Abstract:
Penelitian ini bertujuan untuk mengidentifikasi sejauh mana persepsi lansia terhadap asuhan keperawatan gerontik yang diberikan oleh mahasiswa FIK-UI di Kelurahan Rawa Bunga Kecamatan Jatinegara Jakarta Timur. Metoda yang digunakn adalah deskriptif eksploratif. Sampel yang diambil sebagai responden adalah lansia yang telah dibina mahasiswa pada saat melakukan praktek keperawatan gerontik. Hasil dari penelitian ini menunjukkan bahwa lansia dapat merasakan manfaat dilakukannya asuhan keperawatan gerontik dengan skor rata-rata 4,04. Untuk menindaklanjuti penelitian ini direkomendasikan untuk melakukan penelitian tentang lansia terhadap asuhan keperawatan gerontik dengan menggunakan metode kualitatif, karena persepsi merupakan penjelasan tentang pengalaman yang perlu dipelajari secra lebih mendalam sehingga tidak cukup hanya menggunakan metode kuantitatif. The purpose of this research was to identify how to elderly perception of the nursing care of elderly that was given by the students of Faculty of Nursing University of Indonesia. The area of this research was in 3 sub districts of Rawa Bunga district, East Jakarta. The research utilized explorative of descriptive design. Sample that was taken as the respondent were elderlies who have been cared by the student while they do their gerontological clinical practice. The result of this research showed that elderlies got the benefit from gerontic from gerontic nursing care by score 4,04. This research recommend to conduct the same research using qualitative method to gain more information related to perception and experience of elderlies.

Abstract:
Introduction: Rheumatoid arthritis is the most common autoimmune disease in the community. This disease is characterized by inflammation of the lining of the synovium of the joint. It can cause long-term joint damage, prolonged pain, loss of function and disability. Objective: This study aims to determine nursing care for the elderly with a medical diagnosis of rheumatoid arthritis. Methods: The research design used a case study with a nursing process approach. The sampling technique used is convenience sampling. The sample in this study was Mrs. U age 70 years. The research was conducted at the Jagakarsa Public Health Center, Jakarta in May 2021. The data was collected by means of interviews, observations, and documentation studies. The research instrument is a gerontic nursing care format. Results: The results of nursing care on Ny. U was found that the client had fallen and sprained his leg two months ago. Currently the client feels pain in the left knee which is dislocated and looks swollen. Nursing diagnosis i.e. chronic pain related to physical injury agent Conclusion: The priority nursing problem for clients is pain can be overcome by giving warm compresses.

Nursing practice directly in the field is one of supporting the achievement of student skills. However, since the status of COVID-19 disaster response in DI. Yogyakarta is determined, giving effect to the learning process of nurses in STIKES Guna Bangsa Yogyakarta. The preceptor must modify the online learning method while still gaved attention to the achievement of the target competency for students. This research aims to identify student perceptions related to the appearance of preceptors, used a modified questionnaire. The design of this study uses descriptive analytic analysis with cross sectional approach. The results showed in general the process that was passed into the category very well. The biggest obstacle faced is the unstable internet network. But that does not reduce the achievement of competencies of students. The conclusions of the results of this study the professional practice learning methods conducted online at the gerontik nursing stage are in the excellent category and are able to support the achievement of student skills.

Abstract:
Introduction : Continuous urinary incontinence is the continued and uncontrolled passing of urine without distension and a feeling of fullness in the bladder due to neurological dysfunction. This condition is often found in the elderly post-ischemic stroke. The study aimed to describe nursing care of continuous urinary incontinence in the elderly post-ischemic stroke in Lamongan. Methods : This study used a case study approach (case report) on a participant through the nursing process approach. The sample in this study is one patient, an elderly post-ischemic stroke. This research was conducted in 2021 in Lamongan with three visits to patients' homes for seven days. Data collection techniques include interviews, observation, and physical examination. This research instrument uses the gerontic nursing care format and sandvik severity index. Results : Continuous urinary incontinence nursing care with the main intervention for urinary incontinence treatment (I.04163) and the supporting intervention is pelvic muscle exercises (I.07215) can be applied to elderly post-ischemic stroke according to the expected outcomes. The continuous urinary incontinence has a partially resolved on the third home visit. The client can hold urine several times when the bladder feels full, reduced the frequency of night urination, and rarely wakes up because of wet diapers. Sandvik Severity Index shows a value of 4 (moderate incontinence). Conclusions: Improving nursing care need for collaboration between patients, families, and health workers in performing pelvic muscle exercises independently, continuously, and regularly to increase the strength of the detrusor-sphincter muscles and showed improvement in urinary continence.

