Abstract:
Two materials of α-type manganese dioxide were synthesized and examined. They were prepared by the pyrolysis of mixtures of MnCO3 and (CH3)3COK. An ill-ordered material was obtained when prepared at large (CH3)3COK content. Both samples behave as acids, but their apparent capacities are obviously different: about 0.9 meq/g for a well-ordered sample and about 2.6 meq/g for an ill-ordered sample. Ion-exchange properties were examined on Kielland's plot. Zero intersections of the two samples are almost the same but slopes are different (about −50 for a well-ordered sample and about −10 for an ill-ordered sample). The difference in slope is likely caused by the flexibility. An evidence of the flexibility can be seen by x-ray diffractometry.

Background: Hypophosphatemia can complicate and prolong the treatment of critically ill patients, and it is even thought to be related to mortality rate. Objectives: The aim of this study is to determine whether using extemporary prepared phosphate buffer in pharmacy would help correct serum phosphate in critically ill patients. Methods: A prospective study was conducted at the medical intensive care unit over a period of 1 year and included 50 patients who were diagnosed with hypophosphatemia. Phosphate buffer was prepared at the pharmacy, and the dose range was recommended by a clinical pharmacist. Results: Patients were administered phosphate buffer via the nasogastric tube, and the doses chosen by the physicians depended on serum phosphate level and the severity of the patients’ clinical status. Serum phosphate levels were successfully corrected in all treated patients. The most frequently used dose was 60 mmoL/d, and in most patients 1-day therapy was sufficient. No adverse effects were observed. Conclusion: The phosphate buffer is an adequate alternative for the treatment of hypophosphatemia of nonsurgically critically ill patients. One-day therapy with the 60 mmoL phosphate dose divided into 3 single doses resulted in normalization of serum phosphate values in most patients.

TiO2nanocrystals were prepared by a photo-assisted sol-gel process in which tetrabutoxide titanate was hydrolyzed in acidic medium under ultraviolet irradiation. X-ray diffraction and Raman spectra showed that the as-prepared TiO2particles without further annealing were well-crystallized anatase. Such TiO2particles were easily immobilized on dacron cloth and showed very high photocatalytic activity. In contrast, TiO2particles were ill crystallized and showed lower activity when no light was introduced under otherwise equal conditions.

We have evaluated the effectiveness of nutritional support to critically ill patients undergoing treatment in ICU. We provided commercially prepared branded food in one group and compared this, with those of homemade preparations. Fifty adult critically ill patients getting treatment in ICU aged between 20 to 60 years having no diabetes, hypertension, ischaemic heart disease and chronic renal failure, were scheduled for providing enterai nutritional support. They were randomly allocated into two groups. Group A (n=25) received commercially prepared branded food containing balanced food consisting of protein, fat, and carbohydrate in calculated proportion. The aim was to provide 2500 Kcal of energy per day for an average adult in his/her critical state. Group B (n=25) received homemade food preparations in proper proportion of those of group A and of equal calories. The purpose of the study was to assess, formulate and compare the efficacy, tolerability and cost effectiveness of homemade prepared food with commercially formulated branded food in maintaining nutritional status of critically ill patient in ICU. Patient's nutritional assessment was done before the commencement of nutritional support, Nutritional assessment was repeated weekly for successive two weeks. Group A showed better result than Group B, significant improvement has been observed also in group B and the differences between two groups are statistically not significant Both the methods of nutritional support were adequate and effective in maintaining the nutritional status. Considering the poor socioeconomic environment in an under developed country like Bangladesh, a less costly homemade preparation can be an effective alternative over the branded food. Based on a variety of primary sources including recently declassified documents, interviews in Cameroon, letters, and Peace Corps Volunteers’ personal correspondence, this study examines the service of the US Peace Corps in “Agroforestry” and “Small Enterprise Development” in Cameroon. The study argues that Volunteers were ill trained, ill prepared, and ill equipped for service in Cameroon, and as a result did not achieve Goal 1 of the Peace Corps Act, which calls on the agency to assist developing nations in gaining “trained manpower”. The study has broader implications, as it raises questions about the relevance of Peace Corps-like organizations in Cameroon, and in African nations as a whole. It focuses on Cameroon for a variety of reasons, among which is that Cameroon is one of only three nations in Africa in which Volunteers have served uninterruptedly since 1962.

