Full textUltrasound of carotid arteries is a duplex ultrasonography that has the ability toimage as well as measure the flow rate in the vessels. With this analysis it ispossible to detect any changes in the shape of plaque, stenosis and occlusions incarotid arteries. Internal carotid artery (ICA) is considered to be an essentialvessel, supplies the anterior part of the brain with blood. Internal carotid stenosisleads to several severe symptoms such as transient ischemic attack, transientblindness and stroke. The main reason of these symptoms and changes in thevessels is atherosclerosis. Degree of stenosis is considered very important to givepatients the proper treatment. The aim of this study is to investigate therelationship between the degree of stenosis evaluated with maximum systolic flowrate in ICA and degree of stenosis evaluated with calculated flow rate ratio ICAand common carotid artery (CCA) and end diastolic flow rate in ICA. The studyincluded a total of 200 vessels (right and left), of which 130 vessels were used.Parameters needed for the study were vessels with stenosis degree of internalcarotid artery, end diastolic flow rate in ICA and maximum systolic flow rate inICA and CCA. The patients had different degrees of stenosis between <40% ->95% in the right and left ICA. The rate ratio ICA/CCA was calculated and used toinvestigate the relationship using statistical analysis methods. The results of thestudy showed that there is a strong and significant correlation between the degreeof stenosis assessed with maximum systolic flow rate in ICA and the degree ofstenosis assessed with the rate ratio and end diastolic flow rate. In contrast, therate ratio ICA/CCA has stronger compliance with the degree of stenosis assessedat maximum systolic flow rate, compared with that for end diastolic flow rate.This leads to the conclusion that the rate ratio can be used for a safe assessment ofthe degree of stenosis in ICA and should with great certainty reflect the degree ofstenosis in ICA.

Resumo: Objetivo ¿ Avaliar os efeitos da terapia de reposição hormonal sobre os índices de pulsatilidade das artérias uterinas e carótidas internas em mulheres na pósmenopausa. Sujeitos e Método ¿ Realizou-se um ensaio clínico prospectivo, controlado, aleatorizado e duplo cego. Oitenta mulheres foram alocadas e acompanhadas por 12 semanas em dois grupos de tratamento: 40 pacientes receberam 2mg de estradiol associados a 1mg de acetato de noretisterona de forma contínua, comparadas com 40 pacientes que receberam placebo. Setenta e seis mulheres concluíram o estudo, 38 em cada grupo. Foram realizadas medidas dos índices de pulsatilidade das artérias uterinas e carótidas internas através de ultra-sonografia bidimensional com Doppler em cores utilizando equipamento Aloka SSD 2000 com transdutor linear de 7,5MHz para as artérias carótidas e de 5,0MHz para as artérias uterinas. As aferições foram realizadas antes do início, com quatro e 12 semanas de tratamento. Análise dos dados ¿ Foi utilizado o teste t de student para amostras independentes na comparação das médias entre os grupos. A comparação das médias entre grupos ao longo do tempo foi feita com análise de variância para medidas repetidas (ANOVA). O coeficiente de correlação de Pearson foi utilizado para testar a correlação entre as diversas variáveis. Resultados ¿ A comparação das características das mulheres nos dois grupos mostrou que foram semelhantes em relação à idade, tempo de menopausa, índice de massa corporal, pressão arterial, colesterol total e frações e estradiol plasmático pré-tratamento. Nas artérias uterinas houve queda significativa no índice de pulsatilidade ao redor de 32%, atingindo o máximo na 12ª semana do estudo no grupo de usuárias de TRH. Nas artérias carótidas internas, os resultados mostraram que não houve diminuição significativa do índice de pulsatilidade nos grupos estudados. Observou-se através do coeficiente de correlação linear, que as variáveis idade e índice de massa corporal mostraram-se diretamente associadas ao índice de pulsatilidade da artéria carótida interna. No grupo que utilizou placebo, não houve alterações significativas entre quatro e 12 semanas de tratamento quando comparadas aos valores pré-tratamento. Conclusão ¿ A Terapia de Reposição Hormonal contínua foi efetiva em reduzir o índice de pulsatilidade da artéria uterina, sendo que este mesmo efeito não foi observado nas artérias carótidas internas. O efeito observado neste estudo sugere que a terapia de reposição hormonal combinada com estrogênios atua de forma diversa e individual sobre os vasos, dependendo do território estudado
Abstract: Objective: To compare the short-term effects of oral hormone replacement therapy (HRT) and placebo on carotid and uterine vascular impedance. Methods: Eighty postmenopausal women were randomized to 3 months treatment with oral continuous combined HRT or placebo. Carotid and uterine arteries pulsatility indices (PIs) were assessed by color Doppler at baseline, and after 4 and 12 weeks of treatment. Seventy-six women completed the trial, 38 in the TRH group and 38 in the placebo group. The study had a 90% power to detect a difference between treatment groups of 0.05 in the carotid artery and of 0.25 in uterine artery PI at the 5% significance level. Results: The carotid PI did not decrease significantly in both groups. In the uterine arteries, the drop in PI was steeper and greater and reached its maximum at 3 months (32%). Drops in carotid PI correlated positively with baseline PI values, but were affected positively by age, time since menopause and BMI. Drops occurred at despite of the supposed counteract effect of norethisterone acetate. In the placebo group, there was no significantly difference between 4, and 12 weeks of treatment with the baseline. Conclusion: Oral continuous HRT are effective at 12 weeks in reducing impedance to flow in uterine circulation. This effect did not occur in the carotid circulation. This short-term vascular effect suggests that the vascular effect of HRT is not the same when comparing different territories

