Abstract:
This paper reports rapid and eco-friendly biosynthesis of silver nanoparticles using latex of two different plants Jatropha curcas (Jc) and Lannea grandis (Lg). Latex of these plants act as both reducing as well as capping agent. The surface plasmon resonance (SPR) band of as-synthesized silver nanocolloids (AgNC) appeared at 440 nm and 415 nm for the AgNC synthesized by latex of Jc and Lg, respectively. TEM images show that the latex of Lg produced smaller regular-shaped particles than those produced by latex of Jc. The influence of different synthesis parameters like concentration of latex, concentration of metal ion, time of reaction etc., on the morphology of the particles were also studied by monitoring UV-Vis spectra of the samples.

Abstract:
This study focused on the physical properties of latex extracted from five species of Artocarpus J.R.Forster & G.Forster, namely: A. altilis (Parkinson) Fosberg, A. blancoi Merr, A. camansi Blanco, A. heterophyllus Lam., and A. ovatus Blanco as potential natural adhesives. Surface morphology showed that all five Artocarpus spp. have no specific forms, but otherwise flexible and viscoelastic. Contact angle measurements showed that all samples of Artocarpus spp. were hydrophilic with low contact angle values owing to the contents of natural source of the latex. FTIR analysis matched all Artocarpus latex samples to that of Polyvinyl acetate. Highest resin content was found on A. ovatus with all the species containing natural resin. It was also confirmed that out of the three stress strain analyses, A. camansi had the highest values for tensile strength, A. ovatus had the highest values for Young’s modulus of elasticity and the highest percentage elongation values belonged to A. heterophyllus. Adhesive shear strengths with maximum force values were highest in A. ovatus. Through cluster analysis, out of the eight variables tested A. heterophyllus was the outgroup being attributed to its latex gum-like property. All the above tests and analyses suggested that latex of all five Artocarpus spp. were similar in characteristics to polymer adhesive. Among which A. camansi and A. ovatus exhibited high results on adhesive strength tests.

Abstract:
Natural Rubber (NR) latex obtained from Hevea brasiliensis contains a wide particle size distribution. The aim of this study is to investigate the effect of small rubber particles (SRP) and large rubber particles (LRP) on the characteristics of film formation. The rubber particle with different mean diameters can be separated by centrifugation at various speeds to prepare SRP and LRP latex. The average size of SRP and LRP were characterized by light scattering technique to show that the size of SRP was in the range of 0.20 μm, while that of LRP was larger with the wide distribution. SRP and LRP latex were dried at room temperature to study the film formation behaviors. The results showed that the film compaction time increased with increasing the particle size of NR. Furthermore, the rubber film were aged at room temperature for 3 weeks in order to observe the surface morphology using atomic forced microscopy (AFM) by tapping mode. The AFM images showed that SRP readily formed a coalescence film, while LRP showed individual particles on the surface of film at 24 h of storage time. The surface of both SRP and LRP films was smoother after storage. However, LRP film still showed individual particles on the surface after 3 weeks of storage time.

