Penelitian ini bertujuan untuk mendapatkan hasil produksi dan karakterisasi antibodi anti Fim-C S.typhimurium pada mencit DDY menggunakan metode ELISA dan Western Blot. Metode yang digunakan adalah eksperimen dan eksplorasi dikelompokkan menjadi empat, yaitu : Kelompok perlakuan I ( protein Fim – C S.typhimurium + Adjuvant ) , Kelompok perlakuan 2 ( protein Fim – C S.typhimurium ), kelompok kontrol 1 ( FCA ). Penelitian ini menyimpulkan bahwa produksi antibodi anti Fim-C Salmonellatyphimurium telah berhasil dilakukan dengan dibuktikan melalui karakterisasi menggunakan teknik ELISA dan Western Immunoblotting. FIA adjuvant ), kelompok kontrol 2 (PBS1x). Protein Fim-C Salmonellatyphimurium dari penelitian sebelumnya digunakan sebagai antigen, disuntikan sebanyak empat kali pada mencit dengan 20 ?g, 40?g, 60?g, dan 40?g. Hasil karakterisasi menggunakan teknik ELISA menunjukan adanya kenaikan titer antibodi dari bleed 0 – bleed 4 pada kelompok eksperimen. Hasil karakterisasi menggunakan teknik Western Immunoblotting menunjukan adanya interaksi antibodi anti Fim-C Salmonella typhimurium dan antigen Fim-C Salmonella typhimurium secara spesifik.

Abstract:
Tifus merupakan penyakit yang banyak diderita oleh masyarakat di negara berkembang termasuk Indonesia. Sampai saat ini vaksin tifus masih terus dikembangkan. Penelitian ini bertujuan untuk memproduksi dan mengkarakterisasi antibodi anti Fim-C S. typhi secara in vivo sebagai kandidat vaksin pada mencit galur ddY. Metode yang digunakan dalam penelitian adalah metode eksperimen. Antigen yang digunakan adalah Protein rekombinan Fim-C S. typhi dalam bentuk murni hasil penelitian sebelumnya. Mencit ddY sebagai hewan coba dikelompokkan menjadi 4, yaitu: Kelompok uji I (protein Fim-C+Freund’s Complete/incomplete Adjuvant), Kelompok uji 2 (protein Fim-C S. typhi), kelompok kontrol sakit 1 (FCA/FIA adjuvant), kelompok kontrol sakit 2 (PBS1x). Hasil penelitian dengan metode ELISA (Enzyme Linked Immunosorbent Assay) menunjukkan adanya respon antibodi yang diproduksi oleh mencit ddY setelah injeksi subkutan dengan protein Fim-C+adjuvant maupun tanpa adjuvant. Respon antibodi dengan antigen Fim-C+adjuvant memberikan nilai yang lebih tinggi dibandingkan antigen Fim-C tanpa adjuvant. Hal tersebut dibuktikan dengan adanya kenaikan titer antibodi dari penyuntikan ke-1 hingga penyuntikan ke-4. Pada hasil penelitian dengan metode Western Blot menunjukkan terjadinya interaksi antigen spesifik Fim-C terhadap Antibodi anti Fim-C. Dari kedua metode tersebut, data ini mengkonfirmasi bahwa Protein Fim-C S. typhi memiliki imunogenisitas yang tinggi. Sehingga dapat disimpulkan bahwa protein Fim-C S. typhi dapat dijadikan sebagai kandidat vaksin rekombinan yang potensial untuk melawan penyakit tifus.

ABSTRACT Objective: To correlate disease duration in spinocerebellar ataxia type 2 (SCA2) with disease severity, balance and functionality. Method: Sixteen SCA2 patients were analyzed for: disease duration, disease severity (SARA score), balance (Berg balance scale score) and functionality (FIM and Lawton scores). Results: Greater severity was correlated with worse functionality (Lawton: r = −0.0561, FIM: r = −0.6402) and balance (r = −0.7188). Longer disease duration was correlated with greater severity (p = 0.0002) and reduced functionality (FIM: p = 0.005; Lawton: p = 0.0402) and balance (p = 0.0036). A year increase in disease duration corresponded to a 0.8-point increase on the SARA scale, a 1.38-point decrease in FIM score, a 2.30-point decrease on the Berg balance scale and a 0.45-point decrease on the Lawton scale. Conclusion: Longer disease duration in this series of SCA2 patients was correlated with greater disease severity, worse balance and greater functional dependency.

Abstract:
The paper presents a detailed analysis on the performance of reinforced concrete (RC)/ aerated autoclaved concrete (AAC) hybrid lintels, studied using finite element method (FEM), through numerical modelling. The analyzed lintels are a combination of modules, made of AAC and RC or pre-stressed concrete, which can be used for closing any span of windows or doors, for any wall thickness. To simulate the hybrid modules lintels in their service state, FEM modelling was used, together with rigorous numerical calculus, in order to better understand their flexural behaviour, the AAC-RC interface area and the way the material properties affect the structural performance of the analyzed lintels. The numerical analysis was initiated with validation of the reference model with analytically determined efforts in concrete component of the lintel. A subsequent model was constructed by adding the steel reinforcement and AAC component to the reference model. The analysis was conducted for two scenarios of a perfect and possibly very weak bond of RC/ AAC interface on “bonded interface model”, BIM and respectively “frictional interface model”, FIM models. The results showed that steel reinforcement had greater effect on the lintel`s stiffness than AAC component, while the effect of AAC is almost cancelled in the case of a weak bond of the interface. Future investigation needs to be developed considering an analytical model based on FIM model calibrated with laboratory measurements.

