Abstract:
Abstract Objectives The majority of epidemiological studies show that there is an increased burden of somatic comorbidities among people with epilepsy (PWE) compared with the general population. We sought that in the subgroup of patients with satisfactory and stable seizure situation and healthy style of living, the general health and quality of life might be similar to people without epilepsy and investigated the long-term outcome and the prevalence of comorbidities and the social outcome of adult patients who had been continuously treated at our center for at least 25 years. Material and methods We consecutively collected our adult out-patients for 10 months and identified those patients who had been treated at our center for at least 25 years. Among this group we assessed demographic data, epilepsy syndrome, seizure situation, antiepileptic therapy, the number of previous AEDs, the socioeconomic situation and co-morbidities. Results Out of a total of 1672 patients, 14.4 % (n = 241) patients fulfilled our inclusion criterion. In 200 the files allowed an appropriate analysis of the data. Mean treatment duration in Kork was 36 years (25 – 52). 60% of patients were seizure-free for more than one year. 80% of the seizure-free patients did not complain of adverse events. Adverse events were more often among patients with ongoing seizures. Somnolence, gait disturbances and tremor were the leading symptoms. Better seizure outcome correlated with higher education, better professional education and lower unemployment rates. Diseases such as cardiovascular diseases or diabetes mellitus were not more frequent than in the general population. PWE do not have necessarily an impaired prognosis of their general health.

Abstract Introduction In two waves of data we collected in Brooklyn New York, we observed blacks were at high risk for obstructive sleep apnea (OSA). In the NIH-funded study ‘Metabolic Syndrome Outcome Study (MetSO), blacks enrolled from primary-care settings had a 59% risk of OSA. Similarly, blacks surveyed in churches and barbershops had a 43% risk of OSA. While these studies showed higher than expected risk as noted in the general population (29%), it remains uncertain how many of those blacks would be diagnosed with OSA in that population. The purpose of this study was to explore the rate of OSA using the WatchPat device in a community-based setting. Methods Data were collected from an NIH-funded study ‘Peer-Enhanced Education to Reduce Sleep Ethnic Disparities, designed to navigate blacks at risk of OSA to receive timely diagnosis and treatment using peer-delivered linguistically and culturally tailored sleep health education. Blacks were screened for OSA using the Apnea Risk Evaluation System (ARES) Questionnaire; a score ≥6 denoted moderate-high OSA risk. Individuals were asked to wear the WatchPAT 200 for one night during a week-long sleep assessment. WatchPat 200 measures SaO2 to determine respiratory-related arousals, defined as an Apnea-Hypopnea Index (AHI) ≥5, which is used to identify and diagnose OSA. We used SPSS 25.0 to perform logical regression analysis to assess associations between ARES and WatchPat AHI. Results A sample of 111 blacks provided valid ARES and WatchPat data for the present analyses. Of the sample, the mean age was 62.26 (SD=13.52 years; female = 55%); 49% reported annual income >20K and 79.5% reported a high school education. Moreover, 27% reported high blood pressure, 13%, diabetes, and 65% were overweight/obese. Multivariate-adjusted logical regression analyses indicated that blacks at risk for OSA were 66% more likely to receive an OSA diagnosis based on WatchPat AHI data (OR = 1.662, p < 0.01). The model adjusted for age, sex, income, and education. Conclusion The present study demonstrated that blacks at risk for OSA at the community level have a significant likelihood of receiving an OSA diagnosis using home-based recordings. Background: Readmissions within 30-days of hospital discharge are a problem. The aim was to determine if the Better Outcomes for Older Adults through Safe Transitions (BOOST) risk assessment tool was applicable within the UK. Methods: Patients over 65 readmitted were identified retrospectively via a casenote review. BOOST assessment was applied with 1 point for each risk factor. Results: 324 patients were readmitted (mean age 77 years) with a median of 7 days between discharge and readmission. The median BOOST score was 3 (IQR 2-4) with polypharmacy evident in 88% and prior hospitalisation in 70%. The tool correctly predicted 90% of readmissions using two or more risk factors and 99.1% if one risk factor was included. Conclusion: The BOOST assessment tool appears appropriate in predicting readmissions however further analysis is required to determine its precision.

