Abstract:
The current status of gerontological nursing curricula in Canadian generic baccalaureate nursing programs has not been studied. As the Canadian society changes the health care system is struggling to provide adequate health care to the growing population over the age of 65. Nurses must be prepared to assist the elderly in the community and in institutions to cope with increasing disabilities. Therefore, the study of gerontological nursing should be a requirement in a nurses' basic education to prepare them to work with older clients in all settings. This study used a survey methodology to determine the present status of gerontological content in baccalaureate nursing education curricula. Questionnaires were sent to each of the 22 deans/directors of the Canadian generic baccalaureate nursing programs and to 31 provincial reputational "experts" in the field of gerontology. A return rate of 90% and 93% was obtained respectively. The study asked five questions: 1) What nursing model or concepts are used by the generic baccalaureate schools of nursing? 2) What gerontological content is included in these programs? 3) What gerontological content is integrated in courses or taught in required or elective specific gerontology courses? 4) What gerontological clinical experiences are required? and 5) Are faculty academically prepared to teach gerontological content? Answers to these questions were compared with similar questions asked of reputational "experts". A quarter of the schools did not use nursing concepts or models while many schools chose a nursing model which was not consistent with their philosophy of health. Ninety percent of the schools taught gerontology content in integrated courses; half of the schools also offered a specific gerontology course of which 40% were elective courses. Even though all 49 listed gerontology topics and 28 patient problems and care techniques were taught by the majority of the schools there is little evidence the schools are producing gerontology prepared nurses. The gerontology clinical hours accounted for only 7.4% of the total clinical experiences. The "experts" recommended gerontology receive 21% of the clinical hours and that it be dispersed in a variety of community and clinical settings. Few (5%) faculty members were prepared with a post graduate degree in gerontology to act as positive role models for the students. Few (2%) students chose a gerontology practicum in their last year. The findings and recommendations are meant to assist educators with the task of expanding the gerontological curricula in generic baccalaureate nursing programs. The nine recommendations address ways to assist schools to reevaluate their curriculum and improve nursing care to the elderly in Canadian society.

Abstract:
The population of older adults has grown rapidly in recent years and is expected to continue to grow into the middle of this century. The aging of the population means that nurses need to have specialized gerontological knowledge in order to properly care for older adults. In spite of the current need for specialists in this field, gerontological nursing is not a popular choice and nurses often lack adequate preparation to care for older adults. The complex reasons behind these issues are rooted in the history of the development of this specialty. This study takes a historical look at the development of gerontological nursing in British Columbia through the stories of seven nurse educators who were leaders and innovators in their field. The findings of the study tell a story of the nurses’ work to change unacceptable nursing practice, improve standards of care and professional status of gerontological nursing and advocate for older adults. In doing so, these nurses challenged cultural values about aging and care of older adults and worked toward giving gerontological nurses a voice in policy and decision-making. The findings from this study can be used to guide today’s gerontological nurses as they continue to develop this specialized field of nursing knowledge.

Abstract:
Access to healthcare is needed and wanted by people of all ages and especially by those of the older population. The number of people in the 65 years of age and older population is rapidly growing with their needs expected to have a significant impact on the existing healthcare system and healthcare providers. The impact will be critical given the severe shortage of healthcare providers, especially of nurses and the rate of services being more often provided in non-hospital settings. The objectives of the study were to discover the plans of graduating nursing students as they choose their first place of employment, if they have future plans to pursue a nursing advance practice degree, and if they are very happy with their decision to become a nurse. Data for the study were obtained from a questionnaire presented to senior graduating nursing students. The findings were: (a) Most students prefer a hospital setting. (b) Younger students are three times as likely to seek out the hospital, and 1/3 of the students seek out the hospital setting because they were encouraged to become a nurse. (c) About 70% of the students want to work with their friends while 1/3 will seek the hospital worksite, as it is perceived as being the strongest resource in paying back loans. (d) Nearly 87% are considering the nursing advance practice role, and 52% have interest in the nurse practitioner role. The majority of students identified as very happy with their decision to become a nurse. This study provided insight for schools of nursing as they make curriculum decisions and to businesses as they learn of the preferences and plans of the new emerging nurses.

Background: There is a growing need for skilled gerontological nursing care. Nursing students often fail to appreciate the importance of implementing gerontological nursing competencies.

Purpose: The problem is that nursing education research on students' attitudes about gerontology has been hampered by a lack of valid and reliable instruments.

Theoretical framework. This study is based on and tests the propositions of the theory of planned behavior.

Methods. Qualitative interviews were conducted to determine the terms used to describe the constructs by the target population. Using these terms, an instrument was developed and subjected to tests for psychometric estimates. A pretest-posttest design was used to test the impact of an educational intervention on students' attitudes.

Results. The instrument demonstrated acceptable reliability and validity. Students' scores increased significantly for attitude, subjective norm and behavioral intention regarding implementing gerontological competencies from pretest to posttest. Attitude and subjective norm predicted behavioral intention in the regression model.

Conclusions: The results support that an educational intervention can positively impact nursing students' intent to implement gerontological competencies.

Abstract:
The aim of the research was to improve Taiwanese undergraduate nursing students' attitudes toward and knowledge about older people in order to encourage them to work with older people. People aged 65 and over currently make up 9.7% of the Taiwanese population (Department of Statistics 2006). With the increasing population of older people, health care professionals will have more experiences of caring for older people. However, an increasingly large body of literature suggests that most health care professionals have negative attitudes toward older people and little knowledge about older people. Studies from Western countries have indicated that attitudes toward and knowledge about older people can be improved through a variety of educational efforts. Two studies were conducted to examine these issues. Study 1 involved a cross sectional survey of 302 nursing students from four-year and two-year programs in a university in southern Taiwan. Overall, the results showed that nursing students held positive attitudes toward older people but had poor knowledge about older people. Moreover, the findings suggested that nursing students' intention to work with older people and gender were important factors influencing their attitudes toward older people. Age, nursing program, and living with older people were the variables which made independent contributions to knowledge about older people. Study 2 was a quasi-experimental design using pre-post tests with an intervention (experiental based learning) and control group (usual lecture based learning) (n = 60) to test the impact of a gerontological educational subject. Focus group data were also collected to examine students' reactions to the gerontological nursing subject and the experiential learning strategies used in an experiential-based learning group. The sample was students in the second semester of their second year from the same university used for Study 1. All 60 students were randomly assigned into either experiential-based learning or lecture-based learning groups for their gerontological nursing subject. The data were collected across three time points (pre-test, week 16 and week 20) using 2 validated instruments from Study 1. Qualitative data were also collected from the experimental group after students' clinical practice at week 20. In order to test for the effect of the intervention over time, repeated measures analysis of variance was used to determine the effectiveness of the experiential learning approach and clinical practice on each of the dependent variables of attitudes and knowledge. The results of Study 2 indicated that students' attitudes toward and knowledge about older people did not differ between the two groups In addition, there was no change in attitudes following the completion of the gerontological nursing subject. Students in both groups had improved their level of knowledge at the end of the gerontological subject. Therefore, the study hypotheses were not supported. Several factors such as lack of linkage between theoretical concepts and experience, the dominant 'exam culture', students' usual learning style and the structure of the program may explain the results. This was the first study which had introduced experiential learning into the selected university. It was necessary to conduct this initial study to understand the students' reaction to it. Therefore, based on the research findings from both the quantitative and qualitative results, the study indicates that additional studies are needed to continue exploring how experiential learning strategies may be used to improve students' attitudes toward and knowledge about older people.