ABSTRACT Aging is a continuous process of tissue's ability damage to repair and maintain the structure and function normally. A cataract is known as a visual impairment and disease that is common in the elderly, but it also can be caused by congenital anomalies. Health education is the activity which is carried out by spreading the knowledge in order that the people realize and understand in conducting health suggestions. The method used descriptive case study design by using the nursing process approach which includes assessment, diagnosis, intervention, implementation, and evaluation. The population in this study were the patients with the research subject of two elderly with the same case. The results obtained the descriptions of the first and second cases named Mr. M and Mrs. S after conducting 4 days of health education. The patient's knowledge increased by being able to explain about cataracts. The statement and observation of the patients showed that the problem of sensory perception disorders in patients was resolved. It can be concluded that the nursing process by using health education could increase the knowledge in the elderly. The results of the study can be used as advice for other researchers to conduct further research on gerontic nursing care with visual disorders: Cataracts in the elderly. Katarak dikenal sebagai gangguan penglihatan dan merupakan penyakit yang umum terjadi pada lansia, selain itu katarak juga dapat disebabkan oleh kelainan kongenital. Pendidikan kesehatan adalah kegiatan yang dilakukan dengan menyebarkan pengetahuan agar masyarakat sadar dan mengerti dalam melakukan sugesti kesehatan. Metode yang digunakan adalah desain studi kasus deskriptif dengan menggunakan pendekatan proses keperawatan yang meliputi pengkajian, diagnosis, intervensi, implementasi dan evaluasi. Populasi dalam penelitian ini adalah pasien dengan subjek penelitian dua orang lansia dengan kasus yang sama. Hasil penelitian didapatkan gambaran kasus pertama dan kedua bernama Tn. M dan Ny. S setelah dilakukan penyuluhan kesehatan selama 4 hari. Pengetahuan pasien bertambah dengan mampu menjelaskan tentang katarak. Pernyataan dan observasi pasien menunjukkan bahwa masalah gangguan persepsi sensorik pada pasien teratasi. Dapat disimpulkan bahwa proses keperawatan dengan menggunakan pendidikan kesehatan dapat meningkatkan pengetahuan pada lansia. Hasil penelitian ini dapat dijadikan sebagai masukan bagi peneliti lain untuk melakukan penelitian lebih lanjut tentang asuhan keperawatan gerontik dengan gangguan penglihatan: Katarak pada lanjut usia. AbstractStroke is a brain disorder that occurs due to damage to part of the brain caused by blocked blood vessels so that the flow of oxygen is not met properly. Stroke can cause death, paralysis, impaired speech, and decreased consciousness. The purpose of this case study was to increase lower extremity muscle strength by using ROM exercises. The method used was a case study by providing nursing care to two elderly post-stroke using Active Rom exercises. The results of this case study before doing active ROM on client I experienced a decrease in muscle strength with a percentage of 25 degrees 2 and client II experienced a decrease in muscle strength with a percentage of 25 degrees 2. After doing active ROM there was an increase in muscle strength in client I with a percentage of 75 degrees 4 and on client II with a percentage of 50 degrees 3. This case study can be said that the application of active range of motion (ROM) exercises can increase lower extremity muscle strength in the elderly after stroke. This case study is a consideration for nurses and other health workers to apply active ROM exercises in post-stroke patients.Key words : Gerontic Nursing Care, Elderly Post-Stroke, Active ROM. AbstrakStroke adalah gangguan pada otak yang terjadi karena adanya gangguan kerusakan pada sebagian otak disebabkan karena pembuluh darah yang tersumbat sehingga aliran oksigen tidak terpenuhi dengan baik. Penyakit stroke dapat menyebabkan kematian, kelumpuhan, gangguan berbicara, dan menurunkan kesadaran. Tujuan dari studi kasus ini adalah untuk meningkatkan kekuatan otot ekstremitas bawah dengan menggunakan latihan ROM. Metode yang digunakan adalah studi kasus dengan memberikan asuhan keperawatan pada dua lansia pasca stroke dengan menggunakan latihan Rom Aktif. Hasil studi kasus ini sebelum dilakukan ROM aktif pada klien I mengalami penurunan kekuatan otot dengan presentase 25 derajat 2 dan klien II mengalami penurunan kekuatan otot dengan presentase 25 derajat 2. Setelah dilakukan ROM aktif terjadi peningkatan kekuatan otot pada klien I dengan presentase 75 derajat 4 dan pada klien II dengan presentase 50 derajat 3. Kesimpulan studi kasus ini dapat disimpulkan bahwa penerapan latihan range of motion (ROM) aktif dapat meningkatkan kekuatan otot ekstremitas bawah pada lansia pasca stroke. Studi kasus ini sebagai pertimbangan bagi perawat dan tenaga kesehatan lain untuk menerapkan latihan ROM aktif pada pasien pasca strokeKata kunci: Asuhan Keperawatan Gerontik, Lansia Pasca Stroke, ROM aktif.

AbstractElderly is someone who enters teh age of 60 years or more and experiences physical and physiological changes. Health problems that are commonly experienced by the elderly are hypertension. Hypertension is a health problem characterized by an increase in systolic and diastolic blood pressure of more than 140/90 mmHg. Non-pharmacological action that is useful in reducing blood pressure is listening to murottal Al-Quran therapy. The purpose of the action is to reduce blood pressure in the elderly with hypertension. This research used the case study method of gerontic nursing care with the therapeutic technique of listening to Al-Quran murottal. The application of Al-Quran murottal listening intervention which was carried our for 7 visits was proven to redue blood pressure in the elderly. The first clien’t initial blood pressure was 170/100 mmHg to 120/90 mmHg (the average drop in systolic pressure is 45-50 mmHg and diastolic is 10 mmHg), while the second clien’t initial blood pressure was 175/100 mmHg to 120/100 mmHg(the average drop in systolic pressure is 45-50 mmHg and diastolic is 10 mmHg). this result can be used as a consideration for nurses to provide Al-Quran murottal therapy as an alternative nursing action for the elderly with hypertension.Keywords : Hypertension, Elderly, Al-Quran Murottal listening therapy AbstrakLansia merupakan seseorang yang memasuki usia 60 tahun atau lebih dan mengalami perubahan fisik maupun fisiologis, salah satu masalah kesehatan yang biasa dialami lansia adalah hipertensi. Hipertensi merupakan masalah kesehatan dengan ditandainya kenaikan tekanan darah sistolik dan diastoliknya lebih dari 140/90 mmHg. Tindakan non-farmakologi yang bermafaat dalam penurunan tekanan darah yaitu terapi mendengarkan murottal Al-Quran. Tujuan dilakukan tindakan yaitu untuk menurunkan tekanan darah pada lansia hipertensi. Karya Tulis Ilmiah ini disusun dengan menggunakan metode studi kasus asuhan keperawatan gerontik dengan teknik terapi mendengarkan murottal Al-Quran. Penerapan terapi mendengarkan murottal Al-Quran yang dilakukan selama 7 kali kunjungan terbukti dapat menurunkan tekanan darah pada lansia. Lansia klien 1 dengan tekanan darah awal 170/100 mmHg menjadi 120/90 mmHg (rata-rata penurunan tekanan sistolik 45-50 mmHg dan diastoliknya 10 mmHg), klien lansia 2 dengan tekanan darah awal 175/100 mmHg menjadi 120/100 mmHg mmHg (rata-rata penurunan tekanan sistolik 45-50 mmHg dan diastoliknya 10 mmHg). Perawat diharapkan dapat memberikan terapi Murottal Al-Quran secara maksimal sebagai salah satu alternatif tindakan keperawatan bagi lansia dengan hipertensi.Kata kunci : Hipertensi; Lansia, Terapi Murottal Al-Quran

