Abstract:
This thesis reports on an ethnography of a hospital social work team in Wales. The aim of this study was to explore the nature of the statutory social work role within hospitals, to examine how hospital social workers do their work, and to shed light on how social work fits into the hospital context. My findings indicate that hospital social workers face constant pressure from managers and clinicians to expedite patient discharges, and exclude almost all other tasks from their role. Their daily work is a sequence of bureaucratic tasks, focused on management of the failing body, often to the exclusion of considering the wider social or psychological needs of the patient. Drawing on the work of Bauman, I argue that the bureaucratic and managerial systems in which hospital social workers operate produce dehumanising practices and distance decision makers from the human consequences and moral dimensions of their decisions. Even within these systems, however, some levels of discretion are maintained and hospital social workers use their discretion in a variety of ways. The hospital social workers in this study consistently expressed values derived from anti-discriminatory practice and, despite the constraints they encountered, were able to perform work that showed a concern for social justice, human rights and empowerment at the individual’s level. Thus, I argue that hospital social work in the UK is driven by liberal, rather than radical values, and is largely unconcerned with addressing wider issues of structure, social disadvantage and oppression. The hospital social work role involves the co- ordination of knowledge provided by clinical professions, which must then be processed to match the needs of the patient to the services that are available. Social workers are outsiders within the hospital setting and there is a considerable amount of distrust between them and the clinical professionals, which occasionally manifests in open conflict. I draw on Goffman’s dramaturgical insights to analyse how social workers manage their position within the hospital and draw on his theory of frame analysis to understand the way conflicts arise. Hospital social workers maintain a distinct identity within the hospital that is tied to their liberal values. I argue that their practices can be interpreted both as arising from the zeitgeist of liquid modernity and as adapting to the human need brought about by liquid modernity. I suggest that social work must either pursue individual liberation further, following the liberal values currently underpinning these hospital social workers’ practice, or adopt a more radical or critical approach in seeking to influence government policies around social care.

Abstract:
This study examined the effect of interdisciplinary discharge planning rounds on timing of social work intervention, length of stay (LOS), and readmission for patients aged 65 and over. Data sources were the medical charts of 449 patients discharged during two corresponding 28 day periods (one before end one after the implementation of rounds) supplemented by Discharge Planning Committee minutes (DPCM) and interviews with four key informants. No significant differences in the timing of social work intervention, LOS, or readmissions were found between the two samples. Qualitative research revealed that essential components were either missing (physician participation), or not uniformly included (family participation) in rounds, and that staff felt that rounds improved communication among the disciplines and contributed to improved efficiency in planning hospital and posthospital services. These findings highlight the need to further study all aspects of the complex discharge planning process to identify factors that would reduce LOS and readmissions.

Abstract:

Some 120 to 150 people per year, in Sweden, injure their spinal cord in an often traumatic accident. During the initial trimester in a university hospital there are several issues arising for the patients, not only the fact that they suddenly cannot walk. The aim of this undergraduate thesis is to provide a review of selected literature on evidence-based interventions by hospital social workers for spinal cord injured. The method used in the thesis is database searches. Fourteen articles were included in the review. These articles have been divided into the categories: coping and interventions. Research shows that spinal cord injured have an increased risk of anxiety and depression. The most important findings were that spinal cord injured in both Sweden and Finland are dissatisfied with the societal services, that a spinal cord injury does not necessarily lead to anxiety and depression and that there are even positive outcomes of a spinal cord injury. Social and societal services are important in the adjustment process for spinal cord injured. Psychological interventions based on cognitive-behaviour therapy have been proven effective in the care of spinal cord injured.

Abstract:
Transitioning into a long-term care home has been described as a difficult experience for both patients and families. This is particularly true of transitions from hospital where pressure to make immediate decisions is increased, choices are limited and policies resulting in multiple moves have been implemented. This study quantitatively analyzed hospital data in a Quebec based hospital prior to and following the implementation of a new policy requiring all patients deemed medically stable to be discharged to a temporary facility prior to their permanent relocation. More specifically secondary data analysis was used to explore (1) if patients' decisions to opt out of the public system increased when the new transitional system was implemented and (2) if rates of return through the ER increased under the new system. Data from 321 patients were analyzed including 161 patients under the old system (LTC) and 160 patients under the new system (PHPE). Analysis of the data revealed that more patients followed the public process in the new system and that there was a trend towards increased visits to the ER by patients in the new system. As provincial governments across Canada continue to adopt policies that force multiple moves from hospital more research is needed on the impact of such policies on patients' and families' experiences of care, physical and mental health and health care utilization.
Effectuer la transition dans un établissement de soins de longue durée a été décrite comme une pénible expérience à la fois pour les patients et les familles. Ceci est particulièrement vrai pour les transitions de l'hôpital alors que la pression exercée pour prendre des décisions rapides est plus grande, que les choix sont limités et que les procédures engendrant de multiples déménagements ont été appliquées. Cette étude quantitative a analysé les données d'hospitalisation dans un hôpital du Québec avant et après l'application d'une nouvelle politique décrétant que tous les patients jugés médicalement stables soient évacués vers une installation temporaire avant leur réinstallation permanente. Plus précisément, une analyse des données secondaires a été utilisée pour déterminer (1) si les décisions des patients de se retirer du système public ont augmenté quand le nouveau système transitoire a été mis en place et (2) si le nombre des visites à l'urgence a augmenté dans le cadre du nouveau système. Les données recueillies auprès de 321 patients, parmi lesquels se trouvaient 161 patients sous l'ancien système (LTC) et 160 patients sous le nouveau régime (PHPE), ont été analysées. L'analyse des données a révélé qu'un plus grand nombre de patients ont suivi le processus public dans le nouveau système et qu'il y avait une tendance à l'augmentation des visites à l'urgence parmi les patients du nouveau système. Alors que les gouvernements provinciaux du Canada continuent d'adopter des politiques qui forcent les déménagements multiples à la sortie de l'hôpital, davantage de recherches sont nécessaires pour connaître l'impact de ces politiques sur les expériences de soins vécues par les patients et les familles, sur la santé physique et mentale et sur l'utilisation des soins de santé.

