Abstract:

This thesis investigates health trends among very old people and the allocation of public home-help services. A further aim is to examine methodological issues in mortality analysis. Three data sources are used: (1) The Tierp study of community-dwelling persons (n=421, ages 75+), (2) the SWEOLD nationally representative samples (n=537 in 1992 and n=561 in 2002, ages 77+), and 3) SNAC-K comprised of home-help recipients in a district of Stockholm (n=1108, ages 65+).

Study I suggests that the length of the follow-up period may explain some of the differences found in predictor strength when comparing mortality studies. Predictors that can change rapidly (e.g., health) were found to be strongest for the short term, with a lower average mortality risk for longer follow-ups. Stable variables (e.g., gender) were less affected by length of follow-up.

Studies II and III present a measure of complex health problems based on serious problems in at least two of three health domains. These were diseases/symptoms, mobility, and cognition/communication. Prevalence of complex health problems increased significantly between 1992 and 2002. Older age, female gender, and lower education increased the odds of having complex problems. Complex problems strongly predicted 4-year mortality. Controlled for age, gender, health, and education, mortality decreased by 20% between 1992 and 2002. Men with complex problems accounted for this decrease. Thus, in 2002 the gender difference in mortality risk was almost eliminated among the most vulnerable adults.

Study IV revealed that physical and cognitive limitations, higher age, and living alone were significantly related to home-help allocation, with physical and cognitive limitations dominating. Psychiatric symptoms did not affect the assessment.

The increased prevalence of complex health problems and increased survival among people with complex needs have important implications concerning the need for collaboration among service providers.

Abstract:
My study is concerned not simply with the what and the how of hone care for the elderly but also with the I ask about how the domiciliary services operate: what home helps do for elderly people and how they feel about their caring role, and, what the circumstances of elderly people needing care are and how they feel about using help. But I also want to know why home help operates in this way: why home helps care in the way they do and why elderly people feel as they do about using that care? Such an approach cannot fail to take into consideration the wider ecological and structural context within which elderly people and home helps live and work. Part one of my thesis, composed of three chapters, therefore provides this backcloth. I use it to introduce the location of the study, to present a brief history and discussion of the development of domiciliary services for the elderly in Britain, and 10 describe the philosophy and policy shaping domiciliary provision within Salford Social Services Department. As I shall show, current Government economic policy is inextricably woven into the fabric of this backcloth. In Part Two, I detail the findings of my fieldwork, painting a picture of the lives of the frail and impaired elderly people using domiciliary care, and of the work of the home helps providing that care. As far as possible, I have used the interviewees' own words to explain perceptions of (in)dependence and need, of stiqma and taboo, of material and ideological motivations, and of emotional involvements and commitments. A number of writers have argued that dependency - a concept at the centre of inquiries into the care of the elderly - is a socially constructed relationship, both with respect to elderly users and female providers of care. In Part Three, I question whether and in what way the evidence supports or denies this claim. I ask what are the implications of my findings for social policy. I also justify the use of anthropological perspectives in policy-related research. Finally, I present an account of my experience as a researcher which can be approached from a number of different levels., At a'basic level, it represents an immediate account of doing fieldwork. It is also my account, as a post-graduate, of the experience of writing-up a thesis. I consider the effect of the passing of time on context and consciousness and hci this feeds into the analysis and presentation, of work. And I attempt to address concerns with the writer/reader/subject relationship which pose questions to do with communication.

Abstract:

This paper studies, from a client perspective, how three Swedish municipalities work with information about a voucher system in homecare. The paper is a qualitative study using structured informant interviews and analysis of documents. The interviewees are responsible of information about the voucher system in the municipalities. The studied documents describe and clarify the voucher system of the municipality. Neo-institutional organization theory is used to analyse the result. Existing research has showed how pensioners find information on the voucher systems lacking. The research also shows how care managers have difficulties relating to their new roles as information coordinators on the voucher system. With this background, the study focuses on examining how the municipalities work with and organises the information on the voucher system as well as how the responsibiliy of the information is handled. The result shows that the studied municipalities mainly deal with the issue from a market point of view, where the care providers are to be presented in a neutral way. However, one of the municipalities has reconsidered the principle of neutrality and works to clarify the information to the elders. All three municipalities place the responsibility of information on the care managers in their interaction with the elder. There is no further communication between the municipality and the elderly people in the form of client studies or fora of discussion and review of the information on the voucher system.

Abstract:

The overall purpose with this study was to examine the influence elderly people have over their granted home help. Specifically, the aim was to examine in what extent the image of the elderly as a unique individual emerges in the plans where the home help is described [“genomförandeplaner”]. A plan which is personally designed is used as an indicator of personal influence. A text analysis inspired by Michel Foucault’s discourse concept, was carried out and the study adopts the concepts standardization and categorization as well as the theory of “Sense of coherence”. The result showed that the content of the plans mainly lacks a personal imprint, which is to be understood as a form of standardization. Furthermore, the analysis reveals that the content that is excluded and included creates an image of the elderly as someone who is not a unique individual. This study highlights the value of establishing more salutogenic infused plans. Also other studies, emphasize that elderly in need of care wants to be able to predict and influence the help they receive, which in turn is a condition for the elderly to achieve a Sense of coherence.