Tujuan penelitian ini untuk mengetahui pengaruh terapi reminiscence terhadap tingkat stress pada lanjut usia di Banjar Tangkas Wilayah Kerja Puskesmas Tegallalang 1. Metode : Metode penelitian ini menggunakan desain penelitian ini menggunakan jenis penelitian Pra-eksperiment dengan rancangan penelitian One-group pre-test post-test design. Teknik sampling yang digunakan non probability sampling dengan teknik purposive sampling didapatkan sampel sejumlah 20 orang. Instrument pengumpulan data menggunakan DASS 42 Psychometric Properties Of The Depression Anxiety Stress Scale. Hasil penelitian rata-rata tingkat stress pada lansia pre-test 59,20 dan post-test 41,20 di dapatkan nilai p value =0,000<α=0,05 menunjukkan ada Pengaruh Terapi Reminiscence terhadap tingkat stres pada lanjut usia yang berarti adanya pengaruh yang signifikan. Diskusi : Terapi Reminiscence berpengaruh secara signifikan menurunkan tingkat stres pada lanjut usia di Banjar Tangkas Wilayah Kerja Puskesmas Tegallalang 1. Di sarankan agar terapi reminiscence di jadikan metode alternatif tindakan keperawatan yang dapat dilaksanakan di keperawatan gerontik guna membantu lansia dalam menurunkan stres. The purpose of this study is to determine the influence of Reminiscence therapy toward stress levels in elderly at Banjar Tangkas within healthcare coverage of Tegallalang community health center 1. Method: This research method is designed to use Pre-experimental research type with One-group pre-test post-test design. The sampling technique used is a non probability sampling with purposive sampling technique that produces sample of 20 people. Data collection instrument uses DASS 42 Psychometric Properties of the Depression Anxiety Stress Scale. Result: The result shows the average stress level of elderly in pre-test is 59,20 and in post-test is 41,20 which resulted p value = 0,000 <α=0,05 indicate that there is influence of Reminiscence therapy toward stress level in elderly which means that there issignificant influences. reminiscence therapy significantly influences lower stress level in elderly in Banjar Tangkas With in Health care Coverage of Tegallalang Community Health Center 1. Kemandirian lansia dalam ADL (Activity Daily Living) didefinisikan sebagai kemandirian seseorang dalam melakukan aktivitas dan fungsi –fungsi kehidupan sehari-hariyang dilakukan oleh manusia secara rutin dan universal. Rancangan penelitian yang digunakan adalah penelitian deskritif kuantitatif dengan metode survey. Penelitian ini dilaksanakan selama bulan April – September 2021 di Kampung Nagha 1, Kampung Pokole, Kampung Balane, Kampung Binala Kecamatan Tamako Kabupaten Kepulauan Sangihe. Teknik analisa data merupakan cara mengelolah data dengan Editting, Coding, dan Data Entri. Penelitian tentang tingkat kemandirian lansia dalam pemenuhan ADL dengan Metode Barthel Indeks di Posyandu Lansia Kecamatan Tamako Kabupaten Kepulauan Sangihe menunjukkan bahwa sebagian besar lansia berada pada kategori mandiri (tidak ketergantungan). Hasil penelitian ini dapat dijadikan informasi tentang status tingkat kemandirian lansia dalam pemenuhan ADL di Wilayah Kerja Puskesmas Tamako dan diharapkan lebih giat meningkatkan kualitas pelayanan pada lansia agar tingkat kemandirian ini tetap dijaga. Hasil penelitian ini juga menambah literatur dan wawasan bagi pembaca khususnya keperawatan gerontik sehingga memudahkan pembaca untuk dijadikan referensi tambahan yang dapat digunakan untuk meningkatkan ilmu pengetahuan. Elderly independence in ADL (Activity Daily Living) is defined as a person's independence in carrying out activities and functions of daily life carried out by humans routinely and universally. This study aims to determine the level of independence of the elderly in fulfilling ADL (Activity Daily Living) with the Barthel Index method at the Elderly Posyandu, Tamako District, Sangihe Islands Regency. The research design used is descriptive quantitative research with a survey method. This research was carried out from April to September 2021 in Nagha 1 Village, Pokole Village, Balane Village, Binala Village, Tamako District, Sangihe Islands Regency. The data analysis technique is a way of managing data with Editing, Coding, and Data Entry. Research on the level of independence of the elderly in fulfilling ADL with the Barthel Index Method at the Elderly Posyandu, Tamako District, Sangihe Islands Regency shows that most of the elderly are in the independent category (not dependent). The results of this study can be used as information about the status of the level of independence of the elderly in fulfilling ADL in the Tamako Health Center Work Area, and it is hoped that they will be more active in improving the quality of services for the elderly so that this level of independence is maintained. The results of this study also add to the literature and insight for readers, especially in gerontic nursing, making it easier for readers to be used as additional references that can be used to improve knowledge.

Goals : This study aims to determine the description of the nutritional status of the elderly in the Nurse Study Program area of STIKes Santa Elisabeth Medan Method : The present study is descriptif to asses the nutritional status of the elderly Gerontik Ners Nursing Area STIKes Santa Elisabeth Medan using Body Mass Index (BMI) by measuring body weight and height in 30 elderly people. Results: The results showed that the majority of elderly people in the Elderly category (60- 74) 56.4%, the elderly who had gout acid comorbidities were 63.3% and over nutrition status 53.4%. Conculsion : From the results of the study it was found that the majority of the elderly experienced over nutrition 53.4%, based on the study suggested that the Gerontik Nursing group could provide optimum for the elderly. optimum nutrition for the elderly.

