In the monograph reviewed the development of the inventory management of railway transport in the new economic environment of market economy. According to the results of theoretical research, innovative and production potential of the supply system of railway transport the main directions and methods of transformation of the restructuring process under the corporate changes of JSC "RZD", positioned value system of the logistics of railway transportation, and developed a classification model used logistical resources. Evaluation of activity of structural divisions of Russian Railways supply is proposed to be viewed through an integrated and comprehensive approach to the development of systems of balanced indicators of supply and prompt handling of material resources, the implementation of which allows to distribute the strategic objectives of the company "Russian Railways" activities in the system of logistics of the Railways and also to involve in economic circulation of excessive and unused inventories of material and technical resources and efficiently reallocate them among enterprises at the site of the railway. Recommendations for the implementation of the developed algorithms and models are long term in nature and are based on the concept of logistics management and improve the business processes of the logistics system. Will be useful for managers and specialists of directorates of logistics of Russian Railways supply, undergraduates and graduate students interested in the economy of railway transport.

Rather than a prescribed model of health service delivery, integrated care is a conceptual framework that can be implemented using a variety of styles and models. The concept of integration is based in a biopsychosocial perspective of health and wellness. Effective integration is associated with a set of common elements including team-based care delivery, a patient-centered orientation, care coordination, and a population-based approach. While the most common application of integrated care incorporates behavioral health services into primary care settings, effective health care reform will include a variety of specialty and locally tailored models developed to serve the needs of specific patient populations. This chapter describes the essential components and rationale of integrated care, establishes a framework for evaluation, and encourages continued innovation.

Abstract:
Many patients with psychotic disorders, for systemic and personal reasons, are treated in primary care, even though there currently are no evidence-based integrated care models supporting this practice. This chapter describes the screening and salient clinical features of schizophrenia and psychotic disorders, management of emergencies, the biopsychosocial-cultural evaluation, differential diagnosis (medical and psychiatric), and medications and other treatments that can be delivered by an integrated multidisciplinary team. Psychiatric specialty services are also described, so that primary care referrals to specialty psychiatric services can be offered. Special considerations are outlined for the care and treatment of psychotic women and psychotic geriatric populations. The Denver Health Medical Center’s model of integrated care is introduced, including lessons learned during its development and implementation. An integrated care model for the treatment of psychotic disorders in primary care is proposed that unites best practices of specialty psychiatric care with the fundamentals of integrated care.

Nonepileptic seizures resemble epileptic seizures but lack epileptiform activity on an electroencephalogram and presumably have psychopathologic origins. Psychiatric comorbidities are common, and effective management requires psychiatric treatment. Unfortunately, many patients fear that seeing a psychiatrist implies their episodes are not being taken seriously and that their neurologist might perceive them as producing their symptoms willfully. Patients might feel abandoned if their neurologist refers them to a psychiatrist and indicates that they no longer need to be seen by the neurologist. Consequently, patients often resist undergoing psychiatric evaluation. To help address this problem our team piloted a program integrating psychiatric and neurologic approaches, placing a therapeutic treatment group within the neurology outpatient department. This chapter reviews the clinical features of non-epileptic seizures, including diagnosis and treatment, and presents our team’s integrated treatment approach.

The central premise of this chapter on providing integrated care for both pain and addiction is that all patients presenting with pain are at risk for development of substance use disorders. Assessment and treatment of the complex interplay between pain syndromes and substance use disorders proceed most productively by employing an integrated model, with a multidisciplinary approach and with employment of multiple diagnostic instruments. The author describes an integrated care model as it applies to each of the common substances of use: opioids, tobacco, alcohol, benzodiazepines, cannabinoids, barbiturates, and stimulants. The basis for a decision to refer for evaluation or treatment is described. The chapter concludes with an argument for collaboration between disciplines, notably pain medicine and addiction medicine, as being the current standard of acceptable care for patients whose illnesses dwell in both camps. The IMS-Arbovirus is a model that provides a methodological framework for arboviral disease prevention and control. It divides the compendium of actions to be taken into the following components, which are not listed in their order of importance: management, epidemiology (with emphasis on health surveillance), laboratory, patient care (clinical), integrated vector management (IVM), and environment (with emphasis on water, sanitation, and hygiene). It also proposes common crosscutting themes for each component: operations research and health communication and promotion for behavioral change. Each component and crosscutting theme is overseen and executed by personnel trained for this purpose. The Integrated Management Strategy for Arbovirus Disease Prevention and Control in the Americas contains a group of indicators selected by the countries, and a trained professional regularly conducts an informal evaluation of the strategy. This evaluation may be based on what the coordinator for each component or the participants in the process report, often based only on their own experiences. Generically, this methodology attempts to organize ideas and the methodologies that should be followed for best performance in an evaluation. The IMS-Arbovirus currently includes monitoring and evaluation from the outset, thus systematically coordinating its planning, monitoring, and evaluation. The main objective is for monitoring and evaluation to serve as a good mechanism for management, course correction, and accountability to advance and improve the quality and impact of management with the preparation of the IMS Arbovirus. The specific objectives are as follows: determine the progress made and barriers implementing the IMS-Arbovirus, formulate recommendations to improve the IMS-Arbovirus Implementation process, and create a monitoring plan based on the evaluation's results.

