Abstract:
Antiseptics in the liquid form known as hand sanitizers effectively reduce the spread of disease-causing microorganisms without hindering human movement during the COVID19 pandemic. Hand sanitizer is a purchased and homemade product to meet the family’s antiseptic needs using easy-to-find ingredients. Kadiri University Community Service Team, through Kukerta 2021 program in collaboration with Gayam Village Government, conducted a counseling and training program on hand sanitizer making. The hand sanitizer utilized materials accessible to the public, namely Ivory Betel (Epipremnum aureum), commonly found as an ornamental plant around the Gayam Village area. Hand sanitizer products have the potential to be developed. Making natural hand sanitizers in liquid form is easy and requires low costs with simple equipment and economic value. The results of this community service activity can be used to meet the preservation needs of the family and be sold through village grocery stores and village unit cooperatives in Gayam Village by installing a product label for the broader market.

Abstract:
ABSTRACT BACKGROUND. There have been few studies conducted in hospitalized patients in Viet nam on the epidemiology of surgical site infections (SSIs) and the impact of hand hygiene practices. This study aimed to assess the impact of a bedside hand sanitizer program on SSIs in orthopaedic and neurosurgical patients. DESIGN. A prospective quasi-experimental study was conducted with an untreated control group design in neurosurgical patients and before-after design in orthopaedic patients. A cost analysis based on data derived from the results of this study was also performed. SETTING. Cho Ray Hospital, a tertiary university hospital in Ho Chi Minh City, Viet nam. PATIENTS. All patients admitted for operation between 11 July and 15 August 2000 (Before), and 14 July and 18 August 2001 (After) were included, except those who had undergone another operation within one month prior to admission or were admitted because of SSIs. INTERVENTION. Bedside hand sanitizers were introduced into the Orthopaedic ward and one Neurosurgical ward (Ward A) from September 2000. Training on proper use was also provided to ward staff. Another Neurosurgical ward (Ward B) was used as a control group with no intervention conducted. RESULTS. A total of 1368 patients were recruited into the study. After intervention, in Ward A of the neurosurgical department, the SSI rate between the two periods was reduced by 54% (8.3% to 3.8%; p=0.09). Superficial SSIs were eliminated after the intervention (p=0.007). Comparison between Ward A (intervention) and Ward B (control) showed that, before the intervention, there was no difference in incidence of SSI between the two wards (Ward A: 8.3%, Ward B: 7.2%, p=0.7); however, after intervention, the incidence of SSI in Ward A was significantly lower than Ward B (3.8% and 9.2%, p=0.04). For orthopaedic patients, the SSI rate between the two periods was reduced by 34% (14.8% to 9.8%; p=0.07). SSI patients had a median post-operative length of stay of 19 days longer than patients without SSI (p<0.001). Costs were 2.5 times higher in patients with in-hospital SSI compared to uninfected patients (p<0.001). Mean SSI-attributable costs were conservatively estimated at US$368 in neurosurgical patients and US$207 in the orthopaedic patients in the before period. SSIs were responsible for at least 14 percent of the annual budget before intervention. The savings per SSI prevented were estimated at US$332 in neurosurgical patients and US$157 in orthopaedic patients. Annual cost savings arising from the intervention were estimated at US$11,112 in orthopaedic patients and US$19,320 in neurosurgical patients. CONCLUSIONS. The incidence of SSI in the hospital was high. The use of hand sanitizers reduced SSI rates, particularly impacting on the incidence of superficial SSIs. The hand sanitization program was found to be a dominant intervention being both more effective and cost saving as compared with no intervention in both study departments. The use of bedside hand sanitizers should be encouraged in the hospitals in Viet nam, where there often is a lack of other hand-washing facilities.

Abstract:
"One of the major concerns among the relevant public authorities during the 2019 coronavirus disease (Covid-19) pandemic is the attitude and behavior of the Malaysian society regarding compliance with self-care Covid-19. Although the number of Covid-19 cases is decreasing, public authorities, such as the Malaysian Ministry of Health continually remind people to adhere to the Standard Operating Procedure (SOP) for Covid-19 to reduce the number of cases. To support the authorities’ efforts, a one-day self-care Covid-19 programme involving 10 youths (3 males & 7 females) with a mean age of 17.35 (SD=3.36) was implemented in Bongol village, Tamparuli. To adhere the Covid-19 SOP regulation which prohibits a large number of people from gathering in a confined, crowded and closed spaces, only a few participants were involved. The programme, which was conducted at the Bongol village community hall, involved various organized activities emphasising the three elements of attitude: cognitive, affective, and psychomotor. Before the programme began, all the participants were registered, and their body temperatures scanned to ensure that they were free from any Covid-19 symptoms. Each participant was given a mask and a small bottle of hand sanitiser that could be used throughout the programme. The activities comprised an ice-breaker, a talk on personal self-hygiene, a 20.02-minute self-care video produced by 28 psychology students, personal self-reflections by the participants, a group exercise, a community song, and a two-way discussion on self-care. The Covid-19 self-care programme, implemented with guidance from the Yale Attitude Change Model, emphasizes the practical issue of ‘who says what to whom and with what effects. The participants’ attitude was measured before and after they completed the one-day programme. The results of a Wilcoxon signed-ranked test study showed that there is a significant difference between the participants’ pre- and post-study attitudes towards self-care. The study results showed that the Covid-19 self-care programme, which is based on the social psychology approach, can help foster positive youth attitudes towards self-care. In regard to the authorities’ efforts to lower the number of Covid-19 cases to zero, it is suggested that each party needs (either governmental and non-governmental agencies) to support the Covid-19 campaign and programme by sharing and delivering self-care messages in creative ways to Malaysian communities, especially those in rural areas."