Uppsatsens syfte är att ur ett personalperspektiv undersöka och beskriva kunskapsläget i äldreomsorgsarbete inom hemtjänsten med brukare som har alkoholproblematik. Syftet belyses via följande frågeställningar; 1) Hur beskrivs arbete med vård och omsorg hos brukaren generellt i forskningen? 2) Hur påverkas brukarens självbestämmande och livskvalitet generellt vid behov av vård och omsorg från äldreomsorgen? 3) Hur beskrivs arbete med vård och omsorg hos brukaren med alkoholproblematik i forskningen? 4) Hur påverkas brukarens självbestämmande och livskvalitet generellt vid behov av vård och omsorg från äldreomsorgen om brukaren har alkoholproblematik? För att besvara frågeställningarna valdes selektiv kunskapsöversikt som består av 11 utvalda primärdokument. Syfte och frågor i vår uppsats analyserades och besvarades via socialkonstruktionistisk och DBO-teorin, en handlingsteori. Resultatet av uppsatsen visar att: relationen mellan personal och brukare är avgörande för brukarens självbestämmande och livskvalitet. Är relationen dålig påverkas självbestämmandet negativt då brukaren alltid kan ses vara i beroendeställning, har brukaren även alkoholproblematik kan relationen försämras då personalen i vissa fall inte vet hur de skall förhålla sig till dessa brukare. Relation byggs upp genom tid, kontinuitet och lojalitet. I ett föränderligt och flexibelt arbete som hemtjänstarbetet är, kan tid, kontinuitet och lojalitet vara en bristvara beroende på hur verksamheten styrs och är organiserad. Arbetet hos äldre med alkoholproblematik kan bli en växande problematik som äldreomsorgen inom varje kommun måste ta ställning till hur de skall hantera. Skall man acceptera olika livsstilar hos olika brukare eller skall de som är i behov av äldreomsorg dras över samma kam och alla får samma typ av insats? Hur skall framtidens äldreomsorg se ut?
Purpose of this thesis is that from a personnel perspective, examine and describe the state of knowledge in the home care work with elderly clients who have alcohol problems. The purpose is highlighted by the following questions: 1) How is work with health and care of elderly clients in generally described in the research? 2) How is the elderly client’s autonomy and quality of life in general affected when in need of home care from the eldercare? 3) How is work with health and care of clients with alcohol problems described in the research? 4) How is the elderly client’s autonomy and quality of life in general affected when in need of care from the eldercare if the patient has an alcohol problem? In order to answer the questions a selective knowledge review consisting of 11 selected primary documents was chosen. The purpose and questions in this thesis were analyzed and answered by social construction theory and DBO theory, an action theory. The result of the knowledge review shows that: the relationship between caregivers and elderly clients is crucial for the client’s autonomy and quality of life. If the relationship is bad it will affect the patient’s autonomy negatively as the client can be seen to be the dependent part, and have the client also a alcohol-related problem the relationship may deteriorate when the caregivers in some cases do not know how to respond to these clients. Relationships are built through time, continuity and loyalty. In a changing and flexible work as home care work is, time, continuity and loyalty could be scarce, much depending on how the eldercare is managed and organized. Work with clients with alcohol problems may be a growing problem that the eldercare in each community must decide how to deal with. Should different lifestyles be accepted among various clients or shall all clients who are in need of eldercare just get the same kind of help as everybody else with no acceptance of different needs? How shall the future eldercare be organized?

Abstract:
This thesis is a study of the role of the domiciliary home help and community auxiliary nursing services in England and Wales. The origins of the two services, interwoven as they are with political, social and demographic changes, are traced. Factors in the development of the services such as the effects of war, infant mortality, the professionalisation of nursing and medicine, the low status of domestic service, the role of voluntary organisations and the policies of successive governments in particular since 1948, are discussed. The parallel development of both services since 1948 is analysed. The increase in the numbers employed in the auxiliary nursing field, since the separation of nursing and the home help service in 1972, and its effect upon the role of the home help is examined. The concern at the possible duplication and overlapping of a role, because of the independent development of the two services since 1972, is also discussed. In an attempt to identify the actual tasks carried out by both groups of workers, I carried out a large study of the role of each group. A postal questionnaire, listing 110 tasks was completed by 1037 home helps in 20 Local Authorities and 1368 nursing auxiliaries in 81 District Health Authorities. The completed questionnaires were analysed under the headings of: Professional Nursing; Basic Nursing; Administrative; Domestic; Personal; Advisory; Escorting and Miscellaneous Tasks. The data collected on the role of the home help and auxiliary nurse is examined and areas of potential overlap discussed, along with an analysis of the perception that each group has of their role. The variation in practice in differing authorities is also discussed. The thesis is concluded by a summary and conclusions. In the appendices each group of tasks is analysed, in tables, by frequency, perception of role, age, sex, time in post, marital status, qualifications, political affiliation of each authority and type cf authority. A comparison of the number of tasks and the percentage of staff carrying them out in each authority is also included.

