Abstract:
Medical device systems Clinical Engineering (CE)/Health Technology Management (HTM) strategies and best practices are now well established in most first world and many developing countries (DC).Progress is being made to address identified gaps in DC CE/HTM, such as appropriate equipment selection and lifecycle management. One contributor to this progress is the 25 years of CE/HTM Seminars provided by WHO-PAHO, ACCE, and more recently, IFMBE CED, to 80 countries. There is also a new emerging challenge; the requirement for medical device (clinical data) integration (MDI) into electronic health records (EHRs) to improve care quality and safety (aka CE-IT).This study will review CE/HTM progress, gaps, and new challenges since the last study in 2011. It will provide a framework to direct the global CE/HTM movement forward in collaborative fashion, alongside other initiatives in 2015, such as the 1st International CE-HTM Congress and the Global CE Summit held in Hanzhou, China, in October, 2015.

Yelüabaoji(耶律阿保機)’nin Hitaylar(Qidan:契丹)’ı birleştirmesiyle 916 yılında Mançurya(滿洲)’da Liao Hanedanlığı(遼) kurulmuştur. Daha sonra çevre bölgeleri de fethederek gelişme göstermiştir. 926 yılında Balhae Krallığı(渤海)’nı yıkarak Mançurya bölgesini hakimiyeti altına almış ve Çin’in Song Hanedanlığı(宋) ile yaptığı birçok savaş sonucunda Kuzeydoğu Asya’nın lider devleti konumuna yükselmiştir. Daha sonra Song Hanedanlığı ile Chanyuan Antlaşması(澶淵之盟)’nın yapılması sonucunda Kuzeydoğu Asya’da güç dengesi kurulmuştur. Bu dönemde aktif rol oynayan kişi ise Handerang(韓德讓)’dır. Hanzu(漢族) kökenli olan Handerang(韓德讓:941-1011), Liao Hanedanlığı’ndan Jingzong(景宗) döneminde memuriyete başlamış ve Shengzong(聖宗) döneminin sonuna kadar vezir olarak görev yapmıştır. Handerang’ın dedesi Hanzhigu(韓知古), Liao Hanedanlığı’na esir olarak gelmiştir. Handerang’ın babası Hankuangsi(韓匡嗣)’nin ise Liao Hanedanlığı’nda bürokrat olması sonucunda aile yükselme göstermeye başlamıştır. Daha sonra Handerang, Shengzong’un Liao Hanedanlığı tahtına çıkmasında önemli bir rol oynayınca ailesi de Liao Hanedanlığı’nda önemli aileler arasına girmeye başlamıştır. Ayrıca Song Hanedanlığı ile yapılan çok sayıda savaşta aktif bir çalışma göstermiştir. Handerang, Liao Hanedanlığı ile Song Hanedanlığı arasında Chanyuan Antlaşması’nın imzalanmasını sağlayarak Liao Hanedanlığı’nı güçlü bir devlet haline getiren ve Kuzeydoğu Asya’da güç dengesinin oluşmasını sağlayan kişidir. Handerang, Hanzu kökenlidir ancak Liao Hanedanlığı’nda bürokrat ve vezir olarak faaliyet göstermiştir. Bu durumun Handerang ile Osmanlı’nın devşirme kökenli vezirleri arasında ortak bir nokta olduğu söylenebilir. Ayrıca Handerang, Büyük Selçuklu Devleti döneminde vezir olan Nizamü’l-Mülk(1018-1092) ile de karşılaştırılabilir. Handerang’ın hayatını öğrenmek amacıyla bu araştırmada Liaoshi(遼史) tarih kaynaklarında geçen Handerang hakkındaki Liezhuan(列傳) kayıtları incelenecektir.

Abstract:
Understanding the spatiotemporal characteristics of trade-offs and synergies among multiple ecosystem services (ESs) is the basis of sustainable ecosystem management. The ecological environment of valley basins is very fragile, while bearing the enormous pressure of economic development and population growth, which has damaged the balance of the ecosystem structure and ecosystem services. In this study, we selected two typical valley basins—Guanzhong Basin and Hanzhong Basin—as study areas. The spatial heterogeneity of trade-offs and synergies among multiple ESs (net primary production (NPP), habitat quality (HQ), soil conservation (SC), water conservation (WC), and food supply (FS)) were quantified using the correlation analysis and spatial overlay based on the gird scale to quantitatively analyze and compare the interaction among ESs in two basins. Our results found that: (1) Trade-offs between FS and other four services NPP, HQ, SC, and WC were discovered in two basins, and there were synergistic relationships between NPP, HQ, SC, and WC. (2) From 2000 to 2018, the conflicted relationships between paired ESs gradually increased, and the synergistic relationship became weaker. Furthermore, the rate of change in Guanzhong Basin was stronger than that in Hanzhong Basin. (3) The spatial synergies and trade-offs between NPP and HQ, WC and NPP, FS and HQ, SC and FS were widespread in two basins. The strong trade-offs between pair ESs were widly distributed in the central and southwest of Guanzhong Basin and the southeast of Hanzhong Basin. (4) Multiple ecosystem service interactions were concentrated in the north of Qinling Mountain, the central of Guanzhong Basins, and the east of Hanzhong Basin. Our research highlights the importance of taking spatial perspective and accounting for multiple ecosystem service interactions, and provide a reliable basis for achieving ecological sustainable development of the valley basin.

