ZusammenfassungDie perkutane koronare Intervention (PCI) wird in Deutschland bei über 350 000 Patienten pro Jahr durchgeführt. Die frühzeitige PCI hat zu einer Verbesserung der Prognose der akuten koronaren Syndrome, insbesondere denen mit ST-Strecken-Hebungs-Myokardinfarkt (STEMI), geführt und wird daher in den aktuellen internationalen und nationalen Leitlinien empfohlen. Neue Studien und die Leitlinien haben sich mit der Auswahl des Zugangswegs, der Identifikation hämodynamisch relevanter Stenosen mittels FFR und iFR und der optimalen Strategie der Revaskularisation bei koronarer Mehrgefäßerkrankung bei Patienten mit akutem Herzinfarkt befasst. Danach sollte in Abhängigkeit von der Erfahrung des Untersuchers der transradiale Zugang bevorzugt werden. Falls kein nicht invasiver Ischämienachweis vorliegt, sollte zur Beurteilung der hämodynamischen Relevanz mittelgradiger Stenosen bei stabiler KHK eine Druckdrahtmessung (FFR oder iFR) erfolgen. Beim STEMI ist bei Patienten mit Mehrgefäßerkrankung nach erfolgreicher PCI des Infarktgefäßes ein zweizeitiges Vorgehen, vorzugsweise mit zwischengeschalteter Ischämiediagnostik, die zu bevorzugende Strategie, sowohl bei hämodynamisch stabilen Patienten, aber insbesondere im kardiogenem Schock.

The topic of this article is the reconstruction of the scope of the contemporary use of visual investigative methods. It begins with a discussion of the social changes and changes of social theory which have led to a substantial redefinition of the form of video and visual techniques for intervention activities applied by the social sciences. Such activities are based on a greater degree of participation of those who will make use of them than was usual in the past, the technology of the creation and distribution of images is much more widespread, and also there has been an important change in the definition of technical images for the use of the social sciences — these all allow their contemporary utilisation, and are described by the author in the form of five models of strategy: advocacy, cultural representation, the animation of local communities, education in visual competence and control of values and safety. In the second part of the article they are presented using selected and representative examples.

Auf Grundlage der wichtigsten strategischen Dokumente, die die Grundlage des behördenübergreifenden Prozesses bildeten, sowie der politischen Zwänge, unter denen die Obama-Regierung operierte, ist es möglich, die Schlüsselfaktoren zu verstehen, die die Formulierung der Strategie bestimmten. Auch erklärt der Artikel die Widersprüche zwischen dem geopolitischen Kontext, den von Obama definierten strategischen Zielen und den Mitteln, diese zu erreichen. Die Analyse erhellt, dass die Politik bestenfalls einen Teilerfolg erzielen konnte.

ZusammenfassungDie Therapie von Knochen und Muskeln im Zusammenhang mit der Osteoporose hat zwei wesentliche Zielpunkte: 1. Ein weiterer Knochenabbau soll verhindert und, wenn möglich, soll ein neuer Knochen wieder aufgebaut werden. 2. Die Muskulatur muss gekräftigt werden, damit der Knochen durch einen zunehmenden Verformungs-Impuls zum Modelling angeregt wird und durch verbesserte Koordination/ Balance das Sturzrisiko vermindert wird. Demzufolge muss eine moderne, suffiziente Therapie der Osteoporose einer Strategie entsprechen, die beide Organsystem gleichermaßen berücksichtigt. Zur Behandlung des Knochens empfehlen die Leitlinien des Dachverbandes deutsprachiger wissenschaftlicher Gesellschaften für Osteologie (DVO; http://lutherhaus.de/osteo/leitlinien-dvo/) Antiresorptiva wie Alendronat, Ibandronat, Raloxifen und Risedronat und Osteoanabolika wie Teriparatid und Strontium-Ranelat, wobei letztgenannte Substanz sowohl antiresorptiv als auch osteoanabol wirkt. Immer soll, soweit es die Evidenzen der klinischen Interventionsstudien nahe legen, der Patient und die Patientin gleichzeitig ausreichend mit Kalzium und Vitamin D3 versorgt werden.Auch die Therapie der Muskulatur kann pharmakologisch erfolgen, wobei nicht primär an androgene Anabolika gedacht wird, sondern eher an Substanzen wie natives Vitamin D3 oder die aktiven D-Hormone wie Alfacalcidol oder Kalzitriol. Die Evidenzen sind aber noch recht schwach und diese Therapieformen haben daher noch keine Berücksichtigung in den Leitlinien des DVO gefunden. Anders sieht es mit dem Muskeltraining aus als Knochenstimulus und zur Sturzrisiko-Reduktion. Zur Kombination von Knochen- und Muskeltherapie bei der Osteoporose fehlen aber noch klinische Evidenzen. Es besteht somit Forschungsbedarf.

