Abstract:
To assess functional relatedness of individual components of the eukaryotic transcription apparatus, three human subunits (hsRPB5, hsRPB8, and hsRPB10) were tested for their ability to support yeast cell growth in the absence of their essential yeast homologs. Two of the three subunits, hsRPB8 and hsRPB10, supported normal yeast cell growth at moderate temperatures. A fourth human subunit, hsRPB9, is a homolog of the nonessential yeast subunit RPB9. Yeast cells lacking RPB9 are unable to grow at high and low temperatures and are defective in mRNA start site selection. We tested the ability of hsRPB9 to correct the growth and start site selection defect seen in the absence of RPB9. Expression of hsRPB9 on a high-copy-number plasmid, but not a low-copy-number plasmid, restored growth at high temperatures. Recombinant human hsRPB9 was also able to completely correct the start site selection defect seen at the CYC1 promoter in vitro as effectively as the yeast RPB9 subunit. Immunoprecipitation of the cell extracts from yeast cells containing either of the human subunits that function in place of their yeast counterparts in vivo suggested that they assemble with the complete set of yeast RNA polymerase II subunits. Overall, a total of six of the seven human subunits tested previously or in this study are able to substitute for their yeast counterparts in vivo, underscoring the remarkable similarities between the transcriptional machineries of lower and higher eukaryotes.

ABSTRACT Under conditions of environmental stress, prokaryotes and lower eukaryotes such as the yeast Saccharomyces cerevisiaeselectively utilize particular subunits of RNA polymerase II (pol II) to alter transcription to patterns favoring survival. In S. cerevisiae, a complex of two such subunits, RPB4 and RPB7, preferentially associates with pol II during stationary phase; of these two subunits, RPB4 is specifically required for survival under nonoptimal growth conditions. Previously, we have shown that RPB7 possesses an evolutionarily conserved human homolog, hsRPB7, which was capable of partially interacting with RPB4 and the yeast transcriptional apparatus. Using this as a probe in a two-hybrid screen, we have now established that hsRPB4 is also conserved in higher eukaryotes. In contrast to hsRPB7, hsRPB4 has diverged so that it no longer interacts with yeast RPB7, although it partially complements rpb4− phenotypes in yeast. However, hsRPB4 associates strongly and specifically with hsRPB7 when expressed in yeast or in mammalian cells and copurifies with intact pol II. hsRPB4 expression in humans parallels that of hsRPB7, supporting the idea that the two proteins may possess associated functions. Structure-function studies of hsRPB4-hsRPB7 are used to establish the interaction interface between the two proteins. This identification completes the set of human homologs for RNA pol II subunits defined in yeast and should provide the basis for subsequent structural and functional characterization of the pol II holoenzyme.

Abstract:
High school research experience programs (HSREPs) provide opportunities for true science education and expose students to scientific investigations in laboratory settings. Various HSREPs models have been practiced to shape students’ research understandings; however, a systematic comparison of the success, challenges, and opportunities of these HSREPs has not been gauged. This article compares the effectiveness of such science, technology, engineering, and mathematics (STEM) based HSREP models reported in the last two decades. We shortlisted seventeen studies on the most effective HSREPs and identified the characteristics of these reports. Results show that student research experiences vary depending on the structure of the model used and the nature of the laboratory setting to which students are exposed. However, there is a dire need to integrate more collaborative and customized research practices to accommodate more students in HSREPs. Additionally, intensive support, mentoring, and coaching are essential to provide students a comprehensive understanding to excel in their research career pathway. Finally, there is a desperate need for further studies to develop the frameworks that can help the smooth transition of high school students into research-oriented university programs.

Modeling user’s fine-grained preferences and dynamic preference evolution from their chronological behaviors are challenging and crucial for sequential recommendation. In this paper, we develop a Hierarchical Self-Attention Incorporating Knowledge Graph for Sequential Recommendation (HSRec). HSRec models not only the user’s intrinsic preferences but also the user’s external potential interests to capture the user’s fine-grained preferences. Specifically, the intrinsic interest module and potential interest module are designed to capture these two preferences respectively. In the intrinsic interest module, user’s sequential patterns are characterized from their behaviors via the self-attention mechanism. As for the potential interest module, high-order paths can be generated with the help of the knowledge graph. Therefore, a hierarchical self-attention mechanism is designed to aggregate the semantic information of user interaction from these paths. Specifically, an entity-level self-attention mechanism is applied to capture the sequential patterns contained in the high-order paths while an interaction-level self-attention mechanism is designed to further capture the semantic information from user interactions. Moreover, according to the high-order semantic relevance, HSRec can explore the user’s dynamic preferences at each time, thus describing the user’s dynamic preference evolution. Finally, experiments conducted on three real world datasets demonstrate the state-of-the-art performance of the HSRec.

