The article provides information about different types of dietary fats and promotes consumption of healthy fats in moderation as part of a balanced diet. The prevalence of fat-free products in grocery stores may give some health-conscious consumers the perception that all dietary fats are unhealthy. However, fats are absolutely vital for proper physiological functioning, and it is imperative that fats are included in a healthy diet. Fats are classified as saturated, monounsaturated, or polyunsaturated. Both types of unsaturated fats, when consumed in moderation, can help lower cholesterol and decrease the risk of heart disease, especially when they replace saturated and trans fats. We provide tips for selecting healthier fats, along with a guide for consuming appropriate portions of fat.

Abstract:
In fact, the relationship of health and sociology, social relations, etc., has been started to be analyzed recently in Lithuania. Recently, public health research is focused on innovative fields as health quality, efficiency of health services, solidarity and integrity of activity, introduction of health policy elements in all fields of social and economic activity. Aging society, social, economic changes, penetration of market elements in the sphere of health services change the nature of interaction between public health condition and integral environment that influences it. These circumstances also alter the assumptions of health policy effectiveness. The latter must be based on information of scientific research. Unfortunately, there has been carried out small number of research of similar direction in Lithuania. Hereby, we seek to explore systematically and assess the differences of the main health and social factors, and the impact of their interactions on public health condition and the emerging social structures of health. The research analyzed the subjectively assessed health condition, morbidity, prevailing health problems and mental health conditions. These aspects were examined in different fields of influence of social factors as well as in their integral interaction.
Lietuvoje tik labai neseniai buvo pradėta analizuoti sveikatos santykis su sociologija, socialiniais santykiais ir pan. Pastaruoju metu visuomenės sveikatos tyrimai orientuojasi į tokias inovatyvias sritis kaip sveikatos kokybė, sveikatos paslaugų efektyvumas, veiklos solidarumas, integralumas, sveikatos politikos elementų diegimas visose socialinės ir ekonominės veiklos srityse. Visuomenės senėjimas, socialiniai, ekonominiai pokyčiai, rinkos elementų skverbimasis į sveikatos paslaugų sferą keičia visuomenės sveikatos būklės ir integralios aplinkos sąveikos pobūdį. Šios aplinkybės keičia ir sveikatos politikos veiksmingumo prielaidas. Jos turi remtis mokslinių tyrimų informacija. Deja, Lietuvoje nėra daug atlikta panašios krypties mokslinių tyrimų. Šiuo tyrimu mes siekiame sistematiškai ištirti ir įvertinti svarbiausių sveikatos ir socialinių veiksnių bei jų tarpusavio sąveikų poveikio visuomenės sveikatos būklei skirtumus ir tuo pagrindu besiformuojančias sveikatos socialines struktūras. Tyrimo metu analizuota subjektyviai vertinama sveikatos būklė, sergamumas, vyraujančios sveikatos problemos ir psichikos sveikatos būklės. Visa tai buvo nagrinėta skirtingose socialinių veiksnių, bei jų integralios tarpusavio sąveikos įtakos srityse.

Abstract:
Monitoring the health of vibrating gears is important to ensure proper operation especially in potentially life-threatening structures, such as helicopters, nuclear power plants, and uninterruptible power supply transitions in hospitals. The most common monitoring technique is casing mounted accelerometers to measure vibration. In contrast, during the last few years acoustic monitoring techniques have also provided a few diagnostic methods for gear failure. Current diagnostic methods to indicate improper gear behavior use either existing vibration data, recorded from defective gear systems, or modern dynamic models predicting gear failure behavior. This thesis uses dynamic models to indicate, predict, and diagnose healthy and unhealthy gear systems. Influence of Tip Relief on contact forces are introduced for a decent understanding of gear dynamics followed by evaluation of common gear failure mechanisms. Two software systems were used to model gear failure: Adams®, a vibration based software that uses a rigid-elastic model for multi-body dynamics, and LSDYNA ®, a transient dynamic finite element solver, capable of solving acoustic problems with the boundary element method. Results describe tooth loads along the line of contact with respect to different Tip Reliefs and contact ratios. Gear failure is examined using a Fast Fourier Transformation to characterize patterns that can be used to diagnose unhealthy gear systems. Agreement of experimental results validates theoretical predictions of analytical and numerical solutions of gear failure especially of tooth breakage.