The purpose of this study was to determine nursing care in patients with congestive heart failure. This study used a qualitative research method with a case study design using the nursing process approach. Sampling uses purposive sampling technique. The sample is Ny. S is 69 years old in a gerontik ward at the RSJ. Prof. Dr. Soerojo Magelang. The results of the study were found Ny. After three days of nursing care, the phlegm and shortness of breath are reduced. Respiratory Rate is 20 times / minute, the pulse is 84 times / minute, and no rhythmic sounds are heard. Conclusion of the problem of primary nursing care in Mrs. S with congestive heart failure is the ineffectiveness of airway clearance.ABSTRAKGagal Jantung Kongestif merupakan kondisi dimana jantung mengalami kegagalan memompa aliran darah yang berguna untuk mencukupi kebutuhan metabolisme sel-sel di dalam tubuh. Penelitian ini menggunakan metode penelitian kualitatif dengan rancangan studi kasus dengan pendekatan proses keperawatan. Pengambilan sampling menggunakan teknik purposive sampling. Sampelnya adalah Ny. S umur 69 tahun di bangsal gerontik di RSJ. Prof. Dr. Soerojo Magelang. Hasil penelitian ditemukan Ny. S setelah dilakukan asuhan keperawatan selama tiga hari betuk berdahak dan sesak napas berkurang Respiratory Rate 20 kali/menit, nadi 84 kali/menit, dan tidak terdengar suara ronchi. Kesimpulan masalah asuhan keperawatan utama pada Ny S dengan gagal jantung kongestif yaitu ketidakefektifan bersihan jalan napas.

Foram objetivos deste estudo: identificar as características do ensino da enfermagem geronto-geriátrica; refletir sobre este ensino, partindo das características identificadas, à luz da Complexidade. Pesquisa qualitativa, teve como fonte de dados nove livros-resumo de congressos brasileiros de enfermagem e sete periódicos da enfermagem nacional, do período de 1991 a 2000. O corpus foi composto por 16 artigos científicos e utilizou-se o QRSNUD* IST4 para análise. Os resultados mostraram a existência deste ensino, por meio de: matérias no currículo; atividades práticas; atividades durante estágio curricular; atividades extracurriculares; participação em eventos específicos ou da enfermagem, com apresentação de trabalhos; outras atividades. Refletir sobre o ensino da enfermagem geronto-geriátrica, por meio da Complexidade, é perceber o ensino enquanto instância que procure transmitir uma cultura que permita ao futuro trabalhador: compreender a condição humana; pensar de forma contextualizada, aberta, globalizada, ética.

Abstract Ongoing evidence of failures to provide cardiopulmonary resuscitation in skilled nursing facilities (SNFs) has resulted in the federal regulation - F678 Cardio-Pulmonary Resuscitation (CPR). Descriptions of CPR-related non-compliance and nursing practice failures are contained in Statements of Deficiencies (CMS-2567). These data provide a unique opportunity to describe practice failures at the point-of-care. A mixed methods case study using content analysis and descriptive statistics was used for a purposeful sample of SODs from 11 SNFs in six states derived from a 2012 first quarter national CMS report of 42 avoidable deaths associated with immediate jeopardy citations. A codebook was developed and tested, based on empirical evidence reported in Office of Inspector General (OIG) 2014 Adverse Events and the Institute of Medicine 2004 nursing surveillance framework. Two trained and independent coders analyzed data. Analysis of SOD quality was conducted. Patterns of practice failures were identified. Ownership included 3 not-for-profit; 1 governmental; and 7 for-profit facilities. The 2012-star ratings ranged from 1.0 – 2.8. The practices of 5 RNs and 5 DONs were described. OIG categories included abuse and neglect, care transitions, and medications. Practice failures were associated with inadequate initiation of CPR resulting from improper processing of orders, poor identification of resident status, poor RN and DON surveillance, absence of CPR certified staff, and a lack of urgency in nursing’s response. The quality of SODs, based on 5 parameters, ranged from 18% - 100%. SODs are useful as data sources. Identified practice failures are useful in developing best practice protocols.

Abstract Revisiting the Teaching Nursing Home is a two-year pilot project to address the long-term care workforce shortage by introducing nursing students to geriatric nursing while also improving quality of care within nursing homes. The initiative has multiple components: enhanced clinical rotations for nursing students with partner schools of nursing, implementation of the Institute for Healthcare Improvement Age-Friendly Health System “4M” quality improvement model, and an online learning network. Undergraduate and graduate nursing students at three Schools of Nursing participated in clinical experiences at regional nursing homes. Students completed an “activity feedback” form after each clinical rotation at the nursing home or related activity, such as a session about the 4Ms or quality improvement/assessment. The activity feedback form asked students to share their most important takeaway and suggestions for improvement. Data from 340 feedback forms was coded qualitatively using conventional and directed content analysis using the American Association of Colleges of Nursing (AACN) Essentials for Professional Nursing Education. Multiple coders and audit trials were used to establish rigor. Students’ takeaways encompassed 7 of 8 key concepts in the AACN Essentials; Knowledge for Nursing Practice, Person-Centered Care, and Interprofessional Partnerships were most frequently mentioned. Students provided numerous suggestions for improving their clinical experiences including facilitated learning from instructors and supported engagement with nursing home staff. In conclusion, the program addressed many of the core competencies designated by AACN. One recommendation that flows from these findings is to enhance the role of clinical preceptors in the nursing home setting to facilitate mentored training.

Abstract Relocation to a nursing home is often assumed to be associated with stress for older adults. This study aimed to explore how stress affect psychological adjustment of nursing home residents. A cross-sectional survey was conducted. A sample of 386 nursing home residents was recruited from 11 nursing homes in Fujian Province, Southeast China. The Nursing Home Adjustment Scale, Perceived Stress Scale, Resourceful Scale, and General Self-Efficacy Scale were adopted to collect data. The path analysis was used to analyze the relationship of stress, nursing home adjustment, resourcefulness, and self-efficacy. The results indicated that stress directly caused poor nursing home adjustment. It also indirectly affected nursing home adjustment through the mediators of resourcefulness and self-efficacy, respectively. Furthermore, chained mediation was found from stress to nursing home adjustment through resourcefulness, and then through self-efficacy to cope with the negative affect. The current study contributes to the understanding of the mechanism of stress on nursing home adjustment in older adults. To improve their psychological adjustment, additional focus should be placed on enhancing resourcefulness and self-efficacy in nursing home residents.

