Abstract:
Background: Spontaneous luminal recanalisation, and peritoneal fistula formation connecting the lumens of cut ends, are common causes for tubectomy failure. Failures rates are high with Pomeroy and Madlener methods. Uchida or Irvin techniques have lower failure rates. However, these techniques are not commonly practised because of technical complexity and long surgical time. We developed a novel surgical technique, which leaves the cut ends far away and peritonised.Methods: In our hospital, modified Pomeroy method, and Parkland method were used during 2000- 09, and our new method was used during 2010-19. In the new method, 0.5cm of the cut end of the tube was turned inside and tied to the stump in such a way that the stumps get peritonised. The failure rates were compared between the groups.Results: In the first group, 997 tubectomies were performed. Modified Pomeroy method was used in 360 women. Among them, one woman had a tubal ectopic pregnancy, and one woman had intrauterine pregnancy with a failure rate of 0.55%. Parkland method was used in 637 women. Among them, two women had tubal ectopic pregnancies with a failure rate of 0.31%. During the second 10 years, 637 tubectomies were performed using the new technique. None of the women had failure, with a failure rate of 0.0%. This difference is statistically significant with P<001.Conclusions: New tubectomy method is simple, and more effective in preventing recanalization than Modified Pomeroy and Parklands methods.

Abstract:
Portuguese schools have high student failure and early school leaving rates (Pordata, 2017) giving rise to a number of initiatives aimed at their reduction. The “Alternative Curricular Course” (ACC) promotes the learning of basic skills, specifically in Portuguese language and Mathematics, to support logical reasoning and artistic, vocational, and professional development. Its main goal is the fulfilment of compulsory schooling and the reduction of academic failure. Research based on attachment theory (Bowlby, 1969) suggests that different internal working models of attachment are associated with different characteristics of social, academic, emotional, and behavioural competencies that may interfere in the quality of relationships that young people establish in school, especially with teachers, and also influence their academic performance. This study evaluates the relationship between internal working models of students, their perceptions of the quality of their relationships with teachers, and their academic performance using three measures: (i) the “Inventory of Attachment in Childhood and Adolescence” (IACA) measure, (ii) the “Inventory of Parent and Peer Attachment” (IPPA) measure—concerning the attachment to teacher”, and (iii) a socio-demographic questionnaire on a sample of 305 students from the 8th grade of regular education (RE) and the ACC. The results reveal that students on the ACC exhibit a less secure internal working model than students in RE, and that the perception of the quality of the student-teacher relationship, regarding the dimension of acceptance and understanding by the teachers, is associated with a better academic performance. These results align with those of other recent studies in support of the conclusion that the process of attachment has a significant influence on educational contexts, consistent with attachment and related theories.

Clear health benefits are associated with intensive glucose control in type 1 diabetes mellitus (T1DM). However, maintaining near-normal glycemia remains an elusive goal for many patients, in large part owing to the risk of severe hypoglycemia. In fact, recurrent episodes of hypoglycemia lead to ‘hypoglycemia-associated autonomic failure’ (HAAF), characterized by defective counter-regulatory responses to hypoglycemia. Extensive studies to understand the mechanisms underlying HAAF have revealed multiple potential etiologies, suggesting various approaches to prevent the development of HAAF. In this review, we present an overview of the literature focused on pharmacological approaches that may prevent the development of HAAF. The purported underlying mechanisms of HAAF include: 1) central mechanisms (opioid receptors, ATP-sensitive K+(KATP) channels, adrenergic receptors, serotonin selective receptor inhibitors, γ-aminobuyric acid receptors, N-methyl D-aspartate receptors); 2) hormones (cortisol, estrogen, dehydroepiandrosterone (DHEA) or DHEA sulfate, glucagon-like peptide-1) and 3) nutrients (fructose, free fatty acids, ketones), all of which have been studied vis-à-vis their ability to impact the development of HAAF. A careful review of the current literature reveals many promising therapeutic approaches to treat or reduce this important limitation to optimal glycemic control.
