Abstract:

Congestion represents a crucial clinical component of both heart failure and cardiorenal syndrome and it has been postulated to modulate heart and kidney cross-link. Diuretic therapy is a corner stone in the treatment patients with heart failure, and renal replacement therapies are mainly used for patients with refractory heart failure who have not reached the worst stages of renal disfunction. Peritoneal dialysis is a home-based therapeutic modality providing both solute clearance and ultrafiltration, together with relief from congestion in decompensated heart failure patients. The following review will focus on sodium removal in refractory decompensated heart failure patients undergoing peritoneal dialysis. (Cardionephrology)

Abstract:

Background &Objectives: Cardiovascular diseases and hypertension has a significant correlation with chronic arsenic exposure through drinking water. This study was designed to investigate the prevention of sodium arsenate induced vascular disorders by Vitamins C& E in fetal renal blood vessels of albino mice. Materials & Methods: Gravid albino mice of BALB/c strain (twenty four) were randomly divided into 4 groups having 6 animals each. Control group A was inoculated with 0.1ml/kg/day distilled water I/P for 18 days. Animals of groups B, C & D were given a single I/P injection of sodium arsenate 35mg/kg on 8th GD, whereas groups C and D were also injected with Vitamin C, 9 mg/kg/day and vitamin E 15 mg/kg/day by I/P route, beginning from 8th GD and continued for the entire pregnancy period. On 18th day of gestation fetal kidneys were extracted. Histological examination of renal blood vessels was performed for any discernable congestion, endothelial disruption and hyalinization and frequency of changes were expressed as percentages. Results: In group (B) sodium arsenate induction resulted in congestion of blood vessels, hemorrhages in glomerular capillaries and thickening of endothelial walls. The addition of Vitamins C and E in groups C & D respectively had reduced the congestion and endothelial thickening. Mean score of histological changes was statistically significant. Conclusions: The results showed the antioxidant prospective of Vitamins C and E in combating against the vascular lesions induced by sodium arsenate.

Abstract:

Although uncommon during pregnancy, cirrhosis results in multiple medical complications impacting both mother and fetus. Previous reports suggest liver dysfunction in pregnancy causes accumulation of neurotoxins within the maternal compartment that increases neonatal morbidity through placental transfer. We present a case of a 36-year-old G2P1female with history of biliary cirrhosis presenting at 32-weeks' gestation with hepatic congestion progressing to hepatic encephalopathy prompting delivery. Umbilical cord sampling and postnatal infant testing demonstrated elevated ammonia levels which resolved by 12 hours of life without intervention. At discharge, the infant did not demonstrate evidence of neurologic deficit. We conclude that acute maternal hepatic encephalopathy and hyperammonemia due to chronic liver disease do not portend adverse neonatal outcomes, notably encephalopathy.

Abstract:

This clinical case report represents our experience of treatment of 36 y. o. pregnant female patient who was previously undergone mitral valve replacement as she was 25. The operation was performed with the use of two leaflet mechanical prosthesis ‘MedEng No. 21’ (Russia). The patient was admitted to our hospital at 29th week of gestation with the signs of heart failure and pulmonary congestion because of prosthetic valve dysfunction. Delivery was performed by Caesarean section followed by mitral valve reprosthetics. In order to stop obstetric bleeding before heparin injection internal iliac arteries was ligated. Total time of extracorporeal circulation was 89 minutes, and myocardial ischemia — 64 minutes. Normothermic perfusion was performed. Mother and newborn were discharged from the hospital in a normal stable state. The childbearing ability of the mother was preserved.

Abstract:

Uterine necrosis is a rare life-threatening condition reported in few case reports associated with uterine artery embolization for uterine fibroids or postpartum hemorrhage. We report a case of hemorrhagic uterine necrosis in a 22-year-old primipara day 4 post emergency caesarean section who presented with breathlessness, abdominal distension and severe anaemia. Patient underwent emergency exploratory laparotomy 2 hours after the caesarean section with intraoperative findings suggestive of rectus muscle hematoma, bilateral uterine artery ligation was done for atonic PPH. On clinical examination uterus was corresponding to 28-30 weeks gestation size with guarding and tenderness. Ultrasonography revealed bulky uterus. CT abdomen with pelvis with contrast was done with findings suggestive of hemorrhagic ischaemic myometrial necrosis. An emergency exploratory laparotomy was performed with intraoperative findings of uterus size of 30 weeks gestation with diffuse congestion, gangrene of bilateral tubes and ovaries. Obstetric hysterectomy with bilateral salpingo-oovarectomy was done. Patient was discharged on postoperative day 9. High index of clinical suspension, prompt CT imaging and exploratory laparotomy on time proved life-saving.

