Abstract:
Purpose This paper aims to present a panel data econometric model of the main determinants of house prices in the ten largest census metropolitan areas (CMA) in Ontario, Canada, for the years 2001, 2006 and 2011. The impact of immigration on the housing market in Canada is little researched; however, immigration plays an important role into the economy of Canada. According to Statistics Canada, not only is immigration key to Canada’s population growth but also without immigration, in the next 20 years, Canada’s population growth will be zero. The motivation for this study is the bursting of housing bubbles in some developed countries (e.g. USA). The authors analyze variables that are related to the immigration policy in Canada, accounting also for the impact of the interest rate, income, unemployment, household size and housing supply to analyze housing price determinants. The study investigates the magnitude of the impact of the top three leading categories of immigrants to Canada, namely, Chinese, Indian and Filipino, on the housing prices in Ontario’s largest cities. The results show the main factors that explain home prices over time that are interest rate, immigration, unemployment rate, household size and income. Over the 10-year period from 2001 to 2011, immigration grew by 400 per cent in Toronto CMA, the largest receiving area in Ontario, while the nonimmigrant population grew by 14 per cent. For Toronto CMA, immigrants, income, unemployment rate and interest rate explain the CA$158,875 average home price increase over the 2001-2011 time period. Out of this, the three categories of immigrants’ share of total home price increase is 54.57 per cent, with the corresponding interest rate share 58.60 per cent and income share 11.32 per cent of the total price growth. Unemployment rate contributes negatively to the housing price and its share of the total price increase is 24.49 per cent. Design/methodology/approach The framework for the empirical analysis applies the hedonic pricing model theory to housing sales prices for the ten largest CMAs in Ontario over the years 2001-2011. Following Akbari and Aydede (2012) and O’Meara (2015), market clearing in the housing market results in the housing price as a function of several housing attributes. The authors selected the housing attributes based on data availability for the Canadian Census years of 2001, 2006 and 2011 and the variables that have been most used in the literature. The model has the average housing prices as the dependent variable, and the independent variables are: immigrants per dwelling (Chinese, Indian, and Filipino), unemployment rate, average employment income, household size, housing supply and the interest rate. To capture the relative scarcity of dwellings, the independent variable immigrants per dwelling was used. Findings This study seems to suggest that one cause of high prices in Ontario is large inflows of immigrants together with low mortgage interest rate. The authors focused their attention on Toronto CMA, as it is the main destination of immigrants and comprises the largest cities, including Toronto, Mississauga, Brampton and Oakville. Looking over the 10-year period from 2001 to 2011, the authors can see the factors that impact the home prices in Toronto CMA: immigration, unemployment rate, household size, interest rate and income. Over the period of 10 years from 2001 to 2011, immigrants’ group from China, India and the Philippines account for CA$86,701 increase in the home price (54.57 per cent share of the total increase). Income accounts for CA$17,986 increase in the home price (11.32 per cent share); interest rate accounts for CA$93,103 of the average home price increase in Toronto CMA (58.60 per cent share); and unemployment rate accounts for CA$38,916 decrease in the Toronto average home prices (24.49 per cent share). Household size remain stable over time in Toronto (2.8 average household size) and does not have a contribution to home price change. All these four factors, interest rate, immigrants, unemployment rate and income, together explain CA$158,875 increase in home prices in Toronto CMA between 2001 and 2011. Practical implications The housing market price analysis may be more complex, and there may be factors impacting the housing prices extending beyond immigration, interest rate, income and household size. Finally, the results of this paper can be extended to include the most recent census data for the year 2016 to reflect more accurately the price situation in the housing market for Ontario cities. Social implications The fact that currently, in 2017, the young working population cannot afford buying a property in the Toronto CMA area means there is a problem with this market and a corresponding decrease in the quality of life. According to The Globe and Mail (July 2017), a new pool in 2017 suggested that two in five Canadians believe housing in this country is not affordable for them. Further, 38 per cent of respondents who consider themselves middle or upper class believe in no affordability of housing. The Trudeau Government promised Canadians a national housing strategy for affordable housing. Designing a national housing strategy may be challenging because it has to account for the differential income ranges across regions. Municipal leaders are asking the government to prioritize repair and construct new affordable housing. Another reason discussed in the media of the unaffordability of housing in Toronto and Vancouver is foreign buyers. The Canadian Government recently implemented a tax measure on what it may seem the housing bubble problem: foreign buyers. Following Vancouver, in April 2017, Ontario Government imposed a 15 per cent tax on foreign buyers who are not Canadian citizens or permanent residents. This tax is levied on houses purchased in the area stretching from Niagara Region and Greater Toronto to Peterborough. Originality/value Few studies use Canadian data to explain house prices and analyze the effect of immigration on housing prices. There is not much research on the effect of the immigrants and immigrants’ ethnicity (e.g., Chinese, Indian and Filipino immigrants), on the housing prices in Canada cities. This study investigates the impact of the most prevalent immigrant races (e.g., from China, India and the Philippines) on housing prices, using data for Canadian major cities in Ontario within a panel data econometric framework. This paper fills this gap and contributes to the literature, which analyzes the determinants of housing prices based on a panel of cities in the Canadian province of Ontario.

