Full textDet har länge varit känt att arkitektur baserad på mellanliggande utrustning, s.k.middlebox-arkitektur, inte alltid rimmar väl med Internet i övrigt. Nätverksfunktioner som implmenteras i middleboxar tenderar att leda till olika typer av förbindelser, modifierande av pakethuvuden eller nya uppkopplingar, vilket medför att det blir utmanande att verkställa olika typer av policy. Middlebox-lösningar är dessutom tätt kopplade till olika typer av metadata vilket innebär svårigheter för dess distribuerade fortlevnad och hindrar ett flexibelt utnyttjande och skalbar utbyggnad middleboxresurser under varierande trafiklast. Befintliga försök att mildra sådana problem inkluderar placering av mellanliggande utrustning, paketmärkning och migration av metadata; vart och ett av dessa löser endast delar av problemet.TRF (Treatment Framework) drar nytta av den utbyggbarhet som finns i IPv6 och använder vägval som styrs helt från avsändaren tillsammans med diskretionär adressrymd. IP-paketen traverserar en behandlings-domän och ett utökat pakethuvud används inom domänen. Det utökade pakethuvudet läggs till när paketet är på väg in i domänen och tas bort när paketet lämnar domänen. Inom domänen används information i det utökade pakethuvudet för att styra vidarebefordringen av paketet. Mekanismerna för vidarebefordring av paket använder sig av SDN och består av en match-operation och tre OpenFlow-åtgärder, varmed TRF kringgår behov av en underliggande transport. En anpass-ningsbart adressrymd gör det möjligt för leverantörer att skräddarsy vägvalsaggregering till sin middlebox-lösning, vilket gör att antalet trafikregler i kärnan av deras nät kan reduceras till förinställbara storlekar.En middlebox-lösning i en behandlings-domän matchar trafik mot respektive lokal policy baserad på information kodad i det utökade pakethuvudet. Tekniken med utökat pakethuvud är inbyggd i IPv6 och standardiserad vilket göra att paketmodifiering i en middlebox kan adresseras utan att kräva ändringar eller insyn i proprietär programvara. Ramverket löser problemet med verkställa olika typer av policy i sin helhet och medger skapandet av assymtriska kedjor av paketbehandlingstjänster. Tiden för att sätta upp paketflöden i kärnan av nätet kan elimineras, men det fotavtryck som ramverket ger i ingressdelen av domänen, där det utökade pakethuvudet skapas och läggs till inkommand paket, kan bli betydande med avseende på flödeshastigheten.

Full textThis study aimed to analyze the integration of assistance to tuberculosis patients (TB) from recording instruments in Ribeirão Preto. Exploratory study with a quantitative approach. The study population consisted of TB patients notified and living in the city who completed treatment between 2012 and 2013, excluding patients in the prison system, transferred or had their diagnoses changed. The survey was conducted by collecting secondary data sources in 4: in the state reporting system (TBWEB) for identification of TB patients; in the clinical records of patients selected in the covering sheet of supervised treatment (yellow plug) and in the book registry of patients and follow-up treatment of tuberculosis cases (Green Paper), which allowed access to records related to medical information, test results and foreclosure situation. The selected variables were grouped according to the cast of dimensions of services and care coordination. Data were analyzed using descriptive statistics. The results showed that among the services offered, 72.5% of patients underwent five or more medical visits, while only 9.2% had 1-2 consultations with the nurse. The return guidance to consultations held by the medical staff (96.3%) and nursing (84.4%) was the one that prevailed. The procedure most often performed by the physician was the clinical outcome (97.2%) and nursing staff was the pre and post consultation (69.7%). According to data from medical records and the yellow plug, 75.2% received supervised treatment and the predominance of supervision was at home. Sputum smear microscopy was performed in 89.0% of patients and the Anti-HIV test in 69.7% of cases. The basket was offered to 23.8% and 18.3% of patients received care with social worker. In 71.3% of cases there were no reports on the control contacts. Regarding the monitoring of cases, 29.4% had faults in scheduled appointments and conduct that prevailed for these situations was the home visit (10.1%). Only 10.1% of patients were referred to other services by the unit that carried out the treatment and other care occurred without any referrals (24.8%). The analysis of medical records, Green and Yellow Sheet revealed a low number of records and numerous gaps in their fulfillment both in relation to the monitoring of cases, as well as the shares offered by health services during treatment. This makes the integration and continuity of care provided and impairs verification of the actual proportion of services that were offered and used by patients. Thus, there is a need for adequate filling of the instruments in order to facilitate the development of planning strategies of assistance from the coordination of programs. The full user-centered care and an appropriate coordination of health services can improve the care provided to TB patients

