Full textIntersectoral collaboration for health is widely recognised as a critical component of interventions to address complex public health issues. However, there is limited research that has examined how intersectoral approaches are formulated and implemented, especially in low- and middle-income countries. As a result, although the literature is populated with calls for action, little exists that can inform the evidence on how to sustain intersectoral action for health. This thesis is a case study of intersectoral action in the Western Cape Province of South Africa, examining the unfolding policy formulation and implementation processes of an initiative referred to as First 1,000 Days, in the period 2016 to 2019. Within early childhood, the First 1,000 Days (FTD) period presents a favourable window for intersectoral interventions that can ensure positive outcomes from early years of life to adulthood. The FTD initiative emerged in the Western Cape Province of South Africa in response to the growing number of children exposed to the social challenges of violence and of alcohol and drug abuse.

Full textInequalities and other complexities produced by the capitalist system and the retraction of the state in the social area are producing, and reaffirming, problems and situations against which knowledge and actions focused, fragmented and sector are unable to explain and tackle. Based on this understanding, the present study was to conduct an investigation about the propositions and debate recent on intersectionality and social policy in the Brazilian scene, trying to characterize the state of the art productions academic- scientific elaborated in the period 1986-2012. Consists, then, in a study based on literature review, a qualitative, whose cutting it back to the productions identified along with bibliographic databases bank summaries of the Coordination of Improvement of Higher Education Personnel (CAPES) and the Scientific Electronic Library Online (SCIELO). We sought to identify studies produced on intersectionality and social policies, as well as treatment theoretical and methodological given to this subject from the examination of literature production identified and selected. We attempted to grasp the concepts and analytical dimensions of intersectionality and social policy and the possible heterogeneity of approaches, and develop the main consensus and disagreements theorists around the theme. Finally, we proceeded to the demarcation of the major theoretical and practical challenges put to the implementation of intersectoral cooperation in the field of social policy. The results indicate the presence of a diversity of understanding regarding the term intersectionality that sometimes has been related only to the technical dimension, functional and management in the management and social intervention, with this debate free, in large part, the analysis critique of the contradictory processes that underlie social policies. The research also showed the advancement of critical debate on intersectionality and social policies, especially in recent years, relating the technical, ethical and political, economic, cultural and social-related discussion and propositions about intersectionality and social policies.

Full textThis study applied input-output principles to the analysis of the relative importance of the primary sectors and specially the forest sector to the Acrean economy, as well as its dependency towards the Rest of the Northern region and the Rest of the Brazilian region. The results also allowed for the characterization of the productive structure of the state of Acre, its key sectors and multiplier effects of each economic sector in terms of production, employment, income and imports. The initially formulated hypothesis that the primary sector and the commerce and services sector form the basis of the Acrean economy was confirmed. The hypothesis that the forest macro-sector, formed by the Plant Extractive, Silviculture, Wood Mills, Furniture Industry, Pulp, Paper and Similar, Editorial and Printing, and Rubber Industry sectors, is a key sector was not confirmed. Its role as one of the most important direct employers was confirmed though. The Hirschman-Rasmussen indices point to the following key-sectors: Basic Metal Industry, Other Metals Textiles, Communications, Food Industry, Chemicals, Industrial Services for Public Utilities, and Finance. The pure index reveal the following sectors as the most important: Commerce, Public Health, Construction, Enterprise Services, and Other Public Management Services. The most important sectors in terms of production, employment, income and import multipliers, simultaneously, are the sectors dealing with the Meat and Food Industry. Individually, the most important sectors in terms of production are the Textiles and Basic Metal Industry; of employment are the Chemical and Communication sectors; of income are the Basic Metals and Automotive sectors; and of import are the Health and Wood Industry. A weak link among all regions is observed when production in each region (Acre, rest of the Northern, and rest of Brazil) is decomposed into production induced by internal demand and production induced by external demand. Meanwhile, when demand varies in Acrean sectors, production is more affected in the Rest of Brazil region than in the Rest of the Northern region. The Acrean Forest Growth Product estimated by the input-output matrix calculated in this study is R$ 17,78 million, which represents 1,15% of the total Acrean Growth Product. This study contributes with data that will allow comparisons of present and future inter-sector and inter-regional relationships; especially after the Acrean economy becomes integrated to neighbor countries and to Pacific ports by new roads.