Abstract:
Background: The functional independence measure (FIM) is a standard tool to provide a detailed evaluation of ADL of patients with disabilities. This study aimed to show the differences in FIM scores as an outcome predictor between patients with anterior circulation (AC) and posterior circulation (PC) strokes. Methods: Consecutive patients with acute ischemic stroke hospitalized within 7 days after onset were investigated. Baseline NIHSS scores, 1st-FIM (< 72 h after ­admission to stroke unit), 2nd-FIM (< 72 h before discharge), and modified Rankin Scale (mRS) scores were collected. Logistic regression analyses were used to identify predictors of a favorable outcome (mRS 0–2) at 3-month after stroke. Results: Three hundred eighty-five patients (median age, 78 years; male, 59%; median length of stroke unit stay, 20 days) were included. The median baseline NIHSS, 1st- and 2nd-FIM scores were 4 (interquartile range 2–9), 65 (33–91), and 98 (54–122) respectively. Baseline NIHSS (3 vs. 4, p = 0.01) and mRS score at 3-month (1 vs. 2, p = 0.01) were lower, and 1st-FIM (75 vs. 64, p < 0.01) and 2nd-FIM (113 vs. 95, p = 0.01) were higher in 82 patients with PC than 303 patients with AC strokes. On multivariate logistic regression analysis, 2nd-FIM score was an independent predictor of favorable outcomes in both PC (OR 1.18, 95% CI 1.04–1.48, p < 0.01) and AC (OR 1.12, 95% CI 1.06–1.20, p < 0.01) strokes. The optimal cutoff scores of 2nd-FIM for predicting favorable outcome were 104 for PC (sensitivity 0.82, specificity 0.88) and 93 for AC (0.88–0.90) strokes. Conclusions: The differences in outcome predictability by FIM score between AC and PC strokes should be considered, although FIM scores at discharge from stroke unit were useful to predict a favorable outcome.

Outcome merupakan keadaan pasien paska cedera setelah mendapatkan penanganan medis di rumah sakit. Disabilitas yang terjadi yaitu 1 tahun setelah cedera.Functional Independence Measure atau FIM adalah suatu instrumen yang bisa digunakan untuk mengukur tingkat kemandirian seseorang atau pasien. Selain itu, FIM juga digunakan untuk menilai tingkat ketergantungan pasien pasca trauma.Penelitian ini menggunakan desain analitik observasional dengan pendekatan cohort retrospektif. Analisis univariat menggunakan uji descriptive statistice. Pasien pasca cedera dan diukur tingkat kemandirian atau ketergantungan pasien cedera kepala menggunakan Functional Independence Measure atau FIM pada bulan ke-1 sampai 6. Sampel pada penelitian ini yaitu 104 pasien. Hasil penelitian didapatkan 104 pasien mengalami perbaikan selama 2 bulan dengan tingkat kemandirian tanpa dibantu orang lain untuk aktifitas sehari-hari (skor FIM 6). Ada 9 orang pasien yang mengalami ketergantungan sampai bulan ke-6 (skor FIM 2). Hal ini disebabkan karena parahnya cedera yang dialami dan disertai dengan komplikasi penyakit lain seperti hipertensi dan diabetes mellitus serta fraktur. Kesimpulan dari penelitian ini adalah FIM bisa digunakan untuk menilai tingkat kemandirian pasien pasca cedera kepala mulai dari bulan pertama sampai dengan keenam. Kata kunci: disabilitas, functional independence measurable, outcome, cedera kepala OUTCOME SCORING OF HEAD INJURIES USING FUNCTIONAL INDEPENDENCE MEASURE ABSTRACT Head injury is the most common case and is a major cause of death and disability due to trauma. Outcome is a post-injury condition of the patient after getting medical treatment at the hospital. Disability that occurs is 1 year after injury. Functional Independence Measure or FIM is an instrument that can be used to measure the level of independence of a person or patient. In addition, FIM is also used to assess the level of dependence of post-traumatic patients. This study uses an observational analytic design with a retrospective cohort approach. Post-injury patients and the level of independence or dependence of head injury patients was measured using Functional Independence Measure or FIM in the months 1 to 6. The sample in this study was 104 patients. The results showed 67 patients experienced improvement for 2 months with a level of independence without the help of others for daily activities (FIM score 6). There were 9 patients who experienced dependence until the 6th month (FIM score 2). This is caused by the severity of the injury suffered and is accompanied by complications of other diseases such as hypertension and diabetes mellitus and fractures. The conclusion from this study is that FIM can be used to assess the level of independence of patients after head injury from the first to the sixth month. Este trabalho tem como objetivo discutir alguns aspectos relacionados ao segundo Objetivo de Desenvolvimento Sustentável da Organização das Nações Unidas (ODS2): “acabar com a fome; alcançar a segurança alimentar e melhoria da nutrição e promover a agricultura sustentável”, especialmente, sob a perspectiva da fome e da pobreza no campo, por meio de uma pesquisa bibliográfica-documental, no intuito de retomar as discussões sobre a importância e a necessidade da alimentação saudável e segura da população rural brasileira na atualidade. Foram abordados os conceitos de Segurança Alimentar e Nutricional e de Direito Humano à Alimentação Adequada e, também, foram apresentados alguns dos desafios para o combate à fome e à pobreza no contexto rural brasileiro. Por fim, concluiu-se que o Estado tem papel primordial neste processo, como promotor de um desenvolvimento rural sustentável, com enfoque na agricultura familiar, que possa corrigir as desigualdades estruturais vigentes e democratizar o acesso à alimentos saudáveis e adequados.

