Abstract:

All EU nationals have the right to health services that are affordable for everyone under the same conditions. Sector-specific regulations provide that health services are services of general interest that must be implemented through a national legal framework. The state must design the universal health services in a way that respects the principle of public health service affordability for all citizens. In the study, we focused on understanding the legal framework which serves as foundation the regulating universal health services in Slovenia, sector-specific regulations and other acts, and tried to assess the strengths and weaknesses of the Slovenian legal framework. Our efforts have been directed towards studying the legislative framework of the European Union and defining the legal guidelines that establish the legal framework for universal health service creation.

Abstract:

Within medical informatics there’s dental informatics, which deals with the management of information, communication, and the application of new technologies in clinical practice and research. These computer systems involve the storage of information and will be in charge of organizing the work in the dental clinic.(Specified, 2009) Objective: The primary objective of this research work is to know the need to manage and administer dental health services through computer systems in the city of Chiclayo. Method: It is a cross-sectional, descriptive, observational, and prospective study. Which comprised conducting questionnaires to the owners of dental clinics, administrative personnel, dentists, and patients who attend the different dental clinics that are in the City of Chiclayo. Results: We verified that there were contrasting hypotheses. Conclusions: We conclude that there is a need to implement dental clinics with computer systems.

Abstract:

Primary health care is pivotal to the growth of a country’s economy by improving the health and social well-being of its people. Several efforts and structures have been put in place to ensure that all South Africans have access to health facilities as well as representation in the health facilities, just as community members are represented in schools and other boards. Health committees play an important and integral role to the community and health facilities by being mediators between the community and the health facility (Padarath &Friedman, 2008). The Eastern Cape is one of the provinces that has health committees that are fully functional and a number of factors including the collaboration of key stakeholders (facility managers, community members, clinic staff and ward councillors) contribute to why health committees have been operational in the Eastern Cape(Padarath &Friedman , 2008). It becomes critical to investigate factors that sustain health committees in Nelson Mandela Bay. This chapter introduces the research topic, purpose, rationale and background as well as the objectives of the study. The research focuses on investigating the sustainability of health committees in Nelson Mandela Bay therefore making it critical to define the term sustainability. Conceptualizing sustainability has been ongoing with various definitions from learners from different backgrounds. Sustainability as a concept emanates from different dimensions namely, scientific, political economic and indigenous dimension. Sustainability has been described by scholars as a state of well-being and as an evolutionary process (Dimitrov, 2010). Sustainability is regarded as a state of well being whereby humans can live in co-exist with the natural world, this definition takes into account issues to do with good health , cultural identity, personal security and freedom of choice. Sustainability- well being brings out a lot of questions and problems in identifying the fundamentals values of sustainability. It is continuously debated amoung scholars if sustainability of well being pertains only to the health conditions of human beings or it should be extended to others (Dimitrov, 2010) “Sustainability that is restricted to the well being of humans is related to Notions about security of individual/community health together with financial security together with financial security; the planet is regarded as a resource to further this end (Dimitrov, 2010:4) If however well-being definitions of sustainability are construed to preserving and protecting the environment as well as biodiversity of other species (plant and animals) then the normative definitions of sustainability from this viewpoint are tendered with notions of global Environmental stability, ecological considerations and security of ecosystems” (Dimitrov, 2010:4).