At the current time of demographic changes in the world , it is observed that the health sector requires restructuring of its practices in caring for the elderly , in order to enable a comprehensive and humane care . In this sense, we identify the need for qualified professionals to assist the elderly and, therefore , it is important that training curricula are adequate to this reality . The study aims to analyze how it develops teaching gerontological nursing component in undergraduate nursing . This is a descriptive , exploratory study with a qualitative approach, in two higher education institutions in the city of Natal / RN . The population involved corresponded to the coordinators of graduate courses in nursing and teachers who teach content of health care for the elderly, a total of seven participants . The data collection instruments were used: a questionnaire containing questions related to the characterization of the participants and the teaching of gerontological nursing in undergraduate courses in nursing and a form with items of analysis for pedagogical projects . The results were presented descriptively in tables and analyzed from coming themes of the main themes of the instruments defined in: knowledge about the research participants , knowledge of teaching gerontological nursing component and knowledge of pedagogical projects , discussed under Resolution paragraph . 03 of 7/11/2001 , which establishes standards for the nursing program , the educational projects of educational institutions , the Theoretical Donald Shoon and thematically relevant literature . The results indicate that the majority of participants were female (four), all have graduate though , none of them stated in the qualification of aging or elderly health area; the organization of the contents of the component in the curriculum, workload theoretical- methodological and practical teaching strategies presented was heterogeneous and sometimes diverging at the same institution . On the definition of gerontological nursing component was identified that is not well defined along the pedagogical design of educational institutions surveyed . It was concluded that in the context of this study , teaching gerontological nursing is present in the course of nursing education, but needs reevaluation and updating in order to teach a wider range of driving and health practices

Nesse sentido, identifica-se a necessidade de profissionais qualificados para assistirem a pessoa idosa e, para isso, ? importante que os curr?culos de forma??o do enfermeiro estejam adequados a esta realidade. O estudo tem como objetivo geral, analisar como se desenvolve o ensino do componente enfermagem gerontol?gica em cursos de gradua??o de enfermagem. Tratou-se de um estudo descritivo e explorat?rio, com abordagem qualitativa, realizado em duas institui??es de ensino superior no Munic?pio de Natal/RN, sendo uma institui??o p?blica e outra. Abstract:
The overall aim of this thesis is to describe what it means to be old and live with multimorbidity. An additional aim is to examine and describe the contextual meaning of the phenomenon in ordinary housing and nursing homes, and a third aim is to deepen our understanding of the situation for old people who also are ill. The thesis uses a caring science perspective and a reflective lifeworld approach founded on phenomenological philosophy. This approach searches for and describes the meaning of a phenomenon, its variations and its essential meaning structure. Interviews were used for data collection and data were analyzed for meaning, searching for the essence of the phenomenon. The findings are presented in two empirical studies and one philosophical excursion. The empirical studies have been further thematized with the essential meanings from the empirical studies. The philosophical excursion is the result of a more profound understanding of the thematized meanings. The essential meaning of being old and living with multimorbidity in ordinary housing is described as a struggle to maintain identity in a life situation that changes. Multimorbidity and aging pose existential barriers at the same time as the possibility of living an independent life and being oneself is hindered. Ordinary housing is experienced as a place where the old can be themselves, and a place that is associated with independence. On the other hand, multimorbidity threatens the possibility of continuing to live in their private homes, as does the failure of others to meet the old as individuals. The essential meaning of being old and living with multimorbidity in nursing homes is described as striving for independence which brings with it a zest for life and a feeling of security. The older’s degree of independence can change due to the fragile health situation, and is characterized by the experience of not being a burden for the busy caregivers and relatives. Independence can change to insecurity, vulnerability and helplessness. The themes of essential meaning that have been extracted from the empirical studies suggest that the experiences of frailty and loneliness differ more between those living in ordinary housing and in nursing homes than the experiences of trust and independence differ. The philosophical excursion illuminates how older people with multimorbidity experience their lives as an ability to manage their daily lives and not merely an absence of disease symptoms. A person is “just” sick, independently of the objective quantity of diseases s/he may suffer from. Health and wellbeing occur from the ability to live in existential coherence, which is encouraged when the older people are allowed to retain their habits, the ability to be oneself, individual’s life story and by social relationships, as well as by continuity among the caregivers.

Abstract:
Trata-se de um estudo quantitativo, com abordagem descritiva e comparativa, que objetivou verificar a influência da consulta de enfermagem gerontológica no nível de adesão terapêutica dos clientes com doenças crônicas não-transmissíveis, acompanhados num ambulatório especializado. Como objetivo específico, buscou-se comparar o nível de adesão terapêutica, perfil sociodemografico e clínico entre o grupo de clientes acompanhados na consulta de enfermagem e o grupo dos não acompanhados. Este foi motivado pela práxis da autora em consultas gerontológica, onde foram percebidos obstáculos enfrentados por muitos clientes com adesão inadequada ao tratamento, acarretando dificuldades no controle de suas doenças, fato este ratificado pela literatura. Os resultados evidenciaram similaridade entre os perfis sociodemográficos, com prevalência de sexo feminino, baixa escolaridade e renda. O perfil clínico revelou, em ambos os grupos, alto índice de hipertensão, diabetes e depressão, destacando-se esta última por ser desfavorável ao comprometimento com o autocuidado. Por fim, verificou-se que os níveis de adesão terapêutica ficaram majoritariamente dentro da faixa estabelecida como adesão ampla, sem diferença expressiva entre os grupos. É relevante citar que, entre os acompanhados pela consulta de enfermagem, observou-se, um maior conhecimento sobre a doença em tratamento e suas manifestações, maior acesso aos medicamentos e auto percepção de conhecimentos sobre efeitos colaterais. Constatando-se que, este último achado, exerce grande influência na adesão ao tratamento farmacológico. Este desfecho reflete a importância de uma assistência sistematizada, embasada em uma teoria de enfermagem, que neste contexto, foi a teoria do autocuidado de Orem, utilizada previamente nas consultas de enfermagem do ambulatório investigado. Os resultados corroboram o uso desta teoria, especialmente numa perspectiva educativa da assistência ambulatorial. Diante disso, é mister buscar novas abordagens de pesquisa, a fim continuar a investigação sobre as contribuições da enfermagem para uma melhor adesão terapêutica dos clientes idosos. Finalmente, a autora espera cooperar para o aprimoramento científico nesta matéria e para uma progressiva qualificação da assistência de enfermagem na prevenção e controle de agravos à saúde.
Using quantitative, descriptive and comparative approaches, this work aims at assessing the influence of the gerontological nursing consultation on treatment adherence of elderly clients with non-transmissible chronic illnesses in a specialized clinic. Urged by her experience, the author noticed difficulties faced by many elderly, leading to low levels of treatment adherence, and consequently to difficulty controlling their illnesses, a fact confirmed by the established literature. This study aims specifically, to compare both the level of adherence, of nursing consulted group with that of a non-consulted, and the sociodemographic and clinical profiles of the aforementioned groups. Results evidenced similarity between sociodemographic profiles, with the prevalence of female gender, low education and income. The clinical profiles showed a high degree of hipertension, diabetes and depression in both groups, the last one, with a considerable disadvantage to the commitment to self-care. The nursing consulted group client displayed a greater knowledge of the disease and its manifestations along the treatment, an increased access to medications, and knowledge about their side effects. Finally it was concluded that adherence levels it was good one, with no significant difference between groups. Such outcome reflects the importance of a systematized care, based on Orem's self-care theory, long deployed in gerontological nursing consultations in the aforementioned clinic. The results corroborate the use of Orem's theory, mostly in an educational perspective of the outpatient care. It is thus momentous to pursue new approaches in order to research nursing contributions to attain a higher degree of adherence of the elderly client. Lastly, the author hopes to contribute to the improvement of the knowledge in this field and to the onward qualification of nursing care in illness prevention and control.

