Abstract:
Tuberculosis continues to represent a severe public health problem in the Region of the Americas, even more so in the case of indigenous peoples, whose TB incidence is much higher than that of the general population. To achieve tuberculosis control in these communities, it is necessary to respond to communities’ diverse needs from an intercultural perspective that allows the application of a holistic approach—from a standpoint of equality and mutual respect—and considers the value of their cultural practices. In the Region of the Americas, although there has been progress toward recognizing the need for an intercultural approach to health services, obstacles rooted in discrimination, racism, and the exclusion of indigenous peoples and other ethnic groups persist. To respond to this situation, the Pan American Health Organization (PAHO) prepared this guidance which––based on an intercultural approach in accordance with the priority lines of the current PAHO Policy on Ethnicity and Health and its practical development in the Region’s indigenous populations––represent a support tool for implementing the End TB Strategy. This publication integrates PAHO’s accumulated experience and best practices developed by its Member States in recent years, including discussions and experiences shared in regional meetings on the issue, and emphasizes innovation and social inclusion. This requires an urgent shift away from traditional paradigms, taking specific actions that gradually reduce TB incidence and moving toward effective multisectoral actions that have proven effective in quickly containing the epidemic. This publication integrates PAHO’s accumulated experience and best practices developed by its Member States in recent years, including discussions and experiences shared in regional meetings on the issue, and emphasizes innovation and social inclusion. Some pairs of virtues are complementary, protecting their holders from the excesses of each. Here it is argued that faith and humility are such balancing characteristics.

The world is currently in the midst of unprecedented challenges—from the impacts of climate change and the humanitarian crisis of forced migration, to the rise of nationalism and epidemic growth of deaths of despair. These challenges require new approaches catalyzing communities, cities, and countries around the globe to embrace a well-being agenda to assess progress and guide solutions. Thus, this book provides ideas and guidance on advancing well-being locally, nationally, and internationally. It illuminates how diverse communities and cultures can work together to strengthen these efforts. Ultimately, the well-being framework offers an equity focus; a more human centered view of how things are going; holistic approaches; and interconnectedness. The goal here is to advance global dialogue and action on the well-being construct, and to inform the Robert Wood Johnson Foundation’s (RWJF) work with others to create a Culture of Health in the United States.

The association between mental health and physical health forms the core of this book. While it is recognized that serious mental illnesses such as schizophrenia carry a reduced life expectancy, it is often assumed that suicide is the main cause of this disparity. But in actuality, suicide accounts for no more than a third of the early mortality associated with schizophrenia: the vast majority is due to cardiovascular factors. This book seeks to put this stark fact in context, detailing the extent of cardiovascular risk, sharing information regarding reasons for this excess, and outlining approved approaches for screening for and treatment of such risk factors in people with schizophrenia. As such, this book seeks to inform those caring for people with schizophrenia of these parameters and suggests ways in which they may be addressed, using a holistic model which embraces shared decision-making and which is compatible with the recovery framework. It provides guidance regarding monitoring as well as information about focused interventions that can help ameliorate risk. It also addresses those physical health factors apart from cardiovascular, that add to the burden of ill health amongst people with schizophrenia: pulmonary health, bone health, sexual health, and cancer risk are just some of these. In addition, the book provides patient and carer information material that can be used to try to ensure that all involved have a truly informed role in decision-making about their treatment and that both psychiatric and physical health issues are taken seriously.

Organic farming is a major global movement that is changing land use and consumer habits around the world. This book tells the untold story of how the organic farming movement nearly faltered after an initial flurry of scientific interest and popular support. Drawing on newly unearthed archives, Barton argues that organic farming first gained popularity in an imperial milieu before shifting to the left of the political spectrum after decolonization and serving as a crucial middle stage of environmentalism. Modern organic protocols developed in British India under the guidance of Sir Albert Howard before spreading throughout parts of the British Empire, Europe, and the United States through the advocacy of his many followers and his second wife Louise. Organic farming advocates before and during the Second World War challenged the industrialization of agriculture and its reliance on chemical fertilizers. They came tantalizingly close to influencing government policy. The decolonization of the British Empire, the success of industrial agriculture, and the purging of holistic ideas from medicine sidelined organic farming advocates who were viewed increasingly as cranks and kooks. Organic farming advocates continued to spread their anti-chemical farming message through a small community that deeply influenced Rachel Carson’s ideas in Silent Spring, a book that helped to legitimize anti-chemical concerns. The organic farming movement re-entered the scientific mainstream in the 1980s only with the reluctant backing of government policy. It has continued to grow in popularity ever since and continues to inspire those who seek to align agriculture and health.
