Full textIn Italy, the process of de-institutionalisation and implementation of a community-based model in mental health care has been characterized by a lack of evaluation. In particular, no activity is or has been in place to develop or maintain standards of quality of care. Aim of this dissertation is to compare the care pathways in mental health services in Emilia Romagna Region using process and outcome indicator obtained from administrative databases. Process and outcome indicators of mental health care for Local Health Authorities of Emilia-Romagna Region (3,724,388 adult inhabitants in 2010) were obtained through linkage of hospital discharge records, community mental health service database and drug prescription database. The study cohorts include patients with a primary ICD-9-CM diagnosis code 290.xx-319 hospitalized or treated in community mental health services in 2010. The set of indicators explores treated prevalence and incidence rates by disorders and settings, continuity between hospital and community care, retention in treatment, re-hospitalizations, antidepressant drug and mood stabilizer prescriptions. Hospitalization rates varied from 159,0 to 290,3 per 100.000 inhabitants. Treated prevalence in the study area was 15‰ and incidence was 6‰. Of the patients treated by community mental health services, about 37% had a severe mental illness. The proportion of patients re-hospitalized within 30 days from discharge was 12%. Less than 6% of patients with severe mental illness discontinued treatment with community mental health services. Calculation of the selected indicators proved to be feasible, conditional on quality checks for completeness and accuracy of the primary diagnosis. Implementation of these indicators on a larger scale and on a regular basis may provide a unique opportunity to set up a mental health surveillance system and monitor treated prevalence and incidence of mental disorders as well as continuity of care and treatment outcomes at the patient level.

Full textThe aim of the project was to assess health status and quality of agricultural soils of the Lombardia region, and some areas with critical environmental situations, through a multidisciplinary monitoring. The use of biological indicators coupled to classic analytical techniques, has allowed the implementation of the information obtained from the chemical-physical point of view. Contrary to the past, the living component of the soil has become crucial to better understand its condition, as it has been recognized as a system closely related to other environmental media, influenced and characterized by all organisms that compose it. Basing on this it was investigated the structure and characteristics of microbial communities, earthworms and ciliated protozoa, the enzymatic activities of the soil, and the genotoxicity of pollutants using clover plants as indicator. We also had the opportunity to apply some of the techniques that are outside the classic monitoring, but that can provide valuable information about the behavior of a substance or an element. These techniques are: the application of isotope dilution to determine the presence of hexavalent chromium in potentially contaminated soils, and bioassays with bioluminescent bacteria to assess the bioavailability of arsenic and mercury in a Site of National Interest.

Full textBackground: patient reported experience measures (PREMs) are quality indicators of care which measure the patient-reported care process. No PREMs questionnaires have been developed and validated to investigate the experience of care in neonatal units. Objective: to develop a PREMs questionnaire through a process of selection via the Delphi method of a list of questions that investigate the experience of neonatal care from admission to discharge, and to validate this instrument. Materials and methods: a rapid review of the literature was conducted to identify questionnaires on the perception of care reported in validation studies published between 2008-2018. The items referring to experience, selected from each included questionnaire, were submitted to the stakeholders in 3 Delphi rounds for the judgment of relevance. Previously defined consensus criteria were used to reach an agreement at the end of round 2 and the retained items were given a priority for inclusion in the questionnaire. This was administered after discharge to parents of newborns admitted to the neonatal unit. An exploratory factor analysis was used to examine the structure of the instrument. Results: 380 studies were identified, 4 were included and 69 items submitted for the stakeholders’ decision (54 parents and 52 professionals). At the end of the Delphi process, the final 18-item questionnaire was validated using 97 parents. The factor analysis with dichotomous scoring of the items identified 2 factors with that fit the model well: the experience of in-hospital assistance and the experience during admission and discharge. Conclusion: the questionnaire is the first validated instrument to measure the experience of care in neonatal units through two specific indicators, which allow for a comparison of the experience of care reported by the parents between homogeneous structures or within the same structure over time, and to identify critical areas for improvement in the quality of care.

Full textPatient reported outcome measures (PROMS) have now become among the most used tools at all levels of care, particularly for oncology. Prostate cancer is the most common malignant cancer diagnosis and the second leading cause of death in men. Patients may develop treatment-related side effects of which urinary incontinence is one of the most frequent and most influencing quality of life. In the first experimental part of this thesis the most important quality indicators for prostate cancer (proposed by the scientific community) were identified and their use was determined. In a second stage, through data collection of the functional outcomes extracted from the electronic medical records (using data from the Stanford University HealthCare database), an extraction and data-mining flow that has been able to capture the documentation provided by the physicians on the most important patient-centered outcomes using unstructured and routine recorded clinical notes has been developed. Furthermore, patient reported outcomes were collected using the EPIC-26 survey (administered during the oncological visit before and after surgery), a questionnaire that over the years has become one of the gold-standard for evaluation of the patients undergoing prostatectomy. The main result of the actual comparison is that what is reported by patients is highly consistent with what is reported by doctors on electronic medical records. The presented case study paves the way for the possibility of using and evaluating this type of indicators in a systematic way. Clinicians can report in the medical records important patient reported information, whose integration will lead, in the future, to a better collection of data on patient-centered outcomes and to an effective quality improvement in all medical fields.