The aging process underway in Brazilian society coupled with increased life expectancy has the effect of the high number of elderly people over 60 years. Given this, it is relevant to new reflections and analyzes of health promotion and disease prevention in the elderly population aging situation. This paper is a literature review conducted in academic articles and books that deal with the theme of Gerontological Nursing. In this sense, I tried searching dissertations conducted by nurses in the Post- Graduate in Gerontology, thinking that contribute to the interfaces between the Nursing and Gerontology, reflecting on the contributions of Gerontological Nursing to think about the present and the challenges for nursing professionals regarding the expansion of the aging population and specific care for older people
O processo de envelhecimento em curso na sociedade brasileira, acoplado ao aumento da expectativa de vida, tem como efeito o elevado número de pessoas idosas acima dos 60 anos. Diante disto, tornam-se relevantes novas reflexões e análises acerca da promoção e prevenção da saúde do idoso na conjuntura do envelhecimento populacional. Introdução: a vulnerabilidade é um conceito que visa ampliar a compreensão da condição de saúde do ser humano. Posto isto, o objetivo geral deste estudo foi analisar a vulnerabilidade individual, social e programática entre idosos que vivem no domicílio e sua associação com os desfechos adversos relacionados aos indicadores da condição de saúde, e mapeá-los espacialmente. Método: trata-se de um estudo epidemiológico de base populacional, do tipo inquérito domiciliar, transversal e observacional, desenvolvido no município de Uberaba (MG). Constituíram a amostra final 701 idosos selecionados por meio da amostragem por conglomerado em múltiplo estágio. Para avaliação do componente individual recorreu-se ao fenótipo de fragilidade; o programático por meio de um indicador de acesso e utilização do serviço de saúde; o social mediante a distribuição espacial dos setores censitários do município deste estudo. Procedeu-se análise descritiva, análise de componentes principais, teste qui-quadrado e modelos de regressão logística, logística multinomial e linear (p<0,05). A análise espacial foi realizada por meio de mapas temáticos. Resultados: constatou-se que 32,0 % dos idosos apresentava as três condições de vulnerabilidade (individual, social e programática) concomitantemente. Evidenciaram-se como fatores associados à condição de fragilidade física: sexo feminino (p=0,015) e faixas etárias de 70 a 79 anos (p=0,013) e 80 anos ou mais (p<0,001). Para condição de elevada/muito elevada vulnerabilidade social predominaram: faixas etárias de 60+ 70 anos (p=0,009) e 70+ 80 anos (p=0,039); ausência de escolaridade (p<0,001) (p<0,001) e 1+4 anos de estudo (p=0,001); e renda mensal < 1 salário mínimo (p=0,007), 1 salário (p=0,013) e 1+3 salários (p=0,027). Já os fatores associados à moderada vulnerabilidade programática foram: faixa etária 70+ 80 anos (p=0,039); não possuir escolaridade (p=0,017), 1+4 anos de estudo (p=0,003) e 4+9 anos de estudo (p=0,029). Na análise de associação entre os componentes da vulnerabilidade, constatou-se menor proporção de idosos frágeis com elevada vulnerabilidade programática quando comparados aos pré-frágeis e não frágeis (p=0,011). Na distribuição espacial dos desfechos adversos de saúde segundo a vulnerabilidade social, houve maior ocorrência de quedas, dependência para AIVD e péssima/má autopercepção de saúde entre idosos residentes em áreas de elevada/muito elevada vulnerabilidade social comparados aos demais. Em relação à associação entre os componentes de vulnerabilidade e os desfechos de saúde, observou-se associação da fragilidade física com todos os desfechos analisados (quedas, hospitalização, dependência para ABVD e AIVD, autopercepção de saúde e número de morbidade). O componente social associou-se à dependência para ABVD e AIVD e à autopercepção de saúde. Já o componente programático permaneceu associado apenas ao maior número de morbidade. Conclusão: os resultados evidenciaram que os idosos estão sujeitos às condições de vulnerabilidade sob as perspectivas biológica, social e no acesso e utilização de serviços de saúde. Infere-se que os componentes individual, social e programático de vulnerabilidade podem influenciar nas condições de saúde de idosos, sendo que a fragilidade física se relacionou a todos os desfechos analisados
Vulnerability is a concept intended to broaden understanding regarding the health condition of people. This study\'s general objective was to analyze the individual, social and programmatic vulnerability of elderly individuals living in the community and its association with adverse outcomes related to health status indicators, in addition to mapping them spatially. Method: This population-based, cross-sectional and observational epidemiological study included a survey conducted in the individuals\' homes in the city of Uberaba, MG, Brazil. The final sample included 701 elderly individuals selected through multistage cluster sampling. The fragility phenotype was used to assess the individual component; the programmatic component was verified through an indicator for access and attendance to health services; and the social component was verified through the spatial distribution of the census sector of the city under study. Descriptive analysis, principal components analysis, and the Chi-square test, in addition to logistic regression models and multinomial logistic regression (p<0.05) were performed. Spatial analysis was conducted using thematic maps. Results: 32.0% of the elderly individuals concomitantly presented three vulnerability conditions (individual, social and programmatic). The factors that appeared associated with a physically frail condition were: being a woman (p=0.015), aged from 70 to 79 years old (p=0.013) or 80 years old or older (p<0.001). The following predominated for conditions of high social vulnerability: aged from 60+ 70 years old (p=0.009) and 70+ 80 years old (p=0.039); no formal education (p<0.001) (p<0.001) and 1+4 years of schooling (p=0.001); and monthly income < 1 times the minimum wage (p=0.007), 1 times the minimum wage (p=0.013) and 1+3 times the minimum wage (p=0.027). The factors associated with moderate programmatic vulnerability were: ages between 70+ 80 (p=0.039); no formal education (p=0.017), 1+4 years of schooling (p=0,003) and 4+9 years of schooling (p=0,029). Analysis of association among vulnerability components revealed a lower proportion of frail elderly individuals with high programmatic vulnerability when compared to their pre-frail and non-frail counterparts (p=0.011). The spatial distribution of adverse health outcomes according to social vulnerability revealed a greater occurrence of falls, dependency for IADLs, and poor/very poor self-perception of health status among those living in areas of high/very high social vulnerability compared to their counterparts. In regard to association among vulnerability components and health outcomes, physical frailty was associated with all outcomes (falls, hospitalization, dependency for ADLs and IADLs, self-perception of health and number of morbidities). The social component was associated with ADL and IADL dependency and self-perception of health. The programmatic component remained associated only with a greater number of morbidities. Conclusion: The results evidenced that elderly individuals are subject to vulnerable conditions from a biological and social perspective also considering access and use of health services. The results suggest that individual, social and programmatic components of vulnerability influence the health conditions of elderly individuals, while physical frailty was related to all the outcomes under study

