Abstract:
Hearing loss is the commonest sensory disability worldwide, and the World Health Organisation has estimated that 278 million people suffer a moderate to profound hearing loss in both ears, with 80 per cent of deaf and hearing-impaired people living in low- and middle-income countries (WHO 2006). Tinnitus affects approximately 10 per cent of developed populations (Coles 1984) and of these, 5 per cent find the symptom troublesome and seek help (Davis 1995). Tinnitus and hearing loss are primary symptoms of disordered cochlear function, but may also present as a result of central auditory pathology with normal cochlear function. Pathology affecting the central auditory pathways characteristically presents as difficulty hearing in conditions of poor signal-to-noise ratio, for example, in a classroom in the presence of background noise, listening to transmitted sound, for example on the telephone or on a television, and sound localization. As a consequence of multiple relays and bilateral representation above the level of the cochlear nuclei, central auditory dysfunction does not present with hearing loss. Hearing loss and/or tinnitus, with or without associated vestibular abnormalities, will most commonly be the result of otological pathology. However, importantly for the neurologist cochlear, VIII nerve, or central auditory dysfunction may be part of the clinical presentation of a neurological disorder.

In some respects, hard-of-hearing children experience the same difficulties as deaf children, whereas other challenges might be easier or more difficult to handle for the hard-of-hearing child than it would be for the deaf child. Research has revealed great variability in the language, academic, and psychosocial outcomes of hard-of-hearing children. Universal newborn hearing screening enables early identification and intervention for this group, which traditionally has been diagnosed rather late; however, best practices regarding the scope and content of early intervention have not yet been sufficiently described for hard-of-hearing children. This chapter summarizes the current knowledge concerning psychosocial development in hard-of-hearing children. Risk and protective factors, and their implications for early intervention, are discussed with a special emphasis on preschoolers.

There are many reasons for delayed recovery, but, usually, it is due to residual effects of anaesthetic agents/premedication. There are guidelines for recognizing and managing these cases. Emergence delirium may be dangerous, and it should be recognized and treated as an emergency. Elderly patients may have impaired hearing and vision. Spectacles and hearing aids should be given back to them as soon as possible in the recovery area to limit disorientation. Pain and its intensity may be difficult to recognize and quantify in the elderly. Increased inter-individual variability in the elderly means that titration to effect rather than a fixed dosage is essential, and when the mental status of the patient allows it, patient-controlled analgesia (PCA) is quite appropriate.

Throughout history, most people have encountered the language of sailors in ports. Such language, including nautical terminology, occupational lore, and the coterie speech that bound crews together, was a product of isolated shipborne communities at sea, but it was when sailors entered the liminal space of the port that the general public heard their speech. The language of sailors was often greeted with contempt and fear. The Boston Recorder warned in 1823 that when sailors are ashore, “Children can with difficulty enter the streets at all, without hearing the very dialect of hell.” Despite such condemnation, it was in the spoken world of sailor talk that Herman Melville, Joseph Conrad, and Jack London first became storytellers. They were all seafarers who transcribed their experiences at sea into stories, first told orally and then transformed into the written word. With their storytelling, they crossed the liminal space of the port and emerged as the great artists that they are. This book defines sailor talk, explores its inextricability from the labor of mariners, and investigates the orality of the shipboard world in which sailors worked. It then concentrates on the complex, multitudinous, and intertwining use of sailor talk in the works of Melville, Conrad, and London.

Lewis was widely admired by his contemporaries, but the recollections, letters, and diaries of those who knew him also make clear that many British intellectuals found him less than satisfying as a writer and scholar or difficult and abrasive as an individual. The plethora of negative critiques from colleagues and students should be seen in light of his persona and his tendency to look back to the authorities and sensibilities of a past age. His stance as a reactionary outsider and his speaking from within a type of British controversialism cost him a more serious hearing on intellectual matters. Then again, the 1930s–1950s were years in which universities and their faculties were being pressed into a more meritocratic and pluralistic future. Lewis’s reception among his intellectual contemporaries was complex, indivisible from his persona and the politics and culture of his insular Oxbridge milieu.

Management of Alport syndrome has in the past been expectant and supportive. Modern hearing aids have substantially improved the function of affected individuals. However, animal data and more recently observational data from Alport registries strongly suggest a protective effect of angiotensin-converting enzyme inhibitors. There is a suggestion that early commencement of treatment may slow progression substantially. These should now be recommended for all with proteinuria, and possibly even before then for those known to harbour mutations certain to cause end-stage renal failure. A very small minority develop the difficult post-transplant complication of Alport anti-glomerular basement membrane disease. This can rarely be treated successfully and leaves some patients on long-term dialysis. However, overall, patients with Alport syndrome have better than average survival and other outcomes than other patients with end-stage renal failure. Most are successfully transplanted. The question of risk to heterozygous carriers from donating kidneys to their affected relatives arises frequently. The risks may be felt acceptable in some circumstances. Additional therapies are under investigation.
