Advanced Genetic Counseling: Theory and Practice addresses educational objectives for second-year genetic counseling students. Building on mastery of genetics principles and baseline clinical skills, this comprehensive textbook begins with the history of the profession as it relates to current practice definitions and goals. Characteristics of clients and counselors that may affect the counseling relationship are presented to guide strategies for achieving positive client outcomes. Throughout the text, a psychotherapeutic counseling approach is advocated. Steps to establishing a therapeutic alliance are outlined and the elements of relational counseling emphasized. The psychological counseling theories presented will help counselors identify interventions to address client needs. Students are encouraged to pursue research to address gaps in evidence needed to guide practice. Health behavior and social psychology theories offer models to assess health perceptions and behaviors. The book concludes with a look toward the future of genetic counseling in the genomics era.

Abstract:
This chapter offers a sober analysis of the history and prospects of genetic counseling in psychiatry. The present intersection of genetics and psychiatry is complex and limited in scope. The argument is made that recent genomic discoveries are unlikely to revolutionize genetic counseling because the complex nature of psychiatric conditions are not reducible to models of prediction. Susceptibility testing for psychiatric disorders is not a stable platform for clinical psychiatry because tests based on “common variation” are clinically unhelpful. Nevertheless, there are expectations that genetic counseling for psychiatry will be an area of growth and potential. The rest of the chapter focuses on the practical work of individuals and experts as they manage their moral or professional obligations to communicate and understand genetic risk in psychiatry.

This chapter considers the challenges for clinicians involved in genetic counseling for Huntington’s disease, referring to the literature and best-practice guidelines. The issues involved in genetic testing and counseling around reproductive options are discussed. Ways of tailoring predictive test counseling to take better account of individuals’ experiences and knowledge are explored, and more complex predictive test counseling scenarios, such as requests from minors and individuals at 25% risk, are also covered. The process of assisting families near the time of diagnosis is explored, including ways of helping individuals in apparent denial of symptoms and facilitating communication among family members. Finally, this chapter reflects on lessons learned from our collective experiences and considers areas in which more work is needed.
