Abstract:
Zielsetzung: Beurteilung der inflammatorischen Reaktion auf intravitreal appliziertes Aflibercept (IVA) oder Ranibizumab (IVR) bei Patienten mit altersbedingter Makuladegeneration (AMD). Methoden: Insgesamt 60 Augen von 60 Patienten mit neovaskulärer AMD und 30 Augen von 30 nach dem Lebensalter gepaarten gesunden Personen als Kontrollgruppe wurden in diese prospektive, vergleichende Beobachtungsstudie eingeschlossen. Die AMD-Patienten wurden im Verhältnis 1:1 mit IVA oder IVR behandelt. Der Tyndall-Effekt in der Vorderkammer wurde an Tag 0, 1 und 30 mit dem Laser-Flare-Meter Kowa FM-600 gemessen (Kowa Company, Ltd., Tokio, Japan). Der mittlere Flare-Wert mit Standardabweichung wurde als Photonenzahl pro Millisekunde erfasst. Ergebnisse: An der Studie nahmen 51 (56,7%) Männer und 39 (43,3%) Frauen teil; das mittlere Lebensalter betrug 72,7 ± 7,5 Jahre. Die mittleren Flare-Werte des Kammerwassers zu Studienbeginn, an Tag 1 und an Tag 30 betrugen in der IVR-Gruppe 7,08 ± 2,44, 7,23 ± 2,56 und 6,99 ± 2,29, in der IVA-Gruppe 6,87 ± 3,18, 6,86 ± 3,19 und 6,53 ± 2,79 und in der Kontrollgruppe 6,4 ± 3,29, 6,41 ± 3,06 und 6,42 ± 3,05. Bei den Flare-Werten zu Studienbeginn bestanden keine statistisch signifikanten Unterschiede zwischen den drei Gruppen (p = 0,666). Bei der Untersuchung an Tag 1 zeigte sich eine leichte, jedoch nicht signifikante Zunahme des Flare in der Ranibizumab-Gruppe. Ein signifikanter Unterschied bei den Flare-Werten bestand weder innerhalb der mit Ranibizumab oder der mit Aflibercept behandelten Gruppe (p = 0,768 bzw. p = 0,387) noch zwischen den Gruppen (p = 0,635). Weder vor noch nach den intravitrealen Injektionen mit Ranibizumab bzw. Aflibercept waren signifikante klinische entzündliche Reaktionen festzustellen. Schlussfolgerung: Die mit Aflibercept oder Ranibizumab behandelten Augen mit neovaskulärer AMD zeigten kurzfristig keine signifikante intraokulare Inflammation. Aflibercept besitzt zwar im Vergleich zu Ranibizumab ausgeprägtere immunogene Merkmale, z.B. einen zusätzlichen Fc-Anteil, und ist ein größeres Molekül, verhindert aber wahrscheinlich durch seine stärkere antiinflammatorische Wirkung die Induktion entzündlicher Reaktionen. Zusammenfassung Gegenstand und Ziel Kreuzreaktive Kohlenhydrat-Seitenketten (cross reactive carbohydrate determinants, CCD) führen bei saisonalen In-vitro-Allergietests zu multipositiven Ergebnissen. Diese Bindung von Immunglobulin E gegen CCD von Pollenallergenen (Anti-CCD-IgE) verursacht falsch-positive/klinisch irrelevante Resultate. Ziel dieser Studie war, die Prävalenz von polysensibilisierten Serumproben zu untersuchen und die Auswirkungen eines CCD-Inhibitors/Blockers auf die Ergebnisse von saisonalen Allergietests bei Hunden zu evaluieren. Methoden Die Analyse berücksichtigte Seren von 4614 Hunden, die für einen Allergietest zum Nachweis von IgE gegen saisonale Allergene (ELISA basierend auf der Fc-Epsilon-Rezeptor-Technologie) von Juli 2017 bis Juni 2018 eingeschickt worden waren. Die Proben wurden als polysensibilisierte Seren (Gruppe A) und nicht polysensibilisierte Seren (Gruppe B) klassifiziert. Seren der Gruppe A wurden nach Zugabe eines modifizierten Glykoprotein-Pflanzenextrakts (Blocker) erneut getestet. Um die Auswirkung des Blockers auf jedes Allergen zu bestimmen, erfolgte eine Analyse der Ergebnisse von 96 zufällig ausgewählten Proben vor und nach dem Blocken. Ergebnisse Eine Polysensibilisierung gegen saisonale Allergene zeigte sich bei 818 (17,7 %) Serumproben. Der Blocker eliminierte die Bindung von Anti-CCD-IgE an Allergenextrakte vollständig (49 %) oder erniedrigte die Reaktionsklassen (40 %), was auf das Vorhandensein von klinisch relevantem IgE hinweist. Negative Reaktionen nach Zugabe des Blockers ergaben sich beim 6-Gräser-Mix (29 %), bei Roggen (22 %) und Sauerampfer (20 %) weniger häufig als bei Brennnessel (82 %), Weide (70 %), Birke-Hasel (65 %), Beifuß-Ragweed (63 %) und Spitzwegerich (57 %). Schlussfolgerung und klinische Relevanz Der Blocker sollte bei polysensibilisierten Seren eingesetzt werden, um die Qualität saisonaler In-vitro-Allergietests zu verbessern und die Verwendung klinisch irrelevanter Allergene in der allergenspezifischen Immuntherapie (ASIT) zu vermeiden.