Trata-se de uma pesquisa qualitativa, descritiva- exploratória, do tipo estudo de caso único holístico, que teve como objeto de estudo o cuidado de enfermagem ao idoso hospitalizado com insuficiência cardíaca e os seguintes objetivos discutir o cuidado descrito pela equipe de enfermagem ao idoso hospitalizado com insuficiência cardíaca, identificar os elementos componentes desse cuidado, descrever os saberes gerontológicos da equipe de enfermagem que cuida desses idosos e analisar as possíveis dificuldades enfrentadas por essa equipe de enfermagem no cuidado ao idoso hospitalizado com insuficiência cardíaca. O cenário de pesquisa foi uma enfermaria clínica-cirúrgica de uma Organização Militar de Saúde, localizada no Rio de Janeiro. Fizeram parte do estudo 16 profissionais de enfermagem que atuam na referida enfermaria clínica-cirúrgica. A coleta de dados se deu através das entrevistas com os sujeitos, da observação sistemática do ambiente físico da enfermaria e da observação participante acerca do cuidado de enfermagem ao idoso hospitalizado. Os dados receberam tratamento qualitativo através de análise de conteúdo, utilizando como técnica, a análise temática. Os resultados apontaram como elementos do cuidado de enfermagem ao idoso hospitalizado com insuficiência cardíaca, àqueles relacionados à dimensão do afeto como o saber ouvir, o carinho, a atenção, e os relacionados à dimensão da prática como a monitorização dos parâmetros vitais e a minimização de esforços. As dificuldades enfrentadas pela equipe de enfermagem na realização do cuidado ao idoso hospitalizado com insuficiência cardíaca estão relacionadas à instituição, ao idoso, ao profissional e a equipe. Os saberes gerontológicos desta equipe têm origem no cotidiano profissional, como por exemplo, o reconhecimento da complexidade do cuidado a ser realizado ao idoso hospitalizado com insuficiência cardíaca. Conclui-se que a equipe de enfermagem desempenha um cuidado baseado em elementos afetivos e práticos, aplicando saberes que advém da prática profissional, havendo a necessidade da integração destes saberes com um saber de origem teórica, adquirido através de cursos de qualificação e/ou atualização com intuito de aumentar a qualidade do cuidado oferecido ao idoso hospitalizado com insuficiência cardíaca. Contribuições para a Enfermagem: espera-se que através desta pesquisa, a enfermagem aprimore seu olhar, suas técnicas e sua abordagem ao cliente idoso com insuficiência cardíaca contribuindo para a realização de um cuidado adequado e resolutivo.
It is a qualitative, descriptive-exploratory research, of the holistic unique case study which had as object of study the nursing care to institutionalized elders with heart insufficiency and the following objectives to discuss the care described by the nursing to the institutionalized elder with heart insufficiency, to identify the elements of these care, to describe the gerontology know-how of the nursing team that takes care of these elders and to analyze possible difficulties faced by this nursing team on caring the institutionalized elder with heart insufficiency. The research landscape is a clinical-surgical ward of a Health Military Organization, located in Rio de Janeiro. Sixteen professional nurses, who work in the mentioned clinical-surgical ward, were part of the study. Data collection was made through interviews with the individuals, by means of systematic observation of the physical environment of the ward and from the participant observation about the nursing care to the institutionalized elder. Data received qualitative treatment through content analysis, using as technique the thematic analysis. The results indicated as elements of the nursing care of the institutionalized with heart insufficiency those related to the dimension of the affection as to how to know to listen, the endearment, and the attention and still those related to the dimension of the practice of monitoring of vital parameters and the minimization of efforts. The difficulties faced by the nursing team in achieving the care to the institutionalized elder with heart insufficiency are related to the institution, to the elder, to the professional and to the team. The gerontology know-how of this team has origin from the professional quotidian, as for instance the recognition of the complexity of the care to be given to the institutionalized elder with heart insufficiency. It is concluded that the nursing team offers a care based in affective and practical elements, using the know-how acquired from the professional practice and there is the need of integration of all this knowledge to a knowledge of theoretical origin, acquired through qualification and/or refreshment courses with the aim of increasing the quality of the service offered to the institutionalized elder with heart insufficiency. Contributions to the Nursing: one expects that through this research nursing improves its focus, its techniques, and its approach to the elder with heart insufficiency, contributing to the implementation of an appropriate and resolutive care.

This study aims to systematize an educational experience in a Mid-Level Course of Professional Training Program for Health (PROFAPS), from Ministry of Health, held in São Paulo on the year 2015, whose purpose was to qualify assistants and nursing technicians of Primary Health Care for the integrated and comprehensive care to the elderly. To compose the experience report, we used the qualitative research, with exploratory and descriptive character and the participant observation was the methodological procedure elected, and Field Diary was the instrument for data collection. The Qualification Course in Gerontology, organized into eight modules, had its theoretical and practical proposition based on the methodology of questioning and culminated in the construction of a territorial action plan focusing on soft technologies. The methodology and didactic procedures adopted led the students to awaken their critical sense to the analysis of its own territory, its work process and its stereotypical vision of the old age (based on themselves and the other individuals), leading to the transformation of your own reality. The results were positive, particularly with regard to strengthening of health care networks, lines of care, continuing education and SUS itself
Este estudo objetivou sistematizar uma experiência de ensino em um Curso do Programa de Formação de Profissionais de Nível Médio para Saúde (PROFAPS), do Ministério da Saúde, realizado na cidade de São Paulo no ano de 2015, cujo propósito foi o de qualificar auxiliares e técnicos de enfermagem da Atenção Básica em Saúde para o cuidado integrado e integral ao idoso. Para compor o relato de experiência, utilizou-se a pesquisa de abordagem qualitativa, de caráter exploratório-descritivo, tendo a Observação Participante como procedimento metodológico e o Diário de Campo como instrumento da coleta de dados. O Curso de Qualificação em Gerontologia, organizado em oito módulos, teve sua proposição teórico-prática fundamentada na metodologia da problematização e culminou na construção de um plano de intervenção territorial com enfoque em tecnologias leves. A metodologia e os procedimentos didáticos adotados levaram o discente a despertar seu senso crítico para a análise de seu território, de seu processo de trabalho e da visão estereotipada da velhice (de si mesmo e do outro), conduzindo à transformação da sua realidade. Os resultados foram positivos, em especial no que tange ao fortalecimento das redes de atenção à saúde, das linhas de cuidado, da educação permanente e da própria escola formadora – o SUS

