Abstract:
SummaryHeparin induced thrombocytopenia (HIT) is a well-known complication of heparin administration but usually resolves upon discontinuation without sequelae. However, a small proportion of HIT patients develop thrombosis associated with HIT, designated as HITT, which is often life-threatening and may lead to gangrene and amputations. Existing laboratory methods of confirming HIT/HITT do not distinguish between HIT and HITT. We report a flow cytometric assay of platelet activation marker CD62P to distinguish the effects of addition of HIT vs. HITT plasma to normal blood. Briefly, normal whole blood was incubated with platelet-poor plasma from 12 patients with HITT, 30 with HIT, and 65 controls, in presence and absence of heparin, and expression of CD62P was assayed by flow cytometry. When the ratios of fluorescent intensity of CD62P with heparin divided by that without heparin were compared, HITT plasma induced significantly higher ratios than HIT plasma (HITT ratios ~2.5 vs. HIT ratios ~1.2; p <0.001). Eleven of 12 HITT patients were positive by this test but only 5 of 30 HIT patients were positive (p < 0.0005). In a case of HIT with silent thrombosis, this assay gave a positive results prior to clinically evident thrombosis. In conclusion, this method distinguishes HITT from HIT and may be clinically useful in the detection of HITT, allowing early intervention for preventing catastrophic thrombosis.

Abstract:
The purpose of this study was to determine the development of physiological aspects of increasing aerobic and anaerobic performance in students who become badminton athletes through optimizing footwork exercises using the high-intensity interval method (HITT). This research will provide a solution for trainers in combining and optimizing footwork exercises as a technique in badminton combined with the principles of interval training to increase badminton athletes' aerobic and anaerobic capacity. This study used an experimental approach by giving footwork training treatment (HITT) to 30 Padang State University students who became badminton athletes. Researchers saw the effect of the exercise given on aerobic capacity (VO2max) and anaerobic capacity. The statistical analysis results showed that footwork exercise with The HIIT (High High-Intensity Interval Training) method positively affects badminton athletes' maximal aerobic capacity (VO2max) anaerobic capacity. In this concept, footwork training using the HIIT method effectively develops aerobic and anaerobic metabolism in response to energy requirements during total energy production in training maximum.

Abstract:
Abstract Heparin-induced thrombocytopenia and thrombosis (HITT), a severe immune-mediated drug reaction, is often underdiagnosed. Early management of HITT with alternative anticoagulants can result in a favorable outcome. However, the morbidity and mortality associated with HITT remain high, posing challenges for patient management. We describe the clinical profile and pattern of platelet count recovery in a cohort of 34 patients diagnosed with HITT. This retrospective study consists of 164 patients screened for HITT over 18 months at our institution based on positive platelet factor 4 (PF4) IgG antibodies in the proper clinical setting. Patients who tested positive were divided on the basis of recovery of platelet count (increase to >100 × 109/L or by 1.5-fold by day 5 of diagnosis) into two groups: delayed and normal recovery. Of the 164 patients screened, 107 were admitted with cardiac diseases while 57 non-cardiac conditions. A total of 34 patients (21%) were tested positive for PF4 antibodies. Patients with underlying cardiac conditions were more likely to be diagnosed with HITT than non-cardiac patients (28% versus 7%, p=0.0012). The male to female ratio was 1:1.1, and the median age was 65 years. Nineteen of 34 patients (56%) had delayed platelet recovery, while the remaining 15 patients (44%) had normal platelet recovery by day 5. The mortality rate was 68% (13 of 19) in the delayed recovery group, compared to only 7% (1 of 15) in the normal recovery group (p=0.0004). Twelve of the 34 patients with liver dysfunction had delayed platelet recovery and died (figure 1). There were 6 venous and 5 arterial thrombotic episodes. A total of 13 patients (38%) were treated with Argatroban, a direct thrombin inhibitor (DTI) indicated for the treatment of HITT, but it did not seem to improve platelet recovery (Table 1). Eight of the 13 treated patients (62%) and 11 of the 21 untreated patients (52%) showed no platelet recovery by day 5. In conclusion, early diagnosis of HITT is critically important for patient management, and yet, diagnosis is often elusive due to a general lack of awareness of this condition. Underlying cardiac diseases appear to predispose patients to HITT, probably due to previous exposure and sensitization to heparin. Despite the availability of new anticoagulants, HITT remains an undertreated and highly fatal condition with overall mortality of 38% in our series. Underlying liver dysfunction appears to be associated with delayed platelet recovery and poor survival. Affect of DTI on platelet recovery needs to be further investigated. Optimal response to therapy requires early diagnosis and intervention. Further studies are necessary to enhance our understanding of this devastating condition to facilitate early diagnosis and proper treatment. Abstract:
Thrombosis is a life-threatening complication that occurs in a subset of patients with heparin-induced thrombocytopenia (HITT). The pathogenic mechanisms underlying the variable occurrence of thrombosis in HITT is poorly understood. It was hypothesized that monocyte activation leading to tissue factor expression may play a role in promoting a thrombogenic state in HITT. This study demonstrates that a human platelet factor 4 (PF4)/heparin-specific murine monoclonal antibody (KKO) binds to peripheral blood-derived human monocytes in a PF4-dependent manner. KKO and antibodies from patients with HITT induce monocytes to synthesize and secrete interleukin-8 and induce cell-surface procoagulant activity, which is abrogated following treatment with antihuman tissue factor antibody. The findings suggest a novel mechanism by which PF4/heparin antibodies may promote a hypercoagulable state in patients with HITT.

