This chapter describes the different inborn errors of metabolism that can present with or be revealed by hypoglycemia, with emphasis on the diagnostic work-up and symptoms.

Abstract:
The detrimental effects of hyperglycemia have been enumerated in critically ill patients, and more rigid control of glucose during the perioperative period has been advocated. The more liberal use of intraoperative continuous insulin infusions, however, has unfortunately led to an increased incidence of hypoglycemia. Anesthetized patients exhibit few, if any, signs of severe hypoglycemia. Because the brain is dependent on glucose as a primary energy source, the most devastating result of unrecognized hypoglycemia may be permanent neurologic injury or death. Therefore, it is imperative that the anesthesiologist recognize patients who are at risk for this complication and frequently measure glucose levels to avoid inadvertent hypoglycemia.

Abstract:
Hypoglycemia is a frequent and often overlooked symptom. Indeed, most cases are related to insulin therapy in diabetic patients, to hormone deficiencies, or to very prolonged fasts. In other cases, however, unusual findings (unexplained hypoglycemia, especially if clinically severe, recurrent, postprandial, or in typical relationship to external factors such as food ingestion or exercise, or associated with hepatomegaly) should lead to a careful clinical and biological evaluation. Hyperinsulinemic hypoglycemia is a dysregulation of the glucose-induced insulin secretion. Besides insulinoma and genetic defects of the pancreatic ß-cell (congenital hyperinsulinism, CHI), some other pathophysiological mechanisms can lead to hypoglycemia with biological evidence for an involvement of the insulin signaling pathway (insulin-like substances, autoimmunity, and other).
