Abstrak Kanser prostat merupakan ketiga tertinggi bagi golongan lelaki di Malaysia. Data 2018 melaporkan kanser prostat mencatatkan insiden 1,807 kes berbanding 1,186 kes pada tahun 2014 (Globocan 2018). Kanser prostat ini turut memberi kesan terhadap beban penyakit serta beban ekonomi kepada negara dan memberi kesan terhadap kualiti hidup pesakit terutamanya pesakit berumur melebihi 65 tahun. Kajian ini dilakukan bagi mengukur tahap kualiti hidup pesakit kanser prostat dan menentukan faktor-faktor yang mempengaruhi kualiti hidup pesakit tersebut. Reka bentuk kajian adalah tirisan melintang yang dilakukan dari Januari sehingga Disember 2018. Sampel kajian terdiri daripada pesakit kanser prostat yang didiagnos dan mendapatkan rawatan di Klinik Urologi, Jabatan Pembedahan dan Jabatan Onkologi dari tahun 2008 hingga 2017. Kajian menggunapakai set soal selidik EORTC QLQ-C30 dan EORTC QLQ-PR25 yang telah diterjemahkan ke Bahasa Melayu. Seramai 193 pesakit telah mengambil bahagian. Analisis mendapati tahap kanser memberi perbezaan yang signifikan kepada status kesihatan serta fungsi dan simptom kanser prostat (p < 0.001). Analisis faktor penentu mendapati umur , skor Gleason dan juga tahap lewat kanser merupakan faktor penentu kepada kualiti hidup pesakit kanser prostat (p < 0.05). Langkah kesedaran, saringan serta rawatan awal dilihat perlu dipertingkatkan bagi meningkatkan tahap kualiti hidup pesakit kanser prostat di hospital ini. Prostate cancer also is known to cause burden of disease and economic burden of the country and affects the quality of life of patients. This study conducted to assess the quality of life of prostate cancer patients and to determine the predictor factors that affect the quality of life of the patient. The study design was a cross-sectional conducted from January to December 2018. The study sample consists of prostate cancer patients diagnosed and sought treatment at the Urology Clinic, Department of Surgery and Oncology Department from 2008 to 2017. The study adopted the set of EORTC QLQ-C30 and EORTC QLQ-PR25 which has been translated into Malay. A total of 193 patients participated. The analysis found that cancer levels had a significant difference in health status and the function and symptoms of prostate cancer (p <0.001) in different stage of cancer. Multivariable analysis found that age, Gleason scores and advance-stage of cancer were predictor factors for the quality of life of prostate cancer patients. Awareness, early detection and treatment measures need to be enhanced to improve the quality of life of prostate cancer patients in the hospital. Prostate cancer is the third commonest type of cancer in Malaysian men with the incidence of 1,807 in 2018 compared to 1,186 cases reported in 2014. Prostate cancer is also known to cause disease, economic burden and lead to significant financial implications for the healthcare system. This study aimed to measure the rate of survival and to determine the prognostic factors for prostate cancer patients at Universiti Kebangsaan Malaysia Medical Centre, Kuala Lumpur. This was a retrospective study design involving prostate cancer patients who had been diagnosed and had sought treatment at Universiti Kebangsaan Malaysia Medical Centre from 2008 to 2017. Data were obtained from the patients’ medical records based on TRUS biopsy. The study period was from July 2017 to December 2018. Survival probabilities were estimated using Kaplan-Meier, and differences were compared using the log-rank test. The adjusted hazard ratio (HR) for variables was estimated after fitting the Cox proportion hazard model. Patients' death status due to prostate cancer was the endpoint of the survival analysis. A total of 283 patients’ data were analysed. Total five-year and ten-year survival rates of prostate cancer patients were 77.8 and 65.5%, respectively, regardless of the stage and treatment. The predictor factors for survival age were ≥75 years (HR = 8.49, 95% C.I: 1.16-62.13), Gleason score ≥8 (HR = 2.36, 95% C.I: 1.18-4.73) and cancer metastasis complications (HR = 5.26, 95% C.I: 2.88-9.63). Survival rates and prognostic factors of prostate cancer in the study were almost similar to those in nearby countries in the region. Preventive actions such as prostate cancer screening, early detection of cancer and early treatment should be empowered to improve the survival rate of prostate cancer patients in this hospital.

In this viewpoint, we share and reflect on the experiences of final-year students preparing for a high-stakes examination at the Faculty of Medicine, Universiti Kebangsaan Malaysia during the COVID-19 pandemic. We highlight the new challenges faced during web-based remote learning and major differences in the clinical learning environment at our teaching hospital, which was one of the designated COVID-19 centers in Malaysia. We also document how a face-to-face professional examination was conducted for final-year medical students at our institution despite in times of a global health crisis. The lessons learned throughout this process address the importance of resilience and adaptability in unprecedented times. Further, we recommend appropriate measures that could be applied by medical schools across the world to improve the delivery of quality medical education during a crisis in the years to come.