Buildings in Canada account for a significant amount of greenhouse gas (GHG) emissions and net zero energy building technology has been identified as part of the solution. This study presents a conceptual model identifying barriers to the adoption of net zero energy housing and tests it by administering a survey to 271 participants in a net zero energy housing demonstration project in Toronto, Canada. Using multivariate correlation and multi-linear regression analyses this study finds that of all the innovation adoption variables it was the construction and design quality that was the most significant contributor to the adoption of a net zero energy home by a potential home owner. This study found that the (a) extra cost compared to a conventional home, b) lack of knowledge about the technology associated with a net zero energy home or (c) not knowing someone who owned a net zero energy home were not significant barriers to accepting net zero energy homes. Our results suggest that policy-makers should promote the diffusion of net zero energy home technology by encouraging housing developers to include net zero energy homes in their collection of model homes, with an emphasis on quality design and construction. Furthermore, engaging in trust building initiatives such as education and knowledge about the technology, its related energy cost savings, and the environmental benefits would contribute to a greater acceptance of net zero energy homes.

Abstract:
Objective: The objective of this article was to conduct a cost analysis comparing the costs of a supportive housing intervention to inpatient care for clients with severe mental illness who were designated alternative-level care while inpatient at the Centre for Addiction and Mental Health in Toronto. The intervention, called the High Support Housing Initiative, was implemented in 2013 through a collaboration between 15 agencies in the Toronto area. Method: The perspective of this cost analysis was that of the Ontario Ministry of Health and Long-Term Care. We compared the cost of inpatient mental health care to high-support housing. Cost data were derived from a variety of sources, including health administrative data, expenditures reported by housing providers, and document analysis. Results: The High Support Housing Initiative was cost saving relative to inpatient care. The average cost savings per diem were between $140 and $160. This amounts to an annual cost savings of approximately $51,000 to $58,000. When tested through sensitivity analysis, the intervention remained cost saving in most scenarios; however, the result was highly sensitive to health system costs for clients of the High Support Housing Initiative program. Conclusions: This study suggests the High Support Housing Initiative is potentially cost saving relative to inpatient hospitalization at the Centre for Addiction and Mental Health.

Purpose Research has shown that youth experiencing homelessness (YEH) face barriers to social inclusion and are at risk for poor mental health. With the COVID-19 pandemic threatening the health, wellbeing, and economic circumstances of people around the world, this study aims to assess the impacts of the pandemic on YEH in Toronto, Ontario, as well as to identify recommendations for future waves of COVID-19. Methods Semi-structured interviews were conducted with YEH (ages 16–24, n = 45) and staff who work in one of four downtown emergency shelters for youth (n = 31) in Toronto, Ontario. Results YEH experienced both structural changes and psychosocial impacts resulting from the pandemic. Structural changes included a reduction in services, barriers to employment and housing, and changes to routines. Psychosocial outcomes included isolation, worsened mental health, and increased substance use. Impacts were magnified and distinct for subpopulations of youth, including for youth that identified as Black, 2SLGBTQ+, or those new to Canada. Conclusions The COVID-19 pandemic increased distress among YEH while also limiting access to services. There is therefore a need to balance health and safety with continued access to in-person services, and to shift the response to youth homelessness to focus on prevention, housing, and equitable supports for subpopulations of youth.