The additional costs incurred by work safety measures to dairy farms were estimated by interviewing 95 dairy farmers and inspecting with them production buildings, machines and the need for personal protective devices at a commune. The present value of the implemented safety and health improvements rose to FIM 22 000 per farm in 1983. About FIM 17000 were due to the tractors, FIM 3 000 to the cow houses and FIM 2 000 to other machines and tools than tractors. The protective equipment of tractors caused the annual maintenance cost of FIM 4 350 and cow house FIM 650, if the interest rate were 10 %. The annual total cost was over FIM 6 000. The replacement value would have been about FIM 31 000 in 1983. If all deficiences still remaining in 1983 had been corrected at once, the total cost would have been about FIM 12000 per farm, which leads to an annual cost of FIM 3 100. As for still remaining deficiencies, the study suggested need for an annual cost of about FIM 1 500 due to personal protectors. Half of this sum was due to need for the acquisition of personal safety equipment for forest work. Correcting the safety and health deficiencies of the cowshed and agricultural machines would produce each an annual cost of about FIM 800 per farm. The present value of the labor protection investments in 1983 was FIM 22 000 per farm, and there still remained need to invest an additional FIM 12000 in order to eliminate the remaining deficiencies. So, the theoretical total of the annual maintenance cost would have been FIM 6 600—9 200 per farm depending on the interest rate. Because the study concerned only one commune, the results cannot be generalized to the whole country.

Abstract:
Objective. To evaluate the factor structure of Functional Independence Measure (FIM®) scale amongst people with spinal cord injury (SCI). Methods. This was a retrospective, register-based cohort study on 155 rehabilitants with SCI. FIM was assessed at the beginning and at the end of multidisciplinary inpatient rehabilitation. The internal consistency of the FIM was assessed with Cronbach’s alpha and exploratory factor analysis was employed to approximate the construct structure of FIM. Results. The internal consistency demonstrated high Cronbach’s alpha of 0.95 to 0.96. For both pre- and postintervention assessments, the exploratory factor analysis resulted in 3-factor structures. Except for two items (“walking or using a wheelchair” and “expression”), the structures of the identified three factors remained the same from the beginning to the end of rehabilitation. The loadings of all items were sufficient, exceeding 0.3. Both pre- and postintervention chi-square tests showed significant p values < 0.0001. The “motor” domain was divided into two factors with this 2-factor structure enduring through the intervention period. Conclusions. Amongst rehabilitants with SCI, FIM failed to demonstrate unidimensionality. Instead, it showed a 3-factor structure that fluctuated only little depending on the timing of measurement. Additionally, when measured separately, also motor score was 2-dimensional, not 1-dimensional. Using a total or subscale FIM, scores seem to be unjustified in the studied population.

Abstract:
Objective To develop a new prediction model by combining independence in eating and bladder management functions, and to assess its utility in an acute care setting.Methods Patients with ischemic stroke who were admitted in our acute stroke care unit (n=250) were enrolled in this study. Functional Independence Measure (FIM) scores for eating and bladder management on the initial day of rehabilitative treatment (median, 3 days) were collected as predictive variables. These scores were divided into low (<5) and high (≥5) and categorized as values 0 and 1, respectively. From the simple summation of these two-level model values, we derived a three-level model that categorized the scores as values 0, 1, and 2. The FIM-motor scores at discharge (median, 14 days) were collected as outcome measurements. The three-level model was assessed by observing the distribution patterns of the outcome FIM-motor scores and logistic regression analyses.Results The median outcome FIM-motor score was 19 (interquartile range [IQR],13.8–45.3) for the value 0 category (n=14), 66.5 (IQR, 59.5–81.8) for the value 1 category (n=16), and 84 (IQR, 77–89) for the value 2 category (n=95) in the three-level model. Data fitting by logistic regression for FIM-motor scores of 41.3 and 61.4 reached 50% probability of values 1 and 2, respectively.Conclusion Despite the simplicity of the three-level model, it may be useful for predicting outcomes of patients with ischemic stroke in acute care.

Os pacientes com traumatismo cranioencefálico e escala de coma de Glasgow < 8 (TCE grave) possuem indicação para ventilação mecânica invasiva e, geralmente, apresentam incapacidade funcional importante. Grande parte, infelizmente, só inicia a marcha em casa, passando o período de internação praticamente no leito. O objetivo deste trabalho é avaliar a evolução da Escala de Medição da Locomoção Independente Funcional (FIM) em pacientes com TCE grave na fase hospitalar, utilizando a fisioterapia motora precoce desde a internação na UTI. Foram avaliados 30 pacientes com TCE grave, submetidos à fisioterapia motora precoce no leito, iniciando a postura ortostática e a marcha com o auxílio do fisioterapeuta, sempre que possível, ainda na UTI. A FIM avaliou a marcha e o subir e descer escadas, apresentando a graduação mínima em 2 e a máxima em 14. O tempo de avaliação foi de 30 dias após o desmame, sendo a FIM mensurada durante a internação. Os pacientes que foram à óbito (3 pacientes) e que permaneceram em estado vegetativo (5 pacientes) durante a internação foram excluídos do estudo. O valor inicial da FIM foi 2 para todos os 22 pacientes avaliados. Porém, até o trigésimo dia após o desmame, a FIM variou de 3 a 14 (7,36 ± 2,32), P = 0,0002 - Wilcoxon Test. Com os resultados obtidos, podemos concluir que a fisioterapia motora precoce proporcionou melhora bastante significativa na locomoção independente (avaliados pela FIM) em pacientes com TCE grave, sendo essencial para o tratamento na fase intra hospitalar.Palavras-chave: idosos, quedas, institucionalização.