Background: Syphilis is common and resurgences predominantly occurs among men who have sex with men (MSM).Objective: The aim of the study was to describe the epidemiology, clinical and serological aspects of patients with syphilis diagnosed at Universiti Kebangsaan Malaysia Medical Centre (UKMMC).Methods: A retrospective study was conducted whereby patients with serologically confirmed syphilis cases between January 2010 and December 2012 were included. The epidemiological and clinical data were gathered from the patient’s progress note.Results: A total of 67 patients were selected in this research (41 males, 26 females). The mean age was 48.97 ± 17.45, where majority were from 20 to 39 years old (37.3%). Most patients were diagnosed at latent stage (65.7%). Identified risk factors for syphilis infection included sexual promiscuity either homosexual or heterosexual, intravenous drug user and partner diagnosed as syphilis. Penicillin is the antibiotic of choice for the treated cases.Conclusions: This study managed to provide an epidemiological and serological data of syphilis infection in our medical center where syphilis was diagnosed predominantly in male especially in young adults.Bangladesh Journal of Medical Science Vol.17(1) 2018 p.62-66

Abstract Background Esophageal cancer is one of the deadliest cancer in Malaysia. However, neither the incidence nor prevalence has been recorded nationally. We report our experience in dealing with esophageal cancer in Penang Hospital (PH) and Universiti Kebangsaan Malaysia Medical Center (UKMMC), two tertiary hospitals in Northern and Central region of Malaysia, respectively. Methods A review of 143 esophageal cancer that were diagnosed in PH and UKMMC in year 2014–2017. Results Among the 143 esophageal cancers, 60 cases were from HPP and 83 from UKMMC. The mean age at diagnosis was 62.5 ± 14.2. 67.4% were male patients. Esophageal cancer was commonest in Chinese ethnicity (41%), followed by Malay (29.9%), Indian (17.4%) and others (11.8%). Dysphagia was the commonest presenting symptom (84.6%) and the mean duration of symptoms were 14 weeks. Majority of the cancers were located at the cardioesophageal junction (38.6%), followed by lower third of esophagus (32.1%), mid esophagus (23%), upper third of esophagus (11.5%) and cervical esophagus (5.7%). More than 90% of patients presented with advanced disease, 35% and 55% of patients with stage 3 and stage 4 disease, respectively. Squamous cell carcinoma (45.8%) was the commonest histology type, slightly more than adenocarcinoma (45.1%). The rest were neuroendocrine tumour, gastrointestinal tumour and lymphoma. To date, surgery with curative intend were done in 44.1% of patients while the rest were managed with palliative treatment. Conclusion It is striking that majority of patients presented with late disease. Disease awareness campaign, early detection and multimodality treatment are crucial to improve outcomes of patients. The method of this study was conducted a group counselling session involving six mental health patients who are recovered and still under doctor observation at the Hospital Canselor Tuanku Mukhriz (HCTM), which is under the Day Care Community Psychiatry, Medical Centre and Faculty of Medicine, Universiti Kebangsaan Malaysia (UKM). The findings show that the responsiveness of respondents’ self-management is still lacking as they are still in the process of recovering and monitoring from Psychiatrists and Counsellor Officers at HCTM for time to time.

Abstract:
Background: When exposed to equal radiation doses, the risks for children and adolescents are more significant than for adults. Children grow quickly, and their cells are more sensitive to radiation. After radiation exposure, children have a higher risk of developing malignancies such as leukemia, thyroid abnormalities, and various types of cancers. The healthcare professionals’ (in this context referring to medical doctors at all levels) awareness of imaging modalities associated with ionizing radiation is essential to ensure optimal patient management of cooperation in dealing with radiation exposure. Therefore, the present study is aimed to evaluate the awareness of healthcare professionals on medical imaging-related radiation exposure in the pediatric population in our center, Pusat Perubatan Universiti Kebangsaan Malaysia. Materials and Methods: A cross-sectional survey was conducted among healthcare professionals using self-administered validated questionnaires in a university hospital for a duration of seven months. Healthcare professionals of all levels participated in this survey. Results: A total of 145 healthcare professionals participated in this study. More than half of the respondents are house officers, 57.2% (n = 83). Results indicated that only 6 out of 145 healthcare professionals who participated in this survey had attended a radiation protection course. This survey showed that 37.2% of the respondents were unaware that chest radiographs would expose patients to ionizing radiation. Finally, results also indicated that senior doctors (21 out of 24 participants) showed better awareness of radiation protection knowledge. Conclusions: In general, healthcare professionals in our institution are inadequate in awareness of medical radiation exposure, particularly among house officers. However, the awareness of radiation safety and exposure improves with the number of years of clinical practice. We propose that some younger healthcare professionals do not take radiation safety seriously. Moreover, we would like to suggest all healthcare professionals must attend a radiation safety course, as we expect this will improve patient outcomes.