Abstract:
The purpose of this study is to assess the perceptions of social workers employed in Florida hospitals in relation to the core values of their profession and the alignment of those values within the culture of their current work setting. The conceptual framework for the study was from organizational behavior theory specific to culture, values, and trust. The Competing Values Framework (Cameron & Quinn, 1999) provided a method to distinguish co-existing competing values within an organization. The research findings indicated that 65% of the professional social workers who participated in the study perceived that the core values of their profession are very much in alignment with the written mission statement of their hospital. However, less than half of the respondents (42%) stated the daily business of the hospital strongly reflected the mission statement. The social workers perceived the current culture of hospitals in Florida as being closely clustered among four cultures: clan, adhocracy, market, and hierarchy. However, they would prefer a stronger clan culture and less of a market culture in the future. A large majority (85%) of all respondents communicated that their work assignments allowed them to demonstrate their professional values on a regular basis. However, only 63% stated that they trusted that their hospital valued the knowledge and skills of their profession.

The overall objective of this study was to examine burnout and job satisfaction from the perspective of the social workers at Grady Memorial Hospital. To obtain this objective, the following areas on burnout and job satisfaction were addressed by the researcher: (a) characteristics of burnout and job satisfaction, (b) how burnout occurs, and (c) strategies for reduction of burnout and improving job satisfaction. This study was an attempt to provide a clear understanding of what burnout is, how it affects the social workers, and suggest ways to reduce burnout and increase job satisfaction among the social workers. The results showed that burnout was not prevalent among the social workers and they were satisfied with their jobs.

Abstract:
This was a descriptive study of the discharge planning for "at risk" elderly patients at Grady Memorial Hospital. The researcher also examined variables associated with the discharge planning process. The variables were: 1) efforts of the medical social workers in discharge planning; and 2) presenting factors necessitating help for the "at-risk" elderly patients. Findings revealed that 73% of the "at-risk" elderly patients had successful post-discharge continuing care placement. Thus, indicating that the medical social workers were also successful in their discharge planning efforts. Of the elderly patients placed, 17% needed twenty-four (24) hours skilled nursing care, versus 3% who needed intermediate (lower level) care. In addition, 76% of the "at-risk" elderly suffered from multiple medical problems. Eighty percent of the elderly patients had families who were unable to provide post-hospital care; 7% had families who were unwilling to provide post-hospital care, and 13% had no family to provide care. Only 1% of the "at-risk" elderly patients needed continuing care placement due to elder abuse. A significant number (53%) of elderly patients were found to have no impaired mental-functioning, many received medical insurance funding, 47% Medicare, 33% Medicaid, and 20% social supplemental income.

Abstract:
One thousand four hundred and twenty two patients in an Acute General Hospital in Mansfield were studied over a one year period, 1989-90, in order to determine their needs for formal and informal care on discharge. The research method included a screening project for 189 patients on admission, an analysis of 1064 referrals to the Hospital Social Workers, and 169 referrals to the Hospital Discharge Scheme for Volunteer support. Patient/client needs for formal and informal care were found, and unmet needs after Hospital discharge were identified. Problems relating to formal care systems and shortage of Public Sector resources were found to cause serious difficulties for patients and Carers. The availability of Carers and lack of family members in informal care structures was a key issue. The work showed how Volunteers from the Discharge Scheme were able to contribute to the work of formal and informal Carers and ensure that safe Hospital Discharges occurred for very vulnerable people.

The overall objective of this study was to determine if there was a significant relationship between job burnout and social support systems among social workers employed in a VA hospital setting. To obtain this objective the researcher examined job burnout in relation to two key social support systems (Le., emotional and practical support). The researcher also explored the impact of demographic predictor variables on the relationship. An exploratory descriptive research design was used in this study. A self administered questionnaire was given to 42 VA hospital social workers employed in 4 VA medical centers located in the Southeast region of the United States. Bivariate analysis was computed using Pearson's r correlations. The study found that there was a significant relationship between job burnout and emotional support among social workers employed in a V A hospital setting.

Perinatal loss is a relatively common occurrence and is defined as any loss, from conception through the first 28 days of life, including miscarriage, stillbirth, and neonatal death. In this dissertation, perinatal will refer to the death of an infant due to miscarriage or stillbirth. Researchers says that understanding how mothers perceive the care after perinatal loss can play a significant role in their care, treatment, and health. The purpose of this appreciative inquiry action research study was to examine what women need to feel supported, emotionally, physically and spiritually within a hospital setting and how these supports impact women experiencing perinatal loss. Through participation interviews from women who experienced the loss and questionnaires answered by nurses and social workers, an investigation of what women perceive to be beneficial in receiving care after perinatal loss. Interviews were used to collect information from a sample of nine women who experienced prenatal loss in a hospital setting, as well as five nurses or social workers who provided support to these individuals were asked to participate in an interview, but if unable to then a questionnaire will be email to them. The information collected in this study was useful to medical and mental health professionals to develop more effective interventions to assist staff to support the women experiencing the loss, while validating the loss and treating the women with respect.

