<p>Koror Babeldaob Bridge, also called Koror Babelthuap Bridge or simply Koror Bridge, connects the islands of Koror and Babeldaob in the Republic of Palau. The design of the bridge began in 1974 and was based on the prevailing AASHO Standard Specifications at that time and was supplemented by ACI and CEB-FIP design recommendations on an as-needed basis. When the Koror Bridge was opened to traffic in April 1977, it was the world's longest concrete girder span. A few years later, the bridge began to deflect more than had been anticipated. The owner commissioned a Japanese engineering firm in 1985 and then a US engineering firm in 1993 to conduct in-depth investigations of the structure. Both firms came to the same conclusion that the bridge was structurally safe and that the excessive deflection was an unexplainable phenomenon. Nevertheless, in order to improve the driving quality of the bridge deck, the owner decided to repair the bridge. The repair scheme made changes to the structural system and added a large amount of post-tensioning force to the bridge. Unfortunately, less than three months after the repair, late in the afternoon on 26 September, 1996,nineteen and a half years after it was opened to traffic, the bridge collapsed. Thereafter, most of the documents were sealed as a result of litigation between the various parties and the debris was cleared. For a long time, it was impossible to study the facts surrounding the bridge's collapse. Only recently, through continuous probing by a group of engineers, were these documents made accessible to researchers.</p>

This book deals with the issue of the quality of education systems by examining the key factors that influence learning outcomes, and by highlighting in a systemic way the determinants of this quality. I have shown the importance of considering the quality of learning outcomes as the ultimate and unavoidable objective of any education system that aims to be efficient and effective. I reviewed the theoretical bases that concern the politics, approaches and mechanisms of quality management applied to education. I developed a model called Elmandjra, which tries to explain the influence links that can exist between the inputs, the processes and the outputs of an education system, and how they must function to improve the quality of the learning outcomes. To answer the problem of the book, I compared the proposed model to the reality of the Moroccan educational system based on a research methodology that combines discussion within a focus group, and analysis of statistical data and results of international surveys concerning the Moroccan education system. The results of the work made it possible to establish a model for the quality of education systems comprising nine criteria. They also generated 52 indicators which use will enable other researchers to apply the model to the institutional assessment of education systems. I ended my book by presenting the conclusions on the state of the Moroccan education system that the exploitation of the Elmandjra model made it possible to draw, as well as by recommendations to improve the quality of the Moroccan education system, which other countries can to be inspired to improve theirs.

The larval stage of the parasite Taenia solium can encyst in the central nervous system causing neurocysticercosis, which is the main cause of acquired epilepsy in the countries in which the parasite is endemic. Endemic areas are those with the presence (or likely presence) of the full life cycle of Taenia solium. The parasite is most prevalent in poor and vulnerable communities in which pigs roam free, open defecation is practiced, basic sanitation is deficient, and health education is absent or limited. Several tools are available for the control of Taenia solium. Preventive chemotherapy for Taenia solium taeniasis, which is directed at the adult tapeworm, is one of them. Other tools focus on pig management, pig vaccination and treatment, sanitation and hygiene, and community education. Three potential drugs—niclosamide, praziquantel, and albendazole—have been considered for use for preventive chemotherapy in Taenia solium taeniasis control programs through mass drug administration or targeted chemotherapy. In this Guideline, we provide recommendations for preventive chemotherapy in Taenia solium-endemic areas using niclosamide, praziquantel, or albendazole, including at which dose and in which population groups. The development of this Guideline is based on the latest standard World Health Organization methods for guideline development, including the use of systematic search strategies, synthesis, quality assessment of the available evidence to support the recommendations, and participation of experts and stakeholders in the Guideline Development Group and External Review Group. The recommendations are intended for a wide audience, including policymakers and their expert advisers, and technical and program staff at governmental institutions and organizations involved in the planning, implementation, monitoring, and evaluation of preventive chemotherapy programs for the control of Taenia solium.

Abstract: This chapter focuses on the work the UMSSW/TAC did to support development of neighborhood organizations, especially smaller nonprofits (NPOs) that did not have large budgets. This entailed a variety of strategies including workshops, a leadership academy designed specifically for NPOs, individual and small group consultations, coaching, and personnel resources including placing graduate student interns and VISTA volunteers. The chapter also summarizes initial recommendations for systems development to create a coordinated system of care in each neighborhood, with a focus on youth development programming for those between the ages of 0 and 18. It ends by describing the youth development strategy that the Skillman Foundation ultimately pursued, as well as efforts to support the alignment of youth programming across the domains of youth development, youth employment, and career-themed education.