Abstract:
A newly designed Uniaxial external fixator which functions as a universal fixator in the application of all types of bone fractures is recently introduced by both Hospital Universiti Kebangsaan Malaysia (HUKM) and Universiti Malaysia Perlis (UniMAP). The Investigation is focused on identifying and measuring the performance in terms of strength or weakness of the fixator that is needed before the application to the human body. Hence, this research was conducted to determine the performance of Uniaxial external fixator which was based on geometry using different screw drilling techniques applied during an angled uniaxial compression load. A three-dimensional fixator-bone was constructed using different screw inserting techniques which was then converted into ANSYS v14.5 for the purposes of conducting a finite element analysis (FEA). Axial compressive loading with various degrees from 60 to 6300 N were applied to bone models to stimulate patient’s daily activities while 10 to 100 N were applied to fixator models for the purposes of reviewing environmental loading to fixator-bone models. Findings revealed that maximum magnitude which caused deformation for predrilling and self-drilling models were located at the highest pin-bone interaction. Conversely, the maximum magnitude of the von Mises strain and stress was located at the lowest pin-bone interaction by omitting the existence of fixator for both Case 1 and 2. There was no obvious difference in the comparison of both models in terms of deformation. However, predrilling models have higher strain and stress than self-drilling models. In sum, findings indicated that self-drilling models have better performance compared to the predrilling models.

Abstract:
Background: Stroke has significant direct medical costs, and direct oral anticoagulants (DOACs) are better alternatives to warfarin for stroke prevention in atrial fibrillation (AF). This study aimed to determine the direct medical costs of stroke, with emphasis on AF stroke and the cost-effectiveness of DOACs among stroke patients in a tertiary hospital in Malaysia. Methods: This study utilised in-patient data from the case mix unit of Universiti Kebangsaan Malaysia Medical Centre (UKMMC) between 2011 and 2018. Direct medical costs of stroke were determined using a top-down costing approach and factors associated with costs were identified. Incremental cost effectiveness ratio (ICER) was calculated to compare the cost-effectiveness between DOACs and warfarin. Results: The direct medical cost of stroke was MYR 11,669,414.83 (n = 3689). AF-related stroke cases had higher median cost of MYR 2839.73 (IQR 2269.79–3101.52). Regression analysis showed that stroke type (AF versus non-AF stroke) (p = 0.013), stroke severity (p = 0.010) and discharge status (p < 0.001) significantly influenced stroke costs. DOACs were cost-effective compared to warfarin with an ICER of MYR 19.25. Conclusions: The direct medical cost of stroke is substantial, with AF-stroke having a higher median cost per stroke care. DOACs were cost effective in the treatment of AF-related stroke in UKMMC.

Introduction Even in a country with a tax-based healthcare financing system, health insurance can play an important role, especially in the management of chronic diseases with high disease and economic burden such as Type 2 Diabetes Mellitus (T2DM). The insurance coverage among T2DM patients in Malaysia is currently unclear. The aim of this study was to determine the insurance status of T2DM patients in public and private healthcare facilities in Malaysia, and the association between this status and patients’ sociodemographic and economic factors. Methods A cross-sectional study among T2DM patients seeking inpatient or outpatient treatment at a public tertiary hospital (Hospital Canselor Tuanku Muhriz) and a private tertiary hospital (Universiti Kebangsaan Malaysia Specialist Centre) in Kuala Lumpur between August 2019 and March 2020. Patients were identified via convenience sampling using a self-administered questionnaire. Data collection focused on identifying insurance status as the dependent factor while the independent factors were the patients’ sociodemographic characteristics and economic factors. Results Of 400 T2DM patients, 313 responded (response rate, 78.3%) and 76.0% were uninsured. About 69.6% of the respondents had low monthly incomes of <RM5000. Two-thirds of participants (59.1%) spent RM100–500 for outpatient visits whilst 58.5% spent <RM100 on medicines per month (RM1 = USD0.244). Patients who visited a private facility had five times more likely to have insurance than patients who visited a public facility. Participants aged 18–49 years with higher education levels were 4.8 times more likely to be insured than participants aged ≥50 years with low education levels (2 times). Conclusions The majority of T2DM patients were uninsured. The main factors determining health insurance status were public facilities, age of ≥ 50 years, low education level, unemployment, and monthly expenditure on medicines.

Objective: This study aims to impute the direct drug cost and to assess the prescribing pattern on patients getting dyslipidemia drugs at outpatients’ Pharmacy Department in Universiti Kebangsaan Malaysia Medical Centre (UKMMC).Methods: Patients receiving at least one dyslipidemia drug were randomly selected from UKMMC pharmacy electronic prescribing system from January 2012 to December 2012. The patients’ gender, age, and ethnic, and all the drugs received by the patients during the visits were recorded. The brand name of the drugs and the cost was recorded for the calculation of generic drug prescription rate (GDPR) and direct drug cost.Results: A total of 380 patients were randomly selected from the system. There were 829 prescriptions with the total amount of 5512 drugs prescribed. From the total of drugs, the number of generic drugs prescribed was 3967 (72%). The percentage of patients with GDPR of more than 75% for this study was 48% (n=183). The total drug cost for all 380 patients was RM240,244. The mean cost for each patient was RM637 (Range: RM1.20– RM27,972.30) and the median cost was RM233. The total formulary dyslipidemia drugs cost was RM11,003. The mean formulary dyslipidemia drugs cost for each patient was RM38 (Range: RM1.20–RM810), and the median cost was RM13.50. Predictors of high drug cost were GDPR of ≤75% and a number of drugs per prescription of >10.Conclusion: This study could contribute toward short- and long-term strategies to reduce the drug cost expenditure of dyslipidemia drugs in the hospital.