To find out the clinical profiles of peripartum neurodeficit, a prospective observational study in 23 patients was conducted from 1st July 2008 to 31st June 2009 in the department of Physical Medicine, Chittagong Medical College. Presenting neurological findings and FIM scores for self-care, sphincter control, transfer and locomotion were measured. Ambulation of the patients was paretic and dependent. Paralysed body parts were mainly both lower limbs (69.6%). Disability pattern was mainly in walking (82.6%). Reflexes of most of the patients and mostly lower limbs were affected. Muscle power was reduced in the lower limbs in 100% and in the upper limbs in 40% of the patients. Neurological profiles are measured with functional independence measure (FIM) scale. Normal FIM scores are of self-care-42, sphincter control-14, transfer-21 and locomotion-14 and combined score is 91. In the observed patients, FIM scores of selfcare were; minimum 9 to maximum 21, sphincter control 2 to 5, transfer 3 to 9, locomotion 2 to 5. Combined total FIM score was found minimum 16 to maximum 49 and mean 36.70 + 11.60 out of normal score 91. From this study it may be concluded that peripartum neurodeficit may cause various neurological disabilities. JCMCTA 2014 ; 25 (1): 18-21

Monthlong hindcasts of the Madden–Julian oscillation (MJO) from the atmospheric Flow-following Icosahedral Model coupled with an icosahedral-grid version of the Hybrid Coordinate Ocean Model (FIM-iHYCOM), and from the coupled Climate Forecast System, version 2 (CFSv2), are evaluated over the 12-yr period 1999–2010. Two sets of FIM-iHYCOM hindcasts are run to test the impact of using Grell–Freitas (FIM-CGF) versus simplified Arakawa–Schubert (FIM-SAS) deep convection parameterizations. Each hindcast set consists of four time-lagged ensemble members initialized weekly every 6 h from 1200 UTC Tuesday to 0600 UTC Wednesday. The ensemble means of FIM-CGF, FIM-SAS, and CFSv2 produce skillful forecasts of a variant of the Real-time Multivariate MJO (RMM) index out to 19, 17, and 17 days, respectively; this is consistent with FIM-CGF having the lowest root-mean-square errors (RMSEs) for zonal winds at both 850 and 200 hPa. FIM-CGF and CFSv2 exhibit similar RMSEs in RMM, and their multimodel ensemble mean extends skillful RMM prediction out to 21 days. Conversely, adding FIM-SAS—with much higher RMSEs—to CFSv2 (as a multimodel ensemble) or FIM-CGF (as a multiphysics ensemble) yields either little benefit, or even a degradation, compared to the better single-model ensemble mean. This suggests that multiphysics/multimodel ensemble mean forecasts may only add value when the individual models possess similar skill and error. An atmosphere-only version of FIM-CGF loses skill after 11 days, highlighting the importance of ocean coupling. Further examination reveals some sensitivity in skill and error metrics to the choice of MJO index.

ABSTRACT In a previous study, it was found that the antibody response to a nonvaccine pertussis antigen in children who were vaccine failures was reduced compared with the response in nonvaccinated children who had pertussis. In two acellular pertussis vaccine efficacy trials in Sweden, we studied the convalescent-phase enzyme-linked immunosorbent assay (ELISA) geometric mean values (GMVs) in response to pertussis toxin (PT), filamentous hemagglutinin (FHA), pertactin (PRN), and fimbriae (FIM 2/3) in vaccine failures and controls with pertussis. In Germany, the antibody responses to Bordetella pertussis antigens PT, FHA, PRN, and FIM-2 were analyzed by ELISA according to time of serum collection after onset of illness in children with pertussis who were vaccine failures or who were previously unvaccinated. Antibody values were also compared by severity of clinical illness. In Sweden, infants who had received a PT toxoid vaccine and who were vaccine failures had a blunted response to the nonvaccine antigen FHA compared with the response in children who had received a PT/FHA vaccine. Similarly, infants who had pertussis and who had received a PT/FHA vaccine had a blunted response to the nonvaccine antigens PRN and FIM 2/3 compared with the response in children who were vaccine failures and who had received a PT, FHA, PRN, and FIM 2/3 vaccine. In Germany, in sera collected from 0 to 15 days after pertussis illness onset, the GMVs for all 4 antigens (PT, FHA, PRN, and FIM-2) were significantly lower in an unvaccinated group than in children who were diphtheria-tetanus-acellular pertussis (DTaP) vaccine failures. In the unvaccinated group, the GMV of the PT antibody rose rapidly over time so that it was similar to that of the DTaP vaccine recipients at the 16- to 30-day period. In contrast, the antibody responses to FHA, PRN, and FIM-2 at all time periods were lower in the diphtheria-tetanus vaccine (DT) recipients than in the DTaP vaccine failures. In both Sweden and Germany, children with less severe illness had lower antibody responses than children with typical pertussis. Our findings indicate that upon exposure and infection, previous vaccinees have more-robust antibody responses to the antigens contained in the vaccine they had received than to Bordetella antigens that were not in the vaccine they had received. In addition, over time the antibody responses to FHA, PRN, and FIM-2 were greater in children with vaccine failure (primed subjects) than in unvaccinated children (unprimed subjects) whereas the responses to PT were similar in the primed and unprimed children, as determined from sera collected after 15 days of illness. Our findings lend support to the idea that DTaP vaccines should contain multiple antigens.