Ewing’s Sarcoma Oncogene (ews) on chromosome 22q12 is encoding a ubiquitously expressed RNA-binding protein (EWS) with unknown function that is target of tumor-specific chromosomal translocations in Ewing’s sarcoma family of tumors. A model of transcription complex was proposed in which the heterodimer Rpb4/7 binds to EAD, connecting it to Core RNA Pol II. The DNA-binding domain, provided by EFP, is bound to the promoter. Rpb4/7 binds RNA, stabilizing the transcription complex. The complex Rpb4/7 can stabilize the preinitiation complexes by converting the conformation of RNA Pol II. EWS may change its conformation, so that NTD becomes accessible. Two different mechanisms of interaction between EWS and RNA Pol II are proposed: (I) an intermolecular EWS-EWS interaction between two molecules, pushing conformation from “closed” to “open” state, or (II) an intramolecular interaction inside the molecule of EWS, pushing conformation of the molecule from “closed” to “open” state. The modified forms of EWS may interact with Pol II subunits hsRpb5 and hsRpb7. The EWS and EFPs binding partners are described schematically in a model, an attempt to link the transcription with the splicing. The proposed model helps to understand the functional molecular interactions in cancer, to find new partners and ways to treat cancer.

Abstract:
Using a screen to identify human genes that promote pseudohyphal conversion in Saccharomyces cerevisiae, we obtained a cDNA encoding hsRPB7, a human homologue of the seventh largest subunit of yeast RNA polymerase II (RPB7). Overexpression of yeast RPB7 in a comparable strain background caused more pronounced cell elongation than overexpression of hsRPB7. hsRPB7 sequence and function are strongly conserved with its yeast counterpart because its expression can rescue deletion of the essential RPB7 gene at moderate temperatures. Further, immuno-precipitation of RNA polymerase II from yeast cells containing hsRPB7 revealed that the hsRPB7 assembles the complete set of 11 other yeast subunits. However, at temperature extremes and during maintenance at stationary phase, hsRPB7-containing yeast cells lose viability rapidly, stress-sensitive phenotypes reminiscent of those associated with deletion of the RPB4 subunit with which RPB7 normally complexes. Two-hybrid analysis revealed that although hsRPB7 and RPB4 interact, the association is of lower affinity than the RPB4-RPB7 interaction, providing a probable mechanism for the failure of hsRPB7 to fully function in yeast cells at high and low temperatures. Finally, surprisingly, hsRPB7 RNA in human cells is expressed in a tissue-specific pattern that differs from that of the RNA polymerase II largest subunit, implying a potential regulatory role for hsRPB7. Taken together, these results suggest that some RPB7 functions may be analogous to those possessed by the stress-specific prokaryotic sigma factor rpoS.

e14609 Background: Resistance acquisition to chemotherapeutic agents is one of the main problems that come up during cancer treatment. hSRBC is a tumor suppressor gene whose inactivation has been associated with malignant tumor progression. In a previous work, we investigated the influence of hSRBC promoter methylation alterations in oxaliplatin (OXA) resistance acquisition by using a CRC “in vitro” model, detecting an hSRBC promoter hypermethylation in OXA–resistant derived cells when compared with the sensitive counterpart. These results were validated by functional analyses in the same model. Taking this into account, our aim in the present work is to determine the role of hSRBC methylation status as a potential biomarker of OXA resistance, in metastatic CRC patients, treated at first line with fluouropirimidines plus OXA based chemotherapy. Methods: hSRBC promoter hypermethylation was analyzed in DNA extracted from paraffin embebbed tissue of 111 metastatic CRC tumors by using Methylation Specific PCR. Methylation data was correlated to overall response (OR) and progression free survival (PFS) by using F-Fisher test and Kaplan-Meyer Survival curves respectively. A multivariate analysis was carried out by Cox regression. p-values under 0.05 were considered statistic significant. Results: Two independent cohorts of stage IV CRC tumors were included. Twenty-two out of 111 patients received radical surgery for metastasis, that became to be a positive prognostic factor (p-value = 0.04). Gene hypermethylation was detected in a 33% of cases. Although OR was not associated with hSRBC methylation, we observed a significant correlation between hypermethylation of gene and a worse PFS in patients without metastasis surgery (Log Rank; p-value = 0.04). Conclusions: Remarkably, hSRBC promoter hypermethylation is associated with worse PFS in metastatic CRC patients. We suggest hSRBC methylation status as a predictive biomarker of OXA-based treatment outcome in metastatic CRC patients. However, further studies are warranted in order to elucidate the clinical application of these findings.

