Abstract:
Solubility of phytochemicals is a major concern for drug delivery, permeability, and their biological response. However, advancements in the novel formulation technologies have been helping to overcome these challenges. The applications of these newer technologies are easy for commercialization and high therapeutic outcomes compared to conventional formulations. Considering these facts, the present study is aimed to prepare a silymarin-loaded eutectic mixture with three different ratios of Polyvinylpyrrolidone K30 (PVP K30) and evaluating their anti-inflammatory, and hepatoprotective effects. The preliminary phytochemical and characterization of silymarin, physical mixture, and solid dispersions suggested and successfully confirmed the formation of solid dispersion of silymarin with PVP K30. It was found that the solubility of silymarin was increased by 5-fold compared to pure silymarin. Moreover, the in vitro dissolution displayed that 83% of silymarin released within 2 h with 2.8-fold increase in dissolution rate compared to pure silymarin. Also, the in vivo study suggested that the formulation significantly reduced the carbon tetrachloride- ( 0.8620 ± 0.05034 ∗ ∗ for 1 : 3 ratio), paracetamol- ( 0.7300 ± 0.01517 ∗ ∗ for 1 : 3 ratio), and ethanol- ( 0.8100 ± 0.04037 ∗ ∗ for 1 : 3 ratio) induced hepatotoxicity in rats. Silymarin solid dispersion was prepared using homogenization methods that have prominent anti-inflammatory effect ( 0.6520 ± 0.008602 ∗ ∗ with 8.33%) in carrageenan-induced rat paw model.

Abstract:
Background:Animal-Assisted Interventions (AAI) is the new way to indicate what was previously known as “Pet Therapy”, as activities can be done either with the conventional “pets” (dogs, cats and rabbits) or with horses and donkeys.Children with JIA have several problems in terms of adherence both due to the atavistic fear of the needle and due to nausea and vomiting - the most important side effects of Methotrexate – often since the 2-3 days before the assumption to immediately after it.Sure that animals can help children to forget this fear and to avoid the psycological conditions which enhance nausea, for the first time in Italy (and probably in Europe) it was designed a specific AAI program for these children. Objectives:To promote a general state of psycho-physical well-being in children and families about:manage of therapy; reduce discomfort and anxiety caused by entering hospital; improve self-esteem and the response to the stress generated by the execution of therapy and disease management; strengthen communication and socialization; stimulate the affective area through the activities of animal care. Methods:Dogs and cats are part of the recreational activities once a week in an equipped area in the OIRM Hospital (no alternative gateway was needed).Paediatric Rheumatologists selected two different groups of children: the first one (5 children in the pilot study) every 15 days; the second one (5 children) every month; the selection was made looking at the therapeutic scheme.Every session, one hour, has 3 clearly distinct stages:Welcome and organization: children say hello to dogs and cats, open the toolkits specifically designed for the intervention, express their state of mind and are encouraged to tell their own stories.Therapy:parents prepare and inject the drug to their children under medical or Health Professional control without discontinuation of the activities with animals.Play and socializing:children are involved in petting and other activities with animals; they are also involved in manipulative activities (design, puppets shows, modelling clay, animal care, ball retriving, etc). This step has the aim to relieve stress and discomfort due to medical procedures.Visual Analogic Scales (VAS) were part of the toolkit, to let the researchers evaluate the effects of the activity directly from the children experience. For the first time, we will control also the animal health status and wellness condition monitoring behavioural parameters and salivary cortisol level during each session. Results:The pilot project started in October 2019 and nowaday we closed 12 meetings, 4 on October, 4 on November, 3 on December and 2 on January, with the participation of 2 dogs (Golden and Labrador Retriever) and 1 cat (Devon Rex) in each one.All children love to play with animals, seek their closeness at the time of therapy and enjoy playing all together with the dogs; no one cry or refuse therapy and, since the third session, no one has nausea before, during or after the injection. Parents have reached a certain level of confidence: they stay quietly in the waiting room or go away to have a drink or to run an errand (it becomes a moment of relaxing for them too).Animals remain in healthy and wellness conditions during the activity. Conclusion:These preliminary data seem that AAI to be useful in helping patients in JIA to overcome some problems related to their pathology.Disclosure of Interests:UGO VIORA: None declared, Patrizia Ponzio: None declared, Maria Teresa Mascarino: None declared, Barbara Picco: None declared, Paolo Guiso: None declared, Eleonora Battista: None declared, Silvana Martino: None declared, Davide Montin Speakers bureau: Not relevant for the topic, Marta Dellepiane: None declared, Germana Rosso: None declared