Abstract:
Abstract Introduction: What was once an under-recognized clinical entity, heparin induced thrombocytopenia and thrombosis (HITT) has become a well-recognized and frequently tested disease. HITT is a serious and potentially fatal antibody mediated drug reaction to platelet factor 4 and early identification and treatment is essential. The 4T score is well-validated to guide appropriate testing of HITT where a low-probability score of 0-3 equates to a 99.8% negative predictive value for HITT2,3. In an effort to promote cost-conscious care and efficient use of healthcare resources, the American Society of Hematology (ASH) recommends against testing for HITT if the 4T score is 3 or less4. HITT laboratory testing at our institution is not restricted based on 4T score. We hypothesize that many providers are not following cost-conscious guidelines regarding HITT testing at our institution. Methods: We performed a single-institution retrospective analysis of patients who had a heparin-induced platelet antibody assay with reflexive serotonin release assay ordered between July 1, 2016 to July 1, 2017 at the University of Virginia Medical Center (UVA). We primarily assessed how our institution's ordering of heparin-induced platelet antibody for HITT compared to ASH's Choosing Wisely recommendation of forgoing laboratory testing on patients with a 4T score of 0-3 by retrospectively calculating 4T scores on all patients who had laboratory testing4. Patients were also assessed for episodes of bleeding and clotting and were scored on severity via the CTCAE and ISTH grading systems. Data on anticoagulant used after HITT testing was collected. We also checked to see if the blood specimen for assay was collected 2 hours after last heparin product as recommended by lab. Patients were excluded if they were not inpatient when their heparin-induced platelet antibody assay was drawn or if they were transferred or discharged immediately after assay was collected. Ultimately, of the initial 196 patients who had heparin-induced platelet antibody assay collected during this time frame, 184 patients were included for analysis. Results: Of the 184 patients who had a heparin-induced platelet antibody assay sent and were included in analysis, 55.4% of the patients (n=102) had a low pre-test probability of HITT with a 4T score of less than or equal to 3. Of the patients who had HITT testing sent, only 44% (n=81) received treatment with a non-heparin anticoagulant. Of the 102 patients who had a low pre-test probability of HITT with a 4T score of ≤3, 37.3% (n=38) were placed on an alternative anticoagulant. Of this low pre-test probability of HITT cohort, 7.8% (n=8) experienced bleeding as a complication. Interestingly, 15.5% (n=29) of all patients who had HITT testing continued to receive heparin products while awaiting results. Additionally, 19.3% (n=36) of samples were drawn within 2 hours of receiving heparin products. Conclusion: The guidelines for HITT testing and treatment have been well-validated and widely disseminated. Despite providers' familiarity with this clinical entity, the results depict that ordering practices at our institution do not follow guidelines in cost-conscious ordering nor in standard of treatment. Applying ASH's Choosing Wisely recommendation of not ordering laboratory testing on patients with a 4T low-probability score of 0-3, we see that 55.6% of the HITT assays ordered in this time period were inappropriate and at a cost of $455 to the institution per assay resulted in $46,865 of unnecessary health care costs to our institution in one year's time. This does not include the cost of alternative anticoagulation. Heparin costs just $0.04 per mL while argatroban costs $3.81 per mL and bivalirudin $12 per mL resulting in a 100 to 300 fold cost increase respectively. Standard work regarding HITT assay collection and treatment does not exist at our institution. Of concern, 15.5% of patients continued to receive heparin products after HITT testing was sent. The results of this study prompted implementation of a quality improvement project to decrease inappropriate HITT testing and standardize treatment of suspected HITT via an electronic medical record order set that uses the 4T score to suggest appropriate ordering of assays. We plan to collect data on changes in our ordering practices after this intervention. Disclosures No relevant conflicts of interest to declare.