Adolescents deprived of liberty under socio-through closed-Educational Center of the Adolescent (ECA), belonging to FUNDAC, have different needs for its development, among these needs, health. These needs are brought by the Child and Adolescent (ECA) as guarantees and rights that must be provided in full. However, what is perceived is the camouflage of such guarantees due to stigma and prejudice that permeates socio reality. In this perspective, the study brought the objective of analyzing the process of intersectoral relationships built from the cross between the politics of health and social protection policy adolescents deprived of freedom, in view of the expansion of social rights and thus outline how intersectoral action are drawn between management levels. We decided to conduct a search of an exploratory nature, given that the theme is not explored, especially in terms of an intersectoral approach. Also, do not nullify the bibliographic and documentary research, as this is the starting point for a preliminary understanding. For data analysis, we use the qualitative dimension in order to intensify the study, not being exempted from a quantitative approach. With respect to the instruments of data collection forms used for each group of respondents: Professional Education Center Adolescent (ECA), with 28% sample of the universe; professionals in public health network represented by the specialists of the Municipal Hospital Santa Isabel and the Center for Integrated Healthcare (CAIS Jaguaribe) whose universe was 104 professionals, the sample was 15% of the universe. Regarding the choice of the group of professionals in the public health system, was a survey of referrals to the demands and thus identified the referral services to which adolescents are repeatedly referred for specialized care. The study results showed that the normative point of view and documentary is actually built plans and specifications that address the health care of adolescents deprived of freedom in Paraíba. Furthermore, the investigated reality is pervaded by significant gaps and weaknesses in how access to the health of adolescents is provided. The professionals can identify the lack of dialogue between social policies and their impacts, caused by the lack of intersectoral actions. With respect to managers, it was realized that there is dialogue regarding the creation of the State Operating Plan (POE) with a view to systematizing and promoting the health of adolescents in conflict with the law, however not yet been translated into a building intersectoral, given the no involvement of other actors and sectors of administration. One can then conclude from the fact that the intersectoral investigated is of an immense challenge, because access to the health of the adolescent becomes a practice based on the law, this view should be the three management levels and their respective institutions and organs of social control.
Os adolescentes privados de liberdade em cumprimento de medida socioeducativa em meiofechado do Centro Educacional do Adolescente (CEA), pertencente à FUNDAC, apresentam diversas necessidades para o seu desenvolvimento, dentre estas a saúde. Tais necessidades são trazidas pelo Estatuto da Criança e do Adolescente (ECA) como garantias e direitos que devem ser providos de forma integral. No entanto, o que se percebe é o escamoteamento de tais garantias em função do estigma e preconceito que permeiam a realidade socioeducativa. Nesta perspectiva, o estudo trouxe como objetivos analisar o processo de intersetorialidade, construído a partir das relações transversais entre a política de saúde e a política de proteção social ao adolescente privado de liberdade, na perspectiva da ampliação dos direitos sociais e assim delinear como as ações intersetoriais são desenhadas entre as esferas gestoras.Optou-se por realizar uma pesquisa de natureza exploratória, tendo em vista que a temática ainda é pouco explorada, sobretudo no que tange à intersetorialidade. Ademais, não anulamos a pesquisa bibliográfica e documental, pois esta constitui o ponto de partida para uma compreensão preliminar.Para análise dos dados, utilizamos a dimensão qualitativa com vistas ao aprofundamento do estudo, não sendo dispensada a abordagem quantitativa. Com relação aos instrumentos de coleta dos dados, utilizamos formulários específicos para cada grupo de pesquisados: profissionais do Centro Educacional do Adolescente (CEA), com amostra de 28% do universo; os profissionais da rede de saúde pública representados pelos médicos especialistas doHospital Municipal Santa Isabel e do Centro de Atenção Integral à Saúde (CAIS de Jaguaribe), cujo universo era de 104 profissionais, a amostra foi de 15% do universo. Com relação à escolha do grupo dos profissionais da rede de saúde pública, foi realizado um levantamento dos encaminhamentos das demandas e assim identificados os serviços de referência para onde os adolescentes recorrentemente são encaminhados para atendimentos especializados. Os resultados do estudo apontaram que do ponto de vista normativo e documental há de fato planos e normas construídos que contemplam o atendimento à saúde do adolescente privado de liberdade na Paraíba. Ademais, a realidade concreta investigada é perpassada por lacunas e expressivas fragilidades na forma como o acesso à saúde dos adolescentes é provido. Os profissionais conseguem identificar a falta de diálogo entre as políticas sociais e seus impactos, provocados pela ausência da intersetorialidade nas ações. Com relação aos gestores, percebeu-se que há diálogo com relação à criação do Plano Operativo Estadual (POE) com vistas à sistematização e promoção da saúde do adolescente em conflito com a lei, todavia não se traduz ainda numa construção intersetorial, haja vista o não envolvimento deoutros atores e demais setores da gestão. Pode-se então concluir, a partir da realidade investigada, que a intersetorialidade se reveste de um imenso desafio, pois para que o acesso à saúde do adolescente se transforme numa prática pautada no direito, esta visão deverá ser das três esferas gestoras e suas respectivas instituições e órgãos de controle social.

Violence in traffic as one of the expressions of the social question, requires public policy strategies that prioritize road safety to life at the expense of the vehicle. Thus, this study aimed to "analyze the implementation of road safety public policy on combating violence in traffic in Manaus." And specific: 1) Categorize the "accidents" of traffic with victims occurred in Manaus in 2015, giving priority to signal their nature, the main roads where they occur and the profile of the victims; 2) Aim prevention strategies to reduce "accidents" transit with victims in Manaus; 3) Identify the vision of the management of road safety public policy about intersectoral action to prevent violence in traffic in Manaus. The exploratory research with quantitative and qualitative approaches, was based on data from the documentary research of Management Reports of locus research institutions and the application form to the nine (9) area managers Traffic, Transportation, Health and Infrastructure, and of the laws on the subject on the screen in order to contextualize the Traffic Insurance and Social Law, emphasizing the regulatory framework of the Road Safety Policy. It stands out among other results, that pedestrians and motorcyclists are in vulnerable groups in the context of Manaus transit. Moreover, it was found that the survey participants institutions recognize the need to prevent the "accidents" of traffic, highlighting interventional projects, mainly in the area of Traffic Education as Transversalizando o Trânsito and Vida no Trânsito. The vision of managers unravels the difficulties in planning and implementation of public policy measures, especially the lack of political will and financial resources for this purpose. Thus, this study presents itself as a starting point that draws the picture of violence in traffic in Manaus, giving visibility to Public Policy for Road Safety.