While housing has been a central object of financialisation, questions regarding how multi-scalar states shape the financialisation of housing remain under-researched. I address this knowledge gap through a case study of the financialisation of affordable housing in Toronto. By analysing pertinent policy documents, I examine the roles and relationship of the federal, provincial and local states in the financialisation of affordable housing. Two findings are highlighted. (1) Although policies from all levels of government show traits of financialisation – in terms of both the connection between social policy and financial markets, and financialised ideologies prevailing in policy discourses, the extent and pattern of the manifestation of financialisation are distinct. This research thus calls for a nuanced understanding of the state’s role in the financialisation of housing from a multi-scalar perspective. (2) Affordable housing policies usually do not give an explicit definition of ‘affordable’. By scrutinising the policy specifications, I found that the target group is mainly moderate-income, rather than low-income, households. It will be increasingly difficult for low-income households to meet their housing needs.

Abstract:
This study examines the Single Room Occupancy Housing (SRO) stock of the City of Toronto and of the City of Vancouver. The term SROs refers to residential hotels and rooming houses. Rooming houses located in converted single family dwellings constitute the primary SRO form of Toronto. Residential hotels constitute the primary SRO form in Vancouver. This study examines the historical evolution of the SRO stock, the characteristics of the units, the socioeconomic characteristics of the residents, and the provincial and municipal policy relating to the SRO stock. SROs were the first form of accommodation for many immigrants and transient male workers. SRO units in rooming houses also housed couples and families during the first decades of the 1900's in both Toronto and Vancouver. A dire shortage of affordable rental housing forced families to live in overcrowded conditions in single rooms. SROs today house primarily two three of population. Those who live in single rooms permanently, those who live in single rooms because they cannot afford to rent an apartment, and those who live in single rooms temporarily. Contrary to what has been commonly assumed, residents of SROs are not transient. SRO residents, although they might move frequently, they do so because they continually face displacement. Evictions are common because of real estate market pressures. Many SRO units are being converted to other residential uses or demolished. The main group of SRO residents still consists of single older men, however, the percentage of women and of young men has increased among the SRO residents in recent years, especially in the case of the rooming houses of Toronto. The majority of SRO residents live on incomes which are well below of the poverty line (approximately, 50% of the poverty line). These residents pay 50% to 75% of their income on housing. SRO housing is an important component of the rental housing market of Vancouver and Toronto. SROs constitute the last housing resort before homelessness. However, with the exception of SRO units in social housing projects, SROs existing today in Toronto and Vancouver do not constitute an adequate form of accommodation. In most cases, the physical condition of the units is substandard and the rents are still very high for the average SRO resident. In Vancouver, SRO units are not fully recognized as part of the rental housing stock because they are not protected by provincial landlord and tenant regulation. The continued availability of SRO accommodation looks more optimistic in Ontario than it does in British Columbia. The main focuss of the housing policy of Ontario and Toronto towards the SRO stock has been to rehabilitate, to improve and to expand the SRO stock. In addition, Ontario has recently drafted legislation which protects the rental housing stock from demolition and conversion and it has extended security of tenure rights to the residents of rooming houses. On the other hand, the main thrust of the housing policy of the province of British Columbia and of the City of Vancouver towards the SRO stock has been to relocate SRO tenants in social housing units

Abstract:
Stormwater is the component of runoff that is generated by human activities, and has gradually become a key issue in achieving sustainability in urban environments. When vegetation and soils are replaced with roads and buildings, less rainwater infiltrates into the ground or is taken up by vegetation, and more becomes surface runoff. A greater area of impervious surfaces leads to increased stormwater runoff volume and velocity, and consequently increases the risk off looding and erosion. Being able to reduce stream flows and pollution of surface flows, green roofs are one technology that may help in alleviating this storm water crisis. This thesis developed a different and effective methodology for quantifying the effects of green roofs on stormwater runoff and calculating the runoff volume and rate for residential housing communities before and after applying green roofs. The method utilizes local climate data like rainfall and evapotranspiration rate, the water use properties of vegetation like crop coefficients of plants, and the areas of impervious surfaces; and then compares the different effects of green roofs in different locations having disparate climatic conditions. It was found that the best way to achieve zero runoff was to green a portion of the total rooftop area and disconnect all impervious surfaces. Implications of this methodology on city planning and site design and for future research are then discussed.