Aim: The aim of this study was to determine Activities of Daily Living/Instrumental Activities of Daily Living (ADL/IADL) evaluations that will enable better understanding of the severity of Alzheimer’s disease (AD). Methods: AD patients were evaluated by Functional Independence Measure (FIM), Hyogo Activities of Daily Living Scale (HADLS), and Assessment of Motor and Process Skills (AMPS) to identify the assessments that will enable highly precise discrimination of AD Clinical Dementia Rating (CDR) 2 (moderate) and CDR3 (severe) using receiver operating characteristic (ROC) curve and discriminant analyses. Results: The participants were 75 AD patients (CDR2 = 50, mean age = 80.3 ± 5.9 years; CDR3 = 25, mean age = 78.3 ± 9.0 years). The evaluation methods consisted of FIM, HADLS, and AMPS. The results were divided into FIM-M, FIM-C, HADLS-ADL, HADLS-IADL, AMPS-motor skills, and AMPS-process skills. The values for the area under the curve (AUC) were compared by ROC curve and discriminant analyses. AUC values for FIM-C and AMPS-process skills were 0.956 and 0.947, respectively. With these two evaluations only, values ≥0.9 were shown. Moreover, the AUC of the discrimination score (combination of the FIM-C and AMPS-process skills) was significantly higher than those for FIM-M, FIM-C, HADLS-ADL, HADLS-IADL, and AMPS-motor skills. Conclusions: The results demonstrated that evaluation by FIM-C and AMPS-process skills methods was useful for discriminating between CDR2 (moderate) and CDR3 (severe) AD. Moreover, the results indicated that these two evaluation methods enabled more accurate determination of severity and the spared capabilities of AD patients.

AbstractIn-situ imaging of catalytic reactions has provided insights into reaction front propagation, pattern formation and other spatio-temporal effects for decades. Most recently, analysis of the local image intensity opened a way towards evaluation of local reaction kinetics. Herein, our recent studies of catalytic CO oxidation on Pt(hkl) and Rh(hkl) via the kinetics by imaging approach, both on the meso- and nano-scale, are reviewed. Polycrystalline Pt and Rh foils and nanotips were used as µm- and nm-sized surface structure libraries as model systems for reactions in the 10–5–10–6 mbar pressure range. Isobaric light-off and isothermal kinetic transitions were visualized in-situ at µm-resolution by photoemission electron microscopy (PEEM), and at nm-resolution by field emission microscopy (FEM) and field ion microscopy (FIM). The local reaction kinetics of individual Pt(hkl) and Rh(hkl) domains and nanofacets of Pt and Rh nanotips were deduced from the local image intensity analysis. This revealed the structure-sensitivity of CO oxidation, both in the light-off and in the kinetic bistability: for different low-index Pt surfaces, differences of up to 60 K in the critical light-off temperatures and remarkable differences in the bistability ranges of differently oriented stepped Rh surfaces were observed. To prove the spatial coherence of light-off on nanotips, proper orthogonal decomposition (POD) as a spatial correlation analysis was applied to the FIM video-data. The influence of particular configurations of steps and kinks on kinetic transitions were analysed by using the average nearest neighbour number as a common descriptor. Perspectives of nanosized surface structure libraries for future model studies are discussed.

Abstract The atmospheric hydrostatic Flow-Following Icosahedral Model (FIM), developed for medium-range weather prediction, provides a unique three-dimensional grid structure—a quasi-uniform icosahedral horizontal grid and an adaptive quasi-Lagrangian vertical coordinate. To extend the FIM framework to subseasonal time scales, an icosahedral-grid rendition of the Hybrid Coordinate Ocean Model (iHYCOM) was developed and coupled to FIM. By sharing a common horizontal mesh, air–sea fluxes between the two models are conserved locally and globally. Both models use similar adaptive hybrid vertical coordinates. Another unique aspect of the coupled model (referred to as FIM–iHYCOM) is the use of the Grell–Freitas scale-aware convective scheme in the atmosphere. A multiyear retrospective study is necessary to demonstrate the potential usefulness and allow for immediate bias correction of a subseasonal prediction model. In these two articles, results are shown based on a 16-yr period of hindcasts from FIM–iHYCOM, which has been providing real-time forecasts out to a lead time of 4 weeks for NOAA’s Subseasonal Experiment (SubX) starting July 2017. Part I provides an overview of FIM–iHYCOM and compares its systematic errors at subseasonal time scales to those of NOAA’s operational Climate Forecast System version 2 (CFSv2). Part II uses bias-corrected hindcasts to assess both deterministic and probabilistic subseasonal skill of FIM–iHYCOM. FIM–iHYCOM has smaller biases than CFSv2 for some fields (including precipitation) and comparable biases for other fields (including sea surface temperature). FIM–iHYCOM also has less drift in bias between weeks 1 and 4 than CFSv2. The unique grid structure and physics suite of FIM–iHYCOM is expected to add diversity to multimodel ensemble forecasts at subseasonal time scales in SubX.

