In their article “Different methods for ethical analysis in health technology assessment: An empirical study” published inInternational Journal of Technology Assessment in Health Care, Volume 27 Number 4, Samuli I. Saarni, Annette Braunack-Mayer, Bjørn Hofmann, and Gert Jan van der Wilt present an empirical study about different methods for ethical analysis in health technology assessment (HTA). They claim that ethical analysis is an important issue which can highlight values inherent in a technology and value-decisions underlying a HTA process (4). Although their study is rich in detail; gives an interesting overview of casuistry, principlism, and axiological ethic frameworks; and make ethical evaluations within HTA more transparent, we suppose, one important point needs further serious discussion: the clarification of what is meant by “value.”

Gert hofmann s'interroge sur les fondements de l'existence humaine : la perception du monde et de la realite, le langage. L'auteur montre les defaillances de ces categories en soulignant leur caractere a la fois, trop universel, et trop subjectif. Hofmann illustre les paradoxes de l'existence humaine : l'homme, mortel, tente d'echapper a la mort par l'illusion ; la bestialite de la nature humainemalgre les acquis de la civilisation. Les rapport avec autrui sont domines par un rapport de force, engendre par la demande et le refus de la reconnaissance de l'autre. Autrui devient un objet de regard deshumanise et interchangeable. L'experience personnelle de hofmann, a l'origine de ses interrogations existentielles, se traduit par des themes recurrents : la reconnaissance, le conflit pere-fils. L'ecriture est pour l'auteur une possibilite de fondement de l'existence. La mise en scene grotesque met en evidence la crise existentielle et souligne le caractere tragique et absurde de l'existence humaine. Par l'ironie, l'auteur met en question les categories litteraires et incite a une reflexion philosophique. L'originalite de l'uvre de hofmann se traduit par l'experimentation dans les domaines de la perspective narrative, et par la creation d'un nouveau type de narrateur. Hofmann concilie les themes litteraires classiques avec une demarche stylistique contemporaine. Dans le domaine du roman, hofmann s'inscrit dans la tradition de la litterature moderne, mais en se distinguant par la multitude des themes ; dans le domaine de la prose courte, il parvient a renouveler le genre de la nouvelle.

Abstract:
There are about 400,000 species of plants in this world. Only a small fraction, perhaps 100 in number, contain hallucinogenic chemicals. Nearly a century ago, Lewis Lewin, professor of pharmacology at the University of Berlin, in speaking of drugs he called phantasticants, said “The passionate desire which . . . leads man to flee from the monotony of daily life . . . has made him discover strange substances (which) have been integral to human evolution both societal and cultural for thousands of years.” An unusual problem presents itself to me in writing about these drugs: They straddle the worlds of science and mysticism. The Encyclopedia Britannica defines mysticism as the practice of religious ecstasies (religious experiences during alternate states of consciousness), together with whatever ideologies, ethics, rites, myths, legends, and magic may be related to them. Science I am comfortable with; mysticism not so much. Yet in our exploration of the agents found in this chapter, we will encounter many persons speaking of drug-induced mystical experiences. I have attempted to get around my unease by first providing the history and the pharmacology of these agents and then touching only lightly on mysticism, allowing readers to draw their own conclusions. What shall we call these chemicals? Hallucinogen, a substance that induces perception of objects with no reality, is the term most commonly encountered and the one that I have settled on for the title of this chapter. However, it comes with a caveat. Albert Hofmann, the discoverer of LSD, our prototypic hallucinogen, has pointed out that a true hallucination has the force of reality, but the effects of LSD only rarely include this feature. Two additional terms that we will find useful are psychotomimetic and psychedelic. We have already considered the former, an ability to mimic psychosis, in our discussion of amphetamine-induced paranoid psychosis in chapter 4 and the effects of phencyclidine in chapter 6. A psychedelic was defined in 1957 by Humphrey Osmond, inventor of the word, as a drug like LSD “which enriches the mind and enlarges the vision.”