Falsch-positive/klinisch irrelevante Resultate aufgrund der Bindung von Immunglobulin IgE an CCD (Anti-CCD-IgE) wurden bei Pollenallergenen identifiziert. Bisher fehlten Studien über das Phänomen bei Katzen. Das Ziel dieser Studie war, die Prävalenz der Polysensibilisierung in Serumproben und die Auswirkung eines CCD-Inhibitors/Blockers bei multipositiven, saisonalen Allergietestergebnissen bei Katzen zu evaluieren. Material und Methoden Insgesamt wurden 472 Katzenseren berücksichtigt, die für einen Allergietest zum Nachweis von IgE gegen saisonale Allergene (ELISA basierend auf der Fc-Epsilon-Rezeptor-Technologie) von Juli 2017 bis Juni 2018 eingeschickt wurden. Die Proben wurden in die polypositive Gruppe A und die nicht polypositive Gruppe B eingeteilt. Seren der Gruppe A wurden nach Zugabe eines modifizierten Glykoprotein-Pflanzenextrakts (Blocker) erneut getestet. Um die Auswirkung des Blockers auf jedes Allergen zu bestimmen, wurden die Ergebnisse bei 48 zufällig ausgewählten Proben vor und nach dem Blocken analysiert. Ergebnisse Die Polysensibilisierung wurde bei 137 (29 %) Serumproben ohne Abhängigkeit von den Jahreszeiten festgestellt. Durch das Blocken wurde die Bindung von Anti-CCD-IgE an Allergenen eliminiert und das Testergebnis entweder negativ (49 %) oder um 1–4 Reaktionsklassen erniedrigt (41 %), was die zusätzliche Präsenz von klinisch relevanten allergenspezifischen IgE anzeigt. Der Anteil an negativen Testreaktionen nach dem Blocken war beim 6-Gräser-Mix (31 %), Roggen (23 %) und Sauerampfer (25 %) viel geringer im Vergleich zu Weide und Birke-Hasel (67 %), Brennnessel und Beifuß-Ragweed (65 %) sowie Spitzwegerich (54 %). Schlussfolgerung und klinische Relevanz Für eine verbesserte Qualität des In-vitro-Allergietests auf IgE gegen saisonale Allergene sollte das Blocken bei multipositiven Ergebnissen angewendet werden, um zu vermeiden, dass bei der allergenspezifischen Immuntherapie (ASIT) klinisch nicht relevante Allergene zum Einsatz kommen.

