This paper describes reasons why Pakistan & India were unwilling initially to join the said SAARC. The qualitative research method has adopted to evaluate the failure and success of ASEAN & SAARC. ASEAN has shown very rapid growth in regional & global trade; previously mentioned organization achievements are high as compared to SAARC. In fact, the idea was to promote peace, political harmony, cultural exchange and economic development in the region. Pakistan tried utmost to reduce trust defect among the regional countries by utilizing forum of SAARC. The cooperation of South Asian nations is linked with internal & external security of regional countries. The hegemony of India and deep-rooted tension between two countries caused great damage to the smooth functions of SAARC. Kashmir is cause of disagreement between two sates its lingering position created hinders to achieve the goals of SAARC. The issue of so-called cross border terrorism and false flag operations by India has also sabotaged this forum. The Sri Lanka laid formal groundwork in first meeting in Colombo on April, 1981.The series of various meeting was held from 1981 to 1992 for the conceptual framework of SAARC. The foundation of this organization is following three conferences, which were held, between 1947 to 1954: 1)Asian Relations Conference 1947; 2)The Baguio Conference 1950; 3)Colombo Power Conference 1954.

Purpose Despite their high number, most mergers end in failure. Academic studies of how these failures occur have remained rare, first, because of the difficulty of accessing the cases, and second, because of the difficulty of obtaining – for the purposes of qualitative analysis – objective and freely shared perceptions from the stakeholders, who tend to avoid speaking about failure. This is unfortunate, however, as failure can serve as a stimulus for organizational learning and readaptation for the future. Design/methodology/approach The author investigated how an organization managed failure during the post-merger integration stage. The author described the merger of two listed French companies using longitudinal data. Findings This in-depth case study provides new insights into failure during post-merger integration. The paper highlights the complexity of post-merger integration processes and the failures that the integration stakeholders had to address. The author underlined how they recognized failures and put into place solutions. They particularly highlighted two failures and how they were managed by the managers who acted as knowledge brokers within the new organization and by stakeholders who deconstructed the organization to ensure its future. Research limitations/implications The limitations are those concerning a single case study. Practical implications The paper identified trigger events in the merger process that prompted stakeholders to step in and manage and resolve failures during the integration period. Such triggers can be considered as steps for managers and stakeholders to solve organizational issues in the merger process. The paper highlighted the complexity of post-merger integration processes and the failures faced by integration stakeholders. The analysis thus contributes to an inclusive and integrative view of the challenges in this process. Social implications Despite their high number, merger and acquisition failures remain surprisingly high. This paper explored how stakeholders deal with failures by identifying which solutions are best adapted to their organization. Originality/value The case provides a vivid illustration of failure management during a merger process. Theoretical concepts and empirical findings from the literature are combined to present a single consistent picture.

Abstract Objective To summarize the occurrence of operative failures after microwave ablation (MWA) in patients with primary hyperparathyroidism (pHPT), analyze the possible reasons, and explore strategies for preventing and managing these situations. Methods This retrospective study reviewed 91 pHPT patients who underwent MWA from April 2015 to November 2019. A cure was defined as the reestablishment of normal calcium homeostasis lasting a minimum of 6 months. An operative failure was defined as a failure to normalize serum intact parathyroid hormone (iPTH) and/or calcium levels at 6 months or longer. Patients who encountered operative failures were compared with patients who were successfully cured. Results Eighty-eight pHPT patients, consisting of 29 men and 59 women, were finally enrolled. The median follow-up duration was 15.9 months (IQR, 6.1–31.5 months). Seventy-eight patients (78/88, 88.6%) were cured. Ten (10/88, 11.4%) patients experienced operative failure, including 9 persistent pHPT (10.2%) and 1 (1.1%) recurrent pHPT. Small parathyroid nodules (maximum diameter < 0.6 cm) and incomplete ablation were the two key factors leading to operative failure. Of the 9 patients with a maximum nodule diameter less than 0.6 cm, 77.8% (7/9) of them encountered operative failure. Conclusion Operative failure occurred in 11.4% of the pHPT patients who underwent MWA. The possibility of operative failure was increased when the maximum diameter of parathyroid nodule was less than 0.6 cm. Complete ablation could help avoid operative failure. Key Points • Failed to ablate the target lesion and incomplete ablation were the key factors attributed to operative failures. • When the maximum diameter of the parathyroid nodules is less than 0.6 cm, the possibility of operative failure was higher.