Abstract:
Calotropis procera is a wild-growing plant that belongs to the family Asclepiadaceae, commonly found in the tropical regions and is used in the treatment of diseases such as ulcer, inflammation, wound healing, etc. This confers to the increased use of medicinal plants as the major source of traditional medicine worldwide. This study is aimed at evaluating the toxicological effects of Calotropis procera latex on biochemical and hematological parameters in albino rats with a view to understanding the safety of the latex following systemic use. Fifteen (15) albino rats were divided into five groups of three rats each. Group one served as the control, group two received 0.05 ml of the latex solution per kg body weight of the rats , group three received 0.2 ml of the latex solution per kg body weight of the rats , group four received 0.4 ml of the latex solution per kg body weight of the rats , while group five received 0.6 ml of the latex solution per kg body weight of the rats for 14 days. Biochemical evaluations revealed a significant increase in the serum levels of aspartate aminotransferase (83.33 ± 1.76c) and total bilirubin (6.27 ± 4.44b) and a progressive decrease in albumin (3.93 ± 0.04a) with increased concentration of the latex. There was no significant change in the levels of alanine aminotransferase and alkaline phosphatase in all the treated groups. Hematological analyses revealed a significant (p<0.05) decrease in packed cell volume (31.33 ± 1.76a), hemoglobin (10.43 ± 0.59a), red blood cell count (3.90 ± 0.06a) and total white blood cell count (4.10 ± 0.15a) in all the treated groups compared to the control. In conclusion, the results of this study suggest that Calotropis procera latex is relatively toxic as observed in the blood and biochemical parameters related to liver function. Hence, it is recommended that caution should be taken in its use for the treatments of diseases such as wound healing as exposure to the circulatory system can cause vulnerability to its toxic effects. Abstract:
Several tests for the biological evaluation of bioceramic materials and medical devices are provided in specific international standards, where in vitro tests have a major role. Tests involving exposure of cells in culture require the use of validated positive controls, which, in the same preparation and treatment conditions, present a substantial and well-known cytotoxicity. The present work aimed to test and validate 3 different sources of low cost, commercially available latex, as positive controls in cytotoxicity tests for bioceramic materials performed by indirect exposure. The tested origins for latex samples were: surgical gloves without powder, 100% pure amber latex hospital-grade tourniquets and 60 % latex White tubing. MC3T3-E1 murine pre-osteoblasts in culture were exposed to conditioned media (extracts) of each material tested, along with sintered stoichiometric hydroxyapatite bioceramics, and polystyrene beads as negative control. Cell viability was determined by XTT and Crystal Violet Exclusion tests. Concentration curves of the extracts were performed to obtain the DC50. Only the 100% pure amber latex tubing was proven to be cytotoxic, with cell survival less than 5%. This material did not affected neighboring groups at the same experimental system. Moreover, latex samples showed great repeatability in different tests against latex and biomaterials, with consistent toxicity under 20% cell survival as shown in 3 different cell viability parameters. We conclude that fragments of latex ambar tubing are suited as effective positive controls in tests of medical bioceramic materials.

Abstract:
To interact with computers, we often rely on computer languages such as Python for creating programs and LaTeX for writing technical documents. In the past few decades, these languages have become increasingly used in a variety of fields ranging from science to arts. They are now being taught to millions of pupils and have become a staple skill in the job market, widening the gap between those who are computer literates and the others. However, for the most part, our interaction with these languages has remained similar to what it used to be fifty years ago: reading and writing code as plain text. Although various alternatives have been introduced, they have often been developed with a modernist approach, in isolation from popular languages and widespread workflows, resulting in systems that hardly cross the borders of academic circles and niche communities. As a result, our interaction with computer languages is now hampered by essentialist views that encourage us to move away from them, at the risk of losing some of the agency they give us, as if dealing with code was nothing but a burden from the past compared to ever more “simple” user interfaces and “intelligent” code synthesis. In this thesis, I argue against this view that I consider harmful to human-computer interaction research and computer-driven democracies. To do so, I introduce a number of arguments that show that it is indeed possible—and even beneficial—to make our interaction with computer languages more pluralist, including with the many languages that already exist that are inherent to the diversity of computing in the 21st century. I first develop a new theory of interaction with computer languages that shows that no such language is inherently bound to a specific representation or type of interaction. To that end, I deconstruct the notion of computer language into five fundamental aspects to isolate interaction from the other constituents of these languages, yielding a more holistic model than those that already exist. I then use this model to identify different levels of interaction with computer languages, which can be hybridised, and show that a single piece of code can very well be projected onto several representations to let end-users decide which representation supports the form of interaction most appropriate for them. I then apply this view of computer languages to two research problems using user-centred design methodologies: helping users author documents written in LaTeX and helping programmers appropriate their text editors by crafting their own projections. For each problem, I assess the limitations of existing solutions with the help of a formative study; I develop a prototype of a text editor equipped with additional projections; and I evaluate it with both qualitative and quantitative user studies. The results show that that complementing text with other representations helps us understand and modify code faster and with a lower workload and that these representations can be created by recomposing existing parts that can be reused from one projection to another. In conclusion, I show that considering interaction with computer languages as projections makes it more protean, an approach which, according to this thesis, is theoretically grounded, technically possible and empirically desirable. It lends itself to the urgent task of equipping an ever-growing public of citizens with new intellectual and technical tools to help them understand and appropriate the computer languages that rule the societies we live in