Full textTiivistelmä Informaation määrä on kasvanut merkittävästi tietoyhteiskunnan kehittymisen myötä. Käytössämme onkin huomattava määrä erimuotoista tietoa, josta voimme hyödyntää kuitenkin vain osan. Jatkuvasti mitattavan datan suuri määrä ja sijoittuminen hajalleen asettavat osaltaan haasteita tiedon hyödyntämiselle. Tietoyhteiskunnassa hyvinvointi ja terveellisen elinympäristön säilyminen koetaan aiempaa tärkeämmäksi. Toisaalta yritysten toiminnan tehostaminen ja kestävän kehityksen edistäminen vaativat jatkuvaa parantamista. Informaatioteknologian avulla moniulotteista mittaus- ja rekisteritietoa voidaan hyödyntää esimerkiksi ennakoivaan päätöksentekoon jolla voidaan edistää edellä mainittuja tavoitteita. Tässä työssä on esitetty ympäristöalan älykkäiden informaatiopalveluiden konsepti, jossa oleellista on loppukäyttäjien tarpeiden tunnistaminen ja ongelmien ratkaiseminen jalostetun informaation avulla. Älykkäiden informaatiopalvelujen taustalla on yhtenäinen tiedonlouhintaan perustuva tiedonjalostusprosessi, jossa raakatieto jalostetaan loppukäyttäjille soveltuvaan muotoon. Tiedonjalostusprosessi koostuu datan keräämisestä ja esikäsittelystä, mallintamisesta, tiedon visualisoinnista, tulosten tulkitsemisesta sekä oleellisen tiedon jakamisesta loppukäyttäjäryhmille. Datan käsittelyyn ja analysointiin on käytetty laskennallisesti älykkäitä menetelmiä, josta juontuu työn otsikko; älykkäät informaatiopalvelut. Väitöskirja pohjautuu viiteen artikkeliin, joissa osoitetaan tiedonjalostusprosessin toimivuus erilaisissa tapauksissa ja esitetään esimerkkejä kuhunkin prosessin vaiheeseen soveltuvista laskennallisista menetelmistä. Artikkeleissa on kuvattu matkailualueen kävijämäärien ennakointiin ja kotitalouksien sähköenergian kulutuksen pienentämiseen liittyvät informaatiopalvelut sekä analyysi selluprosessissa käytettävien kemikaalien määrän pienentämiseksi. Näistä saadut kokemukset ja tulokset on yleistetty älykkään informaatiopalvelun konseptiksi. Väitöskirjan toisena tavoitteena on rohkaista organisaatioita hyödyntämään tietovarantoja aiempaa tehokkaammin ja monipuolisemmin sekä rohkaista tarkastelemaan myös oman organisaation ulkopuolelta saatavien tietolähteiden käyttämistä. Full textInformation technology (IT) is a component that influences organizations´s performance and its effective application constitutes a challenge due to the speed associated to technological changes and the amount of financial resources and expertise of human resources required for its implementation. One of the strategies adopted by organizations to meet the needs of IT is to outsource all or part of the provision of IT services. The objective of this study was to describe the essence of the meaning of advisory experience according to the perception of professionals who provide IT consulting services. To achieve this purpose, a phenomenographic research was conducted in order to allow a group of fifteen consultants working in a regulatory public institution freely put its experience on the provision of IT consulting services. The data analysis resulted in the identification of the themes Acting Area Significance, Competency Advisory Conformation, Services Valuation and Advisory Predisposition related to IT consulting. The meanings attributed for these subjects allowed to propose a description of the essence of this kind of experience. The relevance of the research lies in the fact that, typically, the outsourcing analysis emphasizes the outsourcing contractual relationship established between the contractor and the contracted company and recognizes the consultant as only one of the resources needed to provide the service without considering the peculiarities inherent to the advisory activity.