Depuis quelques années, une vague entrepreneuriale arrive sur le secteur de l’aide à domicile. L’objectif de ces nouvelles organisations est de transformer le secteur et revaloriser le métier d’auxiliaire de vie en proposant un modèle managérial fondé sur l’autonomie. Leurs discours se centrent sur l’idéalisation d’un nouveau secteur de l’aide à domicile à venir, diffusent un fantasme de fusion en renforçant le collectif. Ce travail de recherche propose, par une démarche clinique, d’interroger ces pratiques managériales à travers le concept d’imaginaire. Cette recherche vise à contribuer à l’approche critique du management en considérant l’organisation comme une fabrique d’images qui vient solliciter l’espace psychique de ses membres pour créer une dynamique d’adhésion fusionnelle. Nous proposons également de penser l’imaginaire individuel dans l’organisation et la manière dont il vient parfois se confondre avec l’imaginaire social et collectif. Nous observons qu’il existe des processus d’introjection importants dans les organisations étudiées où l’imaginaire se présente comme un moyen de produire du sens dans le réel, de se relier. Cette recherche présente trois articles pour comprendre la place de l’imaginaire dans le management et la manière dont il s’articule dans les discours. Le premier article explore l’imaginaire de ces entrepreneurs et l’implication de leur réalité psychique dans leur manière de gérer l’organisation. Le deuxième montre comment des mécanismes propres à l’imaginaire sont utilisés comme outil managérial en créant une dynamique que nous nommons « adhésion-confusion ». Enfin, le dernier article propose une contribution méthodologique par un récit auto-ethnographique et invite à tenir compte du rôle de l’imaginaire dans le travail du chercheur. La méthodologie de cette thèse porte une attention toute particulière du processus réflexif du chercheur. La subjectivité est un élément de réflexion essentiel ici. Alors que les chercheurs en sciences des organisations se contentent trop souvent d’évoquer l’imaginaire sans l’analyser, nous proposons une approche psychanalytique en sciences de gestion afin de comprendre et nommer les impensés de l’imaginaire dans l’organisation pour identifier, analyser et expliciter les processus qu’il engendre
For the past few years, an entrepreneurial wave has been arriving in the home care sector. The objective of these new organizations is to transform the sector and revalue the profession of home help by proposing a managerial model based on autonomy. Their discourse is centered on the idealization of a new sector of home help to come, and they spread a fantasy of fusion by reinforcing the collective. This research proposes, through a clinical approach, to question these managerial practices through the concept of the imaginary. This research aims to contribute to the critical approach of management by considering the organization as a factory of images that solicits the psychic space of its members to create a dynamic of fusional adhesion. We also propose to think about the individual imaginary in the organization and the way it sometimes merges with the social and collective imaginary. We observe that there are important introjection processes in the organizations studied, where the imaginary is presented as a means of producing meaning in the real, of connecting. This research presents three articles to understand the place of the imaginary in management and the way it is articulated in the discourses. The first article explores the imaginary of these entrepreneurs and the implication of their psychic reality in their way of managing the organization. The second article shows how mechanisms specific to the imaginary are used as a managerial tool by creating a dynamic that we call "adhesion-confusion". Finally, the last article offers a methodological contribution through an auto-ethnographic account and invites to take into account the role of the imaginary in the work of the researcher. The methodology of this thesis pays particular attention to the reflexive process of the researcher. Subjectivity is an essential element of reflection here. While researchers in organizational science are too often content to evoke the imaginary without analyzing it, we propose a psychoanalytical approach in management science in order to understand and name the unthinking of the imaginary in the organization in order to identify, analyze and explain the processes it generates

The purpose of this thesis is to design a new smart home for urban Chinese elderly people in home living environment. This new smart home design has to be integrated all the ICT solutions which based on a literature review was proposed and also with a functional back-end system for data collection, analysis and storage. The literature review phase of this study was to find out the existing problematic issues and solutions for urban Chinese elderly people in home living, upon comparison of two current Chinese smart home producers, it becomes clear that the current Chinese smart home may not completely meet the needs for home living urban Chinese elderly people. The design phase firstly introduces the ideas on conceptual model of new smart home design and then emphasises on involving all the ICT products, services and backend system into a real home living environment of urban Chinese elderly people. The evaluation of this research according to a scenario of how a general urban Chinese elderly people spends a day in this new smart home living environment and evaluate whether this smart home solve all the urban Chinese elderly home living issues and finally answer the research question. This research aims to show the future direction of new Chinese smart home design and may inspire the current Chinese smart home producers and help them improve their current smart home products.