The development of activities in the oil and gas sector has been promoting the search for materials more adequate to oilwell cementing operation. In the state of Rio Grande do Norte, the cement sheath integrity tend to fail during steam injection operation which is necessary to increase oil recovery in reservoir with heavy oil. Geopolymer is a material that can be used as alternative cement. It has been used in manufacturing of fireproof compounds, construction of structures and for controlling of toxic or radioactive waste. Latex is widely used in Portland cement slurries and its characteristic is the increase of compressive strength of cement slurries. Sodium Tetraborate is used in dental cement as a retarder. The addition of this additive aim to improve the geopolymeric slurries properties for oilwell cementing operation. The slurries studied are constituted of metakaolinite, potassium silicate, potassium hydroxide, non-ionic latex and sodium tetraborate. The properties evaluated were: viscosity, compressive strength, thickening time, density, fluid loss control, at ambient temperature (27 ?C) and at cement specification temperature. The tests were carried out in accordance to the practical recommendations of the norm API RP 10B. The slurries with sodium tetraborate did not change either their rheological properties or their mechanical properties or their density in relation the slurry with no additive. The increase of the concentration of sodium tetraborate increased the water loss at both temperatures studied. The best result obtained with the addition of sodium tetraborate was thickening time, which was tripled. The addition of latex in the slurries studied diminished their rheological properties and their density, however, at ambient temperature, it increased their compressive strength and it functioned as an accelerator. The increase of latex concentration increased the presence of water and then diminished the density of the slurries and increased the water loss. From the results obtained, it was concluded that sodium tetraborate and non-ionic latex are promising additives for geopolymer slurries to be used in oilwell cementing operation

Tucker, Dominic M. "Mapping and Characterization of Phytophthora sojae and Soybean Mosaic Virus Resistance in Soybean." Diss., Virginia Tech, 2009. http://hdl.handle.net/10919/79598.

Full text
Abstract:
Phytophthora sojae, the causal organism of stem and root rot, and Soybean mosaic virus (SMV) cause two of the most highly destructive diseases of soybean (Glycine max L. Merr). P. sojae can be managed either through deployment of race-specific resistance or through quantitative resistance termed partial resistance. In the current study, partial resistance to P. sojae was mapped in an interspecific recombinant inbred line (RIL) population of Glycine max by Glycine soja. One major quantitative trait loci (QTL) on molecular linkage group (MLG)-J (chromosome 16) and two minor QTL on MLG-I (chromosome 20) and -G (chromosome 18) were mapped using conventional molecular markers. Additionally, partial resistance to P. sojae was mapped in the same RIL population using single feature polymorphism (SFP) markers that further fine mapped the P. sojae QTL and identified potential candidate genes contributing to resistance. In a separate study, race-specific resistance was characterized in PI96983 discovering a potentially new allele of Rps4 on MLG-G. Finally, using the newly available whole-genome shotgun sequence of soybean, Rsv4 conferring resistance to strains of SMV known in the US, was localized to an approximately 100 kb region of sequence on chromosome 2 (MLG-D1B). Newly designed PCR-based markers permit for efficient selection of Rsv4 by breeding programs. Identified candidate genes for Rsv4 are discussed. Genomic resources developed in all of these studies provide breeders the tools necessary for developing durable resistance to both SMV and P. sojae.

