Abstract:
This chapter examines community psychiatric service provision for mentally disordered offenders, focussing on the United Kingdom and United States. In doing so, it acknowledges that mentally disordered offenders are at risk of rejection and of falling between services. They are doubly stigmatized by having a mental illness and being offenders. It explores the context, commissioning, components of a service, and models of care (including the evidence base for them). Also considered are the pros and cons of specialist services, as well as how they might differentiate their task from that of the CMHT. The chapter concludes by considering how services can work together to meet the needs of this complex and challenging group of patients.

Abstract:
This chapter provides a brief outline of the development of advance care planning (ACP) and a selective summary of the current evidence base of its benefits and challenges. It discusses the variety of policy, clinical, and legal definitions, and the interface between decision making and documentation. The concept of ACP has evolved over the years towards a broader relational communication, and this chapter also considers how conversations can help patients and their families prepare for death, review their immediate goals and hopes for the future, and strengthen their relationships. The convincing evidence base for ACP is now well established, and despite international variations in definitions and forms of implementation, there is robust evidence of its benefits within person-centred end-of-life care.

Abstract:
Within a theory of morphopragmatics, we give an account of the relationship between morphology and pragmatics starting from two major theoretical premises: first, that pragmatics is not a secondary meaning derived from semantics—on the contrary we assume a priority of pragmatics over semantics—and second, that morphology is capable of a direct interface with pragmatics, not mediated through its semantics. Thus certain morphological patterns may generate autonomous pragmatic meanings, independently of their denotative power. Eligible patterns are primarily evaluative affixes (diminutives, augmentatives, pejoratives), familiarizers, like French -o, and hypocoristics, whose effects extend from the pertinent base word to the entire speech act. Other morphological elements, such as for example the Japanese honorific -masu and the Germanic and Hungarian excessive, limit their pragmatic scope to the word base. Some other morphological patterns are more marginal, for example feminine motional suffixes or pluralis maiestatis.

Abstract:
The chapter investigates aspects of variation in Old English modals with respect to functional status, modal base, modal force, and event realization under the modals in the actual world. The main goal is to view—in tandem, rather than in isolation—aspects of variation that are of interest at the syntax–semantics interface. The chapter focuses on three modals, the cognates of ‘can,’ ‘may,’ and ‘must,’ which show the full range of variation available in the categories investigated. The role of circumstantial modality in the diachronic development is underlined. It is furthermore argued that the modals of the period occupy a structural position in the aspectual domain. Evidence for functional status includes ellipsis, topicalization, and the complementary distribution with the aspectual prefix ge-.

The complexity of pain and addiction is a challenging clinical problem to address. Approaching the treatment of addiction and pain requires a holistic interpretation of a patient, understanding the psychological as well as biological mechanisms involved in both conditions. Given these facts, an interface has been created in this chapter of 2 phenomenal approaches to pain focused in both the psychiatric and mechanistic pain models. The result is a well-rounded and comprehensive view on how to approach pain in the integrative format for patients with addiction. When to consider different conventional and integrative modalities is reviewed including their evidence base. The role of personality, pain perception, and cognitions are all examined. The full array of integrative approaches including mind-body interventions, guided imagery, CBT, hypnosis, spirituality, mindfulness and postural techniques, manipulation, yoga, Tai chi and TCM are all discussed.

E a regularização fundiária, ou as ações sobre as áreas irregulares, não fogem à regra. Este livro visa levantar as diferentes perspectivas sobre o tema da regularização fundiária, desde as razões de seu surgimento, os diferentes entendimentos da irregularidade da ocupação urbana e seu dimensionamento para avaliar a formulação da base normativa sobre o tema e a formulação e implementação das políticas públicas que se apresentam como respostas ao problema. Objetiva avaliar quanto se avançou, bem como os êxitos e os obstáculos. Os achados são resultados de pesquisas dos últimos 15 anos e se dão sob a perspectiva dos investimentos realizados nas diferentes dimensões da regularização, e não só na titulação, como poderia levar uma visão mais restrita da problemática da irregularidade.