Abstract The Covid-19 pandemic has brought terrible difficulties to nursing homes, as they were locations with the highest number of confirmed Covid-19 cases and deaths in the US. The Centers for Medicare & Medicaid Services (CMS) applies the Five-Star Quality Ratings to indicate the quality of care in nursing homes based on health inspection survey, staffing, and resident outcome. Studies to date have contradictory findings regarding the relationship between nursing home reported Quality Ratings and Covid-19 cases and deaths based on US regional data. The purpose of this study is to examine whether nursing homes’ Quality Ratings were related to the total number of resident Covid-19 cases and deaths at the US National level. The study examined US nursing homes (N=13,494) linked with CMS Nursing Home Compare data and Covid-19 Nursing Home data. Using multiple linear regression analyses, results showed nursing home Quality Ratings were significantly associated with Covid-19 residents’ cases and deaths controlling for ownership type, size, occupation rate, and years of operation (p&lt;.001; p&lt;.001). Five-star nursing homes were less likely to have Covid-19 cases and deaths. Further, comparing lower Star Ratings nursing homes, 1-Star nursing homes showed no significant difference to 2-Star and 3-Star nursing homes when examining Covid-19 cases and deaths. Overall, the Five-Star Quality Ratings is a useful measure when investigating nursing homes’ performance during the Covid-19 pandemic. Future policymakers and administrators should also focus on nursing homes with lower star ratings when improving the quality of nursing homes, particularly with regard to resident health.

Abstract Purpose: This study aimed to investigate the levels and correlations of role conflict, nursing professionalism, nursing work environment and retention intention, and the factors influencing retention intention of nurses at long-term care hospitals. Methods: The subjects included 183 nurses at nine long-term care hospitals in one metropolitan city in Korea. A set of self-reported questionnaires was administered to assess general characteristics, role conflict, nursing professionalism, nursing work environment, and retention intention of the subjects. Collected data was analyzed using descriptive statistics, t-tests, one-way ANOVA, Pearson correlation coefficients, and multiple linear regression. Results: 183 subjects with a mean (±SD) age of 41.66 (±12.29) years were included in the final analyses. Retention intention had a significant positive correlation with nursing professionalism (r=.39, p&lt;.001) and nursing work environment (r=.51, p&lt;.001). Nursing work environment had a significant negative correlation with role conflict (r=-.30, p&lt;.001) and a significant positive correlation with nursing professionalism (r=.48, p&lt;.001). In the final multiple regression analysis, the factors influencing retention intention of subjects were number of beds (β=-.15 p&lt;.026), nursing professionalism (β=.19, p=.007) and nursing work environment (β=.36, p&lt;.001). The explanatory power of number of beds, nursing professionalism and nursing work environment on retention intention was 34.0% (F=16.66, p&lt;.001). Conclusion: Improving nursing professionalism and nursing work environment of nurses at long-term care hospitals will ultimately enhance their retention intention and positively impact on the quality of gerontological nursing service.

Abstract The purpose of this pilot project was to explore the experience of an intergenerational learning environment focused on healthy aging for nursing students and older adults. Intergenerational learning experiences provide opportunities for individuals from different age groups to communicate and participate in learning activities together. The growing population of older adults calls for increased geriatric nursing expertise. Nursing students’ attitudes toward older adults are often negative though, and result in decreased interest in geriatric nursing. The opportunity to transform nursing students’ perspectives on older adults has the potential to improve nursing care for older adults, and the number of nurses focused on geriatric nursing care. This qualitative inquiry used a convenience sample of 10 participants from a cross-listed university course on healthy aging for baccalaureate nursing students and older adult members of a lifelong learning institute. Semi- structured focus group interviews were conducted. Narrative transcripts were analyzed using an inductive approach. Analysis illustrated improved nursing students’ perspectives of older adults and aging. A similar theme was noted for older adults’ perspectives of younger adults. The importance of social interaction within an intergenerational learning environment and the need for opportunities to challenge ageist perspectives was illustrated. Increased exposure to healthy older adults, personally and professionally, may increase nursing students’ interest in geriatric nursing and improve nursing care for older adults. Future research should examine more specifically how intergenerational learning experiences can decrease ageism, improve nursing students’ and nurses’ perspectives on older adults, and improve nursing practice for older adults.

Abstract Nursing Homes experienced a colossal loss impacted by the Covid-19 pandemic in the United States. This study applied Covid-19 Nursing Home Dataset (updated on 08/16/2020) released by Data.CMS.gov to explore possible factors behind the death s at nursing homes. The results indicated 2.55 residents died per week at a nursing home averagely. Besides, the absence of nursing staff, aides, clinical physicians, PPE supplies contributes to more deaths at nursing homes. Lastly, the number of positive COVID-19 cases of nursing home staff positively associate with the number of total deaths of residents(R=0.65). These findings provide more pieces of evidence for nursing home administrators and policymakers to make adjustments to help nursing home residents better cope with challenges caused by the pandemic; however, this dataset is not the final data for the pandmic is not over. Also, the dataset covers few demographic information (gender, race, ethinicty and so on) ; therefore, researchers could explore the relationship between the demographic features and COVID-19 deaths at nursing homes.

At the beginning and end of the spring 2022 semester, students rated their competence in: patient assessment, collaborating with the care team, gathering clinical information, medication review, eliciting resident values, and health promotion. Students also rated their preferences for working in long-term care and with older adults. Data from 85 responses at the start of semester and 64 responses to the end of semester survey were analyzed. Analysis of student responses found that students self-rated competencies improved in all areas except eliciting resident values. Prior to their clinical experience, students ranked working in long-term care and with older adults lower than other settings or populations. The rankings were unchanged after their clinical experiences. These initial findings suggest that although the Teaching Nursing Home Program is meeting the pedagogical goals, attitudinal shifts may require different strategies.

