Abstract:
In recent years, the Public Choice School scholars argue that public policymaking has its own failures. Public Choice scholars pointed out that the people who make public decisions – politicians or bureaucrats - are in fact just as self-interested as anyone else. They are, after all, the same people; individuals do not suddenly become angels when they get a job in government. We call it ‘democracy’, but actually it is politics, and political interests to colour the whole process. People do not vote at elections out of ‘public interest’, but they vote to promote their own interests. The politicians and bureaucrats also have personal interests of their own. So the Public Choice School economists suggest that it might be better to leave the markets alone, rather than replace market failure by an even worse government failure. Over the last few decades, the Public Choice School’s arguments have had a growing effect to explain real politics. In established democracies, there is more recognition of the private interests of legislators and bureaucrats, and of the need to restrain them. In addition, policies designed to restrain public decision making, inspired by the Public Choice School, are becoming more common. In this essay, I attempt to explain the Public Choice School’s main idea and various issues in a conceptual and critical way.

Abstract:
This paper is part of a research theme to develop methods that enhance risk assessment studies by the use of ‘automated’ failure analysis. The paper presents an approach to mechanical failure analysis and introduces a mechanical failure analysis module that can be used in a virtual reality (VR) environment. The module is used to analyse and predict failures in mechanical assemblies; it considers stress related failures within components, as well as failures due to component interactions. Mechanical failures are divided into two categories in this paper: material failures and interference failures. The former occur in components and the latter happen at the interface between components. Individual component failures can be analysed readily; a contribution of the mechanical failure analysis module is to predict interference failures. A mechanical failure analysis system that analyses and visualizes mechanical failures in a virtual environment has been developed. Two case studies demonstrate how the system carries out failure analysis and visualization as design parameters are changed.

<p><strong></strong><em>This paper aims to find out the types of pragmatic failures that can lead to incorrect understanding of meaning by the audience. Pragmatic failure in translation consists of two failures, namely: pragmalinguistic failure and sociopragmatic failure. Pragmalinguistic failures in translation are analyzed using speech act theory proposed by Searle and deixis theory proposed by Levinson. Sociopragmatic failures in translation are analyzed using the principle of cooperation theory proposed by Grice and the principle of politeness theory proposed by Brown and Levinson. The research method used is descriptive qualitative. One hundred seventy-six pragmatic failures are found in the subtitles of Moonlight movie, which is divided into one hundred thirty-six pragmalinguistic failures and forty sociopragmatic failures. Pragmalinguistic failures that occur are the failures to transfer speech acts and deixis of oral speech into subtitles. Sociopragmatic failures that occur are the failures to transfer principle of cooperation of oral speech into subtitles.</em></p>

Monitoring of dual service failures (e.g., trends in service failures and consecutive service failures) in business is emphasized for service quality management. Previous studies analyzing negative online reviews to conduct dual service failure monitoring from a managerial perspective are scarce. Numerous negative online reviews are useful sources for dual service failure monitoring because they can be easily collected at a low cost. This article proposes a data-driven approach to monitor service failure trends and consecutive service failures from negative online reviews. In the proposed approach, first a classifier is developed to categorize newly collected negative reviews into service failures by Latent Dirichlet allocation. Subsequently, a threshold value is provided to identify a new type of service failure, which was not achieved previously using a control chart. Finally, the probability of consecutive service failures is obtained by association rule mining. A case study of Uber is conducted to validate the proposed approach. The results exhibit that the proposed approach can perform dual service failure monitoring. This study can increase marketing intelligence for dynamic management of service failure and allow rapid responses to service failures.