The formal scientific communication system is currently undergoing significant change. This is due to four developments: the digitisation of formal science communication; the economisation of academic publishing as profit drives many academic publishers and other providers of information; an increase in the self-observation of science by means of publication, citation and utility-based indicators; and the medialisation of science as its observation by the mass media intensifies. Previously, these developments have only been dealt with individually in the literature and by science-policy actors. The Future of Scholarly Publishing documents the materials and results of an interdisciplinary working group commissioned by the Berlin-Brandenburg Academy of Sciences and Humanities (BBAW) to analyse the future of scholarly publishing and to make recommendations on how to respond to the challenges posed by these developments.As per the working groups intention, the focus was mainly on the sciences and humanities in Germany. However, in the course of the work it became clear that the issues discussed by the group are equally relevant for academic publishing in other countries. As such, this book will contribute to the transfer of ideas and perspectives, and allow for mutual learning about the current and future state of scientific publishing in different settings.

In response, and building on existing neuroethics scholarship and analysis, the National Institutes of Health (NIH) has created a Neuroethics Division as part of its BRAIN Multi-Council Working Group. The division members are delivering guidance documents for pertinent topics, recommendations for high-priority neuroethics research questions, providing neuroethics expertise to BRAIN Initiative® investigators, and holding workshops on some of the most pressing issues. This chapter discusses this major initiative and its implications for the future of neuroethics and new opportunities for action and collaboration.

Dual diagnosis, the co-occurrence of substance abuse and another psychiatric disorder, is common. There is evidence to suggest that impulsivity may serve as a common substrate for these comorbidities, despite behavioral and biochemical differences between disorders. This chapter describes common neurobiological and behavioral findings between individuals with high impulsivity and those with dual diagnosis. Specifically, we focus on the co-occurrence of substance abuse with schizophrenia, eating disorders, attention-deficit/hyperactivity disorder, antisocial and borderline personality disorders, and bipolar disorder. For each type of dual diagnosis, we review literature that provides empirical evidence for the presence of impulsivity and treatment recommendations. In this context, we propose a “bottom-up” conceptualization, in which clusters of co-occurring phenotypes are used to formulate diagnostic and clinical plans; such an approach may produce more homogeneous diagnostic groups than exist in the current system. Further, a bottom-up approach may reveal that dual-diagnosis disorders represent distinct groups that share a common factor of impulsivity.

Crafting prevention and intervention strategies for peer bullying, aggression, and suicidality in youth is a complex, multifaceted task. Involvement in bullying and peer aggression is accompanied by numerous psychosocial consequences, including suicidal ideation and behavior. Care must be taken to examine this relationship in an objective, evidence-based manner, rather than overattributing or causally relating youth suicidality to bullying. Mental health professionals, medical providers, teachers, and school administrators are uniquely positioned to intervene in the risk factors that impact bullying and suicidality in youth. Taking an ecological systems perspective, this chapter will review extant efforts to ameliorate bullying, aggression, and suicidality in children and adolescents, with a focus on individual- and group-level protective factors that can facilitate positive health and academic outcomes. Recommendations for providers, educators, parents, policymakers, and researchers offer evidence-based guidance for future work in these domains.

A public health approach is the most sustainable way to provide palliative care for the majority of the world’s people who live in economically disadvantaged nations. With only 7% of the world’s medical morphine currently being used in these countries, much remains to be done to implement the World Health Organization’s recommendations on policy, drug availability and education. International expertise is crucial for planting palliative care. Local efforts have to propagate it by creating feasible and acceptable models. Partnerships should advocate for palliative care to become an integral part of the national health-care system. For those who are considering starting small projects with very limited resources, this chapter provides practical guidance on strategy, needs assessment, training, and implementation at the grassroots. The Wisconsin Pain and Policy Studies Group and the Palliative Care Toolkit are also presented as resources for advocacy, drug availability, and education. The chapter concludes by discussing ways to prevent burnout and routinization in the face of overwhelming need.

