Although, electronic healthcare records overcome several problems compared with manual records, but still suffer from many issues such as security, the privacy of patient data overall as we should transfer over a database from a central database to a decentralized database. In this paper, we proposed a good security system to manage the data of patients based on blockchain technology and a decentralized database. Depending on decentralized database and blockchain. Our proposed system provides the secure exchange of patient data, reliability, and high efficiency in sharing data during transaction data network equivalence checking to perform this validation of patient information in the blockchain and healthcare centers.

Abstract:
Objective: Prediabetes affects 38 percent of adults in the U.S population, which means that 98 million U.S. adults are affected by prediabetes. Patients diagnosed with prediabetes are often referred to the National Diabetes Prevention Program (National DPP) through a variety of methods. National DPPs are year-long commitment covered by insurance. However, little is known about referral processes of health systems in California to National DPPs including if single standards of screening exist. Community Health Workers (CHWs) assist in the referral processes, but is unclear how much support they provide which is why a survey is necessary to evaluate referral processes. Methods: Because of this gap, the author developed a comprehensive survey and distributed it on March 25,2022 to seven health systems in California that refer prediabetes patients to National DPPs. Results: The study revealed barriers, which include 1) yearlong commitment, 2) compatible referral system, 3) lack gold standard for screening, 4) implementation of CHWs, and 5) COVID-19 pandemic. Conclusion: Session Zero should become standard in National DPP to answer questions about yearlong commitment, program delivery formats, and early commitment. Promoting personalized policies and procedures, and referral maps can help overcome the lack of compatible referral system. HbA1c is the best screening test and should be gold standard. Further implementation of CHW’s in referral processes can help overcome barriers. As a result of this work, barriers were identified. Key changes can make a pivotal difference in screening and diagnosis in the prevention of the often-forgotten epidemic of Type 2 Diabetes Mellitus.

The majority of low and middle-income countries (LMICs) have frequently implemented health information systems (HIS) in response to international pressure to account for health care investments. Health information can facilitate better planning and monitoring health services, which may lead to improved health outcomes, which is the concept behind robust and effective HIS. However, the use of health information systems (HIS) as accountability tools has frequently been criticized for being counterproductive because they give health information more significance to international organizations and national governments than to those in charge of local health services. This paper aims to examine how HIS influence the development of local accountability practices and their effects on health care delivery. The technology domain of HIS is combined with a theoretical perspective derived from structuration theory. A case study of HIS in Kenya is analyzed from this point of view. The findings have implications for the application of structuration theory to comprehending accountability and the significance of IT materiality in structuration processes. It contributes to a deeper comprehension of how HIS can foster enhanced human development and health care. It also contributes to the comprehension of IS as a means of socialization through which users can negotiate multiple accountability goals as well as a means of governing people's behavior. Health is a crucial part of human lives. Nowadays, healthcare is becoming vital each day, as there are lots of diseases that emerge around us. Technology is transforming the medical sector by massively impacting almost all practices and processes of medical professionals. Despite this, many of the people and medical staff still dealing with paper-based medical records and prescriptions while conducting treatment. When a patient wants to appoint any hospital or clinic, to carry previous medical reports or past prescriptions is becoming essential for them. It is necessary because the doctor gets an idea about the patient’s health status by referring to their previous medical histories, helping for better treatments and medications. However, patients cannot maintain every medical documentary for years. Conventionally, the doctor asks patients about their previous diseases, prescription, or medicine details orally, nevertheless it becomes difficult to get exact information from the patient. Sometimes, it becomes more important for the doctor to know about the medical history of a person so that they can provide suitable treatment with better clarity of that person’s health. Focussing on this, a smart medical assistant system is designed where doctors can record all prescriptions, treatment, or medical details of the patient on software instead of writing on a paper. All these records are stored in the central cloud and made visible to doctors as well as patients. Each patient has assigned a unique authentication card for maintaining the privacy of their medical history account. Doctors can access and update a patient’s medical history anytime and anywhere by logging into their account through a smartcard swipe. The system can avoid overdue to treatment decisions. Likewise, the system helps to keep transparency about medicines and treatment.