The Veterans Health Administration (VHA) provides care to over eight million Veterans and operates over 1,700 sites of care distributed across twenty-one regional networks in the United States. Health care providers within VHA report large seasonal variation in the demand for services, especially in healthcare systems located in the southern U.S. that experience a large influx of "snowbirds" during the winter. Since the majority of resource allocation activities are carried out through a single annual budgeting process at the start of the fiscal year, the seasonal load imposed by "traveling Veterans," defined as Veterans that seek care at VHA sites outside of their home network, make providing high quality services more difficult. This work constitutes the first major effort within VHA to understand the impact of traveling Veterans. We found a significant traveling Veteran population (6.6% of the total number of appointments), distributed disproportionately across the VHA networks. Strong seasonal fluctuations in demand were also discovered, particularly for the VA Bay Pines Healthcare System, in Bay Pines, Florida. Our analysis further indicated that traveling Veterans imposed a large seasonal load (up to 46%) on the Module A clinic at Bay Pines. We developed seasonal autoregressive integrated moving average (SARIMA) models to help the clinic better forecast demand for its services by traveling Veterans. Our models were able to project demand, in terms of encounters and unique patients, with significantly less error than the traditional historical average methods. The SARIMA model for uniques was then used in a Monte Carlo simulation to understand how clinic resources are utilized over time. The simulation revealed that physicians at Module A are over-utilized, ranging from a minimum of 92.6% (June 2013) to maximum 207.4% (January 2013). These results evince the need to reevaluate how the clinic is currently staffed. More broadly, this research presents an example of how simple operations management methods can be deployed to aid operational decision-making at other clinics, facilities, and medical centers both within and outside VHA.

Abstract:

The study purpose is to link hospital structure, represented by each hospital’s professional contingent, service mix, and inpatient capacity; and its environment, characterized by the penetration of managed care enrollees. The secondary purpose is to test the relationship between hospital structural change and subsequent hospital performance. The study employs a non-experimental panel design, with a sample of 1882 community hospitals (service type: general medical and surgical). Environmental variables are measured for the base year 1989. Hospital structural variables are measured for 1989 and 1994, with change variables computed. Performance variables are measured for 1989 and 1995, with change computed for cost measures. Hospital structural change is viewed as a dependent variable related to the environment, as well as an independent variable related to performance. Descriptive data are extracted from the American Hospital Association Annual Survey of Hospitals. Hospital cost performance data are from the Health Care Financing Administration Prospective Payment System Minimum Data Sets. Hospital mortality data for 1989 are from Medicare Hospital Mortality Information. HMO enrollment data are extracted from the Interstudy Edge and aggregated to metropolitan statistical area (MSA) level. Market competition data are from the 1989 Area Resource File. A Herﬁndahl-Hirschman index (HHI) is calculated for each hospital’s MSA. Analytical hypotheses are tested using ordinary least squares (OLS) technique. Results from Part 1 suggest that where HMO penetration was relatively high, sample hospitals tended to contain growth in their registered nurse (RN) staff between 1989 and 1994. Higher HMO penetration is also associated with more stabilization in occupancy rates, preventive services, and ambulatory workload. In contrast, market competition is associated with changes to a higher Medicare case-mix index (CMI), and increase in ambulatory visits. Results from Part 2 indicate positive associations between increased RN staff and hospital cost growth between 1989 and 1995. Hospitals which did not experience an increased CMI are similarly linked with cost growth. Alternatively, reduction in hospital bedsize is associated with more controlled growth in hospital cost per patient day. Several control variables display noteworthy associations with the variables of interest. Theoretical and management implications for community hospitals are discussed.

Abstract:

ABSTRACT Background: In the COVID-19 (Coronavirus Disease 2019) pandemic crisis that has hit Indonesia since early March 2020, urgent action is needed to overcome the situation while maintaining and improving relationships with customers so that they are still satisfied with the quality of services in hospitals, especially inpatient services. Since the COVID-19 pandemic, there has been a significant decrease in the number of inpatient visits at Siloam Hospitals TB Simatupang. This study aimed to analyze the comparison of the level of quality of patient service before and after the COVID-19 pandemic in installation of Siloam hospital TB Simatupang installation. Subjects and Method: This was a descriptive observational study conducted at Siloam Hospitals TB Simatupang from September 2020. A sample of 88 patients was selected by purposive sampling. The data was collected by questionnaire. The operationalization of this research variable is service quality, which is measured from five dimensions, namely Tangibles, Reliability, Responsiveness, Assurance, and Empathy. The data analysis used a series of tests such as validity test, reliability test, descriptive statistics and independent sample T-test. Results: During the COVID-19 pandemic, patients had a better perception than before the COVID-19 pandemic, it can be seen from service quality (Mean= 4.11), while during the COVID-19 pandemic (Mean= 4.250). Conclusion: Quality of services provided in outpatient installations during the COVID-19 pandemic is better than before the COVID-19. Abstract:

ABSTRACT Background: Utilization of the telemedicine application is an alternative option for paediatric health services without a direct visit to hospitals, especially in pandemic or disease outbreak conditions. The important telemedicine services, especially for paediatric patients, need to be tackled by stakeholders and hospital management teams. This study aimed to investigate the acceptance and barrier in using telemedicine health services of hospitals among paediatric outpatients. Subjects and Method: A systematic review was conducted by searching from ScienceDirect and Scopus databases. The keywords were “telemedicine OR patient paediatric”. The inclusion criteria were open accessed and English-language articles published between 2019 to 2020. The data were reported by PRISMA flow chart. Results: Nine articles met the inclusion criteria. Feasibility and the easiness to use of the application, cost-effectiveness, less travel time, easy access to medicine, and effective health services were the optimal services received by paediatric outpatients in using telemedicine. The limitations of telemedicine services were lack of physical and diagnostic examinations, information for socio-demographic and socioeconomic status, patient insurance coverage, direct care services, and privacy and confidentiality of patients. Conclusion: Not all the conditions of paediatric outpatients receive optimal health services through telemedicine. An innovative approach is needed to improve telemedicine’s available health services, especially for paediatric outpatients who need direct health care without visiting the hospitals. The objective of this study was to track trends in the signs of higher-quality addiction treatment as defined by the National Institute on Drug Abuse, the National Institute on Alcohol Abuse and Addiction, and the Substance Abuse and Mental Health Services Administration. We analyzed the National Survey of Substance Abuse Treatment Services from 2007 through 2017 to determine the percent of facilities having the characteristics of higher quality. We analyzed the percent by state and over time. • We found improvements between 2007 and 2017 on most measures, but performance on several measures remained low. • Most programs reported providing evidence-based behavioral therapies. • Half or fewer facilities offered medications for opioid use disorder; mental health assessments; testing for hepatitis C, HIV, and sexually transmitted diseases; self-help groups; employment assistance; and transportation assistance. • There was significant state-level variation across the measures.

Implementation of the section 2302 of the 2010 Patient Protection and Affordable Care Act (ACA) enabled children enrolled in Medicaid/Children's Health Insurance Program with a prognosis of 6 months to live to use hospice care while continuing treatment for their terminal illness. Although concurrent hospice care became available more than a decade ago, little is known about the socio-demographic and health characteristics of children who received concurrent care; health care services they received while enrolled in concurrent care, their continuity, management, intensity, fragmentation; and the costs of care. The purpose of this study was to answer these questions using national data from the Centers of Medicare and Medicaid Services (CMS), which covered the first three years of ACA – from January 1, 2011, to December 31, 2013.The database included records of 18,152 children younger than the age of 20, who were enrolled in Medicaid hospice care in the sampling time frame. Children in the database also had a total number of 42,764 hospice episodes. Observations were excluded if the date of birth or death was missing or participants were older than 21 years. To create this database CMS data were merged with three other complementary databases: the National Death Index (NDI) that provided information on death certificates of children; the U.S. Census Bureau American Community Survey that provided information on characteristics of communities where children resided; CMS Hospice Provider of Services files and CMS Hospice Utilization and Payment files were used for data on hospice providers, and with a database of rural areas created by the Health Resources and Services Administration (HRSA). In total, 130 variables were created, measuring demographics and health characteristics of children, characteristics of health providers, community characteristics, clinical characteristics, costs of care, and other variables.

Abstract:

This Evidence Check was commissioned by the Ministry of Health to identify validated health assessment tools for physical health, mental health, development, and family violence for the Wellbeing and Health In-reach Nurse (WHIN) Coordinator program. This program is a partnership between NSW Health and the NSW Department of Education which places nurses in NSW schools to identify the health and social needs of students and coordinate early intervention and referral to services and programs. Validated assessment tools will help the nurses to identify children at risk of academic, behavioural, emotional or health-related difficulties. This Evidence Check also aims to describe how they are used in clinical practice and barriers and enablers to their effective use. Seventy-two assessment tools were found, but coverage of the areas affecting children’s wellbeing was uneven. Mental health had several promising tools, as did development for younger children. However, physical health and family violence did not have well-validated tools. There was little information on how they were used in clinical practice. Enablers for use included minimal training requirements, ease of administration and ready availability. Use of parental as well as teacher reports was seen as valuable. There is a need for further work on tools for physical health and family violence. There is also a need for information on the practicalities of the chosen tools (user acceptability, licensing, costs, and training requirements) and for clear practice guidelines.