Heterotetrameric adaptor protein complexes are important mediators of cargo protein sorting in clathrin-coated vesicles. The cell type–specific expression of alternate μ chains creates distinct forms of AP-1 with altered cargo sorting, but how these subunits confer differential function is unclear. Whereas some studies suggest the μ subunits specify localization to different cellular compartments, others find that the two forms of AP-1 are present in the same vesicle but recognize different cargo. Yeast have two forms of AP-1, which differ only in the μ chain. Here we show that the variant μ chain Apm2 confers distinct cargo-sorting functions. Loss of Apm2, but not of Apm1, increases cell surface levels of the v-SNARE Snc1. However, Apm2 is unable to replace Apm1 in sorting Chs3, which requires a dileucine motif recognized by the γ/σ subunits common to both complexes. Apm2 and Apm1 colocalize at Golgi/early endosomes, suggesting that they do not associate with distinct compartments. We identified a novel, conserved regulatory protein that is required for Apm2-dependent sorting events. Mil1 is a predicted lipase that binds Apm2 but not Apm1 and contributes to its membrane recruitment. Interactions with specific regulatory factors may provide a general mechanism to diversify the functional repertoire of clathrin adaptor complexes.

When precursors decompose or react in the presence of a solvent in a closed system at a temperature higher than the solvent’s boiling point the reaction is called a solvothermal process. This reaction can be carried out either in supercritical or subcritical conditions, in homogeneous or heterogeneous systems, pressure and temperature being both key parameters. As the main interest of such processes is the enhancement of chemical reactivity, solvothermal reactions have been widely involved for nitride elaboration. We report two examples relative to solvothermal syntheses of nitrides. The first one deals with the elaboration of Li3GaN2: this ionic nitride has been successfully synthesized, structurally characterized and tested as nutrient for the ammonothermal GaN crystal growth. The second one is related to the elaboration of a well-crystallized graphitic carbon nitride (g-C3N4) aimed to be developed as a precursor for conversion towards dense CNx phases.

Numerous core-fluorinated perylene bisimide (PBI) dyes with various substituents at the imide positions have been synthesized by different methods. Core-difluorinated PBIs 4a-f are obtained by imidization of difluoro-substituted perylene bisanhydride 1 with appropriate primary amines or, alternatively, by nucleophilic halogen exchange reactions (Halex process) of the corresponding dibromosubstituted PBIs 2a-d,f with potassium fluoride. Core-tetrafluorinated PBIs 5a-c could also be synthesized by halogen exchange reactions of the respective tetrachlorinated PBIs 3a-c. In particular, core-fluorinated perylene bisimide pigments 4h, 5h containing hydrogen atoms in the imide positions could be obtained for the first time by deprotection of α-methylbenzyl-substituted precursors. Compared with core-unsubstituted perylene bisimides, these fluorinated dyes display hypsochromically shifted absorption and fluorescence spectra, and they exhibit fluorescence quantum yields up to unity, enabling bright yellow emission. The electrochemical properties of these electron-poor perylene bisimides have been studied. Furthermore, the packing features of a tetrafluorinated PBI derivative in the solid state have been discussed.