Abstract:
Insurance firms are known to have unique financial failure characteristics that affect the financial environment of the countries. Therefore, the purpose of this study is to assess the validity of the model used in predicting the financial failures of insurance companies. The model is believed to help in stabilizing the financial environment of the countries by predicting any collapses in the insurance sector. A discriminate regression technique was used to test 28 indicators chosen from 11 financial failure model parameters. 11 parameters of the model are the following: solvency, profitability, operational capabilities, structural soundness, capital expansion capacity, capital adequacy, reinsurance and actuarial issues, management soundness, capital expansion capacity, earnings and profitability, and liquidity. The results of the study proved that 22 variables from 11 parameters were significant; the study also validated the use of the financial failure model as a stable predictor of the financial failure of ASE insurance firms. The stability of the insurance industry is interpreted through the minimum deviation between the real and measured performances. The deviation was present in 3 out of 95 observations, and it affected only 3 firms out of 19, 1 firm out of 3 turned out to be affected by the risker deviation which is the type II error distorted observation. To conclude, the study by mentioning that insurance firms are not threatened by failure or distress and the financial failure model is a valid prediction model.

Background: Bracket bonding failure is one of the relevant problems in fixed orthodontics therapy, which affects the total treatment and quality of treatment results. The purpose of this retrospective study was to evaluate the frequency of bracket bond failure and find out risk factors. Methods: A total of 101 patients with an age range of 11–56 years were included in this retrospective study and treated for a mean period of 30.2 months. Inclusion criteria were: males and females with permanent dentition and completed orthodontic treatment in both fully bonded dental arches. Risk factors were calculated using binary logistic regression analysis. Results: The overall bracket failure rate was 14.65%. The bracket failure rate was significantly higher in the younger patients’ group (p = 0.003). In most cases, patients experienced bracket failures in the first month of the treatment. Most of the bracket bond failures occurred on the left lower first molar (29.1%) and were twice as common in the lower dental arch (66.98%). Patients with increased overbite had an increased likelihood of bracket loss (p = 0.042). Class II malocclusion increased the relative risk of bracket failure, while Class III decreased the rate of bracket failure, but the difference was not statistically significant (p = 0.093). Conclusions: The bracket bond failure rate was higher in younger patients than in older patients. Brackets placed on mandibular molars and premolars had the highest failure rate. Class II was associated with an increased bracket failure rate. Increased overbite statistically significantly increases bracket failure rate.

Based on more than three hundred transformer accidents and some statistics data related, this paper manages to draw a standard classification table of transformer failure. 5 degrees of failure possibilities are classified in the paper: extremely high, high, general, low, extremely low. Besides, 4 major factors that affects risk of transformer failure severity are studied: direct lossmonitoring levelmaintenance cost and maintenance period. Also, failure severity of each risk source are classified into 5 levels: not serious, mild, ordinary, less serious, and serious, and Extensional Analytic Hierarchy Process (EAHP) and Extensional Project Appraisal Methods are used to evaluate the severity of risk of transformer failures. Last but not least, risk matrix has been built and divided into four parts: high risk area, critical risk area, medium risk area and low risk area. On that top, some maintenance strategies are proposed accordingly.