Abstract Welcome to the third virtual HSRPP conference in this 28th anniversary of the conference. We hope to provide you with an enjoyable and interesting conference. Little did we know when we were invited to host this conference that we would be unable to welcome you in-person to the lovely city of Bath. After much deliberation in Summer 2021, with too many unknowns due to the COVID-19 pandemic, we had to make the call to run the conference online. By October 2021 we doubted ourselves. As we write this in January 2022, with the Omicron wave upon us, it feels like it was probably the right decision. We do all of course hope that HSRPP 2023 can bring us all together again in-person, but until then, we hope you enjoy our online event. HSRPP 2022 continues to provide an environment for researchers and practitioners, both new and experienced, to come together to share research and exchange ideas. Our conference theme is ‘Delivering impact for patients and other stakeholders’. We chose this theme because we believe it is central to why most of us do health services and pharmacy practice research. The days are long gone when research could remain within the pages of academic journals. We need to be able to demonstrate to our patients, our funders and the public how we will turn our findings into something that improves care. We need to work with patients and the public to make sure our research is relevant to them and efficient in its spend of what is, very often, public money. In short, it is what gets us up in the morning and through the tough times of research, of which there are many! The COVID-19 pandemic has shown just how valuable pharmacists are, whether that be delivering vaccinations, caring for critically ill hospitalised patients or coping with the huge pressure that community colleagues have faced from the worried public. These contributions to the pandemic cause are of course on top of the ‘day job’. There is much learned and to be learned about how we move forward to deliver healthcare that is sustainable, patient-centred and fit for purpose. Research is the tool from which this learning comes. We are going to hear real world evidence under our conference theme from our three keynote speakers: Dr Barbara Farrell, a pharmacist with the Bruyère Continuing Care Geriatric Day Hospital, and Senior Investigator with the Bruyère Research Institute in Ottawa, Ontario, Canada, will discuss her research experience with engaging stakeholders and users to promote the uptake and impact of deprescribing guidelines. She will highlight how her research has influenced and contributed to change in how people approach deprescribing and what difference this has made. Professor Mahendra Patel, University of Bradford, is the national Pharmacy and Ethnic Minority Communities Research Lead with the PRINCIPLE trial, University of Oxford, the world’s largest randomized clinical trial investigating COVID-19 treatment in the community. He will talk to us about promoting inclusion and diversity in clinical trials, which is critical if trial results are to be meaningful and successful when implemented to bring about change. Mr Ade Williams, lead pharmacist and independent prescriber at the multi-award-winning Bedminster Pharmacy in Bristol will discuss how researchers can work with practising pharmacists to co-develop “real-life’’ impact programmes. He will also highlight how transformations in the NHS governance structure, Long Term Plan ambitions and COVID-19 legacy work are unique opportunities for community pharmacy to play a vital role. Mahendra and Ade were both recognised in the New Year Honours List 2022 for their outstanding contributions to Pharmacy. We would like to thank all those who have contributed to making HSRPP 2022 a success. An orthogonal experimental design method was adopted for the mix design of high early strength recycled aggregate concrete (HSRAC), and the influence of water-binder ratio, percentage of recycled aggregate(RA)and highly active compound mineral admixture (HACMA) content on the strength of the concrete was discussed. The test results indicated that water-binder ratio was the most principal and significant influencing factor on the strength of HSRAC, regardless of age. An empirical relationship among the strength of HSRAC, the water-binder ratio, the percentage of recycled aggregate and HACMA content was proposed by using multivariate regression analysis. The demand of opening heavy or heavier traffics and rapid repair concrete on strength to HSRAC could be all met.

Abstract:
Making an accurate diagnosis is essential to ensure that a patient receives appropriate treatment and correct information regarding their prognosis. Characteristics of diagnostic tests are quantified in test accuracy studies, but many such studies have methodological flaws. The HSRC evidence-based diagnosis programme has focused on methods for systematic reviews of test accuracy studies, and the wider context in which tests are ordered and interpreted. We carried out a range of projects relating to literature searching, quality assessment, meta-analysis, presentation of results, and interactions between doctors and patients during the diagnostic process. We have shown that systematic reviews of test accuracy studies should search a range of databases and that current diagnostic filters do not have sufficient accuracy to be used in test accuracy reviews. Summary quality scores should not be used in test accuracy reviews; the Quality Assessment of Studies of Diagnostic Accuracy included in Systematic Reviews (QUADAS) tool for assessing test accuracy studies is acceptable for quality assessment. We have shown that the hierarchical summary receiver operating characteristic (HSROC) and bivariate models for meta-analysis of test accuracy are statistically equivalent in many circumstances, and have developed an add-on module for the statistical software package Stata that enables these statistically rigorous models to be fitted by those without expert statistical knowledge. Three areas that would benefit from further research are literature searching, synthesis of results from individual patient data and presentation of results.

Abstract:
Objective We compared diagnostic accuracy of pleural fluid adenosine deaminase (ADA) and interferon-gamma (IFN-γ) in diagnosing tuberculous pleural effusion (TPE) through systematic review and comparative meta-analysis. Methods We queried PubMed and Embase databases to identify studies providing paired data for sensitivity and specificity of both pleural fluid ADA and IFN-γ for diagnosing TPE. We used hierarchical summary receiver operating characteristic (HSROC) plots and HSROC meta-regression to model individual and comparative diagnostic performance of the two tests. Results We retrieved 376 citations and included 45 datasets from 44 publications (4974 patients) in our review. Summary estimates for sensitivity and specificity for ADA were 0.88 (95% CI 0.85–0.91) and 0.91 (95% CI 0.89–0.92), while for IFN-γ they were 0.91 (95% CI 0.89–0.94) and 0.96 (95% CI 0.94–0.97), respectively. HSROC plots showed consistently greater diagnostic accuracy for IFN-γ over ADA across the entire range of observations. HSROC meta-regression using test-type as covariate yielded a relative diagnostic odds ratio of 2.22 (95% CI 1.68–2.94) in favour of IFN-γ, along with better summary sensitivity and specificity figures. No prespecified subgroup variable significantly influenced the summary diagnostic accuracy estimates. Conclusion Pleural fluid IFN-γ estimation has better diagnostic accuracy than ADA estimation for diagnosis of TPE.

