Full textThe doctor-patient relationship has undergone a transition throughout the ages. Prior to the last three decades, the relationship was predominantly between a patient seeking help and a doctor whose decisions were silently complied with by the patient. This description of the asymmetrical or imbalanced interaction between doctor and patient has been challenged during the last 30 years. Critics have proposed a more active, autonomous and thus patient-centered role for the patient who advocates greater patient control, reduced physician dominance, and more mutual participation. There is strong evidence for positive influences of patient-centered care on several clinical outcomes: more empathetic and patient-centered physicians are more likely to achieve higher patient satisfaction, adherence to treatments, and health outcomes. In the context of medical education, it is thus important to understand what educational interventions can be effective in maintaining and enhancing empathy and patient-centered attitude in undergraduate medical students. For this reason, the aim of this research work was to: (a) assess the relationship between some social and psycho-attitudinal skills (e.g. motivation for being a doctor, personality traits, self-efficacy, well-being) and empathic/patient-centered attitude of freshmen medical students to rethink the selection and evaluation criteria of Italian medical school admission, considering non-cognitive skills of students who are applying to medical schools; (b) examine the longitudinal changes in medical students’ empathy and patient-centeredness levels during medical school to evaluate the efficacy of current medical educational programs to enhance empathy and patient-centeredness among undergraduate medical students; (c) examine the relationship between dispositional mindfulness facets and empathy/patient-centeredness in non-meditating medical doctor students to provide a supportive evidence of the need for training undergraduate medical students in mindfulness practice. The results confirmed statistically significant associations between psychosocial skills and empathy/patient-centeredness. It is suggested that motivation, personality and self-efficacy of students should be taken into account in medical admission and/or in educational programs to enhance empathy in undergraduate medical education. It is concluded, also, that a significant increase in empathy and patient-centeredness occurs during medical school. The increase of empathy occurs during a time when the curriculum is shifting toward patient-care activities (2nd year internship at the GP’s office and 3rd-5th year internship in the hospital wards) and this is when empathy is most essential. Finally, although the statistical evidence points to the usefulness of teaching mindful practices, various issues remain to be considered: when is it best to teach mindfulness in the trajectory of a doctor’s career? What format works best, when and for whom? How can what is learned be maintained over time? Should mindfulness training be integrated into the medical school core curriculum?