Background: Flexor tendon repair failures have primarily been attributed to either core suture rupture or core suture pull out. Recent studies have suggested that knot unravelling may also cause failure of a tendon repair. The aim of this study was to investigate the causes of core suture failure in two types of common flexor tendon repairs. Methods: Twenty four cadaver tendons were divided into three groups of eight. Each group tested a specific flexor tendon repair. The repairs tested included an Adelaide repair using 4/0 Ethibond (Ethicon), an Adelaide repair using 4/0 Fiberwire (Arthrex) and the Tsai repair with 4/0 Fiberloop (Arthrex). The repaired tendons were pull-tested to failure. The mechanism of failure, maximum tensile strength and 2 mm gap force were recorded. Results: The predominant mode of failure was by the knot unravelling. This occurred in 50-88% of the tendon repairs. The sequence of failure was initiated with gapping at the repair site followed by failure of the epitendinous suture. Next the core suture knot unravels. Once the knot unravels, the suture thread slips out of the tendon resulting in the repair failure. Failures due to knot slippage occurred at a lower maximum tensile strength in Ethibond and Fiberloop sutures than failure due to core rupture or pull out. However, given the small number of tendons tested, this result was not significant. Conclusions: This study has clearly demonstrated one of the main causes of flexor tendon repair failure in two common repair techniques is knot unravelling.

Nowadays, U-shaped thin-walled concrete girders have been widely applied in the urban construction of rail viaducts in China as well as worldwide. However, the mixed torsional behaviors of these structures are not well understood. In this paper, the mixed torsional behaviors of the U-shaped thin-walled RC girders are theoretically analyzed, and a method predicting failure modes and ultimate torques is proposed. Nonlinear FE models based on ABAQUS to simulate the mixed torsional behaviors are built and calibrated with the test results. Parametric studies considering three crucial parameters (boundary condition, span length-section height ratio, and ratio of longitudinal bars to stirrups) are conducted based on both the above suggested calculating method and the FE modeling. The calculated and the simulated results agree well with each other and with the test results. It is found that the failure modes of the U-shaped thin-walled RC girders under torsion are influenced by all the three parameters. Three kinds of failure modes are observed: flexural failures dominated by warping moment, shear failures caused by warping torque and circulatory torque, and flexural-shear failures in the cases where flexural failure and shear failure appear almost at the same time.

In the present study, possibility of non-surgical endodontic re-treatment for correction of symptomatic endodontic failure with and without periradicular pathosis was evaluated both clinically and radiologically. The main objective of this study was also to assess, verify and establish the non-surgical re-treatment method as primary approach to resolve endodontic failures irrespective of sex. A total 69 cases of endodontic failure with or without periapical pathosis were studied of which 24 (34.78%) were male and 45 (65.21%) were female .The mostly affected teeth were Mandibular 1st molars 19 in number (27.53%) followed by Maxillary central incisors 17 in number (24.63%). The Failure Modes and Effects Analysis (FMEA) method has been widely recognized as a tool that systematically identifies the consequences and failures of the system or process, and reduces or eliminates the chances of the failure. This study applies that method to evaluate the causes of failure in the use of sugarcane machine that have been designed in the previous studies. FMEA approach anticipated the failures at the design stage, so that a more reliable and ergonomic design can be produced for future sugarcane machine. The potential failure identified from the machine consists of capacity issues, machine maintenance, preliminary treatment, and procedures of use. The study found that capacity issues are the priority problems that cause the machine failure. Then, this study proposed some actions to reduce the risk priority number (RPN) on 12 failures.

Mine ventilation system is a repairable system, with the characters of time-varied and randomness. It made us encounter some difficulties when we discussed the system failure evolution process and its operational reliability, as well as when we sought the system reliability parameters developing trends following with ventilation system operation time variation and air adjustment. Firstly, according to the reliability theory, the failure process characteristic values, which affect the ventilation system, were defined. And then, by mean of the analysis of the failure process deterioration of mine ventilation system, the deterioration and its amelioration discriminants are given basing on two parameters’ Weibull process. Finally, on the basis of adaptive neural network technology, the ventilation system failure processes were simulated, the failure process characteristic parameters were determined, and the failure curves were drawn accordingly. The results show that the failure process of the experiment mine ventilation system is a Non-homogeneous Poisson Process; the mean time between failures of the ventilation system is between 160h and 170h; the failure ratio curves present the trends of concave or convex variation when the system failure process deterioration or amelioration.

