Abstract:
El envejecimiento facial es un proceso que comienza de forma progresiva a los 25–30 años. Durante este existen cambios que se traducen en manifestaciones clínicas como pérdida de luminosidad, pérdida de la elasticidad, aumento de la laxitud, flacidez, presencia de arrugas, atrofia, capilares dilatados y sequedad de la piel. A nivel anatómico también existen cambios estructurales importantes a nivel de huesos, ligamentos faciales, músculos faciales, tejido adiposo facial y la piel en sus diferentes capas. En el siguiente trabajo se seleccionó una paciente con envejecimiento avanzado, para poder realizar un tratamiento full face que es un tratamiento anti-envejecimiento facial integral combinando conocimientos anatómicos y las tecnologías mínimamente invasivas disponibles para lograr los resultados más óptimos de rejuvenecimiento facial. Objetivo: El objetivo de este caso es atenuar signos de envejecimiento mediante el uso de técnicas combinadas y aplicación de conocimientos anatómicos en medicina estética sobre una paciente de 62 años, además de identificar los posibles efectos secundarios o complicaciones que se presenten. Método: Paciente de 62 años con grado de fotoenvejecimiento cutáneo avanzado y con múltiples manifestaciones de envejecimiento fisiológico sobre la cual se utilizaron técnicas combinadas de full face: neuromoduladoras, bioestimuladoras, rellenos dérmicos e hilos redensificadores de la piel. Resultados: Se definió un protocolo de tratamiento en 7 sesiones (1. Bioestimulación con peeling 2. Bioestimulación con plasma rico en plaquetas 3. Relleno de ácido hialurónico en región temporal y cigomática 4. Toxina botulínica en tercio superior 5. Relleno con ácido hialurónico en región perioral y región geniana, reborde mandibular posterior y ángulo mandibular 6. Hilos polidioxanona 7. Fotografías finales). Describiendo detalladamente en cada sesión el abordaje, cantidades, tipo de producto, técnicas utilizadas, efectos secundarios y finalmente los resultados de cada una de ellas. Conclusiones: Se realiza la evaluación final de los resultados obtenidos con 6 sesiones de tratamiento de full face, integrando los efectos conseguidos en cada una de ellas y relacionándolos con evidencia científica, principalmente con el concepto de “línea de los ligamentos” descrito recientemente en la literatura. A partir de los resultados obtenidos se concluye que el uso y combinación de técnicas no es una ciencia exacta, sin embargo lo que si es certero, es que todo procedimiento debe garantizar la seguridad del paciente y a la obtención de resultados naturales y duraderos, para lo cual el conocimiento de la anatomía relacionado con la edad es de vital importancia para la buena praxis de un médico estético.

Abstract:
We assessed clinical features as well as sensory and motor recoveries in 3 full-face transplantation patients. A frequency analysis was performed on facial surface electromyography data collected during 6 basic emotional expressions and 4 primary facial movements. Motor progress was assessed using the wavelet packet method by comparison against the mean results obtained from 10 healthy subjects. Analyses were conducted on 1 patient at approximately 1 year after face transplantation and at 2 years after transplantation in the remaining 2 patients. Motor recovery was observed following sensory recovery in all 3 patients; however, the 3 cases had different backgrounds and exhibited different degrees and rates of sensory and motor improvements after transplant. Wavelet packet energy was detected in all patients during emotional expressions and primary movements; however, there were fewer active channels during expressions in transplant patients compared to healthy individuals, and patterns of wavelet packet energy were different for each patient. Finally, high-frequency components were typically detected in patients during emotional expressions, but fewer channels demonstrated these high-frequency components in patients compared to healthy individuals. Our data suggest that the posttransplantation recovery of emotional facial expression requires neural plasticity.

Abstract:
In addition to the physical injury, deep burns of the face also can cause psychological injury. The purpose of this study was to develop new technical methods for improving engraftment of full-thickness autografts. Since 2017, autologous fibroblasts have been used for skin facioplasty of one full thickness autograft in Scientific Research Institue, Reginal Clinical Hospital No 1. Isolation of fibroblasts was performed by using an enzymatic method and a skin sample with an area of 10 cm2. Cultivation of autologous fibroblasts was performed for 22 days according to a standard procedure. The prepared culture of autologous fibroblasts was used together with a full-layer autologous transplant to close the wound on the face. We found that autologous fibroblasts provided faster adaptation of the autograft. There were no signs of fibrous tissue over the long term. We concluded that fibroblasts promoted rapid engraftment and adaptation of the full-thickness autograft. Improvement of facial plastic surgery methods can help achieve maximum cosmetic treatment results.

Abstract:
The aging process involves physical, social, and psychological changes. With the increase in life expectancy, the demand for aesthetic treatments is increasingly noticed in the search for harmonious and slower aging. This article reports a clinical case of a 42-year-old female patent complaining of onset of skin flaccidity and fall of the labial commissure, in additon to a tired and saddened appearance - known as “Sad Look”. A combination of common procedures in Orofacial Harmonization such as application of botulinium toxin, filling with plasma gel and filling with hyaluronic acid were used as a rejuvenation objective. The main results obtained were the reestablishment of the balance of muscular strength, the stimulation of collagen production and consequently the improvement of skin quality and lessen flaccidity, as well as the the return of volume and lost facial contours.