O número de idosos(as) vem aumentando e esta mudança populacional vem redefinindo as relações sociais e constituindo uma nova e preocupante imagem. O envelhecimento é uma experiência singular, sujeita a influências sócio-culturais e o corpo que envelhece é diferente do modelo social vigente e, em relação ao gênero, as mulheres são mais avaliadas pela sua aparência física, passando o envelhecimento a funcionar como uma ameaça, associada à perda da libido e da sexualidade, uma necessidade humana básica, independente de faixa etária, que está para além da genitalidade. O objetivo desta pesquisa foi verificar o conhecimento produzido acerca da interface das temáticas, mulheres idosas, gênero e sexualidade por meio da avaliação sistemática de periódicos da Enfermagem, Saúde Pública e Gerontologia, no período compreendido entre os anos de 2003 e 2007. É um estudo de abordagem qualitativa, caracterizado como pesquisa bibliográfica. A coleta dos artigos foi realizada por meio de descritores pré-estabelecidos e a análise se deu por categorização. Foram consultados 220 exemplares e pré-selecionados 362 artigos (111 na Enfermagem; 95 na Saúde Pública e 156 na Gerontologia), destes, sete foram selecionados, pois versavam sobre envelhecimento, gênero e sexualidade. Através da leitura desses artigos foi possível perceber uma preocupação em relação à qualidade de vida ou a promoção de um envelhecimento saudável, mas sexualidade e gênero das mulheres idosas continua um tema invisível. Foram abstraídas três categorias: feminilização da velhice, questão social da velhice feminina e idosa assexualizada. Sendo o envelhecimento e a própria gerontologia realidades recentes, falta preparo para a temática e, talvez por isso, o escasso material disponível. Apesar da produção pouco expressiva, é importante salientar que os(as) enfermeiros(as) e os grupos de estudos e pesquisa de gênero vêm despontando e apresentando preocupações relevantes com a temática. Quando se refere ao cuidado integral em idosas, não se pode esquecer a sexualidade, pois esta também está ligada ao cuidado e para tanto, os currículos dos cursos de graduação na área da saúde deveriam trazer mais reflexões sobre gênero e sexualidade das mulheres idosas nos componentes ligados à gerontologia e saúde da mulher, portanto, convidam-se os(as) professores(as) para que introduzam a temática gênero e sexualidade nos Projetos Pedagógicos dos Cursos (PPC) e que façam essa discussão nas disciplinas/conteúdos que contemplem a mulher idosa.
The number of elderly has increased and this change in population is redefining social relationships and is constituting a new and worrying image. Aging is a singular experience, subjected to socio-cultural influences and the aging body is different from the current social model and, regarding gender, women are the most evaluated in their physical appearance, so aging is turned into a threat, associated with the loss of libido and sexuality, a human basic need, regardless the age, which is beyond the genital aspect. The objective of this research is to verify the knowledge produced around the interface of themes, elder women, gender and sexuality, through systematic evaluation of Nursing journals, Public Health and Gerontology, in a period that comprises the years from 2003 to 2007. It is a study of a qualitative approach, characterized as bibliographical research. The collection of articles was done through pre-established descriptions and the analysis was carried through categories. 220 samples were consulted and 362 articles were pre-selected (111 in Nursing; 95 in Public Health and 156 in Gerontology), from these, seven were selected, because they talked about aging, gender and sexuality. Through reading of these articles it has been possible to notice the concern regarding quality of life or the development of healthy aging, but sexuality and gender of elder women continues to be an invisible theme. Three categories were abstracted: feminization of aging, social matter of women’s aging and asexualized elder. Considering aging and gerontology itself as recent realities, there is a lack of preparation for the theme and, maybe due to this, the rare available material. Despite the non expressive production, it is important to highlight that nurses and the groups of study of researches of this gender started to emerge and they are presenting relevant concerns about the theme. When it comes to the integral care of the elderly, one can not forget sexuality, because this is also connected to care and for that the syllabus o the graduation courses on health area should bring more reflections on the gender and sexuality of elder women in the components connected to gerontology and women’s health, therefore, professors are invited to introduce the theme gender and sexuality in the so-called Projetos Pedagógicos dos Cursos (PPC) and to create this discussion in the disciplines / contents which involve the elder woman.
El número de viejos viene creciendo y este cambio poblacional se manifiesta redefiniendo las relaciones sociales y constituyendo una nueva y preocupante imagen. El envejecimiento es una experiencia singular, sujeta a influencias socio culturales y el cuerpo que envejece es distinto del modelo social vigente y, en relación al genero, las mujeres son más evaluadas por su apariencia física, pasando el envejecimiento a funcionar como una amenaza, asociada a la pierda de la libido y de la sexualidad, una necesidad humana básica, independiente de su grupo etario, que está para más allá de genitalidad. El objetivo de esta investigación fue verificar el conocimiento producido acerca de la interface de las temáticas, mujeres mayores, género y sexualidad por medio de evaluación sistemática de periódicos de Enfermería, Salud Pública y Gerontología, en el periodo comprendido entre lo años de 2003 y 2007. Es un estudio de abordaje cualitativa, caracterizado como investigación bibliográfica. La coleta de los artículos fue realizada por medio de descriptores pre establecidos y la análisis se dio por categorización. Cuando se refiere al cuidado integral en mayores, no se puede olvidar la sexualidad, pues esta también está relacionada al cuidado y para tanto, los currículos de los cursos de graduación en la área de la salud deberían traer mas reflexiones sobre genero y sexualidad de las mujeres mayores en las asignaturas relacionadas a la gerontología y salud de la mujer, por eso, se invitan los (as) profesores (as) para que introduzcan la temática genero y sexualidad en los Proyectos Pedagógicos de los Cursos (PPC) y que hagan esa discusión en las asignaturas/contenidos que contemplen la mujer mayor.

A enfermagem brasileira em sua ampla dimensão teórico-prática tem procurado discutir as questões do contexto sócio-político que interferem nos setores da saúde e educação, tanto no nível superior como no nível técnico. Um dos desafios impostos pela atualidade é o envelhecimento populacional, que ocorre de forma acelerada e exige redefinições de papéis e ações para atender a população idosa. O presente estudo, teve como objetivo analisar a formação gerontológica do Técnico em Enfermagem. A escolha da abordagem metodológica deste estudo foi feita pela metodologia qualitativa, fundamentada nos pressupostos teóricos do construtivismo, geriatria e gerontologia e interpretação cultural. Dentre os vários caminhos para apreensão do objeto de estudo optou-se pelo Estudo de Caso, tendo como cenário um curso técnico em enfermagem de uma instituição de educação profissional. Os resultados obtidos permitem verificar que existe deficiência na formação de profissionais técnicos em enfermagem competentes e habilitados ao cuidado da população idosa. Também, ressalta-se que o próprio ser humano deve compreender a utilização de símbolos e significados das experiências vividas como um marco referencial para a promoção de iniciativas transformadoras condizentes com a realidade social.
The brazilian nursing, in its large theorical-practical dimension, has been trying to dicuss the social-political context questions, which interfere in the health and education sectors, both the superior and thecnical levels. One of the challenges enforced nowadays in the demogrphic aging, which occur quickly and demand redefinitions of roles and actions to attend of to the ederly population. The study had as objective to understand the geronthologic formation of the nursing technician. The methodologic broaching choice for this study was made by qualitative methodology, based on theoretical pressuposing of construtivism, geriatrics, geronthology and cultural interpretation. Among various ways for apprehension of the study object, this chose the study of case, having as scenary a nursing thecnical course of an institution of professional education. The gained results allow to verify the deficiency of formation of nursing technician professionals with competence and able in the care of elderly population. This also emphasize that the human creature must understand the using symbols and meanings of lived experiences as a referencial mark to the promotion of transforming initiatives which correspond to the social reality.