Abstract Indolent Non-follicular non-Hodgkin Lymphoma (NFo-NHL) is a group of relatively frequent lymphoproliferative diseases, nevertheless extended clinical and prognostic studies are still lacking. In 2002 the Gruppo Italiano Studio Linfomi (GISL) initiated a LL02 prospective multicenter phase II trial, with the aim to evaluate the efficacy and safety of FC combination in the first-line therapy of NFo-NHL patients younger than 70 years. Between July 2002 and September 2006, 58 adult patients (35 males and 23 females, median age 64 yrs, range 40–75) affected by NFo-NHL in active disease phase, were consecutively enrolled in 12 GISL Hematological Centres. Patients were treated with a dose of 25 mg/mq Fludarabine plus 250 mg/mq Cyclophosphamide administred intravenously daily for 3 days; each cycle was repeated every 28 days for 6 courses. During the treatment patients received oral thrimethoprim-sulphametoxazole prophylaxis. After the intermediate evaluation, 48/58 patients (82.8%) had an objective response (ORR) with a 20.7% of complete remission (CR) plus 62.1% of partial remission (PR); at the final evaluation the ORR percentage was 84.5% with a 41.4% of CR (24 pts) and 43.1% of PR (25 pts); three patients were in progressive disease (5.2%) and one in stable disease (1.7%). The median overall survival (OS) was not reached with an 88% and 84% at 12 and 24 months; the progression free survival (PFS) was 89% and 77% and the event free survival (EFS) was 81% and 66% at 12 and 24 months respectively.About the toxicity profile, the major toxicity was hematological with a 18% cases of WHO grade III or IV anemia, 40% leucopenia, 33% neutropenia and 10% piastrinopenia. The 12% of patients had an infective episode wich a 7.7% of WHO grade III–IV.In conclusion the FC chemotherapy is a useful chance for advanced untreated non follicular low-grade NHL, with an optimal ORR, CR and PFS. The crucial point of FC remains OS, that not seems to be significantly improved in comparison with fludarabine alone or with standard therapy, even though the better quality of responses; Rituximab plus FC association is growing in literature as the probably key to find a real improvement also in this aspect.

No Brasil, existe uma ampla disponibilidade de cultivares de feijoeiro de vários grupos comerciais. Nesse sentido, estudos para avaliar o potencial produtivo dos genótipos são necessários para a indicação das melhores cultivares aos produtores. Objetivou-se avaliar o desempenho agronômico e qualitativo de cultivares de feijoeiro dos grupos comerciais carioca e especial e indicar os melhores genótipos dentro de cada grupo. O experimento de campo foi conduzido na Unesp, Jaboticabal, SP, na safra de inverno de 2017. Foram utilizadas dez cultivares de feijoeiro comum, sendo cinco do grupo comercial carioca: BRS Estilo, BRS MG Majestoso, BRS Ametista, BRS Notável e BRS Cometa e cinco do grupo comercial especial: BRS MG Realce, BRS Embaixador, BRS FC 305, BRS Executivo e BRS Ártico. O delineamento experimental foi o de blocos casualizados, com quatro repetições. Os resultados foram submetidos à análise de variância (Teste F) e as médias comparadas pelo teste de agrupamento de Scott-Knott a 5% de probabilidade. Para as características de desempenho agronômico, a cultivar BRS MG Majestoso se destacou dentro do grupo comercial carioca e as cultivares BRS Embaixador e BRS Executivo dentro do grupo comercial especial. Na qualidade dos grãos, se destacaram as cultivares BRS Estilo no grupo comercial carioca e a BRS Embaixador no grupo comercial especial.