The objective of this study is to explore the extent to which hospital social workers engage in advocacy activities, and the degree to which they feel supported by their departments to advocate on behalf of clients. To obtain this objective, the following areas on hospital social workers will be addressed: (a) work experience, (b) time spent on social work functions and (c) the degree to which encouragement is provided by the department for advocacy. An anonymous questionnaire was administered to twenty six social workers from the Atlanta VA Medical Center located in Decatur, Georgia.

This exploratory study tracks the experience of nine infertile couples on the In Vitro Fertilization-Embryo Transfer (IVF-ET) programme at a provincial hospital. Various theoretical perspectives are explored vis a vis the role of women in society in relation to their infertility status. Quantitative data was obtained from questionnaires completed; and qualitative data was obtained through the use of in-depth interviews. The study supports the notion that infertility has both a medical and a psychological component, the latter being unaddressed by the infertility team. Infertility is a couple issue and needs to be addressed as such. The study provides guidelines for future service delivery and therapeutic interventive strategies, and concludes that for effective service delivery, a holistic interventive approach with the inclusion of a clinical social worker is imperative.

This study explores family satisfaction with services provided to them on their relatives' admission to Valkenberg Psychiatric Hospital in Cape Town. The rationale for doing this research arose from the researcher's encounters with families of psychiatric patients and the perceived lack of involvement of the professionals with the families. In addition, a needs assessment was done to establish the need for family groups. The literature review traced the psychiatric services from the earlier custodial orientation, the subsequent deinstitutionalisation of the services resulting in the families becoming the primary caregivers of their mentally ill relatives, to the emphasis of the biopsychosocial model in the recently established community services. Review of the treatment models indicated supportive family education as the primary form of intervention to ensure compliance, aid the recuperation process, enhance the quality of life for the patient and their families and to equip the families to cope with the illness. A survey method was employed to gather quantitative and qualitative data via questionnaires and face-to-face interviews with 31 relatives of 24 patients who had been admitted during January, February, and in March 1995 to the male and female admission units (wards 9, 10, 14, 15 and 16) at Valkenberg psychiatric hospital in Cape Town. A non-probability convenience sampling procedure, based on the availability and accessibility of the relatives to attend an office interview, was used. Findings from prior studies as well as from this current study have shown a lack of professional involvement with the families in relation to the treatment programme of their mentally ill relatives. The majority of the respondents were not informed about the implications of the illness and the effect of treatment and most of the respondents lacked guidance on how to cope with their mentally ill relatives. Other findings indicated that families are the most concerned about the lack of supported housing, rehabilitation services, adequate financial assistance, day care centres, sheltered employment for their mentally ill relatives and difficulties in relation to admission procedures. More than three quarters of the respondents were interested in family groups and there was a definite need for professional counselling for the parents and siblings. In response to the findings a number of recommendations are made to improve services to families, including the formation of family groups where the relatives of mentally ill people can receive psychoeducation and practical guidance. The need for further research has been identified.

Abstract:
A Feminist Action Research methodology was used as a collaborative process with five ACT Health social workers based at the Community Health Centres and four at the Woden Valley Hospital. The primary purpose of the study was to investigate, both through critical reflection and action in their work setting, the participants' relevance or otherwise to Aboriginal people in the ACT and region. Behind this is the question of how encapsulated social work is by racism. The impetus for the study arose from my unresolved concerns regarding these issues, having been a social worker in ACT Health for 6 years, to 1990. Decisions on how to proceed involved a process of ongoing consultation between the participant social workers and myself. Exploratory meetings were held in March and April, with an ongoing program being held 2-3 weekly from June to September, followed by a review in December. Most gatherings were specific to the Woden Valley Hospital or Community Health settings. However two half-day workshops were held for all the participants. All the sessions from June were taped. Aboriginal leaders were consulted, as were several managers in ACT Health. The phenomena of institutional, cultural and personal racism were addressed by the social workers through discussion, exercises, and anti-racist initiatives in their work setting. They found that significant time restraints presented an example of institutional racism working against their good intentions. Another dimension arose from implicitly racist education in social work courses when most of the participants undertook their undergraduate courses in the 1960s and 1970s. Aspects related to professionalism such as its language and separation of a personal and professional self were indicative of cultural racism. Stories of personal racism were shared, in the context of raised awareness leading to changing those attitudes and behaviours. The fact that the study took place in 1993 - a watershed year for Aboriginal/white relations in Australia - seemed to lead to greater momentum for the project. The social workers found that participation in this study increased their knowledge of, and their confidence - both actual and potential - in interaction with Aboriginal people. However, they also understood these to be just small steps towards greater justice for the indigenous people. An outcome of the project has been involving some colleagues in similar anti-racist actions to those the social workers participated in during the time of the study. The action research project has continued on in different ways, beyond 1993, despite my withdrawal as 'the researcher' who took the initiative.

The importance of ethnic origin as a factor in delivery of social services has been recognised internationally as relevant at different levels of organization. This study examined 500 dossiers, a random sample of clients referred in 1985 to one hospital social service department in Montreal. Age, gender, status of children, referring hospital service, problems experienced and involvement with community social service agencies were found to be related to ethnic origin, using the Kruskal-Wallis and Pearson chi-squared test. After accounting for differences between ethnic groups in age, type of problem and referring hospital service by the use of logit analysis, ethnic origin significantly affected the changes of involvement with Social Service Centres and Departments of Youth Protection. Among the implications of the results for social services in Montreal were the need for the following: recognition that some ethnic minorities have very different social service needs than the larger ethnic groups; development of skills in cross-cultural social service provision because of the clientele's varied ethnic background; consideration of the impact, desirability and viability of ethnic/socioculturally specific agencies and services. The urgent need for further research is emphasised.