The building ́s views are oriented to the Nissan river.The principal object of this project is to provide a place where old people can live with all the cares and where have the necessary personal to take care them.The building has only one floor, there are: dining room, laundry, bathrooms geriatric, gym, common toilets, staff areas, medical office, kitchen, storage area. The hall has very high roof with a big window, with view to the river, to get light in a very big area.Inside the buiding It have 8 double rooms and 6 single rooms, with bathroom included in each. Are also included geriatric bathrooms that satisface the needs of its users. There are two interior courtyard that give light to the corridor which serve to communicate all the rooms and the medical center area and gym. Also exists various free-use bathrooms and entertainment areas for the costumers.

Dans cette thèse, une approche combinant des données issues de capteurs hétérogènes pour la reconnaissance d'activités des personnes âgées à domicile est proposée. Cette approche consiste à combiner les données fournies par des capteurs vidéo avec des données fournies par des capteurs environnementaux pour suivre l'interaction des personnes avec l'environnement. La première contribution est un nouveau modèle de capteur capable de donner une représentation cohérente et efficace des informations fournies par différents types de capteurs physiques. Ce modèle inclue l'incertitude sur la mesure. La deuxième contribution est une approche, basée sur une fusion multicapteurs, pour la reconnaissance d'activités. Cette approche consiste à détecter la personne, suivre ses mouvements, reconnaître ses postures et ses activités d'intérêt, par une analyse multicapteurs et une reconnaissance d'activités humaines. Pour résoudre le problème de la présence de capteurs hétérogènes, nous avons choisi de réaliser la fusion à haut niveau (niveau événement) des différentes données issues des différents capteurs, en combinant les événements vidéo avec les événements environnementaux. La troisième contribution est l'extension d'un langage de description qui permet aux utilisateurs (ex. Le corps médical) de décrire les activités d'intérêt dans des modèles formels. Les résultats de cette approche sont montrés pour la reconnaissance des AVQ pour de vraies personnes âgées évoluant dans un appartement expérimental appelé GERHOME équipé de capteurs vidéo et de capteurs environnementaux. In this thesis, an approach combining heterogeneous sensor data for recognizing elderly activities at home is proposed. This approach consists in combining data provided by video cameras with data provided by environmental sensors to monitor the interaction of people with the environment. The first contribution is a new sensor model able to give a coherent and efficient representation of the information provided by various types of physical sensors. This sensor model includes an uncertainty in sensor measurement. The second contribution is a multisensor based activity recognition approach. This approach consists in detecting people, tracking people as they move, recognizing human postures and recognizing activities of interest based on multisensor analysis and human activity recognition. To address the problem of heterogeneous sensor system, we choose to perform fusion at the high-level (event level) by combining video events with environmental events. The third contribution is the extension of a description language which lets users (i. E. Medical staff) to describe the activities of interest into formal models. The results of this approach are shown for the recognition of ADLs of real elderly people evolving in an experimental apartment called Gerhome equipped with video sensors and environmental sensors. The obtained results of the recognition of the different ADLs are encouraging

Much of the developed world is experiencing an aging population. This requires society to adapt to take care of a growing elderly population and improve their quality of life. Today, mobile systems are available that makes it possible to monitor and improve health (m-health). Even though these systems could be immensely helpful for the elderly population, this has not been the primary demographic for the current m-health systems. This case study aimed at examining how persuasive technology (technology for changing behavior and/or attitude) can be used to promote usage of m-health applications by the elderly. For this purpose, a theoretical framework for supporting m-health systems is proposed. This framework consist of persuasive technology (for motivation and support for fulfillment of human needs), knowledge of elderly issues in interacting with mobile interfaces, smartphone usability heuristics and the Plan-Do-Study-Act (PDSA) cycle (to support goalsetting and incremental progress). To determine how persuasive technology can be used to motivate elderly and find effective strategies for this purpose, the case study examined health behavior, motivations for healthy behavior, attitude to health, general goal setting behavior, needs, preferences, technological experience and self-efficacy, as well as usage of m-health systems using qualitative and participatory methods. Methods used included semi-structured interviews, future workshop, revolutionary rapid prototyping and usability evaluation. The findings of the interviews and the future workshop suggested the primary motivation for healthy behavior was derived from social aspects. Thus, the most effective persuasive strategies for the elderly likely target their need for social belonging and socialization. Based on the findings, an interactive prototype was developed. The prototype proposed an m-health application with self-monitoring that implemented an elderly community around healthy behavior, with opportunities to earn digital rewards and challenge other users to competition. The interactive prototype was then used in a usability evaluation to gauge its usability by the elderly and revised in higher fidelity according to their feedback.

Abstract:
A descriptive study, using a retrospective chart review, investigated the mortality rates for 77 elderly nursing home residents who at the time of the study had been relocated between 10.5 and 13.5 months. Variables identified were age, sex, diagnosis, frequency of acute hospital admissions, nursing home placement, time interval between relocation and date of death, and number of years spent in a nursing home. Findings showed that 31 (40%) of the relocated subjects died within the 13.5 month post-relocation period. Sixteen (52%) of those deaths occurred within six weeks of relocation. Data revealed that mortality increased with age, from 14% for 60 years olds to 73% for those in their nineties. Age was the only variable found to be statistically significant between those who lived or died. The study's findings demonstrated the need for nursing assessment and surveillance of relocated nursing home residents.