(cont.) to encourage the interconnection of planning and mainstream information technology. We find that the PAMML framework can lower costs by leveraging mainstream technology, simplify the most basic data sharing activities, yet still allow organizations with different levels of technical sophistication to collaborate. PAMML captures the semantics of spatial planning problems, allowing them to be decomposed into fundamental information processing operations. Regarding user interfaces, we show that PAMML's structure allows multiple end user applications aimed towards different audiences to be easily built from the same core PAMML document.
This thesis examines systemic problems with the way information is managed and processed in planning support systems. We find evidence of these problems when we attempt to: develop an analysis without spending most of the time gathering and organizing data sets; or build an analysis that can be re-run at low cost; or implement systems that interact collaboratively with those of other experts. This research starts with the hypothesis that these problems are related and systemic, and that a new paradigm of information management is needed if we can hope to address them effectively. The research is divided into two main sections. First, we develop a theory about how information flows within and across planning organizations, and use the MassGIS buildout analysis to understand how physical planning is done in a cross-jurisdictional, real-world setting. We find that modern organizations do are good at creating and disseminating information, but find it difficult to keep users' copies of published information up-to-date. Furthermore, the technology for building interactive front-ends to analytic models is poorly matched to user needs, and the technology for enabling cross-organization collaborative analysis is non-existent. In the second part of the thesis, we re-architect the information framework, guided by our new theoretical foundation and findings from practice. This new framework is based on Web services, an emerging technology for connecting information systems across organizations. It is called the Planning Analysis and Modeling Markup Language framework, or PAMML, consisting of an information processing vocabulary expressed in XML Schema, Web services based on the schema, and guidance on how to best use the framework

Full textIn order to manage public dental services, information is required about what work is being performed by the staff at the various clinics. Tally sheets have been used in the past to record treatment procedures but this is not an effective method of recording the amount of work done by staff at public dental clinics. But tally sheets are inaccurate, open to abuse, and fail to provide the necessary information for managers. Nor is it of any real value for providing feedback to staff on their performance. This inhibits a core aspect of job satisfaction for the staff, which is feedback. The staff just persevere, continue doing the same thing and feel frustrated. This contributes to poor work performance. Instead of using a tally sheet, 4 digit treatment codes are used for all treatment procedures (as used in the private sector for billing purposes) and additional codes were developed for services such as brushing programmes for which billing codes do not exist. These are recorded for each patient, together with a code for the patient category. A relative value unit (RVU) has been developed for each treatment code that has been weighted according to policy guidelines and the amount of time and effort required to provide the service. This was done for clinical treatment procedures as well as for community-based preventive activities. A computer program has been developed that captures the treatment codes which are saved in a number of databases that are linked to Excel pivot tables. The data can therefore be easily manipulated by the user to obtain the required information in the form of counts of procedures, monetary cost of the same clinical services in the private sector (useful with the proposed advent of National Health Insurance) and also in the form of relative value units. iii This is available for the current reporting period as well as for previous periods, allowing a detailed analysis of services rendered and staff performance over a period of time to show trends. Use is also made of an Objectives Matrix where the performance of each staff member can be measured according to seven objectives (Key Performance Areas) (five in the case of oral hygienists) to produce an overall Performance Index – which is a score out of ten. This enables performance appraisal to be carried out much easier than by comparing performance based on a number of diverse treatments provided. The data for all the public dental clinics in the Western Cape Province has been analysed for the period 1994 to 2012 using this system, and it has been shown that the system is sensitive enough to highlight problem areas as well as provide a balanced overall view of the service, as measured by a number of variables. The system is “low tech” in that it runs on a “stand alone” personal computer, but it could easily be applied to an integrated, networked information system provided the latter contained the treatment codes, and certain other patient, staff and clinic identifiers. It is therefore suitable for developing countries, such as South Africa, that may later develop a comprehensive Health Information System based on an electronic medical record. The emphasis is not on the information technology, it is focussed on the concepts behind the processing of the data into meaningful information for managing public dental services.

There are lots of reports about people unceremoniously taking medicine or throwing them away nowadays, these action may caused very bad reaction and environmental hazard all because they are lack of relevant knowledge. According to the statistics, people in Taiwan cost more than one hundred billion on medicine which makes the National Health Insurance(NHI)'s burden increased as well as the number of chronic year by year. In order to prevent the increase of chronic patients, Health Promotion Administration(HPA) has offered a method called "Blood Pressure 3C Management". Blood Pressure 3C Management is a preventive healthcare by testing physiological status regularly. The statistics show that the top ten causes of death in Taiwan are mainly chronic diseases. Among them, Hypertension, Hyperlipidemia and Hyperglycemia are most critical. To prevent chronic increase, people should do general checkup regularly and seek medical treatment immediately if checkup report is not normal. This thesis proposed an App based on mobile smart device named Network Service Information Platform of Smart Medical Treatment and Personal Health Management, which can solve these problems mentioned before. The App can assist citizen know the medicines information, time period of taking and discard method, and manage personal health. This may reduce the situation of taking wrong medicine and environmental pollution. The App can also help with recording measured value of blood glucose, blood lipids, blood pressure and weight, which can offer to doctor a long time record to assessing the condition.