The authors seek to investigate the characteristics of outage-causing faults that can be observed in a short time frame after their occurrence: waveform of the voltages and currents. The aim is to identify which characteristics can be used to estimate the failure type immediately after its occurrence. This paper lays the groundwork to determine which features display a stronger relation to four failure types with the aim of using this information in a later work, not presented in this paper, aimed at designing a reliable failure type estimator from readily available data. This paper focuses on the most common failures of the underground cable MV networks in Portugal: cable insulation; cable joint; secondary substation busbar; and excavation-motivated failures. A set of 206 waveform records of real underground MV network failures was available for analysis. After investigating the waveforms, the authors identified seven waveform characteristics which can be used for failure type estimation. Fault type characteristics can be used to distinguish secondary substation failures from the remaining failure types. Fault evolution does not yield relevant information. Fault self-extinction phenomenon was not observed in excavation-caused failures. There are differences for self-extinction characteristics between secondary substation busbar failures and the cable insulation and joint failures. Fault inception instant and arc voltage are two characteristics which are shown to have a promising merit to the identification process of failure types. Finally, fault intra-cycle repetitive extinction results have been found to be very similar for cable insulation failures and joint failures, but otherwise different regarding the remaining failure types.

This study was conducted to investigate the effects of display failures and rotation of dot-matrix symbols on visual search performance. The type of display failure (cell, horizontal line, vertical line), failure mode (ON, failures matched the symbols; OFF, failures matched the background), percentage of failures (0, 1, 2, 3, 4%), and rotation angle (0, 70, 105 degrees) were the variables examined. Results showed that displays which exhibit ON cell failures greater than 1% significantly affect search time performance. Cell failures degrade performance more than line failures. Search time and accuracy were best when symbols were oriented upright. The effects of display failures and rotation angle were found to be independent. Implications for display design and suggestions for quantifying the distortion due to rotation are discussed.

The spatial and temporal patterns of water distribution pipe failure in the City of Winnipeg are examined. The failures are shown to be strongly clustered in space, where 22% of the total failures examined occur within 1 m of another failure and 46% occur within 20 m of another failure. A strong temporal clustering is also apparent, with 42% of all failures that occur within 1 m of another found also to occur within 1 day of the initial failure in the group. An exponential decrease in the marginal rates of failure with respect to both the temporal and spatial interval parameter is also observed. Earlier failures in a particular location appear to be an important key to assessing potential failures in that vicinity. These results suggest that a fruitful area for further examination for the reduction of failure rates is the change in the ground conditions resulting from an initial leak and its subsequent repair. As a repository that holds computing facilities, storage facilities, network facilities and other facilities, the Software Defined Data Center (SDDC) can provide computing and storage resources for users. For a SDDC, it is important to provide continuous services for users. Hence, in order to achieve high reliability in Software Defined Data Center Networks (SDDCNs), a network failure recovery method for software defined data center networks (REVERT) is proposed to recover failures in SDDCNs. In REVERT, the network failures that occurred in SDDCNs are classified into three types, which are switch failure, failure of links among switches and failure of links between switches and servers. Specially, except recovering the switch failure and failure of links between switches, REVERT can also recover the failures of links between the switches and servers. To achieve that, a failure preprocessing method used to classify the network failures, a data structure for storing and finding the affected flows, a server cluster agent for communicating with the server clustering algorithm and a routing path calculation method are designed in REVERT. Meanwhile, REVERT has been implemented and evaluated on RYU controller and Mininet using three routing algorithms. Compared with the link usage before recovering the network failures, when there are more than 200 flows in the network, the mean link usages only slightly increase at about 1.83 percent. More importantly, the evaluation results also demonstrate that except recovering switch failures, intra-topo link failures, REVERT has the ability of recovering failures of links between servers and edge switches successfully.

Abstract:
Electrical Submersible Pumps (ESPs) are one of the most widely used artificial lift methods in the petroleum industry. However, ESP failures are unanticipated and common occurrences with significant financial impacts for the operators. Analysis of the ESP performance and failures are essential in its design and optimization. This paper presents a statistical approach for diagnosing and evaluating the root causes of ESP failures. The analysis is based on the field data gathered from the surface and downhole ESP monitoring equipment over five years of production of 10 wells. Electrical failures are the most common general cause of ESP failures, accounting for 61% of all failures, followed by motor failure and gas locking. Specifically, power failure, under-voltage, voltage unbalance, and motor underload are the most common occurrences. The data trends are analyzed for the two weeks before each specific failure, and conclusions are made on the warning signs to predict failures. In addition, a Weibull statistical analysis model is constructed to evaluate the reliability features and estimate the ESP failure probability, allowing operators to perform preventive maintenance. The results provide guidelines for ESP operations and contribute to reducing or preventing ESP downtimes and operating costs.