Abstract. In recent years, heavy rainfall conditions have caused damages around the world. To prevent damages by floods, levees have often been constructed in prone-to-inundation areas. This study performed reliability analyses for the Chiuliao 1st Levee located in southern Taiwan. The failure-related parameters were the water level, the scouring depth, and the in-situ friction angle. Three major failure mechanisms were considered, including the slope sliding failure of the levee, and the sliding and overturning failures of the retaining wall. When the variabilities of the in-situ friction angle and the scouring depth are considered for various flood return periods, the variations of the factor of safety (FS) for the different failure mechanisms show that the retaining wall sliding and overturning failures are more sensitive to the variability of the friction angle. When the flood return period is greater than 2 years, the levee can undergo slope sliding failure for all values of the water level difference. The results for levee stability analysis considering the variability of different parameters could assist engineers in designing the levee cross sections, especially with potential failure mechanisms in mind.

High rainfall leads to flood vulnerability and potentially leads to the failure of road pavement which affects the economic loss for the people. However, such road pavement failures need to be studied more deeply to be able to distinguish whether the failure is solely due to flood natural disaster or other factors, such as overloading and poor construction quality. This research is urgently needed because up to now there is no tool for assessing pavement failure as a tool of decision-making system that is able to distinguish road pavement failure causal factors into category of natural disaster or human negligence factors in accordance with the definition of building failure in Indonesia Law no. 2 year 2017. Assessment of the causal factor of failure is very important because it is closely related to the determination of the parties who must be legally responsible. This research intends to develop methods for assessing the damage and failure of pavement as a tool of rapid and accurate decision-making system to determine the factors of pavement failure. To obtain accurate results, a case study was taken on Kendal Timur road to enable a deep evaluation of the factors that causing failure on the road. The results showed that many factors involved in causing the failure of the road, such as un-functional drainage channel, uncontrolled overload due to the closing of weigh stations around the road, and also low quality of road maintenance work so that repeating similar road damages occurred in the not-too-distant future. Based on functional and structural failure analysis, as much as 71%, 10% and 19% of the road were in failed, critical and not-failed conditions, respectively.

ABSTRACTBank failures affect owners, employees, and customers, possibly causing large-scale economic distress. Thus, banks must evaluate operational risks and develop early warning systems. This study investigates bank failures in the Organization for Economic Co-operation and Development, the North America Free Trade Area (NAFTA), the Association of Southeast Asian Nations, the European Union, newly industrialized countries, the G20, and the G8. We use financial ratios to analyze and explore the appropriateness of prediction models. Results show that capital ratios, interest income compared to interest expenses, non-interest income compared to non-interest expenses, return on equity, and provisions for loan losses have significantly negative correlations with bank failure. However, loan ratios, non-performing loans, and fixed assets all have significantly positive correlations with bank failure. In addition, the accuracy of the logistic model for banks from NAFTA countries provides the best prediction accuracy regarding bank failure.

Sea-going ships are unique systems, and each ship—even those which are mass-produced—are different. Once in service, they are subjected to unique environmental exposure due to a variety of factors, including, but not limited to, their mode of operation, sailing area, cargo, hydrometeorological conditions, crew training, etc. This makes it very difficult, if not impossible, to compare individual units. The aim of this study is to present the damage data and analysis of a selected vessel—a complex technical system—during its first year of operation. To that end, the paper analyses the unreliability of a bulk cargo ship’s technical and energetic system components during its first year of operation. The paper also introduces the failure susceptibility of its technical systems, defines concepts of wear and failure and describes the object of analysis. Observed failures in subsystem components of the marine power plant, in the general systems and in the technological system of the ship, were presented in tabular form. Each failure was described by considering the time of operation until the first failure, type of failure, type of wear, nature of an event and methods used to regain efficiency. Selected failures were described in great detail, and the statistics of the ship’s components’ susceptibility to failure were presented by considering the wear type that caused a failure, the component type and the time to the first failure. Additionally the severity of each failure is discussed. Finally, conclusions regarding the susceptibility to failure of particular ship components were presented.

