During the past few decades, research from multiple disciplines has elucidated the profound connections between the immune system and mental health. This chapter provides a review of this literature, placing emphasis on the connections between inflammation and cytokines, and stress, depression, posttraumatic stress disorder, schizophrenia, bipolar disorder, and social support. Additionally, brief overviews of the role of the natural immune system and adaptive immunity, as well as past research investigating stress are included. Further attention is focused on the physical health consequences of immune system dysregulation, ranging from increased susceptibility to infectious diseases, delayed wound healing, autoimmune responses, age-related elevations in proinflammatory cytokines, and decreased effectiveness of protective vaccinations. The chapter concludes with a discussion of the health effects and clinical implications of the relationships discussed throughout, as well as future directions to advance this field of study.

This chapter examines quality of life (QoL) and psychosocial functioning in obsessive-compulsive disorder (OCD). More specifically, it summarizes recent investigations demonstrating that: (1) OCD negatively impacts multiple areas of life, such as social and family relationships, functioning at work and in the household, and aspects of physical and mental health; (2) the negative relation between OCD and QoL is as great as (or greater than) that observed in other psychiatric disorders and chronic medical conditions; (3) the degree of the QoL impairment is generally proportional to the severity of OCD symptoms; (4) specific OCD symptoms differentially impact aspects of psychosocial functioning; and (5) adequate treatment of OCD is associated with significant improvements in QoL. Finally, the chapter discusses future perspectives involved in the evaluation of QoL in OCD populations and the critical need to address these issues in assessment and treatment of individuals with OCD.

Abstract:
In Chapter 10, “Psychoeducational and Social Support Interventions: Evidence-Based Practice,” summarizes the results of meta-analyses, systematic reviews, and randomized controlled trials (RCTs) of psychoeducational and social support intervention studies with older adults and related populations. The majority of studies (n = 11) compared the effectiveness of interventions for deceasing depression and enhancing functioning among caregivers. Psychoeducational intervention studies were more prevalent than social support studies, and stronger evidence exists for the effectiveness of psychoeducational approaches. However, evidence does suggest that interventions types are useful in reducing caregiver depression and increasing caregiver support, well-being, and coping. Inconclusive results were found on caregiver burden. Four studies examined interventions focused on reducing depression, increasing well-being, or enhancing social functioning among older clients with cancer and other health/mental health issues. Positive results were found. Lack of intervention specificity, however, weakens practitioners and researchers’ ability to replicate the interventions included in these studies.

Where treatment and health care is no longer able to bring relief and improve functioning, social care should take over. In this chapter, we discuss the development of social care in the context of adult autism, and the range of its concerns and interests is considered. The key role of the social worker, particularly as a broker of social care, is developed. Health professionals define the need for reasonable adjustments to assessed disability, and the content of a personal passport, summarizing individual’s needs. Health professionals also have a key role in risk management, although the social worker may have a key co-ordinating role. A wide range of contexts for social care within and beyond health care is considered. The distinction between individual need and care planning, and the role of the wider society, which will be covered in Chapter 14, concludes this chapter.

Health services research is a practically focused discipline drawing on the social and behavioral sciences and concerned with the organization, financing, and delivery of services. Investigators studying HIV services examine how well healthcare and other systems meet needs, eliminate disparities, integrate services, eliminate barriers, and provide care to socially marginal and stigmatized individuals, such as injection drug users and people who are incarcerated. These issues have been important from the earliest days of the HIV epidemic and have taken on increased significance as efficacious treatments have been developed and, more recently, changes in the financing of care have reduced the number of uninsured. This chapter this focuses primarily on research in the United States with a review of recent work on financing, service fragmentation, and difficulties integrating different sectors of care, as well as problems related to the functioning of medicine in a complex, stratified society. Also addressed are certain general features of the Affordable Care Act that are relevant to HIV care and psychiatry.

Rates of chronic health conditions in childhood are increasing, and school-based professionals regularly encounter children with chronic health conditions in the school setting. Students with chronic health conditions often require accommodations, assessment, intervention, and close collaboration with medical providers and families. However, most school-based clinicians (school psychologists, counselors, social workers) who are charged with addressing the special needs of these children have not had coursework or experience related to common pediatric conditions. This book is a practical guide for school-based clinicians working with children with chronic health conditions. Section I provides a broad overview of school-related issues for children with chronic health conditions. This includes a review of common medical conditions and terminology and cross-cutting issues related to social and emotional and academic functioning, as well as the role of the school-based professional in collaborating across systems of care. The section also reviews legal and policy issues and alternative educational settings for students with chronic health needs. Section II focuses on prevention, assessment, intervention, and consultation strategies for individual students and entire school systems. Finally, Section III addresses common groups of medical conditions. Each chapter provides an overview of the condition(s), common school-related concerns, risk and protective factors, and cultural considerations, as well as practical strategies, resources, and handouts for the school-based professional. Case examples are used throughout the book to illustrate key concepts and implications for the school setting.