A violência no trânsito como uma das expressões da questão social, requer estratégias de políticas públicas de segurança viária que priorizem a vida em detrimento do veículo. Neste sentido, este estudo teve como objetivo geral “Analisar a operacionalização da política pública de segurança viária no enfrentamento à violência no trânsito em Manaus”. E os específicos: 1) Categorizar os “acidentes” de trânsito com vítimas ocorridos em Manaus no ano de 2015, priorizando assinalar sua natureza, as principais vias onde ocorrem e o perfil das vítimas; 2) Apontar as estratégias de prevenção para redução dos “acidentes” de trânsito com vítimas em Manaus; 3) Identificar a visão dos gestores da política pública de segurança viária acerca das ações intersetoriais de prevenção à violência no trânsito em Manaus. A pesquisa exploratória, com abordagens quantitativa e qualitativa, baseou-se em dados oriundos da pesquisa documental dos Relatórios de Gestão das instituições lócus de pesquisa e da aplicação de formulário à nove (9) gestores das áreas de Trânsito, Transporte, Saúde e Infraestrutura, bem como das legislações acerca do tema em tela, a fim de contextualizar o Trânsito Seguro como Direito Social, enfatizando os marcos regulatórios da Política de Segurança Viária. Destaca-se dentre outros resultados, que os pedestres e motociclistas constituem-se em grupos vulneráveis no contexto do trânsito de Manaus. Além disso, verificou-se que as instituições participantes da pesquisa reconhecem a necessidade de prevenção aos “acidentes” de trânsito, destacando projetos interventivos, principalmente, na área de Educação para o Trânsito como Transversalizando o Trânsito e Vida no Trânsito. A visão dos gestores revela as dificuldades no planejamento e execução das medidas de políticas públicas, principalmente pela falta de decisão política e recursos financeiros destinados para esse fim. Assim, esse estudo apresenta-se como um marco inicial que traça o retrato da violência no trânsito em Manaus, dando visibilidade a Política Pública de Segurança Viária.

Full textThe Brazilian population is heading towards change in demographic profile, accompanied by epidemiological transition with increasing number of affected by chronic diseases. One of the Ministry of Health\'s strategies is the National Program Promoting Health with a view to promoting quality of life and reduce vulnerability and risk to health. Allied to this, in 2011 the Ministry of Health launched the Health Academy Program. The program centers are spaces with equipment, structure and professionals, in order to promote health and care delivery. This study investigates the Health Academy program in two cities in the state of São Paulo in relation to the political cycle and identify limits on inclusion in the network of health care. For the analysis regarding the policy cycle, the data were extracted through literature review and document analysis. As for the empirical case study, aimed at identifying the program limits, semi-structured interviews with managers and professionals were held. We interviewed 05 managers and 10 professionals. The analysis was made from the dimensions: political and historical context, organizational model and management model of the poles through successive approximations. A simple descriptive analysis of the information processed were created database in Stata 12 platform is performing. The agenda entry in the cities studied meets the policies adopted within the federal government through the induction methods used by the health ministry. The induction process was accentuated by the poor state of government presence in decision making and implementation of federal programs. The state involvement occurred formative way. The implementation of projects are nonspecific, bringing little data on its proposed action, integration with the network of health care, specifically the Primary, and the intersectoral network of community support. The articulation with the assistance of network occurs irregularly and fragile. Lack of knowledge of the aims for professionals and managers, which hindered labor relations. Although attention belong to the network, the program is shifted contextually, since there is no integration between the service and others. Services are held in isolation for each professional and each health unit. Although introduced in a context of changing relations between the federal entities is configured as an action that is still implemented through strong federal induction to the municipality and through incentives and financial resources to support a federal agenda

This study analyzes intergovernmental and intersectoral relations in the implementation of daycare centers and the Brasil Carinhoso (BC) program, with a focus on cooperation between the Federal government and municipalities, from the municipal perspective. It seeks to understand how this policy is implemented locally and what the federal government (and on a lesser scale, the state government) has done to support municipalities in policies aimed at this educational segment. It recognizes the importance of actions of the Federal government through their regulatory and equalization functions that seek to ensure a certain uniformity and parameters in the implementation of public policies. It also shows that there is cooperation between the federal government and municipalities through programs and other forms of technical support, especially since the first decade of this century. The study starts with an analysis of Federal regulations between 1988 and 2015 and their influence on enrollment (2001-2015) and variations between States. It acknowledges that the expansion of access to daycare and municipal participation in federal programs occurred gradually and unevenly between municipalities. The study shows that the Fund for the Maintenance and Development of Basic Education and Enhancement of Education Professionals and the Brasil Carinhoso (BC) program, an initiative of the Brazil Without Misery plan both had an impact on enrollment and had mechanisms to influence municipal actions. BC was chosen because of the extensive participation in this initiative in the educational field (97% of Brazilian municipalities in 2015), because of impact it had on access to daycare for children from low-income families and because it was conceived as an intersectoral intervention. To complement this analysis, multiple case studies were carried out in 6 municipalities in two Brazilian states to identify the different ways day care policy and BC were implemented in Farias Brito, Horizonte, and Sobral in the State of Ceara, and Tarumã, Votuporanga and Osasco in the State of São Paulo). The study reveals different interpretations regarding the implementation of the right to daycare and about technical and financial cooperation between states, as well as differences in the implementation in municipalities. The Legacy of educational policy and child services in the two states explain part of these differences. The Federal government has expanded its coordination and cooperation with municipal governments, through a range of strategies, but decision-making authority has remained with the federal government, while implementation is by the municipalities, with restricted and at times informal spaces for negotiation. Contrary to expectations, states turned out to have an important role in intergovernmental cooperation, but very few states have played this role. The research suggests that, despite advances in access to daycare, there is a need to improve intersectoral coordination and BC has not led to effective intersectoral action at the local level. The challenge remains of meeting the goal established by the PNE of meeting 50% of children in daycare by 2024, and reducing inequality of access.