This thesis deals with CNB foreign-exchange intervention from November 2013 in context of assumptions and impacts on the Czech economy. The theoretical part presents important monetary policy aspects, from conventional and unconventional tools to inflation targeting and deflation, to understand theoretical framework of analysis. Analytic part takes a look on Czech economy condition before interventions, especially inflation. Intervention impacts and exit strategies are examined further. Based on the analyzed data this thesis concludes that foreign-exchange interventions by Czech National Bank were not necessary and inevitable action.

Abstract:
Infectious diseases still represent one of the major threats for human health due to both their direct and indirect effects on public health and worldwide economies. Despite the current possibility to eradicate or control certain infections like smallpox, polio and measles, the increase in incidence of new infections (so called emerging diseases), or the increase in incidence or geographic range of ones that have existed previously (re-emerging diseases), poses a new threat to public health. To further complicate things, the role of animals in the insurgence and spread of new diseases is central. Of all emerging diseases indeed, the 60.3% originate by, or involve into their cycles, animals, and represent the so called zoonoses. The increase in number of emerging and re-emerging infections and their potential to fast spread into animal and human populations make central the development of tools to reduce human infection risk. Epidemiological studies become then central to understand the relationship among events, investigate their causal effects and understand risk factors. Despite of that, classical epidemiology, centred on the study of the relationships between events, show limits in the investigation of mechanisms underlying infection spread, and in considering the interactions among populations, thus possibly leading to simplistic and spurious conclusions. Mathematical modelling instead, and the development of a “dynamical epidemiology”, allows the investigation of dynamics of infections, thus providing us a mechanistic point of view to understand infection spread. The strengths of mathematical modelling applied to epidemiological studies are several. At first, they can investigate the extent to which an event can mechanistically influence another consequential event. This characteristic of mathematical modelling has a great application in public health, as it allows to prioritise interventions or studies on those events that have a major impact on disease outbreak. Another strength of mathematical modelling is its ability to describe the dynamics of an infectious disease by accounting for interactions among populations and sub-groups of populations within the same population. At least, mathematical modelling permits for theoretical investigations of mechanisms of transmission and to answer to “what if?” questions, allowing to explore theoretical scenarios which have not yet occurred or which needs to be preventively tested, like the application of an intervention strategy. With the present work we then provide four applications of mathematical modelling to infectious disease. We focused on two wildlife-originating infections: West Nile virus (WNV) and baylisascariasis. Both infections are emerging or re-emerging in Italy and can represent a threat for human beings due to their possible severe outcomes. Due to the potential harm they are for human beings, a thorough surveillance and a wide intervention and control plans are ongoing both to promptly identify the presence and circulation of their causative agents and to reduce human infection risk. A full understanding of WNV cycle is fundamental to reduce human infection risk, but several knowledge gaps still exist, especially on the role played by different bird species involved in its spread. For both infections moreover, despite several are the control strategies proposed a quantitative analysis of their performance has never been performed. Aimed at filling these gaps, we developed a mathematical model to simulate WNV spread, and used it to explore mechanisms driving infection spread. We found birds recovery rate and mosquito biting rate having the major influence on disease spread and thus being the most urgent mechanisms to be investigated via field and laboratory experiments. Birds’ susceptibility and their competence to infection have a negligible influence on disease spread, thus making investigations to understand them of secondary importance. These results might be of aid also in defining the characteristics of a bird species to be a good WNV spreader, by focusing the attention on species that have a small recovery rate or are frequently bitten by mosquitoes. Moreover, we found a negative effect of birds’ abundance in affecting WNV prevalence in mosquitoes, further helping us in distinguishing among species that are suitable to have a role in WNV spread. We then exploited the built model to explore intervention strategies against WNV. We showed that a reduction of the vector population is more effective than a reduction of birds’ abundance in an area. In particular, the best efficacy is shown by the reduction of mosquito breeding sites, followed by the active elimination of their eggs and larvae. On the contrary, reducing the abundance of competent birds or their reproductive sites can obtain an increase in human infection risk. Similarly, we also studied the effectiveness of different intervention strategies to reduce the number of Baylisascaris procyonis eggs in the environment. The ingestion of B. procyonis eggs indeed is the cause of baylisascariasis, an infection that can have severe health consequences in human beings. With our work we explore the effects both in terms of efficacy (i.e., potential to eliminate eggs from environment) and efficiency (i.e. the timing needed) of three different intervention strategies. The interventions tested are: the active culling of raccoons, raccoons’ anthelmintic treatment and faeces removal. We found that raccoon culling might have the best and faster results, highlighting the importance of assessing the intervention on the base of an objective prove on its efficacy. With the proposed work then, we highlighted the role of mathematical modelling in epidemiological studies, by, at first, exploiting their potential to investigate the extent to which an event can influence another consequential event. Secondly, we used them to describe the dynamics of an infectious disease, by accounting for interactions among populations, and focusing on mechanisms underlying infection spread. Moreover, we also exploited them for theoretical investigations, like the simulation of the application of an intervention strategy to reduce human infection risk is. In conclusion, mathematical modelling can widely help our understanding and management of infectious diseases through a new and different point of view from that provided by classical epidemiology. Mathematical modelling indeed includes the investigation of spreading mechanisms and non-linearity of interactions among individuals and subgroups of populations, thus allowing a more complete comprehension of diseases spread. The cooperation of diverse health professionals is fundamental to fully exploit both classical epidemiological studies and dynamics ones, and the effects of their cooperation can lead to a better knowledge of infections and a consequent reduction of human infection risk.