Abstract:
Objective. High intensity interval treadmill training (HIITT) has been gaining popularity for gait rehabilitation after stroke. In this study, we examined the changes in excitability of the lower limb motor cortical representation (M1) in chronic stroke survivors following a single session of HIITT. We also determined whether exercise-induced changes in excitability could be modulated by transcranial direct current stimulation (tDCS) enhanced with a paretic ankle skill acquisition task.Methods. Eleven individuals with chronic stroke participated in two 40-minute treadmill-training sessions: HIITT alone and HITT preceded by anodal tDCS enhanced with a skill acquisition task (e-tDCS+HIITT). Transcranial magnetic stimulation (TMS) was used to assess corticomotor excitability of paretic and nonparetic tibialis anterior (TA) muscles.Results. HIIT alone reduced paretic TA M1 excitability in 7 of 11 participants by ≥ 10%. e-tDCS+HIITT increased paretic TA M1 excitability and decreased nonparetic TA M1 excitability.Conclusions. HIITT suppresses corticomotor excitability in some people with chronic stroke. When HIITT is preceded by tDCS in combination with a skill acquisition task, the asymmetry of between-hemisphere corticomotor excitability is reduced.Significance. This study provides preliminary data indicating that the cardiovascular benefits of HIITT may be achieved without suppressing motor excitability in some stroke survivors.

Abstract:
The efficiency of the financial markets is something that has been studied through many perspectives. What the efficient market hypothesis says is that new information that reaches the market fast incorporates in the stock price without any chance for the investors to act on it. This results in no chance in arbitrage opportunities hence no chance to positive abnormal return. This study's way of examining this phenomenon was to study the return on the implementation of stock splits which has been implemented on the swedish stock exchange to see if any positive abnormal return exists. This event study has examined stock splits during the periods of year 2000 - 2006 and 2015 - 2019 and statistical tests have been made. These different periods of time have at first been examined by themselves before comparing them with each other to see if there might be any different between them. This study shows that there are significant positive abnormal returns associated with stock returns on the swidish stock exchange during both periods. The study also shows tendencies of that the abnormal returns differ between the two time periods.