Abstract:
Many animal species cooperate, but the underlying proximate mechanisms are often unclear. We presented chimpanzees with a mutualistic collaborative food-retrieval task requiring complementary roles, and tested subjects' ability to help their partner perform her role. For each role, subjects required a different tool, and the tools were not interchangeable. We gave one individual in each dyad both tools, and measured subjects' willingness to transfer a tool to their partner as well as which tool (correct versus incorrect) they transferred. Most subjects helped their partner and transferred the tool the partner needed. Thus, chimpanzees not only coordinate different roles, but they also know which particular action the partner needs to perform. These results add to previous findings suggesting that many of chimpanzees' limitations in collaboration are, perhaps, more motivational than cognitive.

Abstract:
Abstract Introduction Although PAP therapy is the gold standard treatment for obstructive sleep apnea, adherence to treatment is suboptimal. Without sustained therapy adherence, patients are at risk of serious negative health outcomes. The objective of this study was to test whether a digitally delivered monetary and social reward program helped patients new to PAP therapy. Financial incentive schemes are effective in helping patients adhere to difficult medication or therapy plans. Additionally, there is an abundance of evidence that social support is a critical component to long-term health behavior change. Methods This prospective, randomized, single site pilot is evaluating the effectiveness of an app-based intervention in helping patients adhere to PAP therapy. The financial incentive design leverages loss aversion, and the social incentive design leverages the strength of close ties and variable reinforcement. The primary endpoint is mean PAP usage at 3 months. Secondary endpoints include Medicare compliance, change in functional status, and baseline scores of perceived disease severity, claustrophobia, coping skills, and health literacy as moderators of the intervention’s effectiveness. Study recruitment is ongoing, with an expected sample size of 150 subjects. Results Of the 132 subjects enrolled, 56% are male, 61% are Caucasian, and 65% are married. The mean age is 49.6 ± 12.0 years and mean BMI is 32.4 ± 8.4 kg/m2. Additional demographics such as income level, education level, and number of children along with the primary and secondary endpoints will be presented. A subgroup analysis of the primary endpoint will be generated for subjects identified as strugglers within the first 3 days of usage. Conclusion The results of this study will provide insight into methods such as financial and social incentives delivered via a smartphone on initial compliance with PAP therapy, as well as provide more information on the behavioral change associated with beginning PAP therapy. Abstract:
34 Background: The Partners Against Cancer (PAC) program pairs first and second year medical student volunteers with individual cancer patients in need at the Division of Hematology/Oncology at Creighton University Medical Center. A review of the scientific literature yields no publications about student run programs at medical institutions pairing students with cancer patients. Methods: Prior to beginning the program, each student completed an entry survey. While in the program, students were asked to submit log entries after each patient encounter. A total of 137 student logs from 30 students were surveyed for themes (e.g. student addressing patient’s physical needs), which were counted. Results: PAC trained 129 medical students about patient resources and matched over 51 patient-student pairs. In the entry surveys, 98.5% of students entering the PAC program wanted to learn more about the psychosocial issues cancer patients experience and their quality of life. Most students (98.5%) wanted to gain more experience talking about death and dying with patients and working with a dying patient. Of reported patient contact, 39% was in clinic, 44% phone, 8% email, and 6% was an inpatient or treatment facility visit. In the log data, students noted helping patients emotionally (85%) and with access to resources or promoting health and compliance (28%). Medical students demonstrated empathy in 75% of logs and gaining knowledge of medicine in 26%. 12% of logs noted schedule conflicts or contact difficulties. Conclusions: PAC is a longitudinal experience where medical students play an active role in their individual patient’s care. Students provide physical assistance to patients, such as helping with access to resources, and emotional assistance by listening to the patient. In return, the student has the opportunity to learn empathy by understanding the impact of chronic and, often terminal, disease on the patient. The challenges of the program have included scheduling difficulties and student confusion with the program. A website and mentor program have been developed to address these issues. Despite challenges, PAC exhibits great potential for enhancements in patient care and medical education, most notably empathy.