Abstract:

Intracranial pathological changes can occur as a result of impaired craniocervical venous return. Thrombosis of central venous access catheters was demonstrated in two neonates born at 38 and 27 weeks' gestation. Neither infant developed hemorrhage of prematurity as confirmed on cranial ultrasonography. Clinical evidence of vena cava thrombosis and associated spontaneous intraventricular hemorrhage developed on Day 24 and 36, respectively, and these findings were confirmed on imaging studies. In one infant the hemorrhage was accompanied by communicating hydrocephalus. The cause of the intracranial disease was attributable to the retrograde cerebral venous congestion. This, together with the primitive venous bed developing in the periventricular region, was associated with the spontaneous hemorrhage in the region of the foramen of Monro. To the authors' knowledge, this is the first report in the English-language literature of spontaneous neonatal intra-cerebral hemorrhage, due to thrombosis of the superior or inferior vena cava. The natural history of this condition is resolution without sequelae after appropriate therapeutic intervention for the vena cava thrombosis.

Abstract:

This experiment was designed to investigate the lipid peroxidation and histological effects of chronic fluorosis on first and second generation rat lung tissues. Sixteen, virgin, female Wistar rats were mated with eight males (2:1) for approximately 12 h to obtain first-generation rats. Pregnant rats were divided into two experimental groups (control and fluoride supplemented). The pregnant rats in the fluoride-supplemented group were exposed to 30 mg/L sodium fluoride (NaF) in commercial drinking water containing 0.07 mg/L NaF throughout the gestation and lactation periods. After the lactation period, young animals (first generation; F1) were exposed to the same amount of NaF in drinking water for four months. At the end of the four-month experimental period, nine randomly-chosen male rats (F1) were sacrificed and lung tissues were removed for histopathological and enzymatic lipid peroxidation examination. The second generation rats were obtained from the remaining rats by the same method. They were also treated similarly. At the end of the four-month experimental period, nine randomly-chosen male rats (F2) were sacrificed, and the lungs were removed for histological and lipid peroxidation examination. The rats in the control groups underwent the same procedure without NaF supplementation. It was found that the plasma fluoride and the lung TBARS levels of fluoride supplemented F1 and F2 rats were higher than controls. There were marked histological changes in the lung tissues of fluoride supplemented F1 and F2 rats, as follows: in F1 rats; loss of alveolar architecture, emphysematous areas, descuamation of alveolar epithelium and alveolar congestion were observed. There were thickened interalveolar septae and congestion of alveolar septal vessels. Intraparenchymal thick-walled vessels were also observed. There were markedly perivascular and intraparenchymal focal mononuclear cell infiltrations. In F2 rats, in addition to these changes, there were lipid cell hyperplasia and increased connective tissue mass in the parenchymal areas. It is concluded that chronic fluorosis causes a marked destruction in lung tissues of F1 and F2 rats by causing lipid peroxidation. Toxicology and Industrial Health 2006; 22: 375-380.

Abstract:

PURPOSE: To evaluate hepatic morphological-histological abnormalities in newborns from female rats exposed to ethylenethiourea. METHODS: A randomized study was conducted on fifty-five newborn Wistar rats were studied: 34 in the experimental group, whose mothers had been exposed to 1% ethylenethiourea; and 21 in the control group, whose mothers had received 0.9% physiological solution. The solution was administered via gavage on the 11th day of gestation. Cesarean section was performed on the 20th day of gestation. The newborns' livers were examined and any morphological-histological abnormalities were registered. The presence of megakaryocytes was quantified in 50 microscope fields, as the total number of these cells per mm². RESULTS: The entire experimental group presented abnormalities of embryonic formation, with musculoskeletal anomalies, digestive system anomalies, hepatic congestion and friability, hydrops and delayed intrauterine growth. The histopathological analysis showed that morphological-histological hepatic destructuring had occurred in all entire experimental with removal of the hepatic trabeculae and severe hepatic megakaryocytosis. The mean megakaryocyte density ranged from 107.9 to 114.2 per mm², and it was eight times greater than in the control group, thus characterizing a situation of extramedullary hematopoiesis. CONCLUSION: The fetal exposure to ethylenethiourea caused hepatic damage characterized by severe extramedullary hematopoiesis.