In the first part of this chapter the focus is on research comparing the effectiveness (i.e., psychotherapy outcomes) of various treatment modalities: individual, group, couple, and family therapies. In the second section the discussion shifts to focus on research that examines therapy process similarities and differences across the various treatment modalities. The chapter includes a review of the research literature comparing individual, group, couple, and family treatments. Although there are numerous studies comparing treatment approaches (e.g., cognitive behavior therapy vs. psychodynamic therapy), far fewer studies have compared treatment modalities. For treatment outcome differences, a number of meta-analyses examining similarities and differences across treatment modalities are reviewed, summarized, and critiqued. Exploring differences in therapeutic processes involved reviewing, summarizing, and critiquing studies that examined similarities and differences in the character of the therapeutic alliance, helpful events, and therapist behaviors and techniques. The chapter concludes with recommendations for future research. Two major approaches to new research are recommended: focus on treatment goals and systemic processes and an increased focus on the therapeutic processes that cut across and differentiate the treatment modalities

Complementary and alternative medicine (CAM) therapies are a group of diverse medical and health systems, practices, and products not currently considered to be part of conventional medicine. These therapies have growing popularity and it is suggested that more than a third of patients with bipolar disorder (BD) use some form of CAM therapy. Although there are several forms of CAM therapies, including physical therapies, nutraceuticals, herbal remedies, and mindfulness-based interventions, reports in BD are few in number and often of poor quality. Sleep deprivation has the strongest evidence for benefit as an augmentation therapy in bipolar depression. There is promising evidence for other CAM therapies, including exercise, bright light therapy, omega-3 fatty acids, N-acetylcysteine, and the traditional Chinese medicine formulation, Free and Easy Wanderer Plus. However, limitations such as small number of studies, small sample sizes, few randomized investigations, and contradictory findings currently preclude definitive recommendations.

The possession, use, transport, and disposal of radioactive materials are controlled through regulation to limit exposure of the public and workers. This chapter describes the methodologies employed. Regulation is enacted through a system of notification and licensing, based upon recommendations from the IAEA and ICRP. A competent authority is empowered to permit an organization to hold, use, or dispose of any radionuclide, provided certain conditions are met. These take the form of limits on the quantities of different radioactive materials held, and requirements for security and protection. They require an evaluation of the impact of waste disposal. Methodologies for estimating doses received by critical groups from release of radioactive material into the environment are explained. Minimization of the waste produced and regulation of its disposal are essential components in the overall strategy to protect the environment. Controls over the transport of radioactive materials and medical administrations to patients are considered.

Como parte de este movimiento regional la Directora de la OPS, la Dra. Carissa F. Etienne tomó la iniciativa de crear una Comisión de Alto Nivel, denominada “Salud Universal en el Siglo XXI: 40 años de Alma-Ata”, presidida por la Dra. Michelle Bachelet y el Embajador Sr. Néstor Méndez, y conformada por un grupo interdisciplinario de expertos regionales. Entre ellos había representantes de la comunidad, la academia y actores políticos, como ex ministros de salud y líderes de sindicatos y movimientos de diferentes grupos sociales. El objetivo de la Comisión fue elaborar recomendaciones para la Directora de OPS que permitieran hacer efectivo el derecho a la salud de las personas, entendido como un derecho humano fundamental, a partir del análisis de los avances y los desafíos que tienen los sistemas de salud en la Región de las Américas. El presente documento refleja el posicionamiento de la Comisión en torno a la Atención Primaria de Salud (APS) y la búsqueda de soluciones para hacer efectivo el derecho a la salud, además del enfoque utilizado para orientar el debate, el análisis y las recomendaciones sobre cómo garantizar este derecho. The objective of the Commission was to develop recommendations for the PAHO Director on how to give effect to the right to health as a fundamental human right, based on an analysis of the progress and challenges faced by health systems in the Region of the Americas. This document reflects the Commission’s position regarding primary health care (PHC), the search for solutions to ensure the right to health, and the approach taken in discussions, analysis, and recommendations on how to guarantee this right. It is based on reports prepared by the five thematic groups addressing: a) health care model, b) institutional model, c) financing model, d) health and social protection, and e) human resources for health (see annexes to the present report). The thematic groups were led by members of the Commission, bringing together a great number of academic experts and social movements from different countries in the Region. In this report, the Commission presents 10 recommendations for achieving health for all in the Region of the Americas in the 21st century.