Summary Objectives: Meta-analysis allows to summarize pooled sensitivities and specificities from several primary diagnostic test accuracy studies. Often these pooled estimates are indirectly obtained from a hierarchical summary receiver operating characteristics (HSROC) analysis. This article presents a generalized linear random-effects model with the new SAS PROC GLIMMIX that obtains the pooled estimates for sensitivity and specificity directly. Methods: Firstly, the formula of the bivariate random-effects model is presented in context with the literature. Then its implementation with the new SAS PROC GLIMMIX is empirically evaluated in comparison to the indirect HSROC approach, utilizing the published 2 x 2 count data of 50 meta-analyses. Results: According to the empirical evaluation the meta-analytic results from the bivariate GLIMMIX approach are nearly identical to the results from the indirect HSROC approach. Conclusions: A generalized linear mixed model with PROC GLIMMIX offers a straightforward method for bivariate random-effects meta-analysis of sensitivity and specificity.

Abstract:
Mismatch repair (MMR) is activated by evolutionarily conserved MutS homologs (MSH) and MutL homologs (MLH/PMS). MSH recognizes mismatched nucleotides and form extremely stable sliding clamps that may be bound by MLH/PMS to ultimately authorize strand-specific excision starting at a distant 3′- or 5′-DNA scission. The mechanical processes associated with a complete MMR reaction remain enigmatic. The purified human (Homo sapien or Hs) 5′-MMR excision reaction requires the HsMSH2–HsMSH6 heterodimer, the 5′ → 3′ exonuclease HsEXOI, and the single-stranded binding heterotrimer HsRPA. The HsMLH1–HsPMS2 heterodimer substantially influences 5′-MMR excision in cell extracts but is not required in the purified system. Using real-time single-molecule imaging, we show that HsRPA or Escherichia coli EcSSB restricts HsEXOI excision activity on nicked or gapped DNA. HsMSH2–HsMSH6 activates HsEXOI by overcoming HsRPA/EcSSB inhibition and exploits multiple dynamic sliding clamps to increase tract length. The transport management mode fundamentally determines the sustainable development of high-speed rail passenger transport (HSRPT), which was shocked by the COVID-19 pandemic in 2020. In order to study the sustainable development mechanism of HSRPT and the impact of COVID-19, primarily based on the data from the Beijing–Shanghai high-speed rail (HSR) taken from 2018, we adopt system dynamics (SD) to provide a scenario simulation method to examination sustainable operation status of HSRPT under the entrusted transportation management mode (ETMM) by VENSIM, and take into account the following two evaluation indicators: economic and operational. The results show the following: (1) Transportation demand and commissioned transportation management fees play a vital role in the sustainable operation of the Beijing–Shanghai HSR, causing significant changes in transportation revenue and transportation costs. (2) COVID-19 had a great impact on the sustainable operation of the Beijing–Shanghai HSR. In 2020, the turnover and transportation profit of the Beijing–Shanghai HSR fell by 74.31% and 49.19%, respectively. In 2022, the transportation profit can be restored to the level of 2019. The study results reveal that Beijing–Shanghai HSR under the ETMM has a good sustainable development capability.

Background: Olaparib maintenance therapy has shown efficacy and tolerability in patients with platinum-sensitive, high-grade serous recurrent ovarian cancer (HSROC) with BRCA1/2 mutation (BRCAm). Our aim was to present real-world experience with olaparib in Korea. Method: We included HSROC patients with BRCAm treated with olaparib maintenance at four institutions in Korea between 2016 and 2018. Medical records were reviewed for clinico-pathologic characteristics, objective response, survival outcomes, and safety. Results: One hundred HSROC patients with BRCAm were included. BRCA1 mutation was present in 71 patients (71.0%), and BRCA2 mutation was present in 23 patients (23.0%). In terms of the best objective response with olaparib maintenance in 53 patients with partial remission from most recent chemotherapy, complete remission occurred in 12 (22.6%) and partial remission in four (7.5%), while 33 patients (62.3%) had stable disease. The 24 month progression-free survival was 42.4%, and 24 month overall survival was 82.1%. Grade 3 or more adverse events were as follows: anemia in 14 patients (14.0%), neutropenia in seven patients (7.0%), thrombocytopenia in two patients (2.0%), oral mucositis in one patient (1.0%), and soft tissue infection in one patient (1.0%). Conclusions: The safety and effectiveness of olaparib maintenance treatment in a real-world study were consistent with those reported in previous clinical trials.

Hierarchical models such as the bivariate and hierarchical summary receiver operating characteristic (HSROC) models are recommended for meta-analysis of test accuracy studies. These models are challenging to fit when there are few studies and/or sparse data (for example zero cells in contingency tables due to studies reporting 100% sensitivity or specificity); the models may not converge, or give unreliable parameter estimates. Using simulation, we investigated the performance of seven hierarchical models incorporating increasing simplifications in scenarios designed to replicate realistic situations for meta-analysis of test accuracy studies. Performance of the models was assessed in terms of estimability (percentage of meta-analyses that successfully converged and percentage where the between study correlation was estimable), bias, mean square error and coverage of the 95% confidence intervals. Our results indicate that simpler hierarchical models are valid in situations with few studies or sparse data. For synthesis of sensitivity and specificity, univariate random effects logistic regression models are appropriate when a bivariate model cannot be fitted. Alternatively, an HSROC model that assumes a symmetric SROC curve (by excluding the shape parameter) can be used if the HSROC model is the chosen meta-analytic approach. In the absence of heterogeneity, fixed effect equivalent of the models can be applied.