ABSTRACT Objective: To present a unique case of blindness resulting from fungal rhinosinusitis involving multiple sinuses mimicking a malignant process in a pregnant patient. Methods:Design: Case ReportSetting: Tertiary Government Training HospitalPatient: One Result: A 36-year-old pregnant woman developed unilateral blindness during her 20th week of gestation with a history of binocular diplopia, unilateral nasal obstruction and anosmia for 13 months during the pre-pregnancy period. Sphenoid sinus malignancy was suspected on imaging. The planned biopsy was intraoperatively shifted to endoscopic sinus surgery when clay-like materials were seen involving the left maxillary sinus and bilateral sphenoid and ethmoid sinuses. Histopathologic examination confirmed fungal growth. Postoperatively, nasal symptoms resolved but blindness of the left eye and blurring of vision of the right eye persisted. Conclusion: Fungal rhinosinusitis rarely occurs in multiple sinuses and is commonly misdiagnosed. It can afflict pregnant patients and mimic a malignant process. A high index of suspicion early on, especially in the presence of nasal congestion and diplopia may prevent potentially irreversible complications.

Los sistemas de transporte público son eje central del desarrollo de las ciudades ya que garantizan la movilidad y el acceso de sus habitantes a diferentes actividades productivas y sociales. No obstante, los largos recorridos de los buses, y la congestión de tráfico resultante, reducen la calidad de vida y la productividad económica de una región, al tiempo que complican los procesos asociados al control de las rutas y los eventos producidos durante el desplazamiento de los vehículos. En este entorno, este artículo presenta los aspectos generales del proyecto (S.M.A.R.t)1 que busca diseñar e implementar una plataforma tecnológica integral para el monitoreo, gestión de flota y recaudo para las empresas de transporte público de Medellín y del Valle de Aburrá (Colombia), bajo la visión de un Sistema Inteligente de Transporte. De acuerdo con lo anterior, en la primera sección se presentará una introducción al contexto de los sistemas inteligentes de transporte, para posteriormente abordar los procesos de diseño de alto nivel y terminar con el esquema de la solución concebida. ABSTRACTPublic transportation systems are the main support of urban development as they ensure passenger mobility and access to jobs and social activities. However, the length of bus routes and the resulting traffic congestion can reduce the quality of life and economic productivity of a region, while at the same time complicating the processes associated with the control of the routes and the operation of the vehicles. In this context, the present article introduces the S.M.A.R.t1 project, which will design and implement an integrated technology platform for monitoring, fleet management and fare collection in the public transportation companies of the city of Medellin and the Aburrá valley (Colombia) under the concept of Intelligent Transportation System. The first section introduces the context of Intelligent Transport Systems, then high-level design processes are addressed, and the concept of the proposed solution is presented. Abstract:

We describe a patient with Class C diabetes who presented for nonstress testing at 36 weeks and 4 days of gestation with nonreassuring fetal heart tones (NRFHT) and oligohydramnios. Upon delivery, thrombosis of the umbilical cord was grossly noted. Pathological analysis of the placenta revealed chorangiosis, vascular congestion, and 40% occlusion of the umbilical vein. Chorangiosis is a vascular change of the placenta that involves the terminal chorionic villi. It has been proposed to result from longstanding, low-grade hypoxia in the placental tissue and has been associated with such conditions such as diabetes, intrauterine growth restriction (IUGR), and hypertensive conditions in pregnancy. To characterize chorangiosis and its associated obstetric outcomes we identified 61 cases of “chorangiosis” on placental pathology at Henry Ford Hospital from 2010 to 2015. Five of these cases were omitted due to lack of complete records. Among the 56 cases, the cesarean section rate was 51%, indicated in most cases for nonreassuring fetal status. Thus, we suggest that chorangiosis, a marker of chronic hypoxia, is associated with increased rates of cesarean sections for nonreassuring fetal status because of long standing hypoxia coupled with the stress of labor.

