Abstract:
Fluid and circulatory management is an integral part of the peri-operative care of critically-ill patients. Precisely estimating the volumetric needs of post-operative patients remains difficult. While the majority of patients tolerate intra-operative fluid loss easily, patients with reduced physiological reserve present more of a challenge. Targeting specific physiological goals and optimizing haemodynamics with fluids and inotropes, means outcomes of these patients can be improved. This approach is often referred as goal-directed therapy (GDT). ‘Individualized goal-directed therapy’ can vary in timing, monitoring techniques, and endpoints used. The emergence of minimal invasive devises has allowed us to integrate cardiac output monitoring as a safe and reliable tool in the routine care of high risk patients. This dynamic assessment of haemodynamics provides a reliable technique to assess volume responsiveness and guide fluids to optimize cardiac output and oxygen delivery.

Chapter abstract The extent to which the concepts of field and social space are linked to a concrete mode of empirical research—and in particular to a set of original statistical tools—has seldom been acknowledged. This chapter aims to re-establish the close link between the field concept and geometric data analysis (GDA), Bourdieu’s preferred technique for mapping the “social distances” between individuals. The elective affinity between the two is based on a relation of tight interdependence: on the one hand, the emergent practice of GDA sustains and strengthens the “implicit philosophy” of the theory of fields; on the other hand, the method’s widespread use by Bourdieu and his collaborators has facilitated GDA’s international reception in the social sciences. The chapter concludes by discussing the empirical research program that results from wedding a sociology of fields with the systematic use of GDA.

The Les Houches Summer School 2015 covered the emerging fields of cavity optomechanics and quantum nanomechanics. Optomechanics is flourishing and its concepts and techniques are now applied to a wide range of topics. Modern quantum optomechanics was born in the late 70s in the framework of gravitational wave interferometry, initially focusing on the quantum limits of displacement measurements. Carlton Caves, Vladimir Braginsky, and others realized that the sensitivity of the anticipated large-scale gravitational-wave interferometers (GWI) was fundamentally limited by the quantum fluctuations of the measurement laser beam. After tremendous experimental progress, the sensitivity of the upcoming next generation of GWI will effectively be limited by quantum noise. In this way, quantum-optomechanical effects will directly affect the operation of what is arguably the world’s most impressive precision experiment. However, optomechanics has also gained a life of its own with a focus on the quantum aspects of moving mirrors. Laser light can be used to cool mechanical resonators well below the temperature of their environment. After proof-of-principle demonstrations of this cooling in 2006, a number of systems were used as the field gradually merged with its condensed matter cousin (nanomechanical systems) to try to reach the mechanical quantum ground state, eventually demonstrated in 2010 by pure cryogenic techniques and a year later by a combination of cryogenic and radiation-pressure cooling. The book covers all aspects—historical, theoretical, experimental—of the field, with its applications to quantum measurement, foundations of quantum mechanics and quantum information. Essential reading for any researcher in the field.

Gastrointestinal (GI) tumours can affect any part of the GI tract, and colorectal cancer is the most common. Throughout the GI tract, chronic inflammation seems to promote the development of neoplasia: for example, chronic reflux oesophagitis is linked to oesophageal adenocarcinoma; chronic Helicobacter pylori infection is linked to gastric cancer; chronic pancreatitis is linked to pancreatic cancer; cirrhosis is linked to hepatocellular cancer; chronic biliary inflammation is linked to cholangiocarcinoma; untreated coeliac disease is linked to intestinal lymphoma; and chronic inflammatory bowel disease is linked to colorectal cancer. Symptoms depend on the location of the tumour, and occur as a result of local anatomical disruption, with consequent functional consequences and, less frequently, as a result of hormonal, metabolic, and immune effects. Weight loss is a common symptom seen in the gastroenterology outpatient clinic, given the high overall incidence of GI tumours. Often, the associated symptoms will direct the doctor to the site of a possible underlying cancer. Anaemia is another non-specific finding with a strong association with luminal cancers. Patients with anaemia with or without weight loss will normally undergo upper and lower GI investigations, usually via oesophagogastroduodenoscopy and colonoscopy (either CT or endoscopic colonoscopy). In tumours that are difficult to identify or assess the malignant potential, PET scanning can provide a large amount of information. PET scanning is a nuclear medicine scanning technique that utilizes 18F-fluorodeoxyglucose (FDG), which is taken up by metabolically active tissue. When combined with CT scanning it can provide information about both anatomical and metabolic activity. FDG is rapidly taken up by malignant tumours and, as a result, is often used for diagnosing, staging, and monitoring response in cancers.

Abstract:
Radiology plays and important role in the evaluation of patients who have undergone a bariatric surgical procedure. This chapter discusses the imaging appearance of complications after the most common bariatric surgeries, Roux-en-Y gastric bypass, gastric banding, and sleeve gastrectomy. The fluoroscopic upper GI examination and computed tomography (CT) are the most commonly used imaging modalities for evaluating these patients. For each surgical procedure, fluoroscopic and/or CT images are used to illustrate the normal postoperative anatomy and to provide examples of common and uncommon complications seen in the early and late postoperative periods. Appropriate imaging technique is highlighted. A role for interventional radiology in the minimally invasive management of some of these complications is also briefly discussed.