This diploma thesis analyzes the approach of the Soviet Union and that of the US-led coalition to state-building in Afghanistan, which today, as in the past, takes place against the backdrop of counterinsurgency warfare. The analysis in the field of politics, economy and security shows that in both cases the intervening powers have focused on building a strongly centralized system of government, in spite of the fact that the Afghan countryside's relationship to the Kabul-based government had traditionally been characterized by broad autonomy. The intervening powers' efforts have futhermore been associated with the export of exogenous political structures and for this reason attention is also drawn to the question to what extent the intervening powers' approach to the modernization of Afghan society has contributed to the escalation of unrelenting conflict.

The aim of this study was to investigate intervention strategies that have the potential to produce treatments reducing rotavirus-related disease in swine. Rotavirus is the biggest cause of viral gastroenteritis in young swine so is a huge economic burden to farmers. Cell lines deficient for innate anti-viral mechanisms were used to passage rotavirus. This novel vaccine strategy was investigated for its ability to elicit virus that is dependent on the modified cell lines for efficient replication and growth. Rotavirus isolates dependent on modified cell lines represent promising attenuated viral vaccine candidates. Whilst no rotavirus isolates were discovered with this characteristic, analysis of the viral genome after serial passage revealed non-synonymous single nucleotide polymorphisms causing non-conserved mutations at the protein level. Rotavirus isolates with these mutations may cause cell line dependency. A second intervention strategy used computer-assisted analysis (in silico) to reveal short potentially therapeutic peptides based on possible epitopes of rotavirus capsid proteins. Each synthetic peptide was not shown to block the rotavirus-host cell interaction in vitro. Peptides with this property could be used as candidate peptide-based anti-viral blocking drugs. In addition, peptides did not block the neutralising ability of anti-rotavirus antibodies found to be present in pig serum, which would then be used as immunogens to raise neutralising antibodies against rotavirus. However, inactivated rotavirus was found to bind serum antibodies in vitro and therefore appears to hold more promise as a therapeutic strategy than the synthetic peptides. Antibodies present in sow serum were discovered to cross-react with four different lab adapted strains of rotavirus. In contrast, antibodies directed against a common porcine genotype (G5P[7]) were not crossreactive against other strains. This study has initiated research into treatments against rotavirus-related disease in swine but potential vaccine and therapeutic candidates have not yet been revealed.