Abstract Racial/ethnic disparities in healthcare have been highlighted by the recent COVID-19 pandemic. Minorities continue to utilize nursing home services at a higher rate than White residents, contributing to existing health inequity concerns. This study examined the relationship between nursing home racial/ethnic mix and COVID-19 resident mortality using the CMS Nursing Home COVID-19 Public File. As of October 25, 2020, high minority nursing homes reported 6.5 COVID-19 deaths as compared to 2.6 deaths for nursing homes that had no racial/ethnic minorities. Four nested sequential negative binomial regressions were used to model the relationship between racial/ethnic disparities in COVID-19 deaths and the separate contributions of facility-level resident characteristics (percent of females, percent of residents 65 years and older, percent of residents with congestive heart failure, hypertension, and obesity, and the average level of residents’ acuity), resource availability (nursing homes’ payer-mix, occupancy rate, county-level Social Deprivation Index, and nursing home location), and other organizational characteristics (nursing home for-profit status, chain affiliation, and self-reported nursing, clinical, aides, and other staff shortages). After controlling for interstate differences, facility-level resident characteristics, resource availability, and organizational characteristics, high-minority nursing homes had 61% more COVID-19 deaths (Incidence Rate Ratio [IRR] = 1.61; p &lt; 0.001) as compared to nursing facilities with no minorities. From a policy perspective, nursing homes, that serve primarily minority populations, may need additional resources, such as, funding for staffing and equipment in the face of the pandemic. The COVID-19 pandemic has sharpened the focus on healthcare disparities and societal inequalities in the long-term care.

Abstract This quasi-experimental study aimed to evaluate the effectiveness of special nursing units in nursing homes by comparing health outcomes and satisfaction between the general nursing units and special nursing units across nursing homes in South Korea. Surveys—paper, phone, and online—were conducted five times (March, August, and November 2019; June and October 2020) for health outcomes (health pattern changes and the number of residents with new health problems) and four times (March and August–November 2019; June and October 2020) for satisfaction. Descriptive analysis, χ²-test, paired t-test, and McNemar test were performed using the SPSS 25.0 program. The results showed an improvement in the health outcomes of residents in the special nursing units. Regarding health pattern changes, there was a decrease in the number of residents facing problems related to consciousness, cardiovascular function, urination, defecation, and pain and of those with new health problems such as aches, falls, pressure sores, and urinary incontinence. Furthermore, caregivers’ satisfaction in the special nursing units was higher than that of their counterparts in the general nursing units. The results of this study will be used as evidence to expand this special nursing unit’s services in the future.

As the population ages and becomes more diverse over time, it is essential to identify mechanisms that may eliminate or mitigate racial/ethnic disparities. Culture change is a movement to transition nursing homes to more home-like environments. The literature on culture change initiatives and quality has been mixed, with little to no literature on the use of culture change initiatives in high Medicaid nursing homes and quality. The purpose of this study was to examine how the involvement of culture change initiatives among high Medicaid facilities was associated with nursing home quality. The study relied on both survey and secondary nursing home data for the years 2017-2018. The sample included high Medicaid (85% or higher) nursing homes. The outcome of interest was the overall nursing home star rating obtained from the Nursing Home Compare Five-Star Quality Rating System. The primary independent variable of interest was the years of involvement in culture change initiatives among nursing homes, which was obtained from the nursing home administrator survey. The final model consisted of an ordinal logistic regression with state-level fixed effects. High-Medicaid nursing homes with six or more years in culture change initiatives had higher odds of having a higher star rating, while facilities with one year or less had significantly lower odds of having a higher star rating. Culture change initiatives may require some time to effectively implement, but these initiatives are potential mechanisms to improve quality in high Medicaid nursing homes.

Abstract This study aimed to elicit psychosocial reactions to relocation to nursing homes from older adults’ perspectives with a qualitative interview design. Narratives from 23 Chinese nursing home residents from Fuzhou, China in a life review program were recorded, transcribed into sentences, and analyzed with the qualitative content analysis. It revealed five stages of psychosocial reactions to relocation to nursing homes as fear, struggle, compromise, acceptance, and contribution. The first stage resulted from negative labels attached to nursing homes, disconnection to the society, difficulties in establishing new relationships, and being abandoned by their families. The second stage described the behaviors of struggle: complain about family members, think of going back home, pray to have a change, and take action to leave. The third stage described the keys to compromise: choices between maintaining the harmony in family relation and companionship of relatives, choices between professional care and family care, and choices between costs and effects of family care and nursing home care. The fourth stage described how they accept nursing home life: accept the life and yet with worries, affirm benefits of living in nursing homes, and embrace the nursing home life. The last stage resulted from sense of ownership and giving full play to self-worth. This study generated new insights into the knowledge on psychosocial reactions to relocation to nursing homes and provided both family members and nursing home staff with a direction for how to promote a smoother relocation process.

Abstract The impact of COVID-19 has been greatest in vulnerable US populations. This study examines the cumulative geographical and racial disparities of COVID-19 cases in nursing homes. Analysis of COVID-19 Nursing Home Data from Centers for Medicare & Medicaid Services was limited to weekly reports from the nursing homes that reported the ratio of black residents, from 2020-05-31 to 2021-01-17 (N=268,222 from 8,026 nursing homes). The outcomes were weekly COVID-19 cases and death per 1,000 occupied beds. Nursing homes were categorized by a geographic (rural vs. urban) and racial composition (&gt;50% of residents are black vs. else). Elapsed time and county-level weekly COVID-19 cases and deaths/1,000 people were the key covariates. Multilevel zero-inflated negative binomial regression revealed evidence of cumulative COVID-19 disparity between rural and urban nursing homes. At the earliest time, COVID-19 incidence was lower in rural nursing homes than in urban nursing homes (IRR=0.406 for cases, 0.034 for death). The significant interaction with time implied that, over and above evolving disease prevalence, rural nursing homes became more likely than urban nursing homes to experience COVID-19 over time (IRR=1.057 for cases, 1.193 for death). Nursing homes, with &gt;50% black residents, were more likely to experience COVID-19 than their counterparts at the earliest time (IRR=1.339 for cases, 5.630 for death), but independent of local disease prevalence, this disparity decreased over time (IRR=0.973 for cases, 0.972 for death). Our findings suggest that racial and geographic factors contribute to the cumulation of disadvantage during the COVID-19 crisis at the second half of 2020.