Abstract:
How people think about the end of the world greatly affects how they live in the present. This thesis examines how popular American thought about “the end of the world” has been greatly affected by Hal Lindsey’s 1970 popular prophecy book The Late, Great Planet Earth. LGPE sold more copies than any other non-fiction book in the 1970s and greatly aided the mainstreaming of “end-times” ideas like the Antichrist, nuclear holocaust, the Rapture, and various other concepts connected with popular end-times thought. These ideas stem from a specific strain of late-nineteenth century Biblical interpretation known as dispensational premillennialism, which has manifested in various schools of premillennial thought over the last 150 years. However, Lindsey translated this complicated system into modern language and connected it with contemporary geopolitics in powerful ways which helped make LGPE incredibly popular and influential in the 1970s and beyond. This paper includes an introduction to some essential concepts and terms related to popular premillennialism followed by a brief history of popular prophecy in America. The second half of this thesis examines the social, religious, and political climate of the 1970s and how Lindsey’s success connects to the culture of the Seventies, specifically conservative reactions to the various social movements of the 1960s. The last major section discusses Lindsey’s malleable theology and the power of interpreting the Bible “literally.” In the 1970s, conservative theologians and denominations won the battle to define certain concepts within Christianity including terms like “literal,” “inerrant,” and related terms, and Lindsey’s treatment of “the end times” reflects these definitions and how they affect Biblical interpretation. Finally, the conclusion fleshes out the appeal of popular premillennialism in the 1970s and into the present day.

Andrikopoulos, Corey J. "Comparative Pollination Efficacies of Bees on Raspberry and the Management of Osmia lignaria for Late Blooming Crops." DigitalCommons@USU, 2018. https://digitalcommons.usu.edu/etd/7086.

Full text
Abstract:
Unlike other rosaceous fruit crops such as apple and cherry, commercial raspberry cultivars are largely self-fertile and can set fruit in the absence of pollinators. However, their floral morphology often prevents complete self-pollaintion. Incomplete pollination yields unmarketable small or crumbly fruits. Insect visitation is therefore essential to maximizing raspberry yield. Honey bees are typically used to pollinate commercial raspberry; however, escalating prices for hive rentals coupled with increasing acreage encourage evaluation of other manageable pollinators. Bumble bees (Bombus spp.) and several mason bees (Osmia spp.) are promising raspberry pollinators. Five bee species were evaluated and compared for their single-visit pollination efficacies on raspberry. From this a pollinator effectiveness index was created and an estimation of the minimum number of visits required to maximize fruit set was calculated. This estimation was then experimentally verified. Finally, in an attempt to synchronize their brief activity period with raspberry bloom, winter management options aimed at delaying the emergence of the mason bee, O. lignaria, were investigated. All five bee species proved excellent pollinators of raspberry. None of the alternative manageable species greatly outperformed honey bees. For this reason honey bees remain the most economical and practical option for open-field raspberry pollination. The adoption of alternative manageable bees could still be justified in other production systems, such as high-tunnel or greenhouse grown raspberry, which hamper honey bees’ ability to forage effectively. The pollinator effectiveness score for honey bees suggested that as few as two visits can achieve maximum fruit set. This estimate was confirmed through experimentation on three different red raspberry cultivars. For two of these cultivars, just one visit yielded drupelet counts similar to openly-pollinated flowers. This information can be used to help refine stocking density estimates for honey bees on raspberry. Wintering bees at 0° or -3° C rather than 4° C effectively delayed emergence of O. lignaria by more than a month without any impact on post-winter performance. These results suggest winter storage at near freezing temperatures is a viable management option for the use of O. lignaria with later-blooming crops.

Abstract:
This thesis, in two volumes, deals with the Late Helladic I pottery from the southwestern Peloponnesos, Greece. It is a study of its origins, development and local variations. All wares (whether painted or unpainted) produced in this part of the Peloponnesos during LH I are considered; particular emphasis is put on the examination of the fine decorated ware; imports, wherever identifiable, are also dealt with. The text volume (Vol.I) consists of the Introduction and chapters I-V accompanied by an Appendix, a number of tables and a List of Abbreviations, as well as Bibliography. Abstract:
It is a well-known fact that hemorrhages are observed in wholly breastfed infants beyond the neonatal period. In order to clarify vitamin K (VK)-deficiency, it is necessary to follow-up the absorption and excretion of VK2.1. To 128 cases of newborns. i) The activity of VK-dependent factors(II, VII, X) were determined by Hepaplastin test(HPT), ii) Using Latex-test, PIVKA-II was tested. We found values of HPT (Y) and PIVKA-II (X) to be inversely proportional in the relation. Y=6l.9 - 6.7 X (r= -0.3). 2. These 15 cases of hypoprothrombinemia, VK2 6mg, VK2 2mgwere given, and plasma VK2-concentration was measured by gas chromatography. After 3 hours VK2 6mgconcentration was 1030ng/ml; VK2 2mg, was 224ng/ml. This clearly shows a dose-response relation. 3. VK2 transplacental transport was also proved by using umbilical venous blood after Cesarean section. (Before Cesarean section, VK2 60mgwas given. ) In umbilical venous blood, relatively high doses of VK2 (50 - 120ng/ml) were demonstrated. Additionally, the y-carboxylglutamic acid-concentration in the urine of newborn, who received VK2-syrup was higherthan those who did not receive it.