Abstract:
At the end of the 18th century and the beginning of the 19th a sense of solidarity shared by the people of the Northern Europe appeared: Pan-Scandinavianism. First it had a certain impact at cultural and spiritual levels. In a narrower sense the term was used when referring to the possibility of creating a political union between the Nordic kingdoms during the middle decades of the 19th century. Although it seemed that the political side of Pan-Scandianvianism had a good chance of accomplishing itself, especially after the Three Years War (1848-1851), the international context from the period that followed did not favour that plan. The collapse of the political Pan-Scandinavianism was in 1864 – the Second Schleswig-Holstein War. Sweden-Norway failed in helping Denmark against Prussia and Austria, despite the fact that King Karl XV was an advocate of Pan-Scandinavianism. Leaving this aspect aside, there were other internal difficulties of the Nordic states that Pan-Scandinavianism had to face, including the emerging nationalism. On a broader view, Pan-Scandinavianism appeared as an opponent of Panslavism and Pangermanism. Thus the dream of a unified Scandinavia was abandoned in the 19th century and Pan-Scandinavianism focused on cultural, scientific and economic cooperation.

Abstract:
Anchor bolts are set in some elastomeric bearings of large-span column-supported spatial steel roofs. Besides helping the bearing in-site assembly, the bolts play the roles of sliding position limitation and vertical anchorage. To analyze the effect of bearings on nonlinear structural behavior, the bearing stiffness change is studied just before and after the elastomer pad is in contact with the bolts. For the bearing that the pad is glued with the top/bottom steel plates, three kinds of relation of pad and bolts are discussed and an approximate horizontal stiffness formula is obtained with parametric analysis method. Based on the analysis results a stiffness expression for sliding elastomeric bearings is deduced. To solve the computational problem caused by the bearing stiffness changes, an improved imbalance force vector formula is proposed. Case study shows that the bolts have influence upon the computed results more significantly.

Abstract:
Recent political science research has suggested that autocrats adopt a variety of institutions such as nominally democratic elections and ruling parties to buttress authoritarian durability. In this article I investigate the role of constituency service in an authoritarian regime. I argue that Singapore's Meet-the-People Sessions (MPS) is a complementary institution that can serve to mitigate the weaknesses of other authoritarian institutions, thereby entrenching authoritarianism, rather than serve as a form of democratic representation. First, it is a mechanism to gain valuable everyday information about grievances within the population, thereby allowing the ruling People's Action Party (PAP) to formulate policies and effectively target its response. Second, it is a convenient venue to recruit and socialize ordinary party members, thus helping the PAP forestall potential party decay. Symbolically, conducting MPS is a material performance of the hegemonic ideology of elitism between PAP politicians and ordinary Singaporeans.

Abstract:
Shifting from the historical TNM paradigm to the determination of molecular and genetic subtypes of tumors has been a major improvement to better picture cancerous diseases. The sharper the picture is, the better will be the possibility to develop subsequent strategies, thus achieving higher efficacy and prolonged survival eventually. Recent studies suggest that urokinase-type plasminogen activator (uPA), uPA receptor (uPAR), and plasminogen activator inhibitor-1 (PAI-1) may play a critical role in cancer invasion and metastasis. Consistent with their role in cancer dissemination, high levels of uPA, PAI-1, and uPAR in multiple cancer types correlate with dismal prognosis. In this respect, upfront determination of uPA and PAI-1 as invasion markers has further opened up the possibilities for individualized therapy of breast cancer. Indeed, uPA and PAI-1 could help to classify patients on their risk for metastatic spreading and subsequent relapse, thus helping clinicians in their decision-making process to propose, or not propose, adjuvant therapy. This review covers the implications for cancer diagnosis, prognosis, and therapy of uPA and PAI-1, and therefore how they could be major actors in the development of a precision medicine in breast cancer.