Guided by the cognitive development view of reading interventions, this qualitative case study addressed the lack of intervention programs in middle schools in a large city in New Jersey State. The purpose of the study was to examine teachers' perceptions of intervention programs for low-performing readers. Research questions addressed the intervention strategies to improve students' learning, school culture, and teaching styles used to teach them. Survey questions were sent to 25 teachers chosen randomly from 5 schools in the school district. Surveys were analyzed by developing codes, clustering themes, and then developing a full description from the teachers' perspectives. The findings showed the need for a policy recommendation to address the gaps in the intervention programs, reduce students' reading predicaments, and enhance teacher participation in constant, job-embedded professional development. These findings will contribute to positive social change by informing a school-wide positive reading culture across these middle schools.

This thesis examines the impact that third-party identity and techniques have on mediation outcome. The roles of the OAS and the Carter Center in the negotiations between the Venezuelan government and the opposition (DC), during the period 2002-2003, and the implementation of the agreement in 2004 are compared as representing track I and track II actors and styles. Using a process-tracing methodology, five conflict mappings and stages of conflict are combined with the results of focused interviews to main participants of the negotiation process. The analysis shows a significant impact of third-party identity and strategies on the outcome of mediation. Moreover, the outcome is more likely to be successful when track II actors, actually track I ½, participate as mediators in the actual negotiations. The most effective strategies used by third parties, dependent on the timing of the intervention and the stage of conflict, are communication and formulation strategies.

In spite of decades of intensive study of practically all aspects of management, it still seems to be almost impossible to complete large development projects according to originally predicted timescales and cost. Examples are the Concorde supersonic airliner, the Channel Tunnel, the Humber Bridge. This research started with the premise that the future is not predictable, and has concentrated on addressing the life-cycle dynamics of projects involved in the development of complex systems. It has also focused on the human characteristics of organisations with initial convergence to "open systems" concepts and the need to increase orderliness with increasing complexity i.e locally decreasing entropy. The theme that runs through the thesis is that of assessing the current risk, and the likely tendency that such risk will increase or decrease in the future, that the project will be completed according to claims made by the contractor in his Bid to the customer. An analysis of data from four actual projects together with various subjective appraisals by managers who were involved in them, and an assessment of the current state of related knowledge, has resulted in the formulation of a new type of management method. The "new" aspects of the method relate to its ability to take into account the dynamics of the project circumstances as the project products pass through the design, manufacturing, testing, and operating phases. This is done by "taking soundings" deep within the projects working infra-structure. During this development a number of conventional concepts have not been used. For example, the concept that a company exists within an environment that can be represented by hard-lined diagrams has been avoided. The method involves the use of static and dynamic risk indicators, open and closed loop systems, and the utilisation of patterns constructed from real time data to identify whether the project dynamics are in a steady state, turbulent or chaotic condition. The method also contains an "intervention" function as a necessary element to ensure that the project and corporation strategic aspects are adequately considered. The method has been developed in a pragmatic manner such that it can be implemented by a practising manager.

This book is a collection of the academic contributions presented at the conference entitled "Lessons from earthquakes: sources of vulnerability, new design strategies and regulatory developments" which was held at Chianciano Terme on 8 October 2010. The issues addressed are central to Seismic Engineering and comprise a wide range of arguments on both consolidated subjects and innovative aspects in the sector. Among these, appropriate attention is devoted to: analysis of the structural instability revealed on the occasion of seismic events and the lessons that can be drawn from the same; the procedures of assessment of the existing buildings, starting from the phase of monitoring and diagnostics through to the definition of the most opportune intervention techniques; the use of composite materials and alternative methods of seismic protection; non-linear field modelling relating to regular and non-regular structures; and finally, the development of the methods of calculation that have characterised the evolution of the regulatory codes.