Diabetes mellitus is a costly chronic disease related to medication, physician consultation and laboratory investigation. The main means of financing healthcare include direct out-of-pocket (OOP) payment and government subsidisation in some countries, or public/private health insurance schemes, or a mix of all. Patient satisfaction is critical in ensuring the use of healthcare services, continuity of care and treatment adherence. In this study, we determined the satisfaction of type 2 diabetes mellitus (T2DM) patients regarding the healthcare services and payment methods at Universiti Kebangsaan Malaysia (UKM) Teaching Hospital, Malaysia. This cross-sectional study involved 313 T2DM patients aged ≥18 years who were included after clinical consultations. We used convenience sampling at the outpatient and inpatient medical centres of Hospital Canselor Tuanku Muhriz and UKM Specialist Centre. A survey consisting of sociodemographic, socioeconomic and payment method types as well as a validated patient satisfaction questionnaire scale were used. The mean age was 59.6 years (SD=13.151), 53.0% of the patients were female, 78.3% were Malay, 76.4% were uninsured, 39.6% were covered by government subsidies, while 36.7% paid OOP. Around 86% were generally satisfied with the overall services. Patients were most satisfied with technical quality (84%), communication skills (83%) and accessibility (80%), but satisfaction was lower in doctors’ service orientation, particularly the interpersonal manner (73%), financial aspect (73%) and time spent with the doctor (70%). Over 86% of patients were satisfied with healthcare services and payment methods; however, patients who paid OOP reported low satisfaction. Full insurance and extending benefits to partially cover both inpatients and outpatients with low co-payment is recommended to increase satisfaction.

The steady growth of pharmaceutical expenditures is a major concern for health policy makers and health care managers in Malaysia. Our study examined the factors affecting the total inpatient pharmacy cost (TINPC) at the Universiti Kebangsaan Malaysia Medical Centre (UKMMC). In this retrospective study, we used 2011 administration electronic prescriptions records and casemix databases at UKMMC to examine the impact of sociodemographic, diagnostic, and drug variables on the TINPC. Bivariate and multivariate analyses of the factors associated with TINPC were conducted. The mean inpatient pharmacy cost per patient was USD 102.07 (SD = 24.76). In the multivariate analysis, length of stay (LOS; B = 0.349, P < .0005) and severity level III ( B = 0.253, P < .0005) were the primary factors affecting the TINPC. For each day increase in the LOS and each increase of a case of severity level III, there was an increase of approximately USD 11.97 and USD 171.53 in the TINPC per year, respectively. Moreover, the number of prescribed items of drugs and supplies was positively associated with the TINPC ( B = 0.081, P < .0005). Gender appears to have affected the TINPC; male patients seem to be associated with a higher TINPC than females (mean = 139.55, 95% confidence interval [CI]: 112.97-166.13, P < .001). Surgical procedures were associated with higher cost than medical cases (mean = 87.93, 95% CI: 61.00-114.85, P < .001). Malay (MYR 242.02, SD = 65.37) and Chinese (MYR 214.66, SD = 27.99) ethnicities contributed to a lower TINPC compared with Indian (MYR 613.93, SD = 98.41) and other ethnicities (MYR 578.47, SD = 144.51). A longer hospitalization period accompanied by major complications and comorbidities had the greatest influence on the TINPC.

This study explored radiology procedure cost across available units in the Radiology’s Department UKMMC (University Kebangsaan Malaysia Medical Centre). In 2011, the total number of radiology procedures carried out in this department was 121,221. Nevertheless, the estimating expenses of offering these procedures are not known. An economic evaluation study was employed and cost centric data mining based on costing activity method was used to determine the charge of the procedure in every centre. Information on seven cost parameters was collected for each procedure: human resources, consumables, equipment, reagents, administration, maintenance and utilities. The results of the study show that the highest percentage of cost parameter for the human resource was Radiology (Mobile) 57.5%, the highest percentage of cost parameter for consumables and reagent was EIR (Endovascular International Radiology) Unit 75.8% and Medical Nuclear Unit 68.1% was the highest percentage of cost parameter for reagent. The MRI (Magnetic Resonance Imaging) Unit 81.4% was the highest cost parameter for equipment. The most top mean cost procedures were EIR MYR4330 and it was revealed that procedures with the highest difference ratio were procedures in EIR (18.50). Finding of this study is very useful to UKMMC management since it helps to enhance the efficiency of services and reduce unnecessary radiology procedures in patient’s management.

Systemic Lupus Erythematosus (SLE) is an autoimmune disease that can affect and cause damage to several human organs such as the skin, kidneys, muscles, joints, lungs, heart and brain. Uncontrolled relapses in SLE patients can lead to death. SLE patients and their families also face difficulties due to lack of information about the disease. Therefore, studies focused on the psychological aspects such as symptoms of anxiety and depression among SLE patients is still needed in Malaysia. Thus, this study investigated the psychological status of SLE such as the occurrence of anxiety and depression in SLE patients and its associated factors. A cross sectional study using the purposive sampling was carried out at University Kebangsaan Malaysia Medical Centre (UKMMC). Hospital Anxiety and Depression Scale questionnaires were used to occurrence of anxiety and depression respectively. Out of 150 SLE patients participated, 46.6% had anxiety while another 33.3% experienced severe depression. Anxiety and depression were significantly correlated with socio demographic factors such as gender (p = 0.006, p = 0.003 respectively), level of education (p = 0.028, p = 0.003 respectively), and monthly income (p = 0.008, p = 0.001 respectively). Overall, socio demographic factors have important influence on the occurrence of anxiety and depression in patients with SLE. Mental health issue among SLE patient in Malaysia should be taken into consideration by health care provider especially by medical social worker.