Abstract:
<div class="section abstract"><div class="htmlview paragraph">In this study, the impacts of fuel volatility and reactivity on combustion stability and emissions were studied in a light-duty single-cylinder research engine for a three-injection catalyst heating operation strategy with late post-injections. N-heptane and blends of farnesane/2,2,4,4,6,8,8-heptamethylnonane were used to study the impacts of volatility and reactivity. The effect of increased chemical reactivity was also analysed by comparing the baseline #2 diesel operation with a pure blend of mono-ether components (CN <i>&gt;</i> 100) representative of potential high cetane oxygenated bioblendstocks and a 25 vol.% blend of the mono-ether blend and #2 diesel with a cetane number (CN) of 55. At constant reactivity, little to no variation in combustion performance was observed due to differences in volatility, whereas increased reactivity improved combustion stability and efficiency at late injection timings. Fuels with higher reactivity were found to reduce engine-out hydrocarbon and carbon monoxide emissions while also achieving stable combustion at post-injection timings later than those achievable with #2 diesel fuel. The pure ether blend had the latest achievable post-injection timing of +30.5 CAD while still maintaining stable combustion (coefficient of variation of gross-indicated mean effective pressure <i>&lt;</i> 5%). With post-injection timing adjusted to achieve a matched exhaust temperature of 300 °C, the ether-diesel CN 55 blend was observed to have slightly higher thermal efficiency in comparison to the baseline #2 diesel fuel. The results also indicate that cetane number may serve as a good indicator of combustion characteristics at late injection timings used for aftertreatment thermal management operation.</div></div>

Abstract:
Requirements Engineering (RE) is getting more attention as it has been recognized as a crucial phase in the development of the software system life cycle. Recent works have made a distinction between early-phase RE and later-phase RE. Early-phases RE activities are typically informal and addresses non-functional requirements. The later-phase RE usually focuses on completeness, consistency, and automated verification of requirements. In this paper, we show how early and late requirements specifications can be integrated. For the organization modeling we use the i* technique [1], which allows a better description of the organizational relationships among the various agents of a system as well as an understanding of the rationale of the decisions taken. For the formal functional specification of the requirements, we use Structured Modal Action Logic - MAL [2]. Some guidelines are presented for the integration of the two phases. Throughout the paper, we make use of a mineral water factory as an example, to describe the approach.

Abstract:
The aim of the research is to identify the elements of the psycholinguistic image of the leading character in the dark fantasy adventure film Maleficent directed by Robert Stromberg (2014). The task consists of two stages, at the first of which I identify the psychological characteristics of the character to determine to which of the archetypes Maleficent belongs. As the basis, I take the classification of film archetypes by V. Schmidt. At the second stage, I distinguish the speech peculiarities of the character that reflex her psychological image. This paper explores 98 Maleficent’s turns of dialogues in the film. According to V. Schmidt’s classification, Maleficent belongs first to the Matriarch archetype and later in the plot to the Scorned Woman archetype. These archetypes are representations of the powerful goddess of marriage and fertility Hera, being respectively her heroic and villainous embodiments. There are several crucial characteristics revealed by speech elements.

Lunsford, Kurt G. Business Cycles and Low-Frequency Fluctuations in the US Unemployment Rate. Federal Reserve Bank of Cleveland, August 2023. http://dx.doi.org/10.26509/frbc-wp-202319.

