Abstract:
This chapter reviews a number of very rare chromosomal rearrangements: centromere fissions, complementary isochromosomes, telomeric fusions, balancing supernumerary chromosomes, neocentromeres, jumping translocations, and chromothripsis. Centromere fission results when a metacentric or submetacentric chromosome splits at the centromere, giving rise to two stable telocentric products. The Robertsonian fission reverses the fusion that had originally generated it. Telomeric fusion leads to a 45-chromosome count. With the balanced complementary isochromosome carrier, two stable exactly metacentric products are generated. A balancing small supernumerary marker chromosome contains material deleted from the normal homolog. A supernumerary chromosome lacking a normal centromere can become stable and functional due to the generation of a neocentromere. In jumping translocations, a segment can move from one chromosome to two or more recipient chromosomes. Chromothripsis takes complex rearrangement to a yet more complex level. The reproductive risks associated with each are noted.

Protons in the Sun’s core are a dense plasma allowing fusion events where two protons initially join to produce a deuteron. Eventually this leads to alpha particles, the mass-four nucleus of helium, releasing kinetic energy. Schrodinger’s equation allows particles to penetrate classically forbidden Coulomb barriers with small but important probabilities. The approximation known as Wentzel–Kramers–Brillouin (WKB) is used by Gamow to predict the rate of proton–proton fusion in the Sun, shown to be in agreement with measurements. A simplified formula is given for the power density due to fusion in the plasma constituting the Sun’s core. The properties of atomic nuclei are briefly summarized.

Abstract:
Local Fusions: Folk Music Experiments in Central Europe at the Millennium explores musical life in Hungary, Slovakia, and Austria between the end of the Cold War and the world financial crisis of 2008. It describes how artists made new social commentary and tried new ways of working together as the political and economic atmosphere changed. The book presents case studies from Budapest, Bratislava, and Vienna, drawing from ethnographic research and from conversations about the arts in Central European publications. The case studies illustrate how young musicians redefined a Central European history of elevating the arts by fusing poetry, local folk music, and other vernacular music with jazz, Asian music, art music, and electronic dance music. Their projects contradicted ethnic exclusions and gender asymmetries in Central Europe’s past expressive culture and in its present far-right political movements. The case studies demonstrate how musicians had to become skilled neoliberal actors, even as they asserted female power, broadened masculinities, and declared affinity with regional minorities such as the Romani (Gypsy) people. The author contrasts the live performances and physical recordings of world music 1.0 with the peer-to-peer networks of world music 2.0, arguing that Central European musicians occupy a liminal space between the two spheres. An epilogue describes how economic shocks of the late 2000s transformed sociality, creative processes, and the market for musical experiments in Central Europe.

Abstract:
Sacroiliac joint (SIJ) pain is the source of back pain in up to 25% of patients presenting with back pain. There is significant individual variation in the anatomy of the sacrum and the lumbosacral junction. SIJ pain is diagnosed with the history and physical examination. SIJ injection of a local anesthetic along with steroids is often used to confirm the diagnosis. Nonoperative treatment includes nonsteroidal anti-inflammatories, physical therapy, joint manipulation therapies, and SIJ injections. SIJ pain can also be successfully treated with radiofrequency ablation in some patients. Surgical treatment includes the open anterior sacroiliac joint fusion technique and minimally invasive techniques. The benefits of minimally invasive SIJ fusion versus open surgery include less blood loss, decreased surgical time, and shorter hospital stay. Further studies are needed to determine the long-term durability of the minimally invasive surgical techniques.