Abstract:
Economic rights refer to the right to property, the right to work, and the right to social security. Social rights are those entitlements necessary for an adequate standard of living, including rights to food, housing, health, and education. Since economic rights have a social basis, and social rights have an economic basis, both classifications are considered of equal importance and interdependent. The intellectual and social dimensions of economic and social rights have evolved from at least four spheres: religion, philosophy, politics, and law. Throughout history, individuals and groups debated and accepted obligations to help the needy and prevent suffering. There were both religious and secular dimensions to these undertakings. Early human rights advocates moreover proclaimed an interdependence between civil and political rights and economic and social rights and criticized those who made too sharp a distinction between them. A central debate over economic and social rights relates to their legal validity. Some scholars argue that by their very nature, economic and social rights are not “justiciable.” Another issue is the link between economic and social rights in meeting basic human needs and the alleviation of global poverty. The right to development is also important in debates on economic and social rights, as it attempts to correct the economic distortions left by the legacy of colonial domination. Perhaps the most promising new approach to economic and social rights is Amartya Sen’s capabilities approach, which focuses on what individuals need for adequate functioning.

This review provides an overview of research on associations between the multi-component, cognitive construct executive functioning (EF) and health. Executive functioning is defined, and issues related to measurement are detailed. The categories of potential mechanisms by which EF may be associated with health and disease are described. Key research examining EF and health behaviors, stress processes, and chronic illness is reviewed with a focus on function (behavioral performance), as well as neuroanatomical research where relevant. Across these domains, there is evidence that EF is associated with health and illness in reciprocal, feed-forward fashion across the life span. Critical limitations of the current literature are noted, along with important future directions.

Adolescents with chronic illness, particularly when accompanied by debilitating, painful, and/or fatiguing symptoms, face challenges that are disruptive to their normal physical, psychological, and social development. The Children’s Health and Illness Recovery Program (CHIRP) is an evidence-based program specifically designed to address the skills needed by adolescents with chronic illnesses to become more confident and independent in coping and managing their illness and lifestyle. The flexible 12-session format of CHIRP can be administered with individual teens and their families or conducted in teen groups with a parallel parent group component. CHIRP integrates and adapts effective treatment components from behavioral family systems therapy, cognitive behavioral therapy, coping strategies intervention, interpersonal psychotherapy, assertiveness training, among others, into therapeutic activities in the companion CHIRP Teen and Family Workbook. This CHIRP Clinician Guide provides detailed instructions for implementing the manualized treatment protocol in the workbook. CHIRP was developed from both a careful review of the evidence-based literature on treatments for adolescents with chronic physical illness and the authors’ more than six decades of combined experience in helping children and families improve their quality of life and independence while coping with a chronic illness. Clinical outcome data on teens who have completed CHIRP demonstrate significant improvement in independent functioning and reduction in symptoms of fatigue and chronic pain; longitudinal data suggest these improvements not only persist but that teens continue to make gains on these factors beyond the completion of treatment, allowing them to pursue meaningful life goals as they transition to young adulthood.

Chapter 4 examines the advancing field of mental health and psychosomatic disease from preconception to the postpartum period. The reader is reminded of the normal adaptation of different organ systems to pregnancy. This adaptation affects both physical and emotional functioning, and is further modified by the pregnant woman's social circumstances. The transition to the pathological or diseased condition may follow an exaggeration of the physiological alterations or could occur due to health conditions specific to pregnancy. This may result in manifestations due to mind-body interactions that cause psychosomatic disease. Common and unfamiliar psychosomatic clinical conditions associated with childbearing such as anxiety and mood disorders, eating disorders, hyperemesis gravidarum, and substance misuse are discussed, along with the unfamiliar, such as schizophrenia and seizures. Pregnancy-related acute-on-chronic psychosomatic presentations, besides those arising de novo in labour, are illustrated by vignettes representing real-life encounters. Controversies in management are debated to acquaint the less familiar with these clinical challenges, which require patient-centred care. Promoting health during childbearing not only pertains to the health of the mother, but also to the well-being of her infant. This entails concomitant attention to both in order to enhance the physical, mental and social health of the mother-infant dyad. An urgency for improved understanding of biopsychosocial initiating factors is reflected in an UK surveillance report, `Saving Lives Improving Mother's Care: It confirms the continuing fall in fatalities from 'direct' pregnancy-related physical causes, but a rise due to under-recognition of 'indirect' psychiatric causes that represent the psychosomatic interface.
