Fetus in fetus es un hallazgo clínico infrecuente, reportado menos de 100 casos en la literatura (FIF) [1]. Se presenta principalmente en recién nacidos, con una incidencia de aproximadamente 1 de cada 500 000 nacidos vivos. [2]. Hasta donde sabemos, existe un solo caso reportado de FIF con presentación pulmonar a nivel mundial. En Perú no existe notificación de casos de FIF con presentación pulmonar, y nuestro hallazgo se trató de un neonato de 15 días de vida, con dificultad respiratoria y hipoxemia severa, requirió soporte ventilatorio mecánico, e imágenes radiográficas torácicas donde evidencia tumoración mediastinal posterior, encontrándose durante la cirugía Fetus in fetus toráxico [Figura 2], poco descrito en la literatura, y que, gracias a la intervención quirúrgica temprana, se obtuvo buenos resultados clínicos. Muchas veces la intervención tardía puede producir un desenlace fatal, por la ubicación mediastinal que comprime estructuras de órganos vitales. Wu y col. informaron un paro cardíaco en un bebé debido a un enorme FIF en el mediastino posterior [4] Las ayudas imagenológicas juegan un rol importante en su diagnóstico.

✓Fetus-in-fetu is a very rare condition in which one fetus is contained within another. About 100 cases have been reported, and in most of these the fetus was located in the retroperitoneum. The authors describe an extremely rare case of an intracranial fetus-in-fetu in an extraaxial location. This is the eighth intracranial fetus-in-fetu to be reported, the first intracranial extraaxial case, and involves the oldest documented patient with this condition. Histopathological analysis of the mass revealed a degenerated amnionic membranelike tissue, well-differentiated extremities (including fingerlike structures), skin, matured lungs, well-formed intestines, cerebellar and cerebral tissue, and a notochord with ganglion cells. DNA analysis using short tandem repeat polymorphisms confirmed that the fetus-in-fetu mass and the host infant had heterozygous alleles and were of identical sex and genotype.

Introducción. Fetus in fetu es una rara entidad congénita en la cual se presenta un gemelo monocigótico que en la mayoría de los casos está incorporado en el abdomen del hermano. La presentación más frecuente es una masa retroperitoneal. La diferenciación diagnóstica entre fetus in fetu y teratoma es discutida. El objetivo de este trabajo es describir el caso de dos pacientes con fetus in fetu, mediante una revisión reducida de la literatura en sus hipótesis etiológicas y métodos diagnósticos. Reporte de casos. Se presentan 2 casos, el primer caso es un lactante menor con distensión abdominal persistente, en quien se palpó masa abdominal. El segundo caso es un recién nacido con diagnóstico prenatal de masa abdominal. Ambos casos presentan imágenes radiológicas que evidencian masa abdominal compatible con fetus in fetu, razón por la cual posteriormente fueron intervenidos quirúrgicamente para resección de la masa. Discusión. Es importante tener presente esta patología como diagnóstico diferencial de masa abdominal en los pacientes infantes, ya que el poder diferenciarlo de un teratoma maduro nos indicaría que se trata de una alteración del desarrollo embriológico y no de una patología tumoral. Para esto, son necesarios métodos imagenológicos y en algunos casos pruebas cromosómicas o histopatológicas, con el fin de esclarecer el diagnóstico del fetus in fetu. Conclusión. El fetus in fetu es una patología rara con pocos casos diagnosticados prenatalmente, la cual se diagnostica principalmente en la infancia. Las imágenes radiológicas y en algunos casos las pruebas cromosómicas o histopatológicas, son de gran importancia para el diagnóstico definitivo.

Fetus in fetu is a rare congenital anomaly in which a malformed fetus grows within the body of its twin. It is detected as an abdominal mass in infancy. It is a heterophagus parasite of a diamniotic, monozygotic twin. It should be differentiated from teratoma which has no axial arrangement and has got malignant potential. Although fetus in fetu is a rare condition, diagnosis using imaging can be made before surgery. It should be considered as a differential diagnosis for lump abdomen in infants. Complete excision is the treatment of choice. We present a case of a 9 years old female with fetus in fetu who was successfully managed by surgical excision.

Abstract:
Fetus in fetu (FIF) constitui uma condição anômala rara benigna, sendo definida como a presença de um feto parasita, monozigótico e diamniótico dentro de outro feto durante uma gestação. Com aproximadamente 200 casos descritos na literatura médica, se caracteriza por um quadro bastante raro e de grande interesse na comunidade científica da área da saúde. FIF se apresenta em diversas idades, sendo mais comum na infância, mais precisamente em pacientes com menos de 18 meses de vida, com poucos casos relatados em adultos O diagnóstico de FIF é realizado, principalmente, através de métodos de imagem, como exemplo a ultrassonografia. Este artigo contempla um relato de caso de uma paciente jovem do sexo feminino que, com aproximadamente 39 semanas de uma gestação com complicações, concebeu um recém-nascido do sexo masculino com exames pré-natais sem alterações, mas com exame físico e exames de imagem realizados pós-natal compatíveis com fetus in fetu em Fortaleza, Ceará, Brasil. Durante a estadia pós nascimento no hospital, o recém-nascido foi submetido a uma cirurgia bem sucedida, para retirada das partes fetais em seu organismo. Diante do que foi abordado, vê-se a importância do tema em questão, tanto por se tratar de uma condição extremamente incomum, quanto por ser de grande valia para a comunidade científica da área da saúde.
