Abstract:
When users interact with computers and technology ‘in the wild’, multitasking is a practically ubiquitous part of their interactions. Human-computer interaction (HCI) researchers and practitioners have increasingly used computational models to better understand these multitasking behaviours and to build new interactive technologies that facilitate interaction and/or mitigate the problems that arise from multitasking and distraction. This chapter outlines three approaches for modelling: cognitive architectures, cognitive constraint modelling, and uncertainty modelling. These approaches are some of the most common and powerful approaches to computational models of user multitasking, and have complementary strengths. It draws on examples from several domains for which multitasking is a central component, giving a particular focus to in-car multitasking and driver distraction.

Abstract:
Policymakers across advanced welfare states have prioritized programs to enhance the employability of unemployed people and help them to find and sustain work. In this regard, analysts have drawn attention to the difference between Work First and Human Capital Development (HCD) models. The former seek to direct people to any available job as quickly as possible; the latter seek to improve long-term employability through investments in human capital (typically via education and training). This chapter deploys a framework for comparing Work First‒ and HCD-oriented approaches to employability, identifying differences in rationales, content, and outcomes. A key conclusion is that policymakers (and indeed researchers) need to adopt a broader, more holistic view of the factors affecting the unemployed. A better understanding can inform the development of programs that combine Work First and HCD elements and address the problems that explain why some people face prolonged periods excluded from the workplace.

In this chapter we explore the potential advantages of modeling the interaction between a human and a computer as a consequence of a Partially Observable Markov Decision Process (POMDP) that models human cognition. POMDPs can be used to model human perceptual mechanisms, such as human vision, as partial (uncertain) observers of a hidden state are possible. In general, POMDPs permit a rigorous definition of interaction as the outcome of a reward maximizing stochastic sequential decision processes. They have been shown to explain interaction between a human and an environment in a range of scenarios, including visual search, interactive search and sense-making. The chapter uses these scenarios to illustrate the explanatory power of POMDPs in HCI. It also shows that POMDPs embrace the embodied, ecological and adaptive nature of human interaction.

From the early days of hemodialysis, viral hepatitis has been recognized as common in dialyzed patients.The prevalence and incidence of HBV infection have decreased markedly over the last decades in HD units. Still, the infectivity of HBV is very high. Vaccinating HD patients, preferably prior to starting dialysis, together with the strict application of hygienic precautions and adequate screening of blood donors remains required, together with the segregation of infective (HBV+) patients in a separate dialysis ward. The level of aminotransferases is markedly lower in HD patients than in the general population: any level above the normal range should thus trigger the suspicion of acute hepatitis (viral or not). The treatment of HBV infection in HD patients is rarely required, unless they are scheduled for a kidney transplant.Screening for HCV infection usually relies on a modern ELISA test. The prevalence and incidence of HCV infection in HD patients has also decreased substantially but remains higher than in the general population. The risk of post-transfusional HCV is currently extremely low, at least in western countries. The actual application of basic hygienic precautions is crucial if nosocomial transmission of HCV is to be prevented. These include optimal hand hygiene practices (hydroalcoholic solution use before contact with patient and after gloves withdrawal), the systematic wearing of gloves, to be changed between patients/stations, an adequate separation of the clean and contaminated items and circuits within the HD unit, and regular cleaning/disinfection of potentially contaminated surfaces. The necessity and usefulness to isolate HCV positive patients in a separate dialysis ward has not been demonstrated and is not recommended by current KDIGO guidelines. The field of the treatment of HCV infection is changing rapidly with many orally active drugs, some of which can be used even in dialysis patients.

Abstract:
Sudden cardiac death (SCD) secondary to ventricular arrhythmias is the most common mode of death in hypertrophic cardiomyopathy (HCM) and can be effectively prevented with an implantable cardioverter defibrillator (ICD). The risk of SCD in HCM relates to the severity of the phenotype and regular risk stratification is an integral part of routine clinical care. For the primary prevention of SCD, risk stratification involves the assessment of seven readily available clinical parameters (age, maximal left ventricular wall thickness, left atrial diameter, left ventricular outflow tract gradient, non-sustained ventricular tachycardia, unexplained syncope, and family history of SCD) which are used to estimate the risk of SCD within 5 years of clinical evaluation using a statistical risk prediction model (HCM Risk-SCD). The 2014 European Society of Cardiology Guidelines provide a framework to aid clinical decisions and consider patients with a 5-year risk of SCD of less than 4% as low risk and recommend regular assessment while those with a risk of 6% or higher should be considered for an ICD. In patients with an intermediate risk (4% to <6%) ICD implantation may also be considered after taking into account age, co-morbid conditions, socioeconomic factors, and the psychological impact of therapy. Survivors of ventricular fibrillation arrest should receive an ICD for secondary prevention unless their life expectancy is less than 1 year. Following device implantation, patients should be followed up for device- and disease-related complications, particularly heart failure and cerebrovascular disease.

Double ionisation of the triatomic molecules presented in this chapter shows an added degree of complexity. Besides potentially having many more electrons, they have three vibrational degrees of freedom (three normal modes) instead of the single one in a diatomic molecule. For asymmetric and bent triatomic molecules multiple modes can be excited, so the spectral bands may be congested in all forms of electronic spectra, including double ionisation. Double photoionisation spectra of H2O, H2S, HCN, CO2, N2O, OCS, CS2, BrCN, ICN, HgCl2, NO2, and SO2 are presented with analysis to identify the electronic states of the doubly charged ions. The order of the molecules in this chapter is set first by the number of valence electrons, then by the molecular weight.

Combinatorial optimization offers a rigorous but powerful approach to user interface design problems, defining problems mathematically such that they can be algorithmically solved. Design is defined as algorithmic combination of design decisions to obtain an optimal solution defined by an objective function. There are strong rationale for this method. First, core concepts such as ’design task’, ’design objective’, and ’optimal design’ become explicit and actionable. Second, solutions work well in practice, even for some problems traditionally out of reach of manual solutions. The method can assist in the generation, refinement, and adaptation of design. However, mathematical expression of HCI problems has been challenging and curbed applications. This chapter introduces combinatorial optimisation from user interface design point of view, and addresses two core challenges: 1) mathematical definition of design problems and 2) expression of evaluative knowledge such as design heuristics and predictive models of interaction.
