Resumo O objetivo é analisar a organização e oferta de atenção especializada (AE) e transporte sanitário nas Policlínicas Regionais da Bahia, via Consórcios Interfederativos de Saúde. Foi realizado estudo de caso em uma Policlínica, com abordagem qualitativa, a partir de entrevistas com atores municipais e estaduais. Buscou-se identificar elementos que caracterizam um modelo de AE integrado às Redes de Atenção à Saúde. Entre os avanços identifica-se ampliação da oferta de AE; garantia de escopo, qualidade e fixação de profissionais; provisão de transporte sanitário; monitoramento da prestação da AE; e avanços na regulação por sistema informatizado. Representam desafios: adequação do planejamento da AE à realidade locorregional; incentivo à coordenação do cuidado pela Atenção Primária à Saúde (APS) e à aproximação entre profissionais da AE e APS; institucionalização da contrarreferência, relação com instâncias de participação social; e incentivo às funções de matriciamento, Educação Permanente, integração ensino-serviço e pesquisa. Considera-se o arranjo das Policlínicas Regionais, mais afeito à integração às Redes de Atenção à Saúde, em que pese os desafios intrínsecos à AE e ao necessário fortalecimento da APS para que possa assumir a condução do sistema.

RESUMO Os Problemas de Saúde Mental (SM) e o uso indiscriminado de psicofármacos são problemas de grande relevância para a Atenção Primária à Saúde (APS) e a saúde pública. O objetivo deste ensaio é apresentar uma fundamentação atualizada da tese de Robert Whitaker, desenvolvida no livro ‘Anatomia de uma epidemia: pílulas mágicas, drogas psiquiátricas e o aumento assombroso da doença mental’. É apresentada uma síntese do livro, acrescida de comentários sobre determinados temas, visando à melhor ancoragem científica dos argumentos. A tese defendida é que se deve evitar prescrever o uso de psicofármacos; e, caso seja iniciado o uso, que seja como sintomático agudo pelo menor tempo possível. Os argumentos giram em torno de que há evidências favoráveis apenas para redução de sintomas, para algumas dessas drogas e para curtos períodos de uso. Com seu uso crônico, há piora em longo prazo quanto à estabilidade, autonomia e funcionalidade social, com problemas graves de abstinência. Especialmente na APS (e também nos serviços especializados em SM), os profissionais deveriam ter uma abordagem mais crítica dos psicotrópicos e investir em outras abordagens terapêuticas, para fazerem algo melhor, menos iatrogênico e tão ou mais eficaz para os pacientes com problemas de SM no longo prazo.

Resumo As lésbicas se deparam com inúmeras barreiras nos serviços de saúde e vivenciam experiências de preconceito, estigmatização e invisibilização de suas demandas de saúde. Neste artigo, buscou-se compreender os sentidos atribuídos por enfermeiras da atenção básica às práticas de cuidado em saúde com lésbicas. Trata-se de uma pesquisa qualitativa realizada com 15 enfermeiras que atuavam na atenção básica de Teresina, Piauí. A análise foi pautada no referencial teórico de habitus, campo e violência simbólica de Pierre Bourdieu. Observou-se que as práticas das enfermeiras no campo da atenção básica seguem protocolos heteronormativos. Portanto, é essencial que essas enfermeiras possam desenvolver outros habitus, a fim de garantir diferentes formas de identidades dentro dos serviços de saúde.

Abstract:
Based on SUS guidelines, with principles of universality, completeness and equity, we carried out a study with the objective of identifying conceptions, desires, fears and suggestions regarding humanized health care for transvestites, in primary health care. The theoretical-methodological path was the Dialectic Hermeneutics using an in-depth interview and use of "scenes", with seven participants. The interviewees reported difficulties in accessing and using health services. In view of the difficulties encountered, in order to carry out a humanized care, we identify some necessary changes in the interlocution of health with the transvestite population: Training of health professionals, dialogue with the social movement, dissemination campaigns and the approximation with the meaning of the experience of being transvestite There is an urgent need for effective work in the training of health professionals, as well as in the daily lives of our lives, surrounded by discriminatory attitudes, in the name of commitment to suffering the other.

Abstract:
Una aproximación para medir el nivel de atención expresada en la calidad de servicio que se presta al paciente en los establecimientos de salud está referida al uso de modelos de medición, que en este artículo se describen desde la propuesta por Donabedian basado en la teoría general de sistemas evaluando estructura, proceso y resultado, pasando a segundo plano la percepción del paciente. Este modelo contrasta con el modelo Nórdico que enfatiza la imagen corporativa, con un balance entre el servicio esperado y la percepción del servicio, basado en el paradigma de la desconfirmación al igual que el modelo presentado por Nguyen. En esta última línea paradigmática se encuentra el modelo más notorio, SERVQUAL que contrasta las expectativas y la precepción del usuario de un servicio identificando brechas en base a las 5 dimensiones propuestas fiabilidad, seguridad, elementos tangibles, capacidad de respuesta, y la empatía, posteriormente basado en este modelo se propone el modelo SERVPERF, basada exclusivamente en la valoración de las percepciones. Posteriormente, se describen el modelo de Desempeño Evaluado que surge como un modelo alternativo al SERVQUAL sustituyendo las expectativas por el concepto de “punto ideal”; el Modelo Jerárquico Multidimensional que desarrolla una solución tridimensional para completar la modelización de la calidad de servicio. De igual manera el modelo multietapa de valoraciones de la calidad que identifica la disconformidad que existe las expectativas del cliente en relación al servicio y las percepciones de las características por niveles de componente.
