Abstract:
Magnetic resonance imaging (MRI) overcomes many of the limitations associated with conventional radiography, the technique historically regarded as the gold standard in imaging of osteoarthritis (OA). MRI allows visualization of changes and pathologies in joint tissues including cartilage and the menisci, the two tissue components responsible for the indirect radiographic marker of joint space narrowing, decreasing the length of time that must elapse before disease progression can be detected. Other elements of the joint can also be analysed simultaneously: a key development in the understanding of OA. This chapter focuses on the utility of MRI in observational studies and clinical trials, detailing the available MRI techniques and quantitative/qualitative measurements, and their correlation with tissue damage. The possible future directions of MRI in OA are also discussed, with a view to its potential utility in identifying disease-modifying interventions.

Abstract:
This chapter on “Imaging” examines the relative advantages and disadvantages of computed tomography (CT) and magnetic resonance imaging (MRI) scans. It compares the modalities to each other and to gross neuropathology. For several decades, neurologists have been able to view cross-sectional images of living patients. Analogous to gross neuropathology, cross-sectional imaging displays the brain as an entire organ but does not demonstrate microscopic tissue or cellular pathology. By allowing practitioners to view sections of brain and spinal cord in vivo, imaging has improved neurologic practice and facilitated clinical research. This chapter deals with imaging topics that are important to the neurologist. The timing of scans, the effects of gravity, and the importance of plane of section are considered. Imaging is compared to gross neuropathology, and MRI is compared to CT.

Abstract:
This chapter focuses on diagnostic and procedural imaging techniques that are essential for the pain medicine practitioner. Attention is given to most modern imaging modalities, including ultrasonography, fluoroscopy, computed tomography, and magnetic resonance imaging. The chapter includes a review of many advanced pain medicine procedures, such as celiac plexus and stellate ganglion blocks. A discussion regarding the use of imaging to elucidate a problem with an implanted intrathecal drug delivery system is included as well. In addition to the procedure suite, this chapter provides a review of common radiological findings that are critical for the proper diagnosis and management of pain patients, including spondylolysis and Modic changes. Special attention is paid to the use of ultrasound in pain medicine, including diagnostic techniques in musculoskeletal disorders. Many questions contain a review of the significant anatomic considerations with each procedural technique.