Introduction: To determine the prevalence of depression and its relationship with health related quality of life among prostate cancer patients in tertiary medical centres, Kuala Lumpur. Methods: A hospital-based, cross-sectional study was conducted at the Surgical Clinic, University Malaya Medical Centre and Universiti Kebangsaan Malaysia Medical Centre over a period of fifteen months. Depression level was measured using Depression Anxiety Stress Scale (DASS-21) and the health related quality of life was measured using the Short Form-36 (SF-36). Results: The prevalence of depression was 11.9% (95% CI: 7.0 – 17.0). The total quality of life for non-depression group was 72.30 ± 13.38 and depression group was 54.11 ± 14.59. All the domains in the health related quality of life and coefficient summaries showed significant differences. Univariate analysis showed significant differences in the physical coefficient summary (p < 0.001), mental coefficient summary (p < 0.001) and total quality of life (p = 0.002) among groups with different depression status. In multivariate analysis, the adjusted mean score of the quality of life was associated with age category of the patients (p = 0.012), urinary complaint of hematuria (p = 0.009) and history of orchidectomy (p = 0.023). Conclusions: The prevalence of depression among prostate cancer patients was relatively low. Treatment of the urination and prostate related problems may improve the quality of life among prostate cancer patients with depression.

Objective: The earliest non-surgical treatment for the correction of congenital idiopathic talipes equinovarus (CTEV) deformity was described by Ignacio Ponseti. He suggested gentle manipulation and serial applications of casts followed by a period of bracing to maintain the correction. The main objective of this study was to evaluate the result of the Ponseti method with a subjective clinical scoring system using Pirani score, to compare the results with the published literature and to evaluate the effectiveness of foot abduction orthosis (FAO) or ankle-foot orthoses (AFO) in preventing relapse following correction.Methods: A cross-sectional study which was conducted at the Paediatric Orthopaedic Clinic, Universiti Kebangsaan Malaysia Medical Centre (UKMMC). A minimum of 12 months follow-up casting was observed before the patient was subjected to the subjective clinical scoring system. A total of 25 patients which included 5 right feet, 9 left feet, and 10 bilateral feet with idiopathic clubfeet were treated with Ponseti serial casting. Percutaneous Achilles tendon tenotomy was done for 28 feet (82.35%) to correct the equinus to achieve full correction.Results: Our results showed that the Ponseti method for treating CTEV was comparable to other published studies. Satisfactory and good results were observed in 97% of cases. A total of 41.18% underwent re-tenotomy of tendon Achilles due to recurrent and persistent equinus.Conclusion: The treatment of CTEV using Ponseti method will increase the successful correction. Meanwhile, the FAO gave better results for maintenance and prevention of recurrent deformity, compared to the AFO.

Introduction: Ultrasound cycloplasty (UCP) is commercially available in Europe. It has been shown to have less side effects than its trans-scleral diode laser cyclophotocoagulation counterpart in reducing intraocular pressure (IOP) in refractory glaucoma patients. Case presentation: This retrospective case series followed seven patients treated with UCP for refractory glaucoma at the Ophthalmology Clinic, Hospital Universiti Kebangsaan Malaysia. Patients were aged 52–80 years, with baseline IOP 14–27 mmHg. All patients received sequential activation of the transducers lasting 8 seconds. Postoperatively, patients were followed-up at 1 week, 1 month, and 3 months. No complications or changes in best-corrected visual acuity were recorded postoperatively. One patient underwent a trabeculectomy 3 months post-procedure, whilst the others continued regular medication. At 1-month postoperative, there was IOP reduction of 6–10 mmHg in six patients. At the 3-month follow-up, IOP returned to the preoperative levels. Conservative power and duration of the shots were used to ensure patients safety. Conclusion: Most studies on UCP safety and efficacy have been conducted on Caucasian populations. A longer duration of UCP may be necessary in the Asian population. Further studies are required to determine the efficacy of UCP in the Asian population.

Background: Diabetes mellitus has become a major public health problem globally. Social media could be useful in assisting clinical practice and sharing health-related information to improve self-management and to promote a positive behavioural change. This study aims to develop a guide on the best online tools by determining the media preference reflected by health- related information received from social media amongst diabetic patients in Hospital Canselor Tuanku Muhriz (HCTM), Universiti Kebangsaan Malaysia, Kuala Lumpur. Methods: This study was conducted cross-sectional on 174 respondents, who were selected by using a simple random sampling method. Socio-demographic data and the use of the internet and media for health-related information were obtained via questionnaires. Results: The most preferred social media used for searching and sharing health-related information was WhatsApp (73.6%), followed by Facebook (67.8%), Instagram (18.4%) and Twitter (17.2%). The social media preference related to socio-demographic data of age was statistically significant (P < 0.002), which had a medium effect. Furthermore, the media preference was not significantly related to health-related information searched or shared on social media and the frequency of usage. Conclusion: Indeed, the social media have been an essential media platform to enhance public awareness concerning public health. This calls for evolution to further enhance the use of social media amongst healthcare practitioners to emphasise health promotion and empower the patients to play an active role in their healthcare. This study provides a guideline for the medical researchers, practitioners or healthcare providers in choosing WhatsApp as an online medium to communicate with diabetic patients in the future, specifically in Malaysia.