Abstract:
Immune heparin-induced thrombocytopenia (HIT) is a potentially serious complication of heparin therapy and is associated with antibodies directed against a complex of platelet factor 4 (PF4) and heparin. Early diagnosis of HIT is important to reduce morbidity and mortality. I developed an enzyme immunoassay that detects the binding of HIT IgG to PF4-heparin in the fluid phase. This required techniques to purify and biotinylate PF4. The fluid phase assay produces consistently low background and can detect low levels of anti-PF4-heparin. It is suited to testing alternative anticoagulants because, unlike in an ELISA, a clearly defined amount of antigen is available for antibody binding. I was able to detect anti-PF4-heparin IgG in 93% of HIT patients. I also investigated cross-reactivity of anti-PF4-heparin antibodies with PF4 complexed to alternative heparin-like anticoagulants. Low molecular weight heparins cross-reacted with 88% of the sera from HIT patients while half of the HIT sera weakly cross-reacted with PF4-danaparoid (Orgaran). The thrombocytopenia and thrombosis of most of these patients resolved during danaparoid therapy, indicating that detection of low affinity antibodies to PF4-danaparoid by immunoassay may not be an absolute contraindication for danaparoid administration. While HIT patients possess antibodies to PF4-heparin, I observed that HIT antibodies will also bind to PF4 alone adsorbed on polystyrene ELISA wells but not to soluble PF4 in the absence of heparin. Having developed a technique to affinity-purify anti-PF4-heparin HIT IgG, I provide the first estimates of the avidity of HIT IgG. HIT IgG displayed relatively high functional affinity for both PF4-heparin (Kd=7-30nM) and polystyrene adsorbed PF4 alone (Kd=20-70nM). Furthermore, agarose beads coated with PF4 alone were almost as effective as beads coated with PF4 plus heparin in depleting HIT plasmas of anti-PF4-heparin antibodies. I conclude that the HIT antibodies which bind to polystyrene adsorbed PF4 without heparin are largely the same IgG molecules that bind PF4-heparin and thus most HIT antibodies bind epitope(s) on PF4 and not epitope(s) formed by part of a PF4 molecule and part of a heparin molecule. Binding of PF4 to heparin (optimal) or polystyrene/agarose (sub-optimal) promotes recognition of this epitope. Under conditions that are more physiological and sensitive than previous studies, I observed that affinity-purified HIT IgG will cause platelet aggregation upon the addition of heparin. Platelets activated with HIT IgG increased their release and surface expression of PF4. I quantitated the binding of affinity-purified HIT 125I-IgG to platelets as they activate in a plasma milieu. Binding of the HIT IgG was dependent upon heparin and some degree of platelet activation. Blocking the platelet Fc??? receptor-II with the monoclonal antibody IV.3 did not prevent HIT IgG binding to activated platelets. I conclude that anti-PF4-heparin IgG is the only component specific to HIT plasma that is required to induce platelet aggregation. The Fab region of HIT IgG binds to PF4-heparin that is on the surface of activated platelets. I propose that only then does the Fc portion of the bound IgG activate other platelets via the Fc receptor. My data support a dynamic model of platelet activation where released PF4 enhances further antibody binding and more release.

This Researchis about history education and is directed toward the understanding of teacher practices in schools of the basic education in Cear?-Mirim, a city located in the state of Rio Grande do Norte. Its purpose is to understand the approaching forms of local history in 5? to 8? grade classrooms in the light of the recent innovations in the fields of the historiografy and education. The study was done with a view of investigating local history is being taught by teachers, as well as reflecting on the necessary conditions to the accomplishment of a teaching whose know how to make possible to break, on the one hand, with the limits of the narrow `local view`, and, on the other hand, with the globalized view, negating the local particularitities and especifications. Such questions had emerged as the understanding of what local history contents can constitute in a significant component in the production of school historical knowledge in 5? to 8? grade classrooms. History education is analyzed, considering the depositions of three teachers of Cear?-Mirim in reagard to its historiography conceptions and history as a school subject. This inquiry is of a qualitative nature and had as a main strategy of data construction from the interviews with the teachers. The analysis indicates the permanence of teacher practices who, even though presenting innovations, bring an implicit value hierarchy where the place or thematic places are not contemplated in the school contents or appear overwhelmed by general history and the Brazilian history, configuring itself as a hierarchical relation to problematic historical ones. Thus the necessity of having historical school knowledge, that considers the local especifications, without, however, ignoring the articulations with other spatial dimensions