Abstract:
This inquiry investigated the phenomenon of 'staying' as a feature of the turnover, recruitment and retention of social workers in hospital settings. In the absence of specific literature on the subject, the study made a significant and original contribution to knowledge about social work staff turnover, including the meaning attached to work, the relationship of social workers to the hospital organisation, and to job satisfaction. Using qualitative methodology, data were collected from multiple sources including in-depth interviews at two large Sydney Teaching Hospitals, a period of engagement in each Social Work Department, a comprehensive review of primary data from archival records of the Directors of Social Work Services in Teaching Hospitals (NSW) between 1981-1999, and secondary source materials of federal and state health policy documents. Two core categories emerged from the study which were social workers 'tolerance' of the hospital environment and their qualities of 'selfactualisation'. Of central importance to the inquiry was the discovery of grounded theory which explained the relationship of these categories to the phenomenon of 'staying'. The intersection of 'tolerance' and 'self- actualisation' qualified 'staying' as either a positive or negative experience for the social worker and the hospital. This theory was presented as an original model which with further refinement, could be used as a predictive tool in studies of turnover of social workers in hospital settings. The findings challenged existing theories that staff retention was preferable to staff turnover in hospital organisations and identified qualities in hospital social workers which would ensure the continuing relevance of social work to the changing hospital environment. The model has implications for hospitals, Social Work Departments and for social work education in the preparation and training of social workers to join the hospital workforce. Specific activities that supported the 'self-actualisation' of social workers, and aspects of 'tolerance' were identified which worked for the benefit of both social workers and hospitals.

Background - UK healthcare has adopted various improvement strategies from business including using satisfaction surveys. However, the way patients form and express judgements of quality care can be more complex than customer purchases. Research in adult patient satisfaction has found capturing patient opinion challenging; however, adult service- user views continue to underpin quality care guidance across healthcare. Development of knowledge to include parental views of quality care is important to inform future guidance specific to children’s services. Methods - This study aimed to examine how parents determined the quality of care provided when their child was hospitalised, factors influencing perceptions of care and whether these judgements changed over time. Using a grounded theory approach, data were collected through a series of in-depth interviews (22) with nine parents following their child’s hospitalisation. Findings - The substantive grounded theory - Parenting in an alien hospital world: on guard and on behalf offers new perspectives on the complex psychosocial processes underlying parents’ quality judgements. When their child was hospitalised parents landed in an ‘alien’ world but continued to try 'to parent' (protect and advocate) their child. Parents' experiences were characterised by landing; moving from being' new parents' to 'old hands'; searching for and judging the 'clues' and facing dilemmas of how best to respond to professionals. Parents were found to use an escalating level of signals to prompt health professionals to respond to their queries and concerns Parents' personal lens altered through their transitional journey and this, together with their perception of professionals' power impacted on their chosen responses to professionals and their quality judgements. Parents held two views of care quality: ‘at the time’ and a final ‘on balance’ view. The final view recognised their own heightened emotions and reflected their current transition. Conclusions - Ultimately parents judge hospital care as high quality when they perceived health professionals acted as their allies in their parenting roles as protectors and advocates for their child in the alien hospital world. Health professionals could improve parents' experiences and quality judgements of care by early recognition and response to their 'signalling' and by explicitly acting as parental allies.

In this study the writer assumed that the psycho-social characteristics of physically disabled patients would influence the role and task of the medical social worker at the Rehabilitation Clinic. Therefore an attempt was made to determine the psycho-social characteristics of patients referred to the Rehabilitation Clinic. Furthermore an attempt was made to determine the role and task of the medical social worker in relation to these psycho-social characteristics. The research method was a descriptive one, using archival data; the latter being social work records of interviews with patients. Prior to the research a literature study was done on various aspects of physical disability and vocational rehabilitation. A sample group of 45 subjects was compiled by a random sampling technique. Two structured schedules were used to extract data from both the social work file and the Rehabilitation Clinic patient file. The research and processing of the information was done manually. The findings revealed the following 1. The majority of the physically disabled patients were Coloured males in the age category of 31-50 years. 2. These patients had poor educational qualifications and were previously unskilled or semi-skilled labourers. 3. The patients were unemployed and had no income. 4. These patients had a passive lifestyle with no hobbies or little religious involvement. They also habitually abused liquor and/or cannabis. 5. The housing conditions of these patients were inadequate and primitive. 6. The various roles of the medical social worker included those of rehabilitation counsellor, team member, co-ordinator/resource manager and teacher, whereas the tasks included assessment, dissemination of information, referrals, co-ordinating of services and education. The final conclusion in this study was that the role and task of medical social worker was influenced by both the psycho-social characteristics of referred patients as well as the expectations of the rehabilitation team of the Rehabilitation Clinic.

Abstract:
Undocumented immigrants have historically been excluded from the medical care system. This study explored the attitudes and experiences of undocumented Latino immigrants when seeking medical services in the San Bernardino County. It aimed to share the stories of undocumented immigrants that often go untold and provide valuable insight into the needs and concerns of this population. The exploratory study utilized a qualitative approach through interviews. Data was collected through in person and over the phone interviews, which were then transcribed and analyzed using a constant comparative method. The findings of this study identified barriers and challenges faced by undocumented immigrants including a lack of equal access to healthcare, a lack of knowledge in navigating the healthcare system, stigma and discrimination, language, finances, culture, and fear. Both negative and positive experiences were shared, along with advice provided by participants for undocumented immigrants. This study carried important implications for medical professionals, agencies, and policy makers to better address the needs and concerns of undocumented patients. Research recommendations included implementing programs based off this study’s findings and exploring the attitudes and experiences of this population when seeking mental health services. Further research is encouraged to work towards dismantling the barriers faced by the undocumented immigrant population.