Background The incidence rate of colorectal cancer (CRC) in Asian countries is increasing. Furthermore, recent studies have shown a concerning rise in the incidence of CRC among younger patients aged less than 50 years. This study aimed to analyze the incidence trends and clinicopathological features in patients with early-onset CRC (EOCRC) and later-onset CRC (at age ≥ 50 years). Methods A retrospective analysis was performed on 946 patients with CRC diagnosed from 1997 to 2017 at Universiti Kebangsaan Malaysia Medical Centre. The time trend was assessed by dividing the two decades into four 5-year periods. The mean age-standardized and age-specific incidence rates were calculated by using the 5-year cumulative population of Kuala Lumpur and World Health Organization standard population. The mean incidence was expressed per 100,000 person-years. Results After a stable (all age groups) CRC incidence rate during the first decade (3.00 per 100,000 and 3.85 per 100,000), it sharply increased to 6.12 per 100,000 in the 2008–2012 period before decreasing to 4.54 per 100,000 in the 2013–2017 period. The CRC incidence trend in later-onset CRC showed a decrease in the 2013–2017 period. Contrariwise, for age groups of 40–44 and 45–49 years, the trends showed an increase in the latter 15 years of the study period (40–44 years: 1.44 to 1.92 to 2.3 per 100,000; 45–49 years: 2.87 to 2.94 to 4.01 per 100,000). Malays’ EOCRC incidence rate increased from 2008–2012 to 2013–2017 for both the age groups 40–44 years (1.46 to 2.89 per 100,000) and 45–49 years (2.73 to 6.51 per 100,000). Nearly one-fifth of EOCRC cases were diagnosed at an advanced stage (Dukes D: 19.9%), and the majority of them had rectal cancer (72.8%). Conclusion The incidence of EOCRC increased over the period 1997–2017; the patients were predominantly Malays, diagnosed at a later stage, and with cancer commonly localized in the rectal region. All the relevant stakeholders need to work on the management and prevention of CRC in Malaysia.

Introduction: Psoriatic Arthritis (PsA) is an inflammatory arthritis associated with Psoriasis. Its recognition as an inflammatory disease distinct from Rheumatoid Arthritis has put forward for consideration several questions regarding its specific CVS mortality and morbidity (9, 11, 16, 26). Carotid intima media thickness is a useful surrogate and sensitive marker to determine atherosclerosis even in its subclinical stages (6, 14, 22, 27, 32). Objective: Prevalence of carotid intima media thickness in patients with Psoriatic arthritis is unknown in Asian population. We aim to identify the presence of subclinical atherosclerosis in patients with psoriatic arthritis and disease activity association and its predictors in a series of patients with PsA attended to the rheumatology clinic, tertiary hospitals. Methods: A total of 63 patients with PsA who fulfilled the CASPAR criteria were recruited from UKM Medical Centre and Hospital Putrajaya. Common carotid intima media thickness (IMT) was measured in both right and left carotid artery by using high resolution B-mode ultrasound. This was a cross sectional study first done in Malaysia for PsA patients. Results: The positive IMT (IMT >1.00 mm) among PsA was observed in 10 out of 63 patients (15.9 %) regardless of background cardiovascular risk. The mean±SD of IMT was 0.725 ±0.260 mm for this study. Variables significantly associated with positive IMT (p<0.05) included age at the time of study (p=0.005), waist circumference (p=0.001), Hypertension (p=0.007), Diabetes (p=0.002) and Metabolic syndrome (p=0.001) and not associated with gender, ethnicity, duration of PsA disease, pattern of PsA, disease activity and severity. Above all, only age had positive IMT independent predictor (p=0.032), with OR 1.116; 95 % CI (1.010–1.234). Conclusions: There was a significant association between CVS risk and positive Intima Media Thickness in Psoriatic Arthritis patients. Otherwise, there was no association in disease activity, disease severity and DMARDS therapy with positive Intima Media Thickness in Psoriatic Arthritis patients. The study was approved by Research and Ethics Committee of the faculty of medicine, Universiti Kebangsaan Malaysia with project code FF-114-2008 and by Community Research Center (CRC) of National Institutes of Health (NIH) for the case study in Hospital Putrajaya with the project code NMRR-08- 970-2125.