Reaction of lithium(neomenthylcyclopentadienid) 3 with dimethyldichlorosilane gives dimethylbis[3-(neomenthyl)cyclopentadienyl]silane 4 regioselectively as a mixture of double bond shift isomers (57% isolated). Deprotonation with 2 equiv. of butyllithium followed by treatment with zirconiumtetrachloride furnished a mixture of the three {dimethylsilylenbis[η5-3-(neomenthyl)cyclopentadienyl][zirconium dichloride diastereomers from which the pure meso-like isomer 6e [p -R (1′S ,3 ′R ,4 ′R),p -S (1′S ,3 ′R ,4 ′R)] was isolated by fractional crystallization. Complex 6 c was characterized by an X-ray crystal structure analysis. Its D 1 - Zr - D 2 angle of 126.8° is very similar as observed in bis(η-cyclopentadienyl) zirconium dichloride (D 1 and D 2 denote the centroids of the cyclopentadienyl rings). The homogeneous 6c/methylalumoxane Ziegler-type catalyst produces polypropylene of low molecular weight (Mη ≈ 670 at ambient temperature). Related catalyst systems may find interesting applications in organic synthesis.

From S,S-diorganosulfodiimides 1a-e with diphenylcyanimidocarbonate (2) via condensation products 3a-e and 6a-d novel heterocycles were obtained. In DMF 3a-c react with aliphatic amines to form 1λ4,2,4,6-thiatriazines 4a-e and 5a, b. With HCl 6a-d can be cyclized to give 1λ6,2,4,6-thiatriazine hydrochlorides 8a-d. These are thermally converted into 1λ4,2,4,6-thiatriazines 4a, f, g. Condensation of 6a with orthoesters and paraformaldehyde affords 1λ6,2,4,6- thiatriazines 9a, b and 10a, whereas 3a, e undergoes ring closure with methyl hydrazine to 1,2,4- triazoles 11a, e.

The complexation reaction between metal cations of different valence states (Al, Bi(III), Cr(III), Fe(III); Ca, Co(II), Cu(II), Mn(II), Ni(II), Pb(II), Zn; Na) and fulvic acids is investigated by the streaming current detection technique. It can be shown that the metal complexation capacity does not only depend on the nature of the cation but also on the origin of fulvic acids, which were isolated from anaerobic and aerobic landfill leachates.

Abstract:
Light trapping effect using rough surface transparent conductive oxide (TCO) is one of the best ways to achieve high efficiency thin-film silicon solar cells. Several types of rough ZnO film fabricated by metal organic chemical vapor deposition technique onto the glass, which are etched by reactive ion etching, have been proposed so far as promising TCO substrates. In this paper, newly developed ZnO substrate with extremely high light scattering property comparing with typical pyramidal texture one was developed. By applying this newly developed ZnO substrate to the solar cell, higher short circuit current of about 2% has been achieved comparing with typical pyramidal texture one without sacrificing other parameters. This result showed that the newly developed substrate is suitable as a front TCO substrate for high performance thin-film silicon solar cell.

Two dimers of cholic acid were synthesized through simple covalent linkers. The dimers form invertible molecular pockets in media of different polarity; hydrophobic pockets are formed in water and hydrophilic pockets are formed in organic media. Fluorescence studies show that pockets formed by these dimers can serve as invertible hosts for the hydrophobic guest pyrene and the hydrophilic guest coumarin 343. The molecular pocket also enhances dissolution of the weakly soluble cresol red sodium salt in organic media. Molecular modeling was performed to better understand the host–guest complexation process of the invertible amphiphilic pockets. The calculated free energy changes indicate that the two dimers form the most stable complexes with coumarin 343 at a host to guest ratio of 2:2, whereas the host to guest ratio differs in the formation of complexes with pyrene for the two dimers. The dimer with the shorter, less flexible linker seems to form host–guest complexes that are more stable in both water and organic solvents.

Herein, we report the catalytic activity of nebivolol nanoparticles a novel organocatalyst for the synthesis of DHPMs and DHPM-5-carboxamides. The nanoparticles are confirmed by DSC, TEM, AFM, and IR spectroscopy. The catalyst can be readily recovered and reused for the next four runs without any significant impact on the yields of the products. The products are fully characterized by FTIR, 1H NMR, 13C NMR, and distortionless enhanced polarization transfer (DEPT) NMR. The methodology adopted here offers several advantages such as solvent-free reaction, low loading of catalyst, short reaction times, and quantifiable yields.