A pesquisa analisa as relações intergovernamentais e intersetoriais na implementação da política de creches e na ação Brasil Carinhoso (BC), com destaque à cooperação da União com os municípios, a partir da perspectiva municipal. Busca-se compreender como se dá a implementação desta política localmente e o que o governo federal (e, em menor escala, o governo estadual) tem feito para apoiar os municípios nas políticas voltadas a este segmento educacional. Reconhece a importância da ação da União com suas funções reguladora e equalizadora que procuram garantir certa uniformidade e parâmetros na implementação de políticas públicas; mas também mostra que há cooperação do governo federal com os municípios por meio de programas e outras formas de apoio técnico, em especial a partir dos anos 2000. O trabalho parte da análise dos atos normativos da União, de 1988 a 2015, e identifica sua influência nas matrículas (2001-2015) e a variação entre os estados. Reconhece que a ampliação do acesso às creches e a adesão municipal aos programas federais ocorre de forma gradual e desigual nos municípios. O estudo mostra que o Fundo de Manutenção e Desenvolvimento da Educação Básica e de Valorização do Magistério (Fundeb) e a ação Brasil Carinhoso (BC), iniciativa do plano Brasil sem Miséria, impactaram o acesso às matrículas e ambos tinham mecanismos de indução. O BC foi escolhido pela grande adesão à iniciativa no eixo da educação (97% dos municípios brasileiros em 2015), pela extensão com que promoveu o acesso à creche de crianças pobres e por ter sido concebido de forma intersetorial. Complementarmente à análise, são desenvolvidos estudos de casos múltiplos em seis municípios de dois estados brasileiros para identificar as diferentes implementações da política de creches e do BC (Farias Brito/CE, Horizonte/CE, Sobral/CE, Tarumã/SP, Votuporanga/SP e Osasco/SP). A pesquisa revela diferentes interpretações sobre a materialização do direito à creche e sobre a cooperação técnica e financeira dos estados, bem como diferenças na implementação nos municípios. O legado da política educacional e da atenção à criança nos dois estados explica em parte essas diferenças. A União tem ampliado a coordenação e a cooperação com os governos municipais, utilizando-se de diversas estratégias, permanecendo a autoridade decisória com o governo federal, ficando a implementação com os municípios, com espaços de negociação restritos e por vezes informais. Diferentemente do que era esperado, observou-se que os estados têm um papel importante na cooperação intergovernamental, mas são poucas unidades federativas que realizam este apoio. A pesquisa sugere que, apesar de avanços no acesso às creches, ainda é preciso aprimorar a articulação intersetorial e o BC não promoveu a intersetorialidade no âmbito local. Full textO programa federal de transferência condicionada de renda Bolsa Família (PBF) criado em 2003 no Brasil tem como eixo principal a redução da pobreza e o combate à fome e,como um de seus objetivos, a promoção de ações intersetoriais do Poder Público. Com o objetivo de analisar as possibilidades e limites destas ações, esta pesquisa abordou a experiência de implementação local do programa em Manguinhos, RJ. Através da estratégia metodológica de estudo de caso foram realizadas entrevistas semi-estruturadas com gestores e profissionais das áreas da saúde, assistência social e educação e observação local. Além disso, foi realizado levantamento documental privilegiando a consulta à legislação e aos documentos oficiais do programa, durante os anos de 2007 e 2008. A análise do processo de implementação do Bolsa Família em Manguinhos mostrou que a criação de instâncias locais como o Grupo Intergestor e o Grupo Descentralizado viabilizou parcerias e interlocução voltada à construção de iniciativas comuns entre as secretarias municipais de saúde, educação e assistência social. No entanto, a comunicação vertical e horizontal entre os setores e os atores chave locais permaneceu frágil. Os sistemas de informação eram superpostos e não apoiavam a integração das ações e a centralização das decisões em torno do uso do Índice de Gestão Descentralizada (IGD) na secretaria de assistência social dificultava a consolidação de compromissos entre os diferentes parceiros e setores ligados ao programa. Ao mesmo tempo, o envolvimento das equipes de Saúde da Família, através de treinamento com profissionais das Coordenações regionais de saúde e assistência social, em junho de 2008, apontou novas possibilidades principalmente em relação à compreensão sobre o Programa, à proximidade com a comunidade, e o alcance de metas no contexto local. O estudo revela, portanto, a necessidade de repensar os fluxos de informação e decisão através do fortalecimento de arenas locais onde técnicos, gestores, lideranças comunitárias e população beneficiária possam compartilhar esforços para o fortalecimento de ações intersetoriais.