A slope riverbank failure is a natural event that occurs globally on each riverbank, and a drawdown event usually causes slope riverbank failure. This case study is aimed at analysing slope riverbank failures by evaluating the seepage and slope stability of the riverbank under slow and rapid drawdown. The riverbank in this case study is located at KM 3.49, Jalan Pantai Luagan in the Sipitang district (N 4° 59 ′ 12.9 ″ E 115° 31 ′ 13.3 ″ ). A literature review was conducted to view the current study pattern and retrieve a methodology based on the current study pattern. GeoStudio is a commercial finite element software. The data obtained from the borehole log report and online resources were utilised to create the riverbank model in software. The phreatic line shows a slow change over time, indicating that the riverbank takes a long time to stabilise after the drawdown. The FOS value decreases during the drawdown occurrence and slowly increases after the drawdown has ended. In conclusion, the drawdown event can cause slope riverbank failure, and the seepage and stability analysis using GeoStudio can show the condition of the riverbank during the drawdown event.

In electromigration failure studies, it is in general assumed that electromigration-induced failures may be adequately modelled by a log-normal distribution. Further to this, it has been argued that a lognormal distribution of failure times is indicative of electromigration mechanisms. We have combined post processing of existing life-data from Al/Cu + TiW bilayer interconnects with our own results from Al/Cu interconnects to show that the Log Extreme Value distribution is an equally good statistical model for electromigration failures, even in cases where grain size exceeds the linewidth. The significance of such a modelling is particularly apparent in electromigration failure rate prediction.

Installations in road tunnels, such as jet fans, are exposed to a harsh environment. After two failures of rotor blades, made of aluminum cast, a failure analysis was initiated. The main goal of this study was to determine the causes, e.g., overloading, material defect, corrosion, and design deficiencies, and to define actions to avoid such events in the future. The fractographical analysis showed corrosion attack and a cleavage type of fracture. These findings pointed to two damage mechanisms acting in both cases: stress corrosion cracking due to hydrogen embrittlement, causing a brittle static failure. The failure analysis did not lead to a full understanding of the damage mechanisms: Service loads were far too low to explain the high stresses needed for a brittle static failure. A wedging effect of corrosion products in the gap between the blade and the mounting was proposed as the cause of high static stresses in the fractured cross section. Alternative materials as well as different corrosion surface protections were evaluated with respect to corrosion and corrosion fatigue resistance. Finally, an appropriate surface protection was proposed as the solution for inhibiting corrosion and therefore the risk of the reproduction of such failures.

Abstract. In recent years, heavy rainfall conditions have caused disasters around the world. To prevent losses by floods, levees have often been constructed in inundation-prone areas. This study performed reliability analyses for the Chiuliao First Levee in southern Taiwan. The failure-related parameters were the water level, the scouring depth, and the in situ friction angle. Three major failure mechanisms were considered: the slope sliding failure of the levee and the sliding and overturning failures of the retaining wall. When the variability of the in situ friction angle and the scouring depth are considered for various flood return periods, the variations of the factor of safety for the different failure mechanisms show that the retaining wall sliding and overturning failures are more sensitive to the change of the friction angle. When the flood return period is greater than 2 years, the levee could fail with slope sliding for all values of the water level difference. The results of levee stability analysis considering the variability of different parameters could aid engineers in designing the levee cross sections, especially with potential failure mechanisms in mind.