ZusammenfassungDiese Arbeit gibt eine Übersicht über die Gruppe blasenbildender Autoimmundermatosen und stellt deren wichtigste Vertreter, das bullöse Pemphigoid, den Pemphigus vulgaris sowie den Pemphigus foliaceus, vor. Die häufigste der insgesamt seltenen blasenbildenden Autoimmundermatosen ist das bullöse Pemphigoid (BP). Es betrifft vor allem Patienten jenseits des 60. Lebensjahrs. Charakterisiert ist es typischerweise durch das Auftreten praller Blasen, die mit einem heftigen Juckreiz einhergehen. Ein längeres, sogenanntes prämonitorisches Stadium ohne Blasenbildung ist nicht ungewöhnlich. Es gibt außerdem Varianten mit anderem Erscheinungsbild wie das lokalisierte BP. Der diagnostische Goldstandard ist die direkte Immunfluoreszenzmikroskopie einer periläsional entnommenen Hautbiopsie, welche lineare Ablagerungen von IgG und C3 an der Basalmembran zeigt. Vervollständigt wird die Diagnostik durch die indirekte Immunfluoreszenzmikroskopie sowie weiterführende ELISA-Untersuchungen, mittels derer zirkulierende Autoantikörper im Patientenserum nachgewiesen werden können. Das wichtigste Zielantigen ist BP180, ein hemidesmosomales, von Keratinozyten exprimiertes Protein. Ergänzend kann eine histopathologische Untersuchung erfolgen, die allerdings nur Hinweise zur Spaltebene und zum (meist Eosinophilen-dominierten) Infiltratmuster geben kann und alleine nicht zur Diagnosestellung ausreicht. Die Pathogenese des BP ist Gegenstand der wissenschaftlichen Diskussion. Medikamente wie Dipeptidylpeptidase-4-Inhibitoren können Auslöser sein; Assoziationen zu neurologischen Erkrankungen finden sich häufig. Entsprechend aktueller Leitlinien wird das BP mit topischen bzw. systemischen Glukokortikoiden ggf. in Kombination mit Doxyzyklin, Dapson oder einem Immunsuppressivum behandelt. Bei Therapieresistenz werden intravenöse Immunglobuline oder der anti-CD-20-Antikörper Rituximab eingesetzt. Aufgrund einer vergleichsweise hohen Mortalität bedingt durch Patientenalter und iatrogener Immunsuppression werden neue Therapieansätze gesucht. Fallserien, Kohortenanalysen und Phase 1-/2-Studien mit anti-IgE-Antikörpern und Inhibitoren der eosinophilen Granulozyten sowie des Komplementsystems zeigen teils vielversprechende Effekte. Die wichtigsten Vertreter der Pemphiguserkrankungen sind der Pemphigus vulgaris (PV), der Pemphigus foliaceus (PF) und der sehr seltene paraneoplastische Pemphigus (PNP). Klinisch präsentiert sich der PV mit meist enoralen Schleimhauterosionen und teilweise zusätzlichen Erosionen an der freien Haut. Der PF manifestiert sich nur an der freien Haut. Wie beim BP wird die Diagnose mittels direkter Immunfluoreszenzmikroskopie gestellt, welche beim PV und PF netzförmige Ablagerungen von IgG und C3 innerhalb der Epidermis zeigt. Die häufigsten Zielantigene sind die Desmogleine 1 und 3. Genetische Prädispositionen für den PV und PF sind bekannt und Grund für eine global unterschiedliche Häufigkeit. Der PNP ist immer mit einer malignen Erkrankung assoziiert und von einem progredienten Verlauf mit hoher Mortalität geprägt. Therapeutisch erfordern die Pemphiguserkankungen oft aggressivere Ansätze als das BP. Neben systemischen Glukokortikoiden und Immunsuppressiva wird für den PV und PF der anti-CD-20-Antikörper Rituximab empfohlen. Neue Therapieansätze sind die Hemmung der Bruton-Tyrosinkinase sowie des neonatalen Fc-Rezeptors (FcRN). In einer Phase 2-Studie zeigte Efgartigimod, ein Antagonist des FcRN, eine hohe Therapieeffektivität für Patienten mit PV und PF.