Full text
Abstract:
I show that business cycles can generate most of the low-frequency movements in the unemployment rate. First, I provide evidence that the unemployment rate is stationary, while its flows have unit roots. Then, I model the log unemployment rate as the error correction term of log labor flows in a vector error correction model (VECM) with intercepts that change over the business cycle. Feeding historical expansions and recessions into the VECM generates large low-frequency movements in the unemployment rate. Frequent recessions from the late 1960s to the early 1980s interrupt labor market recoveries and ratchet the unemployment rate upward. Long expansions in the 1980s and 1990s undo this upward ratcheting. Finally, the VECM predicts that the unemployment rate will be near 3.6 percent after a 10-year expansion and that lower unemployment rates are possible with longer expansions.

Chauvin, Juan Pablo. Local Education Spending and Migration: Evidence from a Large Redistribution Program. Inter-American Development Bank, April 2024. http://dx.doi.org/10.18235/0012910.

Full text
Abstract:
This paper studies the effects of changes in local public education budgets on individual schooling attainment and migration, as well as on local labor market outcomes. I leverage the introduction of FUNDEF, a large federal program that redistributed public education finance across Brazilian municipalities in the late 1990s, as a source of exogenous variation. Using a cohort-exposure design, I find that, at the individual level, doubling the program-related public education budget led to a 1.4 percentage point increase in the likelihood of completing primary school, and a 0.5 percentage point decrease in the likelihood of staying in the local labor market among exposed cohorts, on average. The mobility effects are concentrated among individuals educated in municipalities that received a positive budget shock as a result of the program, which were also characterized by relatively worse local labor market conditions. At the local labor market level, difference-in-differences estimates suggest that higher public education budgets were associated with lower employment rates and average wages, suggesting that the “brain drain” effect depressed local labor demand in the long run.

Abstract:
Large-scale HPC simulations with their inherent I/O bottleneck have made in situ visualization an essential approach for data analysis, although the idea of in situ visualization dates back to the era of coprocessing in the 1990s. In situ coupling of analysis and visualization to a live simulation circumvents writing raw data to disk for post-mortem analysis -- an approach that is already inefficient for today's very large simulation codes. Instead, with in situ visualization, data abstracts are generated that provide a much higher level of expressiveness per byte. Therefore, more details can be computed and stored for later analysis, providing more insight than traditional methods. This workshop encouraged talks on methods and workflows that have been used for large-scale parallel visualization, with a particular focus on the in situ case.

MacFarlane, Andrew. 2021 medical student essay prize winner - A case of grief. Society for Academic Primary Care, July 2021. http://dx.doi.org/10.37361/medstudessay.2021.1.1.

