Full textThe aims of this thesis is to develop the knowledge on the microstructure and mineralization of femoral cortical bone, to assess the quantity and quality of osteoporotic bone. The techniques used are microradiography(2D) and SR μ CT (3D), and image analysis. In the first part, by using microradiography, we studied the mid-femoral cortex in diaphysis on a large adult population of both gender, to obtain normal data and its variation of age. The mean mineralization was higher in women than in men. A more homogeneous distribution of the mineralization of bone was found in men than in women. The total porosity, osteonal porosity, osteonal density and percent osteonal area increased with aging, whereas the percent matrix area decreased with age in females and in males. In the second part, we studied the femoral neck cortex in two olderly women groups: (a) with fractured neck and (b) control, by using SR μ CT and compared with microradiography. The degree of mineralization of bone (DMB) in osteons and matrix were higher in the fracture group than in the controls; the difference was significant by microradiography but not by SR μ CT. Zone-to-zone variability was greater for DMB of matrix than for DMB of osteon, particularly in the fracture group. In the third part, we developped a method of analyse to describe the topology and structural parameters of intra-cortical canals network, with the calculation of its skeleton. The structural and topological parameters describing the organization of the canal network, and their effect in bone fragility are described and measured in 2D and 3D. This may provide a better understanding of the ultrastructural factors for bone fragility

Full textThe Consequences of acute stress on immunosenescent system are unknown. Hip fracture (HF) represent a good model of acute stress because of its frequency and its severity in older patients. A previous study conclude to the prognosis role of neopterin, secreted by monocytes under interferon gamma stimulation, in HF. We hypothesize that hip fracture induce an early activation of innate and adaptive system. This activation need to be transitory to return at the homeostatic state. If this downregulation is lacking, its lead to bad prognosis for patients. Objectives of this work were: 1. Describe the phenotypic immune alteration induce by HF 2. Compared the functional alterations of monocytes between patients who survived at 1 years or not. 3. Explore the functionality of neopterin. Fifty patients (9 deceased at 1 year) and 10 controls were included. We show a transitory hyper activation of innate system associated with a suppression of adaptive system during the first days after the fracture. We found some arguments for a defect of monocyte reprogramming in patients with bad prognosis. Finally, we show that neopterin is not only a biomarker but also a molecule with functional activity, able to induce proinflammatory cytokines, cell death and oxidative stress. In conclusion, HF have an impact on immune system in older population. Monocyte and neopterin play a central role in patient prognosis and stress resilience, that could lead to news therapeutics targets

Full textFracture of the femoral neck may be a life-threatening condition in the elderly and ay have a poor functional outcome in young adults with the risk of avascular necrosis of the femoral head. Perfusion-tomodenstometry might be useful in studying the remaining vascularization after recent femoral neck fracture showing hypoperfusion thus giving an estimation for the risk of necrosis. Perfusion-tomodensitometry was performed in 20 patients. Images were analysed after delineation of an area of interest. Topographic adjustment was performed and results were analysed after modeling according to the pattern of diffusion. A single pattern was not found. Inter-individual variability was greater than intr-individulal variability. Difference in pattern between patients were important. No difference could be shown between the groups of "unaffected hips", "fractured hips", "fracture of the femoral neck", "extracapsular fracture of the proximal femur" although there was a trend against "fracture of the femoral neck". The only correlation found was between both sides of the same patient, although a tendency towards a difference between both sides of the "neck group" could appear. Analysis of our results does not confirm our work hypothesis. The only correlation found is most likely related to a tomodensitometric evaluation of the bone mineral density. Perfusion tomodensitometry clearly is not suitable to assess residual perfusion of the femoral head nor estimating the risk of avascular necrosis. However, the trends observed might indicate that the difference observed coulde become statistically significant by an increased number of cases

Full textFemoral fractures caused by the osteoporosis become major problem of public health, and therefore, this subject becomes an increasingly important goal for both clinicians and biomedical researchers. The purpose of this study is to develop a new coupled approach to predict the fracture of neck femoral. The current study proposes a validated 2D and 3D finite element (FE) models based on continuum damage mechanics in order to simulate human proximal femur fracture considering the progressive cracks initiation and propagation. These models are applied and validated under single limb stance and sideways fall configuration. Quasi brittle behavior laws coupled to damage are implemented in FORTRAN and fed into ABAQUS/Standard codes to describe the constitutive behavior (subroutine UMAT). Bone mineral density (BMD) is measured using dual energy X-ray absorptiometry (DXA) for the region of interest. The models have been developed within two variants (one isotropic, the other anisotropic) and validated with experimental results of tests performed on human femur samples under single limb stance configuration. During these tests, optical measurements based on the method of digital image correlation (DIC) were conducted to acquire the various fields of displacement and deformation. To calculate the fracture risk of the femoral head, it is necessary to assign correctly the bone material properties. The 3D FE models were able to predict the overall force-displacement curve, location and initiation of femur fractures. Moreover, despite its robustness, this 3D FE model is still limited to be used, within clinically acceptable time, for diagnostic purposes. To overcome this, the model was simplified into 2D model which has been preliminarily validated under identical boundary conditions and the results showed a good correlation with experiments. These studies have highlighted the potential of the finite element model based on quasi-brittle damage to become a complementary tool for predicting the risk of bone fracture

Full textThe aims of this thesis were to define the determinants of bone health in young overweight and obese men and to explore the effects of two physical training programs (strength vs. endurance) on bone parameters in young overweight and obese women. Two preliminary studies and six main studies have been conducted. The first preliminary study has shown that VO2 max (L/mn) is a positive determinant of bone mass in overweight children. The second preliminary study has shown that BMI is a negative determinant of CSI, BSI and ISI in young women. The first main study has shown that obesity is associated with lower CSI, BSI and ISI values in young women. The second main study has shown that VO2 max (L/mn) is positively correlated to CSI and ISI in young women. The third and the fourth study have shown that being overweight is associated with higher BMC and BMD values but not with TBS values and that TBS is positively associated with VO2 max (L/mn). The fifth main study conducted on 68 young overweight and obese women has shown that vertical jump, VO2 max (L/min), maximal power (watts) and 1-RM half-squat are positively correlated with bone parameters in overweight and obese adult women. The sixth main study has shown that the two training programs (strength and endurance) are effective to increase whole body BMC and lumbar spine BMD, and to decrease body weight and fat mass. Strength training was also effective to increase TBS and composite indices of femoral neck strength (CSI, BSI and ISI)

Full textIn a context of depressed health-conditions for elderly people, the levels of dependency are rising. It induces deep consequences for the entire health care organisation. For instance, the fractures of the femoral neck (FFN) represent a healthcare challenge regarding their strong impact on patients’ quality of life and on their morbity-mortality. We conducted a qualitative and quantitative study (Jan. 2012- Jan. 2013) within the main department of orthopedic surgery in Alsace in order to track the healthcare paths of 107 patients of 75 years-old or more. Our main conclusion is that patients are poorly involved in the decisions for their care. This is all the more harmful because the FFN is in fact dual. It is both a physical (higher assistance needed) and a symbolic (earlier dependency) breakage. Our interactionist study identifies areas for pragmatic actions in health-care, public policies and ethic that could contribute to absorb / reduce the pernicious effects of this pathology