Abstract:
Humans are social creatures, bound by relationships. This dissertation investigates the structure and function of such interpersonal ties in two different contexts: patient-doctor trust and the diffusion of public health interventions across social networks. Each chapter of the dissertation focuses on a particular type of relationship: in Chapter 1, the patient-doctor relationship; in Chapter 2, friendship; and in Chapter 3, ties among friends, spouses, and siblings. The hope is that a better understanding of the nature of these ties and the work they do will enable policymakers to develop regulations and programs that promote the benefits and minimize harm in these exchanges. In Chapter 1, I present the results of a randomized controlled lab study on the effects of disclosure of payments that physicians receive from the pharmaceutical and medical device industry on patient-doctor trust. I find that certain dimensions of patient-doctor trust—honesty and fidelity—are lower for individual physicians when participants view payments of over $13,000 compared to lesser or no payments, but ratings of the physician’s competence do not change. In addition, trust in the medical profession and industry are not affected by disclosure. These findings suggest that industry payments to physicians that exceed $13,000 may need to be regulated, or at least carefully monitored. In Chapter 2, I present a sex-stratified analysis of a randomized controlled field experiment on the network-based diffusion of public health interventions. While men and women are more likely to have same-sex friends, they are both more likely to distribute the public health products to women than to men, revealing a contrast between the nominated ties and activated ties. This finding demonstrates that a traditional name generator for friend ties is a poor predictor of behavior regarding the spread of health-related phenomena. Further research is needed to explore alternative ways to elicit social network data to optimize diffusion, or else network-based strategies for the spread of socially beneficial innovations may need to be reconsidered. In Chapter 3, I use simulations based on the Susceptible-Infected (SI) process to explore the generalizability of the Honduras Study findings to other contexts. In particular, I look at the spread of public health interventions in the Karnataka (India) networks and compare the spreading processes to those in Honduras. The simulations reveal underlying network processes that can variably constrain and promote access to health information and products. As a whole, these studies seek to quantify the nature of social ties and related implications for health care providers and public health programming.