Abstract:

Literature shows that IDPs struggle to access services, which has an impact on their ability to live healthy and fulfilling lives. In the field of health, IDPs frequently have worse outcomes than both host community and refugees. This rapid literature review finds evidence of a number of policy and administrative barriers to access of services for internally displaced persons (IDPs). IDPs remain citizens of the countries in which they are displaced, and the national authorities retain responsibility for meeting their basic rights. However, their displacement, loss of livelihoods and assets, lack of documentation, as well as discrimination against them, lack of protection under international law, lack of policy to address their needs, poor services and conflict or disaster conditions, can all make it more difficult for IDPs to access basic services than non-displaced citizens. There is relatively little literature systematically addressing the issue of administrative and policy barriers to service access among IDPs. Much of the literature discusses IDPs alongside refugees (who have a different legal status and access to different national and international support), or discusses the whole range of difficulties facing IDPs but does not focus on administrative or policy barriers. The literature frequently does not compare IDPs and other citizens and service users. Nevertheless, policy and administrative barriers are discussed, ranging from analysis of international instruments on IDPs to documentation procedures in particular countries. Much of the literature shows the prevalence of disease, lack of school attendance, limited provision of services etc. faced by IDPs, but does not discuss the policy and administrative barriers in detail.

Abstract:

As a leading funder of global health programs, the United States has the power to make a tremendous impact on people’s sexual and reproductive health and rights. But restrictions on funding that target abortion care internationally have had broad, detrimental impacts on reproductive health care systems, advocacy and outcomes. Such is the case with the so-called global gag rule, a policy that conditions US global health assistance on nongovernmental organizations’ agreement not to provide or promote abortion. Our multiyear research study in Uganda and Ethiopia examines the impact of this policy in two countries that rely on US assistance for their family planning programs but where the legal context around abortion differs—highly restrictive in Uganda and liberal in Ethiopia. Until now, no research has fully captured the effects of the most recent implementation of the global gag rule, which, during the four-year Trump administration, was the greatest expansion of the policy in its history. The research shows how, in both countries, the gag rule stalled and even reversed progress toward expanded access to modern contraception, impacting the countries’ reproductive health outcomes, the ability of people to decide whether and when to have children, and overall bodily autonomy. Abortion care cannot be separated from reproductive health care; evidence clearly demonstrates that the US government’s attempts to limit abortion care through the gag rule also limit access to other essential sexual and reproductive health services. Although the gag rule is currently not in effect, the risk of an anti-abortion president reinstating and expanding the gag rule and causing significant harm to reproductive health progress globally remains. And even after the gag rule is rescinded, its effects persist. The time for a permanent end to the global gag rule is now.

Abstract:

The COVID-19 pandemic changed how autistic people accessed services and engaged in their communities, ultimately impacting their quality of life. Access to appropriate services and accommodations help autistic individuals in maintaining employment, pursuing education, caring for their health, and establishing independence. Changes in access to services result in long-term consequences, which can be dire for autistic people. In an effort to improve policies and programs for autistic individuals, documentation of disruptions in accessing services during the COVID-19 pandemic informs better evidence-based practices for future public health emergencies. This report examines the impact of the COVID-19 pandemic on health and healthcare among autistic children and adults. To build a comprehensive picture, we included various data sources, including health care claims and administrative records. We explored the availability of services for autistic children based on caregiver report from the National Survey of Children’s Health (NSCH). To understand hospitalization covered by both private and public health insurance, we used national emergency hospitalization records (via the National Emergency Department Sample [NEDS]) and hospital admissions data (via the National Inpatient Sample [NIS]). Finally, we used patient medical records from Kaiser Permanente Northern California (KPNC) to look at service utilization among adult autistic patients from Northern California over the same period. These data sources cover various populations, some of which provide nationally representative pictures of autistic children and adults, others cover specific or regional populations but cover diverse populations in terms of income and race and ethnicity.