Abstract Adoptive T-cell therapy with single-chain variable fragment (scFv)-derived chimeric antigen receptors (CARs) has transformed cancer therapy. Nevertheless, treatment failure can occur and is often associated with loss of the targeted antigen-an outcome affecting nearly 65% of patients that have failed CD19-targeted CAR T cell therapy (1). Targeting more than one tumor antigen expressed on the surface of tumor cells to mitigate antigen loss is a strategy that has been tested preclinically with CAR-T therapy. This approach is challenging due to the complexities of validating numerous CAR constructs or generating functional CARs which contain several tandem scFvs. The Antibody-Coupled T-cell Receptor (ACTR) platform is a universal, engineered T-cell therapy technology developed to mediate anti-tumor activity in combination with tumor-targeting antibodies. The ACTR construct, derived from human CD16 and coupled to T-cell signaling domains, is designed to engage the Fc domain of therapeutic antibodies, resulting in a novel platform for T-cell targeted cancer therapy. In contrast to CAR T-cell constructs that are restricted to a single antigen, the ACTR platform is highly adaptable, which can be targeted against a diverse set of tumor antigens-thus circumventing the need to generate and characterize multiple CAR-T therapeutics. We first determined whether ACTR expressing T cells could be combined with a diverse array of tumor-targeting antibodies directed against B cell malignancies. Using cell lines derived from B cell lymphomas or multiple myeloma, we found that ACTR-expressing T cells could be activated with antibodies against multiple B-cell targets including CD19, CD20, CD22, CD38, and CS1. One approach to address CD19 antigen loss in patients relapsing following CD19 CAR-T therapy is to treat with a CD22 CAR-T therapy (2). Given that ACTR can be paired with single antibodies as described above, a flexible sequential administration of targeted antibodies could be utilized. However, we also sought to determine whether combinations of antibodies against CD19 and CD22, when used concurrently, could improve ACTR-activity. We found that dual antigen targeting with anti-CD19 and anti-CD22 antibodies could enhance both ACTR-mediated cytotoxicity and cytokine production in an antibody dose-dependent manner. Taken together, our results provide preclinical evidence for ACTR as a universal, chimeric receptor that could engage multiple tumor antigens with potential to improve patient outcomes by eliminating antigen negative relapse. IFITM3 encodes a surface receptor, basally expressed on the plasma membrane, that is associated with known B cell co-receptors including CD19, CD81 and CD21 on mouse and human B cells. However, besides its interaction with CD19, specific functions of Ifitm3 in normal B cells development and, potentially leukemogenesis, are not known. Recent approaches based on CD19-specific chimeric antigen receptors (CAR) have achieved spectacular successes in eliminating pre-B ALL cells based on surface expression of CD19 (e.g. Grupp et al., N Engl J Med 2013). However, in some cases, CD19 CAR treatment was followed by ALL relapse developing from a clone that lacked CD19 surface expression. Therefore, we studied factors that can potentially regulate CD19 surface on normal and leukemic pre-B cells. Results: Studying IFITM3 mRNA levels in ALL cells at the time of diagnosis in clinical trials for childhood (COG P9906) and adult ALL (ECOG E2993), we found that higher than median expression levels of IFITM3 predicted shorter overall and relapse-free survival (P=0.014). In addition, patients with higher than median IFITM3 mRNA levels at the time of diagnosis were significantly more likely to experience ALL relapse and had a higher risk of a positive MRD status at the end of induction chemotherapy. To study the function of Ifitm3 in a model for human pre-B ALL, pre-B cells from Ifitm3-/- mice were transformed with BCR-ABL1. Strikingly, deletion of IFITM3 resulted in near-complete loss of CD19 expression on the surface of normal and leukemic pre-B cells. Besides loss of surface expression, loss of Ifitm3 also caused impaired phosphorylation of CD19-Y513, which mediates downstream activation of PI3K-AKT signaling in both B cell progenitors and pre-B ALL cells. Reconstitution of IFITM3 in patient-derived pre-B ALL cells rescued both CD19 expression and increased phosphorylation of CD19-Y513 together with downstream SRC, SYK, PI3K signaling. Co-immunoprecipitation experiments revealed that the cytoplasmic tail of IFITM3 interacts with CD19, LYN, SYK, PI3K p110δ and AKT, pointing to a central role of IFITM3 in regulating CD19/PI3K-AKT signaling in pre-B cells and pre-B ALL. Deletion of Ifitm3 also resulted in significant cell cycle arrest in the G0/G1 phase (P<0.001) and increased protein levels of p53 and p21 compared to wild-type pre-B ALL cells. Moreover, treatment with low dose adriamycin (25 ng ml-1) for induction of modest DNA damage significantly induced cellular senescence in Ifitm3-/- but not wild-type pre-B ALL cells. Consistent with Ifitm3-mediated proliferative defects of Ph+ ALL cells, Ifitm3-/- Ph+ ALL cells exhibited reduced self-renewal capacity (P=0.0004) decreased colony in colony forming assay compared to wild-type cells. Furthermore, Ifitm3 deficient B cell progenitors also showed significant inhibition of proliferation with inhibition of phosphorylation of Stat5Y694and c-Myc expression. 