The present research, located in the Pontificia Universidade Cat?lica de Campinas Psychological Prevention and Intervention line of research in the Post-graduation stricto senso program, aims to understand how the work category presents itself to those who have the streets as their main subsistence means and constantly wander from town to town surviving through socially disqualified activities, typical of the streets universe, with a special focus on the trecheiros , a category that describes the street population known to wander constantly from place to place. This study was developed throughout six months by means of Ethnography, in parks and public circulation places in the state of S?o Paulo. The data was collected and later analyzed using Social Psychology It was possible to conclude that there is a wide range of people who, despite occupying the same economical condition, have culturally distinct profiles as well as substantially different means of experiencing the constant wandering through towns and the (non) use of social welfare offered by the state to those who live in the streets. It was also possible to conclude that work presents itself to them as a category constitutive of their subjectivity, given that they call themselves workers in order to state their honesty and social relevance. Work is also a daily activity of survival maintenance without the pretention to accumulate capital or material possession, but to provide the necessary for one day only, situation that leads such category to a constant instability and great vulnerability, given that they are not covered by the welfare social net useful in case there is an impossibility to work, momentarily or not .

Abstract:

Sarkar, Prasenjit 1970. "Hint-based cooperative caching." Diss., The University of Arizona, 1998. http://hdl.handle.net/10150/288892.

Full text
Abstract:
This dissertation focuses on caching in distributed file systems, where the performance is constrained by expensive server accesses. This has led to the evolution of cooperative caching, an innovative technique which effectively utilizes the client memories in a distributed file system to reduce the impact of server accesses. This is achieved by adding another layer to the storage hierarchy called the cooperative cache, allowing clients to access and store file blocks in the caches of other clients. The major contribution of this dissertation is to show that a cooperative caching system that relies on local hints to manage the cooperative cache performs better than a more tightly coordinated fact-based system. To evaluate the performance of hint-based cooperative caching, trace-driven simulations are used to show that the hit ratios to the different layers of the storage hierarchy are as good as those of the existing tightly-coordinated algorithms, but with significantly reduced overhead. Following this, a prototype was implemented on a cluster of Linux machines, where the use of hints reduced the average block access time to almost half that of NFS, and incurred minimal overhead.

Abstract:
A Hint of Meaning contains a scholarly preface, "Language, Experimentation, and Craft: Creating a Vivid, Continuous Fictional Dream," that discusses the ambiguities of language and how they relate to different aspects of the craft of writing. Six original short stories follow the preface. "Musical Chairs" explores a woman's conflicting emotions about her ex-husband. "Baby Steps" depicts the struggle of a woman against her father's alcoholism. "Go Home Happy" depicts a day in the life of a video store employee. "Bargain Basement Perfection" contrasts the reality of a relationship with an imagined, perfect relationship. "Did You Hear about Donald and Bitsy?" is an experimental piece that tells a story through gossip. "Glass Angels" explores a minister's relationship with his homosexual son and how that relates to the minister's faith.

Abstract:

"A Data Analytics Framework for Analyzing the Effect of Frac Hits on Parent Well Production." In SPE Hydraulic Fracturing Technology Conference and Exhibition. SPE, 2021. http://dx.doi.org/10.2118/204160-ms.