Abstract:

In this paper, we test whether promoting digital government tools increases the take-up of an important public health prevention service: cervical cancer screening. We implemented an at-scale field experiment in Uruguay, randomly encouraging women to make medical appointments with a digital application or reminding them to do it as usual at their local clinic. Using administrative records, we found that the digital application nearly doubled attendance of a screening appointment compared to reminders and tripled the rate compared to a pure control group (3.2 percentage point increase over a base of 1.9 percent). Survey data suggests that the impacts of the intervention were mostly mediated by reduced transaction costs. Our results highlight the potential of investing in digital government to improve the take-up of public services.

Abstract:

This report is part of a larger cross-country comparative project and constitutes an account and analysis of the measures comprising the Norwegian national response to the COVID-19 pandemic during the year of 2020. This time period is interesting in that mitigation efforts were predominantly of a non-medical nature. Mass vaccinations were in Norway conducted in early 2021. With one of the lowest mortality rates in Europe and relatively lower economic repercussions compared to its Nordic neighbours, the Norwegian case stands unique (OECD, 2021: Eurostat 2021; Statista, 2022). This report presents a summary of Norwegian response to the COVID-19 pandemic by taking into account its governance, political administration and societal context. In doing so, it highlights the key features of the Nordic governance model and the mitigation measures that attributed to its success, as well as some facets of Norway’s under-preparedness. Norway’s relative isolation in Northern Europe coupled with low population density gave it a geographical advantage in ensuring a slower spread of the virus. However, the spread of infection was also uneven, which meant that infection rates were concentrated more in some areas than in others. On the fiscal front, the affluence of Norway is linked to its petroleum industry and the related Norwegian Sovereign Wealth Fund. Both were affected by the pandemic, reflected through a reduction in the country’s annual GDP (SSB, 2022). The Nordic model of extensive welfare services, economic measures, a strong healthcare system with goals of equity and a high trust society, indeed ensured a strong shield against the impact of the COVID-19 pandemic. Yet, the consequences of the pandemic were uneven with unemployment especially high among those with low education and/or in low-income professions, as well as among immigrants (NOU, 2022:5). The social and psychological effects were also uneven, with children and elderly being left particularly vulnerable (Christensen, 2021). Further, the pandemic also at times led to unprecedented pressure on some intensive care units (OECD, 2021). Central to handling the COVID-19 pandemic in Norway were the three national executive authorities: the Ministry of Health and Care services, the National directorate of health and the Norwegian Institute of Public Health. With regard to political-administrative functions, the principle of subsidiarity (decentralisation) and responsibility meant that local governments had a high degree of autonomy in implementing infection control measures. Risk communication was thus also relatively decentralised, depending on the local outbreak situations. While decentralisation likely gave flexibility, ability to improvise in a crisis and utilise the municipalities’ knowledge of local contexts, it also brought forward challenges of coordination between the national and municipal level. Lack of training, infection control and protection equipment thereby prevailed in several municipalities. Although in effect for limited periods of time, the Corona Act, which allowed for fairly severe restrictions, received mixed responses in the public sphere. Critical perceptions towards the Corona Act were not seen as a surprise, considering that Norwegian society has traditionally relied on its ‘dugnadskultur’ – a culture of voluntary contributions in the spirit of solidarity. Government representatives at the frontline of communication were also open about the degree of uncertainty coupled with considerable potential for great societal damage. Overall, the mitigation policy in Norway was successful in keeping the overall infection rates and mortality low, albeit with a few societal and political-administrative challenges. The case of Norway is thus indeed exemplary with regard to its effective mitigation measures and strong government support to mitigate the impact of those measures. However, it also goes to show how a country with good crisis preparedness systems, governance and a comprehensive welfare system was also left somewhat underprepared by the devastating consequences of the pandemic.