Nella prima parte dell’articolo esporrò brevemente alcune delle caratteristiche dei soggetti trattati che contribuiscono a rinchiuderli nella categoria della marginalità. Successivamente descriverò la filosofia alla base di Cà dell’Arcoveggio e la metodologia educativa messa in atto. Nella terza parte mi occuperò delle pratiche adottate al fine di istituire un rapporto tra la struttura ed il quartiere circostante, tra i soggetti che ospitava e i cittadini provenienti dall’esterno. Infine mostrerò come alcuni importanti cambiamenti abbiano radicalmente ridimensionato la portata sperimentale del progetto. Through the specific case study of Cà dell'Arcoveggio I will treat the issue of drug addiction as a cause of social marginalization, and will discuss which strategies have been put into place to remove the inhabitants from this categorization. In the first section of the article I will briefly explain some of the characteristics of the subjects treated that contribute to their placement in the category of marginality. Then I will describe the underlying philosophy and methodology of the Cà dell'Arcoveggio educational intervention. In the third section I will deal with the practices put in place in order to establish a relationship between the physical structure and the surrounding district and between the subjects and residents of the neighborhood. In the final section I will present how some crucial changes have radically decreased the experimental nature of the project.

Cannabis is commonly used by young adults (18-25), and these transitional years can be crucial as they tend to have the highest rates (5%; 1.8 million in the U.S.) of Cannabis Use Disorder (CUD; SAMHSA, 2016). Current survey data suggest 25% of university students use cannabis monthly or greater and 6% daily (Schulenberg et al., 2019) and even higher (38-39%) in states with legal recreational cannabis use (Pearson et al., 2017). Burgeoning research has shown that chronic, heavy cannabis use is associated with a host of negative consequences, including symptoms/diagnosis of CUD, as well as anxiety, depression, and psychosis, among other health issues (Arseneault et al., 2002; Buckner et al., 2010; Degenhardt et al., 2003; Midanik, Tam, & Weisner, 2007; Simons et al., 2010; Solowij & Pesa, 2010). With the number of young adults diagnosed with CUD, there is a growing need for intervention strategies. However, rates of individuals seeking formal treatment for cannabis use has been low (Prince et al., 2020). With widespread access, it has become easier to employ mobile technology for health interventions (Kumar et al., 2013). The overall objective of this systematic review was to explore the utilization of different mHealth interventions for cannabis users to investigate different types and ways these interventions are implemented, targeted outcomes, along with the types of studies being conducted (e.g., RCT, feasibility, interviews, usability, etc.). Methods: A search of PsycInfo and PubMed was conducted for relevant articles with the following search terms: mHealth app/web-based intervention/internet-based intervention/telehealth/JITAI/EMI/Ecological Momentary Intervention/Just-in-Time Adaptive Intervention/in the moment/Mobile Intervention/Smartphone Intervention/Wearable Devices AND Cannabis/Marijuana Peer-reviewed original research in English was targeted, which focused on mHealth interventions for adults (over 18 years of age) targeting cannabis use and needed both a mobile intervention and provided data on at least one outcome (e.g., reduced craving or cannabis use) or feasibility of intervention. Qualitative, quantitative, and mixed methods were all included. Results: The total number of records identified that fit the criteria based on a review of titles and abstracts were 33 (9 from PsycInfo and 24 from PubMed). After duplicates were removed, 27 were screened and 9 records were excluded for failure to meet search criteria after full review of the articles. The remaining records (n=18) were assessed for eligibility and data were extracted. Based on preliminary findings, the majority have been feasibility and usability studies (83%). Roughly half were conducted in the U.S., with the rest in European countries, and one in Singapore. The timeframe for using the mobile intervention ranged from 1-hour (for usability studies) to 8 months in duration. The types of mobile interventions have varied from text messaging, using a smartphone app, to directing individuals to websites. Roughly 50% have been standalone mHealth interventions, while the other half have been paired with face-to-face counseling sessions. The majority of these interventions have personalized intervention strategies for their participants.