4-hydroxytamoxifen (4-OHT)-inducible activation of CD19 rescued proliferative defects in Ifitm3 deficient B cell progenitors with release from G0/G1 cell cycle arrest associated with upregulation of phosphorylation of Stat5 Y694 and c-Myc expression. In addition, we found that CD19 positively regulates the surface expression of IL7R in B cell progenitors. Interestingly, CD19low population in Ifitm3 deficient B cell progenitors showed lower surface expression of IL7R compared to CD19high population. In contrast to B cell progenitors, forced expression of CD19 did not increase the proliferation of wild-type Ph+ ALL cells. Surprisingly, upregulation of CD19 induced apoptosis in Ifitm3-deficient Ph+ ALL cells with significant inhibition of BCR-ABL1 activity, phosphorylation of Stat5Y694and Bcl2 expression. Conclusion: These findings identify novel role of IFITM3 in regulating CD19 surface expression and CD19-Y513 mediated PI3K-AKT signaling in pre-B cells and pre-B ALL. Preliminary experiment showed that agonistic antibodies against IFITM3/CD225 increased CD19/PI3K-AKT signaling and proliferation in pre-B ALL cells and future studies will elucidate whether blocking antibodies (e.g. decoy Fc-fusion molecules) will have useful effects in targeting CD19/PI3K-AKT signaling in human pre-B ALL. A finiteness condition is a group-theoretical property which is possessed by all finite groups: thus it is a generalization of finiteness. This embraces an immensely wide collection of properties like, for example, finiteness, finitely generated, the maximal condition and so on. There are also numerous finiteness conditions which restrict, in some way, a set of conjugates or a set of commutators in a group. Sometimes these restrictions are strong enough to impose a recognizable structure on the group. R. Baer and B.H. Neumann were the first authors to discuss groups in which there is a limitation on the number of conjugates which an element may have. An element x of a group G is called FC-element of G if x has only a finite number of conjugates in G, that is to say, if |G : CG(x)| is finite or, equivalently, if the factor group G/CG(⟨x⟩G) is finite. It is a basic fact that the FC-elements always form a characteristic subgroup. An FC-element may be thought as a generalization of an element of the center of the group, because the elements of the latter type have just one conjugate. For this reason the subgroup of all FC-elements is called the FC-center and, clearly, always contains the center. A group G is called an FC-group if it equals its FC-center, in other words, every conjugacy class of G is finite. Prominent among the FC-groups are groups with center of finite index: in such a group each centralizer must be of finite index, because it contains the center. Of course in particular all abelian groups and all finite groups are FC-groups. Further examples of FC-groups can be obtained by noting that the class of FC-groups is closed with respect to forming subgroups, images and direct products. The theory of FC-groups had a strong development in the second half of the last century and relevant contributions have been given by several important authors including R. Baer, B.H. Neumann, Y.M. Gorcakov, Chernikov,L.A. Kurdachenko, and many others. We shall use the monographs , as a general reference for results on FC-groups. The study of FC-groups can be considered as a natural investigation on the properties common to both finite groups and abelian groups. O Sistema Nervoso Autônomo (SNA) modula a Frequência Cardíaca (FC), que pode ser estimulada e melhorada por meio da atividade física. A prática regular de exercícios aeróbicos como Ginástica Aeróbica (GA) propicia mudanças e adaptações ao sistema cardíaco capazes de resultar em melhoria da saúde e diminuição de riscos de doenças cardiovasculares como a Hipertensão Arterial Sistêmica (HAS). Nesta perspectiva, o presente estudo teve como objetivo analisar a modulação do sistema nervoso autônomo por meio da variabilidade da frequência cardíaca (VFC) em mulheres com e sem histórico familiar positivo de HAS praticantes de ginástica aeróbica. Participaram 65 mulheres com idade entre 18 e 35 anos, separados em dois grupos: filhas de normotenso (FN) e filhas de hipertensos (FH). A modulação autonômica cardíaca foi avaliada utilizando o registro do intervalo R-R (ms) pelo período de 15 minutos. Quanto aos valores encontrados, no que diz respeito ao intervalo de pulso (IP), o grupo FN apresentou aumento quando comparado à do grupo FH (p=0,045). Não foram observadas diferenças significativas em nenhuma outra variável (pressão arterial, SD, BF, AF, %BF, %AF e BF/AF). Nossos achados indicam que a prática de exercícios aeróbicos como a GA proporciona melhoria nas funções vasomotora e FC gerando boas adaptações do SNA do grupo FH, ao suscitar similaridades com o grupo FN como consequência.

