Full textMost soils worlwide undergo modifications caused by human activities. For numerous agricultural soils, the intensive practices, as well as the chronical spreading of chemical inputs, lead to a decrease in these soils fertilkity. Yet, while these soils are in danger, only a few relevant soil status evaluation tools exist. The biological component has been neglected for a long time, though soil accomodates a high diversity level of organisms involved in biogeochemical cycles. Among these organisms, microorganisms are likely to be potential bioindicators of soil status. Thus, the aim of this work consists in identifying biological variables describing bacterial diversity that would be sensitive to soil status. Bacterial community structure (abundance, genetic and functionnal diversity) was assessed as a whole, but also through the analysis of Pseudomonas populations, a bacterial genus widely found in soils and sensitive to environmental disturbances. The experimental approach, from the field plot level to the microcosm level, consisted in the confrontation of the impacts of (1) soil copper contamination (agronomical dose x100), (2) natural disturbances (spatial and climatic), and (3) human caused disturbances (agricultural practices) on bacterial communities. The first part of this work was performed in situ on typical silty Haute-Normandie soils managed as grasslands and cultures, in order to define the natural spatial and temporal variation limits of different bacterial descriptors. Besides, this first part enabled the understanding each variable's sensitivity, and validated the different methodological approaches. Thus, the measured variables can be organized in two categories: those which rely on microorganisms cultivation would reflect recent soil disturbances, while those based on molecular approaches would reflect the agricultural history of field plots. In the second part, the impact of a copper contamination on the previously described variables was studied in microcosms. Results showed that variations in the bacterial community structure caused by a single copper contamination were minor, when compared with variations caused by season or agricultural practices. In the same time, a comparative analysis was performed between sieved soil and intact soil microcosms. This revealed that soil status can influence bacterial communities response to a copper contamination; indeed, soil physical disturbance (sieving) induced a transient response of bacterial communities to the copper contamination. This work lead to (1) building, for an agricultural silty soil, a microbiological reference system, which includes spatial and temporal fluctuations of the measured variables, (2) determining for each variable its sensitivity to natural and human impacts, and identifying the limits and complementarities of the cellular and molecular approaches, (3) organizing into a herarchy factors determining the genetic and functionnal sructure of bacterial communities, (4) validating the relevance of using the Pseudomonas genus in the evaluation of the impact of copper on soils

Full textThis work presents an innovative epidemiological surveillance system in environmental health based on self-reported health indicators, closer to the complaints of the local population leaving around a source of environmental pollution: symptoms and loss of the quality of life. This approach is built up from a conceptual framework based on the psychometric theory and the transactional model of stress. A field application of this approach was carried out the population neighboring (n=1 495) the chemical industrial area of Salindres, Gard. This study verifies that self-reported health indicators measured with MOS SF-36 and SCL-90-R can be used to study populations exposed to multifactorial environmental risks. This study confirms that these indicators are relevant and sensitive when applied to environmental risk factors referring to the cognitive representations of risks, such as the sensorial perception of stimuli emitted by the industries. This work shows, besides, that the availability of national references for these indicators confers interpretability to these indicators, an essential quality criterion. This work discusses qualitatively the acceptability of measuring indicators of self-reported health in the population, proposes recommendations on communication for the setting of that kind of approach, and opens up avenues on management orientations