ABSTRACT Objective: To evaluate the self-confidence of nurses in the care of critically ill patients, before and after a simulated intervention. Method: A quasi-experimental study was carried out with 103 nurses who participated in a workshop on the care of critically ill patients in the first semester of 2016. A clinical simulation pedagogical instrument was used throughout the event, and self-confidence was assessed by the Portuguese version of the Self-confidence Scale (SCSvp). Results: Most participants reported not being prepared to provide first care to critically ill patients, which was carried out during care practice. The participants presented a significant increase in self-confidence after the simulated intervention (p<0.001) in the cardiac, neurological, and respiratory dimensions. Conclusion: Simulation has proved to be an effective strategy for the development of individuals’ self-confidence, which contributes to the improvement of skills required for professional practice.

This study explores how medical students feel about caring for terminally ill patients as well as how their medical courses prepare them for addressing end-of-life (EOL) issues with patients. Four hundred and five Mexican medical students were surveyed through the Student Views on Death questionnaire. The vast majority of students (94%) felt that physicians should inform patients of their impending death. Most students said they felt comfortable talking with (61%) or examining (76%) terminally ill patients. However, only half the students actually talked with patients about death. Participants in our study were interested in learning about EOL medical attention, yet most considered themselves poorly prepared to offer this type of care to terminally ill patients. The study provides objective data on a topic that has scarcely been explored in Mexico, data that will be useful in designing educational activities to improve EOL medical training.

Abstract Purpose To describe our hospital pharmacy department’s preparation for an influx of critically ill patients during the coronavirus disease 2019 (COVID-19) pandemic and offer guidance on clinical pharmacy services preparedness for similar crisis situations. Summary Personnel within the department of pharmacy at a medical center at the US epicenter of the COVID-19 pandemic proactively prepared a staffing and pharmacotherapeutic action plan in anticipation of an expected surge in admissions of critically ill patients with COVID-19 and expansion of acute care and intensive care unit (ICU) capacity. Guidance documents focusing on supportive care and pharmacotherapeutic treatment options were developed. Repurposing of non–ICU-trained clinical pharmacotherapy specialists to work collaboratively with clinician teams in ICUs was quickly implemented; staff were prepared for these duties through use of shared tools to facilitate education and practice standardization. Conclusion As challenges were encountered at the initial peak of the pandemic, interdisciplinary collaboration and teamwork was crucial to ensure that all patients were proactively assessed and that their respective pharmacotherapeutic regimens were optimized.

Epidemiological investigations demonstrated that the vehicle of infection was food prepared by a foodhandler who was recovering from a mild gastrointestinal illness. The food most strongly associated with illness, coronation chicken, was prepared by the food handler on the second day after symptoms ceased.The investigation confirms the view that foodhandlers may contaminate food with SRSV after cessation of symptoms and should remain off work until at least 48 h after recovery.

Disinfectants containing 3-methyl-4-chlorophenol were tested for their capacity to inactivate the infectious agent of scrapie. Coincubation of brain homogenates prepared from terminally ill scrapie-infected hamsters with the disinfectants rendered the prion protein PrPSc sensitive to proteinase K digestion. SUMMARYA common source outbreak of small round structure virus (SRSV) gastroenteritis affected 81 patients and 114 staff in four hospitals served by one central hospital kitchen. Eating salad items was found to be significantly associated with illness. In a cohort study of a staff buffet function eating turkey salad sandwiches was associated with illness (relative risk = 2·4; 95% CI = 1·4–4·1; P = 0·003), and a case control study of patients in one hospital showed an odds ratio of 6·6 (95% CI = 1·0–71·6; P = 0·04) for eating tuna salad and becoming ill. One of two food handlers who prepared the salads became ill the day following food preparation; she also had a young child at home who had been ill with a gastrointestinal illness during the previous two days. Contamination of food by mechanical transmission of the virus from the child via clothes and hands of the mother, or pre-symptomatic faecal excretion in the mother are possible explanations of contamination of food.