ENGLISH ABSTRACT: This research investigates the indicators, different role players and the assessment process of intentional physical injuries to children who are hospitalized. The basic premise for this research is the importance of professionals working with children in acquiring skills and knowledge on handling physically abused children. Physical abuse affects and requires the involvement of the entire family of the physically abused child. It is therefore necessary to consider the physically abused child as a part of the larger family system, and not assessed or treated in isolation. The purpose of this research is to broaden the theoretical knowledge of professionals working with children, and specifically social workers, in identifying and assessing physically abused children. This research report includes an investigation of risk factors, consequences and the adjustment process of physically abused children. Knowledge of these indicators of physical abuse will increase the awareness and the ability of the social worker or other professional to identify the injury as intentional. The assessment process by the multi-professional team in the hospital is also examined, focusing on the central role of the social worker in managing cases of physically abused children. The phases of the assessment process, the role of each team member, the various techniques applied as well as factors influencing assessment are described. The empirical research involved the use of both qualitative and quantitative methods in order to explore the theoretical part of the research. This section can be divided into three phases. The first phase included setting up a profile, over a period of four months, of physically abused children under the age of five, who were admitted to the hospital and their families (N = 24). Of this sample, the characteristics and circumstances of four parents/perpetrators (n = 4) were investigated in the form of interviews, which formed part of the second phase of the empirical research. The third phase included conducting a survey with the social workers (n = 5) that assessed physically abused children in the hospital. This was done III the form of questionnaires, which included closed and open-ended questions. The findings and responses of the respondents were analyzed and compared with the findings from previous studies undertaken by various authors. The findings of this research can be used as guidelines for professionals in general who work with children, and more specifically social workers in a hospital that assess children presenting with intentional physical injuries.
AFRIKAANSE OPSOMMING: Hierdie navorsing ondersoek die aanduidende faktore, die rolspelers en die asseseringsproses van fisies mishandelde kinders wat gehospitaliseer word. Die uitgangspunt is die noodsaaklikheid van professionele persone wat met kinders werk, en veral maatskaplike werkers, om kennis en vaardighede te hê, ten einde kinders wat fisies mishandel is te kan hanteer. Omdat die hele gesin van die mishandelde kind geaffekteer word en betrokke is, is dit belangrik om die fisies mishandelde kind nie in isolasie, maar as deel van die familie sisteem te kan benader. Die navorsingsondersoek het ten doel om die teoretiese kennis basis van professionele persone wat met kinders werk, en spesifiek maatskaplike werkers, oor die identifisering en assesering van fisies mishandelde kinders uit te brei. Ten einde maatskaplike werkers se kennis van die probleem onders aandag uit te bou, vervat die navorsingsverslag 'n bespreking van die risiko faktore, gevolge en aanpassingsproses van die fisies mishandelde kind. Kennis van hierdie aanduiders sal die bewusdheid asook die vermoë van professionele persone om die fisiese mishandeling te identifiseer, verhoog. Die asseseringsproses deur die multiprofessionele span in die hospitaal word ook ondersoek, met fokus op die sentrale rol van die maatskaplike werker in die hantering van fisies mishandelde kinders. Die fases van die asseseringsproses, die rol van elke spanlid, verskeie tegnieke en faktore wat assesering beïnvloed, word bespreek. Die teoretiese doel van die verslag word uitgebrei in die empiriese ondersoek, deur 'n bespreking van die bevindinge wat deur veskeie metodes verkry is. Kwalitatiewe asook kwantitatiewe metodes is benut tydens die drie fases van die empiriese ondersoek. Die eerste fase was die opstel van 'n profiel wat oor 'n tydperk van vier maande geneem is, van fisies mishandelde kinders wat toegelaat is in die hospitaal en hul gesinne (N = 24). Vanuit hierdie streekproef, is die eienskappe en omstandighede van vier ouers/oortreders (n = 4) ondersoek deur middel van onderhoude, wat die tweede fase van die ondersoek bevat. Die derde fase was om 'n opname te doen met die maatskaplike werkers (n = 5) wat die fisies mishandelde kinders in die hospitaal asseseer. Die bevindinge en response van die respondente is geanaliseer en vergelyk met die bevindinge van vorige studies wat deur verskeie outeurs onderneem is. This study aimed to analyze the work of social workers at the Hospital Universit?rio Onofre Lopes (HUOL), with the analytical approach the contracting process with the HUOL with the National Health System (SUS), which is set from 2004. Thus, this study sought in times of state reform, restructuring and tension between enlargement / reduction of social and labor rights, understanding the limits and possibilities of social work in HUOL, analyzing how these determinations bounce in the practice of social workers included in the collective process of health work. From a theoretical and methodological historical and dialectical materialism, we conducted literature search, in which developed book report and readings of texts, articles, books that focus on the central categories of the study, namely: Work, Social Work, Health, Health Reform , Project ethical and professional politician. Operationalized also a documentary research, on the Brazilian Public Health Policy, (SUS) and of the Education, as well as research field in which we conducted interviews with 11 social workers, employees packed the HUOL. We conclude that social workers did not participate in the discussion process of contracting the HUOL with the Municipal Health Secretariat of Natal, RN, manager of health and full resetting of user access, via reference setting - counter-referral services provided by the hospital brought the main demands on Social Work guidance regarding the functionality of SUS, and the social intervention in the struggle to guarantee such access. However, the data show that the expansion of demands that require the intervention of the social worker at HUOL is not associated with quantitative growth of these professionals need. Such conditions inflect the possibilities of materialization of the professional ethical-political project, even though that these professionals worry and seek the intellectual improvement, quality of service and to guarantee the social rights of users in the professional practice everyday
Esta pesquisa buscou analisar o trabalho dos assistentes sociais, no Hospital Universit?rio Onofre Lopes (HUOL), tendo como recorte anal?tico o processo de contratualiza??o do HUOL com o Sistema ?nico de Sa?de (SUS), que se configurou, a partir de 2004. Assim, este estudo procurou apreender os limites e possibilidades do trabalho do assistente social no HUOL, analisando como as determina??es deste tempo de reforma do Estado, reestrutura??o e tens?o entre amplia??o/redu??o dos direitos sociais e trabalhistas, rebatem no exerc?cio profissional dos assistentes sociais inseridos no processo coletivo de trabalho em sa?de. A partir de um referencial te?rico-metodol?gico materialista hist?rico e dial?tico, realizamos pesquisa bibliogr?fica, na qual desenvolvemos leituras e fichamentos de textos, artigos, livros, que enfocassem as categorias centrais do estudo, quais sejam: Trabalho, Servi?o Social, Sa?de, Reforma Sanit?ria, Projeto ?tico-pol?tico profissional. Abstract:
A small percentage of mentally ill patients, sometimes referred to as 'revolving door' patients, are frequently readmitted to psychiatric hospitals. This study explores how these patients construct meaning and reality and how this enacted reality provides a context for shaping their identities. The study draws on mental health policy, political ideologies and the history of deinstitutionalisation in order to illuminate this problematic phenomenon. The study is framed within the social sciences, but more specifically within the fields of mental health and social work practice. The methodology is qualitative, placing emphasis on a hermeneutic phenomenological approach. The foundation of the study is underpinned by a social construction and social psychology framework. A unique minimalist interview technique based on the Biographic Narrative Interpretive Method is used for data collection and analysis. Data from seven interviews with participants are presented followed by the researcher's reflections on the interviews and post-interview process. The life stories of four of the seven participants are analysed by using reflective teams. The lived life, or chronological chain of events as narrated, is analysed sequentially and separately. The told story, or thematic ordering of the narration, is then analysed using thematic field analysis; this involved reconstructing the participants' system of knowledge, their interpretations of their lives and their classification of experiences into thematic fields. An integrated procedure of abductive and inductive analysis of the stories elicited from participants offered immense potential for constructing meaning. Perspectives on 'revolving door' patients have often regarded them as having a one-dimensional life. This study revealed that these existential lives are complex and diverse and exist within a cultural matrix of social and psychological constructs. Interpretations of these patients' experiences illuminate the complexities arising from multiple admissions to psychiatric hospitals and highlight the problematic aspects which impact their socially constructed identities. These case studies of 'revolving door' patients' personally narrated lives extend the social psychological study of self/identity and contribute to the field of mental health research.