The use of internet connections to develop Virtual Private Network (VPN) lines in companies has been massively applied. An example is the use of Dynamic Multipoint VPN (DMVPN) technology from Cisco, which can connect Spoke or branch offices through HUB or data centers. Besides requiring a secure network, companies also need high network availability. One of them is by using the redundancy method in order to minimize downtime when device damage occurs. In this research, the spoke router will apply two redundancy methods by Cisco, which are Hot Standby Router Protocol (HSRP) and Gateway Load Balancing (GLBP). This research aims to compare the QoS performances and downtime between the two protocols in order to discover the suitable method for DMVPN networks at branch offices. The test scenario is conducted on the GNS-3 simulator using the File Trasnfer Protocol (FTP) service. The obtained test result shows that the HSRP throughput value is smaller from the GLBP with a difference of 0.20%, the increase in the average delay of the HSRP is smaller from the GLBP with a difference of 1.07%. The HSRP data transfer time is faster than GLBP with a difference of 1.49%, and HSRP downtime is 4.13% faster than GLBP. The conclusion is that a suitable redundancy solution for spoke router using the HSRP method

Penelitian ini bertujuan mengimplementasikan dan mengevaluasi sistem First Hop Redundancy Protocol (FHRP) pada jaringan topologi star dengan menggunakan routing EIGRP. Hasil dari pengujian kinerja rata-rata delay menunjukkan selisih antara router master dan backup pada protokol Virtual Router Redundancy Protocol (VRRP) lebih baik yaitu hanya 0,003 detik, sedangkan pada protokol Gateway Load Balancing Protocol (GLBP) 0,076 detik dan protokol Hot Standby Router Protocol (HSRP) 0,075 detik. Pada pengujian rata-rata packet loss protokol GLBP menunjukkan selisih yang cukup besar antara router master dan backup yaitu 12,07 %, sedangkan pada protokol VRRP 0,07 % dan protokol HSRP 5,06 %. Namun pada hasil pengujian rata-rata throughtput protokol HSRP lebih stabil yaitu 0,60 Kbps pada router master dan 0,90 Kbps pada router backup, sedangkan protokol VRRP paling tidak stabil karena mengalami kenaikan yang signifikan yaitu 0,458 Kbps pada router master dan 1,11 Kbps pada router backup.Kata kunci: FHRP, VRRP, GLBP, HSRP, Routing, EIGRP, Protokol ABSTRACTThere are several important considerations in designing a network, including availability, minimizing failure, and handling high traffic data. The current research aims to implement and evaluate the First Hop Redundancy Protocol (FHRP) system on star topology networks using EIGRP routing. The result shows the average performance difference of the master and the backup router in Virtual Router Redundancy Protocol (VRRP) protocol is 0,003 second, while in Gateway Load Balancing Protocol (GLBP) protocol is 0,076 second, and Hot Standby Router Protocol (HSRP) protocol is 0,075 second. The test in GLBP protocol shows a significant difference in packet loss between master and backup router about 12,07%, while in VRRP protocol is 0,07%, and HSRP protocol is 5,06%. The average throughput in HSRP protocol is stable with 0,60 Kbps in the master router and 0,90 in the backup router, while in VRRP protocol is not stable with 0,458 Kbps in the master router and 1,11 Kbps in the backup router. Background In the last decades, several clinical scores have been developed and currently used to improve the diagnosis and risk management of patients with suspected acute appendicitis (AA). However, some of them exhibited different values of sensitivity and specificity. We conducted a systematic review and metanalysis of epidemiological studies, which compared RIPASA and Alvarado scores for the diagnosis of AA. Methods This systematic review was conducted using PubMed and Web of Science databases. Selected studies had to compare RIPASA and Alvarado scores on patients with suspected AA and reported diagnostic parameters. Summary estimates of sensitivity and specificity were calculated by the Hierarchical Summary Receiver Operating Curve (HSROC) using STATA 17 (STATA Corp, College Station, TX) and MetaDiSc (version 1.4) software. Results We included a total of 33 articles, reporting data from 35 studies. For the Alvarado score, the Hierarchical Summary Receiver Operating Curve (HSROC) model produced a summary sensitivity of 0.72 (95%CI = 0.66–0.77), and a summary specificity of 0.77 (95%CI = 0.70–0.82). For the RIPASA score, the HSROC model produced a summary sensitivity of 0.95 (95%CI = 0.92–0.97), and a summary specificity of 0.71 (95%CI = 0.60–0.80). Conclusion RIPASA score has higher sensitivity, but low specificity compared to Alvarado score. Since these scoring systems showed different sensitivity and specificity parameters, it is still necessary to develop novel scores for the risk assessment of patients with suspected AA.

Introduction. Traditionally it is believed that the therapeutic effect of the hexane extract of Serenoa repens (HSRE) in patients with benign prostatic hyperplasia is realized due to its effect on prostate cells. However, in the literature there is data about the possible HSRE effect not only on the prostate, but also directly on the bladder. This paper summarizes data and analyzes the possible cellular mechanisms of HSRE action on the bladder under the development of infravesical obstruction (bladder outlet obstruction – BOO). Materials and methods. Literature data from the PubMed and eLibrary.ru databases for a 25-years period related to the study of the pathogenesis of bladder dysfunction in the development of BOO, cellular and molecular mechanisms by which pathological and adaptive processes in bladder cells are realized, as well as the influence of HSRE on these processes are analyzed. Results. The concept of the pleiotropic effect of HSRE on various links in the pathogenesis of bladder dysfunction caused by BOO has been formed. The main of which are: the antioxidant and anti-inflammatory effects of HSRE components, as well as the regulatory effect on cell growth and proliferation factors and transcription factors. All this reduces the cytotoxic effect of oxidative stress on the cells of the bladder developing in IVO, contributing to a more active proliferation of detrusor smooth muscle cells. The consequence of the complex action is to maintain the contractile function of the bladder in a state of adaptation to increased stress, due to BOO. Conclusions. The present hypothesis about the direct effect of HSRE on the hypertrophied bladder and possible mechanisms of its therapeutic action is based on an understanding of the general biological mechanisms and allows us to explain better the results of previous clinical studies and to assess the prospects for expanding the scope of further drug application.