Parapneumonic effusions (PPE) is a complication of bacterial pneumonia. Factors that contribute to complicated PPE remain uncertain. This study was aimed to describe the characteristics, clinical predictors and outcome of children with parapneumonic effusion (PPE) in Universiti Kebangsaan Malaysia Medical Centre (UKMMC). A retrospective study on children with PPE who were admitted to UKMMC between January 2010 to December 2017 was conducted. Patients were categorised into 2 groups: simple and complicated PPE. Of 45 patients recruited, 20 (44.4%) patients had simple PPE and 25 (55.6%) had complicated PPE. Their median age was 32 months (IQR 16-63). The most common isolated organism was Streptococcus pneumoniae (61.9%), followed by Mycoplasma pneumoniae (19.0%) and Staphylococcus aureus (4.8%). Only 11.1% of patients received pneumococcal vaccination. There was no statistical significant difference in clinical features between simple and complicated PPE. Only 62.2% patients had ultrasound thorax done. Majority (95.0%) of patients with simple PPE were successfully treated with intravenous antibiotics alone. Sixty percent of patients with complicated PPE needed surgical intervention. Mean length of hospital stay for simple PPE was 10 (4.0) days and complicated PPE was 28 (16.5) days. In conclusion, clinical features could not predict complicated PPE. Use of ultrasound thorax as the main investigation tool for diagnosis and staging should be emphasised. Antibiotics therapy alone is effective therapy for simple PPE, while in complicated PPE, larger prospective studies are required to investigate which children benefit significantly from more intensive intervention.

Rapid sequence intubation (RSI) is a procedural skill that house officers (HO) in Malaysia need to learn during their training in emergency department (ED) posting. This study aimed to evaluate the confidence and competency of HO in RSI after completed training in the ED of a university hospital during the COVID-19 pandemic. A prospective cross-sectional study was conducted among 44 HO. When they begin their posting in ED, a pre-test was done with a questionnaire to evaluate their knowledge and confidence level. A post-test was conducted three months later with a similar questionnaire. They underwent an objective structured clinical examination to evaluate their skill. A feedback survey form was filled in by the HO regarding their satisfaction in training in emergency rotation. The mean score for pre-test and post-test were 8.75 ± 3.005 and 14.86 ± 2.258, respectively (P = 0.000). There was an improvement in knowledge level and confidence level between pre-test and post-test (all P = 0.000). 43.2% were competent in knowledge, while 65.9% were competent in skill. 35 house officers filled in the feedback survey form. It showed that 42.9% (n = 15) were very satisfied and 28.6% (n = 10) were satisfied with the training in ED. There was an improvement in knowledge and confidence level in RSI among the HO after ED rotation. The clinical exposure in intubating real patients was limited among HO due to the COVID-19 pandemic. The training of HO for RSI in ED needs to be further improvised to optimize their learning process.

The findings show that majority of IPT students have high knowledge in all dimensions, namely the history, policy and the position Malay language globally. However, there are some items in the section of policy and position of Malay language that indicated low level of knowledge. Hence, knowledge towards national language policy and the position of Malay language in the world should be enhanced through mass media or in the process of teaching and learning. This is vital so that students can have better appreciation of the Article 152 in the Malaysian Constitution which focuses on Malay language as the national language, the foundation for unity and building Malaysian civilization. Abstrak: Makalah ini bertujuan membincangkan tahap pengetahuan bahasa kebangsaan dalam kalangan mahasiswa di institusi pengajian tinggi (awam dan swasta) dari aspek i) sejarah, ii) dasar dan iii) kedudukan bahasa Melayu di dunia. Sehubungan itu, satu kajian telah dijalankan di Universiti Malaya, Universiti Kebangsaan Malaysia, Universiti Multimedia dan Unisel, Selangor. Seramai 460 orang responden diperoleh melalui kaedah pensampelan rawak berstrata. Data dikumpul menggunakan borang soal selidik dan dianalisis secara deskriptif. Hasil dapatan menunjukkan majoriti mahasiswa IPT mempunyai pengetahuan yang tinggi terhadap semua dimensi, iaitu sejarah, dasar dan kedudukan bahasa Melayu di dunia. Namun, terdapat beberapa item dalam dimensi dasar dan kedudukan bahasa Melayu yang menunjukkan pengetahuan yang rendah. Justeru, pengetahuan mengenai dasar bahasa kebangsaan dan kedudukan bahasa Melayu di dunia perlu dipertingkatkan melalui media massa atau dalam proses pengajaran dan pembelajaran agar mahasiswa dapat lebih menghayati Perkara 152 Perlembagaan Malaysia mengenai bahasa Melayu sebagai bahasa kebangsaan yang menjadi teras kepada perpaduan seterusnya tamadun Malaysia.

This is a study on the 2- bins recycling system at Universiti Kebangsaan Malaysia (UKM) year 2010 to reduce waste disposal at landfills. Fifty sets of 2-bins recycling system were allocated in the Faculty of Engineering and Built Environment, Universiti Kebangsaan Malaysia (UKM). The number of recyclable items found in the recycling and mixed waste bins served as dependent measurements. Results indicated that the recycling containers successfully recovered recyclable items by 49.5%, but still a high proportion of recyclable items thrown into the mixed waste container which is 50.5% which indicates wrong bins usage. Awareness programs are required to increase rate of participation among students.

Di era Revolusi Industri ke 4, perniagaan atas talian merupakan salah satu kerjaya yang digalakkan untuk diceburi pelajar namun data menunjukkan bilangan yang menceburi bidang ini masih dianggap rendah. Justeru, kajian ini dijalankan bagi tujuan mengenal pasti cabaran kerjaya keusahawanan atas talian mengikut persepsi pelajar Universiti Kebangsaan Malaysia (UKM). Seramai 150 pelajar telah dipilih secara rawak berstrata dilibatkan dalam kajian ini dan soal selidik digunakan sebagai instrumen kajian. Hasil kajian mendapati bahawa terdapat tiga cabaran utama perniagaan atas talian mengikut persepsi pelajar iaitu berkaitan kos perniagaan, komunikasi interpersonal dan kemahiran ICT. Implikasi kajian ini adalah mendorong pihak universiti untuk memperbanyakkan program berkaitan cabaran tersebut agar dapat diatasi pelajar nanti.