While environmental and economic sustainability have been driving factors in the movement towards a more resilient built environment, social sustainability is a factor that has received significantly less attention over the years. Federal support for low-income housing has fallen drastically, and the deficit of available, adequate, affordable homes continues to grow. In this thesis, I explore one way that architects can design affordable housing that is intrinsically sustainable. In the past, subsidized low-income housing has been built as if to provide a short-term solution—as if poverty and lack of affordable housing is a short-term problem. However, I argue that adaptable architecture is essential for the design of affordable housing that is environmentally, economically, and socially sustainable. Further, architects must balance affordability, durability, and adaptability to design sustainable solutions that are resistant to obsolescence. I conclude by applying principles and processes of adaptability in the design of Apto Ontario, an adaptable affordable housing development in the low-income historic downtown of Ontario, California (Greater Los Angeles). Along a new Bus Rapid Transit corridor, Apto Ontario would create a diverse, resilient, socially sustainable community in an area threatened by the rise of housing costs.

Abstract:
This thesis draws on David Harvey’s concept of “accumulation by dispossession” and an international political economy (IPE) approach centred on the institutional arrangements and power structures that privilege certain actors and values, in order to critique current capitalist practices of primitive accumulation by the global corporate extractive industry. The thesis examines how accumulation by dispossession by the global extractive industry is facilitated by the “free entry” or “free mining” principle. It does so by focusing on Canada as a leader in the global extractive industry and the spread of this country’s mining laws to other countries – in other words, the transnationalisation of norms in the global extractive industry – so as to maintain a consistent and familiar operating environment for Canadian extractive companies. The transnationalisation of norms is further promoted by key international institutions such as the World Bank, which is also the world’s largest development lender and also plays a key role in shaping the regulations that govern natural resource extraction. The thesis briefly investigates some Canadian examples of resource extraction projects, in order to demonstrate the weaknesses of Canadian mining laws, particularly the lack of protection of landowners’ rights under the free entry system and the subsequent need for “free, prior and informed consent” (FPIC). The thesis also considers some of the challenges to the adoption and implementation of the right to FPIC. These challenges include embedded institutional structures like the free entry mining system, international political economy (IPE) as shaped by international institutions and powerful corporations, as well as concerns regarding ‘local’ power structures or the legitimacy of representatives of communities affected by extractive projects. The thesis concludes that in order for Canada to be truly recognized as a leader in the global extractive industry, it must establish legal norms domestically to ensure that Canadian mining companies and residents can be held accountable when there is evidence of environmental and/or human rights violations associated with the activities of Canadian mining companies abroad. The thesis also concludes that Canada needs to address underlying structural issues such as the free entry mining system and implement FPIC, in order to curb “accumulation by dispossession” by the extractive industry, both domestically and abroad.

Ontario’s current regulatory approach to low-income housing lies between two primary challenges: the human right to housing, and political/fiscal constraints. This thesis draws on legal theory and economic analysis of law to articulate the proper goals of housing access policy. A structural theory is proposed to explain the normative relationship between efficiency, communitarianism and justice in housing. An array of regulatory options are compared and considered in light of the features that characterize Ontario’s low-income rental housing markets. This analysis favours demand-side housing subsidies to low-income households, combined with supply-side tax expenditures to improve elasticity in the low-income rental market. Further reform of rent and covenant controls, social and affordable housing supply, and land use planning is recommended to ensure an efficient residential tenancy market. These reforms are offered as a framework for the implementation of the human right to housing in Ontario.