Abstract:
Purpose This study aims to identify the influences that lead to better memorability of a service by focusing on type of service failure, recovery condition and frequency of occurring. Design/methodology/approach This study used a quasi-experimental design in which customers answered questions about a restaurant they had recently patronized and then evaluated experimentally generated failure and recovery scenarios. Two follow-up contacts were made (by phone and e-mail) to assess their memory of the imagined service failures stimulated by the scenarios. Participants were asked how clearly and vividly they could recollect the service failure and to indicate their behavioral intentions at the time of recall. Findings The type of service failure and the subsequent recovery efforts significantly affect whether negative service experiences are memorable. Specifically, individuals showed a higher likelihood of vividly recalling a core service failure than an interactional one. Moreover, service recoveries were found to be helpful in decreasing the memorability of service failures, and that they were effective in decreasing the resulting negative customer behavioral intentions (i.e. switching behaviors and negative word-of-mouth). However, frequently occurred service failures did not significantly influence the memorability of the failures. Practical implications The current study suggested what characteristics of service failures and situations lead to strong memorability and significantly affect future behavior. Thus, the findings provide important implications for avoiding and handling the failures that trigger strong memorability. Originality/value Previous researchers have emphasized on the importance and urgency of preventing critical service failures. However, it is still unclear what type of service failures and/or factors are critical ones. The current study expands the knowledge by incorporating service failures with memory and investigates the characteristics of memorable service failures, which are likely to be remembered more vividly.

Abstract:
This thesis aims to give an understanding of what is behind a failure in a startup industry. To understand why failures are needed for a success, the serial entrepreneur process has to be clarified at an individual level. In the failure process, under analysis are the main phases of turning a failure into a success and the outcomes which should be used as the next venture foundations. This study also aims to give basic tools for entrepreneurships on how to proceed after a failure at a personal level so that the entrepreneurs would be able start a new venture having a positive experience from the last failure. The study begins with a literature review in which concepts of entrepreneurial learning and theoretical failure process model are presented. This created model is then tested in the empirical part. The method used in this study is done as an abductive approach using exploratory research design with a cross-sectional study method to collect and analyze qualitative data. The data in the empirical part has been collected with face-to-face and telephone interviews with professional serial entrepreneurs. The theoretical failure process model created in the literature review part was successfully tested in the empirical part. This process model could be used as the first step in any failure analyses as a tool for an entrepreneur or a manager. Going forward is much easier after analyzing the true reasons behind a failure. Finding root causes for a failure has provided learnings which are key tools for the future. The scientific results of this study show that every entrepreneur that was interviewed has had multiple setbacks as failures during their career. However, they have managed to separate their professional and private lives so that if and when a failure has happened, financial, social and physical costs have been less significant. When these negative costs have been minimal ones, the entrepreneurs have had no or only minor grief before being able to analyze true reasons behind a failure and gaining learnings for the next venture. Learning from past failures, they have been able to avoid similar mistakes happening again. Furthermore, low grief has allowed for the recovery time from failures to reduce significantly. The main managerial finding was that internal motivation is one of the root causes behind the entrepreneur having strength and motivation to continue despite failures. The internal motivation for their actions follows every step from starting a venture to failure analyses, all the way to learning and continuing to the next venture. In some cases, when that motivation was lost, even partly, entrepreneurs were noted to exit from that venture. The purpose of this study was to give understanding of why a failure should be accepted as a natural and sometimes needed, but not wanted, step for learning in a long perspective. The purpose was not to give detailed tools for every reasons behind failure, but this work will rather provide basic tools on how to proceed if a failure happens. If the failure acceptance was wider, the outcome would be beneficial on three different levels. To understand the reasons behind needing failures for success, we need to focus on the entrepreneur’s failure process at an individual level. After understanding that, the process can be scaled up to company and further to community levels.