The federal program of conditioned transfer of income Bolsa Família (BFP), which was created in Brazil in 2003, has as principal issue the reduction of poverty and the combat against hunger and, as one of its objectives, the promotion of intersectoral actions of the government. With the objective of analyzing the possibilities and limits of these actions, this research approached the experience of local implementation of the program at Manguinhos, RJ. Through the methodological strategy of case-study, semi-structured interviews with managers and professionals of the health, welfare and education areas were done and local observation. Besides, a documental survey was done, privileging the consultation of the legislation and of the program official documents, during the years of 2007 and 2008. The analysis of Bolsa Família implementation process at Manguinhos showed that the creation of local instances as the Intermanaging Group and the Decentralized Group made feasible partnerships and interlocution referring to the construction of common initiatives between health, education and welfare municipal departments. However, the vertical and horizontal communication between the sectors and the local key-actors continued fragile. The information systems were overlapped and didn’t support the integration of the actions, and the centralization of the decisions related to the use of the Decentralized Management Index (Indice de Gestão Descentralizada - IGD) in the welfare department made difficult the consolidation of commitments between the different partners and sectors linked with the program. At the same time, the involvement of the Family Health (Saúde da Família) teams, through training with professionals of the health and social assistance regional co-ordinations, in June 2008, pointed to new possibilities especially in respect to the comprehension about the Program, to the proximity with the community, and the aims reach in the local context. The study revels, therefore, the necessity of reconsidering the information and decision flows through the strengthening of local arenas where technicians, managers, stakeholders, community leaderships and beneficiaries can share efforts to the strengthening of the intersectoral actions.

Full textNeste estudo procede-se a análise do processo de implementação descentralizada das condicionalidades do Programa Bolsa Família (PBF) no âmbito do SUS. Considerando que se trata de Programa recém criado e que traz novas requisições para o setor saúde, interessa identificar fatores que facilitam e também essenciais obstáculos postos a este processo. Toma-se o município de Niterói para o estudo de caso por entender que esta localidade reúne atributos favoráveis à observação local da implementação das condicionalidades da saúde visto a história de consolidação da sistema local de saúde (notadamente a atenção básica), onde se destaca a experiência do Programa Médico de Família. O trabalho empírico realizado reporta-se na primeira dimensão à análise documental e entrevistas com gestores e profissionais executores diretos do PBF no município de Niterói- RJ, apoiando-se em demais procedimentos metodológicos entre os quais se destaca a observação participante. Num primeiro movimento analítico abordam-se as origens dos Programas de Transferência de Renda, seu desenvolvimento histórico e tendências recentes quer no plano internacional quer em uma focalização mais estrita no caso brasileiro. Destacam-se questões afetas à associação entre assistência e trabalho e a reconfiguração do direito social traduzida no percurso mais recente que vai da incondicionalidade do direito ao direito condicional. Neste debate acentua-se a polêmica em torno desta nova geração de programas sociais especialmente o Programa Bolsa Família e sua relação com a trajetória da política social entre nós.Em seguida procura-se estabelecer um quadro geral de reflexão acerca da concepção e desenho do Programa Bolsa Família pondo relevo nos seus principais eixos ordenadores e em aspectos que parecem apontar para inflexões no campo da política social. Analisam-se as características do desenho do PBF, bem como a estrutura de indução e estratégias de implementação formuladas pelo governo federal. No itinerário perseguido busca-se elucidar o tema política social e intersetorialidade partindo dos dilemas que remontam os descaminhos da seguridade social brasileira e enfatizando-se a perspectiva inscrita hoje nas promessas de promover a articulação intersetorial a partir das condicionalidades do Programa Bolsa Família e as possíveis formas de interação com o sistema de proteção social. Por fim, o estudo privilegia a análise da implementação local do Programa Bolsa Família procedendo-se: (i) a identificação dos significados que os gestores e profissionais da saúde atribuem ao PBF; (ii) a análise da dinâmica política, institucional e operacional da implementação das condicionalidades do PBF na saúde e (iii) ao exame do processo de implementação local da gestão intersetorial e suas imbricações com o Bolsa Família. Os resultados da pesquisa buscam capturar os principais desafios do setor saúde para implementar as condicionalidades do PBF, indicando promessas, riscos e possibilidades, confirmando que tais desafios são inerentes não apenas ao campo da saúde, mas a gestão local que mostra dificuldade para executar um programa social intersetorial. Com base no aprofundamento que o estudo de caso permite, as análises efetivadas sugerem um conjunto de problemas concretos relacionados à condução, coordenação e gerenciamento do Programa Bolsa Família que afetam o processo de implementação das condicionalidades da saúde.