Full textIntroduction: Improvement of the appropriateness of care is a national priority. In hospitals, it can rely on the measurement of indicators. The objective was to: (i) define the methodological framework and challenges related to the development, validation and feedback of indicators; (ii) implement an approach for the development, validation and feedback of automated indicators of the appropriateness of oral anticoagulant (OA) prescriptions. Methods: Definition of the methodological framework and challenges has been performed through syntheses of literature data and exploratory works. The approach applied to indicators on OA prescriptions was based on: (i) a literature review to define indicators from European guidelines; (ii) a consensus of experts to select useful and operational indicators; (iii) the prospective development of data search techniques for retrieving information aimed at implementing indicators; (iv) a cross-sectional study to assess the ability of indicators to detect inappropriate OA prescriptions. Results: The methodological framework and challenges have been defined. The approach applied to indicators on OA prescriptions allowed: (i) defining 19 indicators; (ii) selecting 16 useful and operational indicators; (iii) developing techniques for implementing indicators from a data warehouse; (iv) developing methods for assessing the ability of tools to detect inappropriate OA prescriptions in comparison to a reference test. Conclusion: This work provides tools to develop, validate and convey indicators. The most performant indicators to detect inappropriate OA prescriptions may be used by European healthcare institutions

Full textFrance is one of the European countries with the highest antibiotic consumption, with primary care accounting for 75% of prescriptions. This PhD subject was to study indicators of good antibiotics’ use (GAU) in relation to the 3 main actors or factors determining their prescribtion: the general practitioner, the patient and the healthcare system.Four studies were carried out. The first validated antibiotic prescribing relevance indicators on a sample of prescribers. The second focused on the indicator relating to antibiotherapy duration, assessing its impact in terms of antibiotic consumption and additional costs for the healthcare system. The third studied the factors associated with late delivery, laying the foundations for a GUA patient indicator. Finally, the last study focused on the healthcare system and its influence on antibiotic prescribing.This Ph work enabled us to make progress on prescriber indicators and to consider preventive actions to be implemented in the field. It has also broadened the potential scope of GUA indicators towards the user and the healthcare system

Full textThe rubber (Hevea brasiliensis) plantations extend to non-traditional area in Thailand where dryer conditions has been reported to impair the growth of rubber trees and latex production. Physiological parameters helpful for breeding adapted genotypes are required, such as water use efficiency (WUE). Carbon isotope discrimination is widely used as a proxy for WUE that can easily be used for selection and breeding programs for drought tolerance. Leaf δ13C and leaf gas exchange were measured on young saplings of 10 rubber clones growing in pot in a common garden. The range of leaf δ13C among 10 clones was narrow and the correlation between δ13C and WUEi was significant under high vapour pressure deficit only, which means the prediction of WUE by δ13C would have low precision. There were large δ13C variations among the genotypes at all seasons in a collection of 49 wild genotypes of rubber in Northeastern Thailand. δ13C was rather stable with a good correlation between rainy and dry season. The genetic variability of δ13C is promising for breeding if a good correlation between δ13C of leaf and WUE can be established. The lack of correlation between δ13C of latex (δ13C-L) and of leaf soluble compounds (δ13C-S) collected from tapped and untapped 20 year-old rubber trees suggests that recent photosynthates are mixed in the large pool of stored carbohydrates that are involved in latex regeneration after tapping. Thus δ13C of latex is not a relevant indicator of WUE of rubber trees

Full textThis thesis proposes to review the available indicators for the epidemiological surveillance of work-related low back pain, including compensation data of occupational diseases and data from the pilot surveillance system for musculoskeletal disorders in the Pays de la Loire region.The proportion of cases of lumbar disc surgery (used as sentinel event of low back pain) attributable to occupational activity was particularly high among blue-collar workers of both genders and lower-grade female white-collar workers. Low back pain is a complex phenomenon and cannot be monitored using a single indicator. Compensation data, despite their limitations, give results comparable to other data sources and don’t require specific gathering. Cohorts’ data, based on a standardized questionnaire, are necessary for international comparisons. Finally, surgical data from private and public hospitals databases provide the finest information, if they are completed with occupational data. However, this is a mixed indicator of morbidity and care. It shows large local disparities in a region, encouraging the development of differentiated prevention programs.In addition to these three indicators, it would be interesting to add a disability or incapacity indicator in the surveillance system to better target prevention programs according to occupational exclusion which is an important dimension in terms of social impact for people and cost for the society