High strength steel reinforced concrete (HSRC) column refers to steel reinforced concrete column using high strength steel with its yield strength over 420MPa. So far, research on seismic behavior of HSRC columns is limited. This paper presents experimental and numerical studies on seismic behavior of HSRC columns. Two Q460 high strength steel reinforced concrete columns have been tested under low cyclic loading with constant axial compression ratio of 0.3. Flexural failure is observed in the test. From the hysteresis curves, the specimens exhibit good ductility and satisfactory energy dissipation capacity. Displacement ductility factors are larger than 2. When load descends to 85% of the peak load, the ultimate drift ratios of two specimens are 1/29 and 1/26 respectively, which meet the requirements of Chinese Seismic Design code (GB 50011). To study the seismic behavior of HSRC columns more comprehensively, a numerical model has been established for simulating the experiment using OpenSees, which adopts nonlinear beam-column element and fiber model. The numerical result fits the test data well, which validates the effectiveness of numerical model. Parametric study is then carried out to further investigate the seismic behavior of HSRC columns.

Penelitian ini dilakukan dengan menambah sebuah router sebagai router backup jika router utama terjadi kendala atau defice failure dengan metode HSRP (Hot Standby Router Protocol). Pada tahap pengujian menggunakan command ping, dan tracert dengan skenario failover HSRP yang sudah diimplementasikan. Dengan penerapan protokol redudansi HSRP diharapkan kendala dapat diatasi dengan baik dan dan menjadi bahan evaluasi pada jaringan tersebut.

Background: In advanced epithelial ovarian cancer (EOC), the LION trial restricted lymphadenectomy indication to patients with suspect lymph nodes before and during surgery. Preoperative imaging is used to assess lymph node status, and particularly CT and PET/CT. The aim of this systematic review and meta-analysis was to evaluate the diagnostic accuracy of preoperative CT and PET/CT to detect lymph node metastasis (LNM) in patients with EOC; Methods: Databases were searched from January 1990 to May 2019 for studies that evaluated the diagnostic accuracy of preoperative CT and PET/CT to detect LNM in patients with EOC with histology as the gold standard. Pooled diagnostic accuracy was calculated using bivariate random-effects models and hierarchical summary receiver operating curve (HSROC). This study is registered with PROSPERO number CRD42020179214; Results: A total of five studies were included in the meta-analysis: four articles concerned preoperative CT and four articles concerned preoperative PET/CT, involving 106 and 138 patients, respectively. For preoperative CT, pooled sensitivity was 0.47 95% CI [0.20–0.76], pooled specificity was 0.99 95% CI [0.75–1.00] and area under the curve (AUC) of the HSROC was 0.91 95% CI [0.88–0.93]. For preoperative PET/CT, pooled sensitivity was 0.81 95% CI [0.61–0.92], pooled specificity was 0.96 95% CI [0.91–0.99] and AUC of the HSROC was 0.97 95% CI [0.95–0.98]; Conclusions: PET/CT has a very high diagnostic accuracy, especially for specificity, to detect LNM in EOC and should be realized systematically, additionally to CT recommended to evaluate peritoneal spread, in the preoperative staging of patients with an advanced disease.

Human cytomegalovirus (HCMV) DNA-binding protein pUL56 is thought to be involved in the cleavage/packaging process of viral DNA and therefore needs to be transported into the nucleus. By using indirect immunofluorescence analysis, HCMV pUL56 (p130) was found to be localized predominantly in the nucleus of infected cells. Solitary expression of wild-type as well as epitope-tagged pUL56 also resulted in nuclear distribution after transfection, suggesting the presence of an endogenous nuclear localization signal (NLS). Deletion of a carboxy-terminal stretch of basic amino acids (aa 816–827) prevented nuclear translocation, indicating that the sequence RRVRATRKRPRR of HCMV pUL56 mediates nuclear targetting. The signal character of the NLS sequence was demonstrated by successful transfer of the NLS to a reporter protein chimera. Furthermore, sequential substitutions of pairs of amino acids by alanine in the context of the reporter protein as well as substitutions within the full-length pUL56 sequence indicated that residues at positions 7 and 8 of the NLS (R and K at positions 822 and 823 of pUL56) were essential for nuclear translocation. In order to identify the transport machinery involved, the potential of pUL56 to bind importin α (hSRP1α) was examined. Clear evidence of a direct interaction of a carboxy-terminal portion as well as the NLS of pUL56 with hSRP1α was provided by in vitro binding assays. In view of these findings, it is suggested that nuclear translocation of HCMV pUL56 is mediated by the importin-dependent pathway.