Abstract:
Risk models developed on one dataset are often applied to new data and, in such cases, it is prudent to check that the model is suitable for the new data. An important application is in the banking industry, where statistical models are applied to loans to determine provisions and capital requirements. These models are developed on historical data, and regulations require their monitoring to ensure they remain valid on current portfolios—often years since the models were developed. The Population Stability Index (PSI) is an industry standard to measure whether the distribution of the current data has shifted significantly from the distribution of data used to develop the model. This paper explores several disadvantages of the PSI and proposes the Prediction Accuracy Index (PAI) as an alternative. The superior properties and interpretation of the PAI are discussed and it is concluded that the PAI can more accurately summarise the level of population stability, helping risk analysts and managers determine whether the model remains fit-for-purpose.

Abstract:
Nitric oxide (NO) is the critical regulator of VEGFR2-induced angiogenesis. Neither VEGF-A over-expression nor L-Arginine (NO-precursor) supplementation has been effective in helping patients with Peripheral Artery Disease (PAD) in clinical trials. One incompletely studied reason may be due to the presence of the less characterized anti-angiogenic VEGF-A (VEGF165b) isoform. We have recently shown that VEGF165b inhibits ischemic angiogenesis by blocking VEGFR1, not VEGFR2 activation. Here we wanted to determine whether VEGF165b inhibition using a monoclonal isoform-specific antibody against VEGF165b vs. control, improved perfusion recovery in preclinical PAD models that have impaired VEGFR2-NO signaling, including (1) type-2 diabetic model, (2) endothelial Nitric oxide synthase-knock out mice, and (3) Myoglobin transgenic mice that have impaired NO bioavailability. In all PAD models, VEGF165b inhibition vs. control enhanced perfusion recovery, increased microvascular density in the ischemic limb, and activated VEGFR1-STAT3 signaling. In vitro, VEGF165b inhibition vs. control enhanced a VEGFR1-dependent endothelial survival/proliferation and angiogenic capacity. These data demonstrate that VEGF165b inhibition induces VEGFR1-STAT3 activation, which does not require increased NO to induce therapeutic angiogenesis in PAD. These results may have implications for advancing therapies for patients with PAD where the VEGFR2-eNOS-NO pathway is impaired.

AbstractThis article examines the diffusion of evaporation technology along multi-centred and overlapping scientific, industrial, and commercial knowledge networks. It follows the circulation of vacuum pan (steam evaporator) technology in the Greater Caribbean, the North Atlantic, and the East Indies in order to understand the dual processes of invention and globalization. The article demonstrates that the tropical sugar plantation served as an experimental laboratory and, as such, vital inventions and engineering developments that took place in this space were subsequently incorporated into manufacturing designs in the North Atlantic, helping to modernize European industries in the process. As transient intermediaries, scientists, industrialists, and engineers modified and adapted vacuum pan technologies to meet the local demands of planters in the Greater Caribbean, thereby integrating this area into an increasingly globalized economy.