This thesis explores the recognition of institutional racism in Ontario. It examines discourses of institutional racism between the late 1960s and early 1980s and argues that the recognition of institutional racism at the provincial and national levels was facilitated by overt acts of racism in one of Canada’s most populous and diverse cities – Toronto. The targets of overt racism were new immigrants from decolonizing nations who utilized the discourse of rights in the context of an increase in racist incidents to press for state recognition of institutional racism. This rise in racially motivated violence concerned most Canadians as it went against Canadians’ self-perception as a raceless, tolerant and peaceful society. The recognition of structural racism was a gradual and contested process as municipal, provincial and federal government actors often denied its existence and deemed overt acts of racism aberrant. When racist acts did occur, state officials and media reports blamed the increased racial tensions on the personal prejudice of extremists. Activist groups composed of visible minorities and human rights activists were key in the formation of a counter-narrative that challenged this persistent denial of structural racism. These groups played a fundamental role in redefining the nature of racism in Canadian society. A central theme of this dissertation is that disintegrating race relations allowed for a redefinition of the Canadian state. It was the increase in racist incidents in 1970s Toronto that fostered a broad discussion on racism in Canada. This discussion emphasized that Canada’s people of colour experienced second-class citizenship because of structural inequalities which were rooted in Canadian institutions. Racial violence in 1970s Toronto was crucial in the recognition of institutional racism as racist incidents brought visible minorities into the public sphere and gave them an opportunity to identify the existence of systemic and institutional racism in Canadian society. However, the recognition of institutional and systemic racism did not result in a deep transformation of the Canadian racial state as policy changes have not been successful in challenging structural inequality.

Appropriate housing with security of tenure is an important factor in the immigrant settlement and integration process. However, many studies of immigrant settlement and the housing careers of immigrants do so within the borders of a nation-state without reference to transnationalism – immigrants’ ties and cross-border connections with the country of origin. This case study of the transnational ties and housing careers of Ghanaian immigrants in Toronto aims to increase our understanding of one recent immigrant group’s settlement and integration process in Canada. Using a mixed-method approach involving both quantitative and qualitative data collection methods, this study explores how transnational housing activities influence the housing careers of Ghanaians in Toronto. The findings include insights into the immigration history and the socio-demographic characteristics of Ghanaians in Toronto; the nature and extent of transnational ties between Ghana and Canada; the nature of housing careers among Ghanaians in Toronto; and the influence of transnationalism on housing careers of Ghanaians in Toronto. Although Ghanaians’ immigration to Canada dates from the late 1950s, Ghanaians started coming to Canada in noticeable numbers after the 1960s. Ghanaian immigration to Canada generally and to Toronto particularly surged in the 1980s and beyond. Deteriorating economic and political conditions in Ghana and relatively favourable immigration policies and a good economic climate in Canada were the driving forces behind Ghanaian migration to Canada. However, the Ghanaian settlement process in Toronto does not culminate in a complete break with the homeland. Rather, Ghanaians in Toronto have engaged in a range of transnational activities with the country of origin, including contacts with family and friends, travelling to or visiting Ghana, following Ghanaian politics, investing in housing or property in Ghana, running businesses in Ghana, attending funerals in Ghana, and making regular remittances to Ghana. With respect to Ghanaians’ housing careers, the study reveals that during their initial settlement period, most Ghanaians lived in public subsidized rental housing or poor-quality private rental housing. They considered their housing conditions as inadequate and unsuitable and were not satisfied with their neighbourhood’s safety and security. At the time of the survey, however, respondents were more likely to own homes and were more likely to feel safe and secure in their neighbourhoods. However, housing affordability remains a major problem for Ghanaians in Toronto. With respect to the influence of transnationalism on housing careers of Ghanaians in Toronto, the study finds that transnational housing activities, especially Ghanaians’ attitudes to and preference for investing in housing in Ghana, affect their housing careers in Toronto. Sending regular remittances to Ghana and investing in housing in the homeland involve mobilizing huge financial resources from Toronto to achieving their housing needs in the country of origin, while many Ghanaians struggle to meet their own needs in Toronto. A logistic regression analysis shows that personal income and strong ties with Ghana are statistically significant predictors of investing in housing in Ghana. At the same time, significant predictors of Ghanaians’ propensity to own a house in Canada include loyalty to Canada and household income. The study contributes conceptually and empirically to three areas of research – transnationalism, housing careers, and immigrant settlement and integration – which hitherto have been studied as separate themes. Conceptually, it breaks away from the traditional way of researching immigrant settlement and housing careers by introducing a new conceptual dimension, transnationalism. Further, this research has added new insights about a recently arrived immigrant group in Toronto. Finally, the study contributes to the social work literature by identifying an emerging field of international social work. It has drawn attention to the fact that in the era of transnationalism, the emergence of a population of migrants whose needs and lives transcend national borders will affect the future of social work research and practice.