AbstractThe genetic variations of the 5′-upstream region and the second intron of the porcine H-FABP gene were investigated by polymerase chain reaction–restriction fragment length polymorphism (PCR-RFLP) in 256 pigs, including Duroc, Large White, Landrace, Neijiang, Rongchang, Bamei pig, Hanjiang Black, Hanzhong White, and wild pigs. The effect of the H-FABP gene on intramuscular fat (IMF) content was analysed by the least square method. Results showed a HinfI-RFLP in these eight pig breeds and wild pigs, among which Large White, Bamei pig, Hanjiang Black, Hanzhong White and wild pigs presented low polymorphism, while the other breeds had intermediate polymorphism. There were no HaeIII or MspI-RFLPs in the four Chinese local pig breeds tested, but Duroc, Landrace, Large White, Hanzhong White and wild pig displayed polymorphism. Landrace, Large White and wild pigs had low levels of HaeIII- and MspI-RFLP, while the others had intermediate polymorphism. H-FABP genotypes significantly affected IMF content (P<0.05). IMF content according to H-FABP genotypes were HH>Hh>hh, DD<Dd<dd, AA<Aa<aa. The genetic effect values were 3.95 (HH), 3.48 (Hh), 3.23 (hh), 2.33 (DD), 2.55 (Dd), 2.97 (dd), 2.34 (AA), 2.76 (Aa) and 3.01 (aa). Results suggest that porcine meat quality may be improved by increasing the frequency of genotype aa-dd-HH in pig breeds.

The rural residences of Northwest China are characterized by a state of high energy consumption and low comfort due to the limited economic level and awareness of energy-saving compared with the urban residences. To remedy this, appropriate passive design strategies should be adopted first, in order to provide a design mode with low energy consumption and low cost for rural residences under the premise of thermal comfort. In this paper, taking Hanzhong region (Shaanxi Province, China) as an example, we establish a benchmark model based on a field survey and develop an optimization process by combining EnergyPlus simulation software, the MOBO optimization engine, and weighted sum method. The action mechanisms of passive design parameters, including the building orientation, length–width ratio, building envelope parameters, external shading system, and window–wall ratio, on heating, cooling, and total energy consumption are analyzed, and the quantitative relationships between single-parameter and energy consumption are established. Then, the mutually restricted indices of total energy consumption and initial investment cost are taken as optimization objectives, and 17 design parameters are selected as the optimization variables. The NSGA-II algorithm is adopted to conduct the multi-parameter, multi-objective optimization design for rural houses in Hanzhong area, through coupling of the EnergyPlus and MOBO software. In this way, Pareto solutions are obtained and the value distributions of the multi-objectives and design parameters are analyzed. Based on the actual requirements of decision-makers and using the weight method, three design schemes focusing on different performance tendencies are proposed. The results indicate that by using the proposed optimization process, the building energy consumption can be significantly reduced while taking initial investment costs into account, where the energy-saving rate is in the range of 31.9–61.5%. When the EC/IC ratio is 1:1, 2:1, and 1:2, the energy-saving rate can reach 51.5%, 57.8%, and 43.5%, respectively. It can provide a beneficial pattern for the energy-saving design and renovation of rural residences in Hanzhong area of China.