The purpose of this study was to describe the characteristics of falls and risk factors for falls in elderly male veterans residing in a Veterans Affairs nursing home care unit. Risk factors included cognitive status, mobility status, and restraint use. Relationships between risk factors and falls were investigated using a two-phase descriptive correlational design. Nursing and medical records of residents who fell were reviewed retrospectively, and assessment of cognitive status was done using the Mini-mental Status Exam. The convenience sample included 20 male veterans, age 65 and older, who had at least one documented fall from the year of January 1991 through December, 1991. Results were not statistically significant; however, trends in the data were identified. Findings were clinically significant and validated literature on risk factors for falling.

Abstract:
The actual fluid intake of forty (40) residents of a nursing home was determined by an analysis of detailed three-day food and fluid intake records obtained by direct observation. Intake was compared to an established standard to determine the adequacy of fluid intake for this population. Subjects were grouped in two groups according to age (<85 years and >85 years) to determine whether age influenced fluid intake. In addition, data such as number and frequency of medications and dependency factors, such as ability to feed self, ability to communicate, ability to move and ability to make decisions, were collected and correlated.There was no significant difference between actual fluid intake and required fluid intake for the population a whole and for the two age groups. However, there were individuals who had very low fluid intakes suggesting other factors which influence fluid intake. There were positive correlations between fluid obtained from non meal feedings and frequency of medication delivery and number of medications delivered during a 24 hour period. In addition, there were positive correlations among the dependency factors. There was no correlation among any of the dependency factors and fluid intake for the group as a whole or for the two age groups.It was concluded from this study that this population of elderly nursing home residents obtained adequate fluid. Medication frequency and number appeared to influence the amount of fluid that a resident might obtain during non meal feedings. However, as a subject became more dependent, this factor did not affect the fluid intake of this population.

A dissertação Modelo de Banheiro Domiciliar para Idosos - Uma Abordagem Ergonômica, apresenta um modelo que fornece orientações para a construção de banheiros residenciais. Através de sugestões para adaptação da arquitetura interna do banheiro residencial, busca-se aqui contribuir para possibilitar a independência e autonomia dos usuários idosos durante a execução de suas tarefas de cunho íntimo. Este estudo tem como objetivo principal, contribuir para a instrução de idosos, seus familiares e geriatras; bem como contribuir para a orientação de profissionais da construção civil, com sugestões de diretrizes a serem somadas ao código de edificações da cidade do Rio de Janeiro. Foram realizadas entrevistas, tanto com idosos, quanto com geriatras, foi organizado um grupo de foco com idosos e, além disso, foram feitas apreciações ergonômicas em cinco banheiros similares de um mesmo edifício, com observações e registros fotográficos in loco, aplicação do método Corlett e de questionários visando o levantamento do perfil. Ao final foi desenvolvido um quadro do parecer ergonômico, sugerindo soluções/ recomendações preliminares para os problemas encontrados. Com o aumento contínuo da população de idosos no país, é importante que medidas preventivas sejam adotadas através da legislação e políticas públicas, a fim de oferecer a toda a população uma velhice ativa, como a Organização das Nações Unidas sugere desde 1982. Através da oferta de conforto e segurança física e psicológica a este segmento da população, bem como através do atendimento às suas necessidades específicas, pode-se alcançar o intuito da ONU.
The dissertation Home Toilet Model for Elderly People - An Ergonomic s Approach, proposes a model to guide the construction of residential bathrooms. Mainly presenting suggestions to adapt the average architecture of the residential bathrooms, this document wishes to contribute to the autonomy of the elderly users at the bathroom. The primarily objective of this study is help to educate elderly people, his relatives and doctors; as well as guide professionals that work in areas related to the civil construction by adding suggestions and guidelines at the construction code of the city of Rio de Janeiro . Interviews had been carried through, as much with aged, how much with doctor geriatrics, an aged focus group was organized, moreover, ergonomic appreciations in five similar bathrooms of one same building had been made, with comments and photographic registers in I lease, application of the Corlett method and questionnaires aiming at the survey of the profile. To the end a document was developed suggesting preliminary recommendations as solutions to stop of the joined problems. With the continuous increase of the population of aged in the country, it is important that writ of prevention the legislation and public politics are adopted through in order to offer to all the population an active oldness, as the Organization of United Nations suggests since 1982. This can only occurs, through offers of comfort and physical and psychological security to this segment of the population, as well as through the attendance to its specific necessities.

Survival status of 45 elderly skilled nursing care residents was examined over a 42 month period following involuntary interinstitutional relocation. Medical and relocation planning records provided data to examine survival status of residents in relation to focal and contextual stimuli. Results were compared to a relocation study previously conducted in the same community. Significant relationships existed between survival status and family support and participation in relocation planning event. A higher percentage of subjects survived who did not have family support and did not participate in planning events. A significant relationship was found between time intervals in which deaths of male and female subjects occurred. In the first nine months, 13 of 14 males died; six of the 14 females died. No significant relationships were found between survival status and age, gender, mobility, or dementia. Findings suggest the need for continued study of variables affecting survival status of relocated elders.