The purpose of this essay is to propose a comprehensive legal theory of implementation allowing for a deeper understanding of the failures of contemporary Public Administration. Such a theory requires its proper methodology, which is, In fact, the missing link between administrative law and the so-called new sciences. The thesis takes the view that the systemic approach satisfies the above requirements, for it makes possible the broader concepti6n of law, viewed within Its social context. It also makes salient the decision-making element inherent in the implementation process. In this way, it enables the researcher to identify and interrelate important but latent factors of implementation failure, neglected or even overlooked by purely legal or purely empirical implementation studies. This deeper understanding of implementation, drawn from the relevant theoretic systemic models, is a necessary prerequisite for a sound policy making, avoiding the pitfalls of implementation failure. On the other hand, the usefulness of the proposed comprehensive legal study of implementation is shown in the Second Part of the thesis. For this purpose, a characteristic case of administrative failure has been selected and analyzed on the basis of the relevant theoretical model proposed in Part One. The case refers to the failure of urban policy in Greece as expressed in the phenomenon of unauthorized development. It is a case worth studying, since the failure is due to a multiplicity of factors other then the relevant legislation (statute of 17.7.1923), which is of a remarkable quality. The models and findings of the study can be applied to any case of implementation failure, thereby facilitating not only diagnosis but prevention of failures as well.

This dissertation research attempts to explore efficient algorithms and engineering methodologies of analyzing the overall reliability and availability of networks integrated with software failures and hardware failures. Node failures, link failures, and software failures are concurrently and dynamically considered in networks with complex topologies. MORIN (MOdeling Reliability for Integrated Networks) method is proposed and discussed as an approach for analyzing reliability of integrated networks. A Simplified Availability Modeling Tool (SAMOT) is developed and introduced to evaluate and analyze the availability of networks consisting of software and hardware component systems with architectural redundancy. In this dissertation, relevant research efforts in analyzing network reliability and availability are reviewed and discussed, experimental data results of proposed MORIN methodology and SAMOT application are provided, and recommendations for future researches in the network reliability study are summarized as well.

Abstract:

The economical importance and value of small businesses is today recognized by scholars as well as government institutes. The small business does not only contribute with a great amount of entrepreneurial activity and innovations but also as a significant tool in creating jobs. The statistics are however displaying a negative trend in the development of small businesses with over 350.000 – 400.000 business closures every year in UK. In Sweden 35,000 new enterprises entered the market in 2001 but only 62% were still active in 2004.

There are two major factors from which all other explanations are derived from when discussing why a company fails which is the external and internal factor. From the failure model created by Sharma and Mahajan and supported by other researchers, it is known that the problem initiating the failure may have been caused by uncontrollable factors. However the most significant factor behind a failure is derived from insufficient and ineffective management in the strategic process.

In order to understand how and most importantly why the top-managers decisions and actions contributes to a business failure the study focused on exploring the strategic process in numerous of failure cases of small businesses. This understanding is further strengthened by considering the limitations and resistances in the strategic process. There is also a link between the crisis management and strategic management which further provides with valuable insights of the process. Four different small businesses were therefore investigated in the report through an inductive and semi structured approach to explore the contexts of the failures in-depth. From the analysis of the empirical data collected from the top-managers and other employees, owners or managers evidence were collected to study the top-managers contribution to the failure.

The most significant contributing factor found in the business failures were the inefficient internal and external assessments. This was further found to be directly linked to the inadequate knowledge and experience possessed by the top-manager and his staff. Nepotism was also a factor that was found to be a very contributing source to the inadequate assessments. The managers staffed by the top-manager possessed a close relationship with the top-manager and may have been hired due to this reason and not based on the required knowledge which was proven to have a significant impact on all the studied cases.