Given the high percentage of aging population, large number of nursing homes, and staggering surge of COVID-19 cases in Florida, it’s critical to understand factors that may affect Florida nursing homes’ vulnerability to the COVID-19 pandemic. Using Nursing Home COVID-19 Dataset as of July 26, 2020 obtained through Centers for Medicare and Medicaid Services (CMS), and Provider Info Dataset and Health Deficiencies Dataset available through CMS Nursing Home Compare data, we constructed a database of Florida nursing facilities with confirmed COVID-19 cases and deaths, with corresponding facility characteristics and quality deficiencies. We examined the facility characteristics (e.g. facility size, ownership state, chain affiliation, staffing level) and quality deficiencies (e.g. infection control deficiencies) of Florida nursing homes with and without publicly reported COVID-19 cases and deaths. Results indicated that, as of July 26, 2020, 73.3% and 40.8% of Florida nursing homes had resident COVID-19 cases and death, respectively (N=701). Findings also suggested that Florida nursing homes of large facility size, chain affiliated, and for profit, were significantly more likely to have documented resident COVID-19 cases (p&lt;.05). Larger facility size (120 beds or more), staff shortage, and having prior infection control deficiency citation, were significantly related to the odds of having resident COVID-19 deaths (p&lt;.05). Policy and practice implications and future research directions will be addressed to better protect the at-risk nursing home residents.

Abstract Nursing homes that fostered open communication and teamwork were more likely to change their practices and adopt the “It’s Not OK to Fall” program. This pilot study evaluated the feasibility and acceptance of a team-building approach for developing leadership skills to three groups of coworkers: Administrators/Directors of Nursing, charge nurses, and certified nursing assistants (CNA) employed by nursing homes in Oklahoma. Each coworker group received one day of job specific leadership training, with another one half-day session where all levels engaged in team-building exercises. Participant satisfaction with course content ranged from agree-to-strongly agree. All stated that they could apply the leadership strategies at their facility. Administrators/Directors of Nursing found tools for tracking turnover/retention and strategies for improving staff communication helpful. Charge nurses and certified nursing assistants seldom viewed themselves as leaders, found coworker group communication very fragmented, and felt least knowledgeable about nursing home care best practices.

Abstract Nursing homes must comply with numerous federal/state regulations to receive Medicare and Medicaid funding. Failure to comply with these regulations can result in deficiency citations, and depending on the severity of the deficiency, a resulting Civil Monetary Penalty (CMP). Through the Centers for Medicare and Medicaid Services (CMS) Civil Monetary Penalty Reinvestment Program, CMP funds are reinvested to support activities that benefit nursing home residents and that protect or improve their quality of life or quality of care. This symposium presents some of the unique challenges, successes, failures, and surprise findings from CMP-funded nursing home quality improvement projects in two, predominantly rural Midwestern states: Oklahoma and Kansas. Dr. Williams presents findings of a pilot-study testing an adaptation of a successful family caregiver telehealth support intervention in the nursing home setting and implications for future research. Dr. Sturdevant shares successes, challenges, and unanticipated results from the “It’s Not OK to Fall” project, a comprehensive, 3 year fall prevention project implemented in Oklahoma nursing homes. Lastly, Ms. Round’s paper describes the implementation and findings of a Long-term Care Leadership Academy aimed at improving leadership and team building skills of three levels of nursing home staff, including Administrators/Directors’ of Nursing, RN/LPN charge nurses and certified nursing assistants. Abstract This study examined the effect of registered nurse (RN) staffing level, work environment, and education on adverse events experienced by residents in nursing homes. A cross-sectional study was conducted with 216 RNs working in nursing homes who were selected using random stratified sampling by location and bed size. Self-reported questionnaires regarding staffing level, work environment, education level, adverse events, and nurse characteristics were administered. Data from the National Health Insurance Service were used to describe nursing home characteristics. Both multiple and multinomial logistic regressions were used to control for the characteristics of nurses and nursing homes, and investigate the effects of nursing staffing level (number of older adults assigned to a nurse), work environment (Practice Environment Scale of the Nursing Working Index), and level of nursing education on the adverse events experienced by residents. An increase of one resident per RN was significantly associated with a higher incidence of pressure ulcers (OR= 1.019, 95% CI=1.004-1.035). Poor work environment increased the incidence of adverse events such as pressure ulcers (OR= 3.732, 95% CI=1.155-12.056) and sepsis (OR=3.871, 95%CI=1.086-13.800). Compared to RNs with a baccalaureate or higher, RNs with diplomas reported increased incidence rates of pressure ulcers (OR=2.772, 95%CI= 1.173-6.549). RN staffing, work environment, and education level affect the incidence of pressure ulcers, and the work environment affects the incidence of sepsis among residents in nursing homes. Policy-wise, improving the level of nurse staffing, nursing work environment, and nursing education will improve health outcomes of residents.

Abstract Background: Nursing homes were impacted disproportionately by the coronavirus because of their resident’s vulnerabilities and settings. Even many previous studies illustrated factors related to nursing home residents’ Covid-19 infections, there’s no such study epitomizing those factors systematically, while some factors were controversial in different studies. The article aims to summarize major types of factors and provide crucially influential implications for nursing homes to prevent and manage their resident infections. Methods: All articles published between 01 January 2020 - 15 January 2021 in English version were searched through three electronic databases (PubMed, Web of Science, and Scopus). Two authors screened and evaluated a total of 121 studies independently based on selection and extraction criteria. Results: Seventeen identified studies were included in the research, which involved five major types of factors (nursing home’s residence, nursing home, staff, resident, and others). Conclusion: nursing home’s county infection rate, size, and staff residence were the strongest significant factors in many studies. Per-capital income, symptom-based screening and testing, and asymptomatic individuals have impacted resident’s infections variously since the beginning of the pandemic. Nursing home’s star rating and a total count of fines became factors when considered its locations. Other factors, including nursing home’s type, historical health deficiencies, staffing level, and staff working different facilities, etc., were also significant factors. The value of factors suggests healthcare systems reflect appropriate measures and allocate more resources to nursing homes in high prevalence counties on the basis of universal allocation.