Abstract:
O objetivo desta pesquisa é descrever o uso da ventilação não invasiva (VNI) em pacientes no pós-operatório de transplante hepático pediátrico. Para tal, realizou-se um estudo prospectivo realizado na Universidade Federal de São Paulo. Critérios de inclusão: pacientes (0-18anos) que evoluíram com quadro de insuficiência ventilatória aguda (IVA) após a extubação. Protocolo: VNI (bilevel) com os parâmetros: pressão inspiratória 8-12cmH2 O; pressão expiratória final 3-5cmH2 O; FR 8-12cpm. Variáveis coletadas antes e 2 horas após o início da VNI: FR, FC e SpO2 ; gasometria arterial; calculada as relações PaO2 / FiO2 , SpO2 /FiO2 , índice de oxigenação (IO2 ) e aplicada escala HACOR. Estudo aprovado pelo Comitê de Ética e Pesquisa da instituição. Foram inclusas 15 crianças que evoluíram com IVA, submetidas à VNI. Gasometria arterial 2hs após VNI (sucesso vs falha): pH 7,40 ± 0,05 vs 7,32 ± 0,09 (p=0,07); PaCO2 38,5 ± 16,24 vs 42,2 ± 10,76mmHg (p=0.162); PaO2 98,16 ± 21,65 vs 93,08 ± 53mmHg (p=0.801); Sinais vitais 2h após VNI (sucesso vs falha): FR 36,1 ± 12,1 vs 47 ± 20,89ipm (p=0.247); FC 126,11 ± 20,9 vs 122,2 ± 10,85bpm (p=0.706); Relação SpO2 / FiO2 antes e 2hs após VNI (grupo sucesso) 259,67 ± 89,64 vs 267,45 ± 90,63 (p=0.049); IO2 antes e 2hs após VNI (grupo sucesso) 8,70 ± 4,19 (p=0.039); Escala HACOR 2,3 ± 2,11 vs 7,80 ± 3,03 (p=0.001). Conclui-se que a escala HACOR foi preditora de falha da VNI após a segunda hora, com ponto de corte de 5. A melhora dos IO2 , relação SpO2 /FiO2 e escala HACOR tiveram relação com sucesso da VNI.

Abstract:
El modelo educativo ha expeirmentado transformaciones novedosas entre ellas la introducción de nuevas metodologías pedagógicas como es el Flipped Classroom (FC). FC es una herramienta que proporciona una mayor autonomía para el alumno, así como una mayor asimilación de conocimientos. El objetivo principal del estudio es la aplicación de FC en las prácticas, para una mayor adquisición de competencias básicas de aplicación de conocimientos, comunicación e interpretación de resultados para emitir juicios. Los participantes fueron los alumnos de nutrición y dietética, y las prácticas se realizaron de dos modos;en un grupo se impartieron las prácticas con la de metodología de método tradicional (MT) y en otro grupo la aplicación de FC. Al final de las prácticas se realizó un cuestionario para la evaluación de la adquisición de conocimientos y el grado de satisfacción. Los resultados obtenidos mostraron un aumento significativo en el máximo número de preguntas correctamente contestadas (FC) y un mayor grado de satisfacción de los alumnos que realizaron las prácticas mediante FC. Como conlusión podríamos indicar que la aplicación de FC en ciencias de la salud presenta mejores resultados en la adquisición de conocimientos que las metodologías didácticas magistrales.

