Abstract:
The number of people with diabetes mellitus is increasing every year, even statistics show that in 2020 Indonesia is ranked 7th in the world. Many people still do not know how to understand diabetes mellitus and a healthy lifestyle, as well as the use of medicinal plants as antidiabetic. The use of herbal medicine has many advantages. One type of medicinal plant that is effective as a diabetes treatment therapy in the Indonesian Traditional Medicine Formulary (FROTI) is cinnamon. This activity has the aim of providing socialization and education to the public in the health sector about diabetes mellitus and anti-diabetic medicinal plants, especially cinnamon which is developed in dye bags. The implementation method is carried out virtually, considering the current state of the COVID-19 pandemic, with 3 stages, namely the planning stage, the implementation stage and the follow-up stage. The result of this PPM activity is the implementation of online seminars and education through infographic posters and videos and shows that the public is becoming more educated in their knowledge about diabetes treatment efforts by utilizing various types of medicinal plants and how to process them. The results of this activity can be concluded that the education and information activities carried out can increase public understanding and knowledge about diabetes mellitus, as well as understanding of various types of antidiabetic medicinal plants that can be utilized by the community .This is based on the results of the pre-test and post-test as a measure of the success of this PPM activity.

Abstract:
Background:Conflicting results have been published regarding the risk of infection with SARS-CoV-2 and development of severe COVID-19 among patients affected by rheumatic musculoskeletal diseases (RMDs). [1-4] Taking into account the lack of effective drugs to treat the COVID-19 and despite the burdensome and costly lockdown measures adopted to counteract the spread of SARS-CoV-2, effective and safe vaccines appear reasonably to be the best strategy for fighting the virus. [6] Before vaccines availability, several reports showed that a non-negligible proportion of subjects, among the general population or within specific categories, would have refused vaccination against COVID-19 once possible;[6, 7] data on vaccination hesitation among patients with RMD are not available yet.Objectives:This study aimed to evaluate the attitude of patients with RMDs to vaccination against SARS-CoV-2 and explore the factors which may influence it.Methods:During the first weeks of Europe vaccination campaign, we proposed an online survey to Italian adult patients with RMDs followed up in the Rheumatology Unit. All patients fulfilled the most recent classification criteria for each disease. HCs were recruited using a “best friend” system. The informed consent was collected for all participants. The questionnaires included the following items: demographic features, presence of comorbidities, educational level, and ongoing therapy. The individual’s perception of the COVID-19 vaccination, as well as the willingness to receive a COVID-19 vaccination with targeted questions was properly assessed.For the statistical analyses, Mann-Whitney and Chi-square tests were used. To account for baseline clinical differences among RMD-patients and controls, multivariable logistic regression analysis was used; covariates were selected according to a clinical criterion. The hypothesis that willingness for COVID-19 vaccine varied in specific subgroups of patients was tested using interaction terms at logistic regression analysis. All statistical tests were performed using the RStudio graphical interface and all tests were two-sided with a significance level set at p<0.05.Results:We provided an online survey to 830 adult RMD-patients and 370 healthy controls (HCs). Overall, 626 RMD-patients and 345 HCs completed the survey. Patients with RMDs were less willing to receive a COVID-19 vaccination compared to HCs (Odds Ratio (OR) 0.24, 95% CI 0.17 – 0.34, p<0.0001) despite they perceived themselves as at higher risk both to get infected (OR 11.3, 95% CI 8 – 15.9, p<0.0001) and develop a severe COVID-19 (OR 11.06, 95% CI 7.8 – 15.6, p<0.0001) and even if they had been vaccinated for influenza and pneumococcus more frequently than controls (OR 1.60 95% CI 1.18 – 2.16, p=0.002; OR 2.23, 95% CI 1.34 – 3.73, p=0.002). However, our results reveal that RMD-patients are more willing to change their minds if properly informed by the rheumatologist (OR 3.08, 95% CI 2.19 – 4.34, p<0.0001) in comparison to controls.Conclusion:The results of our study indicate for the first time that patients with RMDs are less willing to receive COVID-19 vaccination compared to the general population, despite perceiving themselves as at higher risk of getting infected with SARS-CoV-2 and develop severe COVID-19. However, our data underscored a meaningful aspect: patients with RMDs may change their attitude to COVID-19 vaccination if properly informed about risks and benefits by their trusted specialist.The results of this study encourage the entire rheumatologist community to become more committed to patient education, increasing their willingness to COVID-19 vaccine, which is the most promising strategy to protect them from the virus.References:1]Favalli EG et al. Abstract:
Background:Congenital heart block (CHB) is due to placental transfer of maternal anti-Ro/SSA autoantibodies to the fetus. The prevalence of CHB has been estimated as 1-2% in anti-Ro/SSA women while the recurrence rate is 16-19% (1). This condition is associated with a high rate of fetal/neonatal mortality and most of the cases requires pacemaker (PM) pacing. Given the rarity of CHB, limited data are available regarding the long-term follow-up of the offspring other than the cardiovascular complications.Objectives:The results of the Italian Registry of the autoimmune congenital heart block were recently described (2). A peculiarity of this cohort was that most of the mothers had an established diagnosis of systemic autoimmune disease at CHB detection, in contrast with other registries where CHB was mostly incidentally detected in healthy women. Here we report an update, with the preliminary data regarding the long-term outcome of patients with CHB, their unaffected siblings and health controls born from mothers positive for Ro/SSA.Methods:Data regarding demography, treatment, maternal, neonatal outcome, and follow-up were collected through an online electronic datasheet. A dedicated questionnaire was created with the aim to investigate general health, cardiovascular follow-up, and frequency of autoimmune diseases.Results:One-hundred and five cases of CHB in 99 patients were included from 1969 to December 2020. CHB was mostly detected in utero (97 cases, 92.3%) with 8 neonatal cases. Third degree CHB occurred in 71 cases (67.6%). Child mortality was observed in 29 (27.6%) cases: 20 in utero, 7 during neonatal period and 2 during childhood. Overall, a PM was implanted in 54 out of the 85 live births (63.5%). Then, our cohort was divided into 2 subgroups: pregnancy that occurred before (N=61) and after 2010 (N=44) with the aim to evaluate possible differences among the subgroups. Whereas mortality, PM, CHB degree were similar, CHB more frequently occurred in the last 10 years among Ro/SSA asymptomatic carriers than in the group of pregnancies before 2010 (53.6% vs 32.8%, p=0.038). Questionnaires from 14 surviving CHB cases, 8 unaffected siblings 12 controls born from mothers Ro/SSA positive were collected. Among CHB cases, 6 were males and 8 females, median age 12 years (range 6-28). All presented a third degree CHB, 10 required a neonatal PM pacing and one had an implantable ECG recorder. PM was substituted at least once in 9 patients, the oldest patient had to change it four times. No dilated cardiomyopathy occurred and most of the patients maintain an annual follow-up. Two cases of autoimmune diseases were registered among CHB cases, one idiopathic juvenile arthritis and one Cogan’s vasculitis, both born from mothers with Sjogren Syndrome. Four cases of neurodevelopmental disorders occurred: three cases of learning disabilities (one in each group) and one case of speech disorder in the sibling group. In addition, a CHB case presented a stress disorder linked to frequent hospitalizations.Conclusion:This registry is an ongoing project aiming at collecting all Italian CHB. Moreover, here we reported the preliminary data concerning the evaluation of long-term follow-up of CHB patients. Our data, even if need to be confirmed in larger cohort, seems reassuring: no differences were reported comparing CHB patients with unaffected siblings or controls.References:[1]Brito-Zéron et al. Abstract:
ObjectivePatients with thalassemia major do require lifetime blood transfusions that eventually result in iron accumulation in different organs. We described the usefulness of using magnetic resonance imaging (MRI) T2*imaging values for the evaluation of pancreatic iron load in these patients, and we correlated it with MRI T2* haemosiderosis of the myocardium and liver that has been recognized as a non-invasive assessment of iron overload among patients with thalassemia major.Materials and methodsWe conducted a cross-sectional study on 39 patients with thalassemia major in one of the tertiary university hospitals for a 1-year period. Demographic data were collected from the patient's history. MRI T2* of the pancreas, liver, and heart were executed on all patients in the same setting. Objective values of iron overload in these organs were obtained using the MRI post-processing software from online software.ResultsA total of 32 (82.1%) patients had pancreatic iron overload including 2 patients (5.1%) with severe iron overload and 15 patients (38.5%) with moderate and mild iron overload, respectively. Nine patients (23.1%) had myocardial iron overload, which included 3 patients (7.7%) who had severe cardiac haemosiderosis. Notably, 37 patients (94.9%) had liver iron overload, which included 15 patients (38.5%) who had severe liver haemosiderosis. There was a moderate positive correlation between the relaxation time of the pancreas and heart haemosiderosis (r = 0.504, P &lt; 0.001). No significant correlation was found between the relaxation time of the pancreas with the liver and the heart with the liver.ConclusionPancreatic haemosiderosis precedes cardiac haemosiderosis, which establishes a basis for initiating earlier iron chelation therapy to patients with thalassemia major.