KCIC Phase I Training Program will accommodate 500 Participants from the total HR needs of PT KCIC from 1,452 people. The focus is on increasing competence on the Operation System, Passenger Service, and Maintenance divided into 21 Training Posts, most of the participants came from Fresh Graduates who entered the Entry Level. Required Competence Levels ability to apply knowledge and skill High Speed Railway Technology. technical executive level for High Speed Railway operation and maintenance for Electric Multiple Unit (EMU) KCIC400AF Type from China. Gaps participant competence between existing competence levels and required competence levels calculate 100%, thus the training program will be carried out as a whole in accordance with the training curriculum of the HSRCC. The training program consists of 4 parts consisting of (1) Indonesian regulation, instructors from Indonesian Railway Polytechnic; (2) Professional Knowledge, instructor from Tianjin Railway Technical and Vocational College and Southwest Jiaotong University; (3) Safety regulation, instructor from China Railway – Beijing; and (4) Practical skills instructor from China Railway – Beijing. The delivery method uses the online class method which will be accompanied by interpreters and assistant lecturers in each class. With the language interpretation stage, a multiplier coefficient of 2.5 is obtained from normal lesson hours in the HSRCC curriculum if it is implemented in Indonesian participants. The duration of the training implementation about 1 – 8.5 Month. The next competency requirement is High Speed Railway trainer from Indonesia. With the stipulation, which was conveyed by the HSRCC, that participants who have taken the training phase I and received an evaluation score of training and competency certification above the average of other participants, are able to take part in the Training of Trainers for High Speed Railway and pass the Competency Test of Trainer Candidates.

For expedite the development of high titanium heavy slag concrete, eight high titanium heavy slag high strength reinforced concrete (HTHS-HSRC) scale model column are studied. The eight HTHS-HSRC model columns are tested under reversed horizontal force. Primary experimental parameters include axial load ratio varying from 0.3 to 0.5, volumetric ratios of transverse reinforcement ranging from 1.38% to 1.56%, strength of high titanium heavy slag high strength concrete varying from 55.9 to 61.6 N/mm2 and configurations of transverse reinforcement. It is found from the test result that HTHS-HSRC model columns provides comparable seismic performance to those usually used reinforced concrete column in terms of member ductility, hysteretic and energy dissipation capacity. Primary Factors of Displacement Ductility of Model Columns are also discussed.

With the increase of internet user today, if it only has one router, when the router experiencing a network failure, then the internet cannot be used. To overcome this problem, the HSRP (Hot Standby Router Protocol) method by creating one active link and one backup link has been applied in the previous research. In this research will be applying the HSRP, VTP (Vlan Trunking Protocol), InterVLAN, and VLAN (Virtual Local Area Network) as a comparison. The testing parameter in this research will be using packet loss and throughput. From the result of testing that has been done, it shows that by applying the HSRP method, it gives a network availability with the packet loss of only 0,32%, and throughput of 1,159Mb/s (95,58%) from using 100 PC.

O tratamento da hiperplasia supra-renal congênita por deficiência da 21-hidroxilase (HSRC-21OH) tem como objetivos repor glico e mineralocorticóides, evitar a virilização dos genitais externos, prevenir a desidratação por perda de sal, controlar o hiperandrogenismo sem afetar a velocidade de crescimento, preservar a função gonadal, fertilidade e estatura final. Relatamos a nossa experiência no acompanhamento de 96 pacientes com HSRC-21OH. Nas crianças utilizamos como glicocorticóide o acetato de cortisona (18-20 mg/m²/dia) e nos adultos a dexametasona (0,25-0,75 mg/dia). Quando necessário, a reposição do mineralocorticóide foi feita com 9alfa-fluor-hidrocortisona 50-250 µg/dia dependendo da faixa etária. Apesar da substituição adequada com glico e/ou mineralocorticóides o resultado final do tratamento da HSRC-21OH ainda deixa a desejar, principalmente em relação ao crescimento, já que a estatura final na maior parte das casuísticas está entre -1 e -2 DP em relação a estatura alvo. Novas terapêuticas da HSRC-21OH, como a associação de hidrocortisona, fludrocortisona, flutamida e testolactona ou a suprarrenalectomia cirúrgica ainda estão em fase experimental e a evolução a longo prazo é necessária para avaliar seu real efeito.

Realtime (streaming) data communication has become a major requirement for companies and government institutions. Communication that occurs is not only limited to one particular local area but covers other areas so as to form a broad network (WAN). In building network infrastructure, one of the most important things is how networks can deal with failure (failure) on a computer network consists of failure of devices (devices) that affect Quality of Services (QoS). In every internet network design process, a redundant line is always added to complete the main line. So that if the main line is disrupted, data traffic can be transferred to the backup path, this process is called network redundancy. So that if one gateway dies, another gateway will immediately replace the dead gateway. There are two protocols included in the FHRP, namely Hot Standby Routing Protocol (HSRP), and Gateway Load Balancing Protocol (GLBP), these two protocols that can overcome this problem. So from that this study will discuss which protocol between the two protocols has the best performance in overcoming the problem. Methods of data collection with literature studies and literature studies, simulations carried out with 8 stages (problem formulation, conceptual models, input & output data, modeling, simulation, verification & validation, experimentation, and output analysis). The results of this study provide the best QoS value for the delay value is the HSRP ROUTER protocol, and for the value of packet loss is HSRP, and for throughput value is HSRP and the routing protocol that is appropriate for the network for video streaming services is HSRP