This study provides an analysis of the decentralized process of implementation of the conditionalities involved in Brazil‟s Family Allowance Program (PBF) within the scope of the National Health System (SUS). Considering that the program was recently created and places new demands on the health sector, it is important to identify facilitating factors and major obstacles in this process. The city of Niterói in Greater Metropolitan Rio de Janeiro is used as a case study, since the municipality presents favorable characteristics for local observation of the implementation of health conditionalities in the PBF, given the city‟s history of consolidation of the local health system, especially primary care, and featuring the experience with the Family Doctor Program. The first dimension of the empirical research drew on document analysis and interviews with administrators and professionals directly involved in the Family Allowance Program in the city of Niterói, supported by other methodological procedures, especially participant observation. The first analytical stage focused on the origins of income transfer programs, their historical development, and recent international and Brazilian trends, highlighting issues related to the association between assistance and work and the reconfiguration of social rights (recently moving from unconditional to conditional ones). This debate features the controversy surrounding the new generation of social programs – especially the Family Allowance Program – and their relationship to social policy history in Brazil. Next, the study attempts to establish an overview of the reflection concerning the concept and design of the Family Allowance Program, emphasizing its main organizational characteristics and aspects that appear to indicate important changes in the social policy field. Characteristics in the program‟s design are analyzed, along with the induction structure and implementation strategies developed by the Federal government. The discussion seeks to shed light on the issue of social policy and intersectoral collaboration, based on the dilemmas that date to historical setbacks in the Brazilian social security system and emphasizing the current perspective of promises to promote intersectoral linkage based on the conditionalities of the Family Allowance Program and possible forms of interaction with the social protection system. Finally, the study features an analysis of the local implementation of the Family Allowance Program with the following: (i) identification of meanings ascribed to the program by its managers and professional staff; (ii) an analysis of the political, institutional, and operational dynamics of implementing the program‟s health conditionalities; and (iii) examination of the local implementation of intersectoral management and its linkages to the Family Allowance Program. The study‟s results seek to reflect the principal challenges for the health sector in implementing the conditionalities of the Family Allowance Program, suggesting promises, risks and possibilities, confirming that such challenges are inherent not only to the health field itself but also to the local administration, which is experiencing difficulty in executing an intersectoral social program. Based on the in-depth approach provided by the case study, the analyses suggest a set of actual problems related to the coordination and management of the Family Allowance Program that affect the implementation of its health conditionalities.

Full textThis paper discusses the challenges to be overcome in reality management (not) integrated BPC program at school in the city of Palmas - TO, considering that this is a welfare benefit guaranteed by social security policy laid down by the Constitution of 1988 and regulated by the Organic Social Assistance Act (1993) to the disabled person whose welfare system must be guaranteed and discussed widely and democratically, as the perspective of political and administrative decentralization in public activities in order to better access and participation of civil society, at the municipal level, through social relationships that develop processes able to build a new community participation in the format of public social policies management and exercise social control through the Local management Board as representative of relationship between space civil society and the state (municipality) aimed at overcoming the difficulties presented by people in decisions and control of the local and sectorial actions in which occurs the agency relationship, in order to promote the full development of citizenship and run the program regulations. Also discusses the historical conception of Assistance policy as a landmark welfare prevalent in traditional relationships still present in local contemporary practices, poor conditions and turnover of professionals in the management of social assistance policy, and human intersectorials challenges and materials that are still present in the field of social rights to the full guarantee of a social safety net for people with disability in the city of Palmas - TO.
Esta dissertação discute os desafios a serem superados na realidade da gestão (não) integrada do programa BPC na escola no município de Palmas TO, tendo em vista ser este um benefício assistencial garantido por meio da política de seguridade social, previsto pela Constituição Federal de 1988, e regulamentada pela Lei Orgânica da Assistência Social (1993), à pessoa com deficiência, cujo sistema de proteção social deve ser garantido e discutido de forma ampla e democrática, conforme perspectiva da descentralização político-administrativa nas ações públicas, visando melhores condições de acesso e participação da sociedade civil, em âmbito municipal, por meio de relações sociais que desenvolvam processos capazes de construir novo formato de participação da comunidade na gestão das políticas sociais públicas e exercer controle social por meio do conselho gestor local como espaço representativo de relação entre a sociedade civil e o Estado (município), que visem superar as dificuldades apresentadas pela população nas decisões e no controle das ações local e setorial em que ocorre a relação de representação, a fim de promover a construção integral da cidadania e executar as normativas do programa pesquisado. Discute ainda, a concepção histórica da política de assistência como marco assistencialista predominante nas relações tradicionais ainda presentes nas práticas contemporâneas municipais, as precárias condições e rotatividade de profissionais na gestão da política de assistência social na realidade estudada, e os desafios intersetoriais humanos e materiais que ainda se fazem presentes no campo dos direitos sociais para a garantia integral de uma rede de proteção social a pessoa com deficiência no município de Palmas TO.