This thesis analyzes the impact of the political decision to broaden the scope of the Government of Canada's 3-year National Homelessness Initiative (Human Resource Development Canada, NHI, 2002) from an urban focus to one that includes smaller communities. This change provided the opportunity to study the phenomenon of homelessness and how rural responses are formed. This author postulates that this focus of attention on an almost invisible phenomenon—rural homelessness—and the accompanying community planning processes funded by the Supportive Community Partnership Initiative (SCPI) will impact local social policy development. A multi-dimensional analytical approach was adopted and considered three components: first, a policy review, a broad look at the policy agenda framework in Canada; then, a case study to illustrate implementation issues related to the National Homelessness Initiative; and, finally, a reflection on current practice in order to realize a holistic critique of public policy. The influence of socio-economic, political, and cultural factors on local planning and capacity building will be highlighted. Various models of governance were adopted across the country and guided the collaborative processes. This thesis presents an in-depth look at the community action plans and activities of the Simcoe County Alliance to End Homelessness (SCATEH) in both the rural and urban settings of Simcoe County. The processes adopted, capacity building components identified, and outcomes over the 7 years covered by the SCPI agreement are examined. The limitations of using participatory local action planning to respond to complex issues such as homelessness are detailed along with a modified community-based policy development model recommended as a learning tool to be used by those volunteers acting as agents of change. It is widely recognized that safe, affordable social housing is a fundamental need, and one that is extremely difficult to meet. The contribution this research makes is to reveal how effective government-community partnerships can be in a rural setting.

AbstractThis chapter documents and theorizes community-based film festival programming and organizing tactics during the first wave of the Covid-19 pandemic between March and September 2020. Drawing from autoethnographic experience and personal conversations with organizers of the Toronto Queer Film Festival (TQFF) and Toronto Outdoor Picture Show (TOPS), I offer the neologism “cinegoraphilia”—a portmanteau of cinephilia and the Ancient Greek agora—to theorize both festival’s strategies to capture the love of watching cinema in public together. As a theory that centers the communal, public features of cinemagoing, cinegoraphilia orients us toward the ways community-based film festivals creatively tried to retain and engage their audiences. I situate TOPS’s and TQFF’s efforts to capture cinegoraphilia within the broader context of public health restrictions in the Canadian province of Ontario, as well as changes to the arts funding ecosystem and funding policy locally in Toronto, provincially in Ontario, and nationally across Canada.