Full text
Abstract:
As a student undertaking a Longitudinal Integrated Clerkship (LIC)1 based in a GP practice in a rural community in the North of Scotland, I have been lucky to be given responsibility and my own clinic lists. Every day I conduct consultations that change my practice: the challenge of clinically applying the theory I have studied, controlling a consultation and efficiently exploring a patient's problems, empathising with and empowering them to play a part in their own care2 – and most difficult I feel – dealing with the vast amount of uncertainty that medicine, and particularly primary care, presents to both clinician and patient. I initially consulted with a lady in her 60s who attended with her husband, complaining of severe lower back pain who was very difficult to assess due to her pain level. Her husband was understandably concerned about the degree of pain she was in. After assessment and discussion with one of the GPs, we agreed some pain relief and a physio assessment in the next few days would be a practical plan. The patient had one red flag, some leg weakness and numbness, which was her ‘normal’ on account of her multiple sclerosis. At the physio assessment a few days later, the physio felt things were worse and some urgent bloods were ordered, unfortunately finding raised cancer and inflammatory markers. A CT scan of the lung found widespread cancer, a later CT of the head after some developing some acute confusion found brain metastases, and a week and a half after presenting to me, the patient sadly died in hospital. While that was all impactful enough on me, it was the follow-up appointment with the husband who attended on the last triage slot of the evening two weeks later that I found completely altered my understanding of grief and the mourning of a loved one. The husband had asked to speak to a Andrew MacFarlane Year 3 ScotGEM Medical Student 2 doctor just to talk about what had happened to his wife. The GP decided that it would be better if he came into the practice - strictly he probably should have been consulted with over the phone due to coronavirus restrictions - but he was asked what he would prefer and he opted to come in. I sat in on the consultation, I had been helping with any examinations the triage doctor needed and I recognised that this was the husband of the lady I had seen a few weeks earlier. He came in and sat down, head lowered, hands fiddling with the zip on his jacket, trying to find what to say. The GP sat, turned so that they were opposite each other with no desk between them - I was seated off to the side, an onlooker, but acknowledged by the patient with a kind nod when he entered the room. The GP asked gently, “How are you doing?” and roughly 30 seconds passed (a long time in a conversation) before the patient spoke. “I just really miss her…” he whispered with great effort, “I don’t understand how this all happened.” Over the next 45 minutes, he spoke about his wife, how much pain she had been in, the rapid deterioration he witnessed, the cancer being found, and cruelly how she had passed away after he had gone home to get some rest after being by her bedside all day in the hospital. He talked about how they had met, how much he missed her, how empty the house felt without her, and asking himself and us how he was meant to move forward with his life. He had a lot of questions for us, and for himself. Had we missed anything – had he missed anything? The GP really just listened for almost the whole consultation, speaking to him gently, reassuring him that this wasn’t his or anyone’s fault. She stated that this was an awful time for him and that what he was feeling was entirely normal and something we will all universally go through. She emphasised that while it wasn’t helpful at the moment, that things would get better over time.3 He was really glad I was there – having shared a consultation with his wife and I – he thanked me emphatically even though I felt like I hadn’t really helped at all. After some tears, frequent moments of silence and a lot of questions, he left having gotten a lot off his chest. “You just have to listen to people, be there for them as they go through things, and answer their questions as best you can” urged my GP as we discussed the case when the patient left. Almost all family caregivers contact their GP with regards to grief and this consultation really made me realise how important an aspect of my practice it will be in the future.4 It has also made me reflect on the emphasis on undergraduate teaching around ‘breaking bad news’ to patients, but nothing taught about when patients are in the process of grieving further down the line.5 The skill Andrew MacFarlane Year 3 ScotGEM Medical Student 3 required to manage a grieving patient is not one limited to general practice. Patients may grieve the loss of function from acute trauma through to chronic illness in all specialties of medicine - in addition to ‘traditional’ grief from loss of family or friends.6 There wasn’t anything ‘medical’ in the consultation, but I came away from it with a real sense of purpose as to why this career is such a privilege. We look after patients so they can spend as much quality time as they are given with their loved ones, and their loved ones are the ones we care for after they are gone. We as doctors are the constant, and we have to meet patients with compassion at their most difficult times – because it is as much a part of the job as the knowledge and the science – and it is the part of us that patients will remember long after they leave our clinic room. Abstract:
In 1997, the Mexican government designed the conditional cash transfer program Progresa, which became the worldwide model of a new approach to social programs, simultaneously targeting human capital accumulation and poverty reduction. A large literature has documented the short and medium-term impacts of the Mexican program and its successors in other countries. Using Progresas experimental evaluation design originally rolled out in 1997-2000, and a tracking survey conducted 20 years later, this paper studies the differential long-term impacts of exposure to Progresa. We focus on two cohorts of children: i) those that during the period of differential exposure were in-utero or in the first years of life, and ii) those who during the period of differential exposure were transitioning from primary to secondary school. Results for the early childhood cohort, 18-20-year-old at endline, shows that differential exposure to Progresa during the early years led to positive impacts on educational attainment and labor income expectations. This constitutes unique long-term evidence on the returns of an at-scale intervention on investments in human capital during the first 1000 days of life. Results for the school cohort - in their early 30s at endline - show that the short-term impacts of differential exposure to Progresa on schooling were sustained in the long-run and manifested themselves in larger labor incomes, more geographical mobility including through international migration, and later family formation.