Abstract Failure analysis for Static Random Access Memory (SRAM) is the major activity in any microelectronic failure analysis lab. Originating from SRAM array structure, SRAM failure can be simple as single bit, paired bit or quad bit failures, whose defect is located at the failure location, or complicated as logic type failure involving WL or BL patterns or entire blocks, whose defect is often not at the failure location. For such SRAM logic type failures, failure analysis is more challenging and detailed fault isolation is necessary prior to physical failure analysis. This paper has demonstrated how to use SRAM decoder scheme knowledge, detailed layout tracing and Photon Emission Microscope (PEM) analysis to deal with the challenges and find the root causes for several cases of SRAM logic type failures.

Abstract The 2-bit/cell nitride-trapping device (NROM/Nbit) is an important type of NVM (Non-Volatile Memory). The NROM/Nbit is a potential candidate for replacing FG-type NVM devices below the 45nm node. In this study, electrical and physical failure analyses (PFA) were used. The failure mechanisms and root causes of failure for the test vehicle, a 130nm NBit device, were determined. Because cell dimensions and spacing requirements for high-density memory are quite aggressive, these observations help to optimize process and improve yield.

Abstract Main Steam Stop and Control Valves (MSCVs) are safety critical components in modern steam power plants including large, combined cycle gas turbine (CCGT) plants. Repeated failures of the after seat drain piping at a dissimilar metal weld have occurred at one large CCGT that is regulated by the ISO in AGC and therefore experiences few startups. This paper describes the results from a comprehensive evaluation of contributors to the repeated failures in one of two MSCV including: pipe stress analysis of the drain pipe layout, vibration testing of the MSCV and drain piping system using accelerometers and motion amplified video (MAV) technology and other inspections of the large bore pipe hanger system adjacent to the valves. Forensic investigations by a metallographic laboratory were conducted which provide important conclusions about the root cause of the drain weld failures, material condition and heat treatment procedures. Additional calculations of transient thermal stress were also conducted in accordance with the guidance that is available in ASME Code Section III NB-3650 to address additional stress that is present due to transient temperature conditions at the failed DMW weld where a substantial geometric discontinuity is present between the drain pipe and the downstream forged block valve. Results from plant operating data archives were used to establish process conditions during plant startup and indexing the measured vibration characteristics when the MSCV drains opened and closed. Field NDT (PMI, UT thickness testing) and hardness testing was used to confirm the material in the drain lines components which have not experienced failures. Recommendations were developed from this program that identify the principal cause of the high level of vibration in the affected MSCV, the benefits of modifications to the drain system pipe materials, dimension and layout and the improvement of oversight of heat treatment of future repairs.

Water, sanitation, and hygiene (WASH) failures continue to be discussed mostly off the record, with professionals the world over repeating one another’s mistakes. Failure is difficult to talk about, but WASH failures have negative impacts – money is wasted and sometimes people are harmed. We need to acknowledge that not everything we try will succeed, but that if we learn from one another, we can continuously improve our work. Since 2018, we have attempted to foster this change through the ‘WASH Failures Movement’. This issue of 'Frontiers of Sanitation' is a compilation of what we’ve learned about why WASH failures happen, how we can address them, and how we can facilitate a culture of sharing and learning from failure in the WASH sector.

Abstract:
This paper analyzes five Productive Development Policies (PDPs) implemented in Costa Rica, finding that they are not optimally addressing market failures. Moreover, government failures rather than market failures represent the main justification for PDPs. Even in the presence of market failures, the policy instruments applied are not necessarily the most economically efficient but rather the most politically feasible options. In addition, the lack of policy evaluation and monitoring prevents adjustments and corrections of such policies. Addressing the arguments for policy intervention and incorporating the results of evaluation into policy design and reform are necessary conditions for success. In spite of positive policy outcomes, limitations to enhance competitiveness and create the conditions for productivity growth are still present. An umbrella approach in the case of those PDPs that reinforce each other is necessary for productivity growth.