Darbo tikslas. Nustatyti ir įvertinti uždaro tipo baseinų aplinkos higieninius veiksnius visuomenės sveikatos specialisto ir vartotojo aspektu. Uždaviniai. • Ištirti ir įvertinti uždaro tipo baseinų aplinkos higieninius veiksnius; • Įvertinti baseinų vandens paruošimo kokybę ir kontrolę; • Įvertinti paslaugų vartotojų požiūrį į baseinų teikiamų paslaugų kokybę. Tyrimo metodika. Atlikta mokslinės literatūros apžvalga bei anoniminė anketinė apklausa. Anketų duomenys buvo apdoroti naudojant statistinį duomenų analizės paketą SPSS 13.0. ir MS EXEL programa. Duomenų analizė darbe pateikiama lentelėmis ir paveikslais. Rezultatai. Tyrimo duomenimis nustatyta, kad dauguma respondentų baseinuose lankosi žiem�� ir pavasarį. Daugiau nei pusė apklaustųjų respondentų teigė, kad nėra patenkinti persirengimo kambarių patalpų švara. Pastebėta, baseinų lankytojai prieš pradėdami lankyti baseiną, neapsilanko pas gydytoją dėl grybelio profilaktikos. Net 10,8 proc. apklaustųjų teigė, kad po apsilankymo baseine teko užsikrėsti grybelinėmis infekcijomis. Išvados. The selected theme of the work is relevant in the present practical life of society. From the theoretical point of view, it provides a possibility to investigate thoroughly a few subjects – to evaluate and to determine hygienic factors of closed-type water pools from the position of a public health specialist and from the position of a user, and to evaluate view to quality of services provided by closed-type water pools. Aim of the study. To determine and to evaluate hygienic factors of closed-type water pool environment from the point of view of a public health specialist and user. Objectives. • To investigate and to evaluate hygienic factors of closed-type water pools; • To evaluate quality of water-pool water preparation and control; • To evaluate views to quality of services provided by water pools. Methods. A review of scientific literature and a questionnaire survey is performed. Data of the questionnaire survey were processed usable the statistical data analysis software SPSS 13.0 and MS EXEL application. Data analysis is presented in the work in the form of tables and diagrams. Results. The investigation has shown that most respondents visit water pools in winter and spring. More than half of respondents stated that they were not satisfied with cleanness of changing rooms. It was noticed that visitors don’t visit doctors due to fungi prophylactics before going to water-pools/ even 10,8 percent of respondents stated that they had fungal infections after visiting... Abstract:
The treatment and control of individuals with severe mental health difficulties in the community has long been positioned as a problem in need of a solution. This thesis investigates one recently introduced ‘solution’ in English mental health services - community treatment orders (CTOs) - enacted under the Mental Health Act (2007). CTOs work by imposing conditions on how service users live in the community, as well as allowing for them to be recalled for treatment in hospital if they fail to meet these conditions, and/or they are deemed to be a risk to themselves or others. Their introduction has brought with it debate on the ethical implications of extending compulsory treatment into the community, with opinion on their use strongly divided. This thesis aims to get beyond such dichotomous positioning to uncover the potentially multiple and complex ways CTOs are conceptualised, interpreted and used by practitioners and service users, and with what consequences. An ethnographic approach is taken to explore ‘inside’ CTO practice within two Mental Health Trusts, through observations of everyday CTO processes, interviews with practitioners and service users, and document analysis of policy and practice-related sources. In this way a dual analysis is formed which gives a generative explanation of the pathways a CTO might take, and describes how service users and practitioners experience and respond to CTOs as a compulsory intervention. This analysis is guided and deepened by a combined critical realist-governmentality framework, which informs an understanding of why it is CTOs unfold as they do, and how they regulate conduct and encourage self-regulation through assemblages of coercive, disciplinary and reflexive forms of power. As will be shown, a number of distinctive conclusions can be drawn about CTO policy and practice relating to: the gaps and continuities between policy theorisation, practice-level conceptualisation and CTOs in action; the constituting power of agency and interaction alongside institutional and cultural factors in shaping various CTO outcomes; the complex and often ambiguous reactions of practitioners and service users to CTO ends and means as connected to ethical self-work; and the mixed, sometimes unexpected and unintended consequences of CTO use.

The following work focus on the importance of nature as a beneficial resource improving the patient’s welfare in health facilities. This amelioration is felt not only in patients but also benefits the employees of these facilities and the patient’s visits. For better understanding, this dissertation starts with a historical approach about therapeutic gardens and its evolution over time, making references and relations to some examples in Portugal. Secondly this work focuses on the restorative power of nature on human health, followed by some examples and case studies. It’s also referenced an approach to philosophical foundations, theories, tools and elements that have some sort of impact on health. The approach to these themes will help us understand our attraction to nature and the benefits of its presence in comparison with urban environment. This will provide the necessary groundwork for the development of a project to an outdoor space to health facility, as well as adopting the features and elements that make an outdoor space more harmonious and healthy, not only for patients but also for employees or visitors, these elements will be presented in chapter IV. After it’s presented the characteristic and elements afore mentioned and some general principals of drawing and design for health facilities which can be applied to health facilities with multiple specialities, as for health facilities that are intended only for a specific treatment with only a medical speciality. To finish the main principles that are applied in the design and conception of the therapeutic gardens, which can be extensive landscaped grounds, a courtyard, a front porch, etc. These methods are applied in order to enhance, not only our life quality but also the social interactions, the environment and the economy