Homes are becoming increasingly connected as new technologies allow users to access media and information from any-device at anytime. Notebooks, HDTVs, smartphones, media servers, photo cameras, and video cameras, all form part of this new digital ecosystem where - the vision says - information and content will flow easily across devices, enabled by simple and intuitive user interfaces. These new home technologies are, however, often too complex for most users. Only "digital-natives" or technology savvy groups have the necessary skills, knowledge or confidence to adopt them and to use them effectively. For the rest, trying them becomes painful and frustrating. Moreover, the ecosystem itself adds confusion, given the large number of players involved and the many different kinds of relationships. Unless a dominant player gains enough power to establish a dominant digital home architecture, or this happens in some other way, most companies will continue to innovate around device-specific features that don't address the overall complexity of the complete systems that users have to work with. Digital help services can assist users by simplifying the selection, installation, learning and troubleshooting of new services and devices; facilitating the adoption of new convergent technologies. There is a broad range of potential services, including, for example, 'over the top' (OTT) television integration, smartphone mentoring services, WiFi network configuration and desktop support services. Communication service providers should pay close attention to digital help services as an opportunity to differentiate their offer, strengthen their relationship with end-customers, reduce customer support costs and simplify the adoption of bandwidth-intensive technologies. Moreover, digital help services can speed up the adoption of OTT television services, and companies can use them strategically. The technology help space is evolving and communication service providers need to figure out how they want to participate: offer help services themselves; partner or acquire a existing technology support company; and/or create an open marketplace for technology help services.

Recently, there has been a big surge of research and public interest in increasing the math capabilities and skills of American children. This paper serves as a literature review examining how the home environment, specifically parents, can help with that. This meta-analysis delves into the factors of maternal math talk, a parent’s own math anxiety, and the relationship between a parent and child in the context of a parent’s gender stereotypes and a parent’s perception on his or her child’s math abilities. Interventions, suggestions, and future implications are also discussed. This paper will hopefully bring needed awareness to parents about their roles in their child’s math development, abilities, and achievement.

Abstract This study is focused on the self-care of home-dwelling elderly people living in Slovenia. The study has two phases. The purpose of the first phase is to describe the self-care of home-dwelling elderly people living in Slovenia and factors connected to self-care. The purpose of the second phase is to describe the experiences of the elderly people's ability to manage at home. The aim of this study was to produce new knowledge about the self-care of home-dwelling elderly people living in Slovenia and the factors connected with it. The knowledge will be used to develop elderly care and support eldery people who live at home. The knowledge can be also used to educate nurses to care for the elderly people. The study was both quantitative and qualitative. In the quantitative study the sample consisted of 302 home-dwelling elderly people who were clients in domiciliary care. The selection criteria was: aged 75 years or over and the ability to communicate, no hearing problems, no severe mental problem/cognitive disability and gave full consent for their participation. The whole instrument consists of 91 items. A instrument covers background data, types of self-care, self-care orientation, life satisfaction, self-esteem and functional ability. In the qualitative study, 20 interviews were carried out and a qualitative analysis obtained. Data collection methods included open-ended questions concerning the following topics: background data, types of self-care, self-care orientation, life satisfaction, self-esteem and functional ability. Based on factor analyses, four factors were found which described the self-care. All other factors describe elderly people's perceptions concerning either the past or the future. The elderly people who are able to manage their daily activities/routines have a good functional capacity, good family relations, live qualitatively, accept the future healthily and clearly and are satisfied with their life because they can take care of themselves, but their self-esteem is not so high. Those elderly people who do not take care of themselves are abandoned; they are not satisfied with their way of life and have low-esteem. The results are going to be the basis for planning care and nursing care for all caregivers, especially community nurses. On the basis of this result the model of nursing and social care for home - dwelling elderly people living in Slovenia could be planned
Tiivistelmä Tutkimuksessa selvitetään Sloveniassa asuvien ikääntyneiden itsestä huolenpitoa. Tutkimus on kaksivaiheinen. Ensimmäisen vaiheen tarkoituksena oli kuvata slovenialaisten ikääntyneiden itsestä huolenpitoa ja siihen yhteydessä olevia tekijöitä. Toisen vaiheen tarkoituksena oli kuvailla ikääntyneiden kokemuksia itsestä huolenpidosta. Tutkimuksen tavoitteena oli tuottaa tietoa slovenialaisten ikääntyneiden kotona asuvien itsestä huolenpidosta. Tietoa voidaan käyttää tuettaessa ikääntyneitä asumaan kotona. Lisäksi sitä voidaan käyttää hoitotyöntekijöiden koulutuksessa. Tutkimuksessa käytettiin sekä määrällistä että laadullista lähestymistapaa. Määrällisen tutkimuksen otos oli 302 kotona asuvaa slovenialaista ikääntynyttä. Kriteerinä oli vähintään 75 vuoden ikä ja kommunikaatiokyky. Kyselylomake sisälsi 91 väittämää. Siinä oli taustatietokysymyksiä, itsestä huolenpitoon, itsestä huolenpitoon orientoitumiseen, elämään tyytyväisyyteen, itsearvostukseen ja toimintakykyyn liittyviä kysymyksiä. Laadulliseen tutkimukseen osallistui 20 henkilöä. He olivat antaneet suostumuksena haastatteluun tutkimukseen ensimmäisessä vaiheessa. Haastattelut sisälsivät avoimia kysymyksiä koskien itsestä huolenpitoa. Tulosten mukaan neljä faktoria kuvasi itsestä huolenpitoa muut faktorit kuvasivat ikääntyneiden suhdetta menneisyyteen ja tulevaisuuteen. Niillä ikääntyneillä, jotka hallitsivat päivittäiset toiminnot oli hyvä toimintakyky, hyvät perheenjäsenten väliset suhteet, he olivat elämään tyytyväisiä ja hyväksyivät tulevaisuuden. Heidän itsetuntonsa ei ollut hyvä. Ne ikääntyneet, jotka eivät pitäneet huolta itsestään olivat omapäisesti itsestä huolehtimiseen suhtautuvia. He eivät olleet elämään tyytyväisiä ja heillä oli matala itsetunto. Tutkimus tuloksia tullaan hyödyntämään kehitettäessä ikääntyneiden kotihoitoa. Tulosten perusteella tullaan kehittämään malli miten ikääntyneiden kotona asumista voidaan tukea

