be rejected, was related to the lengths of practice at the Safety line and level of mental balance. The goal of this thesis was to chart methods of mental hygiene of the Safety line workers and thus to show them ways, how to nurture themselves and how to reach mental equilibrium while working for the Safety line.