Abstract In the early months of the pandemic, SARS-CoV-2 infected nursing home residents in explosive and deadly outbreaks. Nursing home residents disproportionately accounted for over 40% of COVID-19 mortality nationally. This national emergency drove scientific and public health experts to develop and implement administrative, clinical, and research programs to limit the pandemic’s impact, especially for high-risk individuals, such as those hospitalized or living in nursing homes. Nursing home policies, prompted by the Centers for Medicare & Medicaid Services (CMS) and the Centers for Disease Control and Prevention (CDC) guidance, severely restricted access beginning in March 2020 in an effort to limit disease exposure. In July 2020 we began the process to conduct an investigational SARS-CoV-2 post exposure prophylaxis study of nursing home residents, incorporating FDA guidance developed for conducting investigational drug trials in the context of COVID-19. Our research teams adapted our nursing home engagement, resident consenting and research data collection strategies accordingly. We remotely screened residents living in any of 28 nursing homes for eligibility to participate, ultimately consenting and randomizing individuals in 11 facilities. Of the 2,683 nursing home residents 65 years or older we screened, 48 (1.8%) agreed to consent individually or through proxy, most often a legally authorized representative. We will describe our research methods, with emphasis on how we addressed challenges presented due to performing all research tasks remotely and identify strategies that can qualitatively improve the remote nursing home research experience.

Abstract The COVID-19 pandemic has disproportionately affected nursing home residents and staff, including the nursing assistants who provide critical supports for older adults and people with disabilities. The pandemic has added significant strain to an already vulnerable nursing home workforce, which has historically experienced high levels of turnover, chronic staffing shortages, and high burnout. At the same time, it has generated awareness of the value of the direct care workforce to provide care in places for those most at risk from the disease. Job satisfaction of nursing assistants is as major driver of turnover and intent to leave the job. Research has demonstrated organizational supports and job stresses that are associated with job satisfaction and turnover. However, limited research has investigated the factors associated with job satisfaction in nursing homes during the COVID-19 pandemic. Using data from an employee engagement and management system, we examined organizational supports (e.g., supervision, appreciation, safety) and work-related stresses associated with job satisfaction among nursing assistants in nursing homes (n=402). Higher quality of supervision, feeling appreciated for the job, and feeling safe and comfortable at work were associated with higher levels of job satisfaction. Work-related job stressors –increased workload demands and understaffing – were associated with lower job satisfaction. Our findings provide insights into the importance of employer supports and reducing work-related stressors in nursing assistants’ job satisfaction and practical implications for nursing home leadership regarding how to support workers

Abstract For this qualitative, pilot study, seven Massachusetts nursing home Directors of Nursing (DONs) were interviewed remotely about palliative care provision before and during the COVID-19 pandemic. Interview data were analyzed using thematic analysis. Palliative care addresses physical, emotional, psychological, and spiritual suffering that accompanies serious illness. Symptom management and goals of care is especially valuable for seriously ill nursing home residents. We investigated the solutions nursing home staff developed to provide palliative care during the COVID-19 pandemic despite restrictions on external resources. Before the pandemic, palliative care was delivered primarily by nursing home staff depending on formal and informal consultations from palliative care specialists affiliated with hospice providers. When COVID-19 lockdowns precluded these consultations, nursing staff did their best to provide palliative care, but were often overwhelmed by shortfalls in resources, resident decline brought on by isolation and COVID-19 itself, and a sense that their expertise was lacking. Advance care planning conversations focused on hospitalization status and options for care given COVID-19 resource constraints. Nevertheless, nursing staff discovered previously untapped capacity to provide palliative care on-site as part of standard care, building trust of residents and families. Nursing staff rose to the palliative care challenge during the COVID-19 pandemic, albeit with great effort nursing home payment and quality standards should support development of in-house staff capacity to deliver palliative care while expanding access to the formal consultations and family involvement that were restricted by the pandemic.

Abstract This study explored the role of tested contextual factors (structural, market, and management) in high Medicaid (under resourced) nursing homes performance. Four nursing homes in geographically diverse states were purposefully selected for site visits based on high and low performance (quality/ profitability) indicators. Eight nursing home administrators and directors of nursing, and twenty-one nursing staff (RNs, LPNs, and CNAs) and providers of support services were interviewed. Data were analyzed using an inductive thematic approach with NVivo 12 Plus. Within and across case analysis was used to compare participants’ perspectives across nursing homes and across administrators and staff. Several themes provide insight into varied influences of contextual factors on these nursing homes’ performance: focus on quality care, team-based approach, community support and engagement, and staffing retention. Providing quality care to residents was strategic priority in all facilities, which was enhanced by an adopted team-based leadership approach, open-door policy and home-like atmosphere. Community reputation and availability of local training opportunities for CNAs affected nursing staffing which some facilities addressed using creative retention strategies. These research findings will facilitate interventions, such as leadership training and organizational development activities, aimed at improving the performance of low performing facilities in terms of lower costs and better quality.

Abstract The quality of care provided in nursing facilities has long been a societal concern and an important focus of policy at the federal and state level. The COVID-19 pandemic has drawn increased attention to nursing facilities due to high infection and mortality rates as well as increased media attention on staffing shortages that have occurred in the wake of the pandemic. The Office of the Assistant Secretary for Planning and Evaluation (ASPE) and RTI International have recently completed a series of studies focused on nursing facility quality and staffing. This symposium will present the results of four of those studies. First, we will report the results of a study exploring states’ use of value-based payment (VBP) programs as part of their nursing facility Medicaid payment systems. Second, we will present the results of a study examining inappropriate discharges from nursing facilities. Third, we will report findings from a study that examined the factors associated with nursing facility closures. Finally, we will present findings from a study that explored the effect of the pandemic on nursing facility staffing. Together these studies provide important information about the state of nursing facilities in the wake of the COVID-19 pandemic and suggest key areas of focus for policymakers.

Abstract In the midst of rapid transfers to online teaching for experiential learning opportunities in nursing clinical labs this past spring due to the pandemic, nursing simulations with immersive virtual reality (VR) in VR headsets were deemed impossible. In partnership with Embodied Labs, nursing faculty pivoted to facilitating VR using remote learning approaches in groups. In this new VR approach nursing students engaged in active learning, critical discourse, and reflection guided by faculty delivered VR scenarios remotely with in-session debriefing during discussion pause points. Complex scenarios focused on patient or family perspectives (e.g. during end-of-life care or navigating community and healthcare needs as a LGBTQ individual). These were valuable online learning opportunities for undergraduate nursing education. Student feedback was positive, and faculty perceptions indicated using VR remote learning offers rich, engaging discussion through complex topics important to nursing clinical practice.