Object Children experiencing frequent shunt failure consume medical resources and represent a disproportionate level of morbidity in hydrocephalus care. While biological causes of frequent shunt failure may exist, this study analyzed demographic and socioeconomic patient characteristics associated with frequent shunt failure. Methods A survey of 294 caregivers of children with shunt-treated hydrocephalus provided demographic and socioeconomic characteristics. Children experiencing at least 10 shunt failures were considered frequent shunt-failure patients. Multivariate regression models were used to control for variables. Results Frequent shunt failure was experienced by 9.5% of the patients (28 of 294). By univariate analysis, white race (p = 0.006), etiology of hydrocephalus (p = 0.022), years-with-shunt (p < 0.0001), and surgeon (p = 0.02) were associated with frequent shunt failure. Upon multivariate analysis, white race remained the key independent factor associated with frequent shunt failure (OR 5.8, 95% CI 1.2–27.8, p = 0.027). Race acted independently from socioeconomic factors, including income, level of education, and geographic location, and clinical factors, such as etiology of hydrocephalus, surgeon, and years-with-shunt. Additionally, after multivariate analysis surgeon and years-with-shunt remained associated with frequent shunt failure (p = 0.043 and p = 0.0098, respectively), although etiology of hydrocephalus was no longer associated (p = 0.1). Conclusions White race was the primary independent factor associated with frequent shunt failure. Because races use health care differently and the diagnosis of shunt failure is often subjective, a disparity in diagnosis and treatment has arisen. These findings call for objective criteria for the preoperative and intraoperative diagnosis of shunt failure.

In this investigation, the uniaxial short-term creep tests with multi-step loading were conducted on the sandstone-coal composite samples, and the characteristics of creep strength, creep deformation, acoustic emission (AE), and creep failure of composite samples were studied, respectively. The creep strength of the composite sample decreased with the stress-level duration, which was mainly determined by the coal and influenced by the interactions with the sandstone. The creep deformation and damage of sandstone weakened the deformation and damage accumulation within the coal, resulting in the larger strength for the composite sample compared with the pure coal sample. The axial creep strain of composite sample generally increased with the stress-level or the stress-level duration under same conditions. The AE characteristics of composite sample were related to the creep strain rate, the stress level, the stress level duration, and the local failure or fracture during creep loading. The micro or macro failure and fracture within the composite sample caused the rise in the axial creep strains and the frequency and intensity of AE signals, especially the macro failure and fracture. The creep failures of composite samples mainly occurred within the coal with the splitting ejection failure accompanied by the local shear failure, and no obvious failures were found within the sandstone. The coal in the composite sample became more broken with the stress-level duration.

The authors report a repeated measures field study that captures complaining customers' perceptions of their overall satisfaction with the firm, likelihood of word-of-mouth recommendations, and repurchase intent during a 20-month span that includes two service failures and recovery attempts. The findings suggest that though satisfactory recoveries can produce a “recovery paradox” after one failure, they do not trigger such paradoxical increases after two failures. Furthermore, “double deviations” can occur following two consecutive unsatisfactory recoveries or following an unsatisfactory recovery in response to a second failure. The findings indicate that customers reporting an unsatisfactory recovery followed by a satisfactory recovery reported significantly higher ratings at the second postrecovery period than did customers reporting the opposite recovery sequence. The outcome of the second recovery also demonstrated a significant influence on customer ratings (positively if the recovery was satisfactory, negatively if the recovery was unsatisfactory), regardless of whether the customer found the first recovery satisfactory or unsatisfactory. In addition, although the increased change in recovery expectations and failure severity ratings from the first failure to the second is more dramatic for customers who previously reported a satisfactory recovery, the increase in attributions of blame toward the firm is more pronounced for customers who previously reported an unsatisfactory recovery. Last, the results show that recovery efforts are attenuated when two similar failures occur and when two failures happen in close time proximity.

This case study is to investigate the causes of failure of chain system through characterization on the failure component. The failures that occur are relate to welding because this dipping latex industry used customized chain that have to be weld in joining with outer chain links. The analysis revealed that the weld defect such as crater leads the crack propagation and added with cyclic loading that cause the fatigue failure. The fatigue failure occurs due to this generated crack at the outer circumference of the weld within chain attachment and outer chain links plate. This type of defect also can be categories as designing-in defect. Fatigue crack propagation was evident by progressive beach marks and the scanning electron microscopy (SEM) analysis revealed the types of microstructure that resulting at heat affected zone (HAZ). Hardness testing by using Rockwell Tester found the different hardness profile at three areas that are weld metal, base metal and heat affected zone. The maximum hardness values were found at heat affected zone and weld metal. Finite element method (FEM) that is Ansys Workbench was used to review the different size of outer link plate thickness that affected to the stress distribution. It was found that stress can be minimized with increasing the plate thickness.