Abstract:

Inorganic arsenic (iAs) exposure induces a decrease in glucose type 4 transporter (GLUT4) expression on the adipocyte membrane, which may be related to premature births and low birth weight infants in women exposed to iAs at reproductive age. The aim of this study was to analyze the effect of sodium arsenite (NaAsO2) exposure on GLUT1, GLUT3, and GLUT4 protein expression and on placental morphology. Female Balb/c mice (n=15) were exposed to 0, 12, and 20 ppm of NaAsO2in drinking water from 8th to 18th day of gestation. Morphological changes and GLUT1, GLUT3, and GLUT4 expression were evaluated in placentas by immunohistochemical and image analysis and correlated with iAs and arsenical species concentration, which were quantified by atomic absorption spectroscopy. NaAsO2exposure induced a significant decrease in fetal and placental weight (P<0.01) and increases in infarctions and vascular congestion. Whereas GLUT1 expression was unchanged in placentas from exposed group, GLUT3 expression was found increased. In contrast, GLUT4 expression was significantly lower (P<0.05) in placentas from females exposed to 12 ppm. The decrease in placental GLUT4 expression might affect the provision of adequate fetal nutrition and explain the low fetal weight observed in the exposed groups.

Abstract:

A 38-year-old primiparous mother (gravida 1, para 0) at 27 weeks and 6 days' gestation reported that fetal movements had been absent for 6 days. All serological markers for infection were negative. Chorionic villus sampling at stillbirth delivery revealed trisomy 21 (47, XX, +21), indicative of Down syndrome. The macerated baby was female and weighed 1,290 g. There was no evidence of hydrops fetalis. Proliferating blast cells expressing megakaryoblastic/megakaryocytic antigen CD61 were mainly seen within the vessels, and some cells infiltrated outside of the vessels in almost all organs. Vessels of the umbilical cord and chorionic villi were filled with proliferating blast cells, but the blast cells were not apparent in the bone marrow. The diagnosis of transient abnormal myelopoiesis in Down syndrome was made. Hepatomegaly (64.5 g) was due to congestion and infiltration of CD61-positive blast cells within the vascular lumina and expanding outside the lumina accompanied by fibrotic change. The cause of death was attributed to liver insufficiency caused by liver fibrosis. An umbilical cord and chorionic villi examination may be helpful in the diagnosis of transient abnormal myelopoiesis when post-mortem examination is not permitted.

Abstract:

Methotrexate (MTX) is an anticancer agent which is used alone or in combination with other anticancer agents in the treatment of breast cancer, epidermoid cancers of the head and neck, cutaneous T cell lymphoma, and lung cancer. Bombyx mori larvae have a huge value as health food especially for cardiac and diabetic patients, bronchial asthma, primary trigeminal neuralgia, vocal nodules and polyps and in the treatment of facial palsy and pain. In the present study meeting occurred under the normal conditions and about 40 pregnant female rats were used. Herein the pregnant females will be divided into five groups; group 1 will be used as negative control received distilled water, group 2 will used as positive control and recived buffer of Bombyx mori larvae extract, group 3 was treated with MTX at the 12th day of gestation (at organogenesis phase), group 4 was treated with Bombyx mori larvae extract at 7th , 9th , 11th ,13th and 15th days of gestation (during the oraganogensis period), & group 5 was injected by MTX the 12th day of gestation as well as bombyx mori larvae extract at 7th ,9th ,11th ,13th &15th days of gestation. Animals of all groups will be sacrificed at 20th day, the end of gestation periods. Then livers of all pregnant rats were removed for examination. The obtained results showed decrease in maternal body weight gain plus increase in the abortion rate and uterine weight in the MTX-treated group. In addition, MTX induced an elevation in the examined liver oxidative stress biomarkers plus myeloperoxidase activity and decrease in reduced glutathione content and catalase activity; in mothers. Histopathological studies of liver tissues showed congestion of central vein of mothers in MTX group showing ballooning degeneration of hepatocytes, perivascular inflammatory cells infiltration and strong deposition of collagen fibers. Noteworthy, Bombyx mori larvae induced marked improvement in injuries associated with MTX administration. Keywords: Methotrexate, Bombyx mori larvae, Pregnant female rats, Hepatotoxicity.