Este trabalho refere-se ao educar/cuidar/pesquisar centrado na orientação de enfermagem para o autocuidado da pessoa idosa, com diabetes, visando seu bem-estar; visto a evidência do crescimento demográfico da população idosa em todo o mundo e, em especial, no Brasil. Tem como objetivo aplicar, no contexto de um curso de autocuidado ministrado em oficina sociopoética, o Diagrama de Nola Pender como instrumento de produção de dados sobre a construção de ações e apresentação de propostas de comportamentos de saúde, visando o bem-estar de pessoas. O marco teórico enfoca a Teoria de promoção da saúde de Nola Pender, que defende o processo de capacitação da comunidade para buscar sua qualidade de vida e saúde, incluindo maior participação e controle deste processo. O marco teórico metodológico é a sociopoética, que fundamenta um estudo qualitativo, descritivo, desenvolvido em 2012, após aprovação do Comitê de Ética da Universidade do Estado do Rio de Janeiro, mediante a instituição do Grupo Pesquisador, dispositivo analítico, deste método, composto por 10 sujeitos, aplicando-se as técnicas de pesquisa de sensibilidade e artísticas. São resultados da técnica Vivência de Lugares Geomíticos a delimitação das categorias temáticas, a saber: Medo do desconhecido; Transcendendo a convivência com a diabetes através da aceitação; O autocuidado como resolução dos problemas a serem enfrentados; Busca da cura da diabetes através de uma vida saudável; e Expectativas para o futuro. Na técnica Corpo como território mínimo emergiram os temas: Insegurança; Fragilidade; Dificuldade no controle da glicemia; Controle; Autocuidado; Otimismo; Perseverança; Dificuldade no controle da alimentação; Tranquilidade; Dependência; Conformação; Revolta; Equilíbrio; Desânimo; Autoestima; Autoimagem e Descuidado. Aplicando o Diagrama de Nola Pender constatou-se entre os membros do Grupo Pesquisador (GP) os fatores pessoais que influenciam a conduta prévia à adoção do autocuidado, os benefícios de ações percebidas durante o ensino do autocuidado, e as demandas de competência para assumi-lo, visando à promoção da saúde. Conclui-se, que a conduta de promoção de saúde é a variável que conduz a um olhar através de ações dirigidas que tenham resultados positivos para o bem-estar e o equilíbrio das dimensões corporais. Entre as condutas de promoção da saúde adotadas pelo GP estão: ter compromisso na realização de atividades físicas; alimentação saudável, uso de medicação diariamente e regularmente, além do autocuidado com o corpo e mente, através de práticas de atividade física e de lazer.
This paper refers to education/care/research focused on nursing orientation for self-care of the elderly with diabetes, seeking their welfare; as there are evidences related to elderly growth in the world population and, in particular, in Brazil. It aims to apply the Nola Pender Diagram in the context of a course about self-care based on social poetics, as a way to produce data based on the actions and proposals for health behaviors, seeking the welfare of people. The theoretical framework focuses on Nola Penders theory of health promotion, saying that the process of enabling people to seek their quality of life and health, including greater participation and control of this process. The theoretical methodology is social poetics, which establishes a qualitative and descriptive study, through the establishment of a research group, analytical device of this method, consisted of 10 individuals, applying artistic and sensitivity research techniques. The technique "experience in geomitic places results on the delimitation of the following themes: Fear of the unknown; Transcending living with diabetes through acceptance; Self-care as resolution of problems to be exceeded; Searching for a cure for diabetes through healthy living, and expectations for the future. In the technique "Body as minimum territory" the following themes emerged: Insecurity; Fragility; Difficulty in controlling blood glucose; Control, Self-Care, Optimism, Perseverance; Difficulty in controlling feeding; Tranquility; Dependence; Conformation; Revolt; Balance; Discouragement; Esteem; Careless and self-image. By applying the Nola Pender Diagram it was possible to find the research group personal factors that influence behavior prior to the adoption of self-care, the benefits of perceived actions during teaching self-care, and the demands of competence to assume so in order to promote health. It was found that the conduction of health promotion is the variable that leads to a look through targeted actions that have positive outcomes for the welfare and balance the body dimensions. The following actions for health promotion adopted by the research group were found: commitment to physical activity; healthy eating, daily and regular medication and self-care with the body and mind through practice of physical activity and leisure.