Abstract:
Scholastic factors (academic achievement) are hypothesized to be important determinants of health-related behaviors in adolescents, but there is a lack of knowledge on their influence on physical activity levels (PAL), especially considering the COVID-19 pandemic and the imposed lockdown. This study aimed to investigate the associations between scholastic factors and PAL before and during the pandemic lockdown. The participants were adolescents form Bosnia and Herzegovina (n = 525, 46% females), who were observed prospectively at the baseline (before the pandemic lockdown) and during the lockdown in 2020 (follow-up). The scholastic factors (grade point average, behavioral grade, school absences, unexcused absences) were evidenced at the baseline (predictors). The outcome (PAL) was evaluated using the Physical Activity Questionnaire for Adolescents at the baseline and the follow-up. Gender, age, parental/familial conflict, and sport participation were observed as confounders. No significant influence of the predictors on PAL were evidenced at the baseline or at the follow-up. The scholastic variables were significantly associated with the changes of PAL which occurred due to pandemic lockdown, with a lower risk for negative changes in PAL among adolescents who were better in school (OR = 0.56, 95%CI: 0.34–0.81, and OR = 0.66, 95%CI: 0.34–0.97, for the grade point average and behavioral grade, respectively). Students who do well in school are probably more aware of the health benefits of proper PAL, and therefore are devoted to the maintenance of their PAL even during the home-confinement of lockdown. Public health authorities should focus more on helping adolescents to understand the importance and benefits of proper PAL throughout the school system.

Abstract:
Abstract The effect of anoxia on the fibrinolytic potential of cultured endothelial cells derived from bovine pulmonary artery and bovine lung microvasculature was studied. Both cell types reacted with an increase in plasminogen activator inhibitor (PAI) activity and a decrease in the plasminogen activator (PA) activity in the media after incubation under anoxic conditions. Sodium dodecyl sulfate-polyacrylamide gel electrophoresis followed by fibrin autography and reverse fibrin autography indicated that the change in fibrinolytic potential was due to an impaired release of PA and not an increase in the production of PAI. Although anoxia did not affect the viability of the cells as judged by 51Cr release, their metabolism was influenced, which is reflected by increases in the levels of lactate in cell lysates and media. Furthermore, the effect of short-term anoxia on PA and PAI could not be reversed by reoxygenation for 24 hours. The results are discussed in terms of helping to explain the tendency of reocclusion after successful thrombolytic therapy, the development of pulmonary hypertension, and the thrombotic tendency of areas with an impaired circulatory supply.

Abstract:
Abstract Introduction Patients with sleep apnea are often prescribed positive airway pressure (PAP) treatment. Some patients have difficulty consistently using a PAP machine due to problems such as air leak, mask discomfort, and dry mouth. The purpose of this project is to evaluate the satisfaction and efficacy of a PAP Troubleshooting Clinic lead by a RN and a respiratory technician (RT). This clinic seeks to improve PAP compliance, increase PAP tolerability and increase PAP treatment efficacy Methods The PAP Troubleshooting Clinic consists of a RN and RT with specialized knowledge about sleep apnea and PAP treatment. During a 30 or 60-minute clinic visit, these providers review the patient’s experience with PAP, assess mask fit, review PAP data and PAP settings, and recommend treatment adjustments for the primary Sleep Clinic provider’s consideration. Outcomes assessment includes satisfaction (a telephone survey one week after the visit) and efficacy (30-day data on overall use, mask leak, and days with ≥ 4 hours of use downloaded from PAP machines). Results To date, 58 patients have received care in the clinic and 56 (96.5%) patients reported they were satisfied/highly satisfied with the PAP Troubleshooting Clinic. From the sample, forty-three (74.1%) patients were compliant with mask use over 30 days. Twenty-four (55.81%) showed a greater than 10% reduction in mask leak after intervention. Ten patients (23.3%) achieved an improvement with a 10% increase in number of days with > than 4 hours of PAP use. Conclusion Patients and Sleep Clinic providers are very satisfied with the PAP Troubleshooting Clinic. Patients are referred to this clinic because they have significant issues with PAP usage and are at high risk of discontinuing use. Patients find the clinic helpful and encouraging, while sleep medicine physicians and APPs appreciate the assistance in helping patients succeed with PAP, especially during the 30-day time period where PAP compliance receives scrutiny from clinics and payers. Support A PAP Troubleshooting Clinic is an effective way to improve patient PAP Use, Mask Leak, Prolonged Use and Patient Satisfaction. Importantly, this new clinical model offers a valuable alternative to provide patients with the appropriate level of care.