The purpose of this thesis was to write a grant to obtain funding for a comprehensive transitional patient care program and family caregiver support services. Through process analysis, the grant writer developed the Next Step Hospital to Home Caregiver Support Program (NSHCP). This program was developed to bridge the gap in the delivery of services of transitional patient care and family caregiver support services.

California Healthcare Foundation (CHCF) was identified as an appropriate funding agency. Their focus on ideas and innovations that improve quality medical care, increase efficiency, and lower the costs of medical care is compatible with the mission of the NSHCP. In collaboration with CHCF and Anaheim Regional Medical Center, the grant writer developed the NSFCP in an effort to provide quality transitional care and caregiver support services. The actual submission and/or funding of this grant was not a requirement for the successful completion of this project.

"Our society has generally viewed women as less aggressive and less likely to commit acts of violence in comparison to men. Statistics show that only 13 per cent of the violent crimes in the United States have been committed by women (Steffensmeier and Allan, 1996). However, employees at psychiatric hospitals often report problems of increased violent behavior among their female inpatients. This perception of the female inpatient population is further complicated by the mixed research findings regarding gender and violence. Considering the possible other environmental causations, gender alone should not prove an accurate variable in addressing violent behavior. Variables such as age, location, time, ethnicity, institutionalization and violence to self as opposed to violence to others will likely show significant relevance. This study will attempt to demonstrate how these other biopsychosocial variables play a role in violence prediction for this group of forensically committed patients other than gender."

This actually an investigation into the need for an Employee Assistance programme in the Jubilee Community Hospital. This programme is said to be effective in dealing with the employee's problems for it focuses on addressing all the social problems, which influence job performance. The study was conducted within the framework of a survey design. Three categories of questionnaires were designed and handed out to employees, supervisors and members of the management to complete. From their response it was clear that employees at this particular hospital experience problems which influence their job performance and are being left unattended. The study shows that there is no Employee Assistance Programme in this hospital. Most of the troubled employees sought help privately from the neighbouring social workers. This method of seeking help is not conviniet to the employees because it is not accessible and it is time consuming as they need to take a day's leave to attend to their problems. It is evident that there is a need for a qualified employee assistance practitioner in this hospital who will attend mainly to the employees' problems, who will be trained to do so. A formal policy with regard to EAP is also essential in this hospital.

