Abstract:
Standards for the Management of Open Fractures provides an evidence-based approach for the management of open fractures, focusing on lower limb injuries. It builds on and expands the National Institute for Health and Care Excellence (NICE) Guidelines to provide a practical approach with supporting evidence. The new edition has been extensively updated and expanded to include key aspects of management, ranging from setting up an orthoplastic service, through to dealing with bone and soft tissue injuries, including in young and older people, patient rehabilitation and psychological care, blast injuries, as well as complications such as infection. The book provides an important resource for orthopaedic and plastic surgeons managing patients with these challenging injuries.

♦ Displaced cervical fractures must be reduced and then fixed with lag screws♦ Avascular necrosis remains a significant problem♦ Intertrochanteric fractures may be treated closed with traction if an adequate reduction can be obtained and held♦ Dislocated hips should be reduced as soon as possible, open if necessary♦ Pelvic fractures are associated with a high mortality not so much from bleeding from pelvic veins as from accompanying major trauma to the rest of the body♦ The elasticity of children’s bones allows for single breaks in the pelvic ring.

Abstract:
♦ Segmental defects are a significant, but surmountable, problem for the Trauma surgeon♦ The best results will only be achieved if all potential solutions are considered and used correctly in either: isolation; sequentially; or concurrently.♦ Failure to create a bone defect, during debridement of some open fractures, may be catastrophic. An appreciation of the techniques available to treat segmental defects is necessary to allow the surgeon to debride with confidence.

Abstract:
In this article, we review the basic physics of viscoelastic phase separation including fracture phase separation. We show that with an increase in the ratio of the deformation rate of phase separation to the slowest mechanical relaxation rate the type of phase separation changes from fluid phase separation, to viscoelastic phase separation, to fracture phase separation. We point out that there is a physical analogy of this to the transition of the mechanical fracture behaviour of materials under shear from liquid-type, to ductile, to brittle fracture. This allows us to discuss phase separation and shear-induced instability of disordered materials including soft matter, on the same physical ground. Finally it should be noted that what we are going to describe in this article has not necessarily been firmly established and there still remain many open problems to be studied in the future.

Abstract:
Pelvic ring injuries can be life and limb threatening. The mechanism of injury can often be a good indicator of the type of injury; the Young & Burgess classification deploys that concept to full effect. Early identification based on mechanism of injury and improved prehospital care can play a major role in the outcome following such injuries. Pelvic ring injuries can lead to significant haemorrhage. Mechanical measures to stabilize the pelvis, in addition to modern concepts of damage control resuscitation (DCR), have been shown to be effective in early management of potentially life-threatening haemorrhage. Emphasis is now entirely on protecting the primary clot following a pelvic ring injury. Mechanical disturbance by log rolling the patient or springing of the pelvis are strongly discouraged. Early radiological clearance of the pelvis is encouraged. The lethal triad of coagulopathy, acidosis, and hypothermia should be corrected simultaneously to improve outcome. A traffic light system for monitoring venous lactate as an indicator of the patients’ physiological state can help the intensive care practitioner and the surgeon identify optimum timing for surgery. Pelvic ring injuries are associated with significant concomitant injuries. Limb trauma can also be life or limb threatening. Early identification, splinting, and resuscitation follow the same guidelines as pelvic ring injuries. Open long bone fractures should be managed by senior orthopaedic and plastic surgeons.

Abstract:
This chapter covers the basic approaches and techniques used in orthopaedic surgery, including the insertion and positioning of hardware and fixators, closed and open techniques (including manipulation under anaesthetic), and the typical imaging requirements for these. The discussion includes elective and trauma cases, as well as emergency procedures that may be performed out of regular working hours. They generally involve either repair to the skeleton and joints after injury (e.g. resiting a dislocated joint or aligning and supporting a fractured long bone) or alterations (such as fusion or replacement of a damaged joint or lengthening of a bone with a growth defect).

Lie, Knut-Andreas, and Olav Møyner, eds. Advanced Modeling with the MATLAB Reservoir Simulation Toolbox. Cambridge University Press, 2021. http://dx.doi.org/10.1017/9781009019781.

Full text
Abstract:
Many leading experts contribute to this follow-up to An Introduction to Reservoir Simulation using MATLAB/GNU Octave: User Guide for the MATLAB Reservoir Simulation Toolbox (MRST). It introduces more advanced functionality that has been recently added to the open-source MRST software. It is however a self-contained introduction to a variety of modern numerical methods for simulating multiphase flow in porous media, with applications to geothermal energy, chemical enhanced oil recovery (EOR), flow in fractured and unconventional reservoirs, and in the unsaturated zone. The reader will learn how to implement new models and algorithms in a robust, efficient manner. A large number of numerical examples are included, all fully equipped with code and data so that the reader can reproduce the results and use them as a starting point for their own work. Like the original textbook, this book will prove invaluable for researchers, professionals and advanced students using reservoir simulation methods.

Dillon, Michele. Postsecular American Catholics. Oxford University Press, 2018. http://dx.doi.org/10.1093/oso/9780190693008.003.0002.

Full text
Abstract:
This chapter demonstrates how American Catholics embody the mutual relevance of religious and secular expectations that is the hallmark of postsecularity. It argues that individual interpretive autonomy—the secularization of religious authority—is critical to their construal of Catholicism, and it discusses the ironies this entails. The chapter shows that interpretive autonomy is legitimated in official Church teaching, which in part allows Catholics to disagree with Church teachings on sexual morality and other issues while maintaining loyalty to Catholicism. It is also used by them to advocate for doctrinal changes that would more closely align their secular expectations with their attachment to the sacraments. Interpretive autonomy is thus a crucial mechanism in the preservation of Catholicism as a living tradition open to secular realities. The chapter also discusses how intra-Catholic political differences and a large socioeconomic divide between white and a growing Hispanic Catholic population fracture the notion of Catholic communal solidarity.

Abstract:
In 150 years, the trends in regional disparities in economic development within Italy have differed depending on whether they are gauged by longitude or by latitude. The disparities between western and eastern regions first widened and then closed; the North-South gap, by contrast, remains the main open problem in the national history of Italy. This chapter focuses on the underlying causes of the turning points in regional disparities since national unification in 1861. The first came in the late nineteenth and early twentieth century, with the industrialization of the so-called "industrial triangle". This was followed by the "failed new turn" during the interwar years: not only were the beginnings of convergence blocked, but the North-South gap, until then still natural, inevitably, was transformed into a fracture of exceptional dimensions. The second turning point, in the twenty years after the World War, produced the first substantial, lasting convergence between southern and northern Italy, powered by rising productivity and structural change in the South. The last turning point was in the mid-1970s, when convergence was abruptly halted and a protracted period of immobility in the disparity began.