Culturally, care delivered to elderly individuals in Brazil is usually provided by families. Even though only 1% of the Brazilian population older than 60 years of age is cared for in long stay institutions, the search for this type of facility has increased, mainly due to population aging and new family arrangements. This study analyzes the relation between family functionality and the variables of age, gender, duration of institutionalization, family composition, depressive symptoms and cognitive changes in elderly individuals residing in long stay facilities in a city in the interior of São Paulo, SP, Brazil. This is a quantitative cross- sectional study with a descriptive correlational design. A total of 107 elderly individuals were individually interviewed using the following: Family APGAR, Mini State Mental Exam, Katz Index, Pfeffer Functional Activities Questionnaire and Geriatric Depression Scale. Outcomes were analyzed by logistic regression analysis using the hierarchical analysis using the Stepwise technique. This study complied with all ethical aspects. In relation to socio-demographic characteristics, the institutionalized elderly individuals were mostly older elderly women (61%), (45%)with a low education level, (87%) Catholic and (45%) widowed. Most independently performed basic daily living activities (46%), but were dependent for instrumental activities of daily living, had depressive symptoms (67%) and cognitive impairment (53%). The results show that 57% of the elderly individuals had a high level of family dysfunction, 22% had good family functionality and 21% moderate family dysfunction. No statistically significant association was found between the family APGAR and the variables gender and age. Elderly individuals with families composed of more distant relatives such as siblings, nephews and nieces, with greater time of institutionalization, depressive symptoms and cognitive impairment were also more dysfunctional. The institutionalization of elderly with time over 3 years (OR = 3.861, p = 0.0285), presence of depressive symptoms (OR = 5.378, p = 0.0044) and family composition formed by brothers / Nephews / Other (OR = 6.812, p = 0.0155) are more likely to experience family dysfunction. These findings can contribute to planning for care delivered to the family of institutionalized families.
No Brasil, o cuidado com os idosos é culturalmente realizado pelas famílias. Embora apenas 1% da população brasileira com mais de 60 anos seja cuidada em Instituições de Longa Permanência para Idosos - ILPI, a procura por esse tipo de instituição tem sido cada vez maior em função, principalmente, do envelhecimento populacional e dos novos arranjos familiares. O objetivo deste estudo foi analisar a relação entre funcionalidade familiar e as variáveis idade, gênero, tempo de institucionalização, composição familiar, sintomas depressivos e alterações cognitivas de idosos que residem em instituições de longa permanência em uma cidade do interior de São Paulo. Trata-se de um estudo quantitativo, transversal com desenho descritivo-correlacional. Para a coleta de dados, foram realizadas entrevistas individuais com 107 idosos institucionalizados, em São Carlos SP. Foram aplicados os seguintes instrumentos: APGAR de Família, Mini Exame do Estado Mental, Índice Katz, Questionário de Atividades Funcionais e Pfeffer e Escala de Depressão Geriátrica. Os desfechos foram analisados por meio da análise de Regressão Logística, utilizando a análise hierarquizada, utilizando a técnica Stepwise. Todos os cuidados éticos foram observados. Com relação às características sóciodemográficas, os idosos institucionalizados são em sua maioria mulheres (61%), mais longevas (45%), com baixa escolaridade (46%), católicas (87%) e viúvas (45%). A maioria é independente para as atividades básicas da vida diária (46%), dependente para as atividades instrumentais de vida diária (69%), apresentam sintomas depressivos (67%) e alterações cognitivas (53%). Os resultados mostram que 57% dos idosos apresentam elevada disfunção familiar, 22% apresentam boa funcionalidade familiar e 21% moderada disfunção familiar. Não houve associação estatisticamente significante entre o APGAR de Família e as variáveis sexo e idade. Idosos com composição familiar formada por pessoas mais distantes como irmãos/sobrinhos e outros, com maior tempo de institucionalização, com presença de sintomas depressivos e com alterações cognitivas apresentaram-se mais disfuncionais. O idoso com tempo de institucionalização acima de 3 anos (OR = 3,861; p= 0,0285 ), com presença de sintomas depressivos (OR = 5,378; p = 0,0044 ) e com composição familiar formada por Irmãos/Sobrinhos/Outros (OR = 6,812; p = 0,0155 ) tem maior chance de apresentarem disfunção familiar. Esses resultados poderão contribuir para um planejamento do cuidado da família dos idosos institucionalizados.

In this work, the focus of the inquiry is come back toward (1) the configuration of Ample Geriatric Evaluation and the effect of its use in the scope of the Alive Program more good best age (a set of activities proposals for an operator of plans of health in the São Paulo city) e, also, for (2) the quarrel of the contribution of the nursing in the promotion of health of the aged one. The use of the Ample Geriatric Evalution , disgnostic instrument considered by Warren (1930), has been tied with the adoption of new a paradigm in the attention to the health of the aged one, in function of the instauration of a differentiated look - more full and humanized - to approach the health condition and to guide therapeutical come back toward the improvement of the quality of life of this age segment. A qualitative boarding of the data - gotten through the accomplishment of interviews half-structuralized with participant citizens of the related Program associates to the notations, made for me (observing participant) of the accompaniment of the same ones -, was taken as one of the ways (beyond the quarrel properly theoretician) to make possible the commitment of inquiry with the object of this study. One notices that I gave priority when saying of the aged ones. I clarify, still, that the directed reflection took as reference theoretician-methodological the works of Canguilhem (1966), Beauvoir (1970), Geertz (1989) e Bogdan (1994). In the first chapter, I explore the concepts of health and illness and the way as such reflection (directed in the Medicine) has produced effect in the field of the Gerontology. I looked for to make to be valid the beddings of this field (that if distance of an exclusively biological vision of the aging and the oldness) to problematize the subjective-social condition of the aged ones submitted to the Ample Geriatric Evaluation. In as the chapter, what it is in cause is the joint of the team to multidiscipline required for the accomplishment of ample the geriatric evaluation - with prominence for the contribution of the nursing. What it was intended was, from a critical reading of representative authors of this field, to designate its necessary opening to the beddings that underpin the reflection directed in the gerontological studies. Finally, in the third chapter, I offer an interpretation of the data collected in the field research. The results of this enterprise had indicated that she is necessary to place in relief, in the specific formation required the nurse, a resizing of the health notion that includes the opening of listening for the complex demands restored in the practical one of care with the aged one. This creates the requirement of installation of the paradigm to interdisciplinary in the area of the health, what it implies, of my point of view, to take the aged one as protagonist of changes so that to live the oldness well it is possibility each time next to the reality (in such a way of the social point of view, as subjective). I wait that the debate carried through here contributes the reflection and awareness of the multiprofessional team of health on ample the geriatric evaluation and its function of disgnostic complementation, with emphasis in the education in health
Neste trabalho, o foco da investigação está voltado para (1) a configuração da Avaliação Geriátrica Ampla (AGA) e os efeitos da sua utilização no âmbito do Programa Viva melhor a melhor idade (um conjunto de atividades propostas por uma operadora de planos de saúde na cidade São Paulo) e, também, para (2) a discussão da contribuição da enfermagem na promoção de saúde do idoso. A utilização da AGA, instrumento diagnóstico proposto por Warren (1930), tem sido vinculada à adoção de um novo paradigma na atenção à saúde do idoso, em função da instauração de um olhar diferenciado - mais integral e humanizado para abordar a condição de saúde e orientar terapêuticas voltadas para a melhoria da qualidade de vida desse segmento etário. Uma abordagem qualitativa dos dados - obtidos através da realização de entrevistas semi-estruturadas com sujeitos participantes do referido Programa associadas às anotações, feitas por mim (observadora participante) do acompanhamento dos mesmos -, foi tomada como um dos meios (além da discussão propriamente teórica) para viabilizar o compromisso de investigação com o objeto deste estudo. Note-se que dei prioridade ao dizer dos idosos. Esclareço, ainda, que a reflexão encaminhada tomou como referencial teórico-metodológico os trabalhos de Canguilhem (1966), Beauvoir (1970), Geertz (1989) e Bogdan (1994). No primeiro capítulo, exploro os conceitos de saúde e doença e o modo como tal reflexão (encaminhada na Medicina) tem produzido efeitos no campo da Gerontologia. Procurei fazer valer os fundamentos deste campo (que se distancia de uma visão exclusivamente biológica do envelhecimento e da velhice) para problematizar a condição subjetivo-social dos idosos submetidos à AGA. No segundo capítulo, o que está em causa é a articulação da equipe multidisciplinar requerida para a realização da avaliação geriátrica ampla - com destaque para a contribuição da enfermagem. O que se pretendeu foi, a partir de uma leitura crítica de autores representativos desse campo, assinalar sua necessária abertura aos fundamentos que alicerçam a reflexão encaminhada nos estudos gerontológicos. Finalmente, no terceiro capítulo, ofereço uma interpretação dos dados coletados na pesquisa de campo. Os resultados desse empreendimento indicaram que é preciso colocar em relevo, na formação específica requerida ao enfermeiro, um redimensionamento da noção de saúde que inclua a abertura da escuta para as complexas demandas instauradas na prática de cuidado com o idoso. Isso cria a exigência de assunção do paradigma interdisciplinar na área da saúde, o que implica, do meu ponto de vista, tomar o idoso como protagonista de mudanças para que viver bem a velhice seja possibilidade cada vez mais próxima da realidade (tanto do ponto de vista social, como subjetivo). Espero que o debate aqui realizado contribua a reflexão e conscientização da equipe multiprofissional de saúde sobre a avaliação geriátrica ampla e a sua função de complementação diagnóstica, com ênfase na educação em saúde