Key Message Ontario long-term care (LTC) home residents have experienced disproportionately high morbidity and mortality, both from COVID-19 and from the conditions associated with the COVID-19 pandemic. There are several measures that could be effective in preventing COVID-19 outbreaks, hospitalizations, and deaths in Ontario’s LTC homes, if implemented. First, temporary staffing could be minimized by improving staff working conditions. Second, homes could be further decrowded by a continued disallowance of three- and four-resident rooms and additional temporary housing for the most crowded homes. Third, the risk of SARS-CoV-2 infection in staff could be minimized by approaches that reduce the risk of transmission in communities with a high burden of COVID-19. Summary Background The Province of Ontario has 626 licensed LTC homes and 77,257 long-stay beds; 58% of homes are privately owned, 24% are non-profit/charitable, 16% are municipal. LTC homes were strongly affected during Ontario’s first and second waves of the COVID-19 pandemic. Questions What do we know about the first and second waves of COVID-19 in Ontario LTC homes? Which risk factors are associated with COVID-19 outbreaks in Ontario LTC homes and the extent and death rates associated with outbreaks? What has been the impact of the COVID-19 pandemic on the general health and wellbeing of LTC residents? How has the existing Ontario evidence on COVID-19 in LTC settings been used to support public health interventions and policy changes in these settings? What are the further measures that could be effective in preventing COVID-19 outbreaks, hospitalizations, and deaths in Ontario’s LTC homes? Findings As of January 14, 2021, a total of 3,211 Ontario LTC home residents have died of COVID-19, totaling 60.7% of all 5,289 COVID-19 deaths in Ontario to date. There have now been more cumulative LTC home outbreaks during the second wave as compared with the first wave. The infection and death rates among LTC residents have been lower during the second wave, as compared with the first wave, and a greater number of LTC outbreaks have involved only staff infections. The growth rate of SARS-CoV-2 infections among LTC residents was slower during the first two months of the second wave in September and October 2020, as compared with the first wave. However, the growth rate after the two-month mark is comparatively faster during the second wave. The majority of second wave infections and deaths in LTC homes have occurred between December 1, 2020, and January 14, 2021 (most recent date of data extraction prior to publication). This highlights the recent intensification of the COVID-19 pandemic in LTC homes that has mirrored the recent increase in community transmission of SARS-CoV-2 across Ontario. Evidence from Ontario demonstrates that the risk factors for SARS-CoV-2 outbreaks and subsequent deaths in LTC are distinct from the risk factors for outbreaks and deaths in the community (Figure 1). The most important risk factors for whether a LTC home will experience an outbreak is the daily incidence of SARS-CoV-2 infections in the communities surrounding the home and the occurrence of staff infections. The most important risk factors for the magnitude of an outbreak and the number of resulting resident deaths are older design, chain ownership, and crowding. Figure 1. Many Ontario LTC home residents have experienced severe and potentially irreversible physical, cognitive, psychological, and functional declines as a result of precautionary public health interventions imposed on homes, such as limiting access to general visitors and essential caregivers, resident absences, and group activities. There has also been an increase in the prescribing of psychoactive drugs to Ontario LTC residents. The accumulating evidence on COVID-19 in Ontario’s LTC homes has been leveraged in several ways to support public health interventions and policy during the pandemic. Ontario evidence showed that SARS-CoV-2 infections among LTC staff was associated with subsequent COVID-19 deaths among LTC residents, which motivated a public order to restrict LTC staff from working in more than one LTC home in the first wave. Emerging Ontario evidence on risk factors for LTC home outbreaks and deaths has been incorporated into provincial pandemic surveillance tools. Public health directives now attempt to limit crowding in LTC homes by restricting occupancy to two residents per room. The LTC visitor policy was also revised to designate a maximum of two essential caregivers who can visit residents without time limits, including when a home is experiencing an outbreak. Several further measures could be effective in preventing COVID-19 outbreaks, hospitalizations, and deaths in Ontario’s LTC homes. First, temporary staffing could be minimized by improving staff working conditions. Second, the risk of SARS-CoV-2 infection in staff could be minimized by measures that reduce the risk of transmission in communities with a high burden of COVID-19. Third, LTC homes could be further decrowded by a continued disallowance of three- and four-resident rooms and additional temporary housing for the most crowded homes. Other important issues include improved prevention and detection of SARS-CoV-2 infection in LTC staff, enhanced infection prevention and control (IPAC) capacity within the LTC homes, a more balanced and nuanced approach to public health measures and IPAC strategies in LTC homes, strategies to promote vaccine acceptance amongst residents and staff, and further improving data collection on LTC homes, residents, staff, visitors and essential caregivers for the duration of the COVID-19 pandemic. Interpretation Comparisons of the first and second waves of the COVID-19 pandemic in the LTC setting reveal improvement in some but not all epidemiological indicators. Despite this, the second wave is now intensifying within LTC homes and without action we will likely experience a substantial additional loss of life before the widespread administration and time-dependent maximal effectiveness of COVID-19 vaccines. The predictors of outbreaks, the spread of infection, and deaths in Ontario’s LTC homes are well documented and have remained unchanged between the first and the second wave. Some of the evidence on COVID-19 in Ontario’s LTC homes has been effectively leveraged to support public health interventions and policies. Several further measures, if implemented, have the potential to prevent additional LTC home COVID-19 outbreaks and deaths.