In regimen A women were given sildenafil citrate and nifedipine while in regimen B nifedipine alone for 72 hours as tocolysis.Results: Higher mean latency in sildenafil and nifedipine combination regimen compared to nifedipine alone regimen (28.23±18.3 versus 12.98±13.35 days, p<0.001). In regimen A, less days of hospital stay (3.23±0.92 versus 3.9±0.38 days, p<0.0001); improve perinatal outcomes in form of fewer deliveries during hospitalization or within 7 days after discharge (p=0.018); reduction in NICU admission (0.68±1.54 versus 3.18±4.61 days, p=0.002); improvement in birth weight (p<0.001) and fewer neonatal complications. Only minor side-effects like facial flushing (42.5% versus 22.5%, p=0.029), headache (40% versus 25%), dyspepsia (15% versus 2.5%, p=0.047), nasal congestion, palpitation, hypotension, constipation, nausea, dizziness were seen.Conclusions: Our study found combination of sildenafil citrate with nifedipine as superior and successful regimen with future potential as a tocolytic therapy.

Background. Fetal volvulus is a rare, yet life-threatening condition that requires skilful diagnosis and management. Volvulus occurs when bowel loops become twisted and the twisting of the mesenteric artery leads to congestion, impaired venous return, and bowel necrosis. Case Description. We present a case of fetal ileal volvulus suspected on third trimester ultrasound, complicated by premature labour, small bowel necrosis, and meconium peritonitis. Progressive dilatation and decreased peristalsis of echogenic bowel were noted in the early part of the third trimester. Daily surveillance ultrasound was performed and spontaneous labour occurred at 32 weeks’ gestation. A proactive postnatal approach guided by prenatal sonographic findings allowed prompt treatment and an urgent laparotomy was performed for an ileal volvulus with necrosis and meconium peritonitis. A segment of small bowel volvulus was resected and an end-to-end anastomosis was performed with uneventful recovery. Discussion. Clinically signs of fetal midgut volvulus are not pathognomonic, such as intestinal dilatation, abdominal mass, ascites, peritoneal calcifications, or polyhydramnios; thus, the diagnosis is often challenging. Complications reported in the literature include perforation and haemorrhagic ascites, which may lead to anaemia, hypovolemia, heart failure, and fetal demise. Conclusion. This case highlights the importance of assessing the fetal bowel as a part of routine third trimester ultrasound. The case describes the complexity of diagnosis in the fetus, important considerations along with multidisciplinary team approach to management.

Se analizan los diferentes planes de desarrollo urbano aplicados en la ciudad del Cusco; las políticas del INC; misiones como la Misión Kubler, que se limitan a impartir recomendaciones para la conservación y protección de los inmuebles prehispánicos y virreinales. Además, se aborda una experiencia de Cooperación Técnica Internacional, que se practicó en la manzana No. 127; los trabajos de la ONG Guamán Poma de Ayala y la World Monument Watch, rehabilitando casonas y viviendas para mejorar el nivel de vida de 302 habitantes. Se analiza la gestión municipal de la provincia del Cusco, período 2011-2014, que inició la actualización de tres documentos: el Plan Maestro del Centro Histórico del Cusco, el Catastro Urbano, y Gestión y Manejo del Centro Histórico, que ha resultado ser un registro y catalogación de inmuebles y ambientes urbanos, y obras ejecutadas en la gestión 2011-2014 para el mejoramiento de la transitabilidad vehicular en las calles de Arcopata, Meloc, Avenida Alta, Avenida Baja, Vitoque y Nueva Baja. Finalmente, se identifican las distintas problemáticas que aquejan al barrio en cuestión, como el comercio ambulatorio, el abandono de casonas coloniales, y la congestión vehicular y peatonal en la plaza. Además, se aborda la discusión sobre a qué entidad corresponde gestionar y manejar el Centro Histórico. A manera de conclusión, se expone una propuesta que reúne conceptos e interrogantes centrales para determinar la conformación de un ente gestor adecuado, que reúna instituciones públicas y privadas bajo el liderazgo de la municipalidad provincial, de forma que cuenten con respaldo político y financiero para realizar sus labores.