Objective: The Functional Independence Measure (FIM™) and spinal cord injury (SCI)-specific Spinal Cord Independence Measure (SCIM) are commonly used tools for outcome measurement and quality reporting in rehabilitation. The objective of this study was to investigate the psychometric properties of FIM™ and SCIM and to equate the 2 scales.Methods: First, content equivalence of FIM™ and SCIM was established through qualitative linking with the International Classification for Functioning, Disability and Health (ICF). Secondly, a Rasch analysis of overlapping contents determined the metric properties of the scales and provided the empirical basis for scale equating. Furthermore, a transformation table for FIM™ and SCIM was created and evaluated. Subjects: Patients with SCI in Swiss inpatient rehabilitation in 2017–18.Results: The ICF linking and a separate Rasch analysis of FIM™ restricted the analysis to the motor scales of FIM™ and SCIM. The Rasch analysis of these scales showed good metric properties. The co-calibration of FIM™ and SCIM motor scores was supported with good fit to the Rasch model. The operational range of SCIM is larger than for FIM™ motor scale.Discussion: This study supports the advantage of using SCIM compared with FIM™ for assessing the functional independence of patients with SCI in rehabilitation. LAY ABSTRACTIn our study we compared two rehabilitation outcome assessment tools commonly used to measure functional independence in Spinal Cord Injury: the Functional Independence Measure (FIM™), a general tool, and the Spinal Cord Independence Measure (SCIM), a tool specifically developed for Spinal Cord Injury rehabilitation. We first compared the content of the two tools using the International Classification of Functioning Disability and Health (ICF). Then we tested their measurement properties and put them on a common measurement scale, which allows to directly compare scores of the two tools. The common measurement scale was obtained by mean of a so-called Rasch analysis. The results showed that the FIM™ motor items can be compared to the SCIM items from a content but also from a metric point of view. The study showed an advantage in using the SCIM compared to the FIM™ for assessing the functional independence of patients in Spinal Cord Injury rehabilitation.

Objetivo: Compreender o perfil clínico dos pacientes com perda olfatória devido a infecção pelo SARS-CoV-2, além de sua persistência ao longo do período do estudo e a presença de possíveis fatores associados. Métodos: Inicialmente, foi colhido questionário via formulário online a fim de avaliar as queixas do paciente, e em seguida foram realizadas avaliações presenciais para realização de testes olfatórios a fim de mensurar a anosmia. Resultados: 75 pacientes foram incluídos no estudo, 56 (75%) do sexo feminino e 19 (25%) do sexo masculino. Em relação ao estudo do sentido do olfato realizado de forma presencial, foi observado que, à medida que mais avaliações eram realizadas, melhor se tornava a percepção deste sentido pelo paciente. Entretanto, nem todos os 75 pacientes contidos na amostra conseguiram realizar todas as avaliações sugeridas. Conclusão: A investigação para um melhor manejo dos distúrbios olfatórios deve ser contínua, pois muitas questões seguem incertas, principalmente as relacionadas à fisiopatologia, à avaliação objetiva da anosmia e ao seu tratamento.

Federated identity management (FIM) is an arrangement that can be made among multiple organisations that lets subscribers use the same identification data to obtain access to the secured resources of all organisations in the group. In many research communities there is an increasing interest in a common approach to FIM as there is obviously a large potential for synergies. FIM4R [1] provides a forum for communities to share challenges and ideas, and to shape the future of FIM for our researchers. Current participation covers high energy physics, life sciences and humanities, to mention but a few. In 2012 FIM4R converged on a common vision for FIM, enumerated a set of requirements and proposed a number of recommendationsfor ensuring a roadmap for the uptake of FIM [2]. In summer 2018, FIM4R published an updated version of this paper [3]. The High Energy Physics (HEP) Community has been heavily involved in creating both the original white paper and the new version, which documented the progress made in FIM for Research, in addition to the current challenges. This paper presents the conclusions of this second FIM4R white paper and a summary of the identified requirements and recommendations. We focus particularly on the direction being taken by the Worldwide LHC Computing Grid (WLCG), through the WLCG Authorisation Working Group, and the requirements gathered from the HEP Community.