Il crescente utilizzo di pagamenti online ha reso le istituzioni finanziarie e i loro prodotti un obiettivo appetibile per i criminali informatici. I cryptomarkets e i loro forum facilitano sia la vendita di dati finanziari personali e la frode delle istituzioni finanziarie e dei loro clienti. Allo stesso tempo, la frode relative alle carte di pagamento è un crimine che richiede l'acquisizione di un certo livello di abilità per essere perpetrato. Nel darkweb, molti forum si sono trasformati in piattaforme in cui gli utenti possono condividere conoscenze su come effettuare frodi online in modo efficiente. Il presente studio ha avuto come obbiettivo quello di testare la capacità della teoria dell'apprendimento sociale di spiegare la frode online. In particolare, ha cercato di analizzare il ruolo delle piattaforme online nel processo di apprendimento sociale. Per raggiungere questi obiettivi lo studio ha utilizzato tecniche quantitative e qualitative, includendo un'analisi del contenuto di un forum e un questionario online. I risultati hanno dimostrato che gli elementi della teoria dell'apprendimento sociale influenzano la probabilità di essere coinvolti nella frode online. In particolare, la presenza di definizioni favorevoli alla frode online e rinforzi differenziali sembra avere l'effetto più forte su questo tipo di crimine. A differenza dei legami sociali del mondo reale, le associazioni online e l'imitazione online dei modelli hanno influenzato la perpetrazione delle frodi online.
The increased use of online payments and banking have turned financial institutions and their products into cybercriminals’ main target. Cryptomarkets and their forums facilitate both the sale of personal financial data and fraud of financial institutions and their customers. At the same time, carding is a crime that requires the acquisition of a certain level of skills to be perpetrated. Many darknet forums have turned into platforms where users can exchange knowledge on efficient methods to carry out the online fraud. The current study aimed to determine the significance of social learning theory in explaining carding offending. It tried to examine the mediating role of online platforms in the social learning process. To achieve these objectives the study relied on a mixed-method approach, including a content analysis of a forum content and an online survey. The results provided support for the influence of social learning theory elements on the probability of involvement in carding activities. The presence of favourable definitions and differential reinforcement seemed to have the strongest effect on the crime. Unlike real-world social ties, online associations and the online imitation of models influenced perpetration of carding offences.

Abstract:
My thesis analyses the phenomenon of Figwit, a non-speaking elf extra who appeared for only three-seconds in the first instalment of Peter Jackson's 2001 Lord of the Rings movie trilogy. Figwit was initially generated as an online parody by female fans of the movie and as a foil to the 'swooning, drooling girly' fandom that was being directed towards the movie's star actors. However, Figwit evolved into a bona fide, albeit minor, celebrity both on and offline as he attracted attention from worldwide media, a small speaking role in the final movie and genuine adulatory fandom as manifested in the production of Figwit merchandise. In my thesis I argue that Figwit's creation and consequential community formation reflects a dynamic online-offline dialogic in which pre-existing offline and habitus-generated social practices and distinctions, ideal reflexive individuality and celebrity/fandom were dynamically reproduced within online technological frameworks. I also argue that online activity and interactivity is generated by users to strategically express and engage intensified reflexive individuality, affirming sociability and hyper-social distinctions. In this regard I have also argued that these various potentials and imaginaries were significantly enabled by digital architectures and genres of online communication and interactivity. In particular, I discuss the internet's capacity for searchability, traceability, and rhetorical framing processes that facilitate continuous re-editing authorship possibilities, which are not necessarily replicable in face-to-face interactions. Finally, I argue that reflexive online interactivity and identity expressions may transform into online and offline consequences that may be constructive, divergent or even contradictory. The arguments put forward in this thesis are based on a multi-sited ethnography, which utilises a variety of methodologies including participant-observation, subject interviews, communications and media archiving and analysis, and it draws from a variety of sources both online and offline.