A presente pesquisa teve por objetivos: compreender, quanto à fundamentação teóricofilosófica e organizativa, como os Documentos do MS tratam a pesquisa em saúde no Brasil; delinear, a partir da fundamentação teórico-filosófica e organizativa, presente no conjunto de documentos oficiais do MS que trata da pesquisa para a saúde no Brasil, uma Rede Ecossistêmica de Pesquisa em Enfermagem/Saúde nos HUs Federais, para auxiliar na consolidação do SUS. Traçou-se a seguinte TESE: É possível delinear, a partir da fundamentação teórico-filosófica e organizativa, presente no conjunto de documentos oficiais do MS que trata da pesquisa para a saúde no Brasil, uma Rede Ecossistêmica de Pesquisa em Enfermagem/Saúde nos HUs Federais, para auxiliar na consolidação do SUS. Para sustentar a referida tese, procurou-se, no referencial contextual e teórico, os temas: Sistema Único de Saúde – SUS; Hospitais Universitários Federais na perspectiva ecossistêmica; Pensamento Sistêmico, numa Abordagem Ecossistêmica; Interfaces da Pesquisa Ecossistêmica nos HUs Federais e a Pesquisa Ecossistêmica em rede na enfermagem/saúde. Ele mostrou-se coerente, consistente e ancorador em relação à análise e à interpretação dos dados e permitiu aprofundar o conhecimento sobre a temática. O caminho metodológico caracterizou-se como exploratório, descritivo, com abordagem qualitativa. Para capturar os dados, utilizou-se um instrumento de pesquisa documental, estruturado em três pilares básicos, que contemplaram os aspectos capazes de abranger o escopo da proposta. Contemplando objetivo, questão de pesquisa e tese foram desenvolvidos, com base nos dados capturados, três artigos: SUS e a Pesquisa Ecossistêmica em Enfermagem/Saúde: estratégia de inovação na produção de ciência, tecnologia e assistência em saúde; Rede Ecossistêmica de Pesquisa em Enfermagem/Saúde nos HUs Federais e Rede Ecossistêmica de Pesquisa em Enfermagem/Saúde nos Hospitais Universitários Federais: uma possibilidade para a Pesquisa em Enfermagem/Saúde. Os resultados do primeiro artigo mostraram que, ao promover a Pesquisa Ecossistêmica em Enfermagem/Saúde nos HUs Federais, existem possibilidades de produzir inovação no pensar e fazer e, assim, alcançar resultados mais positivos na assistência, induzindo à sustentabilidade ao Sistema. O segundo enfatiza que as ações de pensar e fazer, balizadas pelo princípio sistêmico integrador, expresso em rede, podem aumentar as interfaces entre pesquisadores de diversas áreas investigativas e suas concepções metodológicas e, assim, alavancar ciência e tecnologia e capacitação científica ao trabalhador de enfermagem/saúde e inserir ganho na prestação da assistência/saúde. O terceiro demonstra que, por meio de um modelo mental ancorado em referencial ecossistêmico, é possível delinear uma Rede Ecossistêmica de Pesquisa em Enfermagem/Saúde em HUs Federais. Considera-se que o SUS necessita ser constantemente repensado, a fim de buscar e obter soluções pertinentes; precisa inovar, evoluir, especialmente na pesquisa, e promover qualidade no ensino, na assistência à saúde e no ambiente, até mesmo, para a sua própria manutenção como sistema de saúde.
The objective of this research was to outline an Ecosystemic Net of Research in Nursing/Health in Federal University Hospitals in order to help consolidate the Integrated Healthcare System, based on the theoretical-philosophical and organizational basis which is found in official documents issued by the Ministry of Health about research in health in Brazil. The following THESIS was analyzed: In order to help the consolidation of the Integrated Healthcare System, it is possible to outline an Ecosystemic Net of Research in Nursing/Health in Federal University Hospitals, based on the theoretical-philosophical and organizational basis which is found in official documents issued by the Ministry of Health about research in health in Brazil. In order to support this thesis, the theoretical section of this study involved these themes: the Integrated Healthcare System, Ecosystemic Research in the Integrated Healthcare System and Federal University Hospitals from the perspective of Ecosystemic Research. It was coherent, consistent and primordial regarding the analysis and the interpretation of data and enabled to deepen knowledge of the theme. The methodology was characterized as exploratory and descriptive in a qualitative approach. In order to collect data, a specially designed documentary research tool was used; its structure has three basic pillars which comprised the fundamental aspects and some secondary ones that were able to embrace the whole scope of the proposal. Regarding the objective, the question and the thesis, three papers, based on the data collected in the official documents issued by the Ministry of Health, were written: “The Integrated Healthcare System and Ecosystemic Research in Nursing/Health: an innovation strategy in the production of science, technology and healthcare”; “An Ecosystemic Net of Research in Nursing/Health in Federal University Hospitals”; and “ An Ecosystemic Net of Research in Nursing/Health in Federal University Hospitals: a possibility of research in Nursing/Health”. The results of the first paper showed that, when Ecosystemic Research in Nursing/Health is promoted in Federal University Hospitals, it is possible to bring innovation to thoughts and actions which aim at getting more positive results in healthcare and to make the Integrated Healthcare System more sustainable. The second paper highlights that actions and thoughts that are mediated by an integrative systemic principle, expressed in a net, may strengthen the interfaces among researchers from different areas and their methodological conceptions. Consequently, science and technology will be enhanced, workers in Nursing/health will get more scientific knowledge and healthcare will be considerably enriched. The third one shows that, based on a mental model attached to ecosystemic references, an Ecosystemic Net of Research in Nursing/Health may be outlined in Federal University Hospitals. The Integrated Healthcare System needs to be constantly reviewed so that it may look for and find adequate solutions, innovate, evolve - mainly in research - and improve the quality of teaching, of healthcare and of the environment in order to be maintained as an integrated healthcare system.