Sendo assim, focalizamos neste artigo a necessidade de aprimoramento das habilidades discursivas dos educandos que vivem experiências de ensino de língua a distância, mais especificamente, de ensino em ambientes virtuais de aprendizagem (AVA). A experiência como professores de língua portuguesa, construída nas interações promovidas em AVA, tem nos apontado a necessidade de preparar o educando para o uso de ferramentas digitais de forma consciente e produtiva, a fim de que ele seja capaz de usufruir desses benefícios enquanto leitor e produtor de textos. The purpose of this study was to compare the functional outcomes of two groups of patients with traumatic brain injury (TBI) with attention to the impact of reduced length of stay (LOS) in the trauma center (TC) and rehabilitation hospital (RH). From 1991 to 1994, 55 patients, Group 1, with serious TBI (Abbreviated Injury Scale score ≥3) were admitted to a level 1 TC and subsequently transferred to a comprehensive inpatient RH. These results have been previously published. From 1996 to 2002, 64 similarly injured patients, Group 2, received inpatient care at the same TC and RH. These patients had a marked decrease in length of stay. Functional Independence Measures (FIM) were obtained at admission (Adm), discharge (D/C), and at 1 year follow-up for both groups. The average length of stay at the TC dropped from 36 days in Group 1 to 26 days in Group 2. In addition, the average length of stay at the RH dropped from 46 days (Group 1) to 25 days (Group 2); overall, an average reduction of 31 days of inpatient care. Group 2 had significantly lower FIM scores at the time of RH discharge for self-care, locomotion, and mobility compared to Group 1. At the 1 year follow-up, however, there were no significant differences between Groups 1 and 2 in these FIM scores. FIM scores at 1 year were higher in Group 2 for communication (90% vs 71%) and social cognition (77% vs 49%) compared to Group 1. Over one-fourth of each group returned to work by the 1 year follow-up. Socially disruptive behavior occurred at least weekly in 28 per cent (Group 1) and 23 per cent (Group 2) of patients. The outcome for serious TBI is better than generally perceived. Reduction of inpatient LOS did not adversely affect the ultimate functional outcome. The decreased LOS placed a greater demand on outpatient rehabilitative services as well as a greater burden on the family of the brain-injured patient

Objetivo: O objetivo deste estudo foi analisar o excessivo tempo assistindo televisão e usando computador em dias úteis e no fim de semana em estudantes do ensino médio da cidade do Rio Branco. Método: Trata-se de um estudo transversal de base escolar com 1391 estudantes na faixa etária entre 14 e 18 anos do ensino médio de escolas públicas e particulares. Utilizou-se ponto de corte de igual ou maior a 2 horas por dia para categorizar excessivo tempo assistindo televisão e usando computador em dias úteis e fim de semana. A regressão logística múltipla identificou o modelo final. Resultados: A prevalência para tempo excessivo de assistir televisão foi de 8,9% em dias úteis e 39,7% no fim de semana. Enquanto a prevalência de excessivo uso de computador identificada em dias úteis e fim de semana foram de 6,9% e 27,4%, respectivamente. As prevalências altas de excessivo tempo de tela ocorreram para assistir televisão e uso de computador no fim de semana. A permissão para assistir televisão e usar computador no quarto de dormir foram os principais contribuintes ao excessivo tempo de tela, tanto em dias úteis e como no fim de semana. Além disso, o excessivo tempo assistindo televisão associou-se com as variáveis ir semanalmente a ginásios ou estádios e ler semanalmente em dias úteis e ter idade entre 16 e 18 anos em fim de semana. Enquanto ser filho único em dias úteis e estar matriculado em escola privada no fim de semana associaram-se ao excessivo uso de computador. Conclusão: Maior vulnerabilidade ao excessivo tempo assistindo televisão e usando computador ocorreram no fim de semana em estudantes do ensino médio de Rio Branco. Tanto assistir computador no quarto e quanto usar computador no quarto foram os principais contribuintes para os excessivos tempos de tela em dias úteis e no final de semana. Esses achados sugerem a necessidade de implementação de ações governamentais voltadas a prevenção do excessivo tempo de tela em estudantes. Recombinant protein Fim-C S.typhi is a potential protein that can be used as an alternative typhoid vaccine and produced on a large scale. However, before entering into a large-scale stage, laboratory optimum data on the factors that affect the production process of Fim-C Salmonella typhi proteins are required. This study aims to obtain information regarding the effect of host cell number E.coli BL21 (DE3) pLysS and overexpression time on production of recombinant protein Fim-C Salmonella typhi as the basis for developing candidate of typhoid vaccine. The optimization process of overexpression was done by adding 0.5 mM IPTG (Isopropyl-β-D-thiogalactopyranoside) inducer into bacterial cultures of 2%, 4%, and 6% with 4, 5, and 6 hours over expression. The measurement of the concentration of Fim-C recombinant protein extracted samples were done by a spectrophotometric method used BCA Kit Assay Thermo ScientificTM with wavelength 590nm. The characterization of those protein extracts was performed using SDS-PAGE method. The results from the study concluded that the number of 4% E.coli bacterial cells and four-hour overexpression time are the optimum condition of Fim- C Salmonella typhi characterized by the presence of high-intensity bands at ± 31 kDa.

The substituted Mn2Sb-based ferrimagnetic (FIM) alloys by other elements have attracted much attention recently due to excellent magneto-functional properties. However, almost all investigations have been concentrated on the first-order phase transition between antiferromagnetic and FIM state. In this work, a large topological Hall effect (THE) associated with the spin reorientation transition is observed in the Mn2Sb0.9Bi0.1 alloy. Strikingly, the sign reversal of topological Hall resistivity ([Formula: see text]) is found before and after the SRT temperature TSR ∼260 K. Above the TSR, a positive [Formula: see text] is observed, e.g., ∼0.52 μΩ cm at 320 K. By reducing temperature to 275 K, the [Formula: see text] value decreases to ∼0.42 μΩ cm. It is interesting that, however, the negative [Formula: see text] value is found below the TSR, e.g., ∼−0.66 μΩ cm at 175 K. The physical mechanism about this peculiar THE is discussed in Bi-doped Mn2Sb-based alloys.

Some numerical calculations are performed with the FIM. The results for spherical particles are compared with those by the Mie theory. It is confirmed that the agreement between them is satisfactory for all the models calculated.On the basis of the present method, we examine profiles of the absorption band around»= 10μmfor spherical and ellipsoidal particles composed of crystalline olivine. It is found that the profile strongly depends on the shape of the particle. Even when the particle is moderately elongated (axial ratios are 2: √2:1), the profile is Signifioantly different from that for a sphere.