Aging population is a one of the key problems for the vast majority of so called "more economically developed countries" (MEDC). The amount of elderly people who suffer from multiple disease and require permanent monitoring of their vital parameters has increased recently resulting in extra healthcare costs. Modern healthcare systems exploited in geriatric medicine are often obtrusive and require patients presence at the hospital which interferes with their demand in independent life style. Recent developments on telecare market provide a wide range of wireless solutions for distant monitoring of medical parameters and health assistance. However, most of the devices are programmed for spot checking and operate independently from each other. There is still a lack of integrated framework with high interoperability and on-line continuous monitoring support for further correlation analyses. The current study is a step towards complete and continuous data collection system for elderly people with various types of health problems. Research initiative is motivated by recent demand in reliable multi-functional remote monitoring systems, combining different data sources. The main focus is made on fall detection methods, interoperability, real-life testing and correlation analyses. The list of main contributions contains (1) investigating communication functionalities, (2) developing algorithm for reliable fall detection, (3) multi-sensor fusion analyses and overview of the latest multi-sensor fusion approaches, (4) user study involving healthy volunteers and elderly people. Evaluation is performed through a series of computer simulation and real-life testing in collaboration with the local medical authorities. As a result we expect to obtain a monitoring system with reliable communication capabilities, inbuilt on-line processing, alarm generating techniques and complete functionality for integration with similar systems or smart-home environment.
En åldrande befolkning utgör ett av de viktigaste problemen för de allra flesta så kallade "mer ekonomiskt utvecklade länder" (MEDC). Mängden äldre människor som lider av multi-sjukdomar och kräver ständig övervakning av vitala parametrar har ökat på senare tid, vilket resulterar i ökade sjukvårdskostnader. Geriatrikens moderna sjukvårdssystem kräver ofta att patienterna är närvarande på sjukhuset, vilket kraftigt begränsar en självständig och oberoende livsstil. Den senaste utvecklingen på telemedicinområdet erbjuder ett brett utbud av trådlösa lösningar inom hälsovård för distansövervakning av medicinska parametrar. De flesta lösningarna innebär punktkontroll av enskilda parametrar och arbetar oberoende av varandra. Det saknas fortfarande integrerade lösningar med hög interoperabilitet och kontinuerlig on-line övervakningsstöd för att kunna genomföra ytterligare korrelationsanalyser. Detta arbete utgör ett steg mot ett fullständigt och kontinuerligt datainsamlingssystem för äldre personer med olika typer av hälsoproblem. Forskningsinitiativet motiveras av senaste tidens efterfrågan på tillförlitliga multifunktionella system för distansövervakning, som kombinerar olika datakällor. Huvudfokus utgörs av falldetektionsmetoder, interoperabilitet, verkliga tester och korrelationsanalyser. Listan över de främsta bidragen innehåller (1) att undersöka kommunikationsfunktionaliteter, (2) utveckla en algoritm för tillförlitlig falldetektion, (3) multisensor-fusion-analyser och översikt över multisensor-fusion-strategier, (4) en användarstudie med friska frivilliga äldre. Utvärderingen sker genom en serie av datorsimuleringar och tester i verklig miljö i samarbete med lokala hälso- och sjukvårdsmyndigheter. Målet är ett övervakningssystem med tillförlitliga kommunikationsmöjligheter, inbyggd on-line-bearbetning, tekniker för larmgenerering och funktionalitet för integration med liknande system eller i en smart hemmiljö.