Abstract:

Soccer performance depends on several interrelated factors regarding technical, tactical, physical and psychological areas. Over the last decades there was an increase in match congestion. The increased match frequency and the associated injury risk have highlighted the importance of physical condition, increasing the need to implement new training methodologies with a special focus on load and fatigue management, as well as non-specific complementary training. The main objective of this study was to provide information on the structure and characteristics of the technical staffs, the methodological training practices, as well as the workload and fatigue control methods used in soccer, examining possible differences based on gender, category and competitive level. 190 soccer teams from 20 different countries participated in the study, by answering a survey. The results reveal that there are differences in the structure of the technical staff, the planning models, the methodology and the workload control depending on the category and the competitive level. Gender only appears as a discriminating variable, in relation to the most used complementary training contents. The weekly microcycle is the preferred planning model (80.89%), regardless the competitive level. However, in lower categories, medium and long term periodization are also used (23.80%). The weekly volume of complementary training increases as category (p=0.000) and competitive level (p=0.000) does. Strength training is the most used non-specific content (84.89%). However, its importance is reduced in lower categories (38.5%). Load and fatigue control are only extended among teams of superior category (p=0.000) and competitive level (p=0.000). Resumen. El rendimiento en fútbol depende de varios factores interrelacionados entre los que encontramos las áreas técnica, táctica, física y psicológica. Durante las últimas décadas ha existido un aumento en la congestión de partidos. La mayor frecuencia de partidos y el riesgo de lesión asociado han puesto de manifiesto la importancia de la condición física, aumentando la necesidad de implementar nuevas metodologías de entrenamiento con especial énfasis en el manejo de la carga y la fatiga, así como entrenamientos complementarios. El objetivo principal de este estudio fue brindar información sobre la estructura y características de los cuerpos técnicos y las prácticas metodológicas de entrenamiento, así como los métodos de control de la carga de trabajo y la fatiga utilizados en el fútbol, examinando posibles diferencias por género, categoría y nivel competitivo. 190 equipos de fútbol de 20 países diferentes participaron en el estudio, respondiendo a una encuesta. Los resultados revelan que existen diferencias en la estructura de los cuerpos técnicos, los modelos de planificación, la metodología y el control de la carga de entrenamientos y partidos según la categoría y el nivel competitivo. El género solo aparece como variable discriminante en relación con los contenidos complementarios de entrenamiento más utilizados. El microciclo semanal es el modelo de planificación preferido (80.89%), independientemente del nivel competitivo. Sin embargo, en categorías inferiores también se utiliza la periodización a medio y largo plazo (23.80%). El volumen semanal de entrenamiento complementario aumenta a medida que lo hace la categoría (p<.001) y el nivel competitivo (p<.001). El entrenamiento de fuerza es el contenido de entrenamiento complementario más utilizado (84.89%). Sin embargo, su importancia se reduce en categorías inferiores (38.5%). El control de la carga y la fatiga solo se extiende entre equipos de categoría y nivel competitivo superior (p<.001).

Abstract:

Abstract Several mouse models for sickle cell disease have been developed for the studies of the pathophysiology of sickle cell disease and the investigations of drug and gene therapies. In previous years, we have also succeeded in producing a sickle cell anemia mouse model. In this model the endogenous mouse α and β globin genes were knocked out and replaced by the human α and βs globin transgenes. The βs globin gene is contained in a 240kb YAC that preserves the entire native genomic context of the β-globin locus. These mice have anemia, reticulocytosis and irreversible sickle cells in the peripheral blood, as well as other pathological features of sickle cell disease. However, their γ globin switches to βs globin expression from around 12 days of gestation. The low level of fetal hemoglobin expression in utero led to intrauterine sickling and fetal death so that very few live-born sickle cell anemia mice could be obtained. To rescue these mice from intrauterine death we investigated the effect of placing the pregnant mothers into a high O2 environment. From the tenth day of gestation onwards, we placed the mothers into a chamber containing 50% O2 and kept the newborn pups in it for another 10 days after birth. The frequency of sickle cell anemia mice we obtained was increased from 0.2% to 32%. Moreover, 55% to 88% of the newborn sickle cell anemia mice survived in the oxygen treated group, whereas none of the sickle cell anemia mice survived in regular air breeding conditions. The survived sickle cell anemia mice develop congestion, atrophy, and infarcts in multiple organs by histological analysis. These pathological finding are very similar to those find in patients with sickle cell disease. We conclude that a high oxygen environment can be used to obtain more sickle cell anemia mice in those models that have a high perinatal mortality. The higher yield of these mice will facilitate physiological and therapeutic studies of sickle cell anemia.