Não obstante a complexidade do tema primeira infância, a estratégia está centrada na realização de visitas domiciliares a famílias com crianças na primeira infância em situação de vulnerabilidade social. Desde o seu lançamento, o Criança Feliz suscita incertezas nos principais atores envolvidos na Política de Assistência Social, não só pela sua forma de concepção e lançamento ser estranha à institucionalidade construída nesta política, mas também por sua eventual inadequação em relação aos objetivos protetivos consolidados da área na última década. Considerando a existência de outras ações na política direcionadas ao acompanhamento familiar (por exemplo, Proteção e Atendimento Integral à Família – Paif), incertezas se colocam sobre a novidade da estratégia e principalmente quanto à sua relação com outros serviços desenvolvidos no Suas. Por fim, é analisada a execução orçamentária do Ministério do Desenvolvimento Social (MDS), no período entre 2014 e 2016, sinalizando que a Política de Assistência Social sofreu perdas importantes, e estas poderão ser ainda maiores diante do novo teto de gastos estabelecido pela EC no 95/2016, como aponta o orçamento de 2017.

In recent years, one of the urgent issues for two dimensional (2D) magnetic materials is to find efficient ways in enhancing the magnetic ordering temperature Tc. It is believed that an in-plane (IP) compressive strain can greatly enhance the interatomic interactions by shortening the chemical bond length if at all possible, leading to the enlarged spin exchange and possibly higher Tc. However, a large compressive strain usually favors antiferromagnetic (AFM) ordering due to growing dominance of the Pauli exclusion principle, in contradiction with the common requirement of nonzero magnetization. In compromise, ferrimagnetic (FiM) ordering can be alternated by synthesizing artificial 2D compound with two magnetic sublattices. In this work, we propose a V-implanted CrI3 monolayer, short for V-(CrI3)2, and study its FiM ordering under a series of IP biaxial strains using the first-principles calculations and Monte Carlo simulations. It is found that the V-(CrI3)2 monolayer may evolve from the stripy-type AFM insulator toward the FiM half-metal with net magnetic moment of 5.0 μB/f.u. aligned in parallel to the ab-plane upon increasing the IP biaxial strain up to ∼−3% (compressive strain) and beyond. As the IP biaxial strain increases up to ∼−5%, the Tc of the FiM state may be raised to room temperature. This work suggests that the IP strain engineering combined with spin implantation can be an alternative strategy for enhancing 2D magnetism.

Background: Hip fractures are serious injuries associated with relatively high mortality rates and disabilities, commonly seen in elderly persons. There is an ongoing debate regarding the advantages of various hip arthroplasty devices. This study aimed to analyze the long-term advantages of 2 different surgical procedures and assess if the dislocation rate, Harris Hip Score (HHS), and functional independence measure (FIM) are more favorable in dual mobility (DM) than those in hemiarthroplasty (HA). Hypothesis: Dual mobility procedures provide better postoperative outcomes than HA in terms of HHS, FIM, and dislocation rate. Materials and Methods: The survey was a prospective, comparative interventional single-blinded study performed at the University Clinical Center of Kosovo, a tertiary health-care institution. A total of 94 patients underwent DM or conventional bipolar HA for repair of displaced femoral neck fractures within 2 weeks of injury. Primary outcomes were postoperative dislocation rate, FIM, and HHS. Secondary outcomes included duration of surgery, estimated intraoperative blood loss, time to first postoperative full weight-bearing, time to walking ability with and without crutches, mortality rate, and postoperative infection rate. Results: There were no significant differences for most parameters between the groups. We found a significant difference in the dislocation rate between the 2 groups, wherein there were no dislocations in the DM group and 3 dislocations in the HHS group (0% vs 6.4%). In terms of postoperative HHS at 12 months and 3 years, DM provided better outcomes (<0.034 and <0.014, respectively). Discussion: Dual mobility compares favorably to HA in terms of dislocation rate and HHS, while no difference was found for FIM. In order to have a more complete overview, we recommend more intense long-term studies including several heterogeneous parameters to compare the clinical outcomes between DM and HA. Level of evidence (with study design): Level II.

Aim: The aim of this study is to evaluate salpingectomy specimens of all types of ovarian tumors by opting SEE-FIM grossing protocol. Methods: A total of 79 samples of fallopian tubes were grossed following SEE-FIM protocol, as mentioned by CAP. This study was conducted in Pathology department at Niazi Medical and Dental College Sargodha. Results: One third ovarian tumors (n = 26, 33.8%) were bilateral and two fifth tumors (n = 31, 40.3%) were on the right side. More than ninety percent samples (n = 71, 92.2%) had non-precursor lesions. Lastly, most common cases of ovarian tumors were of mature cystic teratoma (n = 16, 20.8%), dysgerminoma (n = 13, 16.9%) and mucinous cystadenoma (n = 12, 15.6%). The results of chi-square tests showed significant association of both histopathological changes in fallopian tube (χ2 (6) = 31.05, p < .001) and histopathological findings (χ2 (2) = 7.81, p < .05) with types of ovarian neoplasm. Conclusion: The routine following of SEE-FIM is an easily adoptable and valuable protocol in terms of finding unexpected / incidental lesions and their correlation with different ovarian neoplasms.