Data from the 1996–1997 FoodNet case-control study of laboratory-confirmedSalmonellaserogroups B and D infection were used to estimate the effect of specific behaviours and foods on infection withSalmonellaserotype Enteritidis (SE). Persons with laboratory-confirmedSalmonellaof other serotypes acted as the comparison group. The analysis included 173 SE cases and 268 non-SE controls. SE was associated with international travel, consumption of chicken prepared outside the home, and consumption of undercooked eggs prepared outside the home in the 5 days prior to diarrhoea onset. SE phage type 4 was associated with international travel and consumption of undercooked eggs prepared outside the home. The use of ill controls can be a useful tool in identifying risk factors for sporadic cases ofSalmonella.

To a certain extent, many curriculum reforms that have been introduced in Lesotho schools have been unsuccessful. Teachers’ preparedness to implement these reforms has been the major factor for the failures. Involving teacher education providers in the preparation of teacher preparedness could improve the success rate of these school reforms. Therefore, understanding how education educators prepare teachers for curriculum reforms could assist to understand why school teachers often appear to be ill-prepared for curriculum reforms. Accordingly, this paper investigated the views of six teacher educators about their role in the preparation of teachers for curriculum reforms in Lesotho schools. A questionnaire was used to collect data that were analysed thematically. The results obtained indicate that: teacher educators are involved in a limited way in Lesotho curriculum reforms; and they have limited knowledge about the current reforms. As a result, their training practices do not target to prepare teachers for the reforms introduced in schools. This explains why many studies have found teachers to be ill-prepared for any curriculum reforms in Lesotho. These findings raise awareness on the issue that teacher educators should be involved in the reforms in Lesotho. Therefore, the main recommendation of this study is to engage teacher education providers so as to make the reforms successful.

By combining Strichartz estimates with the special structure of the vorticity, we prove that the lifespan of the solutions goes to infinity as the Mach number goes to zero. We also prove strong convergence results of the incompressible parts to the solution of the incompressible Euler system.

The Solution. Recognizing a need for a completely new type of leader, Ohio State created the President and Provost’s Leadership Institute (PPLI) to develop a pool of women and underrepresented minority faculty who might move into leadership positions. The Stakeholders. The PPLI can provide a useful template for HRD practitioners who wish to tailor leadership development and succession planning strategies to address higher education’s unique culture and leadership pool challenges. For institutions interested in broadening the diversity of individuals prepared to lead, and developing leaders prepared to lead change, the PPLI is an example of one successful approach.

Severity of psychiatric illness was assessed using standardised clinical and social measures in 69 in-patients and 41 day patients admitted consecutively from the community. Day and in-patients differed little in terms of psychiatric symptoms and social disability, especially if compulsory admissions were excluded. Protection of self or others was a common reason for in-patient admission given by clinicians, who were otherwise prepared to treat seriously ill patients in the day hospital. Very few of the day patients had to be transferred to the in-patient facility, and at three months and one year the two groups showed similar improvements. It is concluded that day treatment is feasible for some seriously ill psychiatric patients, but a random-allocation study is required to assess more completely the efficacy of day treatment, and define the characteristics of those who require in-patient admission.

Objective: The aim of the study was to explore the lived experience of caregivers providing home care for terminally ill family members, with the objectives of describing their experiences of caring for relatives who are terminally ill with cancer and the needs of home caregiving in Bahrain.Methods: The study adopted a Heideggerian, hermeneutic phenomenological design, with a purposive sample of eight family caregivers. Data was generated through one to one, in-depth interviews and analyzed using Interpretative Phenomenological Analysis.Results: Three main themes emerged from the data: (1) the burden of care, (2) comforts, and (3) coping. The findings showed that the lived experience of home caregiving includes physical, emotional and financial burdens, combined with a lack of professional support. The collective experience was infused with intense emotions because of a lack of structured support, resulting in negative emotions that frequently affected the caregivers’ well-being and their ability to care for the ill person. Nevertheless, caregivers tried to maintain care and comforts for their terminally ill relative in the home. They adopted the home environment, arranged resources to ease caring and provided psychological care. Caregivers utilized three coping mechanisms, faith, personal strategies and distribution of the care responsibilities among family members.Conclusions: Caregivers were not prepared for the commitment and burdens of home care when a family member is terminally ill. A recommendation from the study findings is that training be offered on nursing care before patients discharge. In addition, a reactivation of the palliative care clinic hotline service would support family caregivers. A further recommendation is that home nursing care and hospice services be established to improve homecare services for family caregivers in Bahrain.