La presente investigación tuvo los seguientes objetivos: comprehender, cuanto la fundamentación teórica-filosófica y organizativa, como los documentos del MS tratan la investigación en salud en Brasil; delinear, partiendo de la fundamentación teórico-filosóficas y organizativa, presente en el conjunto de documentos oficiales del MS que trata de la investigación para la salud en Brasil, una Red Ecosistémica de Investigación en Enfermería/Salud en los HUs federales para la consolidación del SUS. Fue elaborada la siguiente TESIS: Es posible delinear, partiendo de la fundamentación teórico-filosófica y organizativa, presente en el actual conjunto de los documentos oficiales del MS que proviene de la Investigación de la Salud en el Brasil, una Red Ecosistémica de Investigación de Enfermería / Salud en los HUs Federal, para ayudar a la consolidación del SUS. Para sostener la referida tesis, fue buscado en el referencial contextual y teórico, los temas: Sistema Unificado de Salud – SUS; Hospitales Universitarios Federales en el enfoque ecosistémico; Pensamiento sistémico, un enfoque ecosistémico; Interfaces de Investigación de Ecosistémica en los HUs Federales y la Investigación Ecosistémica en red en la enfermería/salud. Él resultó ser coherente, consistente y anclado en relación al análisis y la interpretación de datos y permitió profundizar conocimientos sobre el tema. El enfoque metodológico es caracterizado como exploratorio, descriptivo, con enfoque cualitativo. Para capturar los datos, se utilizó la herramienta de investigación documental, estructurado en tres pilares básicos, que contemplan aspectos capaces de cubrir el alcance de la propuesta. Contemplando objetivo, la cuestión de investigación y tesis han desarrollado, sobre la base de los datos capturados, tres artículos: SUS y la Investigación Ecosistémica en Enfermería/Salud: Estrategia la innovación en la producción de ciencia, tecnología y cuidados de salud; Red Ecosistémica de Investigación en Enfermería /Salud en los Hospitales Federales y Red Ecosistémica de Investigación en Enfermería/Salud en los Hospitales Universitarios Federales: una oportunidad para la Investigación Enfermería/Salud. Los resultados del primer artículo mostraron que, mediante la promoción de la investigación ecosistémica de Enfermería / Salud en los HUs Federales, existe la posibilidad para producir la innovación en el pensamiento y la acción y así lograr resultados más positivos en la asistencia, induciendo la sostenibilidad del Sistema. El segundo enfatiza las acciones de pensar y hacer, impulsado por el principio sistémico integrador, expresado en red, pueden aumentar las interfaces entre los investigadores de diferentes áreas de investigación y sus puntos de vista metodológico y por lo tanto, dar importancia a la ciencia, la tecnología y la formación científica del trabajador de la Enfermería/Salud y inserir puntos positivos a la prestación de atención/salud. El tercero muestra que a través de un modelo mental anclado en un referencial ecosistémico, es posible delinear una Red Ecosistémica de Investigación en Enfermería/Salud en HUs Federales. Se considera que el SUS tiene que ser constantemente repensado a fin de solicitar y obtener las soluciones adecuadas; para innovar, desarrollar, especialmente en la investigación y promover la calidad en la enseñanza, en la asistencia a la salud y el ambiente, incluso para su propio mantenimiento como sistema de salud.

Full textThe so called Support Houses are institutions that give attention to the HIV/Aids individuals and who, due to reasons of social nature, are in need of care and provisory or permanent housing. They assist the basic necessities of the people and give support to their living condition of HIV positive patients as a guarantee of treatment, familiar insertion, social and working rights support, entertainment and psychological support. This study uses the triangulation of qualitative and quantitative methods, through different techniques, to characterize the HIV/Aids Support Houses in the state of São Paulo and to analyze its insertion as modality of assistance support net, contextualizing the Non-Governmental Organizations in the ambit of health public politics for the HIV/Aids. The study in course registers a concentration of houses located in the capital city. In the sustainability aspect, the fund raising is diversified, with governmental public resources, financing of international organisms, people?s donation and community support. The Support Houses, a result of the civil society organization, represent a great effort of the community to guarantee the assistance of different necessities of HIV/Aids patients, mainly in the psychic and social aspects. These organizations have been used by the health system as support reference to the net of assistance to HIV patients and individuals, if not in a formal way, at least in an informal one. Its insertion in the net of SUS financing may represent a great advance in the formalization of an intersectional action that is already a fact, optimizing the use of resources, qualifying the services offered, at the same time that the results of public financing made for STD/Aids are maximized