<p>The paper deals with three intervention techniques implemented for the rehabilitation and strengthening of reinforced concrete (RC) structures in the Bulgarian construction practice. Some of the main details of the conceptual design are presented: 1) identification of vulnerabilities and evaluation of deficiencies; 2) understanding the performance of the existing structures;</p><p>3) determining the level of intervention; 4) analysis of possible intervention materials; 5)assessing available information, documents, historical context, etc.; 6) preparing individual strategies for interventions.</p><p>The analyzed examples give the basic of different procedures for improvement the performance of three classic reinforced concrete structures. The analysis aims (1) to discover the problems in the design of interventions in the specific cases, (2) to discuss the question for interventions, (3) to show how interventions on existing structures can be reached successfully under current conditions and restrictions.</p><p>Based on the presented case studies some general recommendations for the realization of the strengthening procedures in buildings are given.</p>

Sustainable behaviour is the basis of social and environmental sustainability. In everyday practice, public facilities can play an important role in promoting such behaviour. Determining how city users understand and use public facilities is crucial to design for sustainability. Drawing on the literature on behavioural change, this study investigates the relationship between design interventions and user behaviour through an analysis of everyday life scenarios in open spaces. Taking public design in Guangzhou, China as a case study, the study examines how intervention strategies affect behaviour through public facilities. We explore how these effects vary with intervention intensity and how city users respond accordingly. The study develops a framework to illustrate the relationship between behaviour change and design intervention, which reveals that changes to user behaviour can be realised through several approaches, depending on the level of intensity of the intervention. The study also analyses each type of intervention based on the dimensions of effectiveness and user acceptance. Finally, from the perspective of inclusiveness, design interventions should be multidimensional and integrated to help achieve long-term sustainability.

Abstract As the subsea well intervention sector ascends from a double-dip downturn, operators are motivated to execute projects with increased agility. By adopting new rationale and supported by strategic partnerships, an enhanced focus on operating efficiency is targeted. This paper discusses how operators can – deliver predictable operational expenditure (opex) results, safeguarding life-of-field economics through routine interventions and robust risk mitigation – align objectives with service providers through technology, strategy, and process – realize value of integrated alliances in the subsea intervention sector. Reliability is the cornerstone on which operational integrity is established. Repeatability is the foundation on which partnerships are formed, forging a synchronized philosophy; pledging operational efficiency as commanded in a dynamic environment; bringing to market a holistic, multileveled, multiskilled solution to subsea interventions; and uniting product and operational experts to evaluate the range of scope requirements selecting fit-for-purpose solutions, delivered with synchronized processes. Here we describe how this approach, coupled with repeatable field-proven product and service reliability and driven by true customer-centric thinking, minimizes operating risk to deliver predictable outcomes. This ethos is demonstrated in action via three case studies that discuss the benefits of combining customer expertise with product and operational specialists during the initiation and planning phases of subsea well intervention projects. The shared examples demonstrate how inculcating a continuous improvement mindset and applying lessons learned to product design, process, and procedures can drive operating efficiency to new levels. Campaigns executed in 2020 by the supplier delivered best-in-class results, exceeding customer expectations. The campaign represented the first fully synchronized service operation on the Q5000 intervention vessel at 90% operating efficiency. In addition, they demonstrated outstanding versatility by outperforming budget on the first deployment of the 15,000 psi intervention riser system (IRS) from the Q4000 intervention vessel. Less than 1% operating nonproductive time (NPT) was attributed to the well access package for the campaign. Sustaining this level of success via collaborative performance management is the next step on the roadmap to ensure this becomes the new normal. Conclusions drawn from customer feedback indicated expectations were surpassed, and that synchronicity is the key that unlocks the door to sustainability in the subsea deepwater intervention sector.

Abstract Resource-consumption systems can be defined by a resource inlet, a control volume where the resource is used, and a waste outlet. Specific to water, many existing conservation strategies focus on reducing the in-flow of water into a control volume. Instead, this work explores reducing waste out-flow, which causes accumulation in the control volume. This strategy aims to motivate users to reduce resource in-flow in response to accumulation in the control volume, and thus modify behavior. To test this strategy, Amazon Mechanical Turk workers completed three randomly ordered handwashing simulations with different sink-outflow rates online. Study participants (N = 74) significantly reduced consumption of water when it accumulated quickly in the sink (p &lt; 0.0001). Participants reduced water consumption, on average by 14% at lower outflow rates, as they decreased inflow rates to prevent sink overflow. Many pro-environmental behavior interventions are limited in their reliance on user motivation and intention to reduce resource consumption. In contrast, the reduced-outflow intervention significantly reduced water usage (p &lt; 0.001) of individuals, regardless of self-reported daily pro-environmental behavior. This result suggests that the developed intervention relies less on user intention. Overall results support that reducing outflow can increase sustainable user behavior when properly executed. In-person testing is discussed as future work.