Estudo de caso, descritivo e analítico, com abordagens quantitativa e qualitativa, fundamentado na Teoria das Representações Sociais e na Teoria do Núcleo Central, com o objetivo de apreender as representações sociais (RS) das enfermeiras de um hospital público de ensino, na cidade de Salvador-Bahia, sobre o trabalho noturno (TN) e analisar o processo de construção dessas RS a partir de sua estrutura. Participaram deste estudo 25 enfermeiras (89,3%) do serviço de assistência intermediária. Os dados foram coletados através das técnicas de Associação Livre de Palavras (TALP) e entrevista, em abril e maio de 2008. Os dados originários do TALP foram submetidos à análise fatorial de correspondência (AFC), pelo Software Tri-Deux Mots, e à análise do quadro de quatro casas, através do Software EVOC, e os da entrevista, à análise temática de conteúdo. A análise do quadro de quatro casas evidenciou que a estrutura da RS tem como elementos centrais responsabilidade, árduo, estresse, sofrimento e autonomia, e, como elementos periféricos, ética, necessidade financeira, dupla jornada, sobrecarga e iniciativa. A AFC revelada no jogo de oposições demonstrou no Fator 1 que as enfermeiras que trabalham nos turnos diurno e noturno simultaneamente estão em oposição às enfermeiras que trabalham no diurno e que tem um vinculo empregatício, evidenciando uma oposição entre os turnos de serviço. Diante disso, apreendeu-se das primeiras, as seguintes representações: estressante, autonomia, sofrimento, cansativo, dupla jornada e responsabilidade. Para essas últimas, o universo semântico presentado foi: dificuldade, dedicação, sacrifício, desgastante e dedicação. Em relação ao Fator 2, o procedimento de análise baseou-se na idade x tempo de serviço: as enfermeiras com idade igual ou maior que 41 anos e tempo de serviço superior a 20 anos, apresentou os seguintes campos semânticos: necessidade, sacrifício, dedicação, autonomia, dedicação, desgastante e dupla jornada; às evocações das enfermeiras com idade entre 30 e 40 anos e tempo de serviço de 10 a 19 anos foram: sofrimento, administração, continuidade, conhecimento, dificuldade e responsabilidade. A análise temática de conteúdo originou cinco categorias simbólicas, Concepções do TN (27,2%), Dimensão Ontológica do TN (16,7%), Aspectos Psicoafetivos do TN (20,6%), Viabilidade do TN (22,8%) e Valorização do TN (12,7%). Estes resultados conduzem à necessidade de reflexão das enfermeiras sobre essas representações para o desenvolvimento de estratégias de trabalho que contribuam com políticas de pessoal considerando a especificidade, subjetividade e complexidade do TN, com um modelo de prática inovadora, valorizando o TN e a trabalhadora, o enfrentamento dos problemas cotidianos e propiciando novas investigações para aprofundamento dessa temática.

Este estudo busca refletir sobre o trabalho cotidiano das(os) Assistentes Sociais, dentro dos pressupostos da Reforma Psiquiátrica Brasileira, na perspectiva de entender como estes profissionais podem, através do seu trabalho, dar materialidade ao Projeto Ético Político da categoria. O questionamento parte da reflexão sobre dois processos em construção no contexto da sociedade brasileira, e que têm seu ponto de convergência no exercício profissional das(os) Assistentes Sociais inseridas(os) no campo da Saúde Mental. São estes processos: A construção, a partir de um processo histórico de organização das(os) Assistentes Sociais brasileiras(os), de um Projeto Ético Político profissional hegemônico para a categoria; e a consolidação, no âmbito das Políticas Públicas brasileiras, de um processo de transformação das respostas sociais à problemática da “loucura” que, no âmbito da saúde, propõe o redirecionamento do modelo de atenção à saúde mental, denominado Reforma Psiquiátrica. Tendo em vista que ambos são processos complexos, inseridos no contexto social contraditório, cuja materialização pressupõe a reflexão e ação dos sujeitos sociais envolvidos, considera-se a existência de fatores, dificuldades, contradições que os condicionam, operando avanços e retrocessos. Ambos, Reforma Psiquiátrica e Projeto Ético Político, inscrevem-se no contexto de disputa de projetos societários, sujeitos ao jogo das forças políticas e econômicas. Deste modo, a Pesquisa tem a relevância de contribuir para o amadurecimento das discussões em torno do trabalho profissional da(o) Assistente Social, em suas relações interdisciplinares, no complexo campo da Saúde Mental, considerando o acúmulo ético – político, teórico – metodológico e técnico – operativo desta categoria. A Pesquisa buscou, pois, analisar como se constrói, no trabalho cotidiano das(os) Assistentes Sociais, o Projeto Ético Político, dentro dos pressupostos da Reforma Psiquiátrica Brasileira. Estabeleceu, como objetivos específicos: Identificar, no trabalho das Assistentes Sociais, no campo da Reforma Psiquiátrica brasileira, a construção dos elementos ético-políticos, teórico-metodológicos e técnico-operativos do Projeto Ético Político do Serviço Social; destacar os pontos de convergência e divergência entre a Reforma Psiquiátrica brasileira e o Projeto Ético Político do Serviço Social; Para tanto, foi realizado um desenho de Estudo de Caso analítico - descritivo, elegendo como campo um Hospital Psiquiátrico localizado em Salvador. Teve respaldo na Teoria da Estruturação de Anthony Giddens, utilizando-se da Etnografia. Como Instrumentos de Coleta de dados, foram utilizados entrevistas com Informantes Qualificados, além de pesquisa documental e bibliográfica. Os dados coletados foram analisados com base na Análise de Conteúdo (BARDIN, 2012). A triangulação dos dados define, como fio condutor da análise, a existência de uma disputa entre o Modelo Asilar e o Modelo de Atenção Psicossocial que atravessa a história da Instituição, sendo o Serviço Social signatário deste último.
This study aims to reflect on the nature of the daily work of Social Workers, considering the assumptions of the Brazilian Psychiatric Reform, within the perspective of understanding how these professionals, through their work, can implement the Ethical and Political Project of that profession. This questioning starts from the reflection on two processes in the construction of the Brazilian social context, related to the professional performance of Social Workers inserted in Mental Health. These two processes are: the construction, trough the historical process of the organization of the Brazilian Social Work, of an Hegemonic Professional Ethical Political Project for that profession; and the consolidation, in the context of Brazilian Public Policies, of the process of giving social answers to the problem of the “madness” which, in the health environment, proposes a new way to the mental health services, called Psychiatric Reform. Considering that the two processes are complex, inserted in a contradictory social context, whose realization involves reflection and action of the concerned social subjects, the reality of existing factors, difficulties and contradictions that are determining them, may create improvements and retrogressions. The psychiatric Reform as well as the Ethical Political Project are enrolled in the fight between opponent social projects, subjected to the political and economic forces in presence. In that way, this research should contribute to maturing discussions on the professional achievement of Social Workers, through interdisciplinary relations, in the complex field of Mental Health, considering the cumulative ethical – political, theoretical – methodological and technical-operational development of this profession.This research aims to analyse how is constructed, in the daily work of Social Workers, the Ethical Political Project, into the Brazilian Psychiatric Reform proposals. The specific objectives are: Identify, in the work of Social Workers, through the Brazilian Psychiatric Reform, the ethical-political, theoretical-methodological and technical-operational elements of the Ethical Political Project of Social Work; highlight the points of agreement or of divergence between the Brazilian Psychiatric Reform and the Ethical Political of Social Work. Therefore, an analytic and descriptive Case Study research design was used, selecting a Psychiatric Hospital of Salvador as research fieldwork, with the support of Anthony Giddens’s Structuration Theory, in an ethnographical perspective. As data collecting instruments were used interviews with key informants, apart documental and bibliographical reviews. The collected data were analysed with the Content Analysis Method of Bardin (2012). The triangulation of data sets, such as thread of the analysis, the existence of a dispute between the asylum Model and the Model of Psychosocial running through the history of the institution, with the signer latter Social Service.