Hindlimb suspension (HS) results in rapid losses of muscle mass, which may in part be explained by attenuated rates of protein synthesis. Mammalian target of rapamycin (mTOR) regulates protein synthesis and has been implicated as a potential mediator of the muscle mass decrement with HS. This study examined the effect of resistance exercise, a muscle hypertrophy stimulant, on rates of protein synthesis after 4 days of HS in mature male Sprague-Dawley rats. Flywheel resistance exercise (2 sets × 25 repetitions) was conducted on days 2 and 4 of HS (HSRE). Sixteen hours after the last exercise bout, soleus muscles were assessed for in vitro rates of protein synthesis, with and without insulin (signaling agonist) and/or rapamycin (mTOR inhibitor). Results demonstrated that soleus mass was reduced ( P < 0.05) with HS, but this loss of mass was not observed ( P > 0.05) with HSRE. Muscle protein synthesis was diminished ( P < 0.05) with HS, with or without insulin. HSRE also had reduced rates of synthesis without insulin; however, insulin administration yielded higher ( P < 0.05) rates in HSRE compared with HS or control. Rapamycin diminished protein synthesis in all groups ( P < 0.05), but insulin rescued synthesis rates in HS and HSRE to levels similar to insulin alone for each group, suggesting that alternate signaling pathways develop to increase protein synthesis with HS. These results demonstrate that the capacity for an augmented anabolic response to resistance exercise is maintained after 4 days of HS and is independent of a rapamycin-sensitive pathway.

In this paper, the author gives an overview of its records, recently uncovered in the institution’s building in Pretoria. To academics, policy makers, and all those interested in South Africa’s intellectual and institutional history, these records are important in seeking to understanding the HSRC itself and other apartheid institutions. In addition, exploration of its history can, amongst other things, help to shape policy in liberated South Africa towards higher educational and research institutions in the light of their historical legacy of apartheid and segregation. The author cautions that, as researchers embark upon exploring the history of the HSRC, they should avoid romanticizing it but rather confront its nuances and challenges head on.

Untuk mengatasi permasalahan kegagalan jaringan tersebut dalam penelitian ini akan dibahas tiga metode yaitu VRRP, HSRP dan GLBP diharapkan dengan penggunaan tiga metode tersebut bisa memberi solusi untuk mengatasi kegagalan jaringan demi lancarnya aktifitas bisnis dan komunikasi. Dari hasil pengujian yang telah dilakukan secara keseluruhan metode GLBP terlihat lebih baik jika dibandingkan dengan metode VRRP dan HSRP, terutama pada pengujian packet loss dan throughput. Having three orientations, the Health System Reform Plan (HSRP) has been administered to financially support and protect people, provide fair health services and promote the quality of health services. To be unsatisfied about health and medical services brings undesirable outcomes. The present investigation aimed to study the effect of Health System Reform Plan (HSRP) on the satisfaction level among medical doctors and nurses of Tabriz state hospitals, East Azerbaijan province, Iran in a period from 21th February; 2015 to 22th June; 2015. Statistical population included those patients who referred to Tabriz state hospitals. We used stratified sampling method. To collect data we used questionnaire being presented to the samples after assessing its validity and reliability. We also utilized descriptive and inferential statistics in a way that we used descriptive method to classify, summarize and interpret of obtained data. Then after demonstrating the abnormality of data by Kolmogorov-Smirnov test we used linear and multiple regressions to test research hypothesis and effect from the perspective of inferential method. Obtained results confirmed the research hypothesis and explained that the administration of HSRP affected medical doctors’ level of satisfaction. Also, according to the results of multiple regression tests, doctors and nurses’ satisfaction level was placed at the top of most affected issues from HSRP.

Human surface respiratory epithelial (HSRE) cells from nasal polyps have been cultured within collagen lattices in a serum-free defined medium. Cell growth observed over a period of 12 days showed a population doubling time of 36 h. Under these culture conditions, we observed a contraction of the lattices. Phase-contrast light microscopy and transmission electron microscopy demonstrated that the HSRE cells formed tubular ductlike structures. Lumens formed by HSRE cells were surrounded by cuboidal-shaped polarized cells with numerous ciliated cells, secretory cells, and undifferentiated cells. Epidermal growth factor (EGF) was observed to stimulate the tubule formation and the contraction of the lattices. Videomicroscopic observations and analysis of the ciliary beating frequency (CBF) demonstrated that the cilia were homogeneously distributed on the whole apical surface of the ciliated cells and that their movement was well coordinated, with a CBF similar to that observed in outgrowth cells from cultured human nasal and tracheal epithelia. Immunofluorescent staining of basement membrane components synthesized and secreted by cells revealed the presence of type III collagen around the tubules. Type IV collagen and laminin were present in the cytoplasm and at the periphery of the cells. The biotin-streptavidin-gold immunocytochemical technique with monoclonal anti-mucin antibody showed intracellular localization of mucins in secretory granules of the secretory cells. With the use of substrate gel electrophoresis polyacrylamide gels impregnated with gelatin, collagenase activity was detected in the conditioned medium of the cultured HSRE cells. These results suggest that both three-dimensional collagen gel and soluble factors such as EGF regulate tubule formation by HSRE cells. Moreover, the capacity of the epithelial cells to contract the gel suggests they may be involved in the wound healing process.