As one of the most important poultry worldwide, ducks (Anas platyrhynchos) are raised mainly for meat and egg products, and muscle development in ducks is important for meat production. Therefore, an investigation of gene expression in duck skeletal muscle would significantly contribute to our understanding of muscle development. In this study, twenty-four cDNA libraries were constructed from breast and leg muscles of Hanzhong Ma ducks at day 17, 21, 27 of the embryo and postnatal at 6-month-old. High-throughput sequencing and bioinformatics were used to determine the abundances and characteristics of transcripts. A total of 632,172,628 (average 52,681,052) and 637,213,938 (average 53,101,162) reads were obtained from the sequencing data of breast and leg muscles, respectively. Over 71.63% and 77.36% of the reads could be mapped to the Anas platyrhynchos genome. In the skeletal muscle of Hanzhong duck, intron variant (INTRON), synonymous variant (SYNONYMOUS_CODING), and prime 3′ UTR variant (UTR_3_PRIME) were the main single nucleotide polymorphisms (SNP) annotation information, and “INTRON”, “UTR_3_PRIME”, and downstream-gene variant (DOWNSTREAM) were the main insertion-deletion (InDel) annotation information. The predicted number of alternative splicing (AS) in all samples were mainly alternative 5′ first exon (transcription start site)-the first exon splicing (TSS) and alternative 3′ last exon (transcription terminal site)-the last exon splicing (TTS). Besides, there were 292 to 2801 annotated differentially expressed genes (DEGs) in breast muscle and 304 to 1950 annotated DEGs in leg muscle from different databases. It is worth noting that 75 DEGs in breast muscle and 49 DEGs in leg muscle were co-expressed at all developmental points of comparison, respectively. The RNA-Seq data were confirmed to be reliable by qPCR. The identified DEGs, such as CREBL2, RHEB, GDF6, SHISA2, MYLK2, ACTN3, RYR3, and STMN1, were specially highlighted, indicating their strong associations with muscle development in the Hanzhong Ma duck. KEGG pathway analysis suggested that regulation of actin cytoskeleton, oxidative phosphorylation, and focal adhesion were involved in the development of skeletal muscle. The findings from this study can contribute to future investigations of the growth and development mechanism in duck skeletal muscle.

The sensitivity analysis of the parameters is an essential part in the hydrological forecasting. In this paper, based on GLUE method and SMAR model, the scatters of likelihood values of nine parameters in both of Hanzhong and Mumahe basin can be generated. From the scatterplots of the parameters in SMAR model, the sensitivity of parameters can be determined by three levels that are non-sensitive level (LevelI), sensitive level (Level II) and basin-sensitive level (Level III). It is concluded that: the parameters of C, Z, Y, Kg belong to Level I; the parameter of K belongs to Level II; the parameters of H, T, G, N belong to Level III) . It indicates that the problem of redundant parameters exists in SMAR model.

We aimed at investigating the prevalence and associated factors of patient delay in hospital visiting and weekend effect of disease surveillance on hand-foot-and-mouth disease and epidemic parotitis/mumps. Methods. Daily report data on hand-foot-and-mouth disease and epidemic parotitis cases between January 1, 2014, and December 31, 2017, in Hanzhong, Shaanxi, China, were collected. The patient delay in hospital visiting was defined by the date difference between disease onset and patient’s visit to hospital. Differences of delayed durations and percentages were compared by using nonparametric or χ2 tests across gender, age, occupation, disease classification, epidemic and nonepidemic seasons, and years of disease onset. Additionally, to determine whether there existed a weekend effect of disease surveillance, the mean cases reported on weekdays and weekends were also compared. Results. A total of 14,814 patients with hand-foot-and-mouth disease and 4013 with epidemic parotitis were recorded, respectively. We found that 43.1% of the hand-foot-and-mouth disease and 36.5% of the epidemic parotitis patients had delayed visiting to hospital. All patients were reported through the online surveillance system on the day of visiting hospital. The percentage of delayed visiting to hospital differed significantly by years and epidemic and nonepidemic seasons and between children in and not in childcare center (all p values <0.05). In addition, the reported numbers of both diseases fluctuated on weekdays but obviously decreased on weekends regardless of the epidemic or nonepidemic seasons. Conclusions. The reported cases of HFMD and epidemic parotitis had an obvious weekend effect, with an increasing tendency of cases delaying in hospital visiting over the recent years in Hanzhong, China. Parents and caregivers rather than health systems should be primarily targeted for the prevention and control of infectious diseases and their local outbreaks such as community-based education on the second-dose vaccination of mumps and/or hand hygiene.

At 22:55 on June 17, 2019, an earthquake of magnitude 6.0 occurred in Shuanghe Town, Changning County, Yibin City, Sichuan Province. In Shaanxi Province, the obvious earthquake sense appeared in some places which are 600 km to 800 km away from the epicenter. Based on the collection, collation and analysis of the data about Weihe Basin, Hanzhong Basin and Ankang Basin in Shaanxi Province, and the analysis of representative seismic network records and seismic wave propagation distance in each basin, the mechanism of the long-period disaster induced by the Changning Ms6.0 earthquake in Shaanxi Province is revealed. This study has a positive significance for the seismic fortification of long-period structures in this area, the scientific popularization of long-term ground motion effect, and the avoidance of earthquake panic.