Background. Timely discharge of hospitalized patients can prevent patient harm, improve patient satisfaction and quality of life, and reduce costs. Numerous strategies have been tested to improve the efficiency and safety of patient recovery and discharge, but hospitals continue to face challenges. Purpose. This Technical Brief aimed to identify and synthesize current knowledge and emerging concepts regarding systematic strategies that hospitals and health systems can implement to reduce length of stay (LOS), with emphasis on medically complex or vulnerable patients at high risk for prolonged LOS due to clinical, social, or economic barriers to timely discharge. Methods. We conducted a structured search for published and unpublished studies and conducted interviews with Key Informants representing vulnerable patients, hospitals, health systems, and clinicians. The interviews provided guidance on our research protocol, search strategy, and analysis. Due to the large and diverse evidence base, we limited our evaluation to systematic reviews of interventions to decrease hospital LOS for patients at potentially higher risk for delayed discharge; primary research studies were not included, and searches were restricted to reviews published since 2010. We cataloged the characteristics of relevant interventions and assessed evidence of their effectiveness. Findings. Our searches yielded 4,364 potential studies. After screening, we included 19 systematic reviews reported in 20 articles. The reviews described eight strategies for reducing LOS: discharge planning; geriatric assessment or consultation; medication management; clinical pathways; inter- or multidisciplinary care; case management; hospitalist services; and telehealth. All reviews included adult patients, and two reviews also included children. Interventions were frequently designed for older (often frail) patients or patients with chronic illness. One review included pregnant women at high risk for premature delivery. No reviews focused on factors linking patient vulnerability with social determinants of health. The reviews reported few details about hospital setting, context, or resources associated with the interventions studied. Evidence for effectiveness of interventions was generally not robust and often inconsistent—for example, we identified six reviews of discharge planning; three found no effect on LOS, two found LOS decreased, and one reported an increase. Many reviews also reported patient readmission rates and mortality but with similarly inconsistent results. Conclusions. A broad range of strategies have been employed to reduce LOS, but rigorous systematic reviews have not consistently demonstrated effectiveness within medically complex, high-risk, and vulnerable populations. Health system leaders, researchers, and policymakers must collaborate to address these needs.

The purpose of this document is to provide readers with the coding protocol that authors used to code 36 research syntheses (including meta-analyses, evidence-based reviews, and quantitative systematic reviews) focused on mathematics interventions for students with learning disabilities (LD), mathematics learning disabilities (MLD), and mathematics difficulties (MD). The purpose of the systematic review of mathematics intervention syntheses was to identify patterns and gaps in content areas, instructional strategies, effect sizes, and definitions of LD, MLD, and MD. We searched the literature for research syntheses published between 2000 and 2020 and used rigorous inclusion criteria in our literature review process. We evaluated 36 syntheses that included 836 studies with 32,495 participants. We coded each synthesis for variables across seven categories including: publication codes (authors, year, journal), inclusion and exclusion criteria, content area focus, instructional strategy focus, sample size, methodological information, and results. The mean interrater reliability across all codes using this coding protocol was 90.3%. Although each synthesis stated a focus on LD, MLD, or MD, very few students with LD or MLD were included, and authors’ operational definitions of disability and risk varied. Syntheses predominantly focused on word problem solving, fractions, computer- assisted learning, and schema-based instruction. Syntheses reported wide variation in effectiveness, content areas, and instructional strategies. Finally, our results indicate the majority of syntheses report achievement outcomes, but very few syntheses report on other outcomes (e.g., social validity, strategy use). We discuss how the results of this comprehensive review can guide researchers in expanding the knowledge base on mathematics interventions. The systematic review that results from this coding process is accepted for publication and in press at Learning Disabilities Research and Practice.