This chapter describes the evolution of collaborative mental health care in Canada over the past 15 years, and the ways in which integrated care is becoming an increasingly integral part of Canada’s provincial and territorial healthcare services. It explores the underlying principles and models that can be found across the country. There is a particular emphasis on three things: (1) changes any mental health service can make to improve collaboration, (2) programs to increase the mental health skills and capacity of primary care, and (3) the integration of mental health services within primary care.A program in Hamilton, Ontario, has successfully integrated mental health counselors and psychiatrists into the offices of 170 family physicians across a city of 500,000 people for the past 20 years. The authors present data from the program’s evaluation, as well as key lessons learned and advice for other programs looking to set up similar models.

Action research and evaluation are human inquiries into human-designed and -populated systems. Both modes of inquiry generate actionable data in specific contexts to facilitate positive social change and to further social justice. This book explores the potential of integrating action research and evaluation frameworks in order to push the barriers that prevent knowledge creation and knowledge democracy. Knowledge is created constantly by those in real-world settings and practical contexts. Practitioners can make and use knowledge to inform practice and decisions and to include stakeholders in ethical ways for sustainability of organizations. The integrated framework presented in this book supports knowledge democracy, ethical practices, and stakeholder inclusion for a more just world. These practices are useful for leaders or practitioners charged with investigating problems in their context.

This chapter reviews contemporary understanding of insomnia disorder. Specifically, it examines insomnia prevalence and associated morbidity, clinical approaches to the evaluation of insomnia, and models of insomnia development and maintenance. Recent evolution of insomnia classification is emphasized, together with novel perspectives on the role of insomnia in conferring risk for mental and physical disease. It is concluded that the development of theoretical models that integrate both psychological and neurobiological levels of analysis will provide a fuller understanding of insomnia pathophysiology and subsequently guide treatment innovation.

This chapter considers the organization and delivery of care for osteoarthritis (OA) within the broader context of chronic condition management, and with a specific focus on the current OA literature. It describes factors that have influenced healthcare reforms for people with chronic conditions such as OA, and the development and characteristics of chronic condition models of care. The chapter also examines existing models of care for OA, which are commonly multidisciplinary in nature and situated within primary care, community care, or secondary/tertiary care settings. It summarizes current evidence for the effectiveness and cost-effectiveness of OA models of care, acknowledging that limited data are available regarding access and efficiency outcomes. Barriers to the successful development, implementation, and sustainability of OA models of care are discussed, as well as enablers that could facilitate the success of models of care. The chapter also presents points to consider when planning the implementation and evaluation of models of care, such as the development of a program logic, use of theoretical models or frameworks, and careful selection of appropriate research designs (including mixed-methods approaches). Finally, it considers future challenges for OA models of care, particularly the rise of multi-morbidity among patient populations that will necessitate integrated chronic condition management rather than stand-alone OA services. Future models of care may require the design of specific OA modules that can be integrated with generic chronic condition models that address risk behaviour modification and optimize self-management and mental health outcomes.

Evaluation of diastolic function by echocardiography is useful to diagnose heart failure with preserved ejection fraction, and regardless of ejection fraction, echocardiography can be used to estimate left ventricular filling pressure. Assessment of diastolic function includes analysis of left ventricular relaxation and compliance, left atrial and left ventricular filling pressures. Proto-State Media Systems explores how militant, non-state organizations create, develop, and sustain media systems in the contemporary environment. It challenges generalized applications of dominant state-based media system models by revealing how such approaches lack full explanatory power for assessing the structures and functions of media systems of groups like al-Qaeda and ISIS. Utilizing theories of constitutive discourse and online networks, as well as revised conceptualizations of proto-states and media systems, the book explores how militant proto-states can create identity-based media systems capable of attracting and sustaining online and offline loyalties. Proto-State Media Systems begins by tracing the structural evolution of the media operations of al-Qaeda over a thirty-year period and of ISIS since it emerged around the turn of the century. It then mines recurrent transhistorical and transpatial referent points in al-Qaeda and ISIS’s Arabic and English media products to identify insightful, embedded criteria for evaluating proto-state media systems. It then identifies events—both material and symbolic—associated with heightened interest levels by online users and traditional media outlets, and explores how patterned uses of multimodal appeals function to sustain viewer interest over time. It concludes by providing an integrated model for comparing proto-state media systems.

This chapter aims to guide practitioners and managers in setting up and reviewing community outreach services for people with severe mental illness from a non-technical service planning perspective. Examples of different service configurations within a comprehensive local system are given, with some observations on their relative merits and drawbacks from evaluations. Service models and structures are important for providing a framework for delivering quality care, yet from the perspective of the service user, many of these service details—integrated care, specialization, caseload size, staffing mix, ownership of beds, and degree of shared caseload—are invisible. For people with severe mental health problems, patients and carers value the principles of good community-based care, such as access, responsiveness, consistency, and continuity.