Abstract:
Plasminogen activator inhibitor-1 (PAI-1) is an acute phase protein that has been shown to play a role in experimental fibrosis caused by bile duct ligation (BDL) in mice. However, its role in more severe models of hepatic fibrosis (e.g., carbon tetrachloride; CCl4) has not been determined and is important for extrapolation to human disease. Wild-type or PAI-1 knockout mice were administered CCl4 (1 ml/kg body wt ip) 2×/wk for 4 wk. Plasma (e.g., transaminase activity) and histological (e.g., Sirius red staining) indexes of liver damage and fibrosis were evaluated. Proliferation and apoptosis were assessed by PCNA and TdT-mediated dUTP nick-end labeling (TUNEL) staining, respectively, as well as by indexes of cell cycle (e.g., p53, cyclin D1). In contrast to previous studies with BDL, hepatic fibrosis was enhanced in PAI-1−/− mice after chronic CCl4 administration. Indeed, all indexes of liver damage were elevated in PAI-1−/− mice compared with wild-type mice. This enhanced liver damage correlated with impaired hepatocyte proliferation. A similar effect on proliferation was observed after one bolus dose of CCl4, without concomitant increases in liver damage. Under these conditions, a decrease in phospho-p38, coupled with elevated p53 protein, was observed; these results suggest impaired proliferation and a potential G1/S cell cycle arrest in PAI-1−/− mice. These data suggest that PAI-1 may play multiple roles in chronic liver diseases, both protective and damaging, the latter mediated by its influence on inflammation and fibrosis and the former via helping maintain hepatocyte division after an injury.

Abstract:
The Trafficking Victims Protection Act (2000, 2011) created new conditional residency visas and new avenues for American citizenship for the victims of human trafficking. Thai migrants have benefited from its provisions, but their disproportionate presence in this category has indicated the depths of this problem within the Thai immigrant community. This paper examines anti-trafficking advocacy, and it begins by criticizing existing Asian American pan-ethnic organizations. It addresses the limits of their approaches, and argues that ethnic-specific organizations still play an important role in helping victims as well as the ethnic communities in which they will settle.

Abstract:
The development of the packaging industry has promoted indiscriminately the use of disposable packing as Tetra Pak, which after a very short useful life turns into garbage, helping to spoil the environment. One of the known processes that can be used for achievement of the compatibility between waste materials and the environment is the gamma radiation, which had proved to be a good tool for modification of physicochemical properties of materials. The aim of this work is to study the effects of waste cellulose from Tetra Pak packing and gamma radiation on the mechanical properties of cement concrete. Concrete specimens were elaborated with waste cellulose at concentrations of 3, 5, and 7 wt% and irradiated at 200, 250, and 300 kGy of gamma dose. The results show highest improvement on the mechanical properties for concrete with 3 wt% of waste cellulose and irradiated at 300 kGy; such improvements were related with the surface morphology of fracture zones of cement concrete observed by SEM microscopy.

Abstract:
One of the institutions tasked with helping to improve the quality of education is the Education Quality Assurance Institution (LPMP). The purpose of this study was to identify the role of the Education Quality Assurance Institution in improving the quality of Islamic Religious Education (PAI) teachers. This study uses a qualitative approach, this approach was chosen based on the research objective, namely to describe and analyze the process of improving the quality of PAI teachers in Suka Makmur District carried out by LPMP Aceh. The data collection techniques are interviews, and document studies. The results showed that the role of LPMP Aceh was as an organizer in the development of the education quality assurance system program, as a facilitator in education and training, and as a companion for education and training in improving the competence of PAI teachers. The form of coordination of the PAI Teacher Quality Improvement Program at LPMP Aceh in collaboration with Educational Institutions/Departments by providing technical assistance to the Education Office, and collaboration with KKG/MGMP teacher groups through mentoring in schools. As for the control that Aceh's LPMP faces is less coordinated the recorded retention schedule that Aceh's LPMP itself or outside the LPMP, and the allocation of the recorded funds also constitutes control because it is not in the List of Budget Project DIPA LPMP Aceh.