Full textEl riesgo de hipoperfusión cerebral durante la endarterectomía carotídea es el principal factor limitante de esta técnica. El shunt permite mantener parcialmente la perfusión cerebral durante el clampaje. Analizamos el comportamiento hemodinámico de las arterias cerebral media y carótida interna ipsilaterales durante la intervención. Se ha realizado un estudio preoperatorio de la hemodinámica intracerebral mediante dúplex transcraneal junto con una monitorización ecográfica de la arteria cerebral media intraoperatoria y una medición de la presión cruenta en carótida común e interna durante la cirugía. Hemos observado una relación positiva significativa entre la presencia o ausencia de circulación colateral en el estudio prequirúrgico y el mantenimiento tanto de presiones carotídeas como de velocidades en la arteria cerebral media durante la intervención. El análisis de los patrones de circulación colateral nos permite identificar a los pacientes con mayor riesgo de presentar isquemia cerebral intraoperatoria y de requerir la utilización de shunt.

Full textIntroduction: Endovascular treatment of extracranial internal carotid artery (ICA) lesion in the setting of tandem occlusions (TIO) is a therapeutic challenge. On the one hand, recanalization rate with fibrinolytic agents is lower compared to isolate intracranial occlusions. On the other hand, there are two competing rationales: to establish and maintain extracranial ICA patency, and to avoid hemorrhagic transformation, one of the leading causes of early neurological worsening and poor functional outcome. Methods: Prospective observational study of consecutive patients admitted to the Emergency Room of Vall d’Hebron University Hospital with TIO undergoing EVT. Extracranial ICA lesion was treated with stenting (balloon-expandable [BX] or self-expandable [SX], depending on the study period) or angioplasty alone. Primary outcome was a combined positive outcome composed by absence of extracranial ICA high-grade restenosis or reocclusion (efficacy) and absence of hemorrhagic transformation (HT) (safety) at 24 hours. Secondary outcomes included efficacy, safety, and clinical and functional outcomes. A pre-specified subanalysis was performed to assess differences between BX and SX stents. Results: One-hundred thirteen patients fulfilled the inclusion criteria. Patients undergoing stenting presented more frequently the combined positive outcome (65.3% vs. 19.5%, P< 0.001) and less frequently extracranial ICA high-grade restenosis or reocclusion (22.2% vs. 80.5%, P< 0.001) than angioplasty alone patients. Similar rates of hemorrhagic transformation, symptomatic hemorrhagic transformation, clinical, and poor functional outcomes were found between groups. Extracranial ICA high-grade restenosis or reocclusion was independently related to poor functional outcome (OR 11 5.76, 95% CI 1.10-14.22). BX-stenting presented less frequently early neurological worsening (3.4% vs. 23.3%, P= 0.041) and tended to present a lower frequency of symptomatic HT (3.4% vs. 16.3%, P=0.069) than SX-stenting. Conclusions: Hyperacute extracranial ICA stenting in the setting of TIO presents more frequently the combined positive outcome of efficacy and safety than AA. BX stents are a promising alternative to SX stents in this particular scenario, due to the similar rate of extracranial ICA reocclusion or high-grade restenosis, a lower frequency of early neurological worsening, and a tendency to present less cases of symptomatic hemorrhagic transformation. Extracranial ICA reocclusion or high-grade restenosis predicts poor functional outcome.

Full textIntracranial aneurysms are the most common lethal predisposition amongst young adults. Its understanding remains limited to date while the development of new innovative endovascular treatments are increasingly available and allow for the treatment of more and more complex aneurysms with a non negligeable rate of complications. Most of the previous studies on intracranial aneurysms are based on low informative clinical series and the use of limited numerical simulation methods. They almost exclusively target the intrasaccular mechanical phenomena irrespective of the changes in the parent vessel induced by the aneurysm. In vitro, the use of silicone aneurysms embedded in a cardiovascular simulator showed an impact of the aneurysm on the the parent vessel flow conditions characterized by a decrease of its resistance. In vivo, flow MRI allowed to quantify this effect by analyzing the volumetric flow rate curves. Downstream to the aneurysm, the blood flow was dampened and presents a systolic diastolic demodulation with a collapse of resistive and pulsatility indexes. This effect was strongly correlated to the aneurysm volume. The flow diverter stents allowed for a measurable « hemodynamic reconstruction » of the parent vessel by restoring a normo modulated flow, and normal resistive and pulsatility indexes. An original method for the segmentation of internal carotid artery in 2D phase contrast MRI was proposed. It is based on the application of the Fourier Transform on the phase images and by taking into account the temporal coherence of velocities within the voxel. Full textStroke is among the leading causes of morbidity and mortality in the developed countries. This motivated a search for the configurations of vasculature that is assumed to be associated with the development of vascular diseases. In the first contribution we improve a vascular segmentation method to achieve robustness in segmenting images coming from different imaging modalities and clinical centers and we provide exhaustive segmentation validation. Once the vasculature is successfully segmented, in the second contribution we propose a methodology to extensively characterize the geometry of the internal carotid artery (ICA). This includes the development of a method to automatically identify the ICA from the segmented vascular tree. Finally in the third contribution, this automatic identification is generalized to a collection of vessels including their connectivity and topological relationships. Identifying the corresponding vessels in a population enables comparison of their geometry using the methodology introduced for the characterization of the ICA.