Background: The elderly Chinese group is growing in Sweden but no studies about their experiences of being cared for were found. In order to meet the elderly Chinese need for care, it is necessary to understand their perspectives of being cared for. Aim: To describe the meaning of the experiences of being cared for by elderly Chinese immigrants in a Swedish context. Method: For this phenomenological study, open-end interviews were carried out with 7 informants originally from Mainland China and Hong Kong. Result and Conclusion: For the elderly Chinese in Sweden, ‘being cared for’ means being involved in a caring community, to have a mutual relationship with someone who can see and understand their needs. In such a relationship, they felt that they were included and respected. They were being treated as valuable with a genuine manner. They had a feeling of satisfaction, happiness, peacefulness, closeness, togetherness, and wholeness when ‘being cared for’. For these elderly Chinese immigrants, informal care was indispensable even when they were receiving formal care. They wished that their family could show concern and formal caregivers could understand and give space to the informal caregivers on caregiving.

O envelhecimento da população mundial é um fenômeno novo, ao qual, mesmo os países desenvolvidos, ainda estão tentando adaptar-se como envelhecer de forma saudável, priorizando esforços na manutenção da independência e autonomia do indivíduo. Os princípios deste envelhecimento com qualidade de vida descrevem o idoso como pró-ativo, definindo seus objetivos e lutando para alcançá-los, reunindo recursos que são úteis na adaptação à mudança e ativamente envolvidos na manutenção do bem-estar. Este modelo compreende desde a satisfação com a vida a modelos baseados em conceitos de independência, controle, competências sociais e cognitivas. O objetivo desta pesquisa é a caracterização do perfil dos idosos que participam do Centro de Convivência do Idoso (CCI) quanto ao sexo, idade, estado civil, etnia autodeclarada, escolaridade, ocupação, renda individual, número de filhos, naturalidade, condições de moradia, meio de locomoção que utiliza para ir ao CCI e atividade física preferida e descrever as condições de saúde e desempenho nas atividades de vida diária dos idosos participantes de CCI, segundo a Classificação Internacional de Funcionalidade, Incapacidade e Saúde(CIF). Pesquisa do tipo exploratória, descritiva, transversal, com abordagem quantitativa, realizado em CCIs do Vale Paraíba, São Paulo. Foram incluídos no estudo aqueles que atendiam aos critérios de inclusão com idade igual ou superior a 60 anos, participantes, há pelo menos três meses completos e no máximo um ano dos CCIs, que constituíram o campo de pesquisa. O estudo caracterizou-se pelo predomínio de mulheres, idade média de 72 anos, casadas, etnia autodeclarada branca, com 5-10 anos de educação escolar, aposentadas, com média de renda mensal de um salário mínimo, por meio do instrumento de coleta foi possível observar que a maior parte das categorias da CIF selecionadas era pouco comprometida com dificuldade leve ou sem dificuldade.
The aging world population is a new phenomenon, which even developed countries are still trying to adapt to aging in a healthy way, prioritizing efforts to maintain the independence and autonomy of the individual. The principles of this aging quality of life describe the elderly as proactive, setting goals and striving to achieve them, gathering resources that are useful in adapting to change and actively involved in maintaining wellness. This model comprises from life satisfaction to models based on concepts of independence, control, social and cognitive skills. The goal of this research is to characterize the profile of the elderly participating in the Elderly Community Centre (CCI) regarding gender, age, marital status, self-reported ethnicity, education, occupation, individual income, number of children, nationality, living conditions , means of transportation you use to go to the CCI and preferred physical activity and describe the health and performance in activities of daily living of the elderly participants from CCI, according to the International Classification of Functioning, Disability and Health(ICF). Research An exploratory, descriptive , Cross-sectional quantitative approach carried out in CCIs Paraíba Valley, São Paulo. The study included those who met the inclusion criteria of age or over 60 years, participating for at least three months and complete within one year of the CCIs, which were the search field. The study was characterized by the predominance of women, mean age 72 years, married, self-reported ethnicity white, with 5-10 years of education, retired, with an average monthly income of a minimum wage, through the data collection instrument was observed that most of ICF categories selected was somewhat compromised with slight difficulty or no difficulty.

As the population ages with economy flourishing, more of us are faced with the prospect of moving our older family member into a nursing or convalescent home. Though it is considered as a family's duty to care for elders, however, in today’s world of smaller families living farther apart and work conflicts, considering a nursing home may be another option. Staying in a nursing home might be a good choice for retired life, but it is not easy to apply. Some problems may occur before applying to stay in a nursing home, such as long waiting and nursing expanses. First of all, it may be very time-consuming to wait for applying to stay in a nurse home, possibly weeks or months. Second, the nursing expanses may cost NT20,000 to NT30,000 per month, which may not be affordable to everyone. Besides, most of the nursing homes are providing nursing care for long-term and charging per month or per year. Short-term geriatric nursing, for hours, days or weeks, may be needed but rarely provided nowadays. There are various situations for geriatric nursing to be discussed. Some families may not be able to afford the continuous expanse for long-term nursing care. Some may care for the elders by themselves at home, but sometimes the family members may be occupied with unexpected events and become unable to care for the elders in a certain period. Others may have felt stressful during the long-time caring for the elders and need a break. This thesis is to discuss the different situations of geriatric nursing. To achieve the purpose of this study, a questionnaire is made via email to compare with the differences between longterm and short-term care and to survey both advantages and disadvantages in these two different ways.

Therefore, when people encountering these issues, it is difficult to make an ethical decision whether choosing home care or long-term care institute. Indiviual consuming decision is a series of complicate process. When people encountering conflicts between disable perents care and self-interests, the process of ethical decision is essential. Finally, ehtheical or unetheical behaviors will be occurred. This study will leverage the Ethical Decision Making (EDM) framework which is strongly dependent on personal views of morality, the magnitude of consequence, and moral intensity. It is expected that applying the EDM framework to the dilemma related to long-term elderly parental care will provide a more enlightened method on which to base this difficult decision. This study uses a multiple-situation survey consisting of vignettes and questions as an assessment tool. The survey population consisted of subjects who met the following conditions: • born between 1963-1973 (36-46 years old) • at least one of the parents is living • adult children are married A total of 450 questionnaires were issued with 312 valid responders (response rate of 89.4%). The Hypothesis Zeyi multiple regression and hierarchical regression analysis was used to analyze the survey results. Demographic characteristics, differences in moral decision making process, moral intensity on ethical decision making and filial responsibility were determined to have a moderating effect on the moral decision making process. This study’s findings have management and practical implications for long-term care providers. One such implication is the opportunity for long-term care provider to educate and reassure family members who are faced with this difficult decision. Such education efforts will begin to influence public opinion such that professional long-term care would be considered an acceptable choice. As such, long-term care providers are likely to experience an increase occupancy rate.