Background. Hyperuricemia has long been associated with increased cardiovascular risk, and arterial stiffness is proposed as a mediator. The present study is aimed at examining the associations of uric acid (UA) in blood and urine with arterial stiffness in a Chinese cohort. Methods. A total of 2296 participants (mean age: 43.0 years) from our previously established cohort of Hanzhong Adolescent Hypertension Study were included. The participants were classified as subjects with or without arterial stiffness, which was defined as brachial-ankle pulse wave velocity baPWV≥1400 cm/s and/or carotid intima-media thickness CIMT≥0.9 mm. Multivariate regression analyses were used to examine the relationship between serum and urinary UA and the risk of arterial stiffness after adjusting for age, gender, systolic blood pressure, fasting glucose, BMI, heart rate, total cholesterol, and triglycerides. Results. baPWV was positively correlated with urinary uric acid/creatinine ratio (uUA/Cre) (β=0.061, P<0.001), while CIMT was correlated with uUA/Cre (β=0.085, P<0.001) and fractional excretion of uric acid (FEUA) (β=0.044, P=0.033) in all subjects. In addition, uUA/Cre was significantly associated with the risk of high baPWV [1.032 (1.019-1.045)] and arterial stiffness [1.028 (1.016-1.040)]. Conclusion. Our study showed that urinary UA excretion was significantly associated with the risk of arterial stiffness in Chinese adults. These findings suggest that UA, especially urinary UA, may be used as a simple, noninvasive marker for early detection of arterial stiffness in otherwise healthy subjects.

The analysis shows that, in Hanzhong basin, Ankang basin, and Weihe basin, serious damage should be caused by the long-period effect of basin of large earthquake in the far field. That magnitude of Wenchuan Earthquake is high, three basins are at the direction where the Wenchuan Earthquake wave propagated, and the epicentre is from 400 to 600km determines that the Wenchuan earthquake has the condition producing long-period effect in the three basins. In addition, the basins have very thick deposits, as well as possible basin edge effect may enhance long period damage. The damage of long-period of water tower results from the resonance between water tower and long-period ground motion.

With the system, we did the visual evaluation for that region considering different motion ways, which differs from the past single-factor qualitative research, for the aim of facilitating the landscape quality quantification.

The study of driver behavior is of great importance to the traffic safety of mountainous freeways. In order to study the characteristics of driver behavior on combination of vertical and horizontal curves (CVHCs) of mountainous freeways in free flow conditions, designated speed measurement tests of two typical segments of the upgrade direction of Xi’an-Hanzhong freeway were carried out. After data processing, vehicles in free flow were screened out and classified into two groups by K-means clustering method, and then the driver behavior with different lanes, different size vehicles, and different CVHCs was analyzed, respectively. Finally, a vehicle distribution prediction model and a speed prediction model were built which were applied to CVHCs, and a verification test was made to test the accuracy of the models. Research results show that the driver behavior is mainly different among vehicle size, longitudinal slopes, and horizontal curves, and the characteristics of speed control and lane distribution on CVHCs vary according to lanes and combination of road alignment. Also, the prediction results of the models are highly consistent with the measured test results.

Wheat stripe rust, caused by Puccinia striiformis f. sp. tritici (Pst), is an important disease on wheat, seriously threatening wheat production worldwide. China is one of the largest stripe rust epidemic regions in the world. The pathogen sexual reproduction and migration routes between Tibet and the other regions in China are still unknown. In this study, we obtained 961 Pst isolates from 1,391 wheat leaf samples from Gansu (277), Shaanxi (253), Sichuan (172), and Tibet (259), comprising 13 natural populations, and genotyped them with simple sequence repeat (SSR) markers. The isolates can be divided into two distinct clusters based on DAPC and STRUCTURE analyses. The genetic diversity of Longnan (in Gansu) and Yibin (in Sichuan) populations was the highest and lowest among the 13 populations, respectively. The hypothesis of multilocus linkage disequilibrium was rejected for the populations from Linzhi in the Himalayan, Longnan, Hanzhong, Guangyuan, Mianyang, Liangshan, and Chendu in the south Qinling Mountains at the level of P = 0.01, which indicated significant linkage among markers in these populations. Populations in the other regions had extensive gene exchange (Nm > 4); little gene exchange was found between Tibet and the other regions (Nm < 1). The results suggest that the Tibet epidemic region of Pst is highly differentiated from the other epidemic regions in China.

The number of breeding pairs of crested ibis (Nipponia nippon) in Hanzhong, China has recovered remarkably from 2 to 511 from 1981 to 2019. Although the crested ibis has been closely monitored, the habitat preference of the bird has not been well studied despite the extensive increase in abundance. We used nest site data from the past 39 years and 30 environmental variables to develop species distribution models for each year. We applied random forest to select important environmental variables, and used logistic regressions to quantify the changes in habitat preferences in 39 years, taking into account the effects of interaction and quadratic terms. We found that six variables had strong impacts on nest site selection. The interaction term of rice paddies and waterbodies, and the quadratic term of precipitation of the wettest quarter of the year were the most important correlates of nest presence. Human impact at nest sites changed from low to high as birds increased their use of ancestral habitats with abundant rice paddies. We concluded that during the population recovery, the crested ibises retained their dependence on wetlands, yet moved from remote areas to populated rural regions where food resources had recovered due to the ban of pesticide use.