Hoitoisuusluokitusmittareiden hyödyntämistä on tutkittu vähän, vaikka hoitotyön tehostamiseen on suuri tarve. Terveydenhuollossa resurssit eli voimavarat ovat rajalliset, mutta potilaiden tarpeet rajattomat, joten potilaiden tarpeeseen vastaamiseen tulisi olla luotettava optimaalisen resursoinnin keino. Tämän kirjallisuuskatsauksen tarkoituksena on kuvailla Tehohoitotyön Luokitusjärjestelmän (THLJ) ja Therapeutic Intervention Scoring System (TISS-76) hoitoisuusluokitusmittareiden hyödyntämistä sekä kuvailla näiden mittareiden hyödyntämisen mahdollisuuksia tehohoitotyön tehokkuuden (efficiency) arvioinnissa. Tutkielma toteutettiin narratiivisena kirjallisuuskatsauksena. Kirjallisuuskatsauksen aineistona käytettiin kansaisvälisiä tieteellisiä artikkeleita/tutkimuksia. Aineistoa haettiin Scopus-, Cinahl-, ja Medic-tietokannoista, vapaasanahaulla ja manuaalisella haulla. Haut tehtiin kesällä ja syksyllä 2017. Aineisto analysointiin induktiivisella sisällönanalyysillä. THLJ-pisteytysjärjestelmää hyödynnetään tulosten mukaan tehohoitotyön tarpeen arvioinnissa ja tarpeeseen vastaamisen määrittämisessä sekä sairaanhoitajien työn kuormituksen allokoinnissa, jolla vaikutetaan työssä jaksamiseen. TISS-76-pisteytysjärjestelmää hyödynnetään tulosten mukaan tehohoitotyön suunnittelun apuvälineenä ja tehohoidon potilasaineistojen keräämiseen. Molempia voisi käyttää myös tehohoitopotilaan sairaalakuolleisuuden ennustamiseen. Tuloksissa löytyi myös ristiriitaa, joista merkittävin oli TISS-76-pisteytysjärjestelmän sisällön luotettavuusongelmat. Tehohoitotyön tehokkuuden arviointiin tarvitaan tietoa hoitotyössä käytetyistä henkilöstöresursseista ja resurssien avulla saavutetuista hoidon vaikuttavuuksista. THLJ:n hyödyntäminen tehohoitotyön tehokkuuden arvioinnissa voisi tulosten mukaan onnistua. TISS-76:n hyödyntäminen tehohoitotyön tehokkuuden arvioinnissa on tulosten mukaan haastavampaa sen sisällön luotettavuusongelmien vuoksi. Hoitoisuusluokituksen luotettavuus on avainasemassa, jotta sen hyödyntäminen on kannattavaa. Tulosten mukaan TISS-76:ssä on sisällön luotettavuusongelmia samoin THLJ:ssä, mutta pienempiä. Molempia hyödynnetään käytännön hoitotyössä, mutta hyötyjen suhdetta mittarista aiheutuvaan työn ja kustannusten määrään ei ole tutkittu. Tutkimuksia hoitoisuusluokitusmittareiden hyödyntämisestä on yleisestikin vähän, joten lisää tutkimuksia tarvittaisiin.

The purpose of this study is to examine the informed consent (IC) procedure as it was experienced by patients who had undergone a de novo transjugular intrahepatic portosystemic shunt (TIPS), chemoembolization (TACE), or radioembolization (TARE) procedure in an Interventional Radiology (IR) Department. The three main study aims and a fourth exploratory aim are as follows: (1) Describe how patients who underwent a de novo TIPS, TACE, or TARE procedure in an IR Department described the IC procedure; (2) Describe what information patients who underwent a de novo TIPS, TACE, or TARE procedure in an IR Department recalled being told during the IC procedure; (3) Describe the satisfaction of patients who underwent a de novo TIPS, TACE, or TARE procedure in an IR Department with the IC procedure; and (4) Explore how the IC experiences of patients who underwent a de novo TIPS, TACE, or TARE procedure in an IR Department differed according to their levels of health literacy. Using a qualitative descriptive design, participants were recruited from an IR department that performed these procedures. A total of 14 participants were interviewed about their IC experiences and the Newest Vital Sign (NVS) Health Literacy assessment was administered. The participants described the IC procedure by discussing the staff they encountered, their feelings during the visit, the support persons who accompanied them, and the decisions they made about the procedure. The participants recalled being told about how their procedure would be performed, the care they would need, and the benefits and risks of the procedure. Most were satisfied with the information received during the IC procedure and found the information consistent with how they experienced the procedure. A few participants would have liked more visual materials, addition details about the procedure, simpler language, or more explanation of the medical terminology. No apparent differences in the IC experience could be attributed to health literacy. These findings suggest that persons’ experiences during the IC process are multi-faceted and affected by their emotions and concerns and the nature of their encounters with their healthcare providers.