ABSTRACT Objectives Evaluation of efficacy and safety of intravenous labetalol and intravenous hydralazine in managing hypertensive emergency in pregnancy. Study design An open, randomized comparison of intravenous labetatol vs intravenous hydralazine was conducted in 100 pregnant women presenting with hypertensive emergency during pregnancy. Materials and methods: Inclusion criteria Women with severe hypertension in pregnancy at 34 weeks of gestation or more, i.e., systolic BP ≥160 mm Hg or diastolic BP ≥ 110 mm Hg with or without proteinuria either in labor or not in labor. Results The antihypertensive efficacy of both the drugs was found to be comparable. The primary outcome in terms of BP control was achieved in both the groups. There were four treatment failures in labetalol group and none in hydralazine group. No significant difference was observed in maternal and fetal outcomes. Palpitation and tachycardia occurred significantly more often in patients treated with hydralazine. Bradycardia was significantly more frequent in labetalol group. The neonatal outcomes were similar per group. Conclusion The randomized clinical trial showed that labetalol and hydralazine fulfil the criteria required for an antihypertensive drug to treat severe hypertension in pregnancy. Hydralazine has its side effects like palpitation, headache, oliguria, dizziness, muscle cramps, and nasal congestion, which mimic symptoms of deteriorating pre-eclampsia. Labetalol has a better side effect profile, but specific concerns have been raised about the risk of neonatal bradycardia. Abstract:

The aim of the research was to study a morphological structure of airway and respiratory parts of lungs in 36 full-term newborns who died at the 2-5th day of life. The main group included 16 died newborns with the inborn cytomegalovirus infection who were diagnosed with moderate to severe cerebral ischemia with hypertension-hydrocephalic syndrome, pseudocysts of the blood vessels plexus, subependymal and subarachnoid haemorrhages, monocytosis, vesiculitis, pneumonia, hepatitis and meningocephalitis against intrauterine hypoxia and chronic subcompensated placental insufficiency. The comparison group consisted of 20 newborns whose mothers during the gestation period did not have acute respiratory viral infections, moderate and severe somatic and obstetrical pathology. The cause of their death was a birth trauma, intranatal and postnatal hypoxia. Describing lungs the following morphologic features were emphasized: 1) the changes of the general composition of the organ; 2) the state of the lumen of the large, medial and small bronchi; 3) the reaction of the epithelial lining, conjunctive tissue and the blood channel of the bronchial tree and aero-hematic barrier. As a result of the studies it was found out that in the newborns of the main group in comparison with the children of the comparison group in the lumen of large bronchi there was a rare case of mucus, there were often found erythrocytes and desquamated epithelial cells. Only infected children were found to have leucocytes. More intensified were alterative processes in epithelial cells; there were often found cells of the bronchial epithelium without ciliary structures, desquamation of highly differential ciliary and goblet cells, cell elements with a small hyperchromic nucleus and light cytoplasm in the cambial layer, sharply expressed vascular congestion, proliferative changes in the bronchial epithelium as well as hydropic-hemorrhagic and aspiration syndromes. Pneumonia and signs of the virus metamorphosis were diagnosed only in the main group.

Abstract:

Congestion is essentially a relative phenomenon that is linked to the difference between the roadway system performance that users expect and how the system actually performs. The approach to traffic congestion (TC) from the different perspectives of countries around the world helps Vietnamese experts to better understand and come up with a consensus on how to understand the problem. this topic. Since then, in the management of traffic safety and control, to curb traffic congestion, traffic police and other departments have the basis to perform the work, properly assess the level of traffic safety to implement the task; doing well the reporting, statistics, building databases on TC; create integration in the context of international cooperation with other countries. Solving the problem of traffic congestion in large cities in Vietnam is a problem that cannot be solved overnight. This problem requires the synchronous participation of many ministries, branches and people in traffic. All measures need to get feedback from the people, thoroughly resolve the critic before implementation. This article briefly presents the situation of traffic congestion in Hanoi and Ho Chi Minh City. The team also focused on analyzing the causes of traffic congestion and proposed solutions to solve this urban problem.