Namun, masih lagi terdapat pelajar yang kurang mahir dengan teknologi maklumat yang tersedia kini tetapi mereka perlu segera memahirkan diri dan belajar dalam masa yang singkat supaya tidak mengganggu aktiviti pembelajaran. Oleh itu, kesediaan pelajar mempunyai peranan yang sangat penting dalam memastikan manfaat kecanggihan teknologi dan keberkesanan perlaksanaan pembelajaran atas talian boleh diterima oleh pelajar secara optimum untuk meningkatkan tahap motivasi mereka. Disebabkan tahap kesediaan dan motivasi adalah berkaitan antara satu sama lain, kajian ini bertujuan untuk mengkaji hubungan antara tahap kesediaan dan tahap motivasi serta pengalaman pembelajaran atas talian pelajar Sarjana Pendidikan di Universiti Kebangsaan Malaysia (UKM). Kaedah persampelan rawak berkelompok telah digunakan untuk mendapatkan seramai 164 orang responden kajian sebelum pautan soal selidik diedarkan melalui e-mel. Hasil analisis data mendapati bahawa tahap kesediaan dan motivasi pelajar Sarjana Pendidikan di Universiti Kebangsaan Malaysia (UKM) berada pada tahap yang tinggi dan tahap kesediaan pelajar juga mempunyai hubungan yang kuat dan positif dengan tahap motivasi serta pengalaman pembelajaran atas talian pelajar-pelajar. Pengkaji menyarankan penglibatan sampel kajian yang lebih besar seperti pelajar-pelajar di universiti-universiti awam Malaysia supaya data yang bakal diperoleh boleh digeneralisasikan dengan lebih meluas dan boleh mempengaruhi pelaksanaan pengajaran dan pembelajaran yang boleh diguna pakai oleh semua Institusi Pengajian Tinggi pada masa hadapan.

Keusahawanan memainkan peranan penting dalam menentukan kemajuan ekonomi sesebuah negara. Bidang keusahawanan merupakan pilihan kerjaya yang digemari oleh generasi muda khususnya di negara maju. Terdapat segelintir graduan Malaysia menganggap keusahawanan sebagai pilihan kedua atau terakhir mereka. Penyelidikan ini bertujuan untuk menguji hubungan antara sikap keusahawanan dengan tingkah laku pemilihan kerjaya keusahawanan pelajar Universiti Teknologi Malaysia. Alat yang digunakan dalam kajian ini ialah soal selidik dalam talian, dan data dianalisis menggunakan Pakej Statistik Sains Sosial (SPSS) versi 26.0 dan teknik statistik deskriptif dan inferensi. Sampel kajian ini terdiri daripada 336 pelajar Universiti Kebangsaan Malaysia (UKM). Borang soal selidik adalah sebagai instrumen kajian. Hasil kajian menunjukkan bahawa hubungan antara sikap dan tingkah laku pelajar terhadap pemilihan kerjaya keusahawanan adalah sederhana tinggi. Secara keseluruhannya, kajian ini mendapati pelajar Universiti Kebangsaan Malaysia secara amnya digalakkan memilih kerjaya keusahawanan selepas tamat pengajian. Analisis regresi bagi sikap dan tingkah laku terdapat hubungan yang signifikan ialah 0.502 dan 0.063, bermaksud kadar hubungan dua pemboleh ubah tersebut adalah tinggi. Nilai p bagi pemboleh ubah adalah kurang daripada tahap signifikan (p<0.001), data sampel memberikan bukti yang cukup untuk menolak hipotesis. Implikasi hasil penyelidikan ini menunjukkan bahawa para pendidik merancang dan menjalankan program pembelajaran keusahawanan dalam kalangan pelajar bagi merangsang minat mereka dalam bidang keusahawanan dan menjadikan kerjaya keusahawanan sebagai pilihan utama mereka pada masa akan datang.

This article is discussing about experiences of conducting AFLES (ASEAN Future Leaders Summit), which started in 2013 with the collaboration between Universiti Sains Malaysia (USM) and Prince of Songkla University, Thailand (PSU). AFLES is an action-packed summit meant for ASEAN youth with exciting forums, discussions, lectures, team-work, outdoor activities, community work, services, sight-seeing and cultural events. With the main objectives of developing ASEAN youth community in terms of their leadership skills and to serve as a platform for ASEAN youth to get together and connected to each other, AFLES for the last sixth term (2013-2018), has evolved to be a looking after summit to participate by ASEAN university student leaders. Over the years AFLES has developed and progressed with a few more collaborators, namely AKEPT (Malaysia Higher Education Leadership Academy), UiTM (Universiti Teknologi MARA), UUM (Universiti Utara Malaysia) and UKM (Universiti Kebangsaan Malaysia). Community development, ASEAN unity and harmonization, and Internationalization of Higher Education Institutions in ASEAN are discussed regarding to the sustainability of AFLES in conducting an international program for youth across ASEAN region.