Abstract:
The purpose of this study was to examine the effects of individualized reality orientation therapy on paranoid behavior in a nursing home resident diagnosed with dementia. There were two hypotheses, the first one hypothesized that reality orientation therapy would increase the resident's awareness of environment. The second hypothesized that reality orientation therapy would produce a decrease in verbal statements of paranoid delusions by the resident. This study was conducted over a nine week period. The design of the study was a single A-B case study. The results showed a decrease in statements by the resident of verbal abuse, physical abuse, and neglect. Findings indicated that the resident experienced improvement from the intervention.

The purpose of this study was to understand the related factors of self-paid for home-help service among well-fixed seniors in Taichung. 232 seniors who were 65 years old and above who using home help service in Taichung community were selected by systematic sampling.  Complete data was obtained on 205 subjects. Data were collected by interview in person with the self-developed structure questionnaire. The valid sample was 205 cases, with completing rate of 88.36%. The data was analyzed by frequency, chi-square test, and logistics regression.  The results of the study found, people would had more desire of home-Help service by self-paid as below: high educational level, high family’s monthly income, felt the service wouldn’t cost any burden, got the utilization’s information from family members, serious ADLs disability level and felt the service hours not enough to use .  The study suggests in the policy, the reimbursement level should be design according to people’s ability to pay and their need, and reducing financial barriers for the user. In the practical applications, home-help service could add more service items and causes the service programs flexibly, and to develop a home-based evaluation scale system for elderly people to avoid the unmet needs in the community. In the future, the study could applicant aualitative research more deeply to explore the subject in need of seniors and caregivers, and then to understand the need and willingness to use respite service among the severely disabled and his caregivers in the community.

The number of citizens’ average age have been prolonged and the population reached higher age which indicates old age population is on rise and the need and demand on long-term care increasing gradually in recent years. However, the quality of life style for senior citizens is still living with their family and in their own community. In addition, the family function of caring order people is deteriorated which is due to family situation changed and growing the number of females have taken part in the labor market etc. Therefore, the issue of Home Help Services is becoming one of key importance in the field of caring for elderly persons. Kaoshiung Country provides Home Help Services for elderly since 1999. However, it is not a universal service to elderly people during the last five years. Therefore, it is hope to understand and explore the related factors of Home Help Services for older persons from the research study, and to discuss the related factors of Home Help Services in more detail and highlighted which areas need to be developed in the future Home Help Services project. This, therefore, the research have focused on the caring for elderly persons in the field of Home Help Services in 2003 by carrying out the method of census and interviews. Furthermore, the findings have shown that only 376 elderly persons were having the Home Help Services. In addition, the findings shows that more female elderly or illiterate older people and elder persons who live alone or only have one single carergiver at home are likely to obtain Home Help Services. Moreover, the point is depended on the term of how much allowance they get and it means that more services would be offered when there have more allowance. However, the rate of user-pays presents is very low and this means the principle of user-pays still has not established effectively in Kaohsiung country. In this research, it applied the dichotomized variable to analyse the individual character, family care support system, personal health status and Home Help Services project in relation to the person’s care provision. Nonetheless, the research also indicates that there have closed relationship with the individual character in relation to marital status and Home Help Services related to service users. In term of family care resources, if lack of family care support system and few child family that would be increased the demand of Home Help Services. On the other hand, it can be seen that, people have higher satisfaction at caring services when the time of family care is increased. In term of personal health, if the person who is lack of self- awareness and suffers certain degree of ADL and IADL that would more likely to obtain Home Help Services. In term of Home Help Services allowance project, the research also found out that the government allowance projects would influence on applying the different kind of Home Help Services. Finally, according to suggestions from government sectors, carergivers and older persons, the purpose of the research is to develop a high quality of Home Help Services for the need of elderly persons through the government and family’s contribution.

Abstract:
While a lot of efforts have been on outdoor intelligent systems, internal living environment system that suits the occupancy’s behaviour has not received much attention. The intelligent living environment designed in this study has three components; the physical world (environment), the database and the decision maker. The study sought to design a model that senses ever changing home conditions such as lights, doors and windows. Other variables that were looked at include, but not limited to the number of people in the room and inside thermodynamics and human activity. Global information such as temperature, gas or electricity usage and time of the day will also be received by the system through various sensing facilities. The information will be sent to a rules engine for a decision on an appropriate action to be taken. The action may include just turning off the lights, in the case of a mild abnormality or a high alert to an emergency response unit in a most severe case. The study proposes a context aware and proactive neural networks control system to control a living environment with a main focus on the aged citizens living alone. The proposed living environment was not developed to an actual or “mock” building containing a representation of subset of sensors, actuators and controllers as used in the actual systems due to lack of funding. However, the study will report on the modelling and simulation of the home system variables based on the chosen Artificial Intelligent technique using MATLAB/SIMULINK. These results indicate a possibility of implementing the designed living environment to increase the resident’s security.