Full text
Abstract:
Abstract Well interference, which is commonly referred to as frac hits, has become a significant factor affecting production in fractured horizontal shale wells with the increase in infill drilling in recent years. Today, there is still no clear understanding on how frac hits affect production. This paper aims to develop a process to automatically identify the different types of frac hits and to determine the effect of stage-to-well distance and frac hit intensity on long-term parent well production. First, child well completions data and parent well pressure data are processed by a frac hit detection algorithm to automatically identify different frac hit intensities and duration within each stage. This algorithm classifies frac hits based on the magnitude of the differential pressure spikes. The frac stage to parent well distance is also calculated. Then, we compare the daily production trend before and after the frac hits to determine the severity of its influence on production. Finally, any evident correlations between the stage-to-well distance, frac hit intensity and production change are identified and investigated. This work utilizes 3 datasets covering 22 horizontal wells in the Bakken Formation and 37 horizontal wells in the Eagle Ford Shale Formation. These sets included well trajectories, child well completions data, parent well pressure data and parent well production data. The frac hit detection algorithm developed can accurately detect frac hits in the available dataset with minimal false alerts. The data analysis results show that frac hit severity (production response) and intensity (pressure response) are not only affected by the distance between parent and child wells, but also affected by the directionality of the wells. Parent wells tend to experience more frac hits from the child frac stages with smaller direction angles and shorter stage-to-parent distances. Formation stress change with time is another factor that affects frac hit intensity. Depleted wells are more susceptible to frac hits even if they are further from the child wells. Also, we observe frac hits in parent wells due to a stimulation of a child well in a different shale formation. This paper presents a novel automated frac hit detection algorithm to quickly identify different types of frac hits. This paper also presents a novel way of carrying out production analysis to determine whether frac hits in a well have positive or negative influence long-term production. Additionally, the paper introduces the concept of the stage-to-well distance as a more accurate metric for analyzing the influence of frac hits on production.

Abstract:
Abstract The rapid development of modern aircraft technology is increasingly demanding aero-engines. It not only requires aeroengines to ignite and take off under suitable conditions, but also faces various severe weather and foreign object impact tests during flight. The stress variation caused by the impact of an aero-engine fan blade is studied. Because the compressor guide vane has a small solidity, the hail may not only hit the vane when striking the blade, but also may hit the first-stage rotor blade. The effects of hail impact on the vanes and rotor blades, especially the impact on the blade strength, were studied. Firstly, the UG software is used for geometric modeling based on the existing vane and rotor geometry data. Secondly, the ANSYS/LSDYNA software is used to simulate the process when the hail hits different positions of the blade and rotor at different speeds. To explore the influence of hail impact on the blade. It is found that when the hail hits the guide vane, the maximum effective stress of the vane does not exceed the yield stress of the material. When the hail hits the rotor, the maximum effective stress of the rotor is closely related to the rotor speed, the hail incident velocity, and the relative position of the rotor and the hail. When the impact is severe, the maximum effective stress will exceed the yield stress of the material, and even the blade may be bent and pitted, which has an important impact on the strength and life of the blade.

Cedeno, Miguel Angel. "Unloading Frac Hits in Gas Wells: How Does the Nitrogen Injection Rate and Pressure Affect the Unloading Process?" In SPE Trinidad and Tobago Section Energy Resources Conference. SPE, 2021. http://dx.doi.org/10.2118/200962-ms.

Full text
Abstract:
Abstract The unconventional resources development has grown tremendously as a result of the advancement in horizontal drilling technology coupled with hydraulic fracturing. However, as more wells are drilled and fractured close to each other, frac hits have become a major challenge in these wells. The aim of this work is to investigate the effect of nitrogen injection flow rate and pressure on unloading frac hits gas wells in transient multiphase flow. A numerical simulation model was created using a transient multiphase flow simulator to mimic the unloading process of frac hits by injecting nitrogen from the surface through the annulus section of the well. Many simulation cases were created and analyzed to comprehend the effect of the nitrogen injection rate and pressure on the unloading of frac hits. The model mimicked real field data from currently active well in the Eagle Ford Shale. The results showed that as the nitrogen injection pressure increases, the nitrogen volume and the time to unload the frac hits decrease. On the other hand, increasing the injection rate of nitrogen will increase the nitrogen volume required to unload the frac hits. In addition, the time to unload frac hits will be decreased as the nitrogen injection rate increases. These results indicate that the time required to unload frac hits will be minimized if higher flow rates of nitrogen were utilized. Nonetheless, the volume of nitrogen required to unload the frac hits will be maximized. An important observation to highlight is that the operators can save money by reducing the time for injecting nitrogen. This observation was verified when increasing the injection pressure in the frac hit well in the Eagle Ford Shale, the time of injection was reduced 20%. This study presents the effects of nitrogen injection flow rate and injection pressure for unloading frac hits in gas wells. Due to the lack of published studies about this topic, this work can serve as a practical guideline for unloading frac hits in gas wells.