Abstract:

The purpose of this project was to identify and create a program that would meet the needs of a target population, identify potential funding sources, and write a grant to fund a program at St. Mary's Hospital in Long Beach, California. An examination of the literature expanded knowledge about the challenges homeless individuals face. Homeless individuals commonly receive inadequate medical care and often access more costly acute care services due to limited resources, discrimination, and cost. Housing and case management services after discharge from a hospital setting can help improve health care outcomes and reduce costly services.

The program was designed to provide specialized discharge planning services to homeless individuals being discharged from St. Mary's Hospital. The California Wellness Foundation was chosen for this grant based on the program areas of the funder. The actual submission and/or funding of this proposal was not a requirement for completion of this project.

INTRODUCTION: This study is about knowing the perception of former students and preceptors relative to the Multidisciplinary Residency in Health at the Federal University of Goiás Public Hospital during the period of 2010 to 2012. OBJECTIVES: To analyze the perception of social workers (preceptors and former students) with respect to their performance, as well as understand the deployment and implementation process of the Multidisciplinary Residency in Health Program (PRMS as the portuguese acronym); identify the necessary skills to labor in the SUS and describe the difficulties encountered in the field of action. METHODS: A descriptive and exploratory study with qualitative approach. The instrument was a semi-structured interview with open questions recorded from five preceptors and twelve former social workers students. The transcribed interviews were submitted to content analysis proposed by Bardin and using the software ATLAS TI. The analyzes were performed taking into account the formation and the professional performance in residence and presented the factors that affect the field of action. RESULTS: It’s necessary to: acquire new knowledges to work in multidisciplinary teams; to develop specific competencies for preceptorship and for work in the SUS; to enhance inter-branch, multi-professional and interpersonal relationships. CONCLUSION: The analysis of the content of the interviews showed that the goals of the health multi-professional residence, specifically in Social Work, have been achieved, though, it is necessary to consider specific actions to improve skills that contribute to the exercise of the preceptor role and to work in the SUS focused on interdisciplinarity and integrality of health care.
INTRODUÇÃO: Este estudo busca conhecer a percepção de preceptores e residentes egressos acerca da Residência Multiprofissional em Saúde ocorrida no Hospital das Clínicas da Universidade Federal de Goiás no período de 2010 a 2012. OBJETIVOS: Conhecer a percepção de assistentes sociais (preceptoras e egressas) em relação à sua atuação, bem como compreender o processo de implantação e implementação do Programa de Residência Multiprofissional em Saúde (PRMS); identificar as competências necessárias quanto ao preparo para o trabalho no SUS e descrever as dificuldades encontradas no campo de atuação. MÉTODOS: Estudo descritivo, exploratório, com abordagem qualitativa. O instrumental utilizado foi entrevista semiestruturada com perguntas abertas, a cinco preceptoras e a doze assistentes sociais egressas. As entrevistas transcritas foram submetidas à análise de conteúdo proposta por Bardin utilizando o software ATLAS TI. As análises foram feitas levando em consideração a formação e a atuação profissional na residência e apresentados os fatores que interferem no campo de atuação. RESULTADOS: Há necessidade de aquisição de novos conhecimentos para o trabalho em equipe multiprofissional; desenvolver competências específicas de preceptoria e para o trabalho no SUS e aprimorar as relações interprofissionais, multiprofissionais e interpessoais. CONCLUSÃO: A análise das entrevistas demonstrou que os objetivos da residência multiprofissional em saúde no Serviço Social foram alcançados, embora, seja necessário pensar em ações específicas para o aprimoramento de competências que contribuam para o exercício da função de preceptor e para o trabalho no SUS focado na interdisciplinaridade e integralidade da atenção em saúde.