We aimed to estimate the measurement uncertainty of some β-lactam antibiotics (β-LA), and to evaluate the impact of reporting the measurement uncertainty on clinicians’ decisions while guiding antibiotic therapy.MethodsMeasurement uncertainty of β-LA (aztreonam [ATM], cefepime [FEP], ceftazidime [CAZ], and piperacillin [PIP]) values, obtained by an UHPLC-MS/MS based-method, was estimated using the top-down approach called the single laboratory validation approach (EUROLAB guidelines). Main uncertainty sources considered were related to calibrators’ assigned values, the intermediate precision, and the bias. As part of an institutional program, patients with osteoarticular infections are treated with β-LA in continuous infusion and monitored to assure values at least 4 times over the minimal inhibitory concentration (4×MIC). We retrospectively evaluated the impact of two scenarios of laboratory reports on clinicians’ expected decisions while monitoring the treatment: reports containing only the β-LA values, or including the β-LA coverage intervals (β-LA values and their expanded measurement uncertainties).ResultsThe relative expanded uncertainties for ATM, FEP, CAZ and PIP were lower than 26.7%, 26.4%, 28.8%, and 25.5%, respectively. Reporting the measurement uncertainty, we identified that clinicians may modify their decision especially in cases where 4×MIC values were within the β-LA coverage intervals.ConclusionsThis study provides a simple method to estimate the measurement uncertainty of β-LA values that can be easily applied in clinical laboratories. Further studies should confirm the potential impact of reporting measurement uncertainty on clinicians’ decision-making while guiding antibiotic therapy.

The distal pigment cells of Orconectes limosus contain two layers of large electron lucent vacuoles that are separated by layers of small right-angled platelets adjacent to the crystalline cones. The crystalline cones of the dioptric apparatus of this species have evaginations into the distal pigment cell cytoplasm. In photoreceptors of Orconectes limosus and Procambarus clarkii a dark pigment accumulation site was detected just distal to the basement membrane at the edges of each retina. These pigment accumulations occurred independent of the state of light adaptation. Ultrastructurally the pigment granules at this accumulation site resemble distal screening pigment granules according to their size (up to 1.2 μm in diameter) and fibrous structure. Distal screening pigment granules were also found in tube-like cell processes or extracellularly within and proximal to the retinal basement membrane, indicating pigment transport to and across the basement membrane. Proximal to the basement membrane screening pigment granules were also observed disintegrated to a gravel-like electron dense material in widely branched cells. Evidence was found that an electron dense material, probably resulting from disintegrating screening pigment granules, was incorporated in the integument of the eyestalk. Four hours after injection of gold particles into the eye stalk distal to the retina they were detected inside and proximal to the retinal basement membrane.

Oligonucleotide-mediated mutagenesis has been used to change isoleucine 229 of the reaction center L subunit of Rhodopseudomonas capsulata to seventeen other amino acids. A system of deletion strains and complementing plasmids has been used to assay the effects of changing this residue, which forms part of the secondary quinone Oв binding site. The reaction center is assembled in all seventeen mutants. Ten of the seventeen mutations (Gly, His, Tyr, Trp, Asn, Asp. Gin, Glu, Arg, and Lys) result in the loss of photosynthetic growth. The remaining seven mutations yield varying levels of photosynthetic growth (Ile, Val, Ala, Leu, Met>Thr, Cys, Ser). Six of these mutations lead to varying atrazine resistance (Met, Leu > Ser, Ala, Thr, Cys>Val, He). None of the mutations lead to atrazine sensitivity. These results suggest that hydrophobic residues of moderate size function best at this position. This set of mutations at isoleucine L229 include the first examples of mutants with null quinone function and the first examples of crystal- lography-based genetic engineering in the design of herbicide resistant reaction centers.