Introdução: As espécies de peixes do gênero Rineloricaria apresentam 76 espécies válidas que sob o ponto de vista citogenético tem sido pouco investigada, contudo, apesar de escassos os estudos demostram que grande variabilidade cromossômica, com número diplóide variando de 2n=36 a 2n=70, além disso, polimorfismos cromossômicos numéricos e estruturais êem sido observados em algumas espécies do gênero, como na espécie Rineloricaria pentamaculata, com maior número de populações estudadas até o presente momento. Objetivo: Desta forma, o presente estudo tem por objetivo realizar uma revisão da literatura sobre os estudos citogenéticos conduzidos na espécie R. pentamaculata. Materiais e métodos: Foi realizada uma busca nas bases de dados PubMed, Google Acadêmico, Scientific Electronic Liberary Online (ScieELO), sendo encontrados 7 estudos (artigos, dissertações e teses em inglês e português) lidos na íntegra e compiladas informações referentes a local de coleta (nome do rio, ribeirão ou córrego), bacia hidrográfica e estado da federação, com relação aos dados citogenéticos foram reunidas informações sobre: número diplóide (2n), número fundamental (NF), fórmula cariotípica (FC), sistema de NOR e par portador da NOR (Ag-NOR e DNA 18S), quantidade e par portador de sítios de DNAr 5S e presença de cromossomos Bs. Resultados: Desta forma, as diferentes populações de R. pentamaculata apresentaram número diplóide variando de 2n=54 a 2n=58 cromossomos, além de diferenças nas fórmulas cariotípicas, número fundamental, sistema de NOR (simples e múltiplo) e presença de cromossomos B, sendo que a maioria dos estudos foram realizados em populações pertencentes a bacia do Alto Paraná. Contudo, a maioria das populações apresentaram características citogenéticas similares exibindo 2n=56 cromossomos distribuídos em 8m/sm+48st/a (número fundamental igual a 64) e sistema NOR simples revelado por hibridização fluorescente in situ (FISH) com sonda de rDNA 18S, nitrato de prata e banda C positiva, tais características foram consideradas conservadas para a espécie. Conclusão: Todos os estudos revisados sugerem que os rearranjos cromossômicos robertsonianos provavelmente contribuíram a evolução cariotípica da espécie, desta forma, discutimos aspectos relacionados a este e outros fatores associados a evolução do grupo.

Introdução: Adolescentes e jovens demandam novos modos de produzir saúde. O ciclo de vida desses indivíduos, particularmente saudáveis, evidencia os agravos em saúde decorrentes de hábitos e comportamentos que levam a vulnerabilidade dos mesmos. Objetivos: Promoção da saúde, educação sexual e reprodutiva para jovens adscritos na área de abrangência de duas Equipes de Saúde da Família de um município do norte de Minas Gerais. Materiais e métodos: No município, o atendimento de jovens na faixa etária de 15 anos, denominado hebicultura, é feito exclusivamente pelo Farmacêutico. No momento da consulta, utiliza-se o método SOAP. No item Subjetivo, questiona-se sobre sinais/sintomas agudos; investigam-se doenças e/ou agravos anteriores e histórico familiar; verifica-se aspectos socioeconômicos; a relação/vínculo familiar, com os pares e escolar; os aspectos comportamentais, violências, dentre outros. No item objetivo, se procede com o registro da antropometria (peso, altura e IMC); sinais vitais (PA, FR, FC, Tax); registro dos resultados de exames realizados anteriormente; registro dos medicamentos em uso, verificação e registro da situação vacinal, registro da menarca (meninas) ou espermarca (meninos) e registro do estágio de maturação sexual segundo estagiamento de Tanner. No item avaliação, especifica-se a hipótese diagnóstica ou diagnóstico codificado pela CIAP 2. Por fim, no item Plano, solicitam-se exames de acordo com a necessidade clínica. Caso necessário, é feito o encaminhamento para realização de testes rápidos de Sífilis, HIV e Hepatites virais na própria unidade e prestam-se orientações/condutas quanto ao uso de preservativos, saúde sexual e reprodutiva, além da orientação quanto à prevenção ao uso de álcool, tabaco e outras drogas. Resultados: Os adolescentes pouco comparecem as unidades, mesmo com a marcação de consultas previamente agendadas. Foram realizadas 20 consultas no período de 05/2021 a 08/2021 e a partir destas, destaca-se o grande espaço de tempo na realização de exames laboratoriais, automedicação, falta de dados da menarca e espermarca e pouco conhecimento sobre saúde sexual e reprodutiva. Conclusão: A atuação do Farmacêutico para com os adolescentes influencia de forma incisiva no fazer saúde desse grupo etário e na inclusão da criação e elaboração de ações que os caracterizem como protagonistas sociais.